優先権の主張
本出願は、引用によって全体的に、明示的に本明細書に組み込まれる、2005年3月9日に出願された米国仮特許出願第60/660512号、および2004年9月9日に出願された米国仮特許出願第60/608355号の、35U.S.C.119(e)による優先権を請求する。
したがって、装置が、比較的小さい直径を有する管路を経腔的に通過するのを可能にし、一方、比較的大きい直径を有する器具の導入に対処する、改良された接近技術が依然として必要である。泌尿器科医がある範囲のカテーテル直径を保持し使用する必要を無くすると有利である。1つのカテーテル直径を多数の患者に適合させることができるとかなり有用である。理想的には、カテーテルは、6〜10フレンチ以下の直径を有する血管または体腔に進入することができ、器具を12〜18フレンチの中枢ルーメンを通過させることができる。これらの要件は、矛盾するように思われるが、後述の本発明によって解決することができる。有利なことに、シースはまた、蛍光透視の下で視覚が最大になり、製造費が比較的安価である。シースまたはカテーテルは、耐ねじれ性を有し、シースまたはカテーテルを通過する器具に対する摩擦および損傷を最小限に抑えることが好ましい。シースまたはカテーテルは、体腔または空洞壁あるいは周囲の構造を損傷する可能性を最小限に抑えることが好ましい。このような損傷は、場合によっては、狭窄、体腔または空洞の内容物の、周囲の空間への漏れ、出血などを生じさせる恐れがある。
Claims (18)
- 侵襲を最小限に抑えながら体腔または空洞に接近するようになっている拡張可能な経腔的接近シースであって、
近位端、遠位端、および中央を貫通する中枢ルーメンを有する、軸方向に細長いシースチューブと、
かけられた圧力に応じて、第1のより小さい横断面形状から第2のより大きい横断面形状へ周方向に拡張可能な、前記シースの遠位領域であって、少なくとも前記第1のより小さい横断面形状において、折られたW字形になるように少なくとも2つの縦方向の折り目で折られた補強構造を有する遠位領域と、
前記シースチューブの前記近位端に固定されたハブと、
挿入時に前記シースの前記中枢ルーメンを閉じる働きをする中枢のオブトラトールと、
標準的な医療ガイドワイヤを通過させることができる、前記オブトラトール内のガイドワイヤルーメンと、
を有し、
前記オブトラトールは、前記シースの前記遠位領域を拡張させることのできるバルーン拡張器であり、
前記遠位領域は拡張して前記少なくとも2つの縦方向の折り目を広げ、前記第2の大きい横断面形状を形成する
経腔的接近シース。 - 前記の少なくとも2つの縦方向の折り目は、概ね互いに向き合う第1の折り目端と第2の折り目端を有する、請求項1に記載の経腔的接近シース。
- 前記の折り目が一旦折り畳まれるとその折り目は維持される、請求項1または2に記載の経腔的接近シース。
- 前記の縦方向の折り目は広げられて前記第2の大きい横断面形状に保持される、請求項1から3のいずれか1項に記載の経腔的接近シース。
- 取り外し可能なジャケットが前記遠位領域の少なくとも一部を前記第1のより小さい横断面形状に制限する、請求項1から4のいずれか1項に記載の経腔的接近シース。
- 前記遠位領域は、拡張させられたとき、約2.67mmから約6.67mm(約8フレンチから約20フレンチ)の内径を有する、請求項1から5のいずれか1項に記載の経腔的接近シース。
- 前記中枢ルーメンは、一旦拡張させられると、前記近位端から前記遠位端に進む概ね一定の直径を有する、請求項1から6のいずれか1項に記載の経腔的接近シース。
- 前記シースは、前記近位端と前記遠位端の間に、応力集中部を減らしまたは最小限にする滑らかな遷移部を有する、請求項1から7のいずれか1項に記載の経腔的接近シース。
- 前記シースチューブは、拡張させられると、前記中枢ルーメンの内径よりも大きい横断面直径を有する物体が通過できるように柔軟である、請求項1から8のいずれか1項に記載の経腔的接近シース。
- 前記シースチューブはポリマーからなる、請求項1から9のいずれか1項に記載の経腔的接近シース。
- 前記遠位領域は、約0.0051センチから0.076センチ(約0.002インチから0.030インチ)の壁厚を有する、請求項1から10のいずれか1項に記載の経腔的接近シース。
- 前記遠位領域は、0.013センチから0.051センチ(約0.005インチから0.020インチ)の壁厚を有する、請求項11に記載の経腔的接近シース。
- 前記遠位領域は、0.020センチから0.051センチ(約0.008インチから0.020インチ)の壁厚を有する、請求項12に記載の経腔的接近シース。
- 前記補強構造は、内側層と、外側層と、補強層と、を有する、請求項1から13のいずれか1項に記載の経腔的接近シース。
- 前記補強層は、前記遠位領域を前記第2のより大きい横断面形状に保持する、請求項14に記載の経腔的接近シース。
- 前記内側層は、前記補強層内の複数の穴によって前記外側層に結び付けられて、複合単体構造を形成している、請求項14または15に記載の経腔的接近シース。
- 前記補強層は、コイル、ブレード、ステント、またはリングからなる、請求項14から16のいずれか1項に記載の経腔的接近シース。
- 前記遠位領域はひだをさらに有する、請求項1から17のいずれか1項に記載の経腔的接近シース。
