本明細書に記載した本発明は、アポトーシスを阻害する細胞シグナル伝達経路に関与するキナーゼを活性化させ、活性酸素種を捕捉することによって、心筋虚血および虚血・再灌流障害をはじめとする虚血、虚血障害および虚血・再灌流障害を治療する必要性に言及したものである。特に、本発明は、ここに開示の化合物またはそれらの薬学的に許容される塩を含む組成物ならびに、細胞保護キナーゼの活性化因子としてのその効果的な用途に関する。
「アルキル」という用語は、1〜10個の炭素原子を有する直鎖状または分枝状鎖の炭化水素ラジカルを意味し、たとえば、メチル、エチル、n−プロピル、イソプロピル、n−ブチル、sec−ブチル、イソブチル、tert−ブチル、n−ペンチル、n−ヘキシル、n−ヘプチル、n−オクチル、n−ノニル、n−デシルなどを含む。
(a)障害の前に、化合物の約2半減期以内の時点で最終用量を被検体に投与し、
(b)最終用量の投与前に、化合物の少なくとも約4半減期の時点で被検体に非最終用量を投与する、虚血障害または虚血・再灌流障害の治療を必要とする被検体においてそれを治療するための方法に関する。

Rリポ酸(RLip−OH、1.00g)をジオキサンに溶解させた。反応フラスコをホイルで覆って溶液を直射光から保護した。DIEA(0.845mL)およびDSC(1.24g)を順次加え、反応物を室温にて一晩攪拌・通気して、Lip−NHSをin situにて形成した。グルタミル−アラニン(H−EA−OH、1.11g)およびDIEA(2.65mL)の水溶液を調製し、Lip−NHSの溶液に加えた。この混合溶液を一晩攪拌した後、分液漏斗に移した。酢酸エチルに続いて5%KHSO4(aq)を反応混合物に加えた。有機相を回収し、5%KHSO4(aq)で洗浄した後、飽和NaCl(aq)で洗浄した。有機相を再び回収し、無水Na2SO4で乾燥させ、濾過後にin vacuoで蒸発させて、粗RLip−EA−OHを淡黄色泡沫として得た。粗泡沫の一部を、2:1水-アセトニトリル(0.5%HOAc)を用いるRP−HPLCでの精製用に溶解させ、アセトニトリル(0.5%酢酸)を水(0.5%酢酸)に入れた増加する勾配で、YMC Pack Pro C18逆相カラムで生成物を単離した。画分を含む生成物を分析用HPLCで同定し、プールし、冷凍し、凍結乾燥させて、RLip−EA−OH(165mg)を100%HPLC純度(220nmでの面積%)で得た。生成物のNMRは構造と一致し、観察された質量が405(M−1)、計算値は406であった。
RLip−EA−OH(20.0g)をエタノール-水(19:1)に溶解させた。2等量のリジン(14.4g)をRLip−EA−OHのエタノール溶液に加え、スラリーを還流するまで温めた。30分間の還流後、溶液をそのまま室温まで冷ました。生成物であるRLip−EA−OHのジリジン塩を濾過により回収し、無水エタノールですすぎ、RLip−EA−OHのジリジン塩を96%回収できる一定重量まで乾燥させた。
細胞の細胞膜における周知のレセプターでシグナル形質導入経路を活性化させることで、Aktをリン酸化して活性化する。リン酸化Aktは、固定細胞および透過処理した細胞にて特異的抗体でin situにて容易に検出される。
Akt活性化用のサイトブロットアッセイ
細胞内ウェスタンブロットすなわちサイトブロットを使用して、RLip−EA−OHがリン酸化Aktを増加させる機能をアッセイした。A549細胞(ヒト非小細胞肺癌細胞系)を、これらの細胞がリン酸化Aktのレベルを増減させるよう操作可能であるという理由で選択した。細胞を培養プレートに播種し、底に付着させた後、処理し、固定し、透過処理した。透過処理後、細胞をphospho−Aktまたは全Aktのいずれかに特異的な抗体で処理した。次に、細胞を蛍光二次抗体で処理して、結合した一次抗体の量を定量化した。全Aktおよびphospho−Aktを同時に検出した。
RLip−EA−OHがリン酸化Aktを増加させる機能をH9c2細胞で免疫細胞化学的にアッセイした後、顕微鏡で可視化した。H9c2細胞は、ラット心臓筋細胞由来の細胞系であり、リン酸化Aktのレベルを増減させるよう操作可能である。細胞を培養プレートに播種し、そのまま底に付着させ、処理し、固定し、透過処理した。透過処理後、細胞をまずは特異的phospho−Akt抗体で処理した後、蛍光二次抗体で処理して結合された一次抗体の量を定量化した。
RLip−EA−OHおよびRLip−OHが、全Aktに比してAktリン酸化に対しておよぼす作用を、上述したようなサイトブロットアッセイを用いてA549細胞で評価した。リン酸化AktがRLip−EA−OHおよびRLip−OHでの処理後に45分後にセリン473で3倍および2倍増加したのが観察された(図2)。RLip−EA−OHおよびRLip−OHの両方で、リン酸化Aktの量が全Aktに比して用量依存的に増加した。ほとんどの用量レベルでRLip−EA−OHのほうがRLip−OHよりも効果的であった。
材料および方法:IRK活性化アッセイ
Caliper LabChip(登録商標) 3000および12−sipper LabChip(登録商標)を用いる移動度シフトアッセイによって、インスリンレセプターキナーゼ活性を容易に測定し、リン酸化基質と未リン酸化基質の両方を検出した(Caliper Life Sciences, Discovery Alliances and Services Division, Hanover, MD)。移動度シフトキナーゼアッセイでは、蛍光ペプチド基質のリン酸化生成物への変換を測定するのにマイクロ流体チップを使用する。マイクロタイタープレートウェルからの反応混合物を毛細管シッパー(capillary sipper)を介してチップに導入し、非リン酸化基質とリン酸化生成物とを電気泳動で分離して、レーザ誘導蛍光で検出した。経時的な蛍光シグナルのサインから、反応の程度が明らかになった。チロシンキナーゼの触媒サブユニットはATPおよび基質の存在下にて内在性の活性レベルを有するため、結果を対照の%として示す。
上述したIRK活性化アッセイを使用して、化合物がインスリンレセプターチロシンキナーゼ(IRK)を活性化する機能を100μMで評価した。このアッセイの結果を表3に示す。
材料および方法:IGF1およびSrcチロシンキナーゼ活性化アッセイ。
IGF1RおよびSrcチロシンキナーゼは、ATPおよび基質の存在下にて内在性の活性レベルを有する。この活性を、Caliper LabChip(登録商標) 3000および12−sipper LabChip(登録商標)を用いる移動度シフトアッセイで容易に測定し、リン酸化基質と未リン酸化基質の両方を検出した(Caliper Life Sciences, Discovery Alliances and Services Division)。酵素、基質、ATP濃度を各アッセイに合わせて最適化した。IGF1Rアッセイでは、酵素、ペプチド、ATPの最終濃度をそれぞれ20nM、1.5μM、1220μMとした。Srcアッセイでは、酵素、ペプチド、ATPの最終濃度をそれぞれ2.5nM、1.5μM、17μMとした。
材料および方法:Jurkat細胞での細胞生存アッセイ
RLip−EA−OHがアポトーシスを予防し、細胞の生存を促進する機能を、細胞死を媒介するタンパク質であるレセプター相互作用タンパク質(RIP)を欠損したJurkat細胞で評価した。Jurkat細胞系は、ヒトT−リンパ球由来である。RIP欠損Jurkat細胞は、腫瘍壊死因子α(TNFα)で処理するとアポトーシスの影響を受ける。これらの細胞をビヒクルまたはRLip−EA−OHのいずれかで処理した後、TNFαでアポトーシスを引き起こした。細胞の生存を評価して、RLip−EA−OHが細胞をTNFα誘導アポトーシスから保護するか否かを調べた。
材料および方法:細胞質カルシウム過負荷アッセイ
細胞質カルシウムは、カルバコールで刺激するとCHO M1−WT3細胞で増加し([Molecular Devices, FlexStation Application Note 2, Comparison of FLIPR(登録商標) and FLEXstation(商標) for Calcium Mobilization Assays])、カルシウムを結合する蛍光染料で検出できる。カルバコール刺激後の蛍光の増加は、細胞質カルシウムの増加と解釈される。チャイニーズハムスター卵巣(CHO)細胞を96ウェルの培養プレートの底に付着させた。蛍光染料および被検試料をプレートに載せ細胞に取り込ませた。2秒ごとにプレートリーダー(Flexstation II, Molecular Devices, Sunnyvale CA)を用いて蛍光レベルを測定した。細胞をカルバコールで刺激し、蛍光データをカルバコール刺激後の基線上の蛍光のピーク増加として報告した。データを対照試料のピークカルバコール応答に正規化した。
RLip−EA−OH、RLip−OH、Ac−EA−OHがCHO M1−WT3細胞においてカルシウム過負荷を予防する機能を判断した。ムスカリニックM1レセプターを発現するCHO細胞の細胞質カルシウムにおけるピーク上昇を、カルバコール刺激に応答して測定した。RLip−EA−OHは細胞質カルシウムにおける流動を用量依存的に減少させたのに対し、RLip−OHおよびAc−EA−OHは最小限の作用であった(図4)。RLip−EA−OHは、細胞質カルシウムのカルバコール刺激増加に対する阻害作用を有し、この阻害は用量依存的である。RLip−OHおよびAc−EA−OHは、おだやかな細胞質カルシウム低減活性を持つにとどまった。
材料および方法:酸素ラジカル吸収能(ORAC)アッセイ
ペルオキシルラジカルは、再灌流時に細胞によって生成される活性酸素の1種である。ペルオキシルラジカルの存在は、フルオレセイン酸化によって検出可能である。ペルオキシルラジカルの存在下、フルオレセイン蛍光が経時的に減衰する。酸素ラジカルスカベンジャーの存在下、減衰率は低下する。対照とスカベンジャーの存在下での減衰率の変化を用いて、被験化合物のペルオキシルラジカル捕捉機能を測定する。
リポイル含有化合物に対するペルオキシルラジカル吸収能を求めた(表6aおよび6b)。RLip−EA−OHは、RLip−OHより大きなペルオキシルラジカル吸収能を呈した。結果はまた、アセチル−グルタミルアラニンには認識できるペルオキシルラジカル吸収機能がないため、リポイル部分がORACアッセイにおけるペルオキシルラジカルの捕捉で重要であることも示している。
材料および方法:心筋I/R障害のラットモデル
心筋I/R障害のラットモデルを、特定のリポ酸誘導体化合物が心臓保護性(心筋虚血・再灌流障害に対するものなど)であるか否かを判断するためのin vivoスクリーンとして使用した。このモデルは、冠動脈閉塞および冠動脈バイパス移植(CABG)などの心臓手術の手技後に心臓患者で観察される虚血・再灌流障害と類似している(Matsui, T., Tao, J., del Monte, F., Lee, K.−H. et al., Akt Activation Preserves Cardiac Function and Prevents Injury After Transient Cardiac Ischemia In vivo, Circulation 2001, 104:330)。
左冠状動脈の回旋枝を一時的にライゲートして、左心室腫瘤に領域的な虚血を誘導した後、蛍光ミクロスフェアを注射して虚血性領域を描写した。動物を再灌流の約24時間後に屠殺し、心臓を切り出し、切開してトリフェニルテトラゾリウムで染色した。心筋梗塞エリア(MI)、危険のある虚血性エリア(AR)、左心室エリア(LV)を測定することで、薬理学的介入の直接的な影響を判断した。ARに対するMIの低下(MI/AR比)をビヒクル対照に対する薬効の一次測定値として使用した。
これらの実験には、300〜350グラムのオスのスプラーグドーリーラットを使用した。誘導チャンバにて3〜4%イソフルランで麻酔を誘導した。誘導後、経口的に気管に導入したRodent Ventilatorによって16ゲージの血管カテーテル経由で投与して、1.5〜2.0%イソフルランでの麻酔を外科平面で維持した。換気装置を1分あたり60〜65呼吸の速度で2.5ccに設定し手術時の換気を維持した。動物のコア温度を監視し、温度コントローラに接続された加熱用ランプおよび経直腸プローブを用いて37℃に維持した。
RLip−EA−OH処理動物(n=64)対生理食塩水ビヒクル処理(n=54)動物の心筋虚血再灌流モデルにおけるメタ解析によって、心室腔内注射(1mg/kg IC)として投与したRLip−EA−OHが、危険のあるエリア(AR)に比して心筋梗塞(MI)の大きさを効果的に減少させることが示された。ビヒクルおよびRLip−EA−OH処理動物のMI:AR比は、それぞれ0.373(n=54)および0.250(n=64)であった。これは、群間の33%の差に相当する(p<0.001)(図5)。RLip−EA−OH処理後に心筋組織セクションで心臓損傷エリアの有意な減少が観察された。
RLip−OH(2mg/kg IC)またはRLip−EA−OH(1mg/kg IC)で処理した動物のメタ解析を、生理食塩水ビヒクル対照と比較した%減少として表7に示す。Ac−EA−OHすなわち非リポイル含有化合物も評価したところ、ビヒクルと統計的に類似することが明らかになった(データ図示せず)。この結果から、RLip−EA−OHでの処理後のI/R障害からの心臓保護は、RLip−OHで処理する場合よりも良好であったことが分かる。
詳細な手順
実施例8で説明したようにして心筋I/R障害のラットモデルを用いるin vivo実験を実施した。表10に示すように、いくつかの単一用量および複数用量での投与計画を試験した。カテーテル処置した(頸静脈)ラットを0.25時間前および/またはこれを含む時点で静脈内用量投与に使用した。
RLip−EA−OHを単一大量瞬時投与すると、I/R障害に比して広い期間(すなわち手術前15分、3時間、6時間または24時間の時点;血管閉塞時;血管への再灌流時)にわたってMI損傷を減らす上での有効性が認められた。この薬剤は、動脈閉塞時と再灌流時に有効であることも示されたが、特定の早い時点(−15分など)で見られたほどではなかった。このラットモデルにおけるI/R障害に比して広い期間にわたる有効性は、RLip−EA−OHの短い血清半減期(すなわちラットでCmaxが20μg/mLの場合に用量30mg/kgで約9分間)を考慮すると、逆説的である。
介入の必要な不整脈および心臓停止に関連した急性心機能不全の出現率およびこれらの併存症による死亡率の出現率について、RLip−EA−OHを投与するための異なる薬用量および投与計画で本明細書で説明する心筋I/R障害のラットモデルで検討した。実施例8で説明したようにして、実施例10で説明した投与間隔で、in vivoでの実験を実施した。治療薬(すなわち、ビヒクル、RLip−EA−OH)を左心室内注射、IV大量瞬時投与、IV点滴または経口的に投与した。具体的には、虚血時および介入(直接的な機械的心臓蘇生)を要したときの非自己回復型の心機能不全(すなわち、フィブリル化の延長、不整脈、突然の心臓停止)の出現率ならびに、心機能不全に関連した死亡率を観察した。
RLip−EA−OHの複数および単一投与によって、命を脅かす心機能不全の出現率とこれを軽減するための付随する介入の必要性ならびに、これらの命を脅かす心機能不全に関連する死亡率が、ビヒクル対照に比して減少した(表14)。複数投与による投与計画では、単一用量での投与に比して有意に良好な有効性が認められた。
RLip−EA−OHへの全身曝露を評価するために、群にしたラットに単一用量のビヒクル(2匹/性)またはRLip−EA−OHを、30、100または200mg/kg(6匹/性/群)の同一の用量レベルで静脈内点滴した。ビヒクル処理した群では、投与前と投与の30分後(+2分間)にだけすべてのラットから血液試料を回収した。RLip−EA−OH処理群では、投与の10分後、30分後、45分後、1時間後、2時間後、3時間後、4時間後(±2分間)に3匹のラット/性/群から血液試料を回収した。

生化学的分析法
正のターボイオンスプレーを使う多反応監視モードで、LC/MS/MS法を用いて、BASi, Inc.(West Lafayette, IN)で血漿および尿中のRLip−EA−OH濃度を測定した。ヒト血漿での定量化可能な範囲は1.00〜500ng/mLであり、QC試料は3.00、75.0、375ng/mLで含まれていた。
PK個体群には、RLip−EA−OHを与えられ、投与後PK測定が≧1の被検体を含んでいた。投与に関して不適合であるとされるか、あるいは不完全なデータを持つ被検体がいた場合は、これを分析に含めるか否かについてケースバイケースで判断した。24例の被検体からの血漿試料(コホート1つあたりRLip−EA−OHの被検体6例)を分析した(表16および17)。
平均血漿RLip−EA−OH濃度対時間データを、線形目盛および片対数目盛でそれぞれ図9Aおよび図9Bに示す。血漿濃度は多相的に急激に減衰した。

Claims (29)
- 虚血障害または虚血−再灌流障害の治療を必要とする被検体における虚血障害または虚血−再灌流障害を治療する方法において使用するための医薬組成物であって、前記医薬組成物は、以下の構造式:
で表される化合物またはその薬学的に許容される塩を含み、前記化合物のR1およびR2が、各々独立に、Hまたは加水分解可能な基であり、前記化合物またはその薬学的に許容される塩が少なくとも90%エナンチオマー的に純粋であり、前記方法は、前記医薬組成物の少なくとも2用量を前記被検体に投与する工程を含み、前記方法は、
非最終用量の前記医薬組成物と、最終用量の前記医薬組成物とを、前記障害の前に前記被検体に投与する工程を含み、
(a) 前記最終用量を前記障害の直前に投与し、
(b) 前記非最終用量および前記最終用量を、前記化合物の単一用量投与に比べて、前記化合物が前記障害を治療する有効性を高める投与間隔で投与する、医薬組成物。 - 前記非最終用量と前記最終用量との間の前記投与間隔が少なくとも約4時間である、請求項1に記載の医薬組成物。
- 前記非最終用量と前記最終用量との間の前記投与間隔が少なくとも約6時間である、請求項2に記載の医薬組成物。
- 前記非最終用量と前記最終用量との間の前記投与間隔が少なくとも約8時間である、請求項3に記載の医薬組成物。
- 前記障害の約0分前〜約90分前の範囲の時点で前記最終用量を前記被検体に投与する、請求項1〜4のいずれか1項に記載の医薬組成物。
- 前記障害の少なくとも約30分前に前記最終用量を前記被検体に投与する、請求項5に記載の医薬組成物。
- 前記障害の少なくとも約60分前に前記最終用量を前記被検体に投与する、請求項6に記載の医薬組成物。
- 虚血障害または虚血−再灌流障害の治療を必要とする被検体における虚血障害または虚血−再灌流障害を治療する方法において使用するための医薬組成物であって、前記医薬組成物は、以下の構造式:
で表される化合物またはその薬学的に許容される塩を含み、前記化合物のR1およびR2が、各々独立に、Hまたは加水分解可能な基であり、前記化合物またはその薬学的に許容される塩が少なくとも90%エナンチオマー的に純粋であり、前記方法は、前記医薬組成物の少なくとも2用量を前記被検体に投与する工程を含み、前記方法は、

(b)前記最終用量の投与の、前記化合物の少なくとも約4半減期前の時点で、前記非最終用量を前記被検体に投与する、医薬組成物。 - 前記障害の前の前記化合物の約0.25半減期〜約2半減期の範囲の時点で、前記最終用量を前記被検体に投与する、請求項8に記載の医薬組成物。
- 前記障害の前記化合物の約1半減期前に、前記最終用量を前記被検体に投与する、請求項9に記載の医薬組成物。
- 前記化合物またはその薬学的に許容される塩の濃度が、前記非最終用量の投与後かつ前記最終用量の投与直前に前記被検体から得られる血液試料中でのそのCmaxの10%未満である、請求項1〜10のいずれか1項に記載の医薬組成物。
- 前記障害の前の約48時間以内の時点で前記非最終用量を前記被検体に投与する、請求項1〜11のいずれか1項に記載の医薬組成物。
- 前記方法が、前記非最終用量を投与する前に1以上の追加の用量の前記医薬組成物を前記被検体に投与する工程をさらに含む、請求項1〜12のいずれか1項に記載の医薬組成物。
- 前記障害の前に3用量の前記医薬組成物を前記被検体に投与する、請求項13に記載の医薬組成物。
- 前記方法が、前記障害後に1以上の追加の用量の前記医薬組成物を前記被検体に投与する工程をさらに含む、請求項1〜14のいずれか1項に記載の医薬組成物。
- 各用量が等量である、請求項1〜15のいずれか1項に記載の医薬組成物。
- 前記医薬組成物を静脈内投与し、各用量における前記化合物の濃度が約0.5mg/kg〜約5.0mg/kgの範囲である、請求項1〜16のいずれか1項に記載の医薬組成物。
- 前記虚血障害が心筋虚血障害である、請求項1〜17のいずれか1項に記載の医薬組成物。
- 前記虚血障害が、心血管虚血、脳血管虚血、腎虚血、肝虚血、虚血−再灌流心筋症、皮膚虚血、腸管虚血、腸虚血、胃虚血、肺虚血、膵臓虚血、骨格筋虚血、腹筋虚血、肢虚血、虚血−再灌流大腸炎、腸間膜虚血、および無症候性虚血からなる群から選択される虚血の結果である、請求項1〜17のいずれか1項に記載の医薬組成物。
- 前記虚血−再灌流障害が心筋虚血−再灌流障害である、請求項1〜17のいずれか1項に記載の医薬組成物。
- 前記心筋虚血−再灌流障害が心筋梗塞の結果である、請求項20に記載の医薬組成物。
- 前記心筋梗塞が急性心筋梗塞である、請求項21に記載の医薬組成物。
- 前記虚血−再灌流障害が脳虚血−再灌流障害である、請求項1〜17のいずれか1項に記載の医薬組成物。
- 前記脳虚血−再灌流障害が脳卒中の結果である、請求項23に記載の医薬組成物。
- 前記虚血−再灌流障害が、脳血管虚血−再灌流障害、腎虚血−再灌流障害、肝虚血−再灌流障害、虚血−再灌流心筋症、皮膚虚血−再灌流障害、腸管虚血−再灌流障害、腸虚血−再灌流障害、胃虚血−再灌流障害、肺虚血−再灌流障害、膵臓虚血−再灌流障害、骨格筋虚血−再灌流障害、腹筋虚血−再灌流障害、肢虚血−再灌流障害、虚血−再灌流大腸炎、腸間膜虚血−再灌流障害、および無症候性虚血−再灌流障害からなる群から選択される、請求項1〜17のいずれか1項に記載の医薬組成物。
