Description
本願は、2007年5月25日に出願された米国特許出願第11/807,227号の一部継続出願であり、それらの全体において本明細書中に引用により組み込まれる。
原核生物フェニルアラニンアンモニアリアーゼ(PAL)及びその組成物、並びに、PALの免疫原性及び/又はタンパク質分解感受性を低減させつつ、PALの触媒活性及び安定性を向上させるそのような組成物の最適化を提供する。また、治療及び産業目的のPALのそのような最適な組成物の使用を提供する。
PALは、植物[Koukolら, J. Biol. Chem. 236:2692-2698 (1961); Hansonら, The Enzymes 7:75-166 (1972); Poppeら, Curr. Org. Chem. 7:1297-1315 (2003)]、ある真菌類[Raoら, Can. J. Biochem. 4512:1863-1872 (1967); Abellら, Methods Enzymol. 142:242-253 (1987)]、及び、細菌[Bezansonら, Can. J. Microbiol. 16:147-151 (1970); Xiangら, J. Biol. Chem. 277:32505-32509 (2002); Hillら, Chem. Commun. 1358-1359 (2003)]に広く分布している非哺乳動物酵素であり、大腸菌(Escherichia coli)において組換えにより生産される。
幾つかの細菌PALは、ストレプトマイセス マリチムスからのPAL(EncPとしても公知、配列番号5、図4)、ノストック パンクチフォルメ(Nostoc punctiforme)からのPAL/HAL(Nostoc punctiforme ATCC 29133からのアクセッション番号ZP_00105927、2004年10月1日提出、NCBI Microbial Genomes Annotation Project)(配列番号2、図4)、アナベナ バリアビリス(Anabaena variabilis)からのPAL/HAL(遺伝子ID 3679622, Ava_3988フェニルアラニン/ヒスチジンアンモニアリアーゼ、Anabaena variabilis ATCC 29413、2006年3月31日)(配列番号4、図4)、光合成原核生物のアナシィスティス ニダランス(Anacystis nidulans)[Lofflehardt, Z. Naturforsch. 31(1 1-12):693-9 (1976)]、エンテロバクテリア科ファミリーからのグラム陰性細菌、フォトラブダス ルミネセンス(Photorabdus luminescens) TT01[Williamsら, Microbiology 151 :2543-2550 (2005)]、及び、ストレプトマイセス バルチチラタス(Streptomyces verticillatus)[Bezansonら, Can. J. Microbiol. 16(3): 147-51 (1970)]を含むが、これらに限定されない、HAL/PALファミリーの一部として既に同定されている。また、PALの活性は、ストレプトマイセス マリチムスで評価されている[Xiangら, J. Biol. Rep. 22(5):580-593 (2005); Beckerら, Gene 325:35-42 (2004); Hoffmanら, Gene 3 11:171-180 (2003)]。本発明は、原核生物PAL又は細菌PALがPKU及び他の疾患の有効な治療に寄与し得るという知見に基づくものである。本発明は、非常に強力な触媒活性、高い生化学的安定性、並びに、治療への適用のための、低減された免疫原性、及び、長い生物学的半減期等の向上された特性を具備する、細菌PALの組成物、及び、生物学的に活性のあるそのフラグメント、突然変異体、変異体又は類縁体を意図するものである。また、本発明は、細菌PAL、及び、生物学的に活性のあるそのフラグメント、突然変異体、変異体又は類縁体の製造方法及び精製方法、並びに、治療及び産業目的のそのような組成物の使用方法を提供する。
PALは、フェニルプロパノイド代謝物質への関与段階における桂皮酸へのフェニルアラニンの非酸化的脱アミノ化を触媒する、遍在の高等植物酵素であるが[Hahlbrockら, Annu. Rev. Plant Phys. Plant Mol. Biol. 40:347-369 (1989)]、PALは、"ストレプトマイセス マリチムス"[Xiangら, J. Biol. Chem. 277:32505-32509 (2002)]及びソランギウム セルロサム[Hillら, Chem. Commun. 1358-1359 (2003)]におけるベンゾイル−CoA生合成、並びに、ストレプトマイセス バルチチラタス[Bezansonら, Can. J. Microbiol. 16:147-151 (1970)]におけるシンナムアミドの生合成に関係する少数の細菌でしかみられていない。静菌剤のエンテロシンは、その生合成が多くの独特の機能を含む生合成海洋細菌"ストレプトマイセス マリチムス"の天然生産物である[Hertweckら, Chem. Biol. 11:461-468 (2004); Pielら, Chem. Biol.7:943-955 (2000); Pielら, J. Am. Chem. Soc. 122:5415-5416 (2000); Xiangら, Proc. Natl. Acad. Sci. USA 101:15609-15614 (2004)]。これらのうち、ポリケチド合成酵素(PKS)スターターユニットベンゾイル−コエンザイムA(CoA)の形成が希少である[Mooreら, Nat. Prod. Rep. 19:70-99 (2002)]。初期の生化学反応は、新規のフェニルアラニンアンモニアリアーゼ(PAL, EC 4.3.1.5)EncP[Xiangら, J. Biol. Chem. 277:32505-32509 (2002)]によるアミノ酸L−フェニルアラニンからtrans−桂皮酸への変換を含む。単一の一連のβ酸化前の桂皮酸のそのCoAチオエステルへの活性化は、マロニル−CoAの7つの分子を有する鎖延長のエンテロシンII型PKSを準備する、ベンゾイルCoAを産生する[Hertweckら, Chem. Bio. 多くの方法は、主としてタンパク質構造情報に基づく合理的な最適化を利用するプロテインエンジニアリングで知られている[Branniganら, Nat. Rev. Mol. Cell. Biol. 3(12):964-970 (2002); Marshallら, Drug Discov. Today 8(5):212-221 (2003)]。構造的特徴の系統的な置換により、改善されたタンパク質特性及び/又は基質特異性の設計変更を生じることがある。ある遺伝子ファミリーのメンバーから他の相同メンバーへの機能の補強は、直接の基質結合付近の限定数のアミノ酸置換の導入によって実行することができる[Bocanegraら, Biochemistry 32(11):2737-2740 (1993); Faillaら, Fold Des. 1(1):35-42 (1996); Hayesら, Proc Natl Acad Sci USA 99(25): 15926-15931 (2002); Voigtら, Nat. 野生型原核生物PALの三次元構造は、X線結晶構造解析を用いて決定することができる。PALタンパク質は、主としてα−ヘリックスからなるそれぞれの単量体を有するホモ四量体であり、中央の触媒ドメイン、N末端ドメイン、シュードモナス プチダのヒスチジンアンモニアリアーゼ[HAL、Schwedeら, Biochemistry 38(17):5355-5361 (1999)]と相同性のある小さなC末端ドメイン、及び、完全な四量体分子の端から突出するC末端ドメイン領域に挿入された付加的なドメインの4つのドメインへ細分化することができる。N末端の最初の25残基は、四量体の4つのすべての単量体においては視認することができず、この領域は、不規則であると考えられる。残基109−123と350−353との間のループ領域は、PAL四量体の単量体A、C及びDにおいて不規則な領域103−123及び350−353と共に、単量体Bにおいて不規則である。ロドスポリディウム トルロイデス PALの2つのX線構造が、2.1Åの分解能(P3221空間群)で、及び2.7Åの分解能[P21空間群;Calabreseら, Biochemistry 43(36):11403-11416 (2004)]で、結晶化プロセス時におけるtrans−桂皮酸及びNH4イオンの添加により決定されている。結晶化に利用された非常に低いpH、及びこれらの構造の非常に低い分解能は、構造(特に、N末端領域における)に固有の不規則、及び、NH2付加物に与えるMIO補助因子に存在する付加的な電子密度の明確な欠如を生じた。
PALの高分解能三次元結晶構造は、突然変異、改良、又は突然変異と改良の組み合わせのタンパク質の領域を選択するコンピューター化された方法で用いてもよい。例えば、市販のプログラムGETAREAは、X線の結晶学的座標に基づくアミノ酸残基の外面露光を算出する。

指向進化方法は、タンパク質の突然変異のスペースの十分に大きな領域を調査するために、関心を惹く遺伝子を無作為に突然変異させる。多数の指向進化方法は、誤りを生じがちなPCR、ランダム挿入、DNA配列の全体にわたって多様性を導入する欠失(RID)突然変異誘発、並びに、部位飽和突然変異誘発、及び他のオリゴヌクレオチド系突然変異誘発法[Branniganら, (2002) 前述の箇所]等の非常に集中された又は“指向された”方法を含んで存在する。また、DNA配列再結合の方法は、突然変異の有利な部位を組み合わせ、同時に、新規のDNA配列を生成する(例えば、DNAシャフリング方法、StEP、RACHITT、ITCHY)有害突然変異を除去するのに使用されている。そして、構造に基づく指向進化技術は、治療の利点(構造に基づく組み合わせのエンジニアリング、SCOPE、及びプロテインデザインオートメーション、PDA、方法)のタンパク質の設計を変更するために使用されている。SCOPE法は、DNA操作技術と結び付けられたタンパク質構造情報を利用して、非相同遺伝子からの多重交差タンパク質変異体ライブラリーを設計し作製する、半合理的なプロテインエンジニアリングアプローチに基づく[O'Mailleら, J Mol Biol. 321(4):677-691 (2002)]。PDAでは、突然変異のスペースのコンピューター上のプレスクリーニングによって、特定のタンパク質の折り畳みと適合する突然変異だけ包含させ、実験的なスクリーニングに適用可能なサイズに配列変異体の数を減らすことができる[米国特許第6,403,312号; Dahiyatら, Proc Natl Acad Sci USA 94(19):10172-10177 (1997); Dahiyatら, Protein Sci. 6(6):1333-1337 (1997); Hayesら, Proc Natl Acad Sci USA 99(25): 15926-15931 (2002) Orenciaら, Nature Struct Biol. 8:238-242 (2001)]。
既報の実験では、PAL突然変異体[Schusterら, FEBS Lett. 349(2):252-254 (1994); Schusterら, Proc Natl Acad Sci USA 92(18):8433-8437 (1995); Langerら, Biochemistry 36:10867-10871 (1997); El-Batalら, Acta Microbiol Pol. 49(1):51-61 (2000); Rotherら, Eur. J. Biochem.269:3065-3075 (2002)]、及びHAL突然変異体[Taylorら, J. Biol. Chem. 269(44):27473- 27477 (1994); Baedekerら, Eur. J. Biochem. 269(6): 1790-1797 (2002)]等の改質されたPALが述べられている。
本発明による野生型PALの生物学的に活性のある部位は、PAL動力学特性を最適化することが望まれるように改質することができる。最大半量活性を提供する基質の濃度であるKmは、許容範囲内(すなわち、120μM〜240μM)でのPhe量を維持するPALの治療有効性と密接に関連している。Kmは、基質における酵素の親和性である。親和性を制御することにより、さまざまな濃度で基質に対する酵素の有効性を制限又は制御することができる。例えば、Kmが1000μM(ロドスポリディウム トルロイデス)である場合、酵素活性は、240μMの血液Phe量で約12.5%、及び60μMの血液Phe量で約3%に低減される。Kmが240μMであれば、酵素活性は、240μMの血液Phe量で約50%、及び60μMの血液Phe量で約12%に低減される。Kmが120μMであれば、酵素活性は、240μMの血液Phe量で約70%、及び60μMの血液Phe量で約35%に低減される。最適には、治療対象は、Pheを低減させるのに十分な活性を有するだけでなく、約120μM〜約240μMの最適範囲内でPheを維持する酵素を有する。高いKm(すなわち1000μM)を有する酵素は、Phe量が正常な範囲に低下し、急速に活性を失い、また、非常に高い濃度又は多い用量である実際的でない投与を必要とする。一方、非常に低いKmを有する酵素は、急速にPhe量を消費し、高フェニルアラニン貧血症(hyperphenylanemias)に致命的であるが、癌の処置には有用であり得る。
多くの戦略は、一般にタンパク質免疫原性を低減させるために利用される。ある実施態様においては、免疫反応を最小限にするために導入される改良は、巨大分子の構造、機能又は安定性を損なわない。用いられる有効な戦略は、ヒト配列含量を増加させること(キメラ及び/又は他の‘ヒト化’アプローチ)、溶液物性を改良すること、抗体エピトープを除去すること、化学誘導体化(ペグ化など)を導入すること、及び/又は、MHCアグリトープを識別及び除去することを含む。注入による治療においては、インビボでの免疫反応性は、改良される合理的な突然変異誘発が生じる及び/又は免疫原性のこれらの部位を突然変異させる前に、単独で、及び、部位特異的ペグ化[Hershfieldら, Proc. Natl. Acad. Sci. USA 88:7185-7189 (1991); Leongら, Cytokine 16(3): 106-119 (2001); Leeら, Pharm. Res. 20(5):818-825 (2003)]、又は、タンパク質免疫原性を許容されるレベルに低減させる他の化学誘導体化方法を組み合わせ、エピトープマッピングをすることで解消することができる。抗原表面タンパク質領域の改良は、免疫原性を低減させる[Chirinoら, Drug Discov. Today 9(2):82-90 (2004)]。ある改良方法は、触媒活性(例えば、吸光度分析が活性測定に用いられる)を保持する、非常に小さなサイズのタンパク質の構造を含む。また、第二の改良方法である、ELISAスクリーニングに結び付くプロテインエンジニアリングにより、低減された免疫反応性を有する突然変異体の同定をすることができる。他の方法は、ペグ化誘導体化のための追加的な表面リジン部位における点突然変異を導入する。試験酵素プリンヌクレオシドホスホリラーゼで免疫原性を低減させる方法が示されている[Hershfieldら(1991), 前述の箇所]。代替的な経路は、タンパク質エピトープ領域に位置する残基の突然変異を用いて、免疫原部位を除去する[Yeungら, J. Immunol. 172(11):6658-6665 (2004)]。抗体のヒト化(antibody humanization)と類似しているアプローチでは、相同ループ領域、及び/又はヒト抗体からの残基は、相同タンパク質の対応するループ領域に置換される。
タンパク質治療上のエピトープは、多くのアルゴリズムを用いて算出され、又は、実験的にインビトロ若しくはインビボで決定することができる。DNAStarからのLasergeneプログラムパッケージでの“Peptide Companion”及び“Protean”等のコンピュータープログラムが、タンパク質の化学組成及び配座に基づくタンパク質の表面エピトープ領域の評価に一般に使用される。タンパク質配列の免疫原領域は、親水性保持係数(hydrophilicity index)に基づく最も高い算出された親水性、並びに、算出された表面タンパク質領域の両親媒性及び他の配座パラメーターに基づく抗原性の領域である。あるいは、タンパク質配列のアグリトープは、潜在的なHLA結合のコンピューターにモデル化された予測に基づいて位置することができる[Robinsonら, Nucleic Acids Res. 31(1):311-314 (2003); De Grootら, Novartis Found. Symp. 254:57-72 (2003)]。また、エピトープは、インビトロで生化学的に[Tangriら, Curr. Med. Chem. 9(24):2191-2199 (2002)]、及びインビトロで細胞系の方法で[Sticklerら, J Immunother. 23(6):654-660 (2000); Sticklerら, J Immunol Methods 281(l-2):95-108 (2003)]、同定することができる。プロテインエンジニアリングにおいては、免疫原性の相対的な低減は、Sticklerらの文献[Toxicol Sci. 77(2):280-289 (2004)]のインビトロでの細胞系分析と同様の分析を用いて監視することができる。
酵素の免疫原領域の、又は付近の表面露出残基は、同定することができる。これらの配置は、タンパク質に存在する溶媒露出表面配置の総数のサブセットであり、免疫原性/抗原性の領域への近接、及び表面への近接に依存する。X線結晶解析、NMR、又は相同モデリングを用いて決定されたタンパク質の三次元構造は、巨大分子溶媒露出表面積を算出する市販のソフトウェアプログラムで使用することができる。プログラムGETAREA 1.1[Fraczkiewiczら, J. Comp. Chem. 19:319-333 (1998)]を用いて提供される出力は、ロドスポリディウム トルロイデス PALの表面露出の信頼できる評価を提供する。GETAREA、PARAREA及び同様のプログラムは、ガウスボンネの定理に基づくパラメーターのアプローチを具備する連続方法を用いて、溶媒露出表面積を算出する。PARAREAは、それぞれの原子の隣接するすべての原子対を用いてガウスボンネ経路で潜在的な交差地点を見出し、GETAREAは、非常に効率的に、二球面交差の平板によって定義された交差半空間を用いて、溶媒露出頂点を算出する。
ストレプトマイセス マリチムスのencPの構造は、シュードモナス プチダの構造に基づいて相同モデル化された。PALより小さいにも関わらず、ストレプトマイセス マリチムスの構造は、ヒトPALよりも小さく、恐らくencPをPALプロテインエンジニアリングに用いるアンモニアリアーゼの免疫原でない形態にする。非免疫原性PAL変異体を得るための代替的な方法は、“ヒト化”encP、又は、活性のための突然変異及び選択工程の前に更にペグ化することができる他の原核生物PALタンパク質を得るための、encP、又はPAL活性を有する他の原核生物突然変異体の選択による突然変異を用いる、ストレプトマイセス マリチムスencP、又は免疫原配列をヒトヒスチジンアンモニアリアーゼ配列[HAL、Suchiら, Biochim. Biophys. Acta 1216(2):293-295 (1993)]に置換する他の原核生物PALの突然変異体を含む。
多くの戦略は、タンパク質プロテアーゼ感受性を低減させるために一般に利用される。ある実施態様においては、タンパク質分解感受性を最小限にするために導入された改良は、巨大分子の構造、機能又は安定性を損なわない。有効な戦略は、競合インヒビター[Gilbertら, (1981) 前述の箇所; 米国特許第6,548,644号; 米国特許第6,451,986号; 米国特許第6,433,158号; 米国特許第6,461,849号]等の種を安定化させる活性部位を有する酵素の提供、感受性のあるLys及び/又はArg部位でトリプシン結合部位をブロッキングするためにアミノ基を化学的に改質すること、切断感受性を停止、PALを他の保護巨大分子(Fc抗体ドメイン、又はボツリヌス神経毒素の無毒成分等)に連結又は結合するために小さい中性アミノ酸にキモトリプシン感受性Tyr、Phe及び又はTrp残基を突然変異させること、及び/又は、PEG又は他の化学的保護基及び安定化基でのその後の化学誘導体化のためにプロテアーゼ感受性部位付近のLys又はCys部位を導入することを含む。
巨大分子の化学修飾は、非特異的な方法(誘導化された種を混合させる)、部位特異的な方法(野生型巨大分子の反応性誘導体化、及び/又は、部位特異的突然変異誘発と化学修飾を組み合わせて用いる部位選択性変性)、又は、発現タンパク質ライゲーション法[Hofmannら, Curr. Opin.Biotechnol. 13(4):297-303 (2002)]で行うことが可能である。ある実施態様においては、化学修飾は、免疫原性を低減させるために用いられる。ペグ化は、タンパク質の免疫原性を低減させる実証された方法である[Bhadraら, Pharmazie 57(l):5-29 (2002)]が、リン酸化、アミド化、カルボキシル化、アセチル化、メチル化、酸付加塩類、アミド、エステル、及び、N−アシル誘導体の形成を用いた修飾により、グリコシル化及び他の化学誘導体化方法も可能である[Davis, Science 303:480-482 (2004)]。
PEG化学試薬とPALタンパク質の比の範囲を用いるPALにおける一連のさまざまなペグ化反応は、それぞれの改良方法のためのPEG−PAL誘導体を提供する。最適なペグ化の程度は、PEG誘導化の範囲を決定するためにPAGEと天然ゲル分析を組み合わせて吸光度分析を用い、それぞれの誘導化PAL種で得られる残存活性に基づいて決定することができる。最適な改良の初期範囲が決定された後、相対的な動力学的解析(Vmax及びKmの測定、基質の結合定数、タンパク質分解安定性、活性のpH依存性、活性の温度依存性を含む)、及び最適なPEG−PAL種の免疫反応性を、ELISA、免疫沈降及びウェスタンブロットで決定することができる。また、プロテインエンジニアリングは、最適な誘導条件を用いて、ペグ化のための最も好ましいPAL突然変異体を生成するために利用することができる。PALタンパク質のサイズを最小限にし、PAL表面の最も強い抗原性領域を改良することのみにより、PEG修飾のコストは、最大の酵素活性及び最低の免疫原性を同時に維持して、低減される。同様に、部位特異性ペグ化は、酵素誘導体を提供するために用いることができる。
活性のあるPAL又はHAL変異体の生成及びスクリーニングにおいては、初期突然変異体クローンの発現は、His標識ベクター発現系等の公知の発現系を利用することができ、タンパク質変異体の単離のための高スループット金属キレート精製工程を促進することができる。三層スクリーニング系は、好ましいタンパク質変異体を同定するために使用することができる。初期ポジティブクローンの同定は、フェニルアラニン栄養要求性の大腸菌の増殖における形質転換及び選択を用いることができる。スクリーニングの第二ラウンドは、高スループット処理に適用可能なOD290吸光測定[Hodgins, Biochem. Biophys. Res. Commun., 32:246-253 (1968)]を利用することができる。そして、タンパク質分解抵抗性(プロテアーゼ反応混液の存在下でインキュベーションを用いて)、あるいは免疫原性の低減(拮抗的なELISA測定を用いて)のスクリーニングは、好ましいタンパク質変異体を同定するために用いることができる。
(1.さまざまな形態の高フェニルアラニン血症(HPA))
本発明は、PALの組成物単独、又はHPA及び/又はPKUの処置のための他の治療規制と併用しての使用を含む方法によるさまざまなHPA患者集団の治療に関する。特に、PALの組成物が、食事療法を通常利用しない程度に十分に低いフェニルアラニン濃度の患者集団(すなわち、軽症型HPAの患者)、中程度のPKUの患者、古典的重症型PKUの患者、及びその下位個体群の治療のために使用することができることを意図するものである。軽症型HPAの影響を改善するPALの組成物で治療することが可能なそのような患者は、200μM未満の血清濃度の妊婦及び幼児を含む。さまざまな患者集団、及びそのさまざまな治療の必要性は、本項で更に詳細に考察される。
重度のPKUは、1200μMより高い血漿Phe濃度を示し、4800μM程度の高い濃度であることがある。この疾患の患者は、血漿Phe濃度を臨床的に許容し得る量(一般に、600μM未満、又は300μM未満)までにするために、Pheが無い食事で治療する必要がある。これらの患者は、1日当たり最大250〜350mgの食事Pheを許容できるのみである[Spaapenら, Mol. Genet Metab. 78:93-99 (2003)]。そのため、これらの患者は、誕生後7〜10日間のPhe制限食から始めて、残りの生涯においてこの食事制限が負担させられる。これらの個体が負担させられる厳密な食事制限の緩和が有益である。
本発明の方法で治療することができる患者の第二のグループは、上昇した血漿Phe濃度を有する[すなわち、200μMより高い濃度であるが、BH4治療(下記のBH4負荷試験で決定される)に非反応性であると診断されている]個体である。そのような患者は、軽症型PKUの個体(すなわち、600μM以内の血漿Phe濃度)である個体、中程度のPKUの個体(すなわち、600μM〜約1200μMの血漿Phe濃度)、及び古典的重症型PKUの患者(すなわち、1200μMより高い血漿Phe濃度)を含み得る。
妊娠を計画する及び妊娠しているPKU女性の血漿Phe量の代謝制御は、胎児のPAH状態に関わらず、子宮内の適度に上昇したPhe量で暴露された胎児への深刻な影響のために重要である。適切な制御を達成しないと、小頭症、精神薄弱及び先天性心疾患を含む疾患を生じるため、血漿Phe濃度の治療による制御は、妊娠第一期で特に重要である。
本明細書全体で考察されるように、幼児(0〜3歳)の血漿Phe濃度の上昇が、子供のIQを顕著に低下させることが判断されている。しかしながら、本明細書の他の箇所で考察されたように、400μMまで血漿Phe濃度が上昇した患者は、通常食事療法を受けない。したがって、0〜3歳の幼児は、本治療から深刻な悪影響を受ける。幼児への適用は、被験者の血漿Phe濃度の有益な低減のために、PALを含む治療組成物で、360μM15μMより高い無制限の血漿Phe濃度の幼児を治療することを意図するものである。
本発明は、PKU/HPAの治療的介入を意図するものである。そのような介入は、PALの使用の初期に基づくものであり、PAL単独、又は食事制限と組み合わせて用いることができる。また、PAL及び/又は食事制限は、設計される他の治療的組成物[例えば、脳のPhe蓄積を防止する大きい中性アミノ酸等の他のPKUの治療を意図するもの(Kochら, Mol. Genet. Metabol. 79:110-113 (2003)参照)、又はチロシン補給]と更に組み合わせてもよい。本項は、本明細書中で意図される治療で用いられ得る組成物についての考察を提供する。
一般に、本発明は、本発明に係る有効量のタンパク質又はその誘導生成物と、1以上の医薬として許容し得る希釈剤、安定化剤、保存剤、溶解剤、乳化剤、佐剤、及び/又はキャリアを含む医薬組成物を意図するものである。そのような医薬組成物は、さまざまな緩衝液内容物(例えば、Tris−HCl、リン酸塩)、pH及びイオン強度の希釈剤;洗剤及び可溶化剤(例えば、ポリソルベート20、ポリソルベート80)、抗酸化剤(例えば、アスコルビン酸、メタ重亜硫酸ナトリウム)、保存剤(例えば、チメロサール、ベンジルアルコール)、及び充填物質(bulking substance)(例えば、ラクトース、マンニトール)等の添加剤を含む。例えば、Remingtonの薬学(Remington's Pharmaceutical Sciences), 第18版 (1990, Mack Publishing Co., Easton, Pa.) ページ1435:1712(本明細書中に引用により組み込まれる)を参照されたい。活性成分の有効量は、当業者により、体重、年齢及び治療目的等の要因を考慮して容易に決定することができる、治療上、予防上、又は診断上有効な量である。
HPA/PKU患者へのPAL及びその類縁体の投与に加えて、患者の食事性タンパク質もまた、限定又は変更可能であることが意図される。当業者は、PKUの治療に用いるさまざまな市販のタンパク質配合物を認知している。そのような配合は、MAXIMAID, PHENEX 1, PHENEX 2(Ross Laboratories社製, Liverpool, UK)、LOFENALAC、PHENYL−FREE(Mead-Johnson社製)等を含む。
本明細書の全体にわたって考察されるように、所定の患者が、PAL治療に感受性があるか否かを決定すること、及び、治療規制の効果を監視するために、治療されるPKU患者の種類を確認する初期と治療規制の実行時の両方で、当該患者のフェニルアラニン濃度を測定することが本発明のさまざまな実施態様において必要である。そのような方法の例は、以下に記載されている。
BH4負荷試験は、BH4の欠乏による、又はPAHの欠乏を通じて、HPA患者間の識別を可能とする。
血液中のPheの測定方法は、多数存在する[Shawら, 「フェニルケトン尿症における分析方法−臨床生化学 (Analytical Methods in Phenylketonuria-Clinical Biochemistry)」、Bickettら, Eds., 「フェニルケトン尿症及び他のアミノ酸代謝の先天異常(Phenylketonuria and Some Other Inborn Errors of Amino Acid Metabolism)」 Stuttgart, Georg Thiem Verlag, 47-56 (1971)]。一般に、フェニルアラニン及びチロシン濃度は、蛍光分析を用いて患者の血清から測定される。この分析は、ロイシルアラニンの存在下でニンヒドリンと共にフェニルアラニンを加熱した場合の蛍光物質の形成に依拠する[McCamanら, J. Lab. Clin. Med. 59:885-890 (1962)]。
本発明の特定の方法は、さまざまな形態のHPAの患者に治療結果をもたらすためのPALと食事性タンパク質制限の併用を含む。本明細書で意図される併用療法での適切な治療結果を達成するために、一般に、被験者に、PALの組成物、及び、所望の治療結果をもたらす(すなわち、血漿Phe濃度を低減させること、及び/又は、血漿Phe濃度の同時増加を生じることなく、多量のPhe/タンパク質摂取を許容する能力)のに有効な併用量の制限食を投与する。このプロセスは、PALの組成物、及び食事性タンパク質の治療用組成物を同時に投与することを含んでもよい。 これは、食事性タンパク質の必要物をすべて含み、また、当該タンパク質配合物でPALを含む、単一の組成物、又は薬理学的タンパク質配合物を投与することにより達成することができる。あるいは、食事性タンパク質(補給物、又は一般のタンパク質食品)を、PALの薬理学的タンパク質配合物(タブレット、注射又は飲料)とほぼ同時に摂取する。また、原核生物PALは、プロテインバー、又は、摂取に適しているブラウニー、パンケーキ、ケーキ等の他の食品に配合してもよい。
本発明の別の態様は、上昇したフェニルアラニン量を有する細胞を細菌PALと接触することによりフェニルアラニンの上昇した量を防止、改善又は低減させることができる、細菌PALの同定、及び、細菌PALが、そのような上昇したフェニルアラニン量を低減させるか否かを判断するためのスクリーニング分析である。特定の実施態様においては、当該方法は、高スループット分析である。ある実施態様においては、細菌種の完全なゲノムは、バイオインフォマティクスによるアプローチを用いて、PAL相同体の存在についてシーケンス及びスクリーニングされる。他の実施態様においては、encPと相同性のある遺伝子が同定され、関連するタンパク質配列のBLAST解析が行われ、同一性の割合が決定される。そのようなタンパク質の一次配列整列は、PAL活性にとって不可欠な保存残基の存在のために評価される。すべてのHAL酵素の位置83(シュードモナス プチダのHAL)は、ヒスチジンであるが、それは、脂肪族アミノ酸(例えば、PAL活性を有する酵素におけるロイシン又はバリン)である。位置83の解析は、酵素であるHALが誤って示されるか否かについて解明し、PAL活性を実証する。例えば、位置413は、大部分のHAL酵素においてグルタミン(Q)であり、すべてのHAL酵素のグルタミン酸(E)が位置414に続く。それに比べて、位置414は、これまでに同定された大部分のPAL酵素のアスパラギン酸(D)又はアスパラギン(N)が位置415に続くグルタミン(Q)である。原核生物PAL酵素としてよく誤って示されがちなこのバイオインフォマティック法で決定される代表的なHAL酵素は、ロドバクター スフェロイデス(Rhodobacter sphaeroides) 2.4.1 (ZP 00005404)、ルブロバクター キシラノフィラス(Rubrobacter xylanophilus) DSM9941 (ZP 00188602)、フォトラブダス ルミネッセンス亜種ラウモンディイ(Photorhhabdus luminescens subsp. Laumondii) TT01 (NP 930421)、及び、サーモアナエロバクター テングコンゲンシス(Thermoanaerobacter tengcongensis) (AA051415)のHAL酵素を含む。
本発明の他の態様は、原核生物PALの製造方法である。ある実施態様においては、IPTG(イソプロピル−β−D−チオガラクトピラノシド)等の誘導プロモーターを用いて、ベクター[例えば、大腸菌BL21(DE3)/pLysS(Invitrogen社製)]において、N末端オクタヒスチジル標識融合タンパク質として過剰発現される。他の実施態様においては、組換えPALは、N末端標識なしで、大腸菌BL21(DE3)/pLysS細胞において過剰発現される。バイオリアクター/発酵槽において、振盪フラスコのグリセロールストックから、種培養物を増殖させる。その後、そのような種培養物を、供給バッチモードの制御されたバイオリアクターへ導入する。グルコースを補充し、pHを塩基(NH4OH)で調整し、1200rpm以下で攪拌する。O2供給は、20%より高くなるように溶存酸素を維持する。当該細胞を、30℃の温度で、70〜100(〜22〜25時間)のOD600になるまで増殖させ、その後、0.4mM IPTGで誘導する。温度を22〜26℃まで低下させ、0.1 IU/mLより低く(約40〜48時間、及び一般に200のOD600)なるまで増殖させる。細胞培養培地は、一般に、酵母エキスタンパク質、ペプトン−トリプトン、グルコース、グリセロール、カザミノ酸、トレース塩類及びリン酸緩衝塩類と定義され、これらから構成される。組換えPAL生産物は、細胞内で生産され、分泌されない。細菌は、連続遠心分離(αLaval, Carr, Ceba、又はこれらの等価物)により回収される。
本発明の更なる態様は、細菌PAL、又は生物学的に活性のあるそのフラグメント、突然変異体若しくは類縁体の精製方法を特徴とする。第一の実施態様によれば、形質転換された細胞群を増殖させ、残りの粗組換え酵素は破壊される。異物は、カラムの汚損を防止するために、通常粗バルクから分離される。クロマトグラフィー精製は、1つ又は幾つかのクロマトグラフィー樹脂を用いて行われる。次いで、精製されたタンパク質は、長期間にわたって安定した活性を提供するために、設計された緩衝液で調製される。他の実施態様においては、細菌PALの精製方法は、(a)圧力ホモジナイザー(但し、滞在的には、ガラスビーズ溶解などの他の物理的手段による)を用いて、組換えPALを含む細菌を溶解する工程;(b)熱処理をする工程;(c)第二の連続遠心分離工程、及び/又は深層ろ過(Cuono Zeta Plus、Maximizer、Pall Filtron、Millipore Millistak、又はOpticaoフィルターで)によりこの溶解物を浄化する工程;(d)炭ろ過工程に通過させる工程(Millipore Millistak 40ACで);(e)最終的なろ過工程に通過させる工程(Sartorious Sartopore 0.2μmフィルターで);(f)ブチル疎水性相互作用クロマトグラフィーに通過させる工程(Tosoh Biosciences社製のToyopearl Butyl 650Mで);(g)Qイオン交換カラムに通過させる工程(BioRad社製のMacroprep High Qで);及び、(h)タンジェンシャルフローろ過での緩衝液交換により最終生産物を回収する工程(Sartorious Hydrosart又はPES 100kDa膜で)。当業者であれば、1以上のクロマトグラフィー工程を省略若しくは置き換えること、又は、本発明の範囲内でクロマトグラフィー工程の順序を変更することができるということを容易に認識する。そして、必要であれば、適切な滅菌工程を行ってもよい。

ノストック パンクチフォルメのゲノムDNAをATCC(29133D)から入手した。また、PAL遺伝子(ZP_00105927)を、プライマー

大腸菌BL21(DE3)細胞(Stratagene社製)をpGro7(Takara社製)で形質転換した。また、適切なBL21(DE3)pGro7細胞をInoue法[Sambrook及びRussell, 「分子クローニング:実験マニュアル(Molecular Cloning: A Laboratory Manual)」, 第3版 (Cold Spring Harbor Laboratory Press, Cold Spring Harbor, 2001)]により調製した。これらの細胞を、pET−28−NpPALで形質転換し、50mg/Lカナマイシン及び20mg/Lクロラムフェニコール添加25ml LB培地において、37℃で一晩培養した。20mLのこの培養物を、カナマイシン、クロラムフェニコール、及び500mg/L L−アラビノース添加1L LB培地に播き、37℃で増殖させた。0.6のOD600で、当該培養物を氷で冷却した。5分後、当該培養物を0.3mM IPTGで誘導し、20℃で16時間増殖させた。細胞を遠心分離により回収した。
培養物を卓上型遠心分離機(bench-top centrifuge)において、5,000gで20分間遠心分離し、上清を廃棄した。細胞のペレットを、更なる処理に先駆けて、通常は、-70℃で凍結した。解凍において、当該細胞のペレットを、TBS(25mM Tris, 150mM NaCl, pH7.8)中で約80光学濃度ユニット(600nm)に懸濁した。細胞を、APV圧力ホモジナイザーに12〜14,000psiで2回通過させて溶解した。次いで、粗溶解物を55℃で2時間加熱処理した。当該溶解物を10,000gで30分間遠心分離し、上清を保存し、0.2μm真空フィルター(コーニング社製)でろ過した。
(タンパク質のペグ化)
ペグ化は、文献の方法[Hershfieldら, (1991), 前述の箇所; U.S. Patent No. 6,057,292; Luら, Biochemistry 40(44):13288-13301 (2001); Nektar Therapeutics, 2003 catalog]の改良を用いる。活性化PEGは、線状PEGスクシンイミジルスクシナート(mPEG-SPA、分子量5kDa又は分子量20kDa)及び分岐PEGヒドロスクシンイミド(mPEG2−NHSエステル、分子量10kDa又は分子量40kDa)を含む。両者は、メトキシ基で一端が覆われており、Nektar Therapeutics社から市販されている。また、最適ペグ化タンパク質の試験的測定が一般に求められている[Veronese, F.M.ら, J. Bioactive Compatible Polymers 12:196-207 (1997)]。最適ペグ化条件は、さまざまなPAL:PEG比(タンパク質単量体当たりのリジンの数に沿ったタンパク質のモル比を考慮する)、さまざまなpH、さまざまな緩衝液、さまざまな温度、及びインキュベーション時間を用いて決定された。野生型PALは、リジンの数が多く[ロドスポリディウム トルロイデス(Rt)及びアナベナ バリアビリス(Av)単量体当たりそれぞれ29及び18]、非変性PALは、マウスでの反復注射に対し免疫反応性を示し、裸の野生型PALは、プロテアーゼへの暴露で急速に不活性化されるので、高いPALタンパク質:PEG誘導体化の比が必要である。ペグ化反応は、-20℃で凍結することで停止され、試料は、SDS−PAGE、MALDI−TOF質量分析、活性評価、タンパク質分解感受性、及び免疫反応性により分析される。
非変性タンパク質試料及びペグ化タンパク質試料において、280nmのPAL吸光係数(RtPAL及びAvPALにおいてそれぞれ、0.5及び0.83mg mL-1cm-1)を用いて、タンパク質濃度を測定した。また、当該濃度は、正確なタンパク質濃度の測定を妨害する非タンパク質汚染物質を除去する試料処理を含む、NIタンパク質分析キット(GenoTechnology社製)を用いて算出される。
動力学的モードのCary UV分光光度計(Cary 50)を用いて、PALの活性測定を行う。290nmでの吸光度の増加によってモニターされたtrans−桂皮酸の生成を測定することにより[Hodgins, (1968), 前述の箇所]、L−フェニルアラニン基質でのPALの活性を室温(25℃)で評価した。290nmのtrans−桂皮酸のモル吸光係数は、10,238リットルM-1cm-1である。反応混合物は、100mM Tris−HCl緩衝液(pH8.5)中22.5mMのフェニルアラニンを含んでいた。標準測定においては、最終酵素濃度が0.0035mg/mLであるが、動力学的試験においては、分析での酵素濃度は、分当たりの290nmでの勾配が0.005〜0.02の範囲にあるように、調節される。活性データは比活性(μmol×min-1mg-1)で表される。1ユニットのPALは、室温で分当たり1μモルのtrans−桂皮酸を生成する酵素量と定義される。
生化学的特性評価の後、最も有望なPEG−PAL候補物を、3つの異なる補足的方法(ウェスタンブロット、ELISA及び免疫沈降(IP))を用いて、野生型PAL(ペグ化されていない)を注入したPKUマウスにより産生された抗体に対する免疫反応性のスクリーニングを行った。
ロドスポリディウム トルロイデスからの野生型PALに関するプロテアーゼマッピング試験では、タンパク質分解性の主要部位が示された。そのような部位の除去は、タンパク質分解感受性を低減させ又は除去させ、有用なPKU酵素基質の開発に寄与した。しかしながら、タンパク質分解感受性のためのそのような部位の除去により、酵素活性低減又は失活となるものと考えられた。
一般に、NpPAL及びAvPALのペグ化は、タンパク質とSUNBRIGHT ME−200HS 20kDa NHS活性化PEG(NOF)を混合することを含むものであった。
タンパク質溶液(0.1mL)を0.9mLの新鮮なMQ水で希釈し、0.5EU/mlの感受性レベルでエンドトキシンのために携帯型のCharles River社の装置(EndoPTS)で試験した。エンドトキシンが0.5EU/mlより高い場合、エンドトキシンをMustang Eろ過で最初に、次いで、Sterogene Etox樹脂で、任意に更にクロマトグラフィー精製により低減させた。低減は制限されるが、pH7.8でDEAE FF(Amersham社製)に通すことで十分に有用であった。
タンパク質を25mg/mLより高く75mg/mL以下に濃縮し、50mM KPO4(pH8.5)に緩衝液交換した。スピンフィルターを用いてこの濃縮物を調製する場合、最初に、当該フィルターを、緩衝液単独で、低減された速度と時間で(3000rpm、3分)回転することでエンドトキシンの試験をし、次いで、工程1)のタンパク質と同様の方法で、エンドトキシンのために保持された緩衝液を試験した。50mM KPO4(pH8.5)の緩衝液バッチの履歴/配合は、水(QS〜1L)、リン酸カリウム(48.75mM、8.4913g/l)、及びリン酸二水素カリウム(1.25mM、0.17011g/L)であった。溶液を0.2μmフィルターでろ過し、室温で保存した。濃縮生産物をMustang Eフィルターアクロディスクでゆっくりろ過した(1〜2mL/分)。滅菌TBS(pH7.5)で希釈及びブランクとされた試料を、タンパク質濃度をA280で測定した。吸光係数は、NpPALで0.83、AvPALで0.75であった。
通常-80℃で保存されたPEGを室温になるまで加温した。KPO4緩衝液をPEGに加え、大きな塊がすべて懸濁されるために、最大速度で攪拌すること、及び、試験管を手で激しく振盪することで再懸濁した。最初に湿らせたPEGのあるよく懸濁されたPEG溶液に、タンパク質を1分以内に加え、非常に穏やかな転移によって混合した。アルミニウムホイルで包まれた試験管を選鉱機の軸に載置し、室温で3時間、非常に穏やかに振動させた。当該試験管をTBS(pH7.5)で充填し、ろ過滅菌した。懸濁液をすぐに配合し、あるいは、配合されるまで4℃で保存した。
配合緩衝液の配合/バッチの履歴は、水(QS〜1L)、トリス系(3.2mM)、Tris−HCl(16.8mM)及び塩化ナトリウムからなり;緩衝溶液を0.2μmフィルターでろ過し、室温で保存した。当該緩衝溶液を、100MWCO再生化セルロース膜で、Vivaflow 50(小さなロット)又はVivafiow 200(大きなロット)を用いてタンジェンシャルフローろ過にかけた。当該溶液を、MQ水、0.1N NaOH及び200mLの水で再度フラッシングした。当該溶液を50mL/分の直交流でTBS(pH7.5)により平衡化した。透過液のpHを7.5となるようにした。
本研究の目的は、次のNpPAL又はペグ化NpPALの皮下投与の血漿フェニルアラニン(Phe)量を決定することであった。
NpPALの短期間投与についての研究の結果では、過剰のフェニルアラニン量の低減において、ペグ化酵素の有効性が実証された。特に、ペグ化されていないNpPALでは、注入後(免疫原性反応がある前であっても)フェニルアラニン量は低減されなかった。したがって、ペグ化は、NpPALによるPKUの潜在的な治療のために絶対的に必要であった(図5)。
本研究の目的は、次のAvPAL又はペグ化AvPALの皮下投与の血漿フェニルアラニン(Phe)量を決定することであった。
AvPALの短期間投与についての研究の結果では、過剰のフェニルアラニン量の低減において、ペグ化酵素の有効性が実証された。特に、ペグ化されていないAvPALでは、注入後(免疫原性反応がある前であっても)フェニルアラニン量は低減されなかった。したがって、ペグ化は、AvPALによるPKUの潜在的な治療のために絶対的に必要であることを示した(図6)。
本研究の目的は、フェニルケトン尿症の疾病モデルであるENUマウスにおける、ペグ化された及びペグ化されていないR91K PALとペグ化NpPALの投与での慢性(90日間)の薬力学的パラメーター及び免疫原性の影響を評価することであった。
図7に示す長期間の投与試験では、10週間にわたる多数の毎週の注入(NpPAL−PEGでの)後の継続的な有効性が明らかとなった。
細菌で発現され組換えタンパク質に生じる凝集を低減させるために、AvPALポリペプチドにおいてアミノ酸置換を行った。タンパク質凝集は、酵素活性を低減させた、及び/又は、インビボでの免疫原性を増加させた。凝集のそのような形成が鎖間ジスルフィド結合の形成を生じさせた。このような可能性を最小限にするために、さまざまなAvPALのシステイン残基を単独で、あるいは組み合わせて、セリン残基と置換した。
フォワードプライマーAvarPALfor
及び、リバースプライマーAvarPALrev
を有する、アナベナ バリアビリスゲノムDNA(ATCC 29413-U、Qiagen DNeasyキット)からAvPAL遺伝子を増幅した。得られたPCR産物をTaqで処理した後、pCR2.1 TOPO TA(Invitrogen社製)にライゲーションした。得られたプラスミドを1p40と命名した。
及び、リバースプライマーNhe−AvPALrev
を有する1p40から増幅した。また、下流AvPALフラグメントをフォワードプライマーNhe−AvPALfor
及び、リバースプライマーAvPALrev−r
を有する1p40から増幅した。1回のPCR反応において、2つのPCR産物をDNAポリメラーゼでアニール化し増長して、完全長のAvPAL遺伝子を生産した後、プライマーN−Nhe−AvPAL及びAvPALrev−rで増幅した。得られたPCR産物をNheIで消化し、Klenow(NEB)で鈍化し、NotIで消化し、pET−28a(+)ベクター(NdeIでの消化、Klenowでの鈍化、及びNotIでの消化により調製した)にライゲーションした。このプラスミドを3p86−23と命名した。
及び、リバースプライマーAvSmaIrev
を有するプラスミド3p86−23で増幅した。得られたPCR産物をEcoRI及びSmaIで消化し、EcoRI及びSmaI消化pIBX7ベクターにライゲーションした。得られたプラスミドを7p56 Av3と命名した。
AvPALポリペプチド位置503及び565に対応する、AvPAL遺伝子の2つのシステインコドンを部位特異的突然変異誘発(QuickChange XL II、Stratagene社製)によってセリンコドンと置換した。位置503のシステインコドンを、フォワードプライマーAv_C503S
及び、リバースプライマーAv_C503Srev
でPCRによってプラスミド7p56 Av3のセリンコドンに変換した。セリンコドンに下線を付した。また、コード鎖のGからCの突然変異(非コード鎖のCからGの突然変異)を太字で示した。得られたプラスミドをj282と命名した。位置565のシステインコドンを、フォワードプライマーAv_C565S

本研究の目的は、インビトロでのフェニルアラニンアンモニアリアーゼ(PAL)酵素活性に対するAvPALポリペプチドのさまざまなシステイン残基のセリンによる置換の影響を判断することであった。
本研究の目的は、(1)促進された安定性、(2)凝集体の形成、及び(3)部位特異的ペグ化に対する、AvPALポリペプチドでのさまざまなシステイン残基のセリンによる置換の影響を判断することであった。
インビトロでの安定性に対する、AvPALのシステイン残基のセリンによる置換の影響を、精製AvPALシステイン突然変異体(ペグ化されている又はペグ化されていない)を37℃でさまざまな期間保存した後、実施例3に記載したとおりに、これらのタンパク質のインビトロでのPAL比活性を測定することにより判断した。
溶液中のタンパク質凝集体の形成に対する、AvPALにおけるシステイン残基のセリンによる置換の影響を、ペグ化されていない精製野生型AvPAL及びAvPALシステイン突然変異体を、変性及び天然ゲル電気泳動又はSEC−HPLCのいずれかによって分離することにより判断した。
部位特異的ペグ化に対する、AvPALにおけるシステイン残基のセリンによる置換の影響を、実施例6に記載したとおりに、野生型AvPAL、及び二重システイン突然変異体AvPAL_C503SC565Sをペグ化し、AvPALリジン残基:K2、KlO、K32、Kl15、K145、K195、K301、K335、K413、K419、K493、K494及びK522での相対的ペグ化を比較することにより判断した。
本研究は、細菌で発現されたAvPALタンパク質の凝集メカニズムを調査するために行なわれた。
本研究の目的は、インビボでのマウスのフェニルアラニン(Phe)量に対する、AvPALポリペプチドの位置503及び565のシステイン残基のセリンによる置換の影響を判断することであった。
AvPAL変異体のペグ化形態を再ペグ化させてペグ化(すなわち、AvPALリジン残基のPEG分子の割合)を増加させることができるか否か、並びに、再ペグ化形態のAvPAL変異体がインビトロで酵素活性を維持するか、及びインビボでの改善された特性(すなわち、低減された免疫原性)を有するか否かを判断するために研究を行った。
二重システイン突然変異体AvPALであるAvPAL_C565SC503Sを、基本的に実施例6に記載したとおりに、さまざまなPAL:PEG比[すなわち、AvPAL_C565SC503S:SUNBRIGHT ME-200HS 20kDaのNHS活性化PEG(NOF)]でペグ化した。
さまざまなPAL:PEG比でペグ化されたAvPAL_C565SC503Sを、基本的には、実施例6で記載したとおりに、さまざまな濃度のペグ化酵素、AvPAL酵素のさまざまな量及び比のリジン残基、及びSUNBRIGHT ME-200HS 20kDa NHS活性化PEG(NOF)分子を用いて再ペグ化した。
全タンパク質分析:
ペグ化及び再ペグ化rAvPAL−PEG試料の濃縮物を、基準としてのウシ血清アルブミンと共に、Pierce社(cat#23227)から市販されている分析キットを用いて、ビシンコニン酸(BCA)分析によって判断した。簡潔には、25μLの試料と基準を37℃で30分間、200μLのBCA試薬とインキュベートした。562nmの吸光度をSPECTRAmax PLUSマイクロプレート分光光度計(Molecular Devices Corporation社製)で読み取った。
ペグ化及び再ペグ化rAvPAL−PEG試料の比活性を、実施例3に記載した確立されたPAL活性分析プロトコールに基づいて測定した。簡潔には、1mL分析反応液にタンパク質試料(25〜50μL)の添加において、L−Pheがtrans−桂皮酸に変換されると、OD290の増加がモニターされる。必要に応じて、分析前に、分析希釈緩衝液(10mM Tris−HCl、135mM NaCl, pH7.3)で試料を希釈した。測定点はそれぞれ、3つの独立した測定の平均値を表わした。
CGE分析をBeckman-Coulter社(cat # 390953)のProteomeLab SDS−MW分析キットを用いて行った。ペグ化及び再ペグ化rAvPAL−PEG試料、並びにびrAvPAL−PEG−RP1試料を95℃で5分間SDS及びβ−メルカプトエタノールの存在下で加熱した後、Beckman-Coulter社のP/ACE MDQキャピラリー電気泳動システムのキャピラリー(PA800 CE, 23cm 溶融シリカ)にロードした。CGEプロファイルは、214nmのUV吸光度を示す。それぞれのCGE試料は、分析のための基準としての10kDaの移動マーカーを含んでいた。キャピラリーは、rAvPAL−PEG試料間で再度調整された。
トリニトロベンゼンスルホン酸(TNBS)分析を用いて、ペグ化及び再ペグ化rAvPAL−PEG試料に存在する遊離アミン基を推定した。
実施例10に記載のrAvPAL−PEG及びrAvPAL−PEG試料のトリプシンペプチドマッピングを行って、AvPAL変異体タンパク質におけるリジン残基のペグ化割合を測定した。ペグ化の量、すなわち、再ペグ化AvPAL_C565SC503SのAvPALのリジン残基K10、K32、K115、K145、K195、K301、K335、K413、K493/K494及びK522のPEG分子の割合を測定した。
rAvPAL酵素活性:
図18(上段)に示すとおり、ペグ化及び再ペグ化は、インビトロでのrAvPAL酵素活性を一時的に低減させた。これは、触媒活性部位付近の特定のチロシン残基の可逆的なペグ化によるものと思われた。単一のペグ化と比較して、再ペグ化は、rAvPAL酵素活性(すなわち、その比活性が一時的に低減され、徐々に時間の経過で回復した)に及ぼすペグ化反応の効果が向上するものと思われた。ペグ化反応におけるPEGの高い濃度は、比活性の大きな一時的低減と関連していた。しかしながら、4℃で数週後に、さまざまなPAL:PEG比並びに酵素及びPEG分子の濃度(AvPAL及びPEGのリジン残基のmM濃度で表される)で最初に生成された再ペグ化rAvPAL−PEGの比活性は、1:3のPAL:PEG比(2mMリジン残基:6mM PEG、●)で生成された単一のペグ化AvPAL−PEGの約80%となった。また、ペグ化及び再ペグ化AvPAL-PEG酵素は、ペグ化後に同程度の活性回復を示した。
CGE分析は、再ペグ化が、単一のペグ化と比べて、全体的な範囲を増加させることを示した。図19Aに示すとおり、再ペグ化により、付加的なPEG分子の付加により増加した分子の大きさを意味する、rAvPAL−PEGの電気泳動移動度が低減された。また、ペグ化の程度は、再ペグ化工程時のPEGの濃度に非常に関連していた。図19Bに示すように、最初のペグ化工程時の反応条件に関係なく、6mM以上の高濃度のmPEG20K−NHSは、rAVPAL−PEGのペグ化を増加させるのに同様に有効であった。要約すると、単一ペグ化rAvPALと比較して、再ペグ化rAvPAL−PEGは、低減されたCGE移動度、したがって増加された分子量を明確に示した。
TNBS分析を行い、単一ペグ化rAvPAL−PEGと比較して、再ペグ化rAvPAL−PEGに存在する遊離アミン基の量を推定した。表13に示すとおり、6mMのような高濃度のPEGの存在下における再ペグ化は、rAvPALタンパク質の遊離アミンが最も有意に低減され、これは、ペグ化の全体的な増加を示すものであった。本明細書中に用いられる条件下においては、化学的改質に利用される再ペグ化されていないrAvPALの大部分の遊離アミンは、再ペグ化後に効果的にペグ化される。
図20に示すとおり、トリプシンペプチドマッピングは、再ペグ化がrAvPAL−PEGのリジン残基のペグ化の割合を改善することを示した。単一のペグ化工程時に既に100%ペグ化されているAvPALの位置10のリジン残基を除いて、残りの結合していないリジン残基は、再ペグ化におけるペグ化の標的とされた。特に、AvPALの位置301のリジン残基のペグ化は、再ペグ化において、500%も増加した。6mMのような高濃度の活性化mPEG20K−NHS存在下での再ペグ化条件は、2mMのような低いPEG濃度より、rAvPAL−PEGのペグ化の程度が非常に効果的に増大すると思われた。
実施例15に記載したとおりに1回及び2回のペグ化反応で1:3のPAL:PEG比で生成された再PEG化AvPAL_C565SC503S(rAvPAL−PEG−RP1)を、PKUのマウスモデルの血漿Phe量を低減させる及びインビボでの抗体反応を誘発する能力に関して、実施例6及び15に記載に記載したとおりに1:3のPAL:PEG比で生成された単一ペグ化AvPAL_C565SC503S(rAvPAL−PEG)と比較した。
次の実施例は、本発明の治療方法における、原核生物PAL、又は、生物学的に活性のあるその変異体、突然変異体、及びそのフラグメント("PAL")を含む組成物の臨床的評価に用いられるパラメーターに関する手引きを提供するものである。本明細書の全体にわたって考察されるとおり、PALを、HPA、軽症型のフェニルケトン尿症(PKU)及び古典的PKUを含むHPAの治療に用いる。臨床試験が行われ、安全性、薬動力学、並びに、代用薬及び明確な臨床的エンドポイントの初期反応におけるPALの経口又は皮下用量の評価を提供する。当該臨床試験は、最小限で行われ、100人の評価可能な患者の十分な安全性情報を収集するのに24週必要であるが、必ずしもこれに限定されない。当該臨床試験の初期用量は、約0.001〜約1.0mg/kg/週で変動する。この用量が、患者の過剰な血漿フェニルアラニン(Phe)量の低減がなされない、又は血漿Phe量の増加のない毎日の経口によるPhe摂取を増加させる能力として測定された重要な直接的な臨床的利益を生まない場合は、当該用量は、必要に応じて増加され、安全性を確立し、更なる有効性を評価するのに24週の、必ずしもこれに限定されない追加的な最小期間維持される。
上昇した血漿Phe量を有する患者は、病歴及び健康診断、神経心理及び認識機能検査、臨床検査(CBC、Panel 20、CH50、UA)の標準セット、尿のプテリン量、ジヒドロプテリジン還元酵素(DHPR)量、並びに、血清アミノ酸の血液(血漿)パネルの絶食という、ベースラインを経験する。患者は、病院への毎週の通院で緊密に続く。患者は、治療期間の終了1週間後、完全な評価のために病院に戻る。用量増加が必要であれば、患者は、上述に概説された同様のスケジュールに従う。安全性が試験全体にわたってモニターされる。
患者は、男性か女性であり、血液中の上昇したPhe量の遺伝子検査及び証拠によって確認されたHPA又は軽症型PKUの裏付けのある診断を受けることができる。本研究は、厳格な食事制限をしないHPA又はPKUの患者を含むものである。出産の可能性のある女性患者は、それぞれの投与直前に、陰性の妊娠テスト(尿β−hCG)をしている必要があり、本研究の全体にわたって、医学的に許容し得る避妊方法を利用するようにアドバイスする必要がある。患者が妊娠している若しくは乳を分泌している、研究登録前30日以内に臨床試験用医薬品を使用している、又は、研究コンプライアンスを顕著に減少させる病状、重大な併発性の疾病若しくは他の軽減する状況がある場合、患者は、この研究から除外される。
初期のランダム化及び2週間の治療期間に従って、研究参加者は全員、食事のカウンセリングを経験し、標準食事及び/又は合計4〜6週のPhe特効性医学的食品で補足された標準Phe制限食が続く。食事は、患者の自宅で管理され、食事摂取は、日誌に記録される。栄養素及び医学的食品の摂取及び推奨食事摂取(RDI)の割合の分析は、治療グループ間で比較される。
PAL治療は、深刻な急性又は慢性の薬物反応が研究時に生じない場合には、安全であると判断される。薬剤の非常に長期的の投与は、深刻な異常が臨床検査、臨床研究所、又は他の適切な研究でみられない場合には、安全であると判断される。
ペグ化二重システイン突然変異体AvPALであるAvPAL_C565SC503Sは、ヒトで臨床的に評価される。本臨床的評価の目的は、安全性、耐用性、薬動力学(PK)及びPKU患者における有効性を判断することである。血液フェニルアラニン(Phe)量は、臨床的なエンドポイントとして有用である。
フェーズ1は、16〜50歳の35人のPKU患者での非盲検の1回投与の上昇試験である。主な目的は、ペグ化二重システイン突然変異体AvPALであるAvPAL_C565SC503Sの安全性及び耐用性を評価することであり、第二の目的は、ペグ化酵素のPK及びPhe低減を評価することである。5人の被験者の7つの集団に、0.001、0.003、0.01、0.03、0.1、0.3及び1mg/kgの上昇する投与量で、ペグ化AvPAL_C565SC503Sを連続的に投与する。それぞれの集団の当該被験者は、1回投与であり、その後、合計6週間まで継続する。包含基準は、スクリーニングでの血液Phe量及び過去3年にわたる平均血液Phe量が、≧600μMである。
フェーズ2の検討は、割り込みのない2部に分けられる:
○第1部は、16週、8週の投与後、投与量の最適化
○第2部は、40週に拡大して投与する。
フェーズ1及び2の試験が完了すれば、付加的な検討は、フェーズ3を滞在的に含んでもよい。当該フェーズ3は、特別の集団[例えば、非PKU高フェニルアラニン血症(HPA)患者、BH4反応性PKU患者及び非BH4反応性PHU患者]での付加的な試験である非常に多数の被験者における6ヶ月の二重盲検試験である。
Claims (30)
- アナベナ バリアビリスフェニルアラニンアンモニアリアーゼ(AvPAL)変異体であって、1以上のシステイン残基がセリン残基によって置換されており、該1以上のシステイン残基が位置64、318、503及び565のシステイン残基からなる群から選択され、該AvPAL変異体が、野生型AvPAL(配列番号4)と比較して、高いフェニルアラニン変換活性、及び/又は、低減された免疫原性を有し、かつ、該AvPAL変異体がペグ化されている、前記AvPAL変異体。
- AvPALの位置503のシステイン残基が、セリン残基によって置換されている、請求項1記載のAvPAL変異体(配列番号9)。
- AvPALの位置565のシステイン残基が、セリン残基によって置換されている、請求項1記載のAvPAL変異体(配列番号10)。
- 位置503及び565のシステイン残基が、セリン残基によって置換されている、請求項1記載のAvPAL変異体(配列番号11)。
- 前記ペグ化が、AvPAL変異体とNHS活性化ポリエチレングリコールとを、AvPAL変異体のリジン残基当たり3のポリエチレングリコールの比で反応させることにより達成される、請求項1記載のAvPAL変異体。
- 前記ペグ化が、AvPAL変異体とNHS活性化ポリエチレングリコールとを、AvPAL変異体のリジン残基当たり少なくとも1.6のポリエチレングリコールの比で反応させることにより達成される、請求項4記載のAvPAL変異体。
- 前記ペグ化が、AvPAL変異体とNHS活性化ポリエチレングリコールとを、AvPAL変異体のリジン残基当たり少なくとも2.4のポリエチレングリコールの比で反応させることにより達成される、請求項4記載のAvPAL変異体。
- 前記ペグ化が、AvPAL変異体とNHS活性化ポリエチレングリコールとを、AvPAL変異体のリジン残基当たり3のポリエチレングリコールの比で反応させることにより達成される、請求項4記載のAvPAL変異体。
- 前記AvPAL変異体の位置2、10、32、145、195、301、413、493及び522のリジン残基の少なくとも28%がペグ化されている、請求項4記載のAvPAL変異体。
- 前記AvPALの位置2、10、195、413、493及び522のリジン残基の少なくとも51%がペグ化されている、請求項4記載のAvPAL変異体。
- 前記AvPALの位置2、10、195、493及び522のリジン残基の少なくとも75%がペグ化されている、請求項4記載のAvPAL変異体。
- 請求項1〜11のいずれか1項記載のAvPAL変異体を含む、医薬組成物。
- 被験者のフェニルアラニンヒドロキシラーゼ(PAH)の欠乏によってすべて又は一部が生じる疾病を治療するための薬剤の調製における、請求項1〜11のいずれか1項記載のAvPAL変異体の使用。
- 前記疾病が、上昇したフェニルアラニン量によって特徴付けられる、請求項13記載の使用。
- 前記被験者が、正常PAH活性の10%以下を有する、請求項13又は14記載の使用。
- 被験者の上昇した血漿フェニルアラニン量を治療するための薬剤の調製における、請求項1〜11のいずれか1項記載のAvPAL変異体の使用。
- 被験者の上昇した血漿フェニルアラニン量を低減させるための薬剤の調製における、請求項1〜11のいずれか1項記載のAvPAL変異体の使用。
- 前記被験者の血漿フェニルアラニン濃度が、前記AvPAL変異体の投与前において、200μMよりも高い、請求項14〜17のいずれか1項記載の使用。
- 前記薬剤が、前記被験者の血漿フェニルアラニン濃度を10%以上低減させるように調製されている、請求項14〜18のいずれか1項記載の使用。
- 被験者のフェニルケトン尿症(PKU)を治療するための薬剤の調製における、請求項1〜11のいずれか1項記載のAvPAL変異体の使用。
- 前記PKUが古典的重症型PKUである、請求項20記載の使用。
- 前記被験者がPAH活性の欠乏を有する、請求項13〜21のいずれか1項記載の使用。
- 前記被験者が突然変異体PAHを有する、請求項13〜22のいずれか1項記載の使用。
- 前記突然変異体PAHが、PAHの触媒ドメインでの突然変異を含む、請求項23記載の使用。
- 前記突然変異が、F39L、L48S、I65T、R68S、A104D、S11OC、D129G、E178G、V190A、P211T、R241C、R261Q、A300S、L308F、A313T、K320N、A373T、V388M E390G、A395P、P407S、及びY414Cからなる群より選択される1以上の突然変異を含む、請求項24記載の使用。
- 前記被験者が妊婦である、請求項13〜25のいずれか1項記載の使用。
- 前記薬剤が、タンパク質制限食と併用して被験者に投与されるように調製されている、請求項13〜26のいずれか1項記載の使用。
- 前記被験者が0〜3歳の幼児である、請求項13、25又は27のいずれか1項記載の使用。
- 前記幼児の血漿フェニルアラニン濃度が、360μM〜4800μMである、請求項28記載の使用。
- 前記被験者がヒトである、請求項13〜29のいずれか1項記載の使用。
