Description
本発明は、手術用ポートアセンブリに関する。本発明のポートアセンブリは、全面的に臍部を介する腹腔鏡手術等の低侵襲外科手術において特に有用である。
腹部の腹腔鏡手術は、従来の(開腹)手術よりも胆嚢の腹腔鏡下摘出の利益が明白になった1980年代後半において普及した。術後の回復期間の短縮、術後疼痛および創傷感染の顕著な減少、ならびに整容性評価の改善は、腹腔鏡手術に関して十分に確立された利益であり、これは、主に、体腔壁のより小さな切開を利用して手術を行なう腹腔鏡外科医の能力から引き出される。
ポートアセンブリの本体は、漏斗形状を有し得る。この場合、管状部材は、その先端部において漏斗形状に取り付けられる。漏斗形状は、円形、楕円形、卵形、または他の断面を有し得る円錐台であり得る。
例えば、本発明は以下の項目を提供する。

(項目2)
前記本体は、外科手術中に、患者から離れる方向を向く外側と、患者の皮膚表面の内側に向くか、または患者の皮膚表面の方を向く内側とを有し、前記第1の管状部材は、該本体の外側において、前記第2の板から離れるように上側または外側にのみ延出する、項目1に記載のポートアセンブリ。
(項目3)
前記第1の管状部材は、全てが前記第2の板に取り付けられ、かつ前記本体の外側において該第2の板から離れるように上側または外側にのみに延出する複数の第1の管状部材のうちの1つである、項目2に記載のポートアセンブリ。
(項目4)
前記第1の管状部材のうちの少なくとも1つは、少なくとも前記第2の板への取り付け点において可撓性であり、該第1の管状部材のうちの該少なくとも1つに挿入される手術用器具の該第2の板における枢動および旋回が可能になる、項目3に記載のポートアセンブリ。
(項目5)
前記第1の管状部材は、各々、手術用器具シャフトがこのような第1の管状部材を長手方向に横断する場合に気腹を維持するための少なくとも1つのシールと、手術用器具シャフトがこのような第1の管状部材を長手方向に横断しない場合に気腹を維持するための少なくとも1つのシールとが設けられる、項目3に記載のポートアセンブリ。

Claims (26)
- 切開を介する患者における器具の設置を容易にするために、該患者の皮膚表面における該切開において該患者に取り付け可能である本体と、
該本体に対して実質的に横断的に該本体内に装着されている主要な第1の板であって、該第1の板は、第1の開口部および第2の開口部を有する、第1の板と、
該本体の中央に位置している軸に実質的に平行である補助軸の周りを回転するために、該第1の開口部に回転可能に配置されている第2の板であって、該第2の板の全体が該本体内に位置決めされており、該第1の板および該第2の板の全体が該本体内に封入されている、第2の板と、
該第2の板に取り付けられ、該第2の板から離れるように少なくとも1つの方向に延出する少なくとも1つの第1の管状部材と、
該第2の開口部において該第1の板に取り付けられ、該第1の板から離れるように少なくとも1つの方向に延出する第2の管状部材と
を備える、手術用ポートアセンブリ。 - 前記本体は、外科手術中に、患者から離れる方向を向く外側と、患者の皮膚表面の内側に向くか、または患者の皮膚表面の方を向く内側とを有し、前記第1の管状部材は、該本体の外側において、前記第2の板から離れるように上側または外側にのみ延出する、請求項1に記載のポートアセンブリ。
- 前記第1の管状部材は、複数の第1の管状部材のうちの1つであり、該複数の第1の管状部材の全てが、前記第2の板に取り付けられ、かつ、前記本体の外側において該第2の板から離れるように上側または外側にのみに延出する、請求項2に記載のポートアセンブリ。
- 前記複数の第1の管状部材のうちの少なくとも1つは、少なくとも前記第2の板への取り付け点において可撓性であり、該複数の第1の管状部材のうちの該少なくとも1つに挿入される手術用器具の該第2の板における枢動および旋回が可能になる、請求項3に記載のポートアセンブリ。
- 前記複数の第1の管状部材の各々には、手術用器具シャフトがこのような第1の管状部材を長手方向に横断する場合に気腹を維持するための少なくとも1つのシールと、手術用器具シャフトがこのような第1の管状部材を長手方向に横断しない場合に気腹を維持するための少なくとも1つのシールとが設けられている、請求項3に記載のポートアセンブリ。
- 前記本体は、外術手術中に、患者の皮膚表面の内部において、該患者の外部に配置可能な外側と、該患者の内部に配置可能な内側とを有し、前記第1の管状部材は、該本体の内側において、前記第2の板から離れるように下側または内側にのみ延出する、請求項1に記載のポートアセンブリ。
- 前記第1の管状部材は、複数の第1の管状部材のうちの1つであり、該複数の第1の管状部材の全てが、前記第2の板に取り付けられ、かつ、前記本体の内側において該第2の板から離れるように下側または内側にのみに延出する、請求項6に記載のポートアセンブリ。
- 前記複数の第1の管状部材のうちの少なくとも1つは、少なくとも前記第2の板への取り付け点において可撓性であり、該複数の第1の管状部材のうちの該少なくとも1つに挿入される手術用器具の該第2の板における枢動および旋回が可能になる、請求項7に記載のポートアセンブリ。
- 前記複数の第1の管状部材の各々には、手術用器具シャフトがこのような第1の管状部材を長手方向に横断する場合に気腹を維持するための少なくとも1つのシールと、手術用器具シャフトがこのような第1の管状部材を長手方向に横断しない場合に気腹を維持するための少なくとも1つのシールとが設けられている、請求項7に記載のポートアセンブリ。
- 前記第1の管状部材には、手術用器具シャフトが該第1の管状部材を長手方向に横断する場合に気腹を維持するための少なくとも1つのシールと、手術用器具シャフトが該第1の管状部材を長手方向に横断しない場合に気腹を維持するための少なくとも1つのシールとが設けられている、請求項1に記載のポートアセンブリ。
- 前記第2の板はドーム状である、請求項1に記載のポートアセンブリ。
- 前記第2の板は、前記第1の板に着脱可能に取り付けられている、請求項1に記載のポートアセンブリ。
- 切開を介する患者における器具の設置を容易にするために、該患者の皮膚表面での該切開において該患者に取り付け可能である本体であって、該本体は、外科手術中に、該患者から離れる方向を向く外側と、該患者の皮膚表面の内部を向くか、または該患者の皮膚表面の方に向く内側とを有する、本体と、
該本体から下側または内側に懸垂する少なくとも1つの管状部材であって、該管状部材は、該本体の該内側にのみ配置されるようにされている、少なくとも1つの管状部材と、
該本体に対して実質的に横断的に該本体内に装着されている主要な第1の板であって、該第1の板は、第1の開口部および第2の開口部を有する、第1の板と
を備え、
該本体は、漏斗形状を有し、該管状部材は、その先端部において該漏斗形状に取り付けられている、手術用ポートアセンブリ。 - 前記管状部材は、複数の管状部材のうちの1つであり、該複数の管状部材が前記本体の前記内側にのみ配置されるように、該複数の管状部材の全てが該本体から下側または内側に懸垂する、請求項13に記載のポートアセンブリ。
- 前記複数の管状部材の各々には、手術用器具シャフトがこのような管状部材を長手方向に横断する場合に気腹を維持するための少なくとも1つのシールが設けられ、該複数の管状部材の各々には、手術用器具シャフトがこのような管状部材を長手方向に横断しない場合に気腹を維持するための少なくとも1つのシールが設けられている、請求項14に記載のポートアセンブリ。
- 前記複数の管状部材の各々には、内面に沿って剛性スリーブが設けられている、請求項15に記載のポートアセンブリ。
- 前記複数の管状部材の各々は、前記本体への取り付け点の周りを枢動可能である、請求項16に記載のポートアセンブリ。
- 前記本体の前記外側には、上側または外側に延出する管状カニューレ部材が存在しない、請求項14に記載のポートアセンブリ。
- 前記本体は、縁部分と、該縁部分によって囲まれる板とを備え、前記複数の管状部材は、該板に連結され、かつ、該板の内側にのみ延出する、請求項14に記載のポートアセンブリ。
- 切開を介する患者における器具の設置を容易にするために、該患者の皮膚表面での該切開において該患者に取り付け可能である本体であって、該本体は、外科手術中に、該患者から離れる方向を向く外側と、該患者の皮膚表面の内部を向くか、または該患者の皮膚表面の方に向く内側とを有する、本体と、
該本体から下側または内側に懸垂する少なくとも1つの管状部材であって、該管状部材は、該本体の該内側にのみ配置されるようにされている、少なくとも1つの管状部材と、
該本体に対して実質的に横断的に該本体内に装着されている主要な第1の板であって、該第1の板は、第1の開口部および第2の開口部を有する、第1の板と
を備え、
該管状部材は、該本体に取り外し可能に取り付けられている、手術用ポートアセンブリ。 - 前記管状部材は、前記本体における開口部において該本体に取り外し可能に取り付けられている結合部材に固定され、該開口部から該結合部材を取り除く際に、該開口部を一時的にシールするためのプラグをさらに備える、請求項20に記載のポートアセンブリ。

該管状部材は、自由端においてカメラを担持し、該自由端および該カメラの配向を変更するための方向制御手段をさらに備える、手術用ポートアセンブリ。 - 第1の開口部と手術用器具が通過し得る別個の第2の開口部とを有する主要な板を有する本体部材と、
該第1の開口部の中に配置されているカニューレ担持部であって、該カニューレ担持部は、少なくとも1つの器具シールが各々に設けられている複数の第1の管状部材に取り付けられている、カニューレ担持部と、
該第2の開口部において該本体部材に取り付けられている少なくとも1つの第2の管状部材であって、該第2の管状部材には、シールが設けられている、少なくとも1つの第2の管状部材と
を備え、該カニューレ担持部および該第1の管状部材は、該本体部材の主軸に実質的に平行である軸の周りを回転するために、該本体部材に回転可能に取り付けられている、手術用ポートアセンブリ。 - 前記第2の管状部材は、前記本体部材に対して固定されている、請求項23に記載の手術用ポートアセンブリ。
- 前記カニューレ担持部および前記第1の管状部材は、前記本体部材に着脱可能に取り付けられている、請求項23に記載の手術用ポートアセンブリ。
- 前記第1の管状部材は、前記カニューレ担持部から離れるように上側または近位方向に延出する、請求項23に記載の手術用ポートアセンブリ。
