Description
本発明は、生体組織の開口部を閉鎖する分野に関し、同手段を実施する方法に関する。
生体組織の開口部を閉鎖するためのデバイスが開示される。デバイスはテンショナーおよびシール適用装置を有する。テンショナーは開口部に張力を印加するように構成される。テンショナーは第1の伸張部材および第2の伸張部材を有することができる。第1の伸張部材は、第2の伸張部材から離れるようにバイアスさせて構成することができる。第2の伸張部材は、第1の伸張部材から離れるようにバイアスさせて構成することができる。
図1は、生体組織閉鎖のため、例えば動脈切開全体にわたって止血を行うための閉鎖デバイス2の拡張(すなわち、拡大された)構造を示す。図1の閉鎖デバイスの遠位端のクローズアップを図2に示す。
閉鎖デバイス2の構成成分は、例えば溶融、ねじ込み、接着、溶接、ハンダ付け、研磨、あるいはクリンプ加工、スナッピング、またはこれらの方法の組み合わせなどの締りばめまたは圧入(pressure fit)の使用により直接付着させることができる。例えば、単一ピースまたは材料からの成形、ダイ打抜き、レーザー切断、放電加工(EDM)または絞り加工により、この構成成分を組み込むことができる。当業者において公知の任意の他の方法を使用することができる。
図11〜13は、閉鎖デバイス2を第1の構造から第2の構造に変更する方法を示す。それぞれ図11および13の閉鎖デバイス2の遠位端の拡大図を図14および15にさらに示す。図11および14に示すように、閉鎖デバイス2は完全に後退した構造である。内側部材6およびエキスパンダーワイヤー16aおよび16bは、供給ガイド4内に隠すことができる。
Claims (28)
- 生体組織の開口部を閉じるためのシステムであって、前記システムは、
張力を印加する要素であって、前記張力を印加する要素は、前記開口部内への前進のために構成され、かつ前記開口部に側方に張力を印加するように構成されており、前記開口部が前記側方とは反対に長手軸方向でつぶれ、前記開口部は、組織壁の外部表面と内部表面との間に位置する、張力を印加する要素と、
前記開口部に適用するために適切なシーラーと
を備えるシステム。 - 前記壁の第1の部分は、前記壁の第2の部分から0.51mm未満離れた位置に運ばれることを特徴とする請求項1に記載のシステム。
- 前記壁の第1の部分は、前記壁の第2の部分から0.38mm未満離れた位置に運ばれることを特徴とする請求項1に記載のシステム。
- 前記壁の第1の部分は、前記壁の第2の部分から0.25mmより大きく離れた位置に運ばれることを特徴とする請求項1に記載のシステム。
- 前記シーラーはエネルギーを含むことを特徴とする請求項1に記載のシステム。
- 前記エネルギーは音響エネルギーを含むことを特徴とする請求項5に記載のシステム。
- 前記エネルギーはRFエネルギーをさらに含むことを特徴とする請求項6に記載のシステム。
- 前記エネルギーはRFエネルギーを含むことを特徴とする請求項5に記載のシステム。
- 前記エネルギーは伝導熱エネルギーを含むことを特徴とする請求項5に記載のシステム。

- 前記シーラーは充填剤を含むことを特徴とする請求項1に記載のシステム。
- 前記シーラーは液体を含むことを特徴とする請求項1に記載のシステム。
- 前記接着剤は液状接着剤を含むことを特徴とする請求項12に記載のシステム。
- 前記開口部に前記シーラーを向けるための手段をさらに含む請求項1に記載のシステム。
- 前記向けるための手段は前記開口部内に展開されるために適切な向けるためのデバイスである請求項16に記載のシステム。
- 前記開口部内に展開されるために適切なウェブをさらに含む請求項1に記載のシステム。
- 前記開口部を閉じた後、少なくとも長期間、前記開口部に残されるように前記ウェブが構成されている請求項18に記載のシステム。
- 少なくとも前記ウェブが完全に生体吸収されるまで、前記開口部に残されるように前記ウェブが構成されている請求項19に記載のシステム。
- 前記開口部の少なくとも一部は動脈切開である請求項1に記載のシステム。
- 中空部材をさらに備え、前記中空部材は、前記中空部材を介して第1の外科手技が行われるように、前記開口部に挿入されるように構成されており、続いて前記中空部材に張力印加が実施される請求項1に記載のシステム。
- 生体組織の開口部を閉じるためのシステムであって、
閉鎖デバイスであって、前記閉鎖デバイスは、前記開口部に挿入され、そして前記開口部内に配置されている間に、前記開口部を長手軸方向の側方に離すように推進し、前記開口部の第1の部分を長手軸方向で前記側方とは反対に前記壁の第2の部分に隣接させるように構成されており、前記開口部は、組織壁の外部表面と内部表面との間に位置している、閉鎖デバイスと、
前記開口部に適用するために適切なシーラーと
を備えるシステム。 - 前記開口部の前記第1の部分は、前記開口部の前記第2の部分から0.51mm未満離れた位置に運ばれることを特徴とする請求項23に記載のシステム。
- 前記開口部の前記第1の部分は、前記開口部の前記第2の部分から0.38mm未満離れた位置に運ばれることを特徴とする請求項24に記載のシステム。
- 前記開口部の前記第1の部分は、前記開口部の前記第2の部分から0.25mmより大きく離れた位置に運ばれることを特徴とする請求項25に記載のシステム。
- 生体組織の開口部を閉じるためのシステムであって、
前記開口部に挿入されるように構成された第1の伸張部材と、
前記開口部に挿入されるように構成された第2の伸張部材と、
前記開口部が第1の方向と直交する第2の方向に閉じるように前記開口部を第1の方向に拡大するために、第1の伸張部材および第2の伸張部材で前記開口部上に1つ以上の力を加えることにより前記開口部を側方に拡大するための手段であって、前記開口部は、組織壁の外部表面と内部表面との間に位置している、拡大するための手段と
を備えるシステム。 - 前記開口部は動脈切開である請求項27に記載のシステム。
