Abstract
Description
a.中空針穿刺による静脈アクセス;
b.標準ガイドワイヤの挿入;
c.カテーテルアセンブリ(例えば、側孔を有するPhleboCath)を所望の位置に前進させること;
d.標準ガイドワイヤの取り外し;
e.ガイドワイヤ(5)をカテーテル内に装填し、拡張型アセンブリ(1)を前記カテーテルのスライドスリーブ(9)内に格納状態で一時的に係止すること;
f.取り付けられたガイドワイヤ(5)を押すことによってカテーテル内で拡張型アセンブリ(1)を前進させること;
g.カテーテルの内側にあるとき、またはカテーテル内でその遠位位置に達したときのいずれかに、スライドスリーブ(9)から拡張型アセンブリ(1)を解放すること;
h.閉塞しようとする場所の上にカテーテルアセンブリを配置すること;
i.拡張型アセンブリ(1)が解放され拡張されるまで、ガイドワイヤ(5)を定位置に保持したままカテーテルを引き抜くこと;
j.任意に、1つまたは複数のコンタクト(12)を介した電流を使用して、静脈壁を収縮させて緊密な閉塞を支持し、かつ/または標的静脈直径を縮小してもよい;
k.標的血管の閉塞を確実にするためにガイドワイヤ(5)を引っ張ることによって拡張型アセンブリ(1)を格納して配置すること;
l.任意に、空気フォーム食塩水または超音波造影剤を注入することによって閉鎖を試験すること;
m.フォーム硬化療法を実施すること:硬化療法の必要性に応じたカテーテルの配置、残存血液の吸引または血液負荷を空にするための脚の拳上、硬化剤フォームの注入、滞留時間1〜3分、任意に:フォームの吸引。フォーム形成(すなわち硬化剤投与)は、カテーテルの格納中にいくつかの側孔(8)を通して、または1つの前側孔もしくは側孔を介して様々な方法で行ってよい;
n.カテーテルを前進させて拡張型アセンブリ(1)を回収すること;
o.ガイドワイヤ(5)をカテーテル内の位置まで引っ張ることによって拡張型アセンブリ(1)を格納すること;
p.任意に、例えば、側枝や穿孔静脈を埋めるため、カテーテルを引く抜く間、さらなる硬化フォーム展開を実施してもよい;
q.任意に、閉塞デバイスを、硬化療法の要件に応じて、例えば、健康な静脈を保護するため、1つまたはいくつかのさらなる場所で使用してもよい;
r.ガイドワイヤ(5)およびカテーテルを静脈から外すこと。
Claims (15)
- カテーテルアセンブリにおいて使用するための可動ガイドワイヤであって、
a.遠位端拡張型アセンブリであって、格納状態から拡張状態へと変化することが可能であり、かつ、実質的に円錐形に配置されている前記拡張型アセンブリ;
b.前記拡張型アセンブリを前記ガイドワイヤに接続する枢動型ジョイントを含む
前記可動ガイドワイヤ。 - 回収可能かつ回収可能な構造に弾性的に変形可能である、
請求項1に記載のカテーテルアセンブリにおいて使用するためのガイドワイヤ。 - 前記拡張型アセンブリが不透過性膜を含み、前記膜は形状記憶材料で作製されており、網状フィルタ構造を有する請求項1または2に記載のカテーテルアセンブリにおいて使用するためのガイドワイヤ。
- 前記拡張型アセンブリが、拡張の前後で、その直径の少なくとも1:5から少なくとも1:10の範囲の拡張比を有する、請求項1〜3のいずれかに記載のカテーテルアセンブリにおいて使用するためのガイドワイヤ。
- 前記拡張型アセンブリが、少なくとも1つの接着剤成分によって血管腔の内壁に接着するように構成される、請求項1〜4のいずれかに記載のカテーテルアセンブリにおいて使用するためのガイドワイヤ。
- 前記ガイドワイヤの枢動型ジョイントが、前記ガイドワイヤに対して二次元または三次元で拡張型アセンブリを偏向させることを可能にしている、請求項1〜5のいずれかに記載のカテーテルアセンブリにおいて使用するためのガイドワイヤ。
- 前記枢動型ジョイントが、非軸ジョイント、一軸ジョイント、二軸ジョイント、多軸ジョイントからなる群から選択される、請求項6に記載のカテーテルアセンブリにおいて使用するためのガイドワイヤ。
- カテーテルアセンブリであって、
a.第1のチューブと、
b.前記第1のチューブ内に配置された請求項1〜3のいずれかに記載のガイドワイヤとを含み、
c.任意に、前記ガイドワイヤが、前記第1のチューブの中に位置する第2のチューブの中に配置されており、
前記第1および/または第2のチューブは、ポリテトラフルオロエチレン(PTFE)またはペルフルオロアルコキシ(PFA)を含む非粘着性材料で作製されているかまたは覆われており、前記第1のチューブは1つまたは複数の側孔を含む前記カテーテルアセンブリ。 - 前記カテーテルアセンブリが、格納状態で、前記カテーテルアセンブリの中に拡張型アセンブリ(1)を収容するスライドスリーブ(9)を含む請求項8に記載のカテーテルアセンブリ。
- 前記第1のチューブが、0.6〜2.2mmの範囲の内径、0.8〜2.8mmの範囲の外径、および15〜85cmの長さを有する請求項8および9のいずれかに記載のカテーテルアセンブリ。
- カテーテルアセンブリが、哺乳動物において蛇行したおよび/または分岐した血管セグメントを一時的に閉塞すると考えられる請求項8〜10のいずれかに記載のカテーテルアセンブリ。
- 血管の一時的閉塞のための請求項8〜11のいずれかに記載のカテーテルアセンブリの使用であって、以下のステップを含む方法:
a.中空針穿刺による静脈アクセス;
b.標準ガイドワイヤの挿入;
c.カテーテルアセンブリを所望の位置に前進させること;
d.標準ガイドワイヤの取り外し;
e.可動ガイドワイヤを前記カテーテル内に装填し、拡張型アセンブリを前記カテーテルのスライドスリーブ内に格納状態で一時的に係止すること;
f.取り付けられたガイドワイヤを押すことによって前記カテーテル内で拡張型アセンブリを前進させること;
g.カテーテルの内側にあるとき、またはカテーテル内でその遠位位置に達したときのいずれかに、スライドスリーブから拡張型アセンブリを解放すること;
h.閉塞しようとする場所の上にカテーテルアセンブリを配置すること;
i.拡張型アセンブリが解放され拡張されるまで、ガイドワイヤを定位置に保持したままカテーテルを引き抜くこと;
j.任意に、1つまたは複数のコンタクトを介した電流を使用して、静脈壁を収縮させて緊密な閉塞を支持し、かつ/または標的静脈直径を縮小してもよい;
k.標的血管の閉塞を確実にするためにガイドワイヤを引っ張ることによって拡張型アセンブリを格納して配置すること;
l.任意に、空気フォーム食塩水または超音波造影剤を注入することによって閉鎖を試験すること;
m.フォーム硬化療法を実施すること;
n.カテーテルを前進させて拡張型アセンブリを回収すること;
o.ガイドワイヤをカテーテル内の位置まで引っ張ることによって拡張型アセンブリを格納すること、任意に、前記ステップは側枝静脈の内腔を埋めるためのフォームの追加の注入を含む;
p.任意に、例えば、側枝や穿孔静脈を埋めるため、カテーテルを引く抜く間、さらなる硬化フォーム展開を実施してもよい;
q.任意に、閉塞デバイスを、硬化療法の要件に応じて、1つまたはいくつかのさらなる場所で使用してもよい;
r.可動ガイドワイヤおよびカテーテルを静脈から外すこと。 - 請求項8〜12のいずれかに記載のカテーテルアセンブリに使用するためのガイドワイヤであって:
a.拡張型アセンブリ、
b.前記拡張型アセンブリ上に位置する1つまたは複数のコンタクトであって、各コンタクトはコンダクタに接続された電極素子を含むコンタクト、
c.前記拡張型アセンブリを前記ガイドワイヤに接続する導電性の枢動型ジョイントであって、前記ガイドワイヤが絶縁された導電性コアを有するジョイント、および、
d.前記ガイドワイヤの導電性コアを介して前記1つまたは複数のコンタクトに伝達される電気信号を選択的に生成することができる電源
を含む前記ガイドワイヤ。 - 前記の1つまたは複数のコンタクトが、前記拡張型アセンブリが拡張状態にあるときに作動する、請求項13に記載のカテーテルアセンブリに使用するためのガイドワイヤ。
- 前記1つまたは複数のコンタクトの作動が、血管内腔の血管攣縮を誘発する、請求項13および14のいずれかに記載のガイドワイヤの使用。
