Description
本発明は、吸入用噴射剤フリーエアゾールの製造のための水溶液の形態の医薬製剤に関する。
この20年、閉塞性肺疾患、特に喘息の治療に投薬エアゾールが主に使用されるようになっている。一般的に、フルオロクロロ炭化水素が噴射剤ガスとして使用されている。これらの噴射剤ガスによるオゾン破壊の可能性が認識されるようになってから、代替物の開発の試みが増している。ある代案としては、医薬的に有効な物質の水溶液を高圧下でスプレーし、吸入可能な粒子のミストを得る噴霧器の開発がある。これらの噴霧器の利点は、噴霧器が、噴射剤ガスを全く使用しないことである。
そのような噴霧器としては、例えば、本明細書に含まれる、PCT特許出願WO91/14468に記載されている。本明細書に記載した噴霧器により規定量の有効成分溶液を高圧下で小さな噴出口を通してスプレーし、平均粒径が3〜10μmの吸入可能なエアゾールを得る。上述の噴霧器をさらに改良した態様が、PCT/EP96/04351に記載されている。図6に描かれている噴霧器は、Respimat▲Ｒ▼の商品名を有する。
一般的に、吸入するための薬剤は、水溶液又はエタノール性溶液に溶解されており、有効物質の溶液特性により、水とエタノールの溶媒混合物もまた適当であり得る。
他の溶媒成分としては、水及び／又はエタノールを除き、任意の他の補助溶媒があげられ、医薬製剤はまた、さらに香料及び他の薬理的添加剤を含有することができる。補助溶媒の例としては、ヒドロキシル基又は他の極性基を含有するもの、例えば、アルコール、特にイソプロピルアルコール；グリコール、特にプロピレングリコール、ポリエチレングリコール、ポリプロピレングリコール、グリコールエーテル、グリセロール、ポリオキシエチレンアルコール及びポリオキシエチレン脂肪酸エステルがあげられる。補助溶媒は、アジュバント物質及び、必要ならば、有効成分の溶解度を増大させるのに適当である。
最終的な医薬製剤中に溶解させた薬剤の割合は、0.001〜30％、好ましくは0.05〜3％、特に0.01〜2％（重量／体積）である。薬剤の最大濃度は、溶媒中での溶解度及び所望の治療効果を得るのに必要とされる投与量に依存する。
吸入による適用に適当であり、かつ特定の溶媒に溶解可能な物質は全て、本発明の新規製剤の薬剤として使用することができる。呼吸性通過疾患（diseases of the respiratory passages）の治療のための薬剤は特別な興味がある。それ故、ベータ作用剤（betamimetics）、抗コリン剤、抗アレルギー剤、抗ヒスタミン剤及びステロイド、並びにこれらの有効成分の組み合わせは特別な興味がある。
一連の試験において、上述の噴霧器は、水性医薬溶液（一般的に、二回蒸留又は脱塩（イオン交換）した水を溶媒として使用している）を使用する場合、スプレーの異常（spraying anomalies）を際立たせ得ることが分かった。これらのスプレーの異常は、エアゾールのスプレーパターンの変化を示す。その結果、極端な場合、小滴の平均サイズ分布が変わった結果（エアゾールが肺に近づきやすい部分への変化）として、正確な投与量はもはや患者に保証できない。これらのスプレーの異常は特に、噴霧器を一定間隔で使用する場合、例えば利用の間約3日間以上中断して使用する場合に起こる。極端な場合、噴霧器の機能不全になり得るこれらのスプレーの異常は、噴出口領域における微視的（microscopic）析出の結果である。
驚くべきことに、スプレーされる水性医薬製剤が、錯化剤、特にEDTA（エチレンジアミン四酢酸）又はそれらの塩を規定有効量含有する場合に、これらのスプレーの異常はもはや起こらないことが見いだされた。本発明の水性医薬製剤は溶媒として水を含有するが、必要によりエタノールを添加して溶解度を70体積％まで、好ましくは30〜60体積％まで増加することができる。
防腐剤、特にベンザルコニウムクロリド等の他の薬理的アジュバントを添加することができる。防腐剤、特にベンザルコニウムクロリドの量は、8〜12mg/100ml溶液であるのが好ましい。
適当な錯化剤としては、薬理的に許容できるもの、特に、既に医療統制当局（medical regulating authorities）により承認されているものがあげられる。EDTA、ニトリロ三酢酸、クエン酸及びアスコルビン酸及びそれらの塩が特に適当である。エチレンジアミン四酢酸の二ナトリウム塩が特に好ましい。
錯化剤の量は、有効量の錯化剤を添加してさらなるスプレーの異常を防ぐように選択される。
錯化剤Na-EDTAの有効量は、10〜1000mg/100ml溶液、特に10〜100mg/100ml溶液である。錯化剤の量の好ましい範囲は、25〜75mg/100ml溶液、特に25〜50mg/100ml溶液である。
以下の名前の化合物は主に、本発明の水性医薬製剤中で、有効成分として、単独又は組み合わせて使用することができる。個々のケースにおいて、溶解度を向上させるために、より多くの量のエタノール又は溶媒媒介物（solution mediator）を添加することを要求することができる。
チオトロピウム（Tiotropium）ブロミド、3-［（ヒドロキシジ-2-チエニルアセチル）オキシ］-8,8-ジメチル-8-アゾニアビシクロ「3.2.1］オクト-6-エン-ブロミド
ベータ作用剤として：
1-（2-フルオロ-4-ヒドロキシフェニル）-2-［4-（1-ベンズイミダゾリル）-2-メチル-2-ブチルアミノ］エタノール、
エリスロ-5’-ヒドロキシ-8’-（1-ヒドロキシ-2-イソプロピルアミノブチル）-2H-1,4-ベンゾオキサジン-3-（4H）-オン、
1-（4-アミノ-3-クロロ-5-トリフルオロメチルフェニル）-2-tert-ブチル-アミノ）エタノール，
1-（4-エトキシカルボニルアミノ-3-シアノ-5-フルオロフェニル）-2-（tert-ブチルアミノ）エタノール。
抗コリン剤として：
イプラトロピウムブロミド
オキシトロピウムブロミド
トロスピウムクロリド
N-β-フルオロエチレンノルトロピン（nortropine）ベンジレートメトブロミド
ステロイドとして：
ブデソニド
ベクロメタゾン（又は17,21-ジプロピオネート）

本発明の医薬製剤中で有効成分として使用できるステロイドの例：
9-α-クロロ-6-α-フルオロ-11-β-17-α-ジヒドロキシ-16-α-メチル-3-オキソ-1,4-アンドロスタジエン-17-β-カルボン酸-メチルエステル-17-プロピオネート。
吸入により適用される水性医薬製剤製造用の他の特に適当な有効成分：β-交感神経刺激作用剤（Sympatico-mimetics）；
例えば、フェノテロール、サルブタモール、ホルモテロール、テルブタリン；
抗コリン剤；
例えば、イプラトロピウム、オキシトロピウム、チオトロピウム；
ステロイド；
例えば、ベクロメタゾンジプロピオネート、ブデソニド、フルニソリド；
ペプチド；
例えば、インシュリン；
痛覚キラー（pain killers）；
例えば、フェンタニール。
必要により、本発明の溶媒に溶解するこれらの医薬的に許容できる塩が使用されるのは明らかである。
以下に、本発明の医薬製剤の利点を実施例によりさらに明らかに説明する。
医薬溶液として、pH値3.4のイプラトロピウムブロミド溶液（c=333mg/100ml）及び防腐剤ベンザルコニウムクロリド（c=10mg/100ml）を使用した。テストした溶液はいずれもEDTAを全く含まないか、又は二ナトリウム塩としてc=0.1mg、1mg、50mg及び75mg/100mlの濃度でEDTAを含有した。
使用できないRespimat▲Ｒ▼噴霧器をテスト用に使用した（技術データ：適用される医薬製剤の量；約15μl、圧力；約300bar、サイズ5×8μmの2つの噴出口から2本の流れが噴き出る）。テストの操作モードは、ユニットを5回使用し、3日間放置し、再度5回使用し、このパターンを繰り返すように設定した。各一連の測定において15ユニットを試験し、スプレーの異常に関する結果を表1に示した。
組成物の実施例（フェノテロール及びイプラトロピウムブロミドについて）
上述の実施例と同様にして、以下の溶液を製造した。
有効成分について、10mg〜20,000mg/100mlの濃度範囲は、操作当たりの投与量及びその溶解度に依存して、考えられるものである。特定の投与量は、操作当たり約12μLの治療上有効な一回の投与量に基づいて計算される。本発明の医薬製剤の有効成分濃度は、個々の投与量を変える場合には変更することができる。
錯化剤（例えば二ナトリウム-EDTA）の濃度範囲は、10〜1000mg/100mlである（溶液のpH値に依存する）。好ましい範囲は25mg〜100mg/100mlである。
ベンザルコニウムクロリドの量は、8〜12mg/100mlの範囲内であるべきである。
溶液のpHは、0.1又は1N HClにより3.2〜3.4に設定する。全ての濃度は最終的な有効成分溶液100mlに関するものである。

Claims (9)
- 吸入用噴射剤フリーエアゾールを製造するための水性医薬製剤であって、該医薬製剤が、ニトリロ三酢酸、クエン酸、EDTA又はそれらの塩である錯化剤とベータ作用剤、抗コリン剤、抗アレルギー剤、抗ヒスタミン剤及びこれらの組合せからなる群から選ばれる医薬的に有効な成分とを含有し、エタノールを70体積％未満含有していてもよい前記医薬製剤。
- 有効成分が吸入用途用である、請求項１記載の医薬製剤。
- 有効成分が呼吸器疾患治療用である、請求項１記載の医薬製剤。
- 有効成分が、フェノテロール、イプラトロピウムブロミド、ベロテック、アトロベント、ベロデュアル、サルブタモール、コンビベント、チオトロピウムブロミド（Ba 679 Br）、3-[（ヒドロキシジ-2-チエニルアセチル）オキシ-8,8-ジメチル-8-アゾニアビシクロ［3.2.1］オクト-6-エン-ブロミド］（BEA 2108 Br）及びオキシベントからなる群から選ばれることを特徴とする請求項1〜３のいずれか１項記載の医薬製剤。
- 錯化剤の濃度が25〜100mg/100ｍLであることを特徴とする請求項1〜4のいずれか１項記載の医薬製剤。
- 医薬製剤が、エタノールを60体積％まで含有することを特徴とする請求項1〜5のいずれか1項記載の医薬製剤。
- 医薬製剤が、エタノールを30体積％から60体積％まで含有することを特徴とする請求項1〜5のいずれか1項記載の医薬製剤。
- 医薬製剤が、0.001〜2g/100ml溶液の濃度の有効成分を含有することを特徴とする請求項1〜7のいずれか1項記載の医薬製剤。
- エタノールを含有しないことを特徴とする請求項1記載の医薬製剤。
