Description
骨髄吸引液には、血漿、有核始原細胞(始原細胞)、有核造血細胞、内皮細胞、および赤血球と血小板とを含む抹消血から得られた細胞が含まれている。骨髄吸引液が抹消血を含むため、吸引液は抗凝固薬を含むシリンジに集めることが好ましい。適当な抗凝固薬として、ヘパリン、クエン酸ナトリウム、およびEDTAを挙げることができる。好適には、本発明の方法に使用する骨髄吸引液は、移植を受ける患者(移植者)から採取する。それ程好適ではないが、骨髄吸引液を免疫学的に適合する別のドナーから採取してもよい。
当該基質は、多孔性で、生物学的に適合性の、移植可能な基質からなる。好適には、基質は生物活性表面を有する。生物活性表面を有する多孔性生物学的適合性の移植可能な移植片基質材料の例としては、ヒドロキシアパタイトまたはリン酸三カルシウムなどのリン酸カルシウムを含むセラミック、ならびに脱塩または鉱化骨基質が挙げられる。その他の適切な基質材料は、ポリ乳酸、ポリグリコール酸、ポリガラクチン酸、ポリカプロラクトン、酸化ポリエチレン、酸化ポリプロピレン、ポリスルホン、ポリエチレンおよびポリプロピレンなどの生体高分子を含む。また、それ以外の適切な基質材料はヒアルロン酸で、架橋、生体ガラス、およびコラーゲンにより精製してもしなくてもよい。
凝固材は、移植者(または免疫学的に適合するドナー)の静脈または動脈から採取した血液から形成される血餅でよい。さらに好適には、凝固材は、最適には移植者から採取した抗凝固処理されていない骨髄吸引液から形成される骨髄クロットである。骨髄クロットが形成される骨髄吸引液は、手術中移植片処理段階において移植者から採取することが好ましい。それができない場合、凝固材は、従来技術に周知のフィブリングルー、フィブリン凝固材、血小板ゲル、および/または血小板濃縮物でもよい。
高濃度基質を有する骨移植片材料が、濃度を高めていない移植片(特許文献1および2参照)よりも高い性能を示すということは既に示した。以下の実験は、高濃度基質と吸引した骨髄クロット(ABMC)を混合した骨材料のさらなる効果を実証した。
Claims (39)
- 多孔性生物学的適合性移植可能基質と凝固材とを含む、始原細胞の濃度が高められた複合型骨髄移植片材料であって、
始原細胞の高濃度化は、所定量の吸引骨髄を比較的少量の前記移植可能基質を通過するように流動させ、それによって吸引骨髄の始原細胞を選択的に基質の表面に付着させたことによって行われ、
前記基質は同一の外形寸法を有する非多孔性の固体と比較して、始原細胞の接着に使用可能な基質表面積が少なくとも2倍となるに十分な多孔性を有する、複合型骨髄移植片材料。 - さらに、機械的な混合手段を用いて凝固材を混合する、請求項1に記載の複合型骨髄移植片材料。
- 前記始原細胞が結合組織始原細胞を含む、請求項1に記載の複合型骨髄移植片材料。
- 前記始原細胞が幹細胞を含む、請求項1に記載の複合型骨髄移植片材料。
- 前記幹細胞が多能性幹細胞を含む、請求項4に記載の複合型骨髄移植片材料。
- 前記移植可能基質が微粒子状骨材料および繊維状骨材料を含む、請求項1に記載の複合型骨髄移植片材料。
- 前記微粒子状骨材料が同種異系移植片海綿骨粒子を含む、請求項6に記載の複合型骨髄移植片材料。
- 前記繊維状骨材料が同種異系移植片脱塩皮質骨繊維を含む、請求項6に記載の複合型骨髄移植片材料。
- 前記基質が、セラミック材、生体ガラス、コラーゲン、鉱化骨、脱塩骨、ヒアルロン酸、および合成生体高分子材料からなるグループから選択される、請求項1に記載の複合型骨髄移植片材料。
- 前記凝固材が骨髄吸引液から得られる骨髄クロットである、請求項1に記載の複合型骨髄移植片材料。
- 前記凝固材が血餅である、請求項1に記載の複合型骨髄移植片材料。
- 前記凝固材が、血小板ゲル、血小板濃縮物、フィブリン凝固材、およびフィブリングルーからなるグループから選択される、請求項1に記載の複合型骨髄移植片材料。
- 全骨髄由来有核細胞に対する始原細胞の比率が少なくとも1:20,000である、請求項1に記載の複合型骨髄移植片材料。
- 前記移植可能基質および前記凝固材が前記移植片材料中に容積比5:1から1:5で存在する、請求項1に記載の複合型骨髄移植片材料。
- 前記始原細胞、前記凝固材、および前記基質のすべてが、前記複合型骨髄移植片材料中に実質的に均一に分布している、請求項1に記載の複合型骨髄移植片材料。
- 細胞接着分子が前記移植可能基質の表面に結合している、請求項1に記載の複合型骨髄移植片材料。
- 前記移植片材料が移植者の骨治癒を引き起こすのに有効である、請求項1に記載の複合型骨髄移植片材料。

c)前記骨髄吸引液と前記基質を接触させて高濃度基質を得る段階であって、所定量の吸引骨髄を比較的少量の前記移植可能基質を通過するように流動させ、それによって吸引骨髄の始原細胞を選択的に基質の表面に付着させて始原細胞を高密度化させる段階、およ
び
d)前記高濃度基質を凝固材と機械的に混合し、始原細胞が複合型骨髄移植片材料に実質的に均一に分布した複合型骨髄移植片材料を生成する段階
を含む複合型骨髄移植片材料の調製方法。 - 前記凝固材によって、移植位置における治癒反応に重要な細胞の付着および移動を促す貴重な追加的基質を得るとともに、移植位置での新規血管の形成が促され、それが新しく移植された始原細胞およびその他細胞の栄養源となって骨治癒反応を促進する、請求項19に記載の方法。
- 前記移植可能基質が微粒子状骨材料および繊維状骨材料を含む、請求項19に記載の方法。
- 前記微粒子状骨材料が同種異系移植片海綿骨粒子を含み、前記繊維状骨材料が脱塩皮質骨繊維を含む、請求項21に記載の方法。
