ペプチド
HCVゲノタイプ1bタンパク質の保存領域からのHLA−A2結合性モチーフまたはHLA−A24結合性モチーフを有する合成ペプチドを使用した(表1)。ネガティブコントロールとして,HLA−A2結合性モチーフを有するHIV由来ペプチドを使用した。これらのペプチドはいずれも市販品を使用し,その純度は逆相高圧液体クロマトグラフィーで分析したところ90%以上であった。
血清中のペプチド特異的IgGはELISAによって測定した。まず,各ペプチドをジメチルスルホキシド(DMSO)に溶解し,−80℃で保存した。ペプチドを,クロスリンカーとしてのジサクシンイミジルスベレート(DSS)を含む0.1M炭酸ナトリウム/炭酸水素ナトリウム溶液で希釈した。ELISAプレートにペプチド20μg/ウェルで4℃で一夜反応させて結合させた。ウェルを0.05%Tween20−PBS(PBST)で3回洗浄し,プレートをブロックエース(登録商標)でブロックして4℃で一夜放置した。血漿または血清サンプルを0.05%Tween20−Block Aceで100倍,200倍および400倍にそれぞれ希釈し,得られたサンプルを各ウェル当り100μlずつ添加した。サンプルを37℃で2時間培養した後,プレートをPBSTで9回洗浄し,1000倍希釈ウサギ抗ヒトIgG(γ鎖特異的)を各ウェル当り100μl添加して37℃で2時間培養した。さらに,PBSTで9回洗浄した後,100倍希釈マウス抗ウサギIgGと共有結合させた西洋ワサビパーオキシダーゼ・デキストランポリマーを各ウェル当り100μl添加し,プレートを室温で40分間培養した。プレートを洗浄した後,テトラメチルベンジジン基質溶液を各ウェル当り100μl添加し,2.0Mリン酸を添加して反応を停止させた。カットオフ値を,健常者の光密度コントロールの平均±3SDとして算出した。
各ウェル当り100μlの血漿または血清サンプルを0.05%Tween20−Block Ace(登録商標)で希釈し,プレートのウェル中に固定したペプチド(各ウェル当り20μg)に37℃で2時間吸収させた。この操作を3回繰り返した後,上清中のペプチド特異的IgGレベルをELISAで測定した。初回の吸着でペプチド固定プレートに結合させた抗体を,各ウェル当り30μlの5M NaCl/50mMクエン酸バッファ(pH3.0)(1.0M Tris−HClの0.05%Tween20−Block Ace(pH7.5)で中和した)によって溶出し,溶出した分画中のペプチド特異的IgGレベルをELISAで測定した。
ペプチド特異的CTLの検出に使用する方法は当該技術分野において慣用されている方法である。PBMCをウェル当たり1x105個の細胞になるようにU底型96ウェルマイクロカルチャープレートの各ウェルに入れ,10μMのペプチドとともに200μlの培地中で培養した。この培地の組成は,45%RPMI−1640と,45%AIM−V(登録商標)と,10%ウシ胎児血清(FCS)と,100U/mlのインターロイキン2(IL−2)と,1mM MEM非必須アミノ酸溶液と,からなっている。培養3日目,6日日,そして9日目にそれぞれ培地半分を除いて,対応するペプチド(20μg/ml)を含んだ新しい培地と入れ替えた。培養12日日に,培養した細胞を採取し,対応するペプチドまたはネガティブコントロールとしてのHIVペプチドのいずれかを予め付着させたCIR−A2402細胞またはT2に応答するインターフェロンガンマ(IFN−γ)の産生能を試験した。各ペプチドに対して4個のウェルを使用し,アッセイは2回行った。HIVペプチドに応答するバックグラウンドIFN−γ産生は得られたデータ値から差し引いた。抑制アッセイには,ペプチド反応性CTLは,CD8単離キットを用いて精製し,そのペプチド特異的IFN−γ産生を,20μg/mlの抗HLAクラスI(W6/32,IgG2a)モノクローナル抗体,抗CD8(Nu−Ts/c,IgG2a)モノクローナル抗体または抗HLAクラスII(H−DR−1,IgG2a)モノクローナル抗体のいずれかの存在下において測定した。バックグラウンドIFN−γ産生は得られたデータ値から控除した。
値は平均+/−SDとして示している。統計的分析は,マン・ウィットニー U検定(Mann−Whitney U)試験およびカイ2乗試験を用いて行った。P<0.05値は統計学的に有意であると考えられる。
HCV感染患者12名と健常者(HD)10名の血清について62種類の各ペプチドに対して反応するIgGレベルを測定した(表1)。各ペプチドに対する抗体の陽性率を表1に示した。その結果,HCV感染患者の血中IgGと高率に反応し,かつ健常人血清とはほとんど反応しないペプチドが複数見い出された。
HLA−A24結合ペプチド6種(No3,12,13,25,32,38)およびHLA−A2結合ペプチド18種(No40からNo57まで)をHLA−A24もしくはHLA−A2陽性のHCV感染患者各5名の末梢血単核球(PBMC)と培養した。コントロールとして該PBMCをHIVペプチドと培養した。そして,それらの培養PBMCについて,対応するペプチドをパルスしたC1R−A2402細胞またはT2細胞に応答するIFN−γ産生能を調べた。その結果,HLA−A24結合ペプチド6種(No3,12,13,25,32,38)は,HLA−A24陽性HCV患者10名のPBMCからCTLを誘導することができた(図4)。また,HLA−A24結合性ペプチドによる(B)HCV由来のHLA−A2結合性ペプチド(No40からNo57)によるHLA−A2陽性HCV患者5名のPBMCからCTLを誘導することができた(図5)。
被験者
さらに広範に,HLA−A24陽性のHCV感染患者において,HCVペプチドと反応性を有するIgGの検出を行った。60名のHCV1b+患者は,第2世代または第3世代イムノアッセイ試験により判定して,抗HCV抗体(Ab)についてセロポジティブであった。患者の血清中のHCVのゲノタイプはRT−PCRにより判定した。血清サンプリングの際の患者の診断は,生化学的分析,超音波検査,コンピュータ連動断層撮影,および組織学的所見により行った。診断の結果は以下のとおりである:慢性肝炎(CH,n=24),肝硬変(LC,n=18),および肝細胞癌(HCC,n=18)。陰性対照としては,正常な肝機能を有し,肝炎ウイルスの病歴を有しない20名の健康なドナー(HD)からの血清サンプルを用いた。
HCV1bタンパク質のよく保存された領域に由来し,HLA−A24分子との高い結合スコア(Bioinformatics and Molecular Analysis Section,NIH,Bethesda,MD)に基づいて選択された44種の合成ペプチドを用いた。陰性対照としてはHLA−A24結合モチーフを有するHIV由来ペプチド(RYLRDQQLLGI(配列番号63))を用いた。結合スコアおよび配列を表3に示す。血清IgGに対するペプチドの反応性についてのスクリーニング用には,BioSyntehsis(Lewisville,TX)またはSynPep(Dublin,CA)からこれらの脱塩等級(>70%)ペプチドを購入した。その後の実験には精製したペプチド(>90%)を用いた。ペプチド特異的IgGの血漿レベルは,酵素抗体法(ELISA)により測定した。血漿サンプルにおける抗ペプチドIgGの特異性の試験は,ペプチド反応性IgGの吸収および溶出により行った。
ペプチド特異的CTL前駆体細胞の検出に用いた方法はHida ら(Cancer Immunol Immunother 2002; 51: 219-228)に記載されるとおりである。阻害試験のためには,20μg/mlの抗HLAクラスI(W6/32,IgG2a),抗CD8(Nu−Ts/c,IgG2a),抗CD4(Nu−Th/i,IgG1b),および抗HLA−クラスII(H−DR−1,IgG)モノクローナル抗体を用いた。
NS5A,HLA−A2402,およびHLA−A2601のcDNAは,それぞれHuh7(NNU50−1)細胞,C1R−A2402細胞,およびKE4−CTL細胞からRT−PCRにより得て,発現ベクターpCR3.1(Invitrogen,SanDiego,CA)にクローニングした。Huh7(NNU50−1)細胞株は,HCV1bに感染した細胞株に由来するレプリコン細胞であり,NS3からNS5Bのタンパク質を発現する(Kishine et al., Biochem Biophys Res Commun 2002; 293: 993-999)。次に,100ngのNS5A,HLA−A2402またはNS5A,およびHLA−A2601cDNA(陰性対照として)を,0.6μlのFugene6(Roche Molecular Biochemicals,Indianapolis,IN)を含む100μlのOpti−MEM(Invitrogen)中で混合し,30分間インキュベートした。次に混合物100μlをCOS−7細胞(1´104細胞)に加え,これを6時間インキュベートした。20%FCSを含む100μlのRPMI−1640培地を混合物に加え,COS−7細胞を2日間培養し,NS5A 2132−2140刺激PBMC(2´105細胞/ウェル)を加えた。18時間インキュベートした後,100μlの上清を取り出し,IFN−γ濃度を3ウェルのアッセイでELISAにより測定した。安定なトランスフェクタント細胞株を作製するために,pCR3.1−NS5AまたはpCR3.1−HLA−A3101(陰性対照)のいずれかを,Gene Pulser(BioRAD,Richmond,CA)を用いて,C1R−A2402細胞にエレクトロポレーションによりトランスフェクションした。これらの細胞をゲネテシン(G418)の存在下で30−40日間培養し,ゲネテシン耐性細胞を回収した。トランスフェクタント中のNS5Aタンパク質の発現は,抗NS5ポリクローナル抗体(AbcamLimited,Cambridge,UK)を用いるウエスタンブロット分析により行った。
Huh7細胞株を,平底96ウェルマイクロカルチャープレートのウェル中で種々の血清の存在下で24,48,および72時間インキュベートした。インキュベート後,細胞計数キット−8(10μl/ウェル)(Dojindo,Japan)でHuh7細胞を数えた。ADCCアッセイのためには,HLA−A24陽性のHDから新たに単離したPBMCを,種々の患者またはHD(陰性対照として)からの非働化血清(56℃,30分間)を含む10%FCSおよびRPMI−1640培地で1.5時間プレインキュベートした。C1R−A2402細胞およびC1R細胞(陰性対照として)をNS5A 2132−2140ペプチド(10μM)とともに2時間インキュベートし,Na2 51CrO4で1.5時間放射性標識し,洗浄し,標的細胞として用いた。ペプチド刺激PBMCを,96U底マイクロカルチャープレートのウェル中で,40,20,および10対1のエフェクター対標的細胞(E/T)比で,標的細胞とともにインキュベートした。37℃で6時間インキュベートした後,無細胞上清を回収してADCC活性を測定した(Shomura, et al., Br J Cancer 2004; 90: 1563-1571)。
統計学的分析は,スチューデントt検定およびマン・ウィットニー U検定を用いて行った。P<0.05の値を統計学的に有意とした。
ペプチドに対する液性応答をスクリーニングするために,HCV−1bに感染した12名の患者の血漿において,44種のペプチドに対するそれぞれのIgG反応性のレベルを測定した。10名のHDの血漿を陰性対照として用いた。ペプチド反応性IgGのレベルは,連続希釈した血漿サンプルについてELISAにより測定し、各サンプルの吸光度(OD)値を示した。1:100の血漿希釈でのこれらのペプチドのカットオフ値を0.18OD{10名のHDの平均(0.08)+2SD(0.05x2)}とした。NS5A−2132,C−176,E2−488,E1−213,C−173,およびNS3−1081ペプチドに対する有意なレベル(1:100の血清希釈で>0.18OD)のIgG反応性は,12名の患者の血漿中,それぞれ12,11,10,9,8,および5名で検出できた(図9)。予測されたように,これらのペプチドのいずれも,10名のHDからの血漿に対しては反応性を有していなかった。ペプチド特異的IgGに対するペプチドの反応性を表3にまとめる。
HCV1bに感染しているHLA−A24陽性の患者(n=12,6名のCH,3名のLC,3名のHCC)およびHCVに感染していないHLA−A24陽性のHD(n=5)からのPBMCを,6種の各ペプチド(純度>90%)または対照HIVペプチドとともに13日間培養し,対応するペプチドでパルスしたC1R−A2402細胞に応答したIFN産生について調べた。HLA−A24陽性HCV1b+の患者(n=12)およびHD(n=5)からのPBMCを6種のペプチドで,4つのウェル(15x104/ウェル)で刺激した。培養の第14日に,各ウェルからのペプチド刺激PBMC(80−120x104/ウェル)を別々に回収し,等量に4つに分割した。そのうち2つを,対応するペプチドでパルスしたC1R−A2402細胞に応答してIFN−γを産生する能力について別々に試験した。残りの2つは,陰性対照ペプチド(HIV)を用いた細胞で試験した。HIVペプチド(<50pg/ml)に応答して産生されたIFN−γをバックグラウンドとして差し引いた。*は両側スチューデントt検定によりP<0.05を示す。
抗ペプチドIgGの生物学的役割の可能性をさらによく理解するために,吸収および溶出アッセイにより,抗NS5A−2132IgGが全NS5タンパク質を認識するか否かを調べた。NS5A−2132およびHIVペプチドをそれぞれ陽性対照および陰性対照として用いた(吸収試験および溶出試験の詳細は上述を参照)。その結果,抗ペプチドIgGは,全NS5タンパク質では吸収も溶出もされなかった(図20)。このことは,このペプチドIgGは全NS5タンパク質とは反応しなかったことを示唆する。
被験者
血清は,1995年から2002年のコホート研究における,HCV関連疾患を有する33名の患者から得た。33名の患者のフォローアップ調査の結果は表4に示される。慢性肝炎(CH,n=68),肝硬変(LC,n=43),および肝細胞癌(HCC,n=52)を有する患者の血清も分析に用いた。これらの患者は,最初の血清サンプリングの際に,生化学および組織学的所見,超音波検査,およびコンピュータ連動断層撮影により診断した。抗HCV抗体は,化学発光酵素イムノアッセイ(CLEIA)キット(Lumipulse II HCV,Fujirebio Inc.,Tokyo,Japan)により,または第2世代もしくは第3世代酵素イムノアッセイ試験(SRL,Tokyo,Japan)により測定した。血清中のHCV−RNAはRT−PCR(SRL,Tokyo,Japan)を用いて検出した。HCVゲノタイプは,患者の血清中のHCVをRT−PCR(SRL,Tokyo,Japan)で直接シークエンシングすることにより決定した。HCV感染していない被験者からも血清を得た。これは,自己免疫疾患を有する24名の被験者(全身性エリテマトーデスを有する6名の患者,ベーチェット病を有する1名の患者およびアトピー性皮膚炎を有する17名の患者),B型肝炎ウイルス(HBV)感染の17症例(HBV表面抗原にポジティブ),ヒト免疫不全ウイルス(HIV)を有する3名の患者,およびヒトT細胞白血病ウイルスタイプI(HTLV−1)感染を有する10症例を含む。陰性対照として,HBVの肝炎ウイルスまたはワクチン接種の履歴を有さず,正常な肝機能を有する37名からの血清を試験した。
純度90%以上の次の2つのペプチドは,BIOSYNTHESIS(Lewisville,TX)から購入した;HCV1bコアタンパク質35−44(YLLPRRGPRL(配列番号1);HLA−A2拘束性CTL活性を誘導することができる),およびHCV1bNS5Aタンパク質2132−2140(RYAPACKPL(配列番号2);HLA−A24拘束性CTL活性を誘導することができる)。陰性対照としては,HLA−A2結合モチーフ(SLYNTVATL(配列番号64))およびHLA−A24結合モチーフ(RYLRDQQLLGI(配列番号63))を有するHIV由来ペプチドを用いた。
ペプチド特異的IgGのレベルは,酵素抗体法(ELISA)により測定した。簡単には,各ペプチドをジメチルスルホキシド(DMSO)に溶解し,−20℃で保存した。化学架橋剤であるジサクシンイミジルスベレート(DSS)(PIERCE,Rockford,IL)を含む0.1M炭酸バッファで希釈したペプチド(20μg/ウェル)をELISAプレートに結合させた。0.05%Tween20−PBS(PBST)でウェルを3回洗浄した。プレートをBlock Ace(Yukijirushi,Tokyo,Japan)で4℃で一晩ブロッキングした。血清サンプルを0.05%Tween20−BlockAceで100倍,200倍,400倍に希釈し,100μl/ウェルのサンプルを各ウェルに加えた。37℃で2時間インキュベートした後,プレートをPBSTで9回洗浄し,100μl/ウェルの1000倍希釈ウサギ抗ヒトIgG(γ鎖特異的)(DAKO,Glostrup,Denmark)とともに37℃で2時間インキュベートした。9回洗浄した後,100μl/ウェルの1:100希釈抗ウサギIgG結合西洋ワサビパーオキシダーゼ・デキストランポリマー(En Vision,DAKO)を各ウェルに加え,プレートを室温で40分間インキュベートした。洗浄した後,100μl/ウェルのテトラメチルベンチジン基質溶液(KPL,Guildford,UK)を加え,1.0Mのリン酸を加えることにより反応を停止させた。
統計学的分析は,χ2を検定を用いて行った。p<0.05の値を統計学的に有意であるとした。
HCV1b由来ペプチドに対して反応性を有する2つの抗体の交叉反応性を調べた。60名のHCV患者,およびコホート研究に含まれていない患者(CH,n=22,LC,n=21,およびHCC,n=17)の血清を,コアの35−44(C−35)およびNS5Aの2132−2140(NS5A−2132)のペプチドに対する反応性について調べた。自己免疫疾患を有する24名の被験者,HBV感染の17症例およびHTLV−1感染の10症例からも血清を入手した。37名のHDからの血清を陰性対照として用いた。連続希釈した血清サンプルのペプチド反応性IgGのレベルをELISAにより測定した。各サンプルの吸光度(OD)の値を示した。100:1の血清希釈でのODの代表的な結果を示す。カットオフ値は0.093(HDからのODの平均+2SD)に設定した。統計学的分析はχ2検定を用いて実施した。p<0.05の値を統計学的に有意とした。
抗NS5A−2132抗体とHCV陽性患者の予後との相関を個々のレベルでさらに調べるために,1990年から2002年に行われた,HCVに感染している住人を毎年スクリーニングするコホート研究からのサンプルについて2つの抗体の血清レベルを調べた。この研究のためには,1995年および2002年に同じ個体から2回採取した33名の患者からの合計66の血清を用いた。1995年の時点におけるこれらの33名の患者の診断は,CH(n=17),LC(n=1),無症候性キャリア(ASC)(n=4),およびHCV感染の過去の病歴(自然回復した個体)(n=11)であり,2002年の時点では,CH(n=13),LC(n=4),HCC(n=2),無症候性キャリア(ASC)(n=1),およびHCV感染の過去の病歴(n=13)であった。すなわち,7年間の間に,26名の患者では疾患の進行が観察されなかったが,残りの7名の患者では進行が観察された(表4)。1995年および2002年に100:1の血清希釈で測定した各患者における抗C−35抗体および抗NS5A −2132IgGのOD値を図29に示す。予測されたように,33症例の大部分においては,疾患の状態にかかわらず,1995年に測定した抗C−35抗体のレベルと2002年に測定したレベルとはほぼ等しかった。これに対し,1995年に測定した抗NS5A抗体のレベルは,疾患が進行した7名の患者すべてにおいては2002年に測定したときに減少していた。一方,疾患が進行しなかった25症例の大部分において2002年で測定した値とほぼ等しかった。疾患が進行した7名の患者の血清中の1995年の時点における抗NS5A−2132抗体の中央値±SD(0.67±0.13)は,2002年に測定した値(0.27±0.11)より有意に高かった(p<0.05)。
ELISA法より感度が高いと考えられるルミネックス(Luminex)法を用いて、実施例1と同様にして、HLA−A24+HCV感染患者の血清におけるHCVペプチドと反応性を有するIgGの検出を行った。
ペプチドを、製造者の指示に従って各蛍光色素の含有量を識別コード(以下、カラーコード)化した微小ビーズ(Luminex社製、xMAP Multi-Analyte COOH Beads)にそれぞれ次のようにして結合した。フィルタープレートの各ウェルにカラーコード化した未調製の微小ビーズを100μlずつ入れて吸引し、その後洗浄用緩衝液(リン酸緩衝液生理食塩水(PBS)(pH7.4±0.1)、Tween(登録商標)20(0.05%v/v))で2回洗浄し各ウェルを同時に吸引した。次に0.1M MES(2−モルホリノエタンスルホン酸)緩衝液(pH7.0)50μlを入れ、EDC(N-エチル-N'-(3-ジメチルアミノプロピル)-カルボジイミド塩酸塩)(1mg/ml/0.1M MES緩衝液(pH7.0))を各ウェル10μlずつ添加した。数百μlのペプチド(1mg/ml,0.1M MES緩衝液、pH 7.0)をこの洗浄した微小ビーズと各ウェル内で混合した。その後ペプチドと混合した微小ビーズを、暗所で20分間、室温で反応させた後、さらにEDC(1 mg/ml/0.1M MES緩衝液(pH 7.0))を各ウェル10μlずつ添加し、暗所で、20分間室温で反応させる操作を2回繰り返した。余剰液を吸引後、1Mトリス塩酸緩衝液(pH7.0)を各ウェル100μlずつ添加し、暗所で、15分間室温で反応させた。次に、各ウェルのビーズを上記洗浄用緩衝液で3回洗浄し、ブロックエース(登録商標)に0.05%アジ化ナトリウムを入れて調整した保存用溶液で回収した。
各ウェルの微小ビーズを調製するときは、上記のようにカラーコード化した微小ビーズにペプチドを結合させて得たビーズ溶液をフィルタープレートの各ウェル当たり微小ビーズが約5000個になるように入れて(各ウェル当たりビーズ1種類約1μl)調製する。また、このように調製した微小ビーズを10種類等量で混ぜて、上記洗浄用緩衝液(PBS、TWEEN(登録商標)20(0.05%v/v))で総量が約25μlになるように希釈してビーズミックスを調製した。
血清を使用した。血清を反応用緩衝液(PBS(pH 7.4±0.1)、Tween(登録商標)20(0.05%v/v)、牛胎児血清アルブミン(BSA)10 mg/ml)を用いて100〜1000倍希釈した血清希釈液をそれぞれ100μl調製した。
ビオチン化二次抗体としては、ビオチン化ヤギ抗ヒトIgG(ガンマ鎖特異的)を上記反応用緩衝液で希釈して使用した。蛍光色素標識ストレプトアビジンとしては、PE(フィコエリスリン)で標識したストレプトアビジン(SRPE)1 mg/mlを上記反応用緩衝液で 20μlに希釈(1/50希釈)して使用した。
HLA−A2陽性またはHLA−A24陽性のHCV感染者において,本発明のC型肝炎ウイルス由来ペプチドを用いたワクチンの抗ウイルス効果を調べた。被験者はいずれも,HCV1bに感染しており,インターフェロン+リバビリンによる治療に応答しなかった患者である。C型肝炎ウイルス由来ペプチドC−35,NS5A−2132,E2−488,E1−213およびNS3−1081はGMP適合下に合成し,精製し,凍結乾燥粉末として保存した。C−35,NS5A−2132,E2−488およびNS3−1081のぺプチドは,少量のDMSOに溶解(1mg/10〜25μl)し,E1−213は7%炭酸水素ナトリウム注射液(メイロン)に溶解(1mg/15μl)した。これらをそれぞれ注射用生理食塩水で希釈(1〜2mg/ml)し,0.22μmのフィルタを通して滅菌した。この溶液を等量のアジュバント(Montanide ISA−51)と混合して,エマルジョン注射剤を得た。このうちHLA−A2陽性の被験者3名にはC−35注射剤を,HLA−A24陽性の被験者5名にはNS5A−2132注射剤を投与した。投与は,2週間ごとに,1回あたり0.3mgのペプチドを含むエマルジョンを側腹部に注射することにより行った。HCV RNAの量およびGOT,GPT,γ−GTP,AFPの値を継続的にモニターした。
Claims (2)
- HLA-A24拘束性細胞傷害性T細胞に認識される、配列番号3から5のいずれか一つで表されるアミノ酸配列からなるペプチド。
- 請求項1に記載のペプチドの少なくとも1つを有効成分とする、HLA-A24陽性のHCV感染者へ投与するための薬剤。
