【産業上の利用分野】
本発明は多層移植片に関する。

【従来の技術】
手術を行っている間に通常の均質移植片が体の中に移植されている。もしこれらが多孔質材料か或は再吸収性を示す材料を含んでいる場合、組織が成長してそれらの孔の中に入るか或は治癒過程中の移植片の場所に入り込むことで、新しい組織構造体を生じる。空洞またはガイディング(guiding)構造を有する非再吸収性移植片を用いると、この非再吸収性移植片は完全な再生を可能にしないことから、特定点における生理学的な細胞の増加がちょうど良く生じない可能性がある。従って、非晶質で無秩序な新しい組織が生じる傾向を示し、瘢痕化と収縮が生じる。その結果として、組織の強度が不適当になる可能性があり、そしてこれは更に、もし移植片が再吸収性を示さないことからある程度の機械的安定性を保証している材料で出来ている場合、合併症がもたらされ得る。従って、その移植片と体自身の組織との組み合わせが示す自然発生的な生物学的機能は、その元の健康な組織が示すそれに相当しなくなる。

【発明が解決しようとする課題】
本発明の目的は、組織再生の異なる段階における組織の成長値および速度に選択的および差別的影響を与える移植片を提供することにある。

【課題を解決するための手段】
この目的は、請求項1の特徴を示す多層移植片を用いることによって達成される。

すなわち、本発明に従う多層移植片には少なくとも2層備わっており、これらの少なくとも2層は異なる間隙率を有している。従って、この移植片の構造は、この移植片が移植されている体内の組織再生過程に特異的に適合することが可能であり、そして望まれている、組織が成長して中に入り込むこと(growing−in)が調節され得る。例えば、急成長する細胞単位は、その移植片の一方の側から、大きな孔を有する層の中に速く進行して入って行くことができる一方、もう一方の側では、有意な間隙率を有していない層、例えばフィルムによって、細胞の入り込みが完全に防止され得る。本発明に従う移植片に備わっているいくつかの層は、種々の再吸収性を示す材料および再吸収性を示さない材料を含んでいてもよく、これらは今度は、異なる速度で劣化し得る。この移植片に備わっている個々の層の材料、間隙率、厚さおよび寸法、並びにこの層の順序および全体的な形状を適切に選択したならば、この移植片が新しい組織を生じさせる刺激は、阻害影響を示さず、治癒過程に選択的に適合し得るものであり、その結果として、促進された秩序ある組織増殖がもたらされる。

与えられた手術中に移植される本発明に従う移植片の構成および外部形状は、従って、置き代えるべき元の組織構造体の種類および形状を基としており、従って場合場合で異なっている。個々の層が有する材料および構造に関する適切な選択を通して、本発明に従う移植片をその与えられた生物学的条件に適合させることができ、その結果として、その新しく生じてくる組織は再び分化し(高い値)、そして好適な場合として、元の健康な組織と同じ秩序ある構造を示すようになる。

本発明に従う多層移植片の好適な変形において、織ったか或は編んだメッシュ(格子)の一つの側に、多孔質フリース(fleece)層を完全にか或は部分的に与える。このフリース層が示す間隙率は、200Paの入り口圧力、50cm2の試験表面積および1mmの試験厚を用いて測定して100から1000L/(m2秒)気体流量の範囲にある。このような構成を用いる場合、このメッシュは、粗い間隙率を有する層を形成しており、これを貫通して組織が実質的に妨げられないで成長する。他方、この組織の増殖速度は、該フリース層が示す間隙率によって調節され得る。従って、この組織は、小さい間隙率を有する材料よりも大きな間隙率を有する材料の中により迅速に成長して入り込む傾向を示す。このメッシュおよびフリース層は、再吸収性を示す材料か或は再吸収性を示さない材料を含んでいてもよい。例えば、このフリース層が再吸収性を示すがそのメッシュが再吸収性を示さない場合、このメッシュは、ある期間経った後、その体内で単にその移植片の構成要素のままであり、必要に応じて、永久的な追加的物理安定性を保証し得る。

別のデザインの本発明に従う多層移植片を用いることで、このメッシュを完全にか或は部分的に占めている多孔質フリース層を、織ったか或は編んだメッシュの1つの側に位置させる一方、もう一方の側に、10から50μmの厚さを有するフィルムを配置する。上記フィルムにより、その隣接する組織への誤った成長が防止されると共に接着性が低下する、と言うのは、この組織の細胞は、そのフィルムが備わっている移植片のその側からは、この移植片の中に入り込むことができないからである。このフィルムが再吸収性を示す場合、この効果が継続するのは一定期間のみである。しかしながら、これは、特定の環境下では、急速に成長する細胞単位がその移植片の中に入り込むのを防止するに充分であり、その間に、もう一方の側から、ゆっくりと成長する細胞単位がその多孔質フリース層の中に成長して入り込むが、これにそのフィルムが備わっていない場合、それらがそうするのを、その進んで来る急成長する細胞単位が妨げる。

フィルムが備わっている本発明に従う他の多層移植片構造もまた可能である。従って、織ったか或は編んだメッシュの片側または両側にフィルムを完全にか或は部分的に与えることができる。少なくとも1つのフリース層が備わっている移植片を用いる場合、これらのフリース層の1つをフィルムで完全にか或は部分的にコートすることができる。さもなければ、上記移植片は、上に挙げた構造の1つを有していてもよい、即ちメッシュを含んでいてもよい。二者択一的に、単に2つの層、即ちフィルム、即ち小さいか或は有意でない間隙率を有する層とフリース層とが含まれている変形もまた考えられ得る。

簡潔さの目的で、これらの図に示す多層移植片は、長方形の基本的形状を有していると共に、それらのフリース層は一定した厚さを有している。しかしながら、与えられた用途では、この移植片の基本的構成、個々の層の材料および間隙率、並びに層の厚さおよび移植片の全体的形状などを、その条件に適合させる。

本発明に従う多層移植片が有する個々の層を互いに結合させる目的で、例えば、異なる融点を有する材料を隣接層として用いることが可能であり、この移植片の製造中に、これを、より低い融点を有する材料の融点よりも高い温度であるが融点が高い方の材料の融点よりも低い温度に加熱すると、これらの層の1つが溶融してその隣接層と結合する。再吸収性を示す材料、好適にはポリジオキサノンおよびポリグラクチンで作成した移植片の成分は、ドイツ特許第38 30 005 C1号に記述されている。

異なる融点を有する2つの層の間に、より低い融点を有する材料から作成したフィルムを位置させて加熱する結果として、このフィルムを溶融させて多孔性を示すようにさせると共に上記2つの層を互いに結合させることも可能である。このようにして、例えば、再吸収性を示さない材料から作成したメッシュと、より低い融点を有する再吸収性を示す材料から作成した多孔質フリース層とを互いに連結させることができる。

1. 少なくとも2つの層が備わっており、これらの少なくとも2つが異なる間隙率を有することを特徴とする多層移植片。

7. 10から50μmの範囲の厚さを有するフィルムを、織ったか或は編んだメッシュの片側または両側に完全にか或は部分的に与えることを特徴とする第1項記載の多層移植片。

8. フリース層を少なくとも1層含んでおりそして該フリース層の少なくとも1層がフィルムで完全にか或は部分的にコートされていることを特徴とする第1から5項の1項記載多層移植片。

9. 該層の少なくとも1層が再吸収性を示すことを特徴とする第1から8項の1項記載多層移植片。

10. 該層の少なくとも1層が再吸収性を示さないことを特徴とする第1から8項の1項記載多層移植片。

11. 再吸収性を示す材料としてポリグラクチン910、ポリジオキサノン、ポリグレカプロン25またはそれらの組み合わせを用いることを特徴とする第9項記載の多層移植片。

12. 再吸収性を示さない材料としてポリプロピレンまたはポリエステルを用いることを特徴とする第10項記載の多層移植片。

Claims (2)
- 上面および底面を有する織ったメッシュと、該メッシュの上面に固定されている多孔質フリース層であって、200Paの入り口圧力、50cm 2 の試験表面積および1mmの試験厚で測定して100から1000L/(m 2 秒)気体流量の範囲にある間隙率を有する多孔質フリース層とからなり、かつ、該メッシュおよび多孔質フリース層がポリグラクチン910、ポリジオキサノン、ポリグレカプロン25およびそれらの組み合わせ物からなる群より選ばれる再吸収性ポリマーから作製されていることを特徴とする外科的移植片。
- 多孔質フリース層が1000L/m 2 秒の間隙率有する第一の領域および200L/m 2 秒の間隙率有する第二の隣接周辺領域を有する請求項1に記載の外科的移植片。
