Description

【発明の詳細な説明】

【産業上の利用分野】本発明は、血管分岐の範囲で身体
血管へ埋込むための放射状に拡張可能な中空円筒素子の
形のステントに関する。
【0002】
【従来の技術】身体血管又は身体の腔における狭くなつ
た個所には、現在この狭くなつた個所を拡張しかつ血管
壁を安定化するため、放射状に拡張可能なステントが使
用される。身体血管のこのような狭くなつた個所は、血
管分岐の範囲にも現われることがある。この場合従来の
ステントの使用は不可能である。なぜならば、このステ
ントの壁が分岐する血管への血液の自由な流通を妨げる
ことになるからである。従つてドイツ連邦共和国特許出
願第29701758号には特別なステントが提案さ
れ、このステントは大きくされた開口のある区域を持つ
ているので、この区域は側方分枝血管の分岐個所にわた
つて位置することができ、血液の流通をもはや妨げない
か又は僅かしか妨げない。しかし分岐範囲の所に主血管
の不利な狭さく症が形成されると、このステントは、全
区域にわたつて延びる大きい放射状開口のため、病気の
ある血管区域を充分に覆うことができない。

【発明が解決しようとする課題】従つて本発明の基礎に
なつている課題は、血管分岐の範囲に使用可能で上記の
欠点を回避するステントを提供することである。

【課題を解決するための手段】最初にあげた種類のステ
ントにおいて、この課題を解決するため本発明によれ
ば、ステントが、大きくされた放射状開口を持ち、血管
への埋込みのためバルーンカテーテルに予め取付けられ
ており、バルーンカテーテルが案内線を通すための空所
を持ち、この案内線が大きくされた開口の中心で空所及
びステントから出ている。
【0005】直接分岐個所にわたつて位置する側壁にあ
る1つだけの大きくされた開口により、血管壁全体がス
テントにより支持されるようにすることができる。この
場合大きくされた開口は、側方分枝血管への血液の妨げ
られない流れを保証するという役割を持つている。しか
しこのステントの正確な位置ぎめのために、無条件に適
当な補助手段が必要である。X線造影剤の使用に基くだ
けでは、分岐上における大きくされた開口の正確な位置
ぎめは不可能である。従つて本発明によるステントは、
大きくされた開口を通つて出る案内線用の空所を持つバ
ルーンカテーテル上にステントが予め取付けられてい
る。この案内線により、ステントの大きくされた開口を
側方分枝血管のちようど分岐点の上に位置させることが
できる。このためカテーテルは、第2の案内線を側方分
枝血管へ導入することが可能になるまで、X線造影剤を
添加しかつX線スクリーンを観察しながら、回転及び移
動により操作される。主血管へバルーンカテーテルを導
入するため、公知のようにバルーンカテーテルは、バル
ーンカテーテルの先端から出る第1の案内線のために、
縦軸に沿つて延びる空所を持つことができる。大きくさ
れた開口の正確な位置ぎめが第2の案内線により行われ
る前に、第1の案内線に沿つてステントを問題なく血管
分岐の所まで送ることができる。カテーテルの正確な位
置ぎめが保証された後、バルーンが膨らまされ、ステン
トが血管壁に接する。続いてカテーテルを両方の案内線
に沿つて血管から再び引出すことができる。ステントは
両方の案内線と共に血管内に残る。側方分枝血管の案内
線を介して別のバルーンカテーテルを分岐部へ導入し
て、必要な場合大きくされた開口を再び大きくすること
ができる。それにより、ステントの壁部分が側方分枝血
管への血液流れを妨げないようにすることができる。
【0006】バルーンカテーテルを構成するために種々
の可能性がある。例えば大きくされた開口から導出され
る案内線用の空所を、バルーン表面上への毛細管の取付
けによつて形成することができる。しかしこの空所は、
二重壁バルーンの間隙によつても形成することができ
る。バルーンの上まで膨張可能なホース片をかぶせるこ
とによつても、このかぶせられたホース片とバルーンと
の間に案内線用の空所を形成することができる。別の構
成では、バルーンカテーテルが同軸的に設けられる3つ
のバルーンカテーテルを持ち、内側にある2つのホース
が2つの案内線を収容する空所を形成することができ
る。
【0007】ステント自体も種々のやり方で構成するこ
とができる。ステントは多細胞壁を持ち、管から作られ
るようにすることができる。しかしステントを線から作
ることも可能である。その際ステントは線から曲げられ
るか、編まれるか又は織られることができる。大きくさ
れた開口を目的にかなうようにステントの中心に設ける
ことができるが、特定の使用事例のために大きくされた
開口の偏心配置も実現可能である。

【実施例】本発明によるステントの好ましい実施例を図
面により以下に説明する。
【0009】図1は、血管分岐個所に現われることがあ
る典型的な狭さく症の3つの例を示している。図1の
(a)では、狭さく症12が側方分枝血管11の分岐個
所の前の主血管10にある。(b)では、非常に大きい
狭さく症12′が側方分枝血管11の分岐個所に直接対
向しており、(c)では、狭さく症12″が主血管10
と側方分枝血管11との間の移行個所にある。特に
(b)による狭さく症12′は、大きくされた放射状開
口を持つ分岐ステントによつては不十分にしか覆われな
い。まさにこのような場合に、図2及び3に示すような
本発明によるステントが適している。

射状開口21を持つステント20の表面構造を示してい
る。ステント20の中心範囲には、ただ1つの大きい菱
形の開口22が形成されて、身体血管内で側方分枝血管
11(図1)の分岐個所の上に正確に位置せしめられる
ことができる。ステント20の正確な位置ぎめを可能に
するために、このステント20は図3に示すようにバル
ーンカテーテル30上に予め取付けられている。図3に
示すバルーンカテーテル30は、その内部に、第1の案
内線31を通すため詳細には図示してない空所を持つて
いる。カテーテル30のバルーン32の範囲でステント
20がかぶせられている。更に第2の案内線34を通す
ための別の空所33がバルーン32の範囲まで延び、大
きくされた開口22の範囲で第2の案内線34が空所3
3及びステント20から出る。この第2の案内線34は
側方分枝血管11へ導入され、従つて主血管10内にお
けるステント20の位置ぎめ用調節補助手段として役立
つ。更に第2の案内線34は、主血管10からバルーン
カテーテル30を引出した後、大きくされた開口22を
拡張するための別のバルーンカテーテル用の案内線とし
て使用することができる。
【0011】バルーンカテーテル30及びステント20
の図示した実施例は模範的であるにすぎない。第2の案
内線34を通すために必要な空所33は、例えば二重壁
バルーン32又は2つの同軸的なホース片によつても形
成することができる。図示したステント20は毛細管か
ら切断されているが、線から曲げられるか、編まれる
か、織られることができる。

Claims (12)

    (57)【特許請求の範囲】
  1. 【請求項1】 中空円筒状素子の形の放射状に拡張可能
    なステントが、大きくされた放射状開口(22)を持
    ち、血管(10)への埋込みのためバルーンカテーテル
    (30)に予め取付けられており、バルーンカテーテル
    (30)が案内線(34)を通すための空所(33)を
    持ち、この案内線(34)が大きくされた開口(22)
    の中心で空所(33)及びステント(20)から出てい
    ることを特徴とする、血管分岐の範囲で身体血管へ埋込
    むための放射状に拡張可能なステント。
  2. 【請求項2】 バルーンカテーテル(30)が、バルー
    ンカテーテル(30)の先端から出る案内線(31)の
    ために、縦軸に沿つて延びる空所を持つていることを特
    徴とする、請求項1に記載のステント。
  3. 【請求項3】 ステント(20)の拡張後バルーンカテ
    ーテル(30)が、両方の案内線(31,34)に沿つ
    て血管(10)から引出し可能であることを特徴とす
    る、請求項1又は2に記載のステント。
  4. 【請求項4】 大きくされた開口(22)から導出され
    る案内線(34)用の空所(33)が、バルーン(3
    2)の表面への毛細管の取付けにより形成されることを
    特徴とする、請求項1ないし3の1つに記載のステン
    ト。
  5. 【請求項5】 大きくされた開口(22)から導出され
    る案内線(34)用の空所(33)が、二重壁バルーン
    (32)の間隙により形成されることを特徴とする、請
    求項1ないし3の1つに記載のステント。
  6. 【請求項6】 大きくされた開口(22)から導出され
    る案内線(34)用の空所(33)が、バルーン(3
    2)上に張られる膨張可能なホース片とバルーン(3
    2)との間の間隙により形成されることを特徴とする、
    請求項1ないし3の1つに記載のステント。
  7. 【請求項7】 バルーンカテーテル(30)が同軸的に
    設けられる3つのホースから成り、内側にある2つのホ
    ースが両方の案内線(31,34)を収容する空所を形
    成することを特徴とする、請求項1ないし3の1つに記
    載のステント。
  8. 【請求項8】 ステントが管から作られ、多細胞壁ホー
    スを持つていることを特徴とする、請求項1ないし7の
    1つに記載のステント。
  9. 【請求項9】 ステントが線から曲げられていることを
    特徴とする、請求項1ないし7の1つに記載のステン
    ト。
  10. 【請求項10】 ステントが線から編まれるか又は織ら
    れていることを特徴とする、請求項1ないし7の1つに
    記載のステント。
  11. 【請求項11】 大きくされた開口(22)がステント
    (20)の中心に設けられていることを特徴とする、請
    求項1ないし10の1つに記載のステント。
  12. 【請求項12】 大きくされた開口(22)がステント
    (20)に偏心して設けられていることを特徴とする、
