、多くの患者に部分的な軽減をもたらすが、OAを有する多くの個人が高齢であるので、
現在の状況で特に懸念される上部消化管出血及び腎不全に関連する。グルココルチコイド
の関節内注射は多くの場合速やかな緩和をもたらすが、その効果は通常数週間しか持続し
ない。グルココルチコイドの反復注射は、感染についての懸念及び持続的な高用量のグル
ココルチコイドが関節軟骨を損傷するという証拠のために、非現実的であり、禁忌である

及び重症度と相関する調節エレメントを同定した。

に除去される。これは、関節内で抗OA薬の持続的な治療用量を達成することを困難にす
る。これに対処するために、小分子を全身的に送達することはできるが、滑膜毛細血管の
有窓内皮を横切る際のサイズ依存性制約のために、タンパク質をこの様式で送達すること
は困難である。さらに、全身送達は、非標的部位を高用量の治療薬にさらし、望ましくな
い副作用をもたらす。典型的には数時間の半減期を有する、関節からのタンパク質の急速

成的プロモータ)に作動可能に連結された導入遺伝子(例えば標的とした部位のタンパク
質、例えば治療用遺伝子産物、例えばIL-1Ra又はその改変型をコードするヌクレオ
チド配列、)を含み得る。

例1は、本発明のsc-rAAV(IL-1Raをコードする)の投与を記載する。本
発明は例1の開示に限定されない。5人の患者が、本発明のsc-rAAVの投与を検討
する臨床試験に参加する。患者は以下の通りである:(1)右膝に変形性関節症を有する
65歳の男性;(2)左膝に変形性関節症を有する59歳の男性;(3)左膝に変形性関
節症を有する58歳の女性;(4)右膝に変形性関節症を有する51歳の男性;及び(5
)右膝に変形性関節症を有する48歳の男性。それぞれの患者に、各膝あたり1×101
2個のウイルス遺伝子の関節内注射によってsc-rAAVを投与する。IL-1Raは
軟骨細胞及び滑膜細胞において発現される。患者1は、2週間以内にOAに関連する症状
の改善を述べている。患者2は、1週間以内にOAに関連する症状の改善を述べている。
患者3は、5週間以内にOAに関連する症状の改善を述べている。患者4は、1週間以内
にOAに関連する症状の改善を述べている。6週の時点で、患者5はOAに関連する症状
の改善を述べていない。
例2は、本発明のsc-rAAV(IL-1Raをコードする)の1回目の投与及び一
定期間後の本発明の同じsc-rAAVの2回目の投与を記載する。本発明は例2の開示
に限定されない。55歳の男性は右膝に変形性関節症を呈する。前記男性の医師は、本発
明のsc-rAAVベクター(IL-1Raをコードする)の単回関節内注射を行う。患
者の症状は2ヶ月以内に解消される。6ヶ月後、医師は、本発明の同じsc-rAAVベ
クター(IL-1Raをコードする)の2回目の(単回)関節内注射を行う。患者の症状
は、2回目の注射から6ヶ月後もまだ存在しない。
例3は、本発明のsc-rAAV(IL-1Raをコードする)の1回目の投与及び一
定期間後の1回目のsc-rAAVとは異なる本発明のsc-rAAV(IL-1Raを
コードする)の2回目の投与を記載する。本発明は例3の開示に限定されない。49歳の
女性は右足首に変形性関節症を呈する。前記女性の医師は、本発明のsc-rAAVベク
ター(IL-1Raをコードする)の単回関節内注射を行う。患者の症状は5ヶ月以内に
改善したが、解消されてはいない。6ヶ月後、医師は、本発明の異なるsc-rAAVベ
クター(IL-1Raをコードする)の2回目の(単回)関節内注射を行う。2回目の注
射から6ヶ月後に、患者の症状は解消される。
例4は、本発明のsc-rAAV(IL-1Raをコードする)と二次療法との同時投
与を記載する。本発明は例4の開示に限定されない。68歳の男性は、左膝に変形性関節
症を呈する。前記男性の医師は、本発明のsc-rAAVベクター(IL-1Raをコー
ドする)及び多血小板血漿の両方の単回関節内注射を行う。患者の症状は2ヶ月以内に解
消される。
