インターロイキン−6(IL−6)は、移植片対宿主病(GvHD)、関節リウマチ、及び全身性エリテマトーデスを含む複数の自己免疫疾患、炎症性疾患、及びリンパ増殖性障害の病因に関与する炎症誘発性サイトカインである。IL−6はまた、キメラ抗原受容体(CAR)でリダイレクトされるT細胞の注入の最も一般的な副作用の1つであるサイトカイン放出症候群(CRS)の発症にも関与している。CRSは、重症であり、場合によっては致命的であり得る7-10。IL−6は、CRSの発症に重要であり、抗IL−6受容体抗体(トシリズマブ)は、現在その効果を抑制するために使用されている7-10。
本明細書で使用される「核酸」という用語は、複数のヌクレオチドモノマー(例えば、リボヌクレオチドモノマー又はデオキシリボヌクレオチドモノマー)を含むポリマーを指す。「核酸」には、例えば、DNA(例えば、ゲノムDNA及びcDNA)、RNA、及びDNA−RNAハイブリッド分子が含まれる。核酸分子は、天然、組換え、又は合成であり得る。加えて、核酸分子は、一本鎖、二本鎖、又は三本鎖であり得る。特定の実施形態では、核酸分子を修飾することができる。二本鎖ポリマーの場合、「核酸」は、分子の一方又は両方の鎖を指し得る。

一例が図1Aのmb−aIL6構築物である膜結合抗サイトカイン構築物を本明細書に記載されるように作製することができる。抗サイトカイン構築物は、IL−6、(TNF)−α、IL−1β、IL−12、IL−17、IL−18、及びIFNγなどの様々なサイトカインに特異的であり得る。
本明細書には、トランスジェニックT細胞などのトランスジェニック宿主細胞を作製する方法が記載されている。トランスジェニック宿主細胞は、例えば、本明細書に記載のベクターの実施形態の1つ又は複数を宿主細胞に導入することによって作製することができる。

一実施形態は、膜結合抗IL6(mb−aIL6)単鎖可変断片(scFv)をコードする核酸を含むベクターである。mb−aIL6 scFvは、a)抗IL−6可変軽鎖ドメイン、抗IL−6可変重鎖ドメイン、及び可変軽鎖ドメインと可変重鎖ドメインを連結するリンカードメインを含む抗IL6単鎖可変断片(抗IL6 scFv);並びにb)抗IL6 scFvに結合されたヒンジ及び膜貫通ドメインを含む。

別の実施形態は、抗IL6キメラ抗原受容体(CAR)をコードする核酸を含むベクターである。抗IL6 CARは:a)抗IL−6可変軽鎖ドメイン、抗IL−6可変重鎖ドメイン、及び可変軽鎖ドメインと可変重鎖ドメインを連結する第1のリンカードメインを含む抗IL6単鎖可変断片(抗IL6 scFv);b)抗IL6 scFvのN末端にある第2のリンカードメイン;c)第2のリンカードメインのN末端にあるヒンジ及び膜貫通ドメイン;並びにd)ヒンジ及び膜貫通ドメインのN末端にある細胞内シグナル伝達ドメインを含む。

別の実施形態は、哺乳動物におけるIL−6依存性細胞の増殖を抑制する方法である。この方法は、膜結合抗IL6(mb−aIL6)単鎖可変断片(scFv)をT細胞で発現させることを含む。mb−aIL6 scFvは、本明細書に記載の任意の実施形態に従うことができる。

細胞
ヒト細胞株Nalm−6(B細胞急性リンパ芽球性白血病、ALL)、Jurkat(T細胞ALL)、THP−1及びU937(急性単球性白血病)、DS−1(B細胞リンパ腫)、及びHEK293T(胚性腎線維芽細胞)をAmerican Type Culture Collection(Manassas,VA)から購入した。B−ALL細胞株OP−1を本発明者らの研究室で開発した11。Nalm−6、OP−1、THP−1、U937、及びJurkat細胞は、10%ウシ胎児血清(FBS)(HyClone GE Healthcare,Logan,UT)及び1%ペニシリン/ストレプトアビジン(P/S)(PAN−Biotech,Aidenbach,Germany)を添加したRPMI−1640(ThermoFisher Scientific,Waltham,MA)で維持した。DS−1細胞は、10%FBS、1%P/S、及び1ng/mLのインターロイキン−6(IL−6)(ThermoFisher Scientific)を添加したRPMI−1640で維持した。HEK−293Tは、10%FBS及び1%P/Sを添加したDMEM(HyClone Laboratories)で維持した。
mb−aIL6における抗IL6 scFvの重鎖及び軽鎖ドメインは、ヒト抗IL6モノクローナル抗体AME−19aの公表された配列に由来し、15個のアミノ酸[(G4S)3]リンカーに結合して単鎖可変断片(scFv)を形成し;構築物をGenscript (Nanjing,China)によって合成した。scFvを、CD8αヒンジ及び膜貫通ドメイン(「mb−aIL6」)に連結した。抗CD19−41BB−CD3ζ構築物は本発明者らの研究室で以前に開発した12。mb−aIL6と抗CD19−41BB−CD3ζを連結するために使用したP2A配列は以前に報告された13。すべての構築物を、EcoRIとXhoIの間のpMSCV−IRES−GFPにサブクローニングした。
mb−aIL6及び抗CD19−41BB−CD3ζの表面発現をフローサイトメトリーによって検出した。mb−aIL6の場合、ビオチン標識ヤギ抗ヒトF(ab)’2(Jackson ImmunoResearch,West Grove,PA)を使用し、続いてアロフィコシアニン(APC)標識ストレプトアビジン(BD Biosciences,San Jose,CA)で二次染色した。細胞はまた、ビオチンに結合したヒトIL−6(Abcam,Cambridge,UK)で標識し、続いてストレプトアビジン−APCで標識した;ビオチンに結合した大豆トリプシン阻害剤(R&D,Minneapolis,MN)を陰性対照として使用した。myc−tagに連結されたヒトCD19の細胞外ドメインを含む、本発明者らの研究室で生成された可溶性融合タンパク質であるCD19−mycを使用して、抗CD19−41BB−CD3ζを特異的に検出した。T細胞をCD19−mycと共に30分間インキュベートし、続いてR−フィコエリトリン(PE)標識抗myc(Cell Signaling Technology,Danvers,MA)と共にインキュベートした。細胞の免疫表現型検査では、T細胞を、抗CD3 APC、抗CD56 PE、抗CD4 V450、及び抗CD8 PerCP(BD Biosciences)で標識した。細胞染色を、Accuri C6又はFortessaフローサイトメーター(BD Biosciences)を用いて分析した。
IL−6枯渇を測定するために、2×106個のT細胞を、1ngのIL−6を含む1mLのRPMI−1640で2時間培養した。別の実験では、0.5〜2×106個のT細胞を、10IUのIL−6を含む1mLのRPMI−1640で2時間培養した。さらに別の実験では、2×106個のT細胞を、1ngのIL−6を含む1mLのRPMI−1640で、20分〜2時間の異なる時間間隔で培養した。さらなる実験では、0.5×106個の細胞を、25〜200pg/mLの組換えヒトIL−6を含む1mLのRPMIで、37℃で2時間培養した。別のさらなる実験では、0.2×106個のT細胞を、1ngのIL−6を含む1mLのRPMI−1640で2〜72時間培養した。培養の最後に、上清を回収し、0.22μmフィルターで濾過し、1:10に希釈した。IL−6のレベルを、ヒトIL−6 Platinum ELISAキット(ThermoFisher)を用いるELISAによって測定した。計算された標準曲線からの補間を使用して、各試料のIL−6の濃度を決定した。
IFNγの産生について試験するために、1×105個のT細胞を、1:1の比率でOP−1細胞と共に培養した。1時間後、GolgiPlug(BD Biosciences)を細胞に添加し、これをさらに5時間培養した。本発明者らの研究室で開発された透過化試薬である8Eによる細胞膜透過化後、細胞をPE標識抗ヒトIFNγ抗体(BD Biosciences)で標識し、フローサイトメトリーによって分析した。
ルシフェラーゼを発現するDS−1細胞を、NOD.Cg−Prkdcscid IL2rgtm1Wjl/SzJ (NOD/scid IL2RGnull)マウス(Jackson Laboratory)に腹腔内注射した(i.p.;1×106個の細胞/マウス)。DS−1接種の2日後、マウスに、GFPのみで形質導入した1×107個のT細胞、MSCV−mb−aIL6で形質導入した1×107個のT細胞、又はT細胞の代わりに10%FBSを含むRPMI 1640のいずれかを腹腔内注射した。すべてのマウスに、20000IUのIL−2及び1000IUのIL−6を2日ごとに腹腔内投与した。腫瘍細胞量を、水性d−ルシフェリンカリウム塩(Perkin Elmer,Waltham,MA)を(マウス当たり2mg)腹腔内注射した後、Xenogen IVIS−200システム(Caliper Life Sciences,Waltham,MA)を用いて週2回測定した。発光を、Living Image 3.0ソフトウェアを用いて測定した。
mb−aIL6の設計、発現、及び特異性
膜結合抗IL−6構築物を作製するために、単鎖可変断片(scFv)を、ヒト抗IL−6抗体AME−19aの可変軽鎖及び重鎖の配列から合成し、これをCD8αのヒンジ及び膜貫通ドメインに連結した(図1A)。この構築物を、IRES及びGFPを含むMSCVレトロウイルスベクターに導入した(図1B)。このレトロウイルスベクターを使用して、JurkatT細胞を形質導入した。MSCV−mb−aIL6で形質導入された細胞ではGFP発現が高かった:細胞の98%がGFP陽性であった。形質導入されたJurkat細胞の表面におけるmb−aIL6を検出するために、細胞を、ビオチン標識ヤギ抗ヒトF(ab)’抗体で標識し、続いてストレプトアビジン−APCで標識した。図1Cに示されているように、mb−aIL6は、本質的にすべてのGFP発現Jurkat細胞で検出されたが、GFPのみを含むベクターで形質導入された細胞(「Mock」)はmb−aIL6陰性であった。
mb−aIL6のIL−6への結合は、Jurkat細胞が組換えヒトIL−6(1ng/mL)を含む培地で2時間培養される実験により裏付けられ;培養物をELISAによって上清で測定された後に残留IL−6を回収した。2時間の培養後、mb−aIL6 Jurkat細胞の上清中のIL−6の濃度が0.163ng/mLであったのに対し、mock−形質導入Jurkat細胞の上清では0.941ng/mLであった(図2A)。
U937は、IL−6によって刺激することができる単球細胞株である15、16。IL−6受容体に結合すると、IL−6がSTAT3リン酸化を引き起こす17。mb−aIL6−又はmock−形質導入Jurkat細胞に2時間曝露した後のIL−6を含む上清の、U937においてStat3リン酸化を引き起こす能力を試験した。1ng/mLのIL−6を含む上清への暴露の15分後に、Stat3リン酸化が容易に検出された(P<0.001;n=3;図3A)。IL−6上清をmock−形質導入Jurkat細胞を含む培養物から収集した場合、同様のレベルのリン酸化が観察された(P=非有意)。対照的に、培養物がmb−aIL6 Jurkat細胞を含んでいた場合、Stat3リン酸化のレベルは、IL−6又はmock−形質導入Jurkat細胞のIL−6を含む上清に曝露されたU937細胞で測定されたレベルよりも著しく低く(いずれの比較でも、P<0.001)、刺激されていない細胞のレベルと同様であった(P=非有意)。
次の一連の実験で、mb−aIL−6が末梢血Tリンパ球の表面で発現され得るか否かを決定した。この目的のために、末梢血単核細胞を、抗CD3/CD28ビーズで刺激し、次いでMSCV−mb−aIL6レトロウイルスベクターで形質導入した。GFPのみを含むベクターで形質導入された細胞(「Mock」)を対照として使用した。図4A及び図4Bに示されているように、mb−aIL6は、形質導入されたGFP+Tリンパ球の表面で高度に発現し、IL−6に効果的に結合した。mb−IL6を発現するT細胞の免疫表現型は、mock−形質導入T細胞の免疫表現型と本質的に同一のままであった(図4C)。
CAR T細胞がCARに加えてmb−aIL6を発現するなら、これは、活性化Tリンパ球及び微小環境におけるマクロファージによって分泌されるIL−6を中和することにより、CRSの発生を防止することになる。
mb−aIL6の発現及びIL−6の中和がCARによって活性化されたT細胞機能に影響を与えるか否かを決定した。mb−aIL6又は抗CD19 CARのいずれかを発現する3つのドナーからのTリンパ球と両方の受容体を発現するT細胞を使用して、CD19+標的細胞OP−1との共培養後のIFNγの産生を試験した。CARを発現する細胞におけるIFNγの産生は、mb−aIL6が発現したか否かにかかわらず高かったが、mb−aIL6のみを発現するT細胞は、mock−形質導入細胞と同様のレベルのIFNγを分泌した(図6A)。
CAR活性化によって引き起こされるCRSの際、マクロファージによって分泌されるIL−6はその重症度に寄与する7-10。
mb−aIL6発現Tリンパ球がin vivoでIL−6を中和する能力を決定するために、ルシフェラーゼ標識DS−1細胞を移植したNOD/scid IL2RGnullマウスを用いた実験を行った。腫瘍の成長をライブイメージングによって測定し、GFPのみで形質導入されたT細胞が投与されたマウスと、mb−aIL6で形質導入されたT細胞が投与されたマウスを比較した。mb−aIL6形質導入T細胞が投与された3匹のマウスのうち2匹では、腫瘍組織量が減少したが、mock−形質導入T細胞が投与されたマウスの腫瘍組織量は、安定したままであったか、又は増加した(図9A及び図9B)。
この研究の結果は、Tリンパ球によって運ばれるmb−aIL6がIL−6の強力な中和剤であることを示している。重要なことに、mb−aIL6は、CARの効力に影響を与えることなく、CARと組み合わせてTリンパ球で発現させることができる。このため、mb−aIL6を発現するCAR−T細胞は、抗腫瘍能力を維持しながら、重度のCRSを引き起こすリスクが小さいはずである。

- 前記細胞内シグナル伝達ドメインのN末端にある共刺激ドメインをさらに含む、請求項11に記載のベクター。
- 前記共刺激ドメインがCD3ζドメインである、請求項13に記載のベクター。
- 哺乳動物のIL−6濃度を低下させる方法であって:
膜結合抗IL6(mb−aIL6)単鎖可変断片(scFv)をT細胞で発現させること、及び前記T細胞をIL−6を含む液体に接触させることを含み、前記mb−aIL6が:
a)抗IL−6可変軽鎖ドメイン、抗IL−6可変重鎖ドメイン、及び前記可変軽鎖ドメインと前記可変重鎖ドメインを連結するリンカードメインを含む抗IL6単鎖可変断片(抗IL6 scFv);並びに
b)前記抗IL6 scFvに結合されたヒンジ及び膜貫通ドメインを含む、方法。 - 前記哺乳動物がヒトである、請求項15に記載の方法。
- 前記T細胞を培養して、前記mb−aIL6を発現する新たなT細胞を作製することをさらに含む、請求項15又は16に記載の方法。
- サイトカイン放出症候群(CRS)のリスクを低減することをさらに含み、前記哺乳動物のT細胞がキメラ抗原受容体を発現する、請求項15又は16に記載の方法。
- 前記哺乳動物が、自己免疫疾患、炎症性疾患、又はリンパ増殖性障害に罹患しており、前記哺乳動物のIL−6濃度を低下させることにより前記自己免疫疾患、前記炎症性疾患、又は前記リンパ増殖性障害それぞれを処置する、請求項15又は16に記載の方法。
- 前記哺乳動物が、関節リウマチ、全身性エリテマトーデス、移植片対宿主病、又はキャッスルマン病に罹患しており、IL−6濃度を低下させることにより関節リウマチ、全身性エリテマトーデス、移植片対宿主病、又はキャッスルマン病それぞれを処置する、請求項19に記載の方法。
