Abstract
Description
Claims (41)
- 人工循環器弁の支持構造であって、
拡張された形状記憶合金から形成された外側フレームと、
初期形状から拡張された第1部分と、初期形状から圧縮された第2部分と、を有する形状記憶合金から形成された内側フレームであって、前記内側フレームは、人工弁小葉アセンブリを前記第1部分内に支持するように構成され、前記内側フレームは、前記第2部分内で繋ぎ材に係合するように構成される、内側フレームと、を備え、
前記内側フレームが前記外側フレームに結合される、支持構造。 - 前記外側フレームに形成されたX線不透過性インジケータをさらに備える、請求項1に記載の支持構造。
- 前記内側フレーム内に支持された弁小葉アセンブリをさらに備える、請求項1に記載の支持構造。
- 前記内側フレームの前記第2部分内に受け入れられた繋ぎ材をさらに備える、請求項1に記載の支持構造。
- 前記内側フレームが機械的結合具によって前記外側フレームに接続される、請求項1に記載の支持構造。
- 予備形成された圧縮性経カテーテル人工循環器弁の自己拡張式ワイヤフレームであって、内腔を規定する円筒状フレーム構造を含み、前記円筒状フレーム構造が3つの全体的にダイヤモンド状の部材を含み、各ダイヤモンド状部材が2つの横方向頂点と2つの長手方向頂点とを規定し、各ダイヤモンド状部材が、他の2つのダイヤモンド状部材のそれぞれに直接接続されるか、他の2つのダイヤモンド状部材のそれぞれと接続する少なくとも1つの接続部材を有し、前記接続が前記ダイヤモンド状部材の前記横方向頂点のそれぞれに又はその周囲にある、自己拡張式ワイヤフレーム。
- 前記自己拡張式ワイヤフレームが、自己拡張式圧縮性ニッケル−チタニウム生体適合性合金から作製される、請求項6に記載の自己拡張式ワイヤフレーム。
- 少なくとも1つの内側横架部材をさらに備え、前記内側横架部材が、前記ダイヤモンド状部材の少なくとも1つの中の場所に結合する、請求項1に記載の自己拡張式ワイヤフレーム。
- 前記ダイヤモンド状部材の少なくとも1つがひし形である、請求項6に記載の自己拡張式ワイヤフレーム。
- 前記少なくとも1つの接続部材がほぼV字形の接続部材である、請求項6に記載の自己拡張式ワイヤフレーム。
- 前記少なくとも1つの接続部材がほぼV字形の接続部材であり、前記ほぼV字形の接続部材が、開放端部と結合端部とを規定する2つの結合された脚部を有し、前記結合された脚部の各開放端部が、各横方向頂点の周囲で前記ダイヤモンド状部材の1つに接続される、請求項6に記載の自己拡張式ワイヤフレーム。
- 前記ダイヤモンド状部材の前記2つの長手方向頂点の一方が前記ダイヤモンド状部材の上側頂点であり、他方が前記ダイヤモンド状部材の下側頂点であり、前記少なくとも1つの接続部材がほぼV字形の接続部材であり、前記ほぼV字形の接続部材が、開放端部と結合端部とを規定する2つの結合された脚部を有し、前記結合された脚部の各開放端部が、各横方向頂点の周囲で前記ダイヤモンド状部材の1つに接続され、前記ほぼV字形の接続部材の前記結合端部が、前記ダイヤモンド状部材の前記下側頂点の垂直な二等分線とほぼ平行な長手方向軸に沿って向く、請求項6に記載の自己拡張式ワイヤフレーム。
- 少なくとも1つの内側横架部材をさらに含み、各ダイヤモンド状部材が、前記2つの長手方向頂点及び前記2つの横方向頂点で結合された4つの交差しないロッドから構成され、前記内側横架部材が前記ダイヤモンド状部材のそれぞれの中で2つの非隣接ロッドを接続する、請求項6に記載の自己拡張式ワイヤフレーム。
- 前記自己拡張式ワイヤフレームに固定される小葉アセンブリをさらに備え、前記小葉アセンブリが安定化組織又は合成材料から構成され、前記小葉アセンブリが、前記円筒状フレーム構造の前記内腔内に配置され、弁を規定する複数の連接式隣接小葉構造を有する、請求項6に記載の自己拡張式ワイヤフレーム。
- 前記安定化組織が、成体、90日齢、又は30日齢のウシ、ヒツジ、ウマ、又はブタの心膜に由来し、又は動物の小腸粘膜下組織に由来する、請求項14に記載の自己拡張式ワイヤフレーム。
- 前記合成材料が、ポリエステル、ポリウレタン及びポリテトラフルオロエチレンからなる群から選択される、請求項14に記載の自己拡張式ワイヤフレーム。
- 前記安定化組織又は合成材料が抗凝血剤で処理される、請求項14に記載の自己拡張式ワイヤフレーム。
- 予備形成された圧縮性経カテーテル人工循環器弁であって、外側僧帽弁環状部カラー構成要素内に内側弁構成要素として取り付けられた請求項14に記載の自己拡張式ワイヤフレームを備え、前記僧帽弁環状部カラー構成要素が、心房カラーを形成するために組織カバーを有する複数の連接式カラー支持構造を遠位端に有する自己拡張式フレームを備え、前記予備形成された圧縮性経カテーテル人工循環器弁の展開が、前記僧帽弁環状部内に弁封止を形成する、予備形成された圧縮性経カテーテル人工循環器弁。
- 前記人工循環器弁が、低い高さ対幅のプロファイルを有する、請求項18に記載の人工循環器弁。
- 前記外側僧帽弁環状部カラー構成要素の断面が半円形のD字形である、請求項18に記載の人工循環器弁。
- 前記自己拡張式ワイヤフレーム及び前記外側僧帽弁環状部カラー構成要素の自己拡張式フレームが、超弾性金属の同一片から形成される、請求項18に記載の人工循環器弁。
- 前記自己拡張式ワイヤフレーム及び前記外側僧帽弁環状部カラー構成要素の自己拡張式フレームが、安定化組織でカバーされ、前記安定化組織が成体、90日齢、又は30日齢のウシ、ヒツジ、ウマ、又はブタの心膜に由来し、又は動物の小腸粘膜下組織に由来する、請求項18に記載の人工循環器弁。
- 前記自己拡張式ワイヤフレーム及び前記外側僧帽弁環状部カラー構成要素の自己拡張式フレームが、ポリエステル、ポリウレタン、及びポリテトラフルオロエチレンからなる群から選択された合成材料でカバーされる、請求項18に記載の人工循環器弁。
- 前記弾性材料、安定化組織又は合成材料が抗凝血剤で処理される、請求項23に記載の人工循環器弁。
- 前記弾性材料、前記安定化組織又は合成材料がヘパリン化される、請求項23に記載の人工循環器弁。
- 前記自己拡張式ワイヤフレームが一体型内側弁繋ぎ材装置を有する、請求項18に記載の人工循環器弁。
- 前記拡張式フレームが近位端に一体型カラー繋ぎ材装置を有する、請求項18に記載の人工循環器弁。
- 前記一体型内側弁繋ぎ材装置が心外膜繋ぎ材固定装置に取り付けられる、請求項26に記載の人工循環器弁。
- 前記一体型カラー繋ぎ材装置が心外膜繋ぎ材固定装置に取り付けられる、請求項26に記載の人工循環器弁。
- 前記一体型内側弁繋ぎ材装置が心外膜繋ぎ材固定装置に取り付けられる、又は、前記一体型カラー繋ぎ材装置が心外膜繋ぎ材固定装置に取り付けられる、請求項26に記載の人工循環器弁。
- 前記弁を前記循環器弁環状部内に取り付けるために、限定せずに、かかり、ピン及び/又はフック、又は、それらの組合せを含む、1以上の標準的な定着要素をさらに備える、請求項30に記載の人工循環器弁。
- 患者の心臓弁の疾患又は障害を治療する方法であって、請求項18に記載の人工心臓弁を患者の心臓弁の本来の環状部に外科的に展開するステップを含む方法。

- 前記人工心臓弁が、経動脈逆行性大動脈弁アプローチを使用して心臓の左心室に接近することによって展開される、請求項32に記載の方法。
- 前記人工心臓弁が、経静脈心室中隔開口術アプローチを使用して心臓の左心室に接近することによって展開される、請求項32に記載の方法。
