I. 概要
免疫細胞、例えばT細胞などの細胞に形質導入することを含む、細胞(例えばT細胞)へのウイルスベクターの移入方法であって、形質導入が、細胞の事前活性化なしに、および/もしくは対象から細胞を得た後24時間以内に開始される期間内に行われ、かつ/または対象から細胞を得た後、形質導入の前に細胞が、15℃〜25℃よりも高い、例えば37°±2.0℃よりも高いまたは約37°±2.0℃よりも高い温度に数時間よりも長く(および24時間以下にわたり)供されたことがない、移入方法が提供される。いくつかの態様では、提供される方法は、細胞をウイルス粒子と一緒に接触させるまたはインキュベーションする前におよび/またはそれに関連して、最初に、すなわち形質導入の前に、エクスビボ刺激試薬(例えば抗CD3/抗CD28試薬)を用いてT細胞を活性化および/または刺激せずに、レンチウイルスベクターなどのレトロウイルスベクター粒子を、免疫細胞、例えばT細胞などの細胞集団と一緒にインキュベーションすることおよび/または接触させることを含む。

いくつかの態様において、さらなるインキュベーションに続いて、細胞を調製するためのプロセスは、細胞を洗浄または製剤化することをさらに含むことができる。したがって、中でも、処理工程は、そのような組成物を製剤化することを含んでよい。
いくつかの局面において、該方法の生成物は、処置法、例えば治療法、例えば養子細胞療法において細胞および組成物を対象に投与するための方法において使用される。また、そのような方法ならびに該方法によって処理および生産された細胞の使用、ならびにその中で使用される薬学的組成物および製剤も提供される。提供される方法は、一般に、細胞または組成物、例えば、アウトプット組成物および/または製剤化された組成物を対象に投与することを伴う。
細胞または組成物は、疾患または病状を処置するためインビボで組換え受容体(例えば、CAR)発現細胞の治療有効量をもたらす用量で投与される。疾患の予防または治療のために、適切な投与量は、処置されるべき疾患のタイプ、細胞または組換え受容体のタイプ、組み合わせられる他の薬物または作用物質、例えば細胞拡大をブースト、増大または強化するものの投与、疾患の重症度および経過、細胞が予防目的で投与されるか治療目的で投与されるか、以前の療法、対象の病歴および細胞に対する応答、ならびに主治医の判断に依存し得る。組成物および細胞は、いくつかの態様において、一度または一連の処置にわたって患者に適切に投与される。
いくつかの態様において、組換え抗原受容体、例えばCARまたはTCRで改変された細胞を含む細胞の用量は、組成物または製剤として、例えば薬学的組成物または製剤として提供される。そのような組成物を、提供される方法に従って、および/または、提供される製造品または組成物と共に、例えば、疾患、病状、および障害の予防または治療において、または検出法、診断法、および予測法において使用することができる。
いくつかの態様において、細胞は、組み合わせ処置の一部として、例えば、別の作用物質、例えば治療用物質、例えば薬物と同時にまたは任意の順序で連続的に投与される。いくつかの態様において、他の作用物質、例えば薬物は、治療介入、例えば抗体または改変された細胞または受容体または作用物質、例えば細胞傷害性物質または治療用物質であることができる。いくつかの態様において、他の作用物質、例えば薬物は、細胞の拡大、増殖、生存および/または効力を増強、増大またはブーストする作用物質であることができる。いくつかの状況では、細胞は、細胞集団が1以上の追加の治療用物質の効果を増強するような十分に短い期間で別の療法と共投与される(逆もまた同様である)。いくつかの態様において、細胞は、1以上の追加の治療用物質の前に投与される。いくつかの態様において、細胞は、1以上の追加の治療用物質の後に投与される。
いくつかの態様において、該方法は、投与された細胞、例えば、組換え受容体を発現するように改変された細胞以外の作用物質を、例えば組み合わせ療法において投与することを含む。いくつかの態様において、作用物質は、導入遺伝子、例えば、組換え受容体をコードする導入遺伝子を特異的にモジュレートすることによって、改変された細胞の拡大、増殖、生存および/または効力を特異的に増大、ブーストまたは強化する。いくつかの態様において、作用物質は、導入遺伝子、例えば、組換え受容体を特異的に標的とする。いくつかの態様において、作用物質は、組換え受容体に結合し、活性化し、かつ/または、その活性および/または導入遺伝子によってコードされる組換え分子の全てもしくは一部の他の機能を増強する。いくつかの態様において、作用物質と組換え細胞を組み合わせた投与は、投与された細胞の増殖、拡大および/または生存を強化、ブーストまたは増大する、例えば、細胞のインビボ拡大を強化することができる。
いくつかの態様において、該方法は、投与される改変細胞を一般に含む免疫細胞または免疫機能の拡大、増殖、生存および/または活性のモジュレーションを含む。いくつかの態様において、該方法は、インビボで、例えば対象の体内で、一般に免疫刺激性である工程、または、投与された細胞を含む免疫細胞の拡大、増殖、生存および/もしくは活性を一般に促進、強化、増大および/もしくはブーストする工程を含む。いくつかの態様において、作用物質は、免疫細胞、例えば投与された細胞の増殖、拡大および/または生存を抑制する阻害因子の効果をインビボで低減、阻害および/または最小化することができる。
いくつかの態様において、該方法は、改変された細胞の拡大を、例えば、投与された細胞、例えば、改変された免疫細胞の増殖、拡大および/または活性化の負の調節因子を阻害することによって、モジュレートすることを含む。対象の体内の特定の環境で、組換え受容体を発現する投与された細胞は、細胞の成長、増殖、拡大および/または生存を抑止または抑制する環境、例えば免疫抑制環境と遭遇し得る。例えば、免疫抑制環境は、免疫抑制サイトカイン、調節モジュレーターおよび共阻害性受容体を含有することができる。いくつかの態様において、追加の作用物質を使用して、投与された細胞の拡大をモジュレートする、例えば、抑制環境を克服することができる。
いくつかの態様において、該方法は、免疫刺激性である追加の作用物質を投与することを含む。いくつかの態様において、追加の作用物質は、一般に、免疫細胞の増殖、拡大、生存および/または効力を促進することができる。いくつかの態様において、追加の作用物質は、投与される細胞、例えば、組換え受容体発現細胞を特異的に増進させることができる。いくつかの態様において、追加の作用物質は、サイトカインである。いくつかの態様において、追加の作用物質は、リガンドである。
いくつかの局面において、提供される方法は、1以上のリンパ球枯渇療法を、例えば、細胞、例えば組換え受容体発現細胞の投与の開始の前にまたはそれと同時に投与することをさらに含むことができる。いくつかの態様において、リンパ球枯渇療法は、シクロホスファミドなどのホスファミドの投与を含む。いくつかの態様において、リンパ球枯渇療法は、フルダラビンの投与を含むことができる。いくつかの態様において、フルダラビンは、リンパ球枯渇療法から除外される。いくつかの態様において、リンパ球枯渇療法は投与されない。
ある特定の態様において、治療有効性または予防有効性が増加するようにかつ/または拡大、増殖、生存および/もしくは効力をモジュレートできるように、いくつもの手法で細胞が修飾される。いくつかの態様において、対象への投与後にインビボで拡大、増殖、生存および/または効力をモジュレートできるように、例えば、強化、ブーストおよび/または増大できるように、細胞が修飾される。いくつかの態様において、導入遺伝子および/または免疫調節因子の発現をモジュレートおよび/または制御できるように、細胞が修飾される。いくつかの態様において、組換え受容体の特定の成分の発現および/または活性をモジュレートするように、細胞が修飾される。いくつかの態様において、作用物質、例えば核酸、例えば阻害性核酸の発現を増加または減少させるように、細胞が修飾される。いくつかの態様において、作用物質を発現および/または分泌するように、細胞が修飾される。
いくつかの態様において、該方法は、細胞と、投与された細胞(例えば、組換え受容体を発現する改変されたT細胞)の負の調節因子の発現を低減するかまたはその低減を達成することができる作用物質とを接触させることによって、投与されるべき細胞を修飾することを含む。細胞の負の調節因子は、免疫チェックポイント阻害剤、阻害性受容体および/またはアデノシンモジュレーターなどの本明細書に記載したいずれかを含む。いくつかの態様において、負の調節因子の発現を低減するかまたはその低減を達成することができる作用物質は、負の調節因子をコードする遺伝子または核酸に相補的である、それを標的とする、阻害するおよび/またはそれに結合するものなどの阻害性核酸分子であるかまたはそれを含む作用物質を含む。いくつかの態様において、作用物質は、Cas9、例えばいくつかの場合に酵素的に不活性なCas9と負の調節因子をコードする遺伝子を標的とするgRNAとを含む、リボヌクレオタンパク質(RNP)複合体を含む複合体であるかまたはそれを含む。

以下の実施例は、例証を目的としてのみ含まれ、本発明の範囲を限定する意図はない。
まず、形質導入の前に細胞をエクスビボT細胞活性化工程に供することなしに、ヒト白血球除去試料から濃縮された初代CD4+およびCD8+T細胞のレンチウイルス形質導入を行った。
この実施例は、大規模プロセスを介した、健常ドナーアフェレーシス由来のCD4+およびCD8+選択T細胞の形質導入を説明する。このプロセスでは、細胞を、様々な条件下で、取得、親和性選択、形質導入(事前活性化ありまたはなし)、および培養またはインキュベートした。洗浄、親和性に基づく選択および形質導入を、例えば、国際公開番号WO2016/073602に記載の通り、実質的に縦型の遠心分離処理チャンバー内で実施した。
白血球除去収集システムを使用して、患者から自己末梢血単核細胞(PBMC)を収集した。血液を処理して、単核細胞およびおよそ1/3の体積の自己血漿および抗凝血剤ACD-A(抗凝血剤クエン酸デキストロースA)を含有する試料を生成した。白血球除去試料を、2〜8℃でおよそ24時間、貯蔵および密閉した。
白血球除去試料を移入パックに無菌移入した。白血球除去試料の細胞を、親和性に基づく選択のために、PBS、EDTA、およびヒト血清アルブミンを含有する選択緩衝液中で洗浄し、再懸濁した。遠心分離処理チャンバー(A-200F)を含む、再生医療において使用するためのBiosafe SAによって販売されている無菌の単回使用ディスポーザブルキット内で洗浄を行った。細胞を含有する移入パックおよび緩衝液を含有するバッグを、Sepax(登録商標)2処理ユニットと連携して設置されたキットに無菌接続した。キャビティー内壁のRCFおよそ200gで180秒行い、続いて、20mL中に細胞を最終再懸濁する、洗浄サイクルを2回実施した。プロトコールの最後に、親和性に基づく選択用の試薬とのその後のインキュベーションのために、遠心分離チャンバーの処理キャビティー中に細胞を保持した。
モノクローナル抗体にカップリングさせた磁気ビーズを使用した免疫親和性に基づく選択によって、CD4およびCD8 T細胞を濃縮し、これを遠心分離チャンバー内の洗浄した白血球除去に添加した。ビーズを上述の選択緩衝液中で混合し、次いで、これを装置に無菌接続した。Sepax(登録商標)2ユニットでプログラムを実行して、洗浄した細胞を有するチャンバー内にビーズ混合物および選択緩衝液を流入させ、チャンバーの内容物を30分間混合した。
Sepax(登録商標)処理ユニットと連携して設置されたキットに内蔵された遠心分離チャンバー中で、レンチウイルスベクターを用いて形質導入を行った。選択された細胞を、形質導入(t=0で形質導入)するかまたは抗CD3/CD28試薬を使用して活性化した(t=0で活性化し、t=24時間で形質導入した)。

- 1種または複数種の刺激剤が、CD3結合分子;CD28結合分子;組換えIL-2;組換えIL-15;および組換えIL-7からなる群より選択される、請求項71記載の方法。
- 1種または複数種の刺激剤が、抗CD3抗体および/または抗CD28抗体を含む、請求項71または72記載の方法。
- 活性化および/または拡大が、インビボで行われる、請求項69記載の方法。
- 活性化および/または拡大が、抗原受容体により特異的に結合された抗原の存在下で起こり、かつ/または導入遺伝子特異的である、請求項69または74記載の方法。
- 前記インキュベーションに続いて、アウトプット組成物中の細胞が、任意でCD3結合分子;CD28結合分子;組換えIL-2;組換えIL-15;および組換えIL-7からなる1種もしくは複数種の刺激剤の存在下でエクスビボでさらにインキュベーションされず、かつ/またはアウトプット組成物中の細胞が、30℃よりも高い温度で24時間よりも長くさらにインキュベーションされない、請求項72または75記載の方法。
- 請求項1〜76のいずれか一項記載の方法により産生される、遺伝子操作T細胞。
- 請求項77記載の遺伝子操作T細胞と、薬学的に許容される担体とを含む、組成物。
- 疾患または状態を有する対象に請求項78記載の組成物を投与する工程を含む、処置方法。
- 前記組成物が、対象から試料を得た後5日以内に対象に投与される、請求項79記載の方法。
- 前記組成物が、対象から試料を得た後1日、2日、3日または4日以内に対象に投与される、請求項80記載の方法。
- 以下の工程を含む、養子細胞療法のための方法:
(a)疾患または状態を有する対象から得られた試料からT細胞を富化または単離する工程;
(b)請求項1〜71のいずれか一項記載の方法により、T細胞を含むインプット組成物にウイルスベクター粒子を形質導入し、それにより、形質導入細胞を含むアウトプット組成物を産生する工程であって、ウイルスベクター粒子が、疾患または障害と関連する抗原と特異的に結合する抗原受容体をコードする組換え核酸を含む、工程;
(c)疾患または状態を処置するために、形質導入細胞を含むアウトプット組成物を対象に投与する工程であって、アウトプット組成物が、対象から試料を得た後9日以内に該対象に投与される、工程。 - 疾患または状態を処置するために、組換え核酸を形質導入されたT細胞を含むアウトプット組成物を対象に投与する工程を含む、養子細胞療法の方法であって、アウトプット組成物が、請求項1〜71のいずれか一項記載の方法により産生される、方法。
- アウトプット組成物が、対象から試料を得た後1日、2日、3日、4日、または5日以内に対象に投与される、請求項82または83記載の方法。
- 前記組成物を投与する前に、アウトプット組成物の形質導入細胞または細胞が、薬学的に許容される緩衝液中で製剤化される、請求項82〜84のいずれか一項記載の方法。
- 形質導入細胞を含むアウトプット組成物を投与する前に、細胞が、形質導入後最大24時間、2日、3日、4日、5日、6日、7日、8日、または9日間エクスビボで培養され、該培養が、30℃よりも高い温度で行われる、請求項82〜85のいずれか一項記載の方法。
- 形質導入に続いて、形質導入細胞を含むアウトプット組成物または細胞が、T細胞を活性化すること、TCR複合体を経由するシグナルを誘導すること、および/またはT細胞の増殖を誘導することが可能な1種または複数種の刺激剤の存在下で培養され、それにより、形質導入細胞を含む組成物を産生する、請求項82〜86のいずれか一項記載の方法。
- 形質導入細胞を投与する前に、形質導入細胞を含むアウトプット組成物または細胞が、1種もしくは複数種の刺激剤の存在下でエクスビボでさらにインキュベーションされず、かつ/または30℃よりも高い温度で24時間よりも長くさらにインキュベーションされない、請求項82〜85のいずれか一項記載の方法。
- 1種または複数種の刺激剤が、CD3結合分子;CD28結合分子;組換えIL-2;組換えIL-15;および組換えIL-7、抗原受容体により特異的に認識される抗原を含むワクチン、ならびに抗原受容体と特異的に結合する抗イディオタイプ抗体からなる群より選択される、請求項87または88記載の方法。
- 1種または複数種の刺激剤が、抗CD3抗体および/または抗CD28抗体を含む、請求項89記載の方法。
- アウトプット組成物の細胞または形質導入細胞が、最適以下の用量で投与される、請求項82〜90のいずれか一項記載の方法。
- 対象に1種または複数種の剤を投与する工程であって、形質導入T細胞のインビボ刺激および/または拡大を誘導または強化する、工程
をさらに含む、請求項82〜91のいずれか一項記載の方法。 - 1種または複数種の剤が、導入遺伝子に特異的であり、かつ/または任意で抗原受容体であるかもしくはそれを含む発現した導入遺伝子を介して細胞を刺激もしくは活性化する、請求項92記載の方法。
- 1種または複数種の剤が、抗原受容体により特異的に認識される抗原を含むワクチン、抗原受容体と特異的に結合する抗イディオタイプ抗体、または抗原受容体の二量体化を化学的に誘導することが可能な剤の中より選択される、請求項92または93記載の方法。
- 1種または複数種の剤が、免疫調節剤;免疫チェックポイント阻害剤;細胞外アデノシンまたはアデノシン受容体、任意でA2aR受容体の阻害剤;キヌレニン経路モジュレーター、およびシグナル伝達経路のモジュレーター、例えばキナーゼ阻害剤である、請求項92記載の方法。
- 標的抗原と特異的に結合するキメラ抗原受容体(CAR)またはトランスジェニックTCRを発現するように遺伝子操作された初代ヒトT細胞集団を含む組成物であって、
該集団が、複数の休止T細胞を含み;かつ
該複数の休止T細胞が、該組成物中の遺伝子操作細胞の少なくとも7.5%を構成する、
組成物。 - 遺伝子操作された休止T細胞が、前記組成物中の遺伝子操作細胞の少なくとも10%、少なくとも20%、少なくとも30%、少なくとも40%、少なくとも50%、少なくとも60%、少なくとも70%、少なくとも80%、または少なくとも90%を構成する、請求項96記載の組成物。
- 休止T細胞が、HLA-DR、CD25、CD69、CD71、CD40L(CD154)および4-1BB(CD137)からなる群より選択されるT細胞活性化マーカーについて表面陰性であり;IL-2、IFN-ガンマおよびTNF-アルファからなる群より選択されるサイトカインの細胞内発現を欠如し;G0もしくはG0G1a期の細胞周期であり;かつ/または活性SAMHD1を含有する、請求項96または97記載の組成物。
- 休止T細胞が、CD25およびCD69について表面陰性(CD25-/CD69-)である、請求項98記載の組成物。
- 休止T細胞が、CD4+および/またはCD8+ T細胞を含む、請求項96〜99のいずれか一項記載の組成物。
- 標的抗原が、疾患または障害に関連する、請求項96〜100のいずれか一項記載の組成物。
- 疾患または障害が、感染性疾患もしくは状態、自己免疫疾患、炎症疾患またはがんである、請求項100記載の組成物。

- 細胞内シグナル伝達ドメインが、CD3-ゼータ(CD3ζ)鎖の細胞内ドメインを含む、請求項104記載の組成物。
- CARが、細胞外ドメインと細胞内シグナル伝達ドメインとを連結する膜貫通ドメインをさらに含む、請求項104または105記載の組成物。
- 膜貫通ドメインが、CD28の膜貫通部分を含む、請求項106記載の組成物。
- CARの細胞内シグナル伝達ドメインが、T細胞共刺激分子の細胞内シグナル伝達ドメインをさらに含む、請求項104〜107のいずれか一項記載の組成物。
