Description
本発明は、粘膜組織に由来する抗原を標的とするT細胞を含む組成物、このような組成物を製造する方法、ならびに、起源が粘膜組織である転移性癌に対して個体を保護するために、および起源が粘膜組織である転移性癌に罹患している個体を処置するためのそれらを使用する方法に関する。
治療における改良および成功にもかかわらず、癌は、世界中で多数の人々の命を奪い続けている。例えば、結腸直腸癌は、西洋諸国において悪性疾患の3番目に多い死因である。世界中で、毎年、少なくとも50万の結腸直腸癌の新たな症例があると概算されている。
粘膜限定抗原(mucosally restricted antigen)の少なくとも1つのエピトープを認識する単離された複数のT細胞を提供する。
定義
本明細書において使用される「粘膜組織」は、鼻、口、肺および消化管を含む体中のいくつかの臓器および体腔を裏打ちする湿潤組織である粘膜の組織を示す。粘膜組織は、限定はしないが、口、例えば、頬、舌下および口腔粘膜組織;鼻、例えば、嗅粘膜組織;肺;消化管、例えば、食道、胃、十二指腸、小腸および大腸、結腸、直腸および肛門;ならびに泌尿生殖器、例えば、膀胱、尿道、膣の一部、陰茎および子宮の一部を含む体のいくつかの様々な一部において見ることができる。粘膜組織は、また、乳房、腎臓および眼の一部として見出される。
米国仮シリアル番号61/099,398(これは、出典明示により本明細書に包含させる)は、癌粘膜抗原(CMA)と称される粘膜(気道消化器、泌尿生殖器、乳房、その他)から生じる腫瘍に対するワクチン標的の1つのクラスを標的化するワクチンを記載している。2004年7月22日に公開された米国公開特許出願第20040141990号(これは、出典明示により本明細書に包含させる)は、転移性結腸直腸癌に対するワクチンを記載している。米国公開特許出願第20010039017、20010036635、20010029020および20010029019号(これらは、それぞれ出典明示により本明細書に包含させる)は、それぞれ、消化管起源の癌細胞を標的化するワクチンを記載している。
粘膜限定抗原の少なくとも1つのエピトープを認識するT細胞のクローン性増殖は、細胞ドナーからこのようなT細胞を単離すること、および、培養技術を使用して、細胞分裂を促進する条件下で維持することにより細胞の数を指数関数的に増大させることを含む。
粘膜限定抗原の少なくとも1つのエピトープを認識する複数のT細胞は、細胞ドナーからT細胞を単離すること、それを粘膜限定抗原の少なくとも1つのエピトープを認識するT細胞受容体をコードする核酸分子で形質転換すること、および、該形質転換された細胞を培養して、形質転換されたT細胞の数を指数関数的に増大させ、このような複数の細胞を生産することにより得ることができる。
粘膜限定抗原の少なくとも1つのエピトープを認識する複数のT細胞は、細胞ドナーからT細胞を単離すること、それを、粘膜限定抗原の少なくとも1つのエピトープに結合する抗体の機能性結合フラグメントおよび細胞、例えば、T細胞において発現するとき、タンパク質を膜結合型タンパク質にする部分を含むCMA結合性膜結合型融合タンパク質をコードする核酸分子で形質転換すること、および、該形質転換された細胞を培養して、形質転換されたT細胞の数を指数関数的に増大させ、このような複数の細胞を生産することにより得ることができる。
ワクチンは、粘膜限定抗原の1つ以上のエピトープに対する免疫応答を誘導するため、およびCMA結合性膜結合型融合タンパク質を製造するために有用なTCR遺伝子ドナーおよび/またはB細胞のドナーを生産するために使用され得る。CD4+ヘルパーエピトープは、広範囲にわたる免疫応答を誘導するために提供される。ワクチンの例は、限定はしないが、以下のワクチン技術を含む。

3)死滅または不活化ワクチンであって、該ワクチンは、a)粘膜限定抗原の少なくとも1つのエピトープおよびCD4+ヘルパーエピトープを含むキメラタンパク質を示す死滅細胞または不活化ウイルス粒子のいずれかを含み、そしてb)個体に投与されるとき、粘膜限定抗原を標的化する免疫応答を誘導する;
4)ハプテン化された死滅または不活化ワクチンであって、該ワクチンは、a)粘膜限定抗原の少なくとも1つのエピトープおよびCD4+ヘルパーエピトープを含むキメラタンパク質を示す死滅細胞または不活化ウイルス粒子のいずれかを含み、b)さらなる免疫原性となるようにハプテン化され、そしてc)個体に投与されるとき、粘膜限定抗原を標的化する免疫応答を誘導する;
5)サブユニットワクチンであって、該ワクチンは、粘膜限定抗原の少なくとも1つのエピトープおよびCD4+ヘルパーエピトープを含むキメラタンパク質を含む;ならびに
6)ハプテン化されたサブユニットワクチンであって、該ワクチンは、a)粘膜限定抗原の少なくとも1つのエピトープおよびCD4+ヘルパーエピトープを含むキメラタンパク質を含み、そしてb)さらなる免疫原性となるようにハプテン化される。
粘膜限定タンパク質は、一般的に、粘膜の外側で発現されない。したがって、粘膜限定タンパク質は、粘膜の外側で存在するとき、免疫系の種々の成分の少なくともいくつかに対して免疫原性であるため、粘膜限定タンパク質を標的とする全身免疫応答が産生され得る。すなわち、それは免疫系が免疫応答を引き起こすことができない自己タンパク質ではない。一般的に、粘膜限定タンパク質は、正常粘膜ならびに粘膜細胞起源の癌細胞、またはそうでなければ粘膜細胞由来の癌細胞で発現される細胞タンパク質である。したがって、粘膜限定タンパク質に対する免疫応答は、粘膜限定タンパク質を発現する粘膜の外側の細胞、例えば、転移性癌細胞を認識し、攻撃する。一般的に、CD4+免疫応答は、粘膜限定タンパク質が粘膜の外側の組織または体液に導入されているとき、存在しないか、または有意に減少している。
CD4+ヘルパーエピトープ中、ワクチンの製造において有用であり得るものは、MHCクラスII HLA血清型HLA−DP、HLA−DQおよびHLA−DRと複合体を形成する物である。一般的に、自己分子は、MHCクラスII HLAと複合体を形成せず、次に複合体がCD4+T細胞受容体に結合する。したがって、CD4+ヘルパーエピトープは、一般的に異なる種、最も一般的には病原性種に由来する。いくつかの型のMHCクラスII HLAと複合体を形成し、次に複合体がCD4+T細胞受容体に結合するCD4+ヘルパーエピトープは、ユニバーサルCD4+ヘルパーエピトープと称される。

分泌された抗原は、筋肉内免疫化後により強力なCD4、CD8および抗体応答を誘導する(Boyle JS, Koniaras C, Lew AM. Influence of cellular location of expressed antigen on the efficacy of DNA vaccination: cytotoxic T lymphocyte and antibody responses are suboptimal when antigen is cytoplasmic after intramuscular DNA immunization. 粘膜限定タンパク質の少なくとも1つのエピトープを認識するT細胞およびB細胞を含む免疫応答を誘導するために有用ないくつかのワクチンは、個体に投与され、それにより、核酸分子は個体の細胞により取り込まれ、発現され、核酸分子によってコードされるタンパク質を生産する核酸分子を含む。個体自身の細胞内でタンパク質を生産することにより、タンパク質は、細胞性免疫応答を確保するように処理され、標的免疫原に対する広くより有効な免疫応答を生産することができる。

ワクチンの他の形態は、ハプテン化されたサブユニットワクチンを含むサブユニットワクチンを含む。サブユニットワクチンは、一般的に、免疫応答が望まれる免疫原性標的を含む単一のタンパク質またはタンパク質複合体を示す。サブユニットワクチンは、本明細書において、粘膜限定抗原の少なくとも1つのエピトープおよびCD4+ヘルパーエピトープを含むキメラタンパク質を含む。サブユニットワクチンは、タンパク質をさらに免疫原性にするためにハプテン化され得る。すなわち、ハプテン化は、粘膜限定抗原の1つ以上のエピトープに対する増強された免疫応答をもたらす。
いくつかの態様において、ワクチンは、サブユニットワクチンを構成するタンパク質または死滅または不活化ワクチンの細胞または粒子である。いくつかの態様において、このようなサブユニットワクチンを構成するタンパク質または死滅または不活化ワクチンの細胞または粒子は、免疫原性を増加させるためにハプテン化され得る。いくつかの場合において、ハプテン化は、大型分子構造を粘膜限定抗原もしくはそのフラグメントまたは粘膜限定抗原もしくはそのフラグメントを含むタンパク質と結合させることである。いくつかの場合において、患者由来の腫瘍細胞は、有効なワクチン製品を製造するための手段として死滅およびハプテン化される。粘膜限定抗原もしくはそのフラグメントまたは粘膜限定抗原もしくはそのフラグメントを含むタンパク質を製造する、および示すために遺伝情報で提供される他の細胞、例えば、細菌または真核細胞が、死滅され、活性なワクチン成分として使用される場合において、このような細胞は、免疫原性を増加させるためにハプテン化される。ハプテン化は、よく知られており、容易に行うことができる。
本発明の局面は、粘膜組織の癌を有する個体を処置する方法を含む。該処置は、全身的に提供される。本明細書に記載されている粘膜限定抗原の少なくとも1つのエピトープを認識する複数のT細胞でこのような個体を処置することにより、T細胞は、特に個体の免疫系の非粘膜区画において、粘膜限定抗原を発現する癌細胞を特異的に標的する。すなわち、T細胞は、粘膜の外側に存在する粘膜組織から生じるあらゆる癌細胞を攻撃する。粘膜限定抗原の少なくとも1つのエピトープを認識する複数のT細胞は、同定された転移性疾患、ならびに、あらゆる検出されていない転移、例えば、微小転移を含むあらゆる転移性疾患を処置するために特に有用である。
粘膜限定抗原の少なくとも1つのエピトープを認識する複数のT細胞は、また、粘膜組織癌を発症する危険性がある個体において予防的に使用され得る。集団と比較して上昇した、または特に高い危険性がある個体を同定するいくつかの方法がある。いくつかの癌の危険性は、ファミリーの病歴および/または遺伝子マーカーの存在に基づいて予測することができる。ある挙動またはある環境因子への暴露によって、個体を高い危険性がある集団に分類され得る。除去されたまたは寛解期の原発性疾患との以前の診断は、個体をより高い危険性に分類される。当業者は、個体の危険性を評価すること、および個体が粘膜組織由来の癌の上昇した、または高い危険性があるか否かを決定することができる。
いくつかの態様における粘膜限定抗原の少なくとも1つのエピトープを認識する複数のT細胞は、該細胞と共に薬学的に許容される担体を含む。細胞を含む医薬製剤はよく知られており、当業者により日常的に製剤化され得る。適当な医薬担体は、この分野における標準文献であるRemington's Pharmaceutical Sciences, A. Osol(これを出典明示により本明細書に包含させる)に記載されている。本発明は、注入用の医薬組成物に関する。
T細胞は、白血球除去輸血による結腸直腸癌患者のPBMCから、または、結腸直腸癌患者の腫瘍浸潤リンパ球(TIL)から回収され得る。TIL外植片またはPBMC由来T細胞を、6000IU/mlのIL−2を含む完全培地(10%のヒト血清を補ったRPMI1640ベース培地)中で培養する。培養物は、数百万のTIL細胞は利用するまで、通常2−4週間、1mlあたり5×105から2×106細胞の細胞濃度で維持してもよい。多数の独立した培養物を、CMAエピトープの認識のためのサイトカイン分泌アッセイによりスクリーニングしてもよい。最も高いサイトカイン分泌を示す2から6つの独立したTIL培養物を、細胞数が5×107個を越えるまで(この細胞数には、一般的に、腫瘍切除後に3−6週間で達する)、1mlあたり6000IUのIL−2を有する完全培地中でさらに増殖させてもよい。CMA認識を維持したTIL培養物を、照射された同種異系の支持細胞、OKT3(抗CD3)抗体、および1mlあたり6000IUのIL−2での迅速な拡張プロトコールの1サイクルを使用して、処置のために増殖させる。この迅速な拡張プロトコールは、一般的に、拡張の開始後14−15日間の投与時間により1000倍の細胞の拡張をもたらす。患者は、0.5から1時間にわたって、1×1011個の細胞のボーラス静脈内注入を受けてもよい。
操作するためのT細胞は、50ng/mlのOKT3を補った、300IU/mlのIL−2、100U/mlのペニシリン、100μg/mlのストレプトマイシン、1.25μg/mlのアンホテリシン、10μg/mlのシプロフロキサシンおよび5%のヒトAB血清を有するT細胞培養培地AIM−V(Invitrogen Corp, Grand Isle, NY)中で1×106/mlの濃度で細胞を培養することによる白血球の採取の後にPBMCから得ることができる。培養の2日後、細胞を回収し、OKT3を含まない新鮮なT細胞培養培地に再懸濁する。マウス幹細胞ウイルス(MSCV)末端反復(LTR)および高度に効率的な内部リボソーム侵入部位(IRES)を使用して、CMA特異的TCRαおよびβ鎖またはCMA結合性膜結合型融合タンパク質のいずれかを発現するレトロウイルスベクター(例えば、pMSGV1)は、ヒトポリオウイルス(TCRのみに対する)に由来であった。臨床等級のレトロウイルスベクター上清は、商業的に生産されており、任意の選択方法の使用なしに高度に効率的な遺伝子導入をもたらす固相形質導入プロトコールにおいて使用される。最大5×108個の細胞の形質導入を、6mlのベクターおよび1ウェルあたり最大5×106個の細胞を使用して、Retronectin(CH−296、Takara Bio. Inc, Japanから購入したGMP等級Retronectin)で被覆され、ベクターで前負荷された6ウェルの組織培養プレート上での一晩培養により行う。患者は、0.5から1時間にわたって、1×1010−1011個の細胞のボーラス静脈内注入を受けてもよい。
CMA反応性T細胞を、PBMCまたはTILから上記のとおり増殖させる。個々の全長cDNA鎖のクローニングのためにPCRプライマーを設計するようにTCRαおよびβ鎖使用を決定するために、CMA反応性T細胞クローンから単離されたRNAを、RACE(cDNA末端の迅速な増幅)ポリメラーゼ連鎖反応(PCR)およびDNA配列分析に付す。PolyA+RNAは、Poly(A) Pure mRNA精製キット(Ambion, Austin, TX)を使用してT細胞から単離される。逆転写ポリメラーゼ連鎖反応(RT−PCR)は、再配列されたαおよびβTCR鎖に対するオリゴヌクレオチドプライマーの対を使用するTitan One Tube RT−PCRキット(Roche, Indianapolis, IN)を使用して行った。増幅された産物をゲル精製し、レトロウイルスベクター骨格にクローニングする。クローニングされたαおよびβセグメントを、シーケンシングにより確認する。
CMA特異的B細胞ハイブリドーマを生産する。マウスをCMA特異的B細胞(抗体)応答を生産するためにCMAで免疫化する。脾臓を回収し、抗体を生産するB細胞を回収する。これらを、メチルセルロースベースの培地系、ClonaCell−HYモノクローナル抗体生産キット(StemCell Technologies, Inc.)を使用して、SP2/0−Ag14骨髄腫細胞系と融合させる。融合した細胞を限界希釈法によりクローニングし、CMA特異的抗体生産のためにスクリーニングし、CMA抗体を生産するハイブリドーマを同定する。これらを、CMA特異的モノクローナル抗体の恒久的な供給源として維持する。
トランスファー(Transfer)は、サイトカイン投与(主にIL−2)、CMA指向性ワクチン接種および/または抗体治療、化学療法、シクロホスファミドおよびフルダラビン全身照射(TBI)での宿主調製リンパ球枯渇、その他の可能性のある補助剤処置を含む種々の処置と組み合わせてもよい。
Claims (9)
- グアニル酸シクラーゼCの少なくとも1つのエピトープを認識する複数のT細胞であって、該複数のT細胞が、グアニル酸シクラーゼCの少なくとも1つのエピトープを認識する抗体の少なくとも機能性抗原結合フラグメントを含むグアニル酸シクラーゼC結合性膜結合型融合タンパク質をコードする核酸分子で形質転換されたT細胞のクローン性増殖に由来する、複数のT細胞。
- 請求項1に記載の複数のT細胞の有効量を個体に投与する工程を含む、グアニル酸シクラーゼCを発現する癌細胞を有する癌と診断された個体を処置する方法において有用な医薬の製造のための、請求項1に記載の複数のT細胞の使用。
- 請求項1に記載の複数のT細胞の有効量を個体に投与する工程を含む、グアニル酸シクラーゼCを発現する癌細胞を有する癌を発症する危険性が高いと同定された個体においてグアニル酸シクラーゼCを発現する癌細胞を有する癌を予防する方法において有用な医薬の製造のための、請求項1に記載の複数のT細胞の使用。
- T細胞をグアニル酸シクラーゼCの少なくとも1つのエピトープを認識する抗体の少なくとも機能性抗原結合フラグメントを含むグアニル酸シクラーゼC結合性膜結合型融合タンパク質をコードする核酸配列で形質転換すること、ここに、発現時に、該融合タンパク質は膜結合型タンパク質である;および
該形質転換されたT細胞を、その複製を促進する条件下でグアニル酸シクラーゼCを認識する複数のT細胞を生産するために十分な期間培養すること
を含む、グアニル酸シクラーゼCの少なくとも1つのエピトープを認識する複数のT細胞を製造する方法。 - グアニル酸シクラーゼC結合性膜結合型融合タンパク質をコードする核酸配列が、グアニル酸シクラーゼCの少なくとも1つのエピトープを認識する抗体を生産するB細胞からグアニル酸シクラーゼCの少なくとも1つのエピトープを認識する抗体をコードする核酸配列を単離すること、グアニル酸シクラーゼCの少なくとも1つのエピトープを認識する抗体の機能性フラグメントおよび該融合タンパク質を膜結合型タンパク質にするタンパク質配列をコードするキメラ遺伝子を製造すること、および該キメラ遺伝子を含む発現ベクターを製造することにより得られる、請求項4に記載の方法。
- グアニル酸シクラーゼC結合性膜結合型融合タンパク質をコードする核酸配列が、細胞内で発現されるとき、グアニル酸シクラーゼCの少なくとも1つのエピトープに結合する抗体の少なくとも機能性フラグメントおよび融合タンパク質を膜結合型タンパク質にする部分に対するコード配列を含む、グアニル酸シクラーゼC結合性膜結合型融合タンパク質をコードする核酸配列を含む核酸分子であって、該グアニル酸シクラーゼC結合性膜結合型融合タンパク質をコードする核酸配列が所望により調節因子に作動可能に連結している、核酸分子。
- グアニル酸シクラーゼCに結合する一本鎖Fv抗体をコードする核酸配列;
リガンド結合領域が切断されているCD28細胞外ドメインをコードする核酸配列;および
FcRIγ鎖の細胞内配列をコードする核酸配列
を含む、請求項6に記載の核酸分子。 - 核酸分子がプラスミドまたはウイルスゲノムである、請求項6または7に記載の核酸分子。
- 請求項6から8のいずれかに記載の核酸分子を含むT細胞。
