Abstract
Description

別の実施形態では、投与される一つまたは複数の医薬製剤、すなわち、代謝障害またはその主要要素を治療するために組み合わされて投与される活性薬剤を含有する一つまたは複数の医薬製剤、貯蔵の間および使用前に前記一つまたは複数の製剤を収容するための容器、好ましくは密封容器、および患者における代謝障害またはその主要要素を治療するための薬剤投与を効果的に実行するための指示書を含有する、パッケージ化されたキットが提供される。指示書は、典型的には、添付文書上および/または標識上の書面による指示である。製剤の種類および意図される投与様式に応じて、キットは製剤を投与するための装置も含む場合がある。製剤は、本明細書に記載されるような、あらゆる適切な製剤であり得る。例えば、製剤は、選択された活性薬剤の単位投与量を含有する経口剤形であり得る。キットは、同一の作用剤の異なる投与量の複数の製剤を含有している場合がある。
プロトコル
雌スプラーグドーリーラット(12)を、14/10明暗周期を有する一定の温度および湿度の空間に収容し、適宜通常食を与えた。各ラットに、SCNに対する逆行性トレーサーであるフルオロゴールド(fluorogold)(4%、20nl)を微量注入し、10日間休息させた。その後、ラットに4%パラホルムアルデヒドを灌流し、脳を免疫組織化学用に解剖した。SCNに投射しているドーパミンニューロンを、フルオロゴールド(FG)およびチロシン水酸化酵素(TH)の二重免疫化学で特定した。機能的な繋がりを検証するため、SuMNへのAMPA(0.3mM/0.5l)の注入後の自由行動ラットにおける微量透析法により、SCNにおけるドーパミン代謝産物を測定した。
SCNにおける逆行性トレーサーフルオロゴールドの注入後、内側視索前野(MPO)、傍室傍核領域(SPVZ)、視床下部背内側核(DMH)、脳室周囲視床下部核(periventricular hypothalamic nucleus)(Pe)、室傍核(PVN)、不確帯(zona inserta)(ZI)、弓状核(Arc)、正中隆起(ME)、腹尾側視床下部後部(ventrocaudal posterior hypothalamus)(PH)、乳頭体上核(SuMN)および外側脚傍核(lateral parabranchial nucleus)(LPB)および孤束核(NST)等の、SCNから直接的な神経支配を受けることが知られている多くの脳領域で、フルオロゴールドの免疫反応性が確認された。

雌スプラーグドーリーに高脂肪食(HFD)または通常食を適宜与え、1日あたり14時間の明期に維持した。4ヵ月の摂食の後、通常食を与えられた10匹のラットおよび高脂肪食摂食により肥満になった10匹のラットを、微量透析試験のために選定した。微量透析実験の最後に、ラットを、それぞれ明期開始の4時間後(4「HALO」(4 hours after light onset))、16HALOに、4%パラホルムアルデヒドの経心臓灌流によ
って屠殺した。脳を、c−Fosおよびチロシン水酸化酵素の二重免疫組織化学用に解剖した。SuMN内のドーパミンニューロンの活性の指標として、SuMNにおけるc−Fosおよびチロシン水酸化酵素に対し二重免疫陽性のニューロンの数を計数した。
図2に示される結果は、SuMN内のドーパミン発現ニューロンの概日リズムを示しており、この概日リズムが脂肪食を与えられた肥満ラットにおいて乱されたことを示している。通常食を与えられた非肥満ラットは、明期(4HALO)と比較して暗期(すなわち、16HALO)において、SuMN内の二重標識されたc−Fos/TH発現ドーパミンニューロンの数の増加を示した。高脂肪食誘導性肥満ラットにおいて、SuMN内のc−Fos発現ドーパミンニューロンの夜におけるピークは減少し、SuMN内のc−Fos発現ドーパミンニューロンのレベルは明期において上昇した。図2を参照されたい。
プロトコル
別々のプロトコルで、雌スプラーグドーリー高脂肪食抵抗性ラット(HF−R)(16匹の動物)に高脂肪食を4ヵ月間与え、または、雌スプラーグドーリー高脂肪食感受性ラット(HF−S)(16匹の動物)に高脂肪食を3ヵ月間与えた。最初の摂食プロトコルの後、AMPA(0.03mM/0.5l)またはビヒクルを、自発運動活性の開始時に、13日間(HF−R動物)または14日間(HF−S)毎日、SuMNに微量注入した。AMPA処置の最後に、HF−R動物を断頭により屠殺した。体幹血をグルコース分析およびインスリン分析用に採取し、後腹膜脂肪の質量を量った。AMPA処置の最後に、HF−S動物を耐糖性試験(GTT)にかけ、GTTの2日後に、屠殺した。体幹血をグルコース分析およびインスリン分析用に採取し、後腹膜脂肪の質量を量った。体重および摂食量を両プロトコルの全体を通じて毎日測定した。
図3は、HF−Rラットに対する長期にわたるAMPA処置の結果を示している。長期にわたるAMPA処置(N=5 AMPA処置;N=6 ビヒクル)は、HF−Rラットにおいて、体重(p<0.05)、後腹膜脂肪33%(13.4g 0.48 対 9.02g 0.48;p<0.01)および総腹部脂肪32%(19.1g 0.8 対 12.9g 0.6;p<0.01)を有意に減少させた。一日摂食量のわずかであるが有意な減少も認められた(17.7g 2.1 対 13.7g 1.0;p<0.05

プロトコル
SCN領域へのドーパミン入力阻害がSCNの光感受性を変化させ得るかどうかを決定するため、SuMN内のドーパミンニューロンをCNQXおよびムシモールのSuMN内注入により阻害した。
分間の光パルス(〜200ルクス)に暴露し、残りの半分を暗赤色灯下に維持した。光パルス開始の1時間後、ラットを4%パラホルムアルデヒドの経心臓灌流により屠殺した。脳をc−Fosの免疫組織化学染色用に解剖した。SCN内のc−Fosの発現レベルを定量化した。
光パルスにより、SCNにおいて強いc−Fos発現が誘導された。c−Fos発現のレベルは、CNQX/ムシモール処置ラットのSCNの核心内で有意に減少した。これらの結果は、SuMN内のドーパミンニューロンの活性の阻害が光パルスに対するSCNの光感受性を変化させたことを示している。
SuMNを対象とするNMDA作動薬によるインスリン抵抗性高脂肪摂食ラットの処置は、インスリン抵抗性および耐糖能障害を改善した。雌スプラーグドーリーラットに高脂肪食を1ヶ月間与え、その後、前記食餌に対し最も感受性であった動物(群内で最大の重量を増加した動物)を、NMDA作動薬またはビヒクルで、光(14時間の日周明期)の停止時に3分間、SuMNに向けられたカニューレを介して、2週間毎日、高脂肪食を継続しながら、処置した。治療期間の最後に、動物を耐糖性試験(GTT)(3g/kgのグルコース)にかけ、グルコース処置の前およびグルコース処置の30、60、90、および120分後に血漿試料を採取した。血漿試料をグルコースレベルおよびインスリンレベルについて解析した。
NMDA処置は、対照ビヒクルで処置された高脂肪摂食動物と比較して、GTT中に血漿中のグルコースおよびインスリンのレベルを有意に減少させ、インスリン抵抗性を有意に改善した。結果の詳細については図6を参照されたい。
高脂肪食を与えられているインスリン抵抗性ラットのSCNへの直接的なドーパミンの注入によって、インスリン抵抗性および耐糖能障害が改善された。およそ12週齢の雄SHRラットに高脂肪食を1ヶ月間与えた後、ドーパミンまたはビヒクルで、光(14時間の日周明期)の停止時に3分間、SCNに向けられたカニューレを介して、2週間毎日、高脂肪食を継続しながら、処置した。治療期間の最後に、動物を耐糖性試験(GTT)(3g/kgのグルコース)にかけ、グルコース処置の前およびグルコース処置の30、60、90、および120分後に血漿試料を採取した。血漿試料をグルコースレベルおよびインスリンレベルについて解析した。
