慢性創傷、治癒が緩徐な創傷、および治癒の不完全な創傷は、結果として感染症をもたらすことが多く、切断術または死亡をもたらしうる。本出願で記載または言及される化合物を含めた特定の化合物の使用により、細胞連絡を遮断することも、阻害することも、変化させることもでき、これにより、慢性創傷、治癒が緩徐な創傷、および治癒が不完全な創傷における閉鎖および治癒を促進することができる。
癒着
本発明の他の態様内では、患者に抗コネキシンポリヌクレオチドを投与することにより、癒着、手術による癒着、および/または手術による続発性癒着を治療し、これらの発生を低下させるかもしくはこれらの重症度を軽減し、かつ/またはこれらを予防するかもしくは遅延させるための方法が提供される。

抗コネキシンポリヌクレオチドは、コネキシンアンチセンスポリヌクレオチドのほか、それらによるコネキシン発現の下方調節を可能とする機能性を有するポリヌクレオチドも含む。他の適切な抗コネキシンポリヌクレオチドは、RNAiポリヌクレオチドおよびsiRNAポリヌクレオチドを含む。抗コネキシン43ポリヌクレオチドが好ましい。

ギャップジャンクション調節剤は、ギャップジャンクションおよび/またはヘミチャネルを閉鎖もしくは遮断するか、またはギャップジャンクションを介する細胞間情報伝達を別の形で防止するかもしくは低下させるか、またはヘミチャネルを介する細胞外環境への細胞の情報伝達を別の形で防止するかもしくは低下させる薬剤を含む。これらは、ヘミチャネルまたはギャップジャンクションの活性、機能、または形成を、全体的または部分的に防止するか、低下させるか、または阻害する薬剤または化合物を含む。
本出願で用いられる抗オステオポンチン剤は、オステオポンチン発現の下方調節(例えば、mRNAの転写または翻訳の下方調節による)をもたらすか、または他の形でオステオポンチンタンパク質の活性を低下させるかもしくは阻害する。
治療有効量の本発明の各薬剤は、同時投与、個別投与、または逐次投与が、しかも任意の順序で可能である。薬剤は、個別投与される場合もあり、固定の組合せとして投与される場合もある。固定の組合せとして投与されない場合、好ましい方法には、単独の、または1または複数の抗コネキシン剤(例えば、抗コネキシンポリヌクレオチド、抗コネキシンペプチド、または抗コネキシンペプチド模倣剤)と組み合わせた1または複数の抗オステオポンチン剤の逐次投与が含まれる。
Software for Dose Effect Analysis(Biosoft(D,Cambridge UK)などのコンピュータソフトウェアツールを用いて実施することができる。
一態様において、1もしくは複数の活性成分は、包帯材またはマトリックスの形態で提供される。一部の実施形態では、本発明の1または複数の薬剤が直接的な適用のための液体組成物、半固体組成物、もしくは固体組成物の形態で提供されるか、あるいは組成物が包帯材ガーゼもしくはマトリックスなどの固体接触層の表面へと適用されるか、またはこの中へと組み込まれる。包帯材組成物は、例えば、流体またはゲルの形態で提供することができる。1または複数の活性成分は、局所適用のための従来の医薬賦形剤と組合せて提供することができる。適切な担体は、プルロニックゲル、ポロキサマーゲル、セルロース誘導体(ヒドロキシエチルセルロース、ヒドロキシメチルセルロース、カルボキシメチルセルロース、ヒドロキシプロピルメチルセルロース、およびこれらの混合物を含む)を含有するヒドロゲル;ならびにポリアクリル酸を含有するヒドロゲル(Carbopols)を含む。適切な担体はまた、局所医薬調製物に用いられるクリーム/軟膏、例えば、セトマクロゴール乳化軟膏に基づくクリームも含む。上記の担体は、アルギン酸塩(増粘剤または刺激剤として)、ベンジルアルコールなどの防腐剤、リン酸水素二ナトリウム/リン酸二水素ナトリウムなどのpH調整のための緩衝液、塩化ナトリウムなどの浸透圧を調整するための薬剤、およびEDTAなどの安定化剤を含みうる。
一般的な態様
本発明は、治療有効量の、単独のまたは1もしくは複数の他の抗コネキシン剤を組み合わせた1もしくは複数の抗オステオポンチン剤を含む医薬組成物およびそれらの使用法に関する。組成物は、急性創傷、および慢性創傷などの予測される速度では治癒しない創傷、ならびに従来の創傷治療または創傷治癒促進療法に対して治癒が遅いかあるいは不応な場合もある他の創傷を含む創傷の治癒を増強または促進するのに有用である。
組成物
したがって、一態様において、本発明は、抗オステオポンチン剤を含む、治療的な創傷の処置において用いられる組成物を提供する。別の態様において、本発明は、抗オステオポンチン剤および少なくとも1つの抗コネキシン剤を含む、治療的な創傷の処置において用いられる組成物を提供する。好ましい実施形態において、組成物は、薬学的に許容される担体またはビヒクルをさらに含む。別の態様において、本発明は、PDGF受容体の遮断剤またはアンタゴニストを含む、治療的な創傷の処置において用いられる組成物を提供する。別の態様において、本発明は、抗オステオポンチン剤および/または抗コネキシン剤と共にPDGF受容体の遮断剤またはアンタゴニストを含む、治療的な創傷の処置において用いられる組成物を提供する。
場合によって、単独の1または複数の抗オステオポンチン剤、あるいは1または複数の抗コネキシンポリヌクレオチド、および1もしくは複数の抗コネキシンペプチドまたは抗コネキシンペプチド模倣剤、ならびに/あるいはギャップジャンクションリン酸化剤もしくはヘミチャネルリン酸化剤またはコネキシンカルボキシ末端ポリペプチドなどの他の抗コネキシン剤、ならびに/あるいはPDGF受容体遮断剤もしくはアンタゴニストと組み合わせた1または複数の抗オステオポンチン剤はまた、医薬の製造においても用いることができる。
本発明の組成物および製剤は、創傷の治癒を促進するための組成物と共に用いることもでき、これと組み合わせて用いることもでき、これらはまた、例えば、腫脹、炎症、および/または瘢痕形成を軽減することも可能である。本発明の組成物および製剤はまた、急性創傷または慢性創傷(治癒が緩徐な創傷および治癒が遅延している創傷を含む)の治癒を促進および/または改善するための組成物と共に用いることもでき、これと組み合わせて用いることもできる。一態様において、創傷は、手術もしくは外傷、または、例えば、糖尿病、末梢浮腫、血管炎、もしくは心血管疾患などの基礎的な医学的状態の結果である。
以下の材料および方法は、本出願に記載の実施例全体で用いられた。
すべての実験は、英国内務省の法令に従い実施された。マウス(年齢を適合させた雄のICRマウス;7〜11週齢)にハロタン麻酔をかけ、剃毛した背側の皮膚に4カ所の全層切除創傷(4mmの生検パンチ;Kai Industries社製)または1cmの全層切開創傷2カ所を無菌で作製した(図2A)。創傷形成直後の創傷腔内に、ピペットでODN(対照またはOPN AS)を局所適用した[50μl;30%プロニックF−127ゲル(Sigma−Aldrich社製)(付録「材料および方法」を参照されたい)中に1、2、または5μM]。SteREO Lumar.V12顕微鏡(Carl
Zeiss社製)を用いて創傷を記録し、Openlab(商標)4.0.2(Improvision社製)ソフトウェアを用いて面積を計算した。6mm生検パンチにより創傷組織を回収した。
組織は、10%ホルマリンまたは4%パラホルムアルデヒド中で固定し、それぞれ、パラフィンまたはTissue−Tec(登録商標)O.C.T.化合物内に包埋した。切片(6μm)を、H&E、Masson社製Trichrome、Toluidine BlueもしくはPicrosirius Redによる染色、F4/80、またはCD31によるIHC(StreptABComplex/HRP、DAKO社製)、MPOによるIHC(Envision、DAKO社製)、またはF4/80−OPN二重免疫蛍光にかけた。用いられた抗体には、以下が含まれる。
0.1Mカコジル酸ナトリウム緩衝液中に2%のグルタルアルデヒド(gluteraldehyde)および2%のパラホルムアルデヒド中、4℃で一晩、またその後、1%四酸化オスミウム中で1時間にわたり検体を固定し、次いで、処理後、TEM(Philips社製CM100型)にかけた。コラーゲンバンドル密度および線維直径の測定値の詳細は、補足資料において概括されている。
RNAの単離およびRT−PCR解析
TRIzol(Invitrogen社製)を用いて総RNAを抽出し、RNeasy
MinElute Cleanupキット(Quiagen社製)を用いてさらに精製した。RNA(5μg)を逆転写(SuperScriptIII;Invitrogen社製)し、標準的なPCR(HotStarTaq(登録商標);Qiagen社製;補足資料で記載)によりOPNおよびGAPDHの発現について解析した。増幅および転写物量のリアルタイムでの検出には、TaqMan(登録商標)遺伝子発現アッセイ(Applied Biosystems社製)を用いた。
ウェスタンブロット法
タンパク質試料(20μg;T−PER(登録商標)試薬(PIERCE Biotechnology社製)中で回収)は、トリス−グリシンゲル(Invitrogen社製)上で分離し、PVDFへと転写し、標準的なプロトコールによりブロット処理した(抗体の詳細は、上記に記載されている)。タンパク質バンドは化学発光(Roche Diagnostics GmbH社製)により可視化し、バンド強度はImage J 1.34sソフトウェアを用いて計算した。
細胞培養物
スイス3T3細胞、J774.1マクロファージ、およびRBLマスト細胞を、DMEM+ 10%ウシ胎仔血清中で培養した。チオグリコール酸誘導腹膜炎の4時間後において、腹腔洗浄により好中球初代細胞を回収し、マウス骨髄からはマクロファージ初代細胞を誘導し、LPS(500ng/ml)により活性化させた。RPMI 1640+ 10%血清中において、両方の初代細胞型を維持した。標準的な条件下において48時間にわたり細胞を培養(2.5×105細胞/ml)することにより馴化培地(CM)を調製し、次いで、培地を濾過(0.2μm)した。CMまたは以下の成長因子/サイトカイン:OPN、PDGF−BB、EGF、TGFβ1もしくはTGFβ3、IL−6、TNF−α、Gleevec(STI571、イマチニブメシレート;Novartis社製)、ウォルトマニン、またはPDGF−Rβ中和抗体により、8時間またはこれらの半減期にわたりスイス3T3細胞を処理した。
図2Dに示す上皮突端長、上皮ギャップ、および創傷幅は、H&E染色した組織切片上において、Openlab(商標)4.0.2(Improvision社製)ソフトウェアを用いて測定した。
透過電子顕微鏡解析
コラーゲン直径の測定には、無傷組織および創傷組織から治療群当たり少なくとも5カ所ずつ無作為に選択した視野(倍率=28500倍、皮膚表面下の深さ=360μm)を撮影し、Image Jソフトウェアを用いて原線維の周長を測定し、直径を計算した(周長/3.14)。コラーゲン密度の測定には、無傷皮膚および創傷皮膚における8カ所の無作為視野(倍率=3900倍)を撮影し、コラーゲン線維により満たされていない細胞外腔の面積(nm2/μm2)[面積に変換された細胞外腔中の白色画素の数(nm2)/(総視野面積(46μm2)−細胞により占拠されている面積]を計算し、Adobe PhotoShopソフトウェア中の手書きの描線ツールを用いて細胞領域を画定した。
この実施例は、AS ODN候補分子の設計および最適化について記載する。

この実施例は、創傷治癒、線維症、および瘢痕形成に対する、オステオポンチン(「OPN」)発現の下方調節効果を示す。上記の表に記載のアンチセンスオリゴデオキシヌクレオチド(「AS ODN」)または対応するスクランブル配列の対照ODNを、30%プルロニックゲル中に1μMの濃度で、成体マウスの背中における、in vivoの4mmパンチ生検による創傷に適用した。OPNは、創傷形成の6時間後までに創傷肉芽組織内で発現し、3日後にレベルがピークに達し、次いで、7日後までに創傷前のレベルまで低下することが典型的である(図1A)。
ODNで治療した創傷に由来する切片におけるOPNについての免疫組織化学(IHC)染色の低下と符合した。送達されたODNの取込みおよび局在化を可視化するため、本出願者らは、Cy3でタグしたOPN AS ODNを作製したところ、これは、創傷周縁部から1mm内側まで、また、創傷内部において300μmの深さまでの細胞層に広がる、非細胞株特異的な形での取込みを示す(図1D)。

Yellowシグナルが50を下回った場所までの距離を記録する。各動物の3つの画像の最小値を分析する。コネキシンタンパク質のレベルを比較するために、真皮または表皮の6つの単一光学切片画像を、各創傷について異なる切片から撮影する。レーザー出力、ピンホール、ゲイン/オフセットおよび対物レンズの全てのパラメータを、対照および糖尿病群の両方にわたって一定に保つ。コネキシン発現を、Saitongdee Pら(2000年)(同上)に記載の通りに定量する。最小のバックグラウンドノイズを有するギャップジャンクションプラークを検出するために閾値を設定し、全ての画像について一定に保つ。各画像に関してコネキシンプラークの数および大きさを記録し、表皮100mまたは真皮10000m2当たりで表す。このアプローチは、細胞レベルでのタンパク質発現に関する情報を発するため、ウェスタンブロットよりもはるかに正確であることが分かった。ウェスタンブロットは、表皮細胞と真皮細胞を区別することも、創縁に近接していることの影響も検出できない。このアプローチを使用して、創縁(WE)の領域および500μm離れた(AD)領域におけるケラチン生成細胞中のコネキシンレベルを、創傷の1または2日後に定量することができる。創傷後5日目に、追加の、新生表皮の先端(LE)領域も撮像する。H&E染色の画像を、DC300Fデジタルカメラを備えたLeica DMLFS顕微鏡を使用して撮影する。再上皮化率の測定は、Qiu, Cら(2003年)Targeting connexin43 expression accelerates the rate of wound repair. 抗オステオポンチンAS ODN組成物を単独で、あるいは以下の例示的な配列:SRPTEKTIFII(配列番号19)を用いて調製された抗コネキシン43ペプチド製剤の投与に続いて、以下の例示的な配列:GTAATTGCGGCAGGAGGAATTGTTTCTGTC(コネキシン43)(配列番号2)およびGACAGAAACAATTCCTCCTGCCGCATTTAC(センス対照)(配列番号176)を用いて調製された抗コネキシン43ポリヌクレオチド製剤と組み合わせて、糖尿病オスSprague
Dawleyラットにin vivoで逐次投与した後、糖尿病対象における創傷治癒効果を調査する。糖尿病対象における創傷治癒を定量するために、創傷の引張強度を調査し、引張強度の増強は創傷治癒の向上を反映する。
抗オステオポンチン剤もしくはPDGF受容体阻害剤またはこれらの両方(場合によって、抗コネキシン剤と共に)を組み合わせることにより、本発明の特定の製剤を調製する。これらの有効成分は、治療所望量、例えば、約0.01%〜10%の濃度で、ナトリウムカルボキシメチルセルロース;塩化ナトリウム;酢酸ナトリウム三水和物;氷酢酸;メチルパラベン、プロピルパラベン、およびm−クレゾールの1もしくは複数またはすべてなどの防腐剤あるいは防腐剤の組合せ;L−リシンヒドロクロリド(必要または所望の場合のタンパク質安定化剤);および注射用水と共に供給される。ナトリウム含量およびpHは、それぞれ、生理的塩濃度に近似し、(1または複数の)活性薬剤の最大の安定性を確立するように選択する。ポリエチレンライナーでラミネート加工されたアルミニウムチューブ内にゲルを満たし、単回使用産物または複数回使用産物を提供することができる。複数回使用産物を調製する場合には、メチルパラベン、プロピルパラベン、およびm−クレゾールの1もしくは複数またはすべてなどの防腐剤あるいは防腐剤の組合せを用いる。
