Description
[0001] 本出願は、2010年9月10日出願の発明の名称「正に帯電した神経血管内皮化デバイス」の米国仮特許出願第61/381,770号の優先権及び利益を主張し、その開示の全体が参照により本明細書に組み込まれる。
[0103] 本発明の種々の実施形態を上述したが、それらは単に例として示したものにすぎず、限定的でないものと理解すべきである。上述した方法及び工程が特定の順序で起こる特定の事象を示す場合、本開示の利益を有する当業者は、特定の工程の順序は変更してもよく、そのような変更は本発明の変形形態に従うものであることを理解するだろう。さらに、特定の工程は、可能である場合は並列工程で同時に実施してもよく、上述のように順次実施してもよい。例えば、拡張可能な挿入物は拡張可能なカテーテルを動脈瘤の近傍に配置するのと同時にカテーテル内に挿入され得る。
Claims (39)
- 挿入部と、
動脈瘤内に配置されるように構成されて前記挿入部に結合された拡張可能な挿入物であって、当該拡張可能な挿入物は、第1部分と第2部分とを有しており、前記第2部分が前記第1部分に結合されており、当該拡張可能な挿入物は、前記第1部分と前記第2部分とが実質的に直線的に整列している第1構成と、前記第2部分が前記第1部分に少なくとも部分的に重なっている第2構成と、の間において移行可能である、拡張可能な挿入物と、を備える装置。 - 前記第1部分及び前記第2部分がそれぞれ多孔性メッシュを含み、前記多孔性メッシュが、前記第1構成にある場合に第1多孔率を有し、前記第2構成にある場合に第2多孔率を有する、請求項1に記載の装置。
- 前記拡張可能な挿入物が、前記第1構成において、血管を通じた挿入のために構成されており、前記第2構成において、動脈瘤の嚢を占有するように構成されている、請求項1に記載の装置。
- 前記挿入部が前記拡張可能な挿入物に脱着可能に結合されている、請求項1に記載の装置。
- 前記第1部分及び前記第2部分が、前記第2構成において付勢されるように構成されている、請求項1に記載の装置。
- 前記第1部分が多孔性メッシュの第1層と多孔性メッシュの第2層とを含み、前記第1層及び前記第2層が同じ多孔率又は異なる多孔率のいずれか一方を有する、請求項1に記載の装置。

- 前記拡張可能な挿入物が、前記第1構成において第1幅を有し、前記第2構成において第2幅を有し、前記第2幅が前記第1幅よりも大きい、請求項1に記載の装置。

- 前記第1部分及び前記第2部分がそれぞれ多孔性メッシュを含み、前記拡張可能な挿入物が前記第2構成にある場合、前記第1部分の前記多孔性メッシュが第1幅を有する多次元形状を有し、前記拡張可能な挿入物が前記第2構成にある場合、前記第2部分の前記多孔性メッシュが、前記第1幅よりも小さい第2幅を有する多次元形状を有する、請求項1に記載の装置。
- 前記第1部分及び前記第2部分がそれぞれ多孔性メッシュを含み、前記第1部分及び前記第2部分の少なくとも一方の前記多孔性メッシュが、複数の編組ワイヤを含み、前記編組ワイヤが、正に帯電した金属を含む、請求項1に記載の装置。
- 前記第1部分及び前記第2部分がそれぞれ、ニチノール、白金、タンタル、延伸充填管の少なくとも1つ又はそれらの組み合わせでできている多孔性メッシュを含む、請求項1に記載の装置。
- 前記第1部分及び前記第2部分の少なくとも1つが、前記第1部分又は前記第2部分の前記少なくとも一方に結合された放射線不透過性マーカを含む、請求項1に記載の装置。
- 前記第1部分及び前記第2部分の少なくとも一方が多孔性メッシュを含み、前記拡張可能な挿入物が前記第2構成にある場合、前記多孔性メッシュの少なくとも一部が前記動脈瘤の頚部上に配置されるように構成されており、前記多孔性メッシュの前記一部が前記動脈瘤の前記頚部における内皮細胞の付着用の足場となっている、請求項1に記載の装置。
- 前記第1部分が多孔性メッシュを含み、前記第1部分が、その第1構成において第1容積を占有するように、及び、その第2構成において前記第1容積よりも大きい第2容積を占有するように構成されており、
前記第2部分が多孔性メッシュを含み、前記第2部分が、前記多孔性メッシュの第1セグメントが前記多孔性メッシュの第2セグメントと重なるように、その第2構成において3次元構成に湾曲するように構成されており、その第2構成にある前記第2部分が、その第2構成にある前記第1部分を受容するように構成された内部領域を規定している、請求項1に記載の装置。 - 前記拡張可能な挿入物が前記動脈瘤内に配置されると、前記拡張可能な挿入物の前記第1部分及び前記第2部分の少なくとも1つの少なくとも一部が前記動脈瘤の頚部上に配置され、前記動脈瘤の前記頚部上に配置された前記部分が血管から前記動脈瘤への体液の流れを遮断するように構成されている、請求項1に記載の装置。
- 第1端部及び第2端部を有する第1多孔性部材であって、前記第1多孔性部材が血管を通じて挿入するための折畳構成を有し、前記第1多孔性部材は、実質的に細長く、かつ、その折畳構成において第1幅を有し、前記第1多孔性部材は、動脈瘤の嚢を占有するための拡張構成を有し、前記第1多孔性部材はその拡張構成において第2幅を有し、前記第2幅が前記第1幅よりも大きい、第1多孔性部材と、
第1端部及び第2端部を有する第2多孔性部材であって、前記第2多孔性部材が前記第1多孔性部材に結合されており、前記第2多孔性部材は、前記血管を通じて挿入するための折畳構成を有し、前記第2多孔性部材が実質的に細長く、かつ、その折畳構成において第3幅を有し、前記第2多孔性部材は、前記動脈瘤の前記嚢を占有するための拡張構成を有し、前記第2多孔性部材はその拡張構成において第4幅を有し、前記第4幅が前記第3幅よりも大きい、第2多孔性部材と、を備える装置。 - 前記折畳構成において、前記第1多孔性部材の前記第1端部が前記第1多孔性部材の前記第2端部から第1距離だけ離間しており、前記拡張構成において、前記第1多孔性部材の前記第1端部が前記第1多孔性部材の前記第2端部から前記第1距離より小さい第2距離だけ離間している、請求項19に記載の装置。
- 前記第1多孔性部材及び前記第2多孔性部材の少なくとも一方が、複数の正に帯電した金属フィラメントのウィーブ又はブレードを含む、請求項19に記載の装置。
- 前記第1多孔性部材及び前記第2多孔性部材の少なくとも一方がその拡張構成に向かって付勢されている、請求項19に記載の装置。

拡張可能な挿入物を前記カテーテル内部の第1位置から前記カテーテル外部の第2位置に移動させる工程であって、前記拡張可能な挿入物が第1部分と第2部分とを含み、前記第1部分及び前記第2部分のそれぞれが、前記第1位置にある場合は第1構成、前記第2位置にある場合は第2構成を有し、前記第2構成において前記第1部分が前記第2部分に実質的に重なっている、工程と、を含む方法。 - 前記拡張可能な挿入物の近位端から挿入部を取り外す工程と、
前記カテーテルを通じて前記挿入部を除去する工程と、をさらに含む、請求項28に記載の方法。 - 前記拡張可能な挿入物を移動する前記工程の前に、前記拡張可能な挿入物を前記カテーテル内に挿入する工程であって、前記挿入する工程の前に前記拡張可能な挿入物をその第2構成に付勢する工程をさらに含む、請求項28に記載の方法。
