序
癌患者の生存期間は、癌を初期に治療すると非常に増大する。膀胱癌の場合には、早期疾患と診断された患者は、5年生存率が、増悪した疾患と診断された患者がおよそ15〜30%であるのに比較して、>90%である。したがって、膀胱癌の早期診断に結びつく進歩は、患者の予後の改善をもたらすことができる。尿試料を用いて膀胱癌を検出するための確立された方法は、細胞診である。しかし、細胞診は、浸潤性膀胱癌の検出については感度がわずか約75%であり、また表在性膀胱癌の検出では感度がわずか約25%であることが知られている(Lotan and Roehrborn, Urology 61,109-118 (2003))。

膀胱を含む腫瘍の検出および評価のためのマーカーを提供する。多数の遺伝子およびタンパク質が、膀胱腫瘍に関連していることが発見された。膀胱腫瘍を有する患者からおよび正常な尿路上皮の非悪性試料から採取した試料のマイクロアレイ分析により、多くの膀胱腫瘍において、ある遺伝子の過剰発現または遺伝子産物の尿中の蓄積の特異的パターンが、疾病に関連しているという驚くべき発見がもたらされた。最も驚くべきことに、膀胱癌の患者からの尿試料中には高いレベルで存在するが、健康な個人、および特に、血尿症を示す個人を含む非悪性泌尿器疾患を有する個人では低レベルで存在するマーカーが、単離されたのである。マーカー、例えば遺伝子産物(例えば、mRNAなどのオリゴヌクレオチド)ならびにオリゴヌクレオチドから翻訳されたタンパク質およびペプチドの検出は、したがって腫瘍、特に膀胱腫瘍の存在を示唆する。
次の手法は、BTMまたはUBTMファミリーメンバーを用いて、膀胱癌を含む癌を検出するために使用することができる方法である。しかしそれらに限定されない。
これらの方法は、核酸プローブを支持体へ結合させる工程、および適切な条件下で検査試料に由来するRNAまたはcDNAとハイブリダイズさせる工程を含む(Sambrook, J., E Fritsch, E. and T Maniatis, Molecular Cloning: A Laboratory Manual 3rd. ColdSpring HarborLaboratory Press: Cold Spring Harbor(2001))。これらの方法を、腫瘍組織または液体試料に由来するBTMまたはUBTMに適切に適用することができる。RNAまたはcDNA調製物を、検出および定量化を可能にするために、通常は蛍光性または放射性分子でラベルする。いくつかの応用では、信号強度を増強するために、ハイブリダイズさせるDNAに、分岐した蛍光ラベル構造を有するタグを付することができる(Nolte, F.S., Branched DNA signal amplification for direct quantitation of nucleic acid sequences in clinical specimens. Chem. 33, 201-35 (1998))。ハイブリダイズしていないラベルを、0.1×SSC、0.5%SDSなどの薄い塩溶液中での十分な洗浄により除いた後、ゲル像の蛍光検出またはデンシトメトリーによって、ハイブリダイゼーションの量を定量化する。支持体は、ナイロンまたはニトロセルロース膜などの固体であり、あるいは液体懸濁液中でハイブリダイズするミクロスフェアまたはビーズから成ることができる。洗浄および精製が可能なように、ビーズは磁性体であってよく(Haukanes, B-I and Kvam, C, Application of magnetic beads in bioassays. Bio/Technology 11, 60-63 (1993))、または、フローサイトメトリーが可能なように蛍光ラベルしてもよい (例えば: Spiro, A., Lowe, M. and Brown, D., A Bead-Based Method for Multiplexed Identification and Quantitation of DNA Sequences Using Flow Cytometry. Appl. Env. Micro. 66, 4258- 4265 (2000) 参照)。
定量的PCR(qPCR)を、腫瘍試料に、血清、血漿、および尿の試料に、BTM特異的なプライマーおよびプローブを用いて実施することができる。制御された反応において、PCR反応で形成された生成物の量(Sambrook, J., E Fritsch, E. and T Maniatis, Molecular Cloning: A Laboratory Manual 3rd. Cold Spring Harbor Laboratory Press: Cold Spring Harbor (2001))は、開始時点の鋳型の量と相関関係にある。PCR生成物の定量化を、PCR反応を、それが対数期にある時に、試薬が律速になる前に止めることにより、行なうことができる。その後、PCR生成物をアガロースまたはポリアクリルアミドゲル中で電気泳動し、エチジウムブロマイドまたはそれに匹敵するDNA染色で染色し、染色強度をデンシトメトリーにより測定する。あるいはPCR反応の進行を、リアルタイムで生成物蓄積を測定するApplied BiosystemsのPrism7000またはRochのLightCyclerのようなPCR機器を用いて、測定することができる。リアルタイムPCRは、合成されたPCR生成物中へ取込まれたSybr GreenなどのDNA陥入性色素の蛍光、または消光剤分子から切断された時にレポーター分子によって放出される蛍光、のどちらかを測定する;レポーター分子および消光剤分子は、オリゴヌクレオチドプローブに組み入れられ、オリゴヌクレオチドプローブは、プライマーオリゴヌクレオチドからDNA鎖が伸長した後に、標的DNA分子にハイブリダイズする。オリゴヌクレオチドプローブは、次のPCRサイクルで、Taqポリメラーゼの酵素作用により追い出され、分解されて、消光剤分子からレポーター分子を放出する。
簡潔に述べれば、サンドイッチELIZA分析では、BTM/UBTMに対するポリクローナルまたはモノクローナル抗体を、固体支持体(Crowther, J.R. The ELISA guidebook. Humana Press: New Jersey (2000); Harlow, E. and Lane, D., Using antibodies: a laboratory manual. Cold Spring Harbor Laboratory Press: Cold Spring Harbor (1999))、または懸濁液のビーズに結合する。他の方法は当技術分野において公知であり、本明細書でさらに記述する必要はない。モノクローナル抗体は、ハイブリドーマから導出することができ、またはファージ抗体ライブラリー(Hust M. and Dubel S., Phage display vectors for the in vitro generation of human antibody fragments. Methods MoI Biol. 295:71- 96 (2005))から選択することができる。非特異的結合部位を、非標的タンパク質調製物および界面活性剤でブロックする。その後、捕獲抗体を、BTM/UBTM抗原を含む尿または組織調製物とインキュベーションする。その混合物を洗浄した後、抗体/抗原複合体を、標的とするBTM/UBTMを検出する第2の抗体とインキュベーションする。第2の抗体を、通常は蛍光性分子に、または、酵素反応でまたはレポーターに結合した第3の抗体(Crowther,Id.)で検出することができる他のレポーター分子に、結合させる。あるいは、直接ELISAでは、BTM/UBTMを含む調製物を支持体またはビーズに結合させ、標的とする抗原を、抗体−レポーター共役体で直接検出できる(Crowther, 前出)。
腫瘍マーカーの同定および位置確認を、抗マーカー抗体を用いて、膀胱腫瘍、リンパ節、または遠隔転移について行なうことができる。そのような方法を、例えば結腸直腸、膵臓、卵巣、メラノーマ、肝臓、食道、胃、子宮内膜、および脳、を検出するために用いることができる。
一次データを集め、倍率変化解析を、膀胱腫瘍遺伝子発現のレベルを同じ遺伝子の非腫瘍組織中の発現と比較することにより、行なう。発現が増加していると結論付けるための閾値を提供する(例えば1.5倍増加、2倍増加、および他の実施態様では、3倍増加、4倍増加または5倍増加)。本発明の範囲から外れずに、増加した発現が生じたと結論付けるための他の閾値を選択することができることは当然である。腫瘍遺伝子発現のさらなる解析には、増大した発現を示す遺伝子を、腫瘍の診断を提供するために、既知の膀胱腫瘍の発現プロフィールと照合する工程が含まれる。
TCCの迅速な診断および早期検出に加えて、組織、血清、または尿のいずれかで検出されるBTMおよびUBTMマーカーを、患者の治療に対する応答をモニターするために用いることができる。これらの応用では、全身的、膀胱内または血管内化学療法、放射線療法、または免疫療法の開始に続いて、時々尿および/または血清試料を採取すればよい。マーカー蓄積の低下は腫瘍サイズの減少を示すことができ、それは有効な治療を示唆する。低下率を、各患者または治療に対する最適の治療用量を予測するために用いることができる。
したがって、ある局面では、本発明には、尿中のUBTMファミリーメンバーの蓄積を検出する工程を含む、膀胱癌を検出するための方法が含まれる。
腫瘍の収集
膀胱腫瘍試料および非悪性尿路上皮試料を、日本の京都大学病院、および他の共同研究の日本の病院で切除された外科的試験片から収集した。
非悪性の対照および膀胱癌患者からの尿試料を、日本の京都大学病院から得た(図1)。健康な対照試料を、白人および日本人のボランティアから得た。
腫瘍組織をTriReagent:水(3:1)混合液中でホモジナイズし、次にクロロホルムで抽出した。全RNAを次に、水相から、RNeasy(商標)法(Qiagen)を用いて精製した。RNAをまた、16の癌細胞系統から抽出し、参照RNAとして役立てるためにプールした。
エポキシ樹脂コーティングされたスライドグラス(MWG Biotech)に、〜30,000の50量体オリゴヌクレオチド(MWG Biotech)を、Gene Machinesのマイクロアレイ作製ロボットを用い、メーカーのプロトコルに従って、プリントした。
cDNAを、5μgの全RNAから、Superscript II(商標)逆転写酵素(Invitrogen)を用いて、5−(3−アミノアリル)−2’デオキシウリジン−5’三リン酸を含む反応で、転写した。その後、反応物をMicroconカラムで脱イオン化し、次いでCy3またはCy5と、炭酸水素緩衝液中1時間室温でインキュベーションした。取込まれなかった色素を、Qiaquickカラム(Qiagen)を用いて除去し、試料をSpeedVac中で15μlに濃縮した。Cy3およびCy5ラベルされたcDNAを、次に、Ambion ULTRAhyb(商標)緩衝液と混合し、100℃2分間で変性させ、ハイブリダイゼーション室中42℃16時間で、マイクロアレイ・スライドにハイブリダイズさせた。スライドを、その後洗浄し、Axon 4000A(商標)スキャナー中で2回、2つの出力設定で走査した。
53の膀胱腫瘍および20の非悪性(「正常な」)膀胱組織試料からのRNAをCy5でラベルし、Cy3ラベルされた参照RNAとのハイブリダイゼーションを、2重または3重に行った。次に規格化の後、29,718の各遺伝子中の発現の変化を、倍率変化および統計的確率により推定した。
Genepix(商標)ソフトウェアにより検出された蛍光強度の中央値を、局所的なバックグラウンド強度を差し引くことにより補正した。ゼロより低いバックグラウンド補正強度を有するスポットを除外した。規格化を容易にするために、強度比および全スポット強度を対数変換した。対数化した強度比を、LOCFIT(商標)パッケージに含まれている局所回帰を用いて、色素および空間の偏りを補正した。対数化した強度比を、全スポット強度および局所化について同時に回帰推定した。局所回帰の残差が、補正された対数化倍率変化を与えた。品質制御のために、各規格化マイクロアレイの比を、スポット強度および局所化に関してプロットした。続いて、何らかの残存アーティファクトの有無についてプロットを目視検査した。さらに、ピンチップバイアス検出のためにANOVAモデルを適用した。規格化のすべての結果およびパラメーターを、統計解析のためPostgresデータベースに入れた。
測定された倍率変化の、アレイ間の比較を改良するために、log2(比率)を、1アレイ当たり同一の全標準偏差を持つように拡大・縮小した。この標準化により、平均組織内クラス変動が減少した。結果の目視検査を容易にするために、log2(比率)をさらに、各オリゴヌクレオチドがゼロの中央値を持つように移動させた。雑音耐性を改善するために、倍率変化に基づく順位和検定を次に用いた。この検定は次の2工程から成る:i)アレイ内の倍率変化順位(Rfc)の計算、およびii)腫瘍組織の中央値(Rfc)からの正常組織の中央値(Rfc)の減算。両方の中央値順位の差が、倍率変化順位の評点を定義する。標準化データについて3つの追加の統計的検定を行なった:それは1)Studentの2標本t−検定、2)Wilcoxon検定、および3)マイクロアレイの統計解析(SAM)である。統計的方法(倍率変化順位、t−検定、Wilcoxon検定、およびSAM)の各々によって決定された、300個の最も有意に上方調節された遺伝子に、各検査について順位評点が与えられた。ある遺伝子が1つのリストに現われ、他の1つ以上には現われなかった場合、その評点に加重因子500を加えた。次に、すべての順位評点を加え合わせて、1つの合計順位評点とした。
腫瘍と非悪性試料を区別するために、2マーカーまたは3マーカーの組合せを用いることの価値を決定するために、腫瘍および非悪性試料からのqPCRデータを次の解析に付した。非悪性および腫瘍試料についての正規分布を、標本平均および標準偏差を用いて、生成した。次に、腫瘍発現データから得られた値が、非悪性の分布中で定義された閾値(例えば50%、70%、75%、80%、90%、95%、または99%以上)を越える確率(すなわち感度)を決定した。マーカーの組合せについては、少なくとも1つのマーカーが閾値を越える確率を決定した。
いくつかの実施態様では、尿試料についてBTMの分析を望ましく実施することができる。一般に、これらの液体中のオリゴヌクレオチド、タンパク質、およびペプチドを分析するための方法は、当技術分野において公知である。しかし、説明のために述べると、BTMの尿中レベルを、サンドイッチ型酵素結合免疫吸着分析(ELISA)を用いて、定量することができる。血漿または血清分析のために、5μlに小分けした、適切に希釈した試料または連続的に希釈した標準BTM、および75μlのペルオキシダーゼ結合抗ヒトBTM抗体を、マイクロタイタープレートのウエルに加える。30℃30分のインキュベーション期間後、リン酸緩衝食塩水(PBS)中の0.05%Tween20でウエルを洗浄し、結合していない抗体を除去する。次に、BTMと抗BTM抗体の結合複合体を、H2O2を含むo−フェニレンジアミンと15分間30℃でインキュベーションする。その反応を1M H2SO4を加えることにより止めて、マイクロタイタープレート読取機により、492nmの吸光度を測定する。抗BTM抗体はモノクローナル抗体またはポリクローナル抗血清でよいことは当然である。
リアルタイムまたは定量的PCR(qPCR)を、PCR鋳型のコピー回数の絶対的または相対的な定量化に用いる。Taqman(商標)プローブおよびプライマーセットを、Primer Express V 2.0(商標)(Applied Biosystems)を用いて設計した。可能な場合には、その結果得られるアンプリコンに、可能性のあるすべてのスプライシング変異体が含まれ、MWG-Biotech由来のマイクロアレイオリゴヌクレオチドによってカバーされる領域へアンプリコン優位が与えられていた。プライマーおよびプローブの配列を図2に示す。あるいは、標的遺伝子が、所望のアンプリコンをカバーするAssay−on−Demand(商標)expression assay(Applied Biosystems)によって表示されていた場合は、これらを用いた。研究組織内で設計された分析では、プライマー濃度を、SYBRグリーンラベルプロトコルおよび参照RNAから作られたcDNAを用いて滴定した。増幅を、ABI Prism(商標)7000配列検出システムで、標準のサイクル条件下で行った。単一回増幅の生成物を解離曲線中で観察したときは、最適なプライマー濃度および5’FAM〜3’TAMRAリン酸Taqman(商標)プローブ(Proligo)を最終濃度250nMで用いて、625倍にわたる濃度範囲で標準曲線を生成した。0.98を超える回帰係数で標準曲線を与える分析を、次の分析で用いた。
推定血清マーカーを、(i)コードされたタンパク質が細胞から分泌されるか、または細胞膜から剥ぎ取られる可能性、(分泌の可能性はTargetP(商標)(Emanuelsson et al; J. MoI. Biol. 300, 1005-1006 (2000) による分析に基づいていた)および(ii)その合計順位評点、に基づいて、アレイデータから選択することができる。しかしながら、腫瘍試料中の過剰発現の程度の変化は、腫瘍の異質性だけでなく、腫瘍試料への、平滑筋、結合組織、粘膜下細胞(U.S Patent 6,335,170 参照)、間質細胞、および非悪性上皮を含む「正常」組織の混入の程度の変動も反映する。多くの状況で、「正常」の混入は5から70%までの範囲であり、中央値はおよそ25%であった。
本明細書に記述する実施例は、本発明の実施態様を説明する目的のためであって、本発明の範囲を制限することを意図しない。他の実施態様、方法、および分析の型は、分子診断技術における当業者の範囲内であり、本明細書で詳細に記述する必要はない。本明細書の教示に基づく技術の範囲内の他の実施態様は、本発明の一部であると考える。
膀胱の表在性および浸潤性悪性腫瘍のためのマーカーの同定
浸潤性および表在性膀胱癌の遺伝子発現パターンからのマイクロアレイデータの階層的クラスター化が、多数の有意な差異を示した。その結果、これらの癌型を、次の解析において別々に処理した。しかし、両方の癌型において、高い割合の遺伝子が過剰発現される。図3は、浸潤性膀胱悪性腫瘍のマーカーに対するマイクロアレイ研究の結果を示す表である。199の浸潤性マーカーのうちの31が、上述の血清マーカー(図中「S」で表示する)の基準を満たす。図4は、表在性膀胱悪性腫瘍のマイクロアレイ研究の結果を示す表である。170の表在性マーカーのうちの34が、上記の血清マーカーの基準を満たす。図3および4には、遺伝子のHUGO記号(「記号」)、MWG Biotechオリゴヌクレオチド番号、NCBImRNA参照配列番号、タンパク質参照配列番号、腫瘍遺伝子発現と非悪性遺伝子発現との間の倍率変化平均値、個々の腫瘍試料中の発現と非悪性試料中の発現の中央値との間の倍率変化最大値、元の未調整のStudentのt-検定結果、2試料Wilcoxon検定結果、および合計の順位評点、が含まれる。
qPCR分析
より敏感でより正確な遺伝子発現の定量化が、qPCRを用いて、図3および4に示される遺伝子の部分集合について得られた。マイクロアレイ分析(図3および4)によって同定された18の遺伝子に対して、30に及ぶ浸潤性膀胱腫瘍、25の表在性膀胱腫瘍、および正常な尿路上皮の18の試料、からのメッセンジャーRNAを分析し、結果を図5に示した。浸潤性および表在性の型の膀胱癌に対するデータを、マーカーSPAG5、TOP2a、CDC2、ENG、NRPl、EGFL6、SEM2、CHGA、UBE2C、HOXA13、MDK、THYl、BIRC5、およびSMC4L1について示す。表在性の型単独では、マーカーSEMA3F、IGFBP5およびNOVのみが、正常な尿路上皮と比較して過剰発現され、浸潤性の型単独ではMGPのみが過剰発現され;これらのマーカーは、過剰発現されなかった腫瘍試料中では、正常な尿路上皮と同様の発現を維持した。図5には、遺伝子名、遺伝子別名、遺伝子記号、腫瘍(T)と非悪性(N)組織間の倍率変化中央値、個々の腫瘍試料の発現と非悪性組織発現中央値の間の倍率変化最大値、および非悪性試料中の発現レベルの第95百分位数より大きな発現レベルを有する腫瘍試料の%、が含まれる。
本明細書で分析し、開示した2つのシリーズの試料に基づいて、膀胱癌の検出のための感度は、95%を超える。非悪性疾病を有する患者からの試料を含んでいたシリーズ2での特異性もまた、95%を超える。
膀胱癌の検出における複数マーカーの使用
図12a〜12bは、個々の腫瘍試料において、正常な試料と比較して有意に増大した発現(「過剰発現」)を示す遺伝子の数のヒストグラムを表す。ヒストグラムは、図5に示した最初の12のマーカーから得られたqPCRデータに基づいていた。PCR分析での30の浸潤性腫瘍のうちの27(90%)が、少なくとも4つの遺伝子を、第95百分位数を超える過剰発現させた(図12a)。分析での25の表在性腫瘍のうち23(92%)が、少なくとも4つの遺伝子を、第95百分位数を越える過剰発現させた(図12b)。これらの発見は、正常組織に比べて複数の遺伝子が過剰発現される状況下では、癌検出の信頼性が非常に高くなることができ、それにより癌の診断がより確かになることを示す。しかし、ある場合には、単一のマーカー遺伝子発現の上昇によって十分に癌の診断ができる。
表在性および浸潤性膀胱癌の患者における異なる転写物蓄積
図5から、SEMA3F、HOXA13、TOP2A、およびSPAG5を含むいくつかのBTMが、浸潤性膀胱癌と表在性膀胱癌との間で異なる発現を示すことがわかる。この観察を拡張するために、浸潤性および表在性膀胱癌の患者からの尿中のこれらの転写物の蓄積を比較した。
膀胱腫瘍マーカーに対する抗体
追加の局面で、本発明には、BTMに対する抗体の作製が含まれる。本明細書に記載した方法を利用して、新規なBTMを、マイクロアレイおよび/またはqPCR方法を用いて同定することができる。一旦推定マーカーを同定した場合は、それを、免疫反応を誘起するのに十分な量だけ生成することができる。ある場合には、全長のBTMを用いることができ、また他の場合では、BTMのペプチド断片が免疫原として十分である可能性もある。免疫原を適当な宿主(例えばマウス、ウサギなど)に注入することができ、またもし望ましい場合は、フロインド完全アジュバント、フロイント不完全アジュバントなどのアジュバントを、免疫応答を増加させるために注入することができる。抗体を作成する工程は、免疫学の技術分野においては日常業務であり、本明細書にさらに記述する必要はないのは当然である。その結果、同定されたBTMまたはUBTMに対する抗体を、本明細書に記述した方法を用いて生成することができる。
キット
本発明の発見に基づいて、いくつかの型の検査キットを構想し、生産することができる。第1に、検出分子(または「捕獲試薬」)を前もって搭載した検出装置を有するキットを作成することができる。BTM mRNA検出のための実施態様では、そのような装置は、基質(例えばガラス、シリコン、石英、金属など)、を含むことができ、その上に、検出するべきmRNAとハイブリダイズする捕獲試薬としてのオリゴヌクレオチドが、結合している。いくつかの実施態様では、mRNA(cy3、cy5、放射性ラベル、または他のラベルでラベルされた)を、基質上のオリゴヌクレオチドにハイブリダイズさせることによって、直接mRNAを検出することができる。他の実施態様では、最初に所望のmRNAに相補的なDNA(cDNA)を作成することにより、mRNAの検出を遂行することができる。その後、ラベルされたcDNAを基質上のオリゴヌクレオチドにハイブリダイズさせて、検出することができる。
膀胱癌検出のために用いられるBTMの複数の組合せI
一連の実施態様の中で、BTMである、HOXA13、MGP、SEMA3F、およびTOP2Aを検査するための試薬を単独でまたは組合せて、膀胱癌を検出するための、未分画の尿または尿細胞沈殿物の検査用キットに組み入れることができる。癌患者および対照中のこれらのBTMの蓄積の範囲を、図20に示す。尿試料を、疾病増悪の、または治療応答のモニタリングを必要とする、膀胱癌であると診断された患者、巨視的または微視的血尿症を含む泌尿器症状を有する個人、あるいは無症候の個人から集めた。未分画の尿中のBTMを測定するキットで検査される患者または個人から、約2mlの尿を検査のために採取すればよい。尿沈殿物の検査のためには>20mlの尿を集めればよい。
膀胱癌の検出に用いられるBTMの複数の組合せII
別の一連の実施態様では、マーカーの組合せ:TOP2A/SEMA3FおよびTOP2A/HOXA13のどちらかまたは両方の、尿中での蓄積を用いることにより、任意の型の尿検査または血液検査を用いて膀胱癌と診断された患者に存在する膀胱癌の組織型を、確実に予測することができる。したがって、膀胱鏡検査および組織学的検査は、膀胱癌の型を診断するためには必要ではない可能性がある。
BTMを用いる膀胱癌の増悪の評価
膀胱腫瘍の増悪を評価するために、膀胱癌を有する患者から時間とともに、膀胱壁の生検によって組織試料を得るか、または尿試料を集める。BTM、UBTM、またはそれらの組合せの蓄積の評価を、異なる時に採取した試料について行う。BTMまたはUBTMの、個々のまたは組合せの増大した蓄積は、膀胱癌の増悪を示唆する。
BTMを用いる膀胱癌の治療の評価
膀胱腫瘍に対する治療の効能を評価するために、治療を開始する前に組織および/または尿の試料を得る。1つ以上のBTMまたはUBTMのベースラインレベルを決定し、また様々なBTMおよびUBTMの互いに対する比率も決定する。治療を開始し、治療は、その疾病の型および病期に適切な、手術、放射線治療、または化学療法を含む任意の公知技術の治療を含むことができる。治療過程の間に、組織および/または尿の試料を集め、BTMおよび/またはUBTMの存在および量について解析する。様々なBTMおよびUBTMの比率を決定し、結果を:(1)治療前の患者のベースラインレベル、または(2)膀胱癌を有しない個人の集団から得られた正常値、と比較する。

- 膀胱癌の存在を検出するための方法であって:
BIRC2、CDC2、HOXA13、IGFBP5、MDK、MGP、NOV、NRPl、SEMA3F、SPAG5、TOP2A、から成る群より選択される1つ以上のマーカーの、尿試料中の存在を測定する工程を含み、前記マーカーは血液中に実質的に存在しない、方法。 - 前記マーカーが、IGFBP5、MGP、SEMA3F、およびHOXA13から成る群より選択される、請求項30記載の方法。
- 悪性膀胱疾患を非悪性膀胱疾患から区別するための方法であって:
HOXA13、IGFBP5、MDK、MGP、NRPl、SEMA3F、SMC4L1、TOP2A、およびUBE2Cから成る群より選択される1つ以上のマーカーの、前記患者の尿中における蓄積を測定する工程;および
前記試料中の前記マーカーの比率を決定する工程であって、その比率は膀胱癌の存在に関連している
を含む方法。 - 少なくとも第2のBTMの尿中における蓄積を測定する工程をさらに含む、請求項30記載の方法、
