Abstract

（背景）
（１．技術分野）
本発明は、成形可能な外科固定具に関し、そしてより特定すると、ステープルを成形するためのアンビルポケットを有する外科ステープルにおいて使用するための、特定方向に偏倚される成形可能ステープルに関する。本発明はまた、外科ステープラーと共に使用するための、アンビルポケットを備えるアンビルアセンブリに関する。
（２．関連技術の背景）
外科ステープリング器具は、生命を守る多くの外科手順のために重要になっている。外科ステープルは、通常、外科ステープリング器具（例えば、胃腸吻合デバイスおよび横行吻合（ｔｒａｎｓｖｅｒｓｅ ａｎａｓｔｏｍｏｓｉｓ）デバイスが挙げられる、吻合デバイスとして公知のもの）を用いて、組織に機械的に挿入される。このようなデバイスにおいて、ステープルは、１つ以上の細長い列として、カートリッジ内へと装填される。ステープラーを押すため、すなわち駆動するための機構が起動されて、組織の２つ以上の部分を通して、ステープルを変形アンビルの方へと駆動する。駆動操作の仕上げにおいて、各ステープルの脚が、アンビルによって、閉じた構成へと好都合にクランプまたは屈曲されて、組織の部分の縫合および連結を完了する。胃腸吻合型のデバイスは、列として整列したステープルを、迅速な手順で連続的に駆動および屈曲させ、一方で横行吻合型のデバイスは、全てのステープルを同時に駆動および屈曲させる。例えば、特許文献１および特許文献２を参照のこと。円形吻合型のデバイスは、ステープルの環状の列を組織に同時に適用する。例えば、特許文献３を参照のこと。
１つの型の従来のステープル２０（図１〜３に示す）は、胃腸吻合型の外科ステープリングデバイスと横行吻合型の外科ステープリングデバイスとの両方と共に使用され、ステンレス鋼またはチタンで作製される。未変形のステープル２０（図１）は、ほぼＵ字型であり、そしてバックスパン２２、およびこのバックスパンから実質的に垂直にたれ下がる２つの脚２４を備える。各脚２４は、身体器官または組織を穿孔するための、鋭利なのみ型の先端部２６を有する。こののみ型の先端はまた、ステープルにトルクを生じさせて、その成形を可能にする。ステープルは、片側から組織を貫通して、この組織の反対側に間隔を空けて位置するアンビルに係合する。このステープルは、脚がアンビル２５と係合することによって、そしてアンビル２５の形状に従って屈曲され、図２に示すようなＢ字型の閉じたステープル２８を成形する。この閉じた構成において、組織は、このステープルの脚とバックスパンとの間で圧縮される。

これらの従来のステープルの、正確かつ一貫した成形を保証するために、かなりの研究および開発が、構造体を成形および駆動する分野において、なされてきた。例えば、特定のコーティングおよび／または構造を有するアンビルが開発された（例えば、特許文献４および特許文献５を参照のこと）。また、ステープルカートリッジは、ステープル成形の間に遭遇する力の釣り合いをとるためのドライバー構造を有して構成された。同一人に譲渡された、Ｇｒｅｅｎに対する特許文献６を参照のこと。従って、ねじれも変形もない、一貫したステープル成形を制御および保証するためには、極度に厳密な製作公差（ｍａｎｕｆａｃｔｕｒｉｎｇ ｔｏｌｅｌａｎｃｅ）が果されてきた。
異なる型の器具のための、他の型のステープルもまた、先行技術において見出される。いくつかは、非円形の断面を有する。図４、４Ａおよび４Ｂは、例として、ＭＵＬＴＩＦＩＲＥ ＥＮＤＯ ＨＥＲＮＩＡおよびＥＮＤＯ ＵＮＩＶＥＲＳＡＬ ６５ステープルと共に使用するための、Ｕｎｉｔｅｄ Ｓｔａｔｅｓ

このステープルは、ステープル成形の間のねじれに適合して、ステープルの脚が、図４Ｅに示されるように交差することを可能にするよう特に構成されている。従って、このステープルは、ステープルを成形するための力が、このステープルを不格好にするかまたなねじるための力よりわずかに大きいように、設計される。この成形は、ステープルの脚を、バックスパン３４の内側表面３２に隣接して位置するアンビルの周囲で屈曲させることによって、達成される。

記載されるような用途のためのこのステープルは、実際に、より大きな力を生じて、所望の形状を生成する。実際に、このステープルの脚は、おそらく交差する前に互いに接触して、その交差した構成になる。
従って、このタイプのヘルニアステープル（すなわち、組織がただ１つの部位から接近される場合、アンビルがバックスパン（ｂａｃｋｓｐａｎ）に隣接しているステープル）は、脚が組織を貫通してアンビルポケットと接触する本発明のステープル（例えば、Ｂ字型ステープル）とは全く異なることが明らかである。これらのアンビルポケットは、このステープルを閉構造に成形するようにステープルの脚を方向付ける。従って、ステープルの構造、ならびにこれらのヘルニアステープルのねじれ、曲げおよびステープル成形の考察は、アンビルポケットで方向付けられたステープル（例えば、Ｂ字型ステープル）についてのこれらの考察に適用できない。

（要旨）

（好ましい実施形態の詳細な説明）
本明細書に開示される特定方向に偏倚されるステープルの好ましい実施形態は、図面と共にここに詳細に記載されており、ここで、参照している数字は、種々の図の各々において同一または対応するエレメントを示す。

Ｉｙが、発射されるときのステープルの適切な成形に対応する寸法を規定し、そしてＩｘが、捻れおよび／または成形異常に対応する寸法を規定する場合、特定方向に偏倚される構造が、従来のステープルと比較した場合に成形異常を捻るための力の２倍までの力を必要とするが、従来の円形ステープルと「機能的に同様な」成形力を提供することは容易に明らかである。この新規なステープルは、従来のステープルよりも実質的な改善を提供する。

従って、新規なステープルの本実施形態は、捻れおよび／または成形異常に対してより耐性である（例えば、従来のステープルが＿１４，０００ｋｓｉであるのに対してこの新規なステープルは＿７，５００ｋｓｉである）だけでなく、このステープルは、最小の発射力も保持する。この特定方向に偏倚されるステープルは、ステープル成形の間、誤整列の効果に対して有効に感受性を低下されるが、同時に最小の発射力を維持する。この指向性で知的な設計は、誤整列または捻れによって生じる成形異常を減少し、そしてアンビルおよびアンビル成形カップ、カートリッジなどに対する非常に感受性の製作公差の必要性を減少させ得る。
新規なステープルの利点はまた、図１１Ａおよび１１Ｂのグラフを参照することによって理解され得る。ステープルが厚手の組織およびステープルカートリッジを介して押し出され、そしてアンビルは、組織が加圧されるときに収縮し得、別のアンビルに対してわずかに移動し得るので、これは、ステープルの脚部点とアンビルとの間の接触点に影響を与える。例えば、アンビルが位置合わせからわずかに外れて移動する場合、このステープルの脚部は、このステープルの均一な成形に影響を与え得る、アンビルの異なる点に接触する。従って、製作公差に起因して、このステープルの点は、正確な最適位置でアンビルに接触し得ない。このようなステープルの成形が、外科的に十分かつ効果的であるが、本出願の新規なステープルは、ステープルの列のより均一な成形を提供し、そしてこのステープルが捻れに対してより耐性である場合、製作公差に適応する。すなわち、この場合、この薄手の寸法が所望の湾曲方向を規定するので、このステープルは、所望される薄手の寸法の方向に湾曲する傾向を有する。製作公差を緩和することによって、製造コストも同様に抑えられる。

細長基部表面５１８は、その横軸に沿って実質的に直線状であり、すなわち、実質的に平面（本明細書中において、平面を含むことが理解される）、そしてその長手方向軸に沿って凹状であるかまたは湾曲している。実質的に直線状の面は、好ましくは、それに対して成形されるステープルの点の形状に対応する。好ましいステープルは実質的に直線状の先端を有するので（図２０〜２２のステープル４００を参照のこと）、このようなステープルに対する好ましい基部表面は、実質的に直線状である。このことは、ステープル先端の平らな表面間で線と線での接触を提供し、そして実質的に直線状の基部表面を提供する。ステープル点の形状に対応する型を有する基部表面を提供することによって、摩擦が低減され、そしてステープル成形の間のステープル先端のかじりが最小化される。基部表面５１８の形状は、異なるステープルのステープル点の形状（これは、円形状、三角形状などであり得る）に合うように、変更され得る。
示される好ましい実施形態において、側壁５２０（これは、ステープル成形カップ５１２および５１４を部分的に規定する）は、チャネリング表面の下部から基部表面５１８へ伸長する場合に角度がある。側壁５２０は、カップ５１２および５１４の外側部分Ｏの外側の広がり（ここで、側壁５２０は、［組織接触表面５０２またはポケットの垂直軸ＶＡに関して（図２９ｆ）］幅広い角度である）から開始し、中心部分Ｃに向って、そして中心部分Ｃを通って、そしてカップ５１２および５１４の内側部分Ｉに向う細長基部表面５１８に沿って、徐々に、次第により垂直となる（または組織接触表面５０２に関して垂直になる）。好ましくは、カップ５１２および５１４の中央部分および内側部分は、実質的に垂直な（本明細書中において垂直を含むと理解される）側壁５２０によって規定され、その結果、実質的に垂直なトラップ５２２は、各ステープル成形カップの少なくとも中央部分および内側部分において成形される。実質的に垂直なトラップは、ステープル成形カップ５１２、５１４の長手軸に沿って、任意の適切な位置で開始し得る（図３０）。第１のピーク力が生じる前または第１のピーク力が生じる場合に、ステープル成形カップの外側部分Ｏでトラップが開始することが好ましい（図３０）が、本発明に従う適切に成形されたステープルはまた、実質的に垂直なトラップがステープル成形カップの中央部分または内側部分で開始する場合に得られ得る。手短に言うと、実質的に垂直なトラップ５２２は、そこにステープルを配列し、そしてステープルを正確に成形するように機能する。実質的に垂直なトラップは、例えば、特定のステープルおよびステープル成形カップの寸法および構成、ならびに完成したステープルの所望の構成に依存して、任意の適切な長さであり得る。実質的に垂直なトラップ５２２の長さは、好ましくは、０．５ｒと約２ｒとの間であり、ここで、ｒは、各ポケットの曲率半径であり、そしてより好ましくは、垂直なトラップの長さは、約ｒである。好ましい半径ｒは、約０．０３０インチ〜約０．１００インチであり、より好ましくは、約０．０５０インチであり、本明細書中でこれは０．０５４インチを含むと理解される。
図３０を参照すると、ステープルがアンビルに対して成形される場合、ステープルに適用される力は、ステープルがステープルポケットに移動するにつれて、力がステープルをバックルするかまたは可塑的に変形するに十分となるまで、増加する。ステープルに適用されるピーク力は、文字「Ｘ」によって図３０に示されるグラフ中に概略的に例示され、そしてステープルの脚の先端部がカップ５１２および５１４の外側部分中の基部表面５１８と係合する場合に、代表的に生じ、ここで、脚は、可塑的に変形し始める。第１のピーク力は、代表的に、ステープル脚の先端部が基部表面５１８に当る場合に生じ、そしておよそ図１０Ｂおよび１０Ｃに示される位置間で移動する場合に生じる。図３０に示されるグラフ中の文字「Ｙ」によって同定される第２のピーク力は、ステープル脚を上方に曲げるためにステープルに適用される。第２のピーク力Ｙは、代表的に、ステープルの脚の部分が、およそ図１０Ｄおよび１０Ｅに示される位置間のカップ５１２および５１４の基部表面５１８と係合して（また外側部分Ｏにおいて）位置する場合に、生じる。アンビルアセンブリ５００のステープルポケット５１０は、好ましくは、徐々に流し込み、そして少なくともピーク力ＸおよびＹに達する時間によって、実質的に垂直なトラップに成形されるステープル先端部およびステープルのレッグの動きを指示するトラフとして構成される。実質的に垂直なトラップは、ステープルの先端部およびレッグを、トラップ内およびトラップに沿って捕捉する（好ましくは、ステープル成形のピーク負荷の間を含む）。本明細書中において、ステープルの先端部およびレッグを捕捉することは、ステープル成形カップの実質的に垂直なトラップの側壁の少なくとも一部（好ましくは基部部分）が、ステープルの先端部および脚の側方移動または横行移動を最小化するスリップばめ関係で、ステープルの先端部および脚を密接に制限し、そしてステープル成形カップの実質的に垂直なトラップ部分を介してステープルを積極的に指示することを意味する。そうすることによって、ステープルの誤配列またはねじれによる成形異常は、最小化または減少される。
図２９ｂ〜２９ｆは、基部表面５１８を有する側壁５２０が、カップ５１２および５１４の外側部分Ｏから、それぞれカップの外側部分Ｏ、中央部分Ｃおよび内側部分Ｉ中の実質的に垂直なトラップへ、ステープルの先端部および脚を徐々に流し込み、カップ５１２および５１４の頂端５１５においてかまたはその近くで終結するトラフを形成することを示す。
図２９ｆは、ステープル成形カップ５１４の外側部分Ｏが、アンビルポケット５１０に放出される場合にステープル脚の先端部を受けるための大きい標的領域を提供するために、組織形成表面５０２または垂直軸ＶＡに関して、広い角度をなす。
図２９ｅ（これはまた、成形カップ５１４の外側部分Ｏを通って取られる）は、側壁５２０が、成形カップ５１４に沿ってステープル先端部および脚をより密接に誘導するための垂直軸ＶＡに関して、より鋭角であることを示す。
図２９ｄは、内部部分Ｉに沿った側壁５２０が、約８°の角度であるが、垂直軸ＶＡに関して実質的に垂直であることを示す。図２９ｃおよび２９ｂにおいて、側壁は、垂直として示される。理想的に、そして最も好ましくは、実質的に垂直なトラップの側壁５２０の主要部分は、実際に垂直である。垂直な側壁を得る試みにおいて、側壁が実際に垂直であるかまたは実質的に垂直であるかは、アンビルポケットがどのように成形されるかに依存し得ることが理解される。好ましくは、経済的理由および製造の容易さのために、アンビルは、金属の薄いシートから成形され、そしてポケットは、そこに押される。押すことによるカップ成形の間にいくらかの跳ね返り（すなわち、弾性の変形）が存在するので、カップの側壁は、いくつかの場合において、実際に、実質的に垂直である。アンビルが、鋳造されるかまたは機械加工される場合、側壁は、代表的に、確かに垂直である。従って、本発明に従って、ステープル先端部および脚を、細長い実質的に垂直なトラップに流し込みそして誘導するトラフを提供することが目的であり、このトラップは、ステープルが成形される場合に頂端への経路でかまたは頂端の近くへの経路で、実質的に垂直なトラップ内または実質的に垂直であるトラップに沿って、ステープル先端部および脚をトラップまたは捕捉しそして積極的にこれらに指示する。実質的に垂直なトラップへ導くステープル成形カップの側壁は、角度があることが好ましいが、このようなことは、必須ではない。十分な高さの下方部分（例えば、実質的に垂直なトラップの側壁の「Ｌ」）が、本発明に従ってステープルを捕捉するために実質的に垂直である限り、実質的に垂直なトラップに沿ったこのような側壁および／または側壁の上方部分は、弓型であり得る（５２０’、図２９ｇ）かまたは他の形状であり得る。「十分な高さの側壁の下方部分」は、特定のステープルが成形される厚さまたは直径の少なくとも約１／２であることを意味すると考えられる。実質的に垂直な側壁は、垂直軸に対して約２０°未満であり、好ましくは、約１５°未満およびより好ましくは、約１０°未満であると考えられる。
垂直トラップの開始、長さ、構成、および端部のパラメーター、ならびに側壁の実質的に垂直な性質が説明されるように試みられてきたが、これらのパラメーターは、実質的に垂直なトラップ中のステープルの先端部および脚を捕捉する原理が適切なステープル成形を最適化するためにステープルの移動および方向を捕捉しそして積極的に指示するために存在するかまたは使用される限り、種々の要素（例えば、出発ステープル構成およびその寸法および所望の最終形状）に依存して変化し得ることが理解される。
ステープル成形カップにおける実質的に垂直なトラップ（特に、実質的に直線状の基部表面を有するトラップ）の使用は、本明細書中に開示される特定方向に偏倚したステープル（特に、実質的に直線状の先端部を有するステープル）と組み合わせての使用のために特に有利である。この組み合わせは、誤成形されたステープル（ステープルで留められる組織の密度、ステープル形状（ｓｈａｐｅ）、形状（ｇｅｏｍｅｔｒｙ）、または材料、およびステープル先端部の構成（例えば、一様でない角度または丸い構成）におけるバリエーションに起因して誤成形されたステープルを含む）の発生を最小化するために、ステープル製造システムおよびステープル成形システムにおけるバリエーションを補償する際に特に有効である。
