Claims (14)
- 切骨術または折骨術が施された大転子(9)の再固定のための転子保持プレートにおいて、
大腿近位部(2)に対して固定できるベースプレート(1,100,102,105,109,114,120)が設けられ、
前記ベースプレートが、大腿骨(2)上に非積極的に保持されるとともに、大転子(9)を通るテンションバンド構造の基盤としての機能を果たし、
前記テンションバンド構造が、少なくとも1つの、好ましくは2つのリテーナ(41,42)を備えるとともに、切骨術または折骨術が施された大転子を積極的におよび/または非積極的に大腿骨(2)に対して再固定できることを特徴とする転子保持プレート。 - 前記ベースプレート(1,100,102,105,109,114,120)の基端側には2つの突出部(16,17,116,117,121,122)が配置され、前記突出部(16,17,116,117,121,122)が、閉じられたテンションバンド構造の前記リテーナ(41,42)から前記突出部に作用する長尺柔軟部材(21,22)を有することを特徴とする、請求項1に記載の転子保持プレート。
- 前記ベースプレート(1,100,102,105,109,114,120)が好ましくは硬質に形成され、前記突出部(16,17,116,117,121,122)が好ましくは柔軟に形成され、前記長尺柔軟部材(21,22)が好ましくはケーブルまたはストラップとして形成されることを特徴とする、請求項1または2に記載の転子保持プレート。
- 互いに離間して配置される前記突出部(16,17,116,117,121,122)が、第1の端部が前記ベースプレート(1,100,102,105,109,114,120)に接続され、それぞれの前記突出部(16,17,116,117,121,122)の少なくとも1つの他の自由端部が柔軟長尺リンク(21,22)と動作可能に相互接続されることを特徴とする、請求項2または3に記載の転子保持プレート。
- 前記長尺柔軟部材(21,22)の第1の端部が、前記ベースプレート(1,100,102,105,109,114,120)に対して或いはそれぞれの前記突出部(16,17,116,117,121,122)の他の自由端部の領域で接続され、前記長尺柔軟部材(21,22)の他の自由端部(142)が、元のベースプレート(1,100,102,105,109,114,120)へと延びて該ベースプレートにて固定されることを特徴とする、請求項4に記載の転子保持プレート。
- 前記長尺柔軟部材(21,22)を引っ張ることによって、前記突出部(16,17,116,117,121,122)の自由端部が引張り中に互いにかみ合うことができ、それにより、テンションバンド構造の固有の安定性が高められることを特徴とする、請求項2〜5のいずれか一項に記載の転子保持プレート。
- 前記ベースプレート(1,100,102,105,109,114,120)を大腿近位部の外側に固定することができ、前記突出部(16,17,116,117,121,122)の自由端部を大転子の内側にかみ合わせることができ、前記長尺柔軟部材(21,22)が、好ましくはこれらが少なくとも転子窩(15)の上側で交差した後、特に好ましくはもう一度小転子の下側で互いに交差した後、元のベースプレート(1,100,102,105,109,114,120)へと延びることを特徴とする、請求項6に記載の転子保持プレート。
- 前記ベースプレート(1)が、下側先端部分(3)と、表面仕上げされた主要部(5)を備える上側基端部分(4)と、互いに離間され、前記主要部に配置されるとともに、前記基端部分(4)の前述した構成要素が寛骨結節を取り囲むように基端方向に延びる肢(6,7)とを有し、前記肢(6,7)が好ましくはそれらの基端でウェブ(13,123)を介して互いに接続されることを特徴とする、請求項1〜7のいずれか一項に記載の転子保持プレート。
- 請求項1〜8のいずれか一項に記載の転子保持プレートを用いて、切骨術または折骨術が施された大転子を手術再固定するための方法において、
切骨術または折骨術が施された大転子(9)断片が正確に位置決めされ、この後、ベースプレート(1,100,102,105,109,114,120)のプレート下肢が大腿近位部(2)上に位置決めされるとともに、テンションバンド構造が大転子(9)を通して位置決めされ、更なるステップにおいて、柔軟な長尺柔軟部材を引っ張ることにより、これらが曲げられて、2つの突出部(16,17,116,117,121,122)の端部が互いにかみ合わされ、もって、切骨術または折骨術が施された大転子が積極的におよび/または非積極的に大腿骨(2)に対して再固定され、この後、前記柔軟な長尺柔軟部材の第2の端部または自由端部(142)がクランプ装置(30)によって前記ベースプレートに締結されることを特徴とする方法。 - 前記突出部から来る前記柔軟な長尺柔軟部材が、転子窩(15)の上側で互いに交差し、そこから前記ベースプレート(1)へと案内されるとともに、該ベースプレートで、長尺柔軟部材の第2の端部または自由端部がクランプ装置(30)によって保持されることを特徴とする、請求項9に記載の方法。
- 前記長尺柔軟部材が、前記突出部から出た直後に、転子窩(15)の上側で互いに交差し、当該交差部から、転子山塊(大腿距)の内周側へと案内され、そこで長尺柔軟部材が、再度互いに交差した後、元の前記ベースプレートのクランプ装置へ向けて下肢の長手方向軸に対して略垂直に案内されることを特徴とする、請求項9に記載の方法。
- M.外側広筋の基部の短い部位の切り込み後に、横方向ウェブを有するベースプレート(123)が上端/基端からM.外側広筋下にスライドされることを特徴とする、請求項9〜11のいずれか一項に記載の方法。
- 横方向ウェブを伴わないベースプレートがM.外側広筋の基部の先端から周囲に導かれ、これを伴わない筋肉基部の前後の上側の2つのプレート突出部(115,116)が切り取られることを特徴とする、請求項9〜11のいずれか一項に記載の方法。
- 前記ベースプレートには、最小侵襲埋め込みのためにターゲットバウ(150)が取り外し可能に締結されることを特徴とする、請求項9〜13のいずれか一項に記載の方法。
