Abstract
Description
図1を参照すると、本発明の一実施形態では、埋込み可能な装置は、心臓12の心室内に配置された心内膜導線10である。導線10は、埋込み可能なCRM装置14の部品であり、装置14と結合された近端部16と、心臓12の1つまたは複数の部分の上または周囲に結合された遠端部18とを有する。CRM装置14は、心筋梗塞に応答して埋め込まれ、導線10は、梗塞部の中または近くに配置される。他の実施形態では、導線は、心外膜導線であってもよい。
図5、6、7を参照すると、本発明の別の実施形態では、埋込み可能な装置は、脈管系内に配置されたステント60である。図5に示すように、ステント60が、ステントを移送し、ステントを冠状動脈、末梢動脈、またはその他の身体内の血管または内腔などの体腔内に埋め込むために使用される従来型のカテーテル・アセンブリ62上に装着されている。カテーテル・アセンブリは、近端部66と遠端部68を有するカテーテル・シャフト64を備える。カテーテル・アセンブリ62は、オーバー・ワイヤ・システム(図示せず)の公知の方法または公知のラピッド・エクスチェンジ・カテーテル・システムのいずれかによって、ガイド・ワイヤ72上を進行させることによって、患者の脈管系を通って進行するように構成されている。
先天的に異常のある、損傷した、病気またはその他の機能不良の、三尖弁、僧帽弁、大動脈弁、肺動脈弁を含む心臓弁などの身体内通路の弁を代替するために人工心臓弁が使用される。このような人工心臓弁は典型的には、胸骨切開術を必要とする開胸手術、または隣接する肋骨間の胸壁切開術を必要とする低侵襲性手術のいずれかによって心臓に埋め込まれる。
図11、12、13を参照すると、本発明の別の実施形態では、埋込み可能な装置が、心房中隔欠損(ASD)を修正するための装置である。ASDは、心房中隔の構造的な欠損によって特徴付けられる心房中隔の先天性異常である。シャントが、心房中隔内に存在してもよく、右心房と左心房の間の流れを可能にする。欠陥部を通る左右シャントでの大きな欠損では、右心房と右心室が、容積過負荷になり、増大した容積が抵抗の低い肺脈管床へ拍出される。図12に示すように、その解放された延びていない状態にある装置100が、短い中部円筒形部分106によって互いに連結された2つの並んだディスク102、104を有する。この装置100はまた、卵円口開存(PFO:patent foramen ovale)として当技術分野で公知の欠陥を塞ぐのに十分適している。
図15を参照すると、本発明の別の実施形態では、埋込み可能な装置は、心室拘束装置(VRD)130である。このような装置130は、損傷した心筋を支持し、心臓壁の外側への膨張を制限するために、心臓の周囲に配置されて、心臓に取り付けられる。心臓に取り付けられるとき、VRD130は、壁側心膜の上または下に配置されてもよい。VRD130は、ジャケットの底部に装着された固定装置によって心外膜に固定されてもよい。適切な構成装置は、たとえば、VRD130の底部の円周を通過する、コード、縫合糸、接着剤また形状記憶素子などの円周取付け装置132を含む。取付け装置132の端部は、VRD130を定位置に固定するために、互いに締結されてもよい。別法として、VRD130の底部が、ジャケットの底部を心外膜に縫合するために補強されてもよい。
本発明の別の実施形態では、抗アポトーシス剤の局所的な投与が、微粒子、薬を周囲の組織内へ解放するポリマー基質投与システムを使用して達成される。生体分解性と非生体分解性の両方の基質が、薬の投与のために使用されてもよいが、生体分解性の基質が好ましい。これらは、天然または合成のポリマーであってよいが、分解と解放のプロファイルのより良好な特徴のため、合成ポリマーが好ましい。ポリマーは、薬の解放が望まれる時間を基にして選択される。微粒子は、薬が中実のポリマー基質内に分散されるマイクロスフィアであっても、芯部がポリマーの殻とは異なる材料製であるマイクロカプセルであってもよく、薬は芯部内に分散または懸濁される。これは、実際に液体でも固体でもよい。
Claims (24)
- 心臓または心臓の近くに配置されるように、かつ抗アポトーシス剤を担持し、治療部位に投与するように構成された埋込み可能な装置。
- 前記抗アポトーシス剤が、カスパーゼ阻害剤、Mapキナーゼ阻害剤、AIF阻害剤、TNF−α受容体遮断薬、ホスホジエステーゼ阻害剤の少なくとも1つを備える請求項1に記載の装置。
- 前記抗アポトーシス剤が、シトクロムCの解放を防止するように構成されている請求項1に記載の装置。
- 前記抗アポトーシス剤が、βアドレナリン受容体遮断薬、ACE阻害剤、抗炎症薬、アンギオテンシンII受容体遮断薬の少なくとも1つをさらに備える請求項1に記載の装置。
- 前記埋込み可能な装置が導線を備え、前記抗アポトーシス剤が、導線に付着された被覆内に含まれる請求項1に記載の装置。
- 前記埋込み可能な装置が導線を備え、前記薬が、前記導線によって担持された薬剤溶出基質内に含まれる請求項1に記載の装置。
- 前記薬剤溶出基質が、流体との接触の際に薬を溶出するように構成されている請求項1に記載の装置。
- 前記薬剤溶出基質が、電場を受けるように少なくとも部分的に配置され、前記電場を受けたときに薬を溶出するように構成されている請求項6に記載の装置。
- 前記埋込み可能な装置が導線を備え、前記抗アポトーシス剤が、前記導線に固定されたシリコーン・ベースの構成要素内に含まれる請求項1に記載の装置。
- 前記埋込み可能な装置が導線を備え、前記抗アポトーシス剤が、前記導線によって担持された浸透圧ポンプ内に含まれる請求項1に記載の装置。
- 前記埋込み可能な装置が導線を備え、前記抗アポトーシス剤が、前記導線によって担持された制御可能なポンプ内に含まれる請求項1に記載の装置。
- 前記埋込み可能な装置がステントを備え、前記抗アポトーシス剤が、前記ステントに付着された被覆内に含まれる請求項1に記載の装置。
- 前記埋込み可能な装置が心臓弁を備え、前記抗アポトーシス剤が、前記弁の構成要素に付着された被覆内に含まれる請求項1に記載の装置。
- 前記埋込み可能な装置が心臓弁を備え、前記抗アポトーシス剤が、前記弁の構成要素内に含まれる請求項1に記載の装置。
- 前記埋込み可能な装置が心房中隔欠損装置または卵円口開存装置を備え、前記抗アポトーシス剤が、前記装置の構成要素に付着された被覆内に含まれる請求項1に記載の装置。
