Abstract

本発明のシステムは、活性薬剤の経皮送達を増強させる装置を含む。本発明の原理によれば、該装置は、それを介して活性薬剤が効率的に送達される新たな皮膚環境を発生させるために用いられる。
(i)対象の皮膚上の領域に少なくとも1つのマイクロチャネルを発生させるステップ、

hGH−コラーゲン薄膜ベースパッチの調製
本実施例に用いられるコラーゲンは、Vitrogen(登録商標)100(3mg/ml、Cohesion Technologies社、米国カリフォルニア州パロアルト)であった。ヒト成長ホルモンは、Genotropin(登録商標)(5.3mg/16IU、Pharmacia and Upjohn、スウェーデン国ストックホルム)であった。リン酸緩衝生理食塩水(PBS)は、Biological Industries(イスラエル国、Kibbutz Beit Haemak)から入手した。
コラーゲン薄膜ベースパッチパッチからhGHの回収
コラーゲン薄膜中のヒトGH含量は、PBS溶液に対するhGHの抽出及びゲルろ過HPLC(Phenomenex SEC、250x4.6mm、Phenomenex)を用いる定量的分析により決定された。コラーゲン薄膜からhGHの回収は、表1に要約されている。
300ミリグラムのhGHを、各パッチ上に装填した。回収結果は、3つの実験の平均値±SDである。表1に示されるように、最初の量の90%以上が、5分以内で薄膜から抽出された。接着剤群内の変化はより高かったが、コラーゲンを接着剤に直接注ぐ場合、又はバッキングライナーに注ぐ場合において抽出量に有意差はなかった。
皮膚を介するhGHのin vitro透過性試験
ブタ皮膚を介するhGHの透過性は、Franz分散細胞系(社内製)によりin vitroで測定した。分散領域は2cm3であった。皮膚切断された(300〜500μm、Electric Dermatom、Padgett Instruments社、米国ミシガン州カンザス)ブタ皮膚を、屠殺白ブタ(Landres and Large Whiteの育種、イスラエル国Kibbutz Lahav地域で飼育)から切除された。経皮水分損失測定(TEWL、Dermalab Cortex Technology、デンマーク国Hadsund)を実施し、15g/m2/時間未満のTEWL濃度を有するものだけを、分散細胞に乗せた。
ViaDerm処理皮膚を介した印刷パッチ又はコラーゲン薄膜からhGHの累積透過性を図1に示す。図1に示されるように、hGH−コラーゲン薄膜ベースのパッチを用いるhGHの投与は、薄膜又は印刷パッチ投与1時間後のhGH印刷パッチにより得られたものと比較して、より低いhGH透過性を生じた。これらの結果、遅延送達は、hGH−コラーゲン薄膜ベースパッチを用いると達成されることを示している。しかしながら、hGH−コラーゲン薄膜ベースのパッチを用いたhGHのより後の時間での投与は、時間の関数として累積hGH量の増加をもたらし、hGH投与20時間後の全透過性は、hGH印刷パッチよりもhGH−コラーゲン薄膜パッチの方が2倍高かった(図1)。hGH経皮流量値(1時間につき1cm2当たりの透過量)を図2に示す。hGH適用1時間後、印刷パッチからのhGHの流量は、コラーゲン薄膜により得られたものよりも6倍大きかった。しかしながら、3時間から20時間までの時点で、コラーゲン薄膜からのhGHの流量は、印刷パッチから見られたものよりも高かった。このin vitro結果は、hGHの持続的送達は、コラーゲン薄膜により達成できることを示唆している。
in vivoでのhGH経皮送達
オスのモルモット(500〜800グラム、Dunkin Hartley、Harlan laboratories社、イスラエル国)及びオスのラット(350〜400、Sprague Dawley、Harlan laboratories社、イスラエル国)に、10%ケタミン/2%キシラジン溶液の70:30の比率、1ml/kgのIP注射により予め投薬した。麻酔は、イソフルオラン又はハロタンガスのいずれかで維持した。腹部皮膚の毛を注意深く剃り、イソプロピルアルコールで清浄した。30分後、経皮水分損失測定(TEWL、Dermalab Cortex Technology、デンマーク国Hadsund)を実施して皮膚の統合性をチェックした。皮膚のマイクロチャネル化は、上記実施例3に記載された条件でViaDerm装置の使用により実施した。次にTEWLを、操作をコントロールするために再度測定した。処置された皮膚は、hGH印刷パッチ又はhGH−コラーゲン薄膜ベースパッチのいずかでカバーし、血液サンプルは、0.2、4、6、9、12及び15時間適用後に、モルモットでは予め挿入された頚動脈カニューレから、或いはラットの尾から採血した。
ViaDerm処理のモルモット皮膚を介するhGHの送達を図3に示す。コラーゲン中のhGH埋め込みは、印刷パッチと比較して持続的送達をもたらした。このin vivo知見は、in vitro結果と相関する。最大の送達は、コラーゲン薄膜では6時間後に、印刷パッチでは2時間後に得られた(図3)。さらに、hGHは、印刷パッチの投与9時間後には検出されなかったが、一方、コラーゲンベースパッチの同じ時間の投与後には有意な濃度が検出された(図3)。

インスリン−コラーゲン薄膜ベースパッチの調製のために、用いられるコラーゲンは、Vitrogen(登録商標)100(3mg/ml、Cohesion Technologies社、米国カリフォルニア州パロアルト)又はアテロコラーゲン(6.5%、Koken社、日本国東京)のいずれかであった。ヒト組換えインスリンHumolog(登録商標)(Lispro−100IU/ml)、Humulin N(登録商標)(NPH−100IU/ml)、及びHumulin U(登録商標)(Ultra Lente(UL)−100IU/ml)は、Lilly(Lilly France S.A.、フランス国Fegershein)から購入した。
経皮適用前後のインスリン−コラーゲンパッチ中のインスリン含量
ViaDerm Engineering Prototypeを使用した。全ての試験に用いられた微小電極アレイの直径は、80μmであり、密度は、75/cm2であった。ラットの皮膚は、330Vの印加電圧、100kHzの周波数、2回のバースト、700マイクロ秒のバースト長、及び電流制限のない条件で処理した。
経皮適用前後のコラーゲンパッチ中のインスリン含量分析の要約を表3に示す。
糖尿病ラットにおけるコラーゲン薄膜に埋め込まれたインスリンの生物活性
オスのラット(300〜325、Sprague Dawley、Harlan laboratories社、イスラエル国)は、パッチ適用前に飼料を与えず、48時間水を自由に摂取させた。糖尿病を誘発するために、パッチ適用24時間前にラットにストレプトゾトシン(クエン酸緩衝液中55mg/kg、0.1M、pH4.5;Sigma、米国ミズーリ州セントルイス)をIP注射した。
糖尿病ラットのマイクロチャネル化皮膚に対するインスリン−コラーゲンパッチの適用後の血中グルコース濃度は、図5及び図6に示している。図5に示されるように、ラットに対して0.4IUインスリンの皮下(SC)投与は、血中グルコース濃度が急速に減少、すなわち、SC投与1時間後、グルコース濃度が200mg/dL以下であった。SC効果は約4時間続き、その後、グルコース濃度は200mg/dL超に上昇した(図5)。ラットの正常なグルコース濃度は、100〜200mg/dlの範囲であることに注意すべきである。対照的に、インスリン−コラーゲンパッチが適用された場合、インスリンのSC投与と比較して血中グルコース濃度の減少遅延が見られた。この血中グルコース濃度の減少遅延は、コラーゲンパッチの再水和化及び発生したマイクロチャネルを介するインスリンの分散のためである。さらに、コラーゲンパッチ中の0.4IUインスリンの経皮適用は、血中グルコース濃度が持続的減少(200mg/dL未満)が反映されるインスリンの持続的放出を生じ、このような低血中グルコース濃度は約8時間継続した(図5)。
インスリン−Vigilon(登録商標)ベースパッチの調製
Vigilon(登録商標)(C.R.BARD、米国ジョージア州コビントン)を、インスリンパッチの調製のために用いた。ヒト組換えインスリンHumulin(登録商標)R(Regular−100IU/ml)は、Lilly(Lilly France S.A.、フランス国Fegershein)から購入した。リン酸緩衝生理食塩水(PBS)は、Biological Industries(イスラエル国Kibbutz Beit Haemak)から入手した。
インスリン−Vigilon(登録商標)パッチ中のインスリン含量
全てのVigilon(登録商標)パッチ中のインスリン含量を、PBS溶液によるインスリンの抽出及び逆相HPLC(LUNA 5um C18、150×4mm、Phenomenex)による定量分析により決定した。
インスリン−Vigilon(登録商標)パッチ中のインスリンのin vitro皮膚透過性試験
ブタ皮膚を介するVigilon(登録商標)パッチにおけるインスリンの透過性は、Franz分散細胞系(社内製)によりin vitroで測定した。分散領域は2cm2であった。皮膚切断された(300〜500μm、Electric Dermatom、Padgett Instruments社、米国ミシガン州カンザス)ブタ皮膚を、屠殺白ブタ(Landres and Large Whiteの育種、イスラエル国Kibbutz Lahav地域で飼育)から切除した。経皮水分損失測定(TEWL、Dermalab Cortex Technology、デンマーク国Hadsund)を実施し、15g/m2/時間未満のTEWL濃度を有する皮膚片だけを、分散細胞に乗せた。
ViaDerm処置皮膚を介してVigilon(登録商標)パッチ中のインスリンの累積透過性を図7に示す。インスリン投与後、その累積量は、時間の関数としての増加し、全透過量は、用量依存的であった(Vigilon(登録商標)中のインスリン0.25、0.5、2.5及び5IUに関して、それぞれ12時間後1.5、2.8、7.8、及び16.0mU/cm2、図7を参照)。インスリン経皮流量値(1時間につき1cm2当たりの透過量)を図8に示す。この流量値も用量依存性を示した。したがって、例えば、6時間後、この値は、Vigilon(登録商標)中のインスリン0.25、0.5、2.5、及び5IUに関して、それぞれ0.1、0.2、0.5、及び1.0mU×cm2−1×hr−1であった(図8を参照)。
糖尿病ラットにおけるVigilon(登録商標)に埋め込まれたインスリンの生物活性
オスのラット(300〜325、Sprague Dawley、Harlan laboratories社、イスラエル国)は、パッチ適用前に飼料を与えず、48時間水を自由に摂取させた。糖尿病を誘発するために、パッチ適用24時間前にラットにストレプトゾトシン(クエン酸緩衝液中55mg/kg、0.1M、pH4.5;Sigma、米国ミズーリ州セントルイス)をIP注射した。注射24時間後にグルコース濃度が300mg/dl超であり、陽性の尿中グルコース及び陰性の尿中ケトンが観察された場合に、ラットは糖尿病として定義された。糖尿病ラットに、70:30の比の10%ケタミン/2%キシラジン溶液を1ml/kgIP注射した。麻酔は、イソフルオラン又はハロタンガスのいずれかで維持した。腹部皮膚の毛を注意深く剃り、イソプロピルアルコールで清浄した。30分後、経皮水分損失測定(TEWL、Dermalab Cortex Technology、デンマーク国Hadsund)を実施して、皮膚の統合性をチェックした。皮膚のマイクロチャネル化は、本明細書の上記の実施例10に記載された条件でViaDerm装置の使用により実施した。次に、操作をコントロールするためにTEWLを再度測定した。処置した皮膚は、種々のインスリンパッチでカバーした。全てのラットは、パッチ適用2時間前に0.1IUインスリンのSC注射を受けた。パッチ適用なしの0.1IUのSC注射は、比較対照として寄与した。血液は、ラットの尾から採血され、血中グルコース濃度は、適用後、予め決められた時間で血糖値試験ストリップを用いて(Ascensia Elite、Bayer)測定した。
糖尿病ラットのマイクロチャネル化皮膚に対するインスリン−Vigilon(登録商標)パッチ適用後の血中グルコース濃度を図9に示す。0.1IUインスリンの皮下投与により、グルコース濃度は直ちに低下した(図9)。SC効果は短時間続き、一定の血中グルコース濃度を維持することはなかった。しかし、SC注射の2時間後に、皮膚にインスリン−Vigilon(登録商標)パッチを適用すると、制御された、一定で、持続した血中グルコースプロファイルが9時間示された。1.5IUパッチと2.5IUパッチとの間に有意差は見られなかった。
hGH−又はインスリン−カラゲナン薄膜ベースパッチの調製
hGH−又はインスリン−カラゲナン薄膜ベースパッチの調製には、以下の材料が用いられた。カラゲナンI型及びII型(Sigma、米国ミズーリ州)、ヒト成長ホルモン(hGH)Genotropin(登録商標)(5.3mg/16IU、Pharmacia and Upjohn、スウェーデン国ストックホルム)、ヒト組換えインスリンのHumulin(登録商標)R(Regular−100IU/ml、Lilly France S.A.、フランス国Fegershein)、及びリン酸緩衝生理食塩水(PBS;イスラエル国Kibbutz Beit Haemak)。
カラゲナン薄膜ベースパッチからのhGH−又はインスリンのin vitro放出
カラゲナン薄膜ベースパッチからのhGH−又はインスリンの放出動態を試験するために、カラゲナン薄膜をPBS溶液に入れ、室温でインキュベーションした。種々の時点で、緩衝液を新鮮な緩衝液と取り替えた。緩衝液に放出されたhGHの量をゲルろ過HPLCによって測定し、一方、緩衝液に放出されたインスリンの量を逆相HPLCによって分析した。
I型及びII型カラゲナン薄膜からのhGH放出を図10に示す。I型カラゲナン薄膜を使用した場合、20分後、hGHの最初の量の約17%が放出された。hGHは8時間にわたって持続した様式で放出された。8時間後の合計回収量は、最初の用量の80%であった。該薄膜は8時間後に溶解し始めたので、さらなる放出は測定されなかった。
カラゲナンベースパッチからhGH及びインスリンのin vitro皮膚透過性試験
ブタ皮膚を介するインスリンの透過性は、Franz分散細胞系(社内製)によりin vitroで測定した。分散領域は2cm2であった。皮膚切断された(300〜500μm、Electric Dermatom、Padgett Instruments社、米国ミシガン州カンザス)ブタ皮膚を、屠殺白ブタ(Landres and Large Whiteの育種、イスラエル国Kibbutz Lahav地域で飼育)から切除した。経皮水分損失測定(TEWL、Dermalab Cortex Technology、デンマーク国Hadsund)を実施し、15g/m2/時間未満のTEWL濃度を有する皮膚片だけを、分散細胞に乗せた。皮膚マイクロチャネル化は、ViaDerm(商標)装置を用いて実施した。微小電極の密度は、200個/cm2であった。ViaDermの適用後、TEWLは、操作をコントロールするために再度測定した。皮膚片を、PBSで3回洗浄し、インスリン−カラゲナン薄膜を角質層側に置いて接着剤でカバーした。次に皮膚プラス薄膜を、角質層を上に向けて受容体細胞上に置き、ドナーチャンバは、その場でクランプで留めた。受容体細胞は、0.1%アジ化ナトリウム(Merk、ドイツ国ダルムシュタット)、1%ウシ血清アルブミン(Biological Industries、イスラエル国Kibbutz Beit Haemak)及びプロテアーゼ阻害剤カクテル(50ml PBS当たり1個の錠剤、Complete Mini、Roch)を含んだリン酸緩衝生理食塩水(PBS、pH7.4)で満たした。受け器溶液からのサンプルを、12時間まで予め決められた時間でチューブに採取した。サンプルは、分析されるまで4℃に維持した。インスリン分析は、ELIZAキット(DSL−10−1600、Diagnostic Systems Laboratories社、米国テキサス州ウェブスター)により実施した。
ViaDerm処置皮膚を介したカラゲナン薄膜ベースパッチ中のインスリン(1IU及び4IU)の累積透過性を図12に示す。カラゲナン薄膜中のインスリン投与後、累積インスリン量は、時間の関数としての増加し、全透過量は、1IUパッチよりも4IUパッチの方が多かった(4IUパッチ及び1IUパッチに関して、それぞれ12時間後9.3mU/cm2及び11時間後1.6mU/cm2、図12)。
Claims (51)
- 対象の皮膚上の領域において少なくとも1つのマイクロチャネルを発生できる装置、及び少なくとも1層の薬物貯留層を含むパッチを含む、対象の皮膚を介した活性薬剤の経皮送達を促進するシステムであって、前記少なくとも1層の薬物貯留層はポリマーマトリックス及び活性薬剤として少なくとも1種の治療用又は免疫原性ペプチド、ポリペプチド、又は蛋白質を含む医薬組成物を含むシステム。
- 前記装置が、
a.複数の電極を含む電極カートリッジ、及び
b.前記電極が皮膚の近くにあるときに、前記電極に電気的エネルギーを加え、前記電極の下の領域の角質層のアブレーションを可能にし、それによって少なくとも1つのマイクロチャネルを発生させるようになされているコントロールユニットを含む主要ユニット
を含む請求項1に記載のシステム。 - 前記電極カートリッジが、均一の形状及び寸法の複数のマイクロチャネルを発生させるようになされている請求項2に記載のシステム。
- 前記電気的エネルギーが、高周波である請求項2に記載のシステム。
