Abstract
【解決手段】基準装置は、使用の直前にイメージング可能材料を充填できる基準ポインタの細長いキャビティを備えることで、有効期間が伸び、臨床的な柔軟性が向上している。基準装置の基準ホルダが、乳房コイル位置決め固定具の照準レールによって患者の外部の所望の位置に案内される。別法では、照準レールによって案内されるスリーブが、内部のイメージング可能なターゲットとなるように基準ポインタを受容できる。

Description
本願は、参照してその開示内容を本明細書に組み入れるヒュース(Hughes)らによる2005年4月12日出願の米国特許出願第11/103,959号(名称「MRI生検装置の位置決め固定具(MRI BIOPSY DEVICE LOCALIZATION FIXTURE)」)の一部継続出願である。
本願は、イメージングを利用した組織の採取方法に関し、詳細には、皮下生検および病変除去のために磁気共鳴映像法(MRI)乳房コイルに対して生検プローブを位置決めするための改良された方法に関する。
近年、乳房組織の病変に適切に狙いを定めるためにイメージング法と針生検装置を組み合わせるようになってきた。市販されているこのような製品の例として、エシコン・エンド−サージェリィ社(Ethicon Endo-Surgery Inc.)によるMAMMOTOME(商標)を挙げることができる。このような装置の例は、参照することを以ってその開示内容の全てを本明細書の一部とする、1996年6月18日にバーバンク(Burbank)らに付与された米国特許第5,526,822号に開示されている。この装置のハンドルは、MRI装置の強磁界から離間した遠隔に配置された制御モジュールから機械的な力および電力を受け取ったり、真空を利用したりすることができる。
本発明は、従来技術の上記および他の欠点を解消するために、使用の直前にイメージング可能な材料で充填できる細長いキャビティを備えた基準装置を提供する。したがって、この基準装置では、滅菌されたイメージング可能な材料を使用者に提供するというオプションが有利に増え、有効期間が伸び、パッケージングの厳密さおよび保管上の制限が緩和され、かつ所望のイメージング可能な充填材料の選択における臨床的な柔軟性が得られる。
ここで図面を参照されたい。全ての図面において、同じ構成要素には同じ参照符号が付されている。図1に示されているように、磁気共鳴映像(MRI)適合生検システム10は、イメージングスキャンから得られる座標情報が後のコア生検または放射線治療の際に十分に利用されうるように、患者の乳房に対して所望の位置にある基準装置11(目立つ位置に拡大して図示されている)を含む。臨床的な柔軟性は、イメージング可能な化合物(例えば、生理食塩水やガドリニウム溶液など)が満たされずにパッケージされた基準装置11を設けることで改善される。したがって、有効期間が伸び、パッケージングおよび環境の制限が緩和され、使用者が、用途に適した広範なイメージング可能な化合物/液体を選択することができる。

前記内部キャビティに連通している前記ハウジングに形成されたポートであって、MRIイメージング可能材料を受容するように機能的に構成されている、ポートと、
充填の際に空気を逃がすことできるように前記ハウジングに形成されたベントと、

前記生検ガイドは、ホーム位置対照(home position reference)をさらに含み、
前記装置は、
前記ホーム位置対照で前記生検ガイドに取り付け可能な基準ホルダ(fiducial holder)、
をさらに含み、
前記基準ホルダが、前記患者の乳房に近接した基準(fiducial)を前記生検器具の刺入軸に沿った挿入点に整列させるような横方向の大きさを有する、
装置。
(15)実施態様(14)に記載の装置において、
前記基準ホルダは、磁気共鳴映像法(MRI)イメージング可能材料を含む一体型基準(integral fiducial)をさらに含む、装置。
前記基準ホルダは、前記生検ガイドに係合するように機能的に構成されたポリマーばねをさらに含む、装置。
(17)実施態様(14)に記載の装置において、
前記基準ホルダは、前記生検ガイドを前記ホーム位置対照にスライド可能に係合させるように機能的に構成された係合部材をさらに含む、装置。
(18)患者の乳房の診断イメージを確認しながら生検を行うための装置において、
前記患者の乳房の選択された側面に対して位置決め可能な第1の圧迫部材と、
前記患者の乳房の反対側の側面に対して位置決め可能な第2の圧迫部材であって、前記第1の圧迫部材に対して移動して前記患者の乳房を圧迫して固定できる、第2の圧迫部材と、
前記第1の圧迫部材に形成された開口と、
イメージング可能な流体で充填できる基準容器であって、前記第1の圧迫部材の前記開口内に挿入できる大きさである、基準容器と、
を含む、装置。
(19)実施態様(18)に記載の装置において、
前記開口は、コア生検装置のプローブを挿入できるように成形された複数のグリッド開口の選択された1つを含む、装置。

- 外側圧迫部材に対して位置を調節可能な生検プローブ支持体を有する前記外側圧迫部材と内側圧迫部材との間で圧迫された患者の乳房の診断イメージを確認しながら、最小侵襲性医療処置を行うための装置において、
磁気共鳴映像法(MRI)適合性材料から形成され、内部キャビティを画定するハウジングと、
前記内部キャビティに連通している前記ハウジングに形成されたポートであって、MRIイメージング可能材料を受容するように機能的に構成されている、ポートと、
充填の際に空気を逃がすことできるように前記ハウジングに形成されたベントと、
