Abstract

【課題】特に内視鏡手技に適した外科用ステープル留め及び切断器械を提供する。
【解決手段】外科用器械（１０）は、長手方向発火運動に応動するエンドエフェクタ（１２）を有する。ユーザは、発火アクチュエータ（３４）に運動を生じさせ、発火機構により選択的に伝達される発火運動を生じさせる。カム板（１３０）は、発火アクチュエータに結合された駆動ウェッジ（１８０）とそれぞれ係合する周囲の少なくとも一部に沿って設けられた複数のカムローブ（１４２〜１４４）を有する。カム板は、ラック（１７０）に噛み合い結合され、間欠回転運動を長手方向発火運動に変換する。特に、取っ手は、エンドエフェクタを発火させる（即ち、ステープル留めして切断する）のに必要な力の所要量を減少させるために多数の発火行程を生じさせる。

Description

本発明は一般に、線状に並んだステープルを組織に留める一方でこれらステープルライン相互間で組織を切断することができる外科用ステープラ器械に関し、特に、ステープラ又はステープル留め器械の改良並びにトリガの多数回の行程で発火を行なうかかるステープル留め器械の種々のコンポーネントを形成する方法の改良に関する。
内視鏡型外科用器械は、伝統的な開放式外科用装置よりも好まれる場合が多い。というのは、小さな切開部は、術後回復期間及び合併症を軽減する傾向があるからである。その結果、大々的な技術開発により、トロカールのカニューレを介して遠位側エンドエフェクタを所望の手術部位のところに正確に配置するのに適した種々の内視鏡型外科用器械が実現した。これら遠位側エンドエフェクタは、多くの仕方で組織に係合して診断又は治療効果を達成する（例えば、体内カッタ、掴み具、カッタ、ステープラ、クリップアプライヤ、接近装置、薬剤／遺伝子投与装置及び超音波、ＲＦ、レーザ等を用いるエネルギ装置）。
公知の外科用ステープラとしては、長手方向切開部を組織に入れると同時に組織の反対側の側部に線状に並んだステープルを留めるエンドエフェクタが挙げられる。エンドエフェクタは、１対の互いに協働するジョー部材を有し、これらジョー部材は、器械が内視鏡用途又は腹腔鏡用途に用いられることが意図されている場合、カニューレ通路を通過することができる。ジョー部材のうちの一方は、少なくとも２つの側方に間隔を置いた列状のステープルを有するステープルカートリッジを受け入れる。他方のジョー部材は、カートリッジ内のステープル列と整列するステープル形成ポケットを有するアンビルを構成する。器械は、遠位側へ駆動されると、ステープルカートリッジに設けられた開口部を通過し、ステープルを支持している駆動装置に係合してアンビルに向かうステープルの発火を行なう１対の往復動ウェッジを有する。
内視鏡用途に適した外科用ステープラの一例は、米国特許第５，４６５，８９５号明細書に記載されており、かかる外科用ステープラは有利には、別々の閉鎖動作と発火動作をもたらす。それにより、医師は、ジョー部材を組織に当てて閉じて組織を発火前に位置決めすることができる。ジョー部材が正しく組織を掴んでいることをいったん医師が確認すると、医師は次に、単一の発火行程で外科用ステープラを発火することができ、それにより組織を切断すると共にステープル留めする。切断とステープル留めを同時に行なうことは、それぞれ切断又はステープル留めのみを行なう互いに異なる外科用ツールで順次かかる動作を行なったときに生じる場合のある合併症を回避する。
発火前に組織を閉鎖することができる特定の利点の１つは、医師が切断部の所望の場所を達成したことを医師が内視鏡により確認することができ、又、十分な量の組織が対向したジョー相互間に捕捉されていることを確認できることである。また、対向したジョーを互いに引き寄せて非常に接近させ、特に、これらの遠位端部のところで挟んで締め付けると、切断された組織に閉じられたステープルが効果的には形成されない。他方の端においては、過剰な量のクランプされた組織により、動かなくなり、発火が不完全になる場合がある。
一般に、１回の閉鎖行程を行ない、次に１回の発火行程を行なうことが、切断及びステープル留めを行なう上で有利且つ効果的な方法である。しかしながら、或る場合には、複数回の発火行程を必要とすることが望ましい場合がある。例えば、外科医は、切断部の所望の長さを得るために或る範囲のサイズのジョーから対応した長さのステープルカートリッジを選択することができる。ステープルカートリッジが長いと長い発火行程が必要である。かくして、発火を行なうために手で握られるトリガは、短いステープルカートリッジと比較してより多くの組織を切断し、より多くのステープルを駆動するためには、これら長いステープルカートリッジについて大きな力を及ぼす必要がある。或る外科医の手の強さを超えないように力の大きさを小さくし、短いカートリッジと同等にすることが望ましい場合がある。加うるに、長いステープルカートリッジに馴染んでいない外科医は、予期しない大きな力を必要とする場合に動かなくなること又は他の誤動作が生じたかどうかに関心を持つようになる場合がある。
発火行程に必要な力を小さくする一手段は、米国特許第５，７６２，２５６号明細書及び同第６，３３０，９６５号明細書に記載されているように発火トリガを多数回にわたって行程動作させることができるラチェット機構である。しかしながら、発火トリガの往復運動を爪により中実ラックに直接変換することは、各発火行程中、所望量の発火運動を得るための設計上の選択肢を制限することが考えられる。加うるに、多行程発火機構を有するこれら公知の外科用ステープル留め器械は、別個の閉鎖及び発火動作の利点を備えていない。

したがって、発火行程移動距離とエンドエフェクタのためにもたらされる長手方向発火運動の所望の関係をもって所望長さの切断及びステープル留めを達成するよう多数回の発火行程を用いる外科用ステープル留め器械が大いに要望されている。
本発明は、発火部材のバックアップを保ちながら一連の多数の発火行程を伝達する回転伝動装置を有する外科用ステープル留め及び切断器械を提供することにより先行技術の上述の欠点及び他の欠点を解決する。それにより、発火力の増大及び（又は）発火移動距離の増大を必要とする器械のエンドエフェクタを多行程発火トリガで容易に発火できる。さらに、器械は、外科医がエンドエフェクタを必ずしも発火させないで組織を位置決めできる別個の閉鎖機能を有する。
本発明の一特徴では、外科用器械は、外科手術を行なうのに長手方向発火運動に応動するエンドエフェクタを有する。ユーザは、発火アクチュエータに運動を生じさせ、発火機構により選択的に伝達される発火運動を生じさせる。具体的に説明すると、カム板は、発火運動を行なう際、発火アクチュエータに結合された駆動ウェッジとそれぞれ係合する周囲の少なくとも一部に沿って設けられた複数のカムローブを有する。カム板は、歯車による噛み合いによりラックに結合され、この間欠回転運動を長手方向発火運動に変換する。
本発明の別の特徴では、外科用器械は、組織を切断すると共にステープル留めするエンドエフェクタを有する。特に、ステープル留め組立体は、複数のステープルから間隔を置いた開放位置から複数のステープルに隣接した閉鎖位置まで動くことができるステープル形成表面を備えたアンビルを遠位側に有する。ステープル留め機構は、少なくとも１つの結合機構と関連した少なくとも１つのカムローブを備えた回転カム部材を有し、回転カム部材の回転によりステープル留め組立体からのステープルの少なくとも一部を留めるようになっている。それにより、多行程発火方式を用いて組織を切断すると共にステープル留めできる。
一連の多数回の発火行程を伝達する回転伝動装置を有する本発明の外科用ステープル留め及び切断器械では、所望長さの切断及びステープル留めを達成するよう多数回の発火行程を用いるが、特に、各行程について力及び移動距離が最適化される。特に、取っ手は、エンドエフェクタを発火させる（即ち、ステープル留めして切断する）のに必要な力の所要量を減少させるために多数の発火行程を生じさせる。また、発火力の増大及び（又は）発火移動距離の増大を必要とする器械のエンドエフェクタを多行程発火トリガで容易に発火できる。さらに、本発明の器械は、外科医がエンドエフェクタを必ずしも発火させないで組織を位置決めできる別個の閉鎖機能を有する。本発明の器械は、内視鏡手技に特に適している。

図面を参照すると（図中、同一の符号は同一の部分を示している）、図１〜図４は、本発明のユニークな特徴を実施することができる外科用ステープル留め及び切断器械１０を示している。外科用ステープル留め及び切断器械１０は、細長いチャネル１６に回動自在に取り付けられたアンビル１４を備えるエンドエフェクタ１２を有し、これは、切断されてステープル留めされるべき組織をクランプする対向したジョーを形成している。エンドエフェクタ１２は、シャフト１８によって取っ手２０に結合されている。エンドエフェクタ１２及びシャフト１８によって形成された作業部分２２は有利には、外科医が取っ手２０を掴むことによって制御された状態で内視鏡外科手技を実施するようトロカール又は小さな腹腔鏡開口部中に挿入可能な寸法になっている。取っ手２０は有利には、発火からのエンドエフェクタ１２の別個の閉じ運動を可能にすると共に外科医に発火の程度を指示しながら多数の発火行程がエンドエフェクタ１２の発火（即ち、切断及びステープル留め）を行なうことができるようにする特徴部を有している。
これらの目的のため、シャフト１８の閉鎖管２４は、エンドエフェクタ１２の閉鎖を生じさせるよう閉鎖トリガ２６とアンビル１４との間に結合されている。閉鎖管２４内では、フレーム２８が細長いチャネル１６と取っ手２０との間に結合されていてエンドエフェクタ１２を長手方向に位置決めすると共に支持するようになっている。回転ノブ３０が、フレーム２８に結合され、これら両方の要素は、シャフト１８の長手方向軸線回りの回転運動に対し取っ手２０に回転自在に結合されている。かくして、外科医は、回転ノブ３０を回すことによりエンドエフェクタ１２を回転させることができる。閉鎖管２４の回転も又回転ノブ３０によって行なわれるが、かかる閉鎖管は、エンドエフェクタ１２の閉鎖を生じさせるよう回転ノブに対し或る程度の長手方向運動を保持する。フレーム２８内では、発火ロッド３２が、エンドエフェクタ１２のアンビル１４と多行程発火トリガ３４との間に結合された状態で長手方向運動可能に位置決めされている。閉鎖トリガ２６は、取っ手２０のピストル型グリップ３６の遠位側に位置し、発火トリガ３４は、ピストル型グリップ３６と閉鎖トリガ２６の両方の遠位側に位置している。
内視鏡操作では、作業部分２２をいったん患者の体内に挿入して手術部位に接近させると、外科医は内視鏡又は他の診断画像化装置を参照して組織をアンビル１４と細長いチャネル１６との間に位置決めする。外科医は、閉鎖トリガ２６及びピストル型グリップ３６を掴んで、組織を繰り返し掴んだり位置決めすることができる。外科医は、エンドエフェクタ１２に対する組織の場所及びこの中に入った組織の量についていったん満足すると、閉鎖トリガ２６をピストル型グリップ３６に向かって完全に押し、組織をエンドエフェクタ１２内にクランプし、閉鎖トリガ２６をこのクランプ（閉鎖）位置にロックする。この位置に満足しなければ、外科医は、解除ボタン３８（図４）を押すことにより閉鎖トリガ２６を解除し、しかる後手順を繰り返して組織をクランプする。なお、解除ボタンの操作については以下に詳細に説明する。
クランプが正確であれば、外科医は、外科用ステープル留め及び切断器械１０の発火に引き続き移ることができる。具体的には、外科医は、発火トリガ３４及びピストル型グリップ３６を掴み、発火トリガ３４を所定回数押す。必要な発火行程の数は、手の最大寸法、各発火行程中に器械に与えられる力の最大量、長手方向距離及び発火中、発火ロッド３２を介してエンドエフェクタ１２に伝達されるのが必要な力に応じて人間工学的に決定される。以下の説明で理解されるように、個々の外科医は、発火トリガ３４を異なる回転運動範囲でサイクル動作させ、かくして発火行程の数を増減するよう選択できる。
図１において、外科用ステープル留め及び切断器械１０の発火後、閉鎖解除レバー４０を作動させて発火機構を引っ込める。閉鎖解除レバー４０を押すことにより、取っ手２０内の回転伝動装置発火機構４２が稼働解除され、ばね１７２が発火ロッド３２をエンドエフェクタ１２から引っ込めることができる。
Ｅビームエンドエフェクタを有する作業部分
多行程発火運動をもたらすことができる取っ手２０の利点は、多くの器械に利用できるということであり、かかるエンドエフェクタ１２の１つが図１〜図４に示されている。エンドエフェクタ１２のアンビル１４は、閉鎖管２４によって長手方向且つ遠位側へ伝達される取っ手２０からの閉鎖運動に応動する。細長いチャネル１６は、向かい合ったジョーを形成するよう並進及び閉鎖アンビル１４に摺動的に係合し、フレーム２８は、取っ手２０への剛性取り付け部を形成するよう静止チャネル１６に固定的に係合する。閉鎖管２４は、アンビル１４と細長いチャネル１６との間の回動連結部の遠位側でアンビル１４に係合する。かくして、フレーム２８に対する閉鎖管２４の遠位側への運動は、エンドエフェクタ１２の閉鎖をもたらし、フレーム２８に対する近位側への運動は、エンドエフェクタ１２の開放をもたらす。
特に図４を参照すると、作業部分２２は、取っ手２０からの発火運動、具体的には、取っ手２０内の発火機構４２と作業部分２２との間で長手方向運動を結合する発火ロッド３２（図４には示されていない）の発火運動に応動する部品を更に有する。特に、発火ロッド３２（図５では分解状態で示されている）は、フレーム２８に設けられた長手方向凹部４８内で発火トラフ部材４６に回転自在に係合する。発火トラフ部材４６は、発火ロッド３２の長手方向運動に直接応答してフレーム２８内で長手方向に動く。閉鎖管２４に設けられた長手方向スロット５０が、回転ノブ３０（図示せず）と作動的に結合し、長手方向スロット５０により更に、回転ノブ３０は、閉鎖管に設けられた小さな長手方向スロット５２のところでフレーム２８に係合して回転を行なわせることができる。タブが、閉鎖管２４のスロット５０の前に設けられ、このタブは、閉鎖管２４をフレーム２８に結合するようフレーム２８のスロット５２内へ下に曲げられている。閉鎖管２４の長手方向スロット５０の長さは、回転ノブ３０との相対長手方向運動が閉鎖運動をそれぞれ達成することができるほど十分長い。
フレームトラフ部材４６の遠位端部は、発火バー５６の近位端部に取り付けられ、この発火バーは、フレーム２８と共に動いてＥビーム６０をエンドエフェクタ１２内へ遠位側へ突き出す。エンドエフェクタ１２は、Ｅビーム６０により作動されるステープルカートリッジ６２を有し、Ｅビームにより、ステープルは、カートリッジ６２のステープル孔６４から押し出してこれらをアンビル１４のステープル形成溝６８に接触させ、それにより“Ｂ”形に形成されたステープルを生じさせる。特に図３を参照すると、ステープルカートリッジ本体８６は、ステープル留めされた状態で組織を切断するようＥビーム６０の遠位端部に沿って設けられた垂直方向に差し向けられている切断面を通過させる近位側に開いた垂直方向スロット７０を更に有している。

多行程発火ハンドル
図５〜図８では、取っ手２０は、閉鎖トリガ２６及び発火トリガ３４の作動に応答してそれぞれ閉鎖運動及び発火運動を作業部分２２に生じさせる。閉鎖運動に関し、閉鎖トリガ２６は、３つの側方孔、即ち、前方に位置決めされたピン穴７８、後方の下方ピボット穴８０及び中央切欠き８２を有する上方部分７６を有している。３つのロッドが、取っ手ハウジング８８の右半部シェル８４と左半部シェル８６との間に側方に差し向けられると共にこれらに係合している（右半部シェル８４は、図５及び図６に示され、左半部シェル８６は、図７に示されている）。特に、後ロッド９０が、閉鎖トリガ２６の上方部分８０の後方ピボット穴８４を通り、かくして、閉鎖トリガ２６は、後ロッド９０回りに回動する。後ロッド９０に対し遠位側に位置決めされた前ロッド９２及び前ロッド９２の上方に位置した頂ロッド９４が、中央切欠き８６を通過し、この切欠きは、トリガ移動距離もそれぞれの終わりのところで前ロッド９２及び頂ロッド９４に接触することにより閉鎖トリガ２６の運動を拘束するよう形作られている。かくして、中央切欠き８６は、閉鎖トリガ２６が前方に（遠位側に）位置しているとき、底面が前ロッド９２に接触する垂直部分及び閉鎖トリガ２６がその前方弛緩位置及びその近位作動位置にあるとき、頂面及び前面がそれぞれ頂ロッド９４に接触する近位側に傾斜した上方部分を有する。
閉鎖管２４に係合する閉鎖ヨーク９６が、取っ手ハウジング８８内に長手方向摺動自在に受け入れられ、この閉鎖ヨークは、その遠位端部が閉鎖管２４の近位端部に係合し、かくしてエンドエフェクタ１２を閉じるために長手方向閉鎖運動を閉鎖管２４に伝達し、それ故アンビル１４に伝達する。この係合により、閉鎖ヨーク９６が回転しない状態で、閉鎖管２４の回転を可能にする。この係合部の上方で、側方ピン穴１００が、前ピン１０４によって閉鎖リンク１０２に結合されており、閉鎖リンク１０２の他端部は、後方ピン１０６により閉鎖トリガ２６のピン穴８２に結合されている。
三角形スペーサ１２０が、ロッド９０，９２，９４をそれぞれ受け入れる穴１２２，１２４，１２６を有し、この三角形スペーサは、カム板１３０と、閉鎖トリガ２６の上方部分８０との間にサンドイッチされている。カム板１３０は、前ロッド１３０回りに回転し、このカム板１３０は、後ロッド９０及び頂ロッド９４を受け入れる半円形スロット１３２を有している。中央穴１３４が、前ロッド９２を受け入れる。カム板１３０の左側では、発火トリガ３４の上端部１３８のところに設けられたロッド穴１３６が、頂ロッド９４を受け入れる。発火トリガ３４が、カム板１３０を三角形スペーサ１２０と発火トリガ３４との間にサンドイッチするようロッド９４に回転自在に取り付けられる。ロッド穴１３６の下で発火トリガに設けられた遠位側に開いた凹部１４０は、前ロッド９２を受け入れるよう位置合わせされ、それにより発火トリガ３４を発火中、遠位側へ引くことができるようになっている。閉鎖トリガ２６の作動により、カムリンク１０２が下方へ揺動して、発火トリガ３４から内方へ延びる駆動ウェッジピン１８４と接触し、それにより発火トリガ３４は、部分的に遠位側へ引っ張られ、発火トリガ３４を掴むためにこれを段階的に作動させる。
特に図５、図９及び図１０を参照すると、カム板１３０は、前方部分（図示のその非発火状態にあるとき）回りに、特にその左側に沿って設けられた一連のカムローブ１４２〜１４４（図９参照）を備え、これらカムローブは、それぞれ上から前への（左側から見て反時計回りの）回転をカム板１３０に与えるよう発火トリガ３４と係合する。この回転は、回転伝動装置付き発火機構４２の歯車列１５０（図５及び図１１）を介して伝達され、この歯車列は、小さな遊び歯車１５４に係合するカム板１３０の右側の下方部分の回りに設けられた歯車部分１５２で始まり、遊び歯車はかくして、カム板１３０に対し増大した速度で上から後方（時計回り）に回転する。大きな遊び歯車１５６が、遊びアクスル１５８によって小遊び歯車１５４に連結され、かくして、同一方向且つ同一速度で回転する。第２の小歯車１６０が、大遊び歯車１５６に噛み合い連結されており、かくして、速い速度で上から前へ（左側から見て反時計回りへ）回転する。細かな歯の付いた大歯車１６２が、第２のアクスル１６４によって第２の小歯車１６０に連結され、かくして、第２の小歯車１６０と同一方向且つ同一速度で回転する。かくして、歯車列１５０は、追加の長手方向発火運動をもたらす二重歯車減速装置としての特徴を有することによりカム板１２０の運動を増幅する。細かい歯の付いた大歯車１６２は、中実ラック１７０の下面に設けられたギアセグメント１６８に係合し、中実ラックの遠位端部は、発火ロッド３２の近位端部に係合する。ラック１７０は、閉鎖ヨーク９６内に長手方向摺動自在に受け入れられたその遠位部分及び取っ手ハウジング８８の右シェル半部８４と左シェル半部８６との間に長手方向摺動自在に受け入れられたその近位部分を有している。
カムローブ１４２〜１４４に対する発火トリガ３４の選択的係合により、発火トリガ３４の多数の発火行程の実行を可能にすることによりそれ以上の長手方向移動距離が得られる。歯車列１５０を発火に備えて準備するため、カム板１３０を歯車列引っ込みばね１７２によってその非発火位置に向かって時計回りに押圧し、この歯車列引っ込みばねは、カム板１２０の下方近位縁部のところの環状凹部１７６内に形成された左側に突き出た一体ピン１７４に取り付けられている（図９及び図１０）。歯車列引っ込みばね１７２は、取っ手ハウジング８８と一体のピン１７８に取り付けられたその他端部を有している。発火トリガ３４の作動により、カム板１３０が反時計回りに回転して引っ込みばねを伸長させる。引き続き発火トリガ３４を作動させると、伸長した引っ込みばね１７２が回転しながらカム板１３０の外周部回りに巻きついて環状凹部１７６（図示せず）内に嵌まり込む。
特に図５、図９及び図１１を参照すると、発火トリガ３４の上端部１２８の下且つ遠位側に、駆動ウェッジピン穴１８０及び近位側ピン穴１９０が設けられている。駆動ウェッジピン１８４及びピン１９６は、発火トリガ３４の穴１８０，１９０（それぞれ）から内方に延びる。駆動ウェッジ１８２及びスタンドオフフィンガ１８６が、駆動ウェッジピン１８４に回動自在に取り付けられ、ねずみ取り形ばね１８８によって互いに作動的に連結されている。駆動ウェッジ１８２とピン１９６との間に設けられた抵抗する引張ばね１９４は、駆動ウェッジ１８２、スタンドオフフィンガ１８６及びばね１８８を時計回りに押圧する（図１０）。発火トリガ３４を作動させると（図９）、スタンドオフフィンガ１８６はカム板１２０の中央非カム作用周面に接触し、スタンドオフフィンガ１８６、ばね１８８及び駆動ウェッジ１８２を反時計回りに回転させる。スタンドオフフィンガ１８６の反時計回りの運動により、駆動ウェッジ１８２はカムローブ１４２〜１４４（図９）と発火係合する。
特に図１２を参照すると、駆動ウェッジ１８２を発火行程相互間でカムローブ１４２〜１４４のうちの１つから引き離すと、カム板１３０は、歯車列引っ込みばね１７２の作用により、しかしながらアンチバックアップレバー２００の作用のために上から後ろに回転する傾向がある。アンチバックアップ振り子２００の側方ピン２０２，２０４はそれぞれ、取っ手ハウジング８８の右半部シェル８４及び左半部シェル８６に係合する。ピン２０２，２０４の上方で、アンチバックアップ引張ばね２０６は、アンチバックアップ振り子２００の遠位側の右半部シェル８４の一体ピン２０８に取り付けられている。特に図５を参照すると、アンチバックアップ振り子２００の下方足部２１０は、中実ラック１７０の上面２１２と摩擦接触する。アンチバックアップ振り子２００の下方足部２１０が引っ込み中実ラック１７０により近位側に引き寄せられると、アンチバックアップレバー２００は、中実ラック１７０への垂直係合部に近づき、それにより中実ラック１７０を係止する摩擦力を増大させ、この摩擦力は、歯車列引っ込みばね１７２によって生じる後方駆動力に打ち勝つのに十分である。中実ラック１７０を発火トリガ３４によって遠位側へ駆動すると、下方足部２１０は、遠位側へ押され、それにより摩擦を減少させて発火を可能にする。下方足部２１０の過度の前方運動は、遊びアクスル１５８及びアンチバックアップ引張ばね２０６からの押圧によって阻止される。
図１２では、解除ボタン３８をその後方ピボットピン２２０，２２２回りに上方に回動させ、その遠位アーム２２４をアンチバックアップ振り子２００の近位側に差し向けられたアーム２２６の上方に持ち上げ、それにより下方足部２１０の遠位側運動を可能にしてラック１７０を発火行程相互間に係止する。クランプを係止レバー２３０が、その側方ピボットピン２３２，２３４回りに揺動して解除ボタン３８のこの持ち上げを行なうようになっている。特に、クランプ係止レバー２３０の近位側且つ上方に突き出たアーム２３６が、解除ボタン３８の遠位アーム２２４の下面に摺動自在に当接する。クランプ係止レバー２３０の遠位側に突き出た係止アーム２３８は、閉鎖ヨーク９６をそのクランプ状態に係止する。特に、近位側且つ上方に突き出たアーム２３６と遠位側に突き出た係止アーム２３８との間で下方に延びるタブ２４０が、引張ばね２４２によって近位側に押圧され、この引張ばねは又、ピン２４４のところで取っ手ハウジング８８の右半部シェル８４に取り付けられている。図６及び図７を参照すると、遠位側に突き出た係止アーム２３８は、閉鎖ヨーク９６の近位側頂部に設けられた段部２４６上に載り、それにより閉鎖ヨーク９６を遠位側へ移動させて閉鎖運動を伝達することができるようになっている。クランプ係止切欠き２４８、即ち、段部２４６の遠位側且つ上方に開いた凹部は、閉鎖ヨーク９６がその遠位作動位置に達すると（図８及び図９）遠位側に突き出た係止アーム２３８を受け入れる。かくして、外科医は、エンドエフェクタ１２がクランプされたままの状態で閉鎖トリガ２６を解除することができる。
図５〜図８及び図１２を参照すると、上述のアンチバックアップ特徴及び閉鎖クランプ特徴に加えて、発火ロックアウトレバー２５０により発火ロックアウト特徴が提供される。外科用ステープル留め及び切断器械１０がその初期開き且つ非発火状態にある場合、発火ロックアウトレバー２５０は、特に図７及び図８に示すように、中実ラック１７０の遠位側発火運動を阻止することにより引っ込み中の閉鎖ヨーク９６に応動する。発火ロックアウトレバー２５０は、中実ラック１７０の近位部分に沿って右縁部２５６と整列する遠位側に傾斜した上面２５４を備えた遠位側に延びるアーム２５２を有している。中実ラック１７０の残りの遠位部分に沿って設けられた凹み右縁部２５８により、発火ロックアウトレバー２５０の遠位側に傾斜した上面２５４は、上方に回転することができ、垂直タブ２６６に連結された引張ばね２６４によって押圧されるその近位側方ピン２６０，２６２回りに回動し、垂直タブ２６６は、遠位側に延びるアーム２５２に垂直且つ近位側に取り付けられている。引張ばね２６４の他端部は、垂直タブ２６６の後方で取っ手ハウジング８８の右半部シェル８４に形成された一体ピン２６８に連結されている。
図８に示すように、遠位方向に傾斜した表面２５４は、発火ロックアウトレバー２５０の下方に回動する遠位側に延びるアーム２５２を受け入れるよう近位側且つ上方に開いた閉鎖ヨーク９６の近位端部を横切って形成された段部２７０により上方へ楔の働きを受けることにより中実ラック１７０の遠位側への運動を阻止する。閉鎖ヨーク９６を図１２に示すようにエンドエフェクタ１２を閉鎖するよう遠位側に移動させた状態で、中実ラック１７０の右縁部２５６は、遠位側へ傾斜した表面２５４を越えるようになり、遠位側に傾斜した表面２５４は、遠位側に延びるアーム２５２を下方へ移動させて高い位置に形成されると共に遠位側に設けられた段部２７０の近位側で閉鎖ヨーク９６に形成された下方段部２７２に係合することによりこれに応動する。下方段部２７２への発火ロックアウトレバー２５０の係合は、中実ラック１７０が引っ込められるまで閉鎖ヨーク９６の引っ込み（近位側への運動）を阻止するという利点がある。かくして、発火機構４２の引っ込みを開始させることは有利には、エンドエフェクタ１２の解除前に生じ、もしそうでなければそれにより発火機構４２が動かなくなる場合がある。さらに、下方段部２７２と発火ロックアウトレバー２５０との間には、有利には外科用ステープル留め及び切断器械１０をその開き引っ込み状態に戻すのに二段階手順を必要とするのに足るほどの摩擦接触が存在する場合がある。特に、解除ボタン３８を押すことにより発火機構４２をいったん引っ込めると、閉鎖トリガ２６に加わる僅かな締め付けは、発火ロックアウトレバー２５０がその発火ロックアウト位置に上昇できるようにする傾向がある。しかる後、閉鎖トリガ２６の解除は、閉鎖ヨーク９６が完全に引っ込められ、かくしてエンドエフェクタ１２が開かれると、高いところに位置する段部２７０の係合が得られるよう発火ロックアウトレバー２５０が位置合わせされた状態で続く。
ラック１７０は有利には、発火機構４２の近位側の一部をハンドル内へ湾曲させることができるリンクで形成でき、それにより一層コンパクトな設計が得られることは更に理解されるべきである。かかるリンク式ラックは、ジェフリー・Ｓ．スワイゼ、フレデリック・Ｅ．シェルトン・フォース名義で２００３年９月２９日に出願された共通所有者の米国特許出願第１０／６７３，９３０号明細書（発明の名称：SURGICAL STAPLING INSTRUMENT INCORPORATING A FIRING MECHANISM HAVING A LINKED RACK TRANSMISSION）に詳細に記載されている。なお、かかる米国特許出願明細書の記載内容全体を参照によりここに引用する。
使用にあたり、外科医は、エンドエフェクタ１２及びシャフト１８をトロカールのカニューレに通して手術部位に位置決めし、アンビル１４を位置決めし、そしてチャネル１６を対向したジョーとして伸長させてステープル留めされて切断されるべき組織を掴む。エンドエフェクタ１２の位置にいったん満足すると、閉鎖トリガ２６を取っ手２０のピストル型グリップ３６に向かって完全に押して閉鎖リンク１０２が閉鎖ヨーク９６を前進させ、かくして閉鎖管２４がエンドエフェクタ１２を閉じるようにする。遠位側に動かされた閉鎖ヨーク９６は、クランプ係止レバー２３０を受け入れるクランプ係止切欠き２４８を有し、エンドエフェクタ１２をクランプする。発火トリガ３４を多数回行程動作させると、発火トリガ３４に結合された駆動ウェッジ１８２がカム板３３０のカムローブ３４２〜３４４に順次係合することにより発火ロッド３２の発火が行なわれる。このラチェット駆動による回転が回転伝動装置付き発火機構１５０を介して伝達されて中実ラック１７０を遠位側に前進させる。閉鎖ヨーク９６を前進させた状態で、ラック１７０は、発火ロックアウトレバー２５０を押してこれが邪魔にならないようにすることができる。発火行程間では、アンチバックアップ振り子１００をラック１７０と垂直係止接触状態に引っ張り、カム歯車３３０に連結された歯車列を引っ込みばね１７２により与えられる引っ込み力に抵抗する。完全発火移動距離がいったん達成されると、まず最初に解除ボタン３８を押して、アンチバックアップ振り子１００を離脱させ、中実ラック１７０が引っ込んで次にクランプ係止レバー２３０を閉鎖ヨーク９６から外してエンドエフェクタ１２の開放から１つの妨げを取り除く。外科医は、閉鎖トリガ２６を握りしめて発火ロックアウトレバー２５０が閉鎖ヨーク９６を解除できるようにし、次に閉鎖トリガ２６を放し、それにより閉鎖ヨーク９６が中実ラック１７０が発火しないように発火ロックアウトレバー２５０を持ち上げる場所まで近位側に移動することができる。しかる後、外科用ステープル留め及び切断器械１０の作業部分２２を例えば別の作業のための準備に備えてステープルカートリッジ６２を交換するために取り外す。
幾つかの実施形態を説明することにより本発明を説明し、例示の実施形態をかなり詳細に説明したが、特許請求の範囲に記載された本発明の範囲をかかる細部に限定することは本願の意図するところではない。当業者には追加の利点及び改造を容易に想到できよう。
本明細書で用いる「近位側」及び「遠位側」という用語は、器械の取っ手を掴む医師に関してであることは理解されよう。かくして、エンドエフェクタ１２は、近位側の取っ手２０に対して遠位側に位置する。さらに、便宜上且つ分かりやすくするために、例えば「垂直」及び「水平」という空間を表す用語は、図面に関して用いられていることは理解されよう。しかしながら、外科用器械は、多くの向き位置で用いられ、これら用語は、本発明を限定するものでもなく絶対的なものでもない。
本発明を内視鏡手順及び装置に関して説明した。しかしながら、本明細書において例えば「内視鏡」という用語を用いることは、本発明を内視鏡管（即ち、トロカール）と関連してのみ用いられる外科用ステープル留め及び切断器械に限定するものではない。それとは逆に、本発明は、小さな切開部への接近が制限されている任意の手技に利用できると考えられ、かかる手技としては、腹腔鏡手技及び開放手技が挙げられるが、これらには限定されない。
例えば、外科用ステープル留め及び切断器械１０を本明細書においては、別個独立の閉鎖及び発火作動を有するものとして説明したが、それにもかかわらず、これは臨床上の融通性をもたらす。しかしながら、本発明の内容と一致した用途は、単一のユーザの動作を、器械を閉じて発火させる発火動作に変換する取っ手を含むことは理解されるべきである。
加うるに、手動式取っ手を図示したが、モータ駆動式又は違ったやり方の動力式取っ手は、本明細書に記載したようなリンク式ラックを有するとこれにより利点を奏し、取っ手のサイズを減少させることができ又は他の利点をもたらす。例えば、リンク式ラックを部分的にピストル型グリップ内に収納することは有利であるが、リンク相互間のピボット連結部は、リンクをシャフト及び取っ手のバレルにより定められる直線部分に平行に収納できるようにすることは理解されるべきである。
本発明の具体的な実施態様は次の通りである。
（１）外科用器械であって、
発火運動に応動するよう作動的に構成されたエンドエフェクタを有し、
エンドエフェクタに取り付けられたシャフトを有し、シャフトは、エンドエフェクタに結合されていて、発火運動を長手方向に伝達するよう運動自在な細長い発火部材を有し、
シャフトの近位側に取り付けられた取っ手を有し、
取っ手は、周囲の少なくとも一部に沿って複数のカムローブを有するカム板と、発火方向と戻り方向に繰り返し動くことができる発火アクチュエータと、発火アクチュエータに結合されていて、発火方向における発火アクチュエータの各運動中、カム板の複数のカムローブの１つ１つに順次係合するよう作動的に構成されたウェッジと、シャフトの発火部材に連結されていて、カム板の間欠運動を長手方向発火運動として伝達するよう歯車の噛み合いによりカム板に結合されたラックとを有することを特徴とする外科用器械。
（２）ラックは、歯車の噛み合いにより、歯車列で構成されたカム板に結合されていることを特徴とする上記実施態様（１）記載の外科用器械。
（３）歯車列は、カム板の間欠運動をラックの長手方向運動の増大に関連付ける歯車減速組立体を有することを特徴とする上記実施態様（２）記載の外科用器械。
（４）発火アクチュエータの発火行程相互間におけるラックの引っ込みを阻止するよう作動的に構成されたアンチバックアップ機構を更に有することを特徴とする上記実施態様（１）記載の外科用器械。
（５）エンドエフェクタは、閉鎖運動に応動する１対の対向したジョーと、発火部材に応動する発火バーとを有し、シャフトは、閉鎖部材を介して閉鎖運動をエンドエフェクタに伝達するよう作動的に構成され、取っ手は、閉鎖運動を生じさせるよう作動的に構成された閉鎖機構を更に有することを特徴とする上記実施態様（１）記載の外科用器械。
（６）ラックの運動を阻止するよう閉じられていない閉鎖機構に応動する発火ロックアウト機構を更に有することを特徴とする上記実施態様（５）記載の外科用器械。
（７）発火ロックアウト機構は、閉鎖機構の開放運動を阻止するよう少なくとも部分的に発火されたラックに応動するよう作動的に構成されていることを特徴とする上記実施態様（６）記載の外科用器械。
（８）取っ手は、閉鎖機構を係止する閉鎖機構の閉鎖運動に応動するクランプロックを更に有することを特徴とする上記実施態様（７）記載の外科用器械。

（１４）結合部材は、ラックから成ることを特徴とする上記実施態様（１３）記載の外科用器械。
（１５）回転カム部材を回転させると共に複数のステープルの少なくとも一部を突き出すよう回転カム部材の少なくとも１つのカムローブと作動的に係合可能な少なくとも１つの駆動部材を更に有することを特徴とする上記実施態様（１３）記載の外科用器械。
（１６）少なくとも１つの駆動部材に作動的に結合された発火部材を更に有し、発火部材の作動により、ステープルの少なくとも一部が突き出されるようになっていることを特徴とする上記実施態様（１５）記載の外科用器械。
（１７）発火部材の多数回の作動により、ステープル留め組立体から複数のステープルの全てが留められることを特徴とする上記実施態様（１６）記載の外科用器械。
（１８）駆動部材は、付勢部材に作動的に結合され、付勢部材は、作動部材の作動時、駆動部材を付勢してこれを回転カムに係合させることを特徴とする上記実施態様（１６）記載の外科用器械。
（１９）駆動部材は、ばね部材により付勢部材に作動的に結合され、ばね部材は、作動部材の運動に応答して駆動部材を動かすことを特徴とする上記実施態様（１８）記載の外科用器械。
（２０）駆動部材は、ばねにより付勢部材に作動的に結合され、ばね部材は、作動部材の作動に応答してウェッジ部材を動かしてこれを少なくとも１つのカムローブに作動的に係合させることを特徴とする上記実施態様（１８）記載の外科用器械。
（２１）外科用器械であって、
発火運動に応動するよう作動的に構成されたエンドエフェクタを有し、
エンドエフェクタに取り付けられたシャフトを有し、シャフトは、発火運動を長手方向に伝達するよう運動可能にエンドエフェクタに結合された細長い発火部材を有し、
シャフトの近位側に取り付けられた取っ手を有し、取っ手は、発火方向と戻り方向に繰り返し動くことができる発火アクチュエータと、シャフトの発火部材に連結されたラックと、発火アクチュエータからの繰り返し発火方向運動をラックに回転自在に結合し、発火アクチュエータの戻り方向運動中、ラックの現在の位置を維持する手段とを有することを特徴とする外科用器械。
（２２）エンドエフェクタは、発火運動に応動するステープル留め及び切断装置を更に有することを特徴とする上記実施態様（２１）記載の外科用器械。
（２３）ステープル留め及び切断装置は、閉鎖運動に応動する１対の対向したジョーを更に有し、取っ手は、１対の対向したジョーを閉鎖する手段を更に有することを特徴とする上記実施態様（２２）記載の外科用器械。

Claims (12)

  1. 外科用器械であって、
    発火運動に応動するよう作動的に構成されたエンドエフェクタを有し、
    エンドエフェクタに取り付けられたシャフトを有し、シャフトは、エンドエフェクタに結合されていて、発火運動を長手方向に伝達するよう運動自在な細長い発火部材を有し、
    シャフトの近位側に取り付けられた取っ手を有し、
    取っ手は、周囲の少なくとも一部に沿って複数のカムローブを有するカム板と、発火方向と戻り方向に繰り返し動くことができる発火アクチュエータと、発火アクチュエータに結合されていて、発火方向における発火アクチュエータの各運動中、カム板の複数のカムローブの１つ１つに順次係合するよう作動的に構成されたウェッジと、シャフトの発火部材に連結されていて、カム板の間欠運動を長手方向発火運動として伝達するよう歯車の噛み合いによりカム板に結合されたラックとを有することを特徴とする外科用器械。
  2. ラックは、歯車の噛み合いにより、歯車列で構成されたカム板に結合されていることを特徴とする請求項１記載の外科用器械。
  3. 歯車列は、カム板の間欠運動をラックの長手方向運動の増大に関連付ける歯車減速組立体を有することを特徴とする請求項２記載の外科用器械。
  4. 発火アクチュエータの発火行程相互間におけるラックの引っ込みを阻止するよう作動的に構成されたアンチバックアップ機構を更に有することを特徴とする請求項１記載の外科用器械。
  5. エンドエフェクタは、閉鎖運動に応動する１対の対向したジョーと、発火部材に応動する発火バーとを有し、シャフトは、閉鎖部材を介して閉鎖運動をエンドエフェクタに伝達するよう作動的に構成され、取っ手は、閉鎖運動を生じさせるよう作動的に構成された閉鎖機構を更に有することを特徴とする請求項１記載の外科用器械。
  6. ラックの運動を阻止するよう閉じられていない閉鎖機構に応動する発火ロックアウト機構を更に有することを特徴とする請求項５記載の外科用器械。
  7. 発火ロックアウト機構は、閉鎖機構の開放運動を阻止するよう少なくとも部分的に発火されたラックに応動するよう作動的に構成されていることを特徴とする請求項６記載の外科用器械。
  8. 取っ手は、閉鎖機構を係止する閉鎖機構の閉鎖運動に応動するクランプロックを更に有することを特徴とする請求項７記載の外科用器械。
  9. 取っ手は、ラックの運動を阻止するよう閉鎖されていない閉鎖機構に応動すると共に閉鎖機構の開放を阻止するよう少なくとも部分的に発火されたラックに応動する発火ロックアウト機構と、閉鎖機構を係止するよう閉鎖機構の閉鎖運動に応動するクランプロック機構と、発火トリガの発火行程相互間におけるラックの引っ込みを阻止するよう作動的に構成されたアンチバックアップ機構と、アンチバックアップ機構を係止解除するよう作動的に構成された解除機構とを更に有することを特徴とする請求項５記載の外科用器械。
  10. 解除機構は、クランプロック機構を係止解除するよう更に作動的に構成されていることを特徴とする請求項９記載の外科用器械。
  11. 外科用器械であって、
    取っ手を有し、
    取っ手から見て遠位側に設けられたステープル留め組立体を有し、ステープル留め組立体は、複数のステープル及びステープル形成表面を備えたアンビルを有し、ステープル形成表面は、複数のステープルから間隔を置いて位置する開放位置から複数のステープルに隣接した閉鎖位置に動くことができ、
    ステープル留め組立体に作動的に結合されたステープル留め機構を有し、ステープル留め機構は、少なくとも１つのカムローブを有する回転カム部材と、回転カム部材の回転によりステープル留め組立体からのステープルの少なくとも一部が留められるよう回転可能部材及びステープル留め組立体と作動的に関連した少なくとも１つの結合部材とを有することを特徴とする外科用器械。
  12. 外科用器械であって、
    発火運動に応動するよう作動的に構成されたエンドエフェクタを有し、
    エンドエフェクタに取り付けられたシャフトを有し、シャフトは、発火運動を長手方向に伝達するよう運動可能にエンドエフェクタに結合された細長い発火部材を有し、
    シャフトの近位側に取り付けられた取っ手を有し、取っ手は、発火方向と戻り方向に繰り返し動くことができる発火アクチュエータと、シャフトの発火部材に連結されたラックと、発火アクチュエータからの繰り返し発火方向運動をラックに回転自在に結合し、発火アクチュエータの戻り方向運動中、ラックの現在の位置を維持する手段とを有することを特徴とする外科用器械。
