본 발명은 내시경 시술 및 장치의 견지에서 설명되었다. 그러나 "내시경"과 본원에서의 용어의 사용은 본 발명을 단지 내시경 튜브(즉, 투관침)와 결합하는 사용을 위한 외과용 스테이플링 및 절개 도구에 한정하는 것으로 해석되어서는 안 된다. 반대로, 본 발명은 접근이 한정적인 것은 아니지만 복강경 시술, 뿐만 아니라 절개 시술을 포함하는 소형 절개에 한정되는 임의의 시술에서의 사용을 발견할 수 있는 것으로 고려된다.
예를 들면, 외과용 스테이플링 및 절개 도구(10)가 본원에 개별 및 별개의 폐쇄 및 발사 작동을 유리하게 갖는 것으로 설명되었지만, 본 발명과 일치하는 적용은 도구를 폐쇄하고 발사하는 발사 운동으로 단일의 사용자 작동을 변환하는 핸 들을 포함할 수 있다는 것이 이해되어야 한다.
부가적으로, 수동 작동식 핸들이 예시되었지만, 모터식 또는 다른 방식의 동력식 핸들이 본원에 설명된 바와 같은 링크형 래크를 구비하여 핸들의 크기의 감소 또는 다른 이점을 허용하는 것으로부터 장점을 가질 수 있다. 예를 들면, 피스톨 파지부로의 링크형 래크의 부분적인 도입이 편리하지만, 링크 사이의 피벗 접속은 링크를 샤프트 및 핸들의 배럴(barrel)에 의해 규정된 직선부에 평행하게 도입시키는 것을 허용한다.
본 발명은 발사 부재의 백업을 방지하면서 다중 발사 스트로크의 시퀀스를 전달하는 회전 전동을 갖는 수술용 스테이플링 및 절개 도구를 제공함으로써 종래 기술의 상술한 및 다른 결점을 극복한다. 이에 의해 증가된 발사력 및/또는 증가된 발사 이동을 필요로 하는 도구의 단부 이펙터가 다중의 스트로크 발사 트리거에 의해 즉시 발사될 수 있다. 또한, 도구는 단부 이펙터를 발사할 필요 없이 외과 의사가 조직을 위치시킬 수 있게 하는 개별 폐쇄 능력을 갖는다.

Claims (23)

  1. 수술용 도구에 있어서,
    발사 운동에 응답하도록 작동적으로 구성된 단부 이펙터(end effector)와;
    상기 단부 이펙터에 부착되고 상기 발사 운동을 종방향으로 전달하도록 운동을 위해 상기 단부 이펙터에 결합된 세장형 발사 부재(elongate fifing member)를 포함하는 샤프트(shaft); 및
    상기 샤프트에 근위측에 부착된 핸들(handle)을 포함하고,
    상기 핸들은,
    원주의 적어도 일부 둘레에 복수의 캠 로브(cam lobes)를 포함하는 캠 디스크(disk)와,
    발사 방향 및 복귀 방향으로 반복적으로 이동 가능한 발사 액추에이터(actuator)와,
    상기 발사 액추에이터에 결합되고 상기 발사 방향으로의 발사 액추에이터의 각각의 이동 중에 상기 캠 디스크의 복수의 캠 로브 중 각각의 하나와 순차적으로 결합하도록 작동적으로 구성된 웨지(wedge)와,
    상기 샤프트의 발사 부재에 연결되고 종방향 발사 운동으로서 상기 캠 디스크의 간헐적인 회전을 전달하는 캠 디스크와 기어 결합에 의해 결합된 래크(rack)를 포함하는 수술용 도구.
  2. 제1 항에 있어서, 상기 래크는 기어열(gear train)로 구성된 캠 디스크와 기어 결합에 의해 결합되는 수술용 도구.
  3. 제2 항에 있어서, 상기 기어열은 상기 래크의 증가된 종방향 운동으로 상기 캠 디스크의 간헐적인 회전을 관련시키는 기어 감소 조립체(gear reduction assembly)를 포함하는 수술용 도구.
  4. 제1 항에 있어서, 상기 발사 트리거(trigger)의 발사 스트로크 사이에 래크의 수축을 방지하도록 작동적으로 구성된 백업 방지 기구(anti-backup mechanism)를 추가로 포함하는 수술용 도구.
  5. 제1 항에 있어서, 상기 단부 이펙터는 폐쇄 운동에 응답하는 한 쌍의 대향 조(opposing jaws) 및 상기 발사 부재에 응답하는 발사 바아(bar)를 포함하고, 상기 샤프트는 상기 단부 이펙터에 폐쇄 부재를 통해 폐쇄 운동을 전달하도록 작동적으로 구성되고, 상기 핸들은 폐쇄 운동을 생성하도록 작동적으로 구성된 폐쇄 기구를 추가로 포함하는 수술용 도구.
  6. 제5 항에 있어서, 상기 래크의 이동을 방지하도록 미폐쇄된 개방 기구에 응답하는 발사 로크아웃(lockout) 기구를 추가로 포함하는 수술용 도구.
  7. 제6 항에 있어서, 상기 발사 로크아웃 기구는 상기 폐쇄 기구의 개방 이동을 차단하도록 적어도 부분적으로 발사되어 있는 래크에 응답하도록 작동적으로 구성되는 수술용 도구.
  8. 제7 항에 있어서, 상기 핸들은 상기 폐쇄 기구를 체결하도록 폐쇄 기구의 폐쇄 운동에 응답하는 클램프 체결부(clamp lock)를 추가로 포함하는 수술용 도구.
  9. 제5 항에 있어서, 상기 핸들은,
    상기 래크의 이동을 방지하도록 미폐쇄된 폐쇄 기구에 응답하고 상기 폐쇄 기구의 개방을 방지하도록 적어도 부분적으로 발사되어 있는 래크에 응답하는 발사 로크아웃 기구와;
    상기 폐쇄 기구를 체결하도록 폐쇄 기구의 폐쇄 운동에 응답하는 클램프 체결 기구와;
    상기 발사 트리거의 발사 스트로크 사이에 래크의 수축을 방지하도록 작동적으로 구성된 백업 방지 기구; 및
    상기 백업 방지 기구를 체결 해제하도록 작동적으로 구성된 해제(release) 기구를 추가로 포함하는 수술용 도구.
  10. 제9 항에 있어서, 상기 해제 기구는 상기 클램프 체결 기구를 체결 해제하도록 또한 작동적으로 구성되는 수술용 도구.
  11. 제5 항에 있어서, 상기 단부 이펙터는 발사 중에 조직을 스테이플(staple)하고 절개하도록 작동적으로 구성되는 수술용 도구.
  12. 제1 항에 있어서, 상기 핸들은 피스톨 파지부(pistol grip)를 추가로 포함하고, 상기 래크는 수축시에 상기 피스톨 파지부에 도입 가능한 만곡 가능한 래크를 포함하는 수술용 도구.
  13. 수술용 도구에 있어서,
    핸들과;
    상기 핸들로부터 원위측에 있고, 복수의 스테이플 및 상기 복수의 스테이플로부터 이격된 개방 위치로부터 상기 복수의 스테이플에 인접한 폐쇄 위치로 이동 가능한 스테이플 형성 표면을 갖는 앤빌(anvil)을 포함하는 스테이플 적용 조립체와;
    상기 스테이플 적용 조립체에 작동적으로 결합되고, 적어도 하나의 캠 로브를 갖는 회전 캠 부재 및 상기 회전 캠 부재의 회전이 상기 스테이플 적용 조립체로부터 상기 스테이플의 적어도 일부의 적용을 초래하도록 상기 회전 캠 부재 및 스테이플 적용 조립체와 작동적으로 결합되는 적어도 하나의 커플링(coupling) 부재를 포함하는 스테이플링 적용 기구를 포함하는 수술용 도구.
  14. 제13 항에 있어서, 상기 커플링 부재는 래크를 포함하는 수술용 도구.
  15. 제13 항에 있어서, 상기 회전 캠 부재를 회전시키고 상기 복수의 스테이플의 적어도 일부를 배출하도록 상기 회전 캠 부재의 적어도 하나의 로브와 작동적으로 결합 가능한 적어도 하나의 구동 부재를 추가로 포함하는 수술용 도구.
  16. 제15 항에 있어서, 상기 발사 부재의 작동이 상기 스테이플의 적어도 일부의 배출을 초래하도록 적어도 하나의 구동 부재에 작동적으로 결합된 발사 부재를 추가로 포함하는 수술용 도구.
  17. 제16 항에 있어서, 상기 발사 부재의 다중의 작동은 상기 스테이플 적용 조립체로부터 복수의 스테이플 모두를 적용하는 수술용 도구.
  18. 제16 항에 있어서, 상기 구동 부재는 바이어스(bias) 부재에 작동적으로 결합되고, 상기 바이어스 부재는 상기 작동 부재의 작동시에 회전 캠과 결합하여 상기 구동 부재를 바이어스시키는 수술용 도구.
  19. 제18 항에 있어서, 상기 구동 부재는 스프링 부재에 의해 바이어스 부재에 작동적으로 결합되고, 상기 스프링 부재는 상기 작동 부재의 이동에 응답하여 상기 구동 부재를 이동시키는 수술용 도구.
  20. 제18 항에 있어서, 상기 구동 부재는 스프링에 의해 바이어스 부재에 작동적으로 결합되고, 상기 스프링은 상기 작동 부재의 작동에 응답하여 적어도 하나의 캠 로브와 작동 결합되도록 웨지 부재를 이동시키는 수술용 도구.
  21. 수술용 도구에 있어서,
    발사 운동에 응답하도록 작동적으로 구성된 단부 이펙터와;
    상기 단부 이펙터에 부착되고 상기 발사 운동을 종방향으로 전달하도록 운동을 위해 상기 단부 이펙터에 결합된 세장형 발사 부재를 포함하는 샤프트; 및
    상기 샤프트에 근위측에 부착된 핸들을 포함하고,
    상기 핸들은,
    발사 방향 및 복귀 방향으로 반복적으로 이동 가능한 발사 액추에이터와,
    상기 샤프트의 발사 부재에 연결된 래크, 및
    상기 발사 액추에이터로부터의 반복된 발사 방향 운동을 래크에 회전 결합하고 상기 발사 액추에이터의 복귀 방향 운동 중에 래크의 현재 위치를 유지하기 위한 수단을 포함하는 수술용 도구.
  22. 제21 항에 있어서, 상기 단부 이펙터는 발사 운동에 응답하는 스테이플링 및 절개(severing) 장치를 추가로 포함하는 수술용 도구.
  23. 제22 항에 있어서, 상기 스테이플링 및 절개 장치는 폐쇄 운동에 응답하는 한 쌍의 대향 조를 추가로 포함하고, 상기 핸들은 상기 한 쌍의 대향 조를 폐쇄하기 위한 수단을 추가로 포함하는 수술용 도구.
