Abstract
Description
本願は、2001年12月6日付けでファイルされた米国仮特許出願第60/338,420号の特典と優先権を主張するものである。
本発明は、一般に、整形外科のインプラントの分野に関する。具体的には、本発明は、拡散硬化酸化層の薄くて高密度で耐摩耗性の高いコーティングでコートされたテクスチャ改質表面を有する整形外科のインプラントを目的とする。好ましくは、テクスチャ改質は、化学的または電気化学的エッチングによって行われ、そして金属製インプラントはジルコニウムを含み、表面層は酸化ジルコニウムを含む。表面コーティングは骨の内方増殖を促進する性能が増大されている。また、本発明は、テクスチャ改質表面を有する金属製の整形外科のインプラントの製造方法に関する。
整形外科のインプラント材料は、高い強度、耐食性および組織適合性を兼ね備えていなければならない。特にインプラントの受容者が比較的若い場合には、患者の全生涯にわたってインプラントが機能することが望ましいので、インプラントの寿命が最も重要である。特定の金属合金は、必要な機械的強度と生体適合性を有するので、人工補装器具の作製に理想的な候補である。これらの合金としては、316Lステンレス鋼、クロム-コバルト-モリブデン合金、そして最近では荷重支持人工補装器具の作製に最も適切な材料であることが証明されている、チタン合金が挙げられる。
本発明は、基板材料のテクスチャ加工表面と酸化層コーティング、およびこのようなテクスチャ加工表面と酸化層コーティングの人工補装器具を目的とするものである。
具体的な実施態様では、人工補装器具は脊椎の人工補装器具である。好ましくは、脊椎の人工補装器具は、ジルコニウムまたはジルコニウム合金を含み、拡散硬化酸化層は、濃い藍色または黒色の酸化されたジルコニウムの層である。
別の具体的な実施態様においては、脊椎の人工補装器具は、脊椎板(spinal disc)の人工補装器具である。好ましくは、脊椎板の人工補装器具は、ジルコニウムまたはジルコニウム合金を含み、拡散硬化酸化層は、濃い藍色または黒色の酸化されたジルコニウムの層である。

典型的な股関節人工補装器具を、図1および2に示す。図1に示すように、股関節幹状部2が大腿骨に嵌合し、人工補装器具の大腿骨頭6が、骨盤に取り付けられている寛骨臼杯10の内張り8に嵌合して関節をなす。多孔質金属製のビーズまたはワイヤメッシュのコーティング12を組み入れて、該多孔質コーティング内への周囲組織の内方増殖によって、インプラントの固定を促進することができる。同様に、このような多孔質金属のビーズまたはワイヤメッシュのコーティングは、寛骨臼の要素にも用い得る。重要なことに、領域12は、多孔質金属のビーズまたはワイヤメッシュのコーティングと同じ目的を達成するのに、テクスチャを改質された領域からなる。
本発明は、先に教示した現場で酸化された拡散硬化層と、金属基板とテクスチャを改質された表面とを有する整形外科の金属製インプラントまたは人工補装器具を提供する。好ましくは、金属基板はジルコニウムまたはジルコニウム合金であり、酸化物層は濃い藍色または青色の酸化されたジルコニウムで構成された拡散硬化層である。限定されないが、ハフニウム、ニオブおよびタンタルならびにそれらの合金のようなその他の金属基板は、本発明の酸化層を形成するのに用い得る。以下の考察では、焦点はジルコニウムとジルコニウム合金であるが、本発明はそれに限定されない。

我々は、各種表面、すなわち1)テクスチャを改質された拡散硬化酸化表面、2)テクスチャは改質されているが酸化されていない表面、および3)酸化もテクスチャの改質もされていない表面の、骨セメントに対するせん断強度を比較する実験を行った。この試験の結果は、同様にテクスチャ加工された酸化されていない表面に比較して、酸化されたテクスチャ加工表面が、実施的に向上された、骨セメントに対する平均せん断強度を有することを示した。
この試験では、羊の動物モデルを使用して、これらマクロテクスチャ加工された酸化ジルコニウムの表面に対する生体内の生物学的応答、およびそれら表面が与えるせん断強度を測定した。商業的に知られているテクスチャ加工法のChemTex (登録商標)5-5-5 (CYCAM,Inc.、Houston、PA)と、商業的に知られている新たに開発された化学的テクスチャ加工プロセスのTecotex (登録商標)I-103 (Tecomet、Woburn、MA)とを選択して、ジルコニウム合金(Zr-2.5Nb)上にマクロテクスチャ加工表面(Rmax>0.4mm)を得た。続いて、これらテクスチャ加工表面を酸化して、主として単斜晶のジルコニアからなる全表面上に約5μmの厚さで均一に硬質セラミック層を形成させた。

Claims (40)
- 金属基板の表面の少なくとも一部のテクスチャを改質し;そして
金属基板の表面の少なくとも一部を酸化して、該金属基板に拡散硬化表面を形成する
工程を含む、金属基板に改質表面を製造する方法。 - 改質する工程が、化学的エッチングまたは電気化学的エッチングを特徴とする請求項1に記載の方法。
- エッチングが、酸を用いるエッチングを特徴とする請求項2に記載の方法。
- エッチングが、マスキング剤を金属基板の表面に塗布するさらなる工程を特徴とする請求項2に記載の方法。
- マスキング剤がランダムに塗布される、請求項4に記載の方法。
- マスキング剤を塗布する工程が、マスキング剤を金属基板の表面にスプレーするか、またはスパッターする工程を特徴とする請求項4に記載の方法。
- スプレーするか、またはスパッターする工程が、マスキング剤のランダム塗布を特徴とする請求項6に記載の方法。
- マスキング剤を金属基板の表面に塗布する工程が、表面をマスキング剤で完全に被覆し、その後、該マスキング剤の部分を部分的に除くことを特徴とする請求項4に記載の方法。
- 部分的に除く工程が、マスキング剤の部分のレーザーアブレーションの工程を特徴とする、請求項8に記載の方法。
- 部分的に除く工程が、マスキング剤の部分を機械的に除く工程を特徴とする、請求項8に記載の方法。
- 改質する工程が、機械的エッチングを特徴とする請求項1に記載の方法。
- 改質する工程が、物質の表面への沈着を特徴とする請求項1に記載の方法。
- 沈着工程が、化学蒸着を特徴とする請求項12に記載の方法。
- 酸化する工程が、空気、水蒸気または水による酸化プロセスを特徴とする請求項1に記載の方法。
- 酸化する工程が、酸化剤としての酸素の使用を特徴とする請求項1に記載の方法。
- 酸化する工程が、塩浴の使用を含む請求項1に記載の方法。
- 金属の表面が、ジルコニウムまたはジルコニウム合金である請求項1に記載の方法。
- ジルコニウム合金が、
約4.5重量%までのハフニウムおよび約3.0重量%までのニオブを含有するジルコニウム;
約4.5重量%までのハフニウムを含有するジルコニウム;
2.5〜2.8重量%のニオブを含有するジルコニウム;および
約13重量%のニオブおよび約13重量%のジルコニウムを含有するチタン
からなる群から選択される請求項17に記載の方法。 - 金属表面が、ハフニウム、ニオブ、タンタルおよびチタンからなる群から選択される金属を含む、請求項1に記載の方法。
- 第一人工補装器具部分が支持表面を有し、該支持表面が第二人工補装器具部分上の第二支持表面と係合または協働するサイズと形状であり;そして
第一人工補装器具部分もしくは第二人工補装器具部分または両者の表面の少なくとも一部が、請求項1〜19のいずれかに記載の方法によりテクスチャが改質され;そして
第一人工補装器具部分もしくは第二人工補装器具部分または両者の少なくとも一部が拡散硬化酸化層を有する、
第一人工補装器具部分と第二人工補装器具部分を含む移植用人工補装器具。 - 第一人工補装器具部分が、少なくとも一つの顆頭を含む支持表面を有することをさらに特徴とする大腿骨要素であり、そして第二人工補装器具部分が、脛骨底であることをさらに特徴とする脛骨要素であり、該脛骨要素が支持表面と協働するようにされている請求項20に記載の人工補装器具。
- 脛骨要素が、有機ポリマーまたはポリマーベースの複合体を含む、請求項21に記載の人工補装器具。
- 金属製人工補装器具の本体が、ジルコニウムまたはジルコニウム合金を含み、拡散硬化酸化層が、濃い藍色または黒色の酸化されたジルコニウムのコーティングである、請求項21に記載の人工補装器具。
- 拡散硬化酸化層が、約20ミクロンまでの厚さである請求項23に記載の人工補装器具。
- 拡散硬化酸化層が、約10ミクロンまでの厚さである請求項23に記載の人工補装器具。
- 第一人工補装器具部分が、ヘッド部分および該ヘッド部分上の支持表面を有する大腿骨要素をさらに特徴とし、そして第二人工補装器具部分が、ヘッド部分上の該支持表面と協働するようにされている内表面を有する寛骨臼杯をさらに特徴とする、請求項20に記載の人工補装器具。

- 拡散硬化酸化層が、約10ミクロンまでの厚さである請求項28に記載の人工補装器具。
- 人工補装器具が脊椎の人工補装器具である、請求項20に記載の人工補装器具。
- 脊椎の人工補装器具が、ジルコニウムまたはジルコニウム合金を含み、拡散硬化酸化層が、濃い藍色または黒色の酸化されたジルコニウムの層である請求項31に記載の人工補装器具。
- 脊椎の人工補装器具が、脊椎板の人工補装器具である請求項31に記載の人工補装器具。
- 脊椎板の人工補装器具が、ジルコニウムまたはジルコニウム合金を含み、拡散硬化酸化層が、濃い藍色または黒色の酸化されたジルコニウムの層である請求項33に記載の人工補装器具。
- 要素の表面の少なくとも一部が請求項1〜19のいずれかに記載の方法でテクスチャを改質され、そしてさらに該要素の表面の少なくとも一部が拡散硬化酸化層を含む、該要素を含み患者の身体組織に挿入される医療用インプラント。
- 骨板および骨ねじからなる群から選択される、請求項35に記載の医療用インプラント。
