Abstract
Description
結び目)の適切な配置を決定するためのリーフレット250の高さ調節、リーフレット250への縫合糸252の結合は、適切なリーフレットの高さを調節するためにリーフレットを通り、鉗子によって上方向に保持された追加の縫合糸によって遂行された。本発明に関して、輪状形成術用人工器官204を下方向に移動させ、弁環に縫合した後、ノット254が締結される適切なポイントを決定するガイドとして縫合糸206を使用し、リーフレット250が隣接するリーフレット251と適切に癒合することを保証する。
Claims (40)
- 輪状形成術用システムであって、
上面、下面、及び心臓弁の環の構成に概ね対応する周方向表面を有する第1ホルダ構成要素と、
周方向表面に隣接して配置され、少なくとも部分的にその周囲に延在する輪状形成術用人工器官と、
ホルダに装着され、人工器官内に延在し、これを周方向表面の隣に保持する、下方向に延在する少なくとも1つの剛性貫入部材とを備えたシステム。 - 前記人工器官は輪状形成術用可撓性人工器官である、請求項1に記載のシステム。
- 前記周方向表面は、人工器官の少なくとも一部が配置された溝を備える、請求項1に記載のシステム。
- 前記周方向表面は、人工器官の下面に並んで延在し、人工器官が下方向に移動して、貫入部材から離れることを防止する少なくとも1つの外側に延在する部材を備える、請求項1に記載のシステム。
- 前記外側に延在する部材は、下方向に延在する部材の下に配置される、請求項4に記載のシステム。
- 前記少なくとも1つの下方向に延在する部材は、複数の下方向に延在する部材を備え、前記少なくとも1つの外側に延在する部材は、貫入部材のそれぞれ1つの下に位置した複数の外側に延在する部材を備える、請求項4に記載のシステム。
- 前記貫入部材は、外側に延在する部材内に延在し、人工器官が下方向に移動して、貫入部材から離れることを防止する、請求項4に記載のシステム。
- 前記少なくとも1つの下方向に延在する部材は、複数の貫入部材を備え、前記少なくとも1つの外側に延在する部材は、複数の外側に延在する部材を備え、この中に貫入部材がそれぞれ1つずつ延在する、請求項4に記載のシステム。
- 前記少なくとも1つの下方向に延在する部材は、複数の貫入部材を備え、前記少なくとも1つの外側に延在する部材は、周方向表面の一部に延在し、複数の貫入部材の下に配置される、請求項4に記載のシステム。
- 前記ホルダは、第1ホルダ構成要素に結合され、その上に延在する第2ホルダ構成要素を備える、請求項1に記載のシステム。

- 前記第2ホルダ構成要素は、貫入部材を人工器官から引き出すために、第1ホルダ構成要素に対して上方向に移動可能である、請求項11に記載のシステム。
- 前記第2ホルダ構成要素は、第1ホルダ構成要素の隣に着脱式に固定され、第2ホルダ構成要素の上方向の移動を一時的に防止する、請求項12に記載のシステム。
- 前記第2ホルダは、切断可能な縫合糸によって第1ホルダ構成要素に着脱式に固定される、請求項12に記載のシステム。
- 前記第2ホルダ構成要素は第1ホルダ構成要素から完全に取り外すことができる、請求項12に記載のシステム。
- 前記第2ホルダ構成要素が上方向に移動した後、第2ホルダ構成要素を第1ホルダ構成要素に保持する手段を備える、請求項12に記載のシステム。
- 前記第1ホルダ構成要素は、減少した周を呈するために変形可能である、請求項12に記載のシステム。
- 前記第2ホルダは、第2ホルダ構成要素が上方向に移動するまで第1ホルダ構成要素の変形を防止する手段を備える、請求項17に記載のシステム。
- 前記少なくとも1つの貫入部材は第1ホルダ構成要素に装着される、請求項10に記載のシステム。

上面、下面、及び心臓弁の環の構成に概ね対応する周方向表面を有する第1ホルダ構成要素を備え、第1ホルダ構成要素が更に、減少した周を呈するために変形可能である少なくとも1つの外側に延在する部材を備え、更に、
外側に延在する部材が人工器官の下面と並んで延在し、人工器官が下方向に移動して、第1ホルダ構成要素から離れることを防止するよう、周方向表面に隣接して配置され、少なくとも部分的にその周囲に延在する輪状形成術用人工器官と、
第1ホルダ構成要素に結合され、その上に延在し、第1ホルダ構成要素に対して上方向に移動可能であり、自身が上方向に移動するまで第1ホルダ構成要素の変形を防止する手段を備える第2ホルダ構成要素とを備えたシステム。 - 前記第2ホルダ構成要素は、第1ホルダ構成要素の隣に着脱式に固定される、請求項20に記載のシステム。
- 前記第2ホルダ構成要素は、切断可能な縫合糸によって第1ホルダ構成要素の隣に着脱式に固定される、請求項21に記載のシステム。
- 前記第1ホルダ構成要素は、隔置された端部を伴う開放リングであり、前記第2ホルダ構成要素は、隔置された端部間に配置可能で、隔置された端部の相互に向かう移動及び第1ホルダ構成要素の周の減少を防止するように配置される突起を備える、請求項20に記載のシステム。
- 前記突起は第2ホルダ構成要素から下方向に延在し、第2ホルダ構成要素が上方向に移動すると、隔置された端部間の位置を出る、請求項23に記載のシステム。
- 前記少なくとも1つの外側に延在する部材は、周方向表面に沿って配置された一連の隔置突起を備える、請求項20に記載のシステム。
- 前記少なくとも1つの外側に延在する部材は、周方向表面に沿って延在する1つの細長い突起を備える、請求項20に記載のシステム。
- 前記少なくとも1つの外側に延在する突起は、周方向表面に沿って延在する溝の下縁を備える、請求項20に記載のシステム。
- 輪状形成術用システムであって、
上面、下面、及び心臓弁の環の構成に概ね対応する周方向表面を有する第1ホルダ構成要素を備え、第1ホルダ構成要素が少なくとも1つの外側に延在する部材及び縫合糸切断器を備え、更に、
外側に延在する部材が人工器官の下面と並んで延在し、人工器官が下方向に移動して、第1ホルダ構成要素から離れることを防止するよう、周方向表面に隣接して配置され、少なくとも部分的にその周囲に延在する輪状形成術用人工器官と、
周方向表面の隣で帯を保持し、縫合糸切断器を通る切断可能な縫合糸とを備えたシステム。 - 前記第1ホルダ構成要素は、減少した周を呈するために変形可能である、請求項28に記載のシステム。
- 前記切断器は、第1ホルダ構成要素の変形に応答して縫合糸を切断する手段を備える、請求項29に記載のシステム。
- 前記第1ホルダ構成要素は、隔置された端部を伴う開放リングであり、前記切断器は隔置された端部に結合され、隔置された端部の相互に対する動作に応答して縫合糸を切断する手段を備える、請求項29に記載のシステム。
- 更に、第1ホルダ構成要素に結合され、その上に延在し、第1ホルダ構成要素に対して上方向に移動可能であり、自身が上方向に移動するまで隔置された端部が相互に向かって移動することを防止する手段を備える第2ホルダ構成要素を備える、請求項31に記載のシステム。
- 前記第2ホルダ構成要素は、隔置された端部間に配置可能で、隔置された端部が相互に向かって移動すること、及び第1ホルダ構成要素の周が減少することを防止する突起を備える、請求項32に記載のシステム。

外側に延在する部材が人工器官の下面と並んで延在するよう、周方向表面の隣に配置され、その周囲に少なくとも部分的に延在する輪状形成術用人工器官を備えるシステム。 - 前記第1ホルダ構成要素は、隔置された端部の相互に向かう動作を容易にするため、隔置されたリング端部の端部に配置された係合手段を備える、請求項35に記載のシステム。
- 前記係合手段は、開放リングの内側に延在する突起を備える、請求項36に記載のシステム。
- 更に、第1ホルダ構成要素に結合され、その上に延在し、第1ホルダ構成要素に対して上方向に移動可能であり、自身が上方向に移動するまで隔置された端部が相互のホルダ構成要素に向かって移動することを防止する手段を備える第2ホルダ構成要素を備える、請求項35に記載のシステム。
- 前記第2ホルダ構成要素は、隔置された端部の相互に対する動作、及び第1ホルダ構成要素の周の減少を防止するため、隔置された端部間に配置可能である突起を備える、請求項38に記載のシステム。
- 前記突起は第2ホルダ構成要素から下方向に延在し、第2ホルダ構成要素が上方向に移動すると、隔置された端部の間の位置から出る、請求項39に記載のシステム。
