Description
Claims (20)
- 縫合糸を担持して患者の拍動している心臓の弁尖に前記縫合糸を挿入することによって心臓弁を修復するように構成されている縫合糸取付カテーテルであって、
近位端、遠位端および100cmを超える長さを有する全体的に可撓性のカテーテルボディと、
前記縫合糸を前記弁尖に案内するための、前記カテーテルボディの前記遠位端の近くの縫合糸取付アセンブリであって、前記縫合糸取付アセンブリが、前記カテーテルボディの前記遠位端に設けられている近位クランプ顎部と、近位部および遠位部を有し、前記近位部が前記近位クランプ顎部に対して選択的に長手方向に摺動可能であるように構成されるレールと、前記レールの前記遠位部に蝶着されている遠位クランプ顎部とを含む、縫合糸取付アセンブリと、
前記縫合糸を前記弁尖に挿入するように構成される針と、
前記カテーテルボディの前記近位端に操作可能に取り付けられている制御ハンドルであって、前記制御ハンドルが、前記近位クランプ顎部に対して前記レールを選択的に長手方向に摺動させるように構成されるレールアクチュエータと、前記縫合糸取付アセンブリを心臓に送達するための第1の位置と、前記近位クランプ顎部と前記遠位クランプ顎部との間に弁尖を捕捉するための第2の位置との間で、前記遠位クランプ顎部を選択的に枢動させるように構成される顎部アクチュエータとを含む、制御ハンドルと、
前記顎部アクチュエータから前記カテーテルボディを通って、前記遠位クランプ顎部まで延びている可撓性部材であって、前記顎部アクチュエータが前記可撓性部材を選択的に動かして、前記遠位クランプ顎部を枢動させる、可撓性部材と
を備える、縫合糸取付カテーテル。 - 前記制御ハンドルに制御ノブをさらに備え、
前記制御ノブは、前記レールアクチュエータおよび前記顎部アクチュエータの両方を制御するように構成される、請求項1に記載の縫合糸取付カテーテル。 - 前記制御ノブは、前記レールアクチュエータを制御するために長手方向に動かされ、かつ前記顎部アクチュエータを制御するために回転させられるように構成される、請求項2に記載の縫合糸取付カテーテル。
- 前記可撓性部材は、前記遠位クランプ顎部のハウジングの中まで延びている、請求項1に記載の縫合糸取付カテーテル。
- 前記可撓性部材はワイヤを備える、請求項1に記載の縫合糸取付カテーテル。
- 前記近位クランプ顎部から延びているワイヤループをさらに備え、
前記ワイヤループは、前記近位クランプ顎部の表面積に対して前記弁尖を補足するための表面積を増やすために、前記近位クランプ顎部と連結するように構成される、請求項1に記載の縫合糸取付カテーテル。 - 前記ワイヤループは、折りたたまれた位置から、前記近位クランプ顎部が心臓内で送達カテーテルから出て延ばされる拡張位置まで自動的に移行するように構成される、請求項6に記載の縫合糸取付カテーテル。
- 前記遠位クランプ顎部は、張力下で縫合糸を保持するように構成される縫合糸配糸ポストを含む、請求項1に記載の縫合糸取付カテーテル。
- 前記遠位クランプ顎部の周縁は、前記遠位クランプ顎部と前記近位クランプ顎部との間の弁尖の保持を高めるように構成される複数の段付き歯を画定する、請求項1に記載の縫合糸取付カテーテル。
- 前記近位クランプ顎部は、前記遠位クランプ顎部と前記近位クランプ顎部との間の弁尖の保持を高めるように構成される複数の段付き歯を画定する、請求項9に記載の縫合糸取付カテーテル。
- 前記制御ハンドルから前記近位クランプ顎部まで延びている1本または複数本の光ファイバーケーブルをさらに備え、
前記近位クランプ顎部は、前記近位クランプ顎部のクランプ面に開口部を含むことによって、前記1本または複数本の光ファイバーケーブルにより前記近位クランプ顎部と前記遠位クランプ顎部との間の弁尖の捕捉を確認可能である、請求項1に記載の縫合糸取付カテーテル。 - 前記針は、前記カテーテルボディ内で選択的に摺動可能であり、
前記針は、弁尖が前記近位クランプ顎部と前記遠位クランプ顎部との間に捕捉されるときに、前記近位クランプ顎部から延びて弁尖に侵入し、弁尖を通して縫合糸を挿入するように構成される、請求項1に記載の縫合糸取付カテーテル。 - 前記制御ハンドルに設けられている針リリース部をさらに備え、
前記針は、前記針リリース部が作動するまでは、前記近位クランプ顎部から延びることが防止される、請求項12に記載の縫合糸取付カテーテル。 - 前記針によって手繰り寄せるために前記遠位クランプ顎部に保持されるように構成される縫合糸と、
前記制御ハンドルに設けられている縫合糸張力付与アセンブリとをさらに備え、
前記縫合糸張力付与アセンブリは、前記縫合糸に張力を加えて、前記縫合糸を張力下で前記遠位クランプ顎部に保持可能に構成される、請求項12に記載の縫合糸取付カテーテル。 - 前記ハンドルに設けられている縫合糸リリースピンをさらに備え、
前記縫合糸リリースピンは、前記縫合糸の張力をリリースして、前記縫合糸を前記遠位クランプ顎部から手繰り寄せることが可能なように構成される、請求項14に記載の縫合糸取付カテーテル。 - 前記縫合糸取付アセンブリは、前記カテーテルボディよりも可撓性が小さい、請求項1に記載の縫合糸取付カテーテル。
- 前記縫合糸取付カテーテルは、心臓の右心房への血管アクセスおよび右心房と左心房との間の経中隔アクセスを介した、患者の心臓の左心房への挿入用に構成される、請求項1に記載の縫合糸取付カテーテル。
- 前記レールは、前記近位クランプ顎部のチャネルで摺動するように構成される、請求項1に記載の縫合糸取付カテーテル。
- 前記レールが前記近位クランプ顎部を超えて遠位方向に摺動することを防止するように構成される遠位レールロックをさらに備える、請求項18に記載の縫合糸取付カテーテル。
- 前記遠位クランプ顎部が前記近位クランプ顎部から所定の最低距離より近くになるほど前記レールが近位方向に摺動することを防止するように構成される近位レールロックをさらに備える、請求項1に記載の縫合糸取付カテーテル。
