Abstract
Description
生体適合性ポリマーと接合されてなる、経鼻粘膜デリバリー(nasal transmucosa
l delivery)用の薬剤組成物に関するものである。
から保護される、経鼻粘膜デリバリーへの使用に適する生体適合性ポリマー−生
物学的に活性のあるペプチド接合体(conjugate)を含む薬剤組成物に関するもの
である。
物は、薬剤の活性を短時間で発現でき、生物学的利用能が改善される。
なペプチドが重要な役割を果たしている。遺伝子工学が最近非常に進歩したこと
に伴い、様々なペプチドを大量に合成して、医薬品として使用することができる
ようになった。
問題点がある。第一に、ペプチドまたはタンパク質は、体内に取り入れられた後
短期間で容易に加水分解あるいは酵素によって分解するため、体内への吸収効率
が極めて低い。さらに、このようなペプチド医薬品は繰り返して投与されると、
免疫反応がしばしば誘導されて、医薬品の生理学的な活性に対して中和する役割
として作用する、投与されたものの生命を脅かすほど重篤な過敏症を引き起こす
ような抗体を産生する。加えて、細網内皮系組織(RES)によるクリアランス
が増加する。したがって、大部分のペプチド医薬品は、現在まで注射によって投
与されてきた。しかしながら、注射による投与は、患者に苦痛を与え、危険を伴
う。特に、長期間治療が必要な患者は、かれら自身を注射によって処置すること
ができない。ゆえに、ペプチドの他の投与経路を開発する必要性が残っている。

させることができる。したがって、薬剤の経粘膜(transmucosal)デリバリーに関
して広範な研究がなされてきた。粘膜を介した薬剤の吸収に影響を与える因子と
しては、薬剤固有の透過率、イオン強度、流量分配係数(flow distribution coe
fficient)及び分子量等の、薬剤の物理学的及び化学的特性、担体の輸送、プロ
テアーゼによる分解、ならびに鼻粘膜の生理学的な状態が挙げられる。事実、鼻
粘膜は、薬剤が、経口投与時の体内での薬剤の利用の大きな障害である、肝臓で
の代謝を回避しうる直接的な吸収経路である。ゆえに、経鼻粘膜経路は、薬剤の
体内での利用を有意に向上できる点で経口経路に比して利点を有する。
はタンパク質が鼻粘膜をよく通過できないため、静脈内注射に比べて吸収効率が
低い。ペプチドの吸収を改善する吸収促進剤が示唆されてきた。示唆された吸収
促進剤の例としては、界面活性剤(Hirai et al, Int. J. Pharm. 9, 165-169, 1
981)、アシルカルニチン、コリンエステル、α−シクロデキストリン、キレート
化剤(Lee, In: Delivery Systems for Peptide Drugs, Plenum, New York, pp.
87-104, 1986)がある。しかしながら、これらの吸収促進剤は、配合時の薬剤の
安定性を下げたり、または鼻粘膜を刺激するもととなるため、実用化が難しい。
製剤が鼻粘膜を介してデリバリーされる可能性を公開する。米国特許第4,25
0,163号には、薬剤の経鼻粘膜デリバリーを促進するために粉末形態のペプ
チド薬剤と混合される粘膜吸収性物質が開示される。欧州特許第123,831
号は、鼻粘膜を介したステロイドの投与に関するものである。ドイツ特許第2,
620,446号には、インシュリンの経鼻粘膜デリバリーに有効である体内吸
収促進剤が記載される。
ミクロスフェア中への薬剤の配合技術が開示される。しかしながら、この配合技
術は、特定の薬剤にのみ適用できる。
粘膜デリバリーに利用される。特開昭59−89619号公報には、経鼻粘膜デ
リバリーの中性吸収促進剤として、エーテル性界面活性剤、例えば、ポリオキシ
エチレンラウリルエーテルが開示される。しかしながら、これらの界面活性剤は
、鼻粘膜を損傷するため、臨床での使用には適さない。
使用される塩基性または中性のアミノ酸が記載される。特開昭63−39822
号公報では、ショ糖脂肪酸エステルが薬剤の経鼻粘膜デリバリー用の吸収促進剤
として使用される。しかしながら、これらの吸収促進剤はまた、粘膜に毒性があ
る。

が短期間で分解するという問題を解決できない。これらの公知の技術は、ペプチ
ド薬剤の経鼻粘膜デリバリーに適用するには困難がある。

hys., 7, 175-186, 1984)。ウリカーゼ−PEG接合体は、インビボ(in vivo)で
の半減期が増加し、尿酸の代謝中の副作用の抑制を示すことが示される(Davis e
t al., Lancet, 2, 281-283, 1981)。
ペプチドまたはタンパク質は、インビボ(in vivo)での半減期及び溶解性を増加
させ、免疫反応を抑制することができる。
リペプチドのアミノ酸残基に反応させることによって達成される。リジン残基及
びN−末端がこの目的に使用されるのに適する。PEGの活性化に関しては、P
EGの一方の水酸基をメチルエーテル基に置換し、他方の水酸基を求電子性の官
能基に結合させる(Abuchowski, A. and Davis, F.F. (1981), in Enzymes as Dr
ugs (Holsenberg, J. and Roberts, J., eds))。活性化されたポリマーの例とし
ては、アミド結合を有する、PEG−N−ヒドロキシスクシンイミド−活性化エ
ステル類、アルキル結合を有する、PEG−エポキシド類及びPEG−トレシレ
ート(tresylate)、ウレタン結合を有する、PEG−カルボニルイミダゾール及
びPEG−ニトロフェニルカーボネート類(nitrophenyl carbonates)、N−末端
にシッフ塩基を有する、PEG−アルデヒドが挙げられる。

インビボ(in vivo)で可逆的に分解されうる。これらの誘導体の利点をもつペプ
チドとしては、インターロイキン−3及びインターロイキン−2がある。

シトニンのN−末端および/またはLys18および/またはLys11に接合して
もよい。得られたカルシトニン−PEG接合体のうち、PEG1分子がカルシト
ニン1分子に接合するカルシトニン−(モノ)PEG接合体は、最も高いカルシ
ウム減少効果を発揮する。PEGがN−末端またはLys18に接合する接合体異
性体は、PEGがLys11に接合する接合体異性体に比べてより一様でかつ有効
なカルシウム減少活性を示す。反応溶液のpHが5、6または7で、80%以上
のPEGがカルシトニンのN−末端に接合する。他方で、pH8以上では、PE
Gは、Lys11及びLys18にますます接合していく。これに対して、反応時間
及びモル比の変化は異性体の割合に影響を与えることができない。

シトール3リン酸(inositol triphosphate))及びDAG(ジアシルグリセロー
ル)の生産を向上する。

USA)(5mg/ml、0.1M リン酸緩衝溶液、pH8.0)を添加し、周
囲温度で30分間、撹拌した。0.1Mのグリシンを添加して反応を停止した。
未反応のカルシトニン及びPEGを、リン酸緩衝生理食塩水(phosphate buffere
d saline)(以後、「PBS」と称する、pH7.4)を用いた分析によって除
去し、PEG5000−sCTを得た。

された画分を、セントリコン−10(Centricon-10)(Amicon, USA)を用いて濃縮
し、冷蔵庫で貯蔵した。

g/ml)を添加し、周囲温度で30分間、撹拌した。1Mのグリシン 5μl
を添加することによって、カルシトニン及びSS−PEGの反応を中止した。未
反応のカルシトニン及びPEGを、PBS(pH7.4)を用いた透析によって
除去した。
製 6.8μlのカルシトニン(10mg/ml)に、10μlのSS−PEG溶
液を添加して、1:1、1:2、1:3、1:5及び1:10のモル比のカルシ
トニン及びSS−PEGの混合物を得、これらの5種の混合物に78.2μlの
PBS(pH8.0)を加えた。これらから、PEG−sCTを、実施例<3−
1>におけるのと同様の方法で調製した。
液を添加して、1:3のモル比のカルシトニン及びSS−PEGの混合物を得た
後、この混合物に78.2μlのPBS(pH8.0)を加えることによって、
5個の同じ反応溶液を調製した。5個のこの溶液を、攪拌しながら5、10、2
0、30、60分間、室温で反応させて、実施例<3−1>に示されるのと同様
にしてPEG−sCTを調製した。
G−sCT接合体を、トリ−PEG−sCT、ジ−PEG−sCT及びモノ−P
EG−sCTに分離した。この分離は、実施例2と同様にして行なわれた。

mm、5μM、Merck)をカラムとして使用し、ペンタフルオロプロピオン酸(p
entafluoropropionic acid)(PFPA)を含むアセトニトリルの直線的な濃度
勾配(linear gradient)を移動相(mobile phase)として使用した。濃度勾配条件
は、36〜42%の溶媒B(0.1% PFPAが添加されたアセトニトリル)
及び64〜58%の溶媒A(0.1% PFPAが添加された蒸溜水)を用いて
変化させた。定量測定は、UV吸収計(UV absorption meter)(215nm)ま
たは蛍光吸収計(fluorescent absorption meter)(励起280nm、発光315
nm)を用いて行なった。
性体は、それぞれ、カルシトニンのN−末端、Lys18及びLys11に接合した
PEGを含むものであると同定されたことが示された。加えて、反応液のpHが
5、6または7であると、80%以上のPEGがN−末端に選択的に接合するこ
とが見出された。pHが8以上では、PEGは、図2に示されるように、Lys 11 及びLys18にますます接合していった。しかしながら、図3及び4に示され
るように、反応時間及びモル比を変えても異性体の割合に差異はなかった。

を、PBSを用いた透析によって除去して、PEG5000−PTH(1−34
)を得た。

た。20μlのプラシーボ及び実験薬剤を、ハミルトンシリンジ(Hamilton syri
nge)でラットの鼻腔に投与した。プラシーボには生理食塩水を使用し、実験薬剤
には実施例2から調製されたPEG5000−sCT、PEG12000−sC
T、PEG2000−sCT及びsCTを使用した。サケのカルシトニンを、ラ
ット1匹当たり0.05〜4.5IUの量で投与し、ラットを4群に分けた。プ
ラシーボ及び実験薬剤を投与する前、投与してから5、10、30、60、12
0、240、480及び360分後に200μlの血液を採取し、すぐに遠心し
て、100μlの血漿を得た。血漿中のカルシウム濃度を、カルシウムアッセイ
キット(calcium assay kit)(Sigma, USA)を用いて測定し、投与前のカルシウム
濃度を100として、カルシウム残存率を算出した。血漿中の測定されたカルシ
ウム濃度の単位は、mg/dlであり、残存率は、初期値に対する比率(%)と
して算出された。

加えて、カルシウム減少効果は、PEGの分子サイズに従って有意に変化した。

。この結果は、PEG分子量がより増加するほど、経鼻粘膜による体内への吸収
度合いはより減少したことによるものであった。その結果、より低分子量のPE
Gと接合したカルシトニンは、PEGを用いずに接合されたカルシトニンに比べ
てより良好なカルシトニン減少効果を有しており、そのカルシウム減少効果は長
期間維持された。

合では、N−末端異性体より低いカルシウム減少効果を有するLys18異性体に
より、sCTまたはLys11異性体に比してかなりの減少効果が示された。

ccinate)で活性化し、活性化されたSS−PEG 2mgを用いて、PEG20
00−GLP−1を調製した。全ての過程は、実施例<1−1>に記載されるの
と同様にして行なった。加えて、PEG2000−GLP−1を、実施例<5−
1>に記載されるのと同様にして単離、精製した。
物は、水に難溶性である、ペプチドの水溶性を増加させ、プロテアーゼによる分
解から保護することによって安定性を改善し、その結果、薬剤の投与回数を減ら
して薬剤の乱用によって誘導される副作用を抑制する。
薬剤組成物は鼻腔を介してデリバリーされるので、薬剤の活性を短期間で発現す
ることができ、生物学的利用能を改善する。
特定の実施態様は本発明の同様の目的を行なうために他の実施態様を修飾するま
たは設計するための基礎として容易に利用されると考えるであろう。当該分野に
おける通常の知識を有するものはまた、このような等価な実施態様は添付の特許

Claims (10)
- 【請求項1】 生物学的に活性のあるペプチドが活性のある生体適合性ポリ
マーと接合されてなる経鼻粘膜デリバリー用薬剤組成物。 - 【請求項2】 ペプチドは、カルシトニン、副甲状腺ホルモン(PTH)、
インシュリン、合成エンケファリン、成長ホルモン放出ペプチド(GHRP)、
黄体ホルモン放出ホルモン(LHRH)及びその誘導体、視床下部の分泌物質、
カルシトニン遺伝子関連ペプチド(CGRP)、甲状腺刺激ホルモン、ならびに
胸腺液性因子(THF)よりなる群から選択される、請求項1に記載の経鼻粘膜
デリバリー用薬剤組成物。 - 【請求項3】 生体適合性ポリマーは、200〜20,000の分子量を有
する、請求項1に記載の経鼻粘膜デリバリー用薬剤組成物。 - 【請求項4】 生体適合性ポリマーは、500〜12,000の分子量を有
する、請求項1に記載の経鼻粘膜デリバリー用薬剤組成物。 - 【請求項5】 生体適合性ポリマーは、ポリエチレングリコール、ポリプロ
ピレングリコール(PPG)、ポリオキシエチレン(POE)、ポリトリメチレ
ングリコール、ポリ乳酸及びその誘導体、ポリアクリル酸及びこれらの誘導体、
ポリアミノ酸、ポリビニルアルコール類、ポリウレタン、ポリホスファゼン、ポ
リ(L−リジン)、ポリアルキレンオキシド(PAO)、ならびに多糖、デキス
トラン等の水溶性ポリマー、ならびにポリビニルアルコール及びポリアクリルア
ミド等の非免疫原性ポリマーよりなる群から選択される一種である、請求項1に
記載の経鼻粘膜デリバリー用薬剤組成物。 - 【請求項6】 ペプチド及び活性のある生体適合性ポリマーのモル比は、1
:1〜1:7である、請求項1に記載の経鼻粘膜デリバリー用薬剤組成物。 - 【請求項7】 ペプチド及び活性のある生体適合性ポリマーの反応pHは、
5.0〜8.0である、請求項1に記載の経鼻粘膜デリバリー用薬剤組成物。 - 【請求項8】 1分子のペプチドに1〜3分子の活性のある生体適合性ポリ
