Abstract
Un montaje de unidad de carga adaptado para acoplarse de manera desmontable a un instrumento quirúrgico, en donde el montaje de unidad de carga comprende una parte de cuerpo (502) y un montaje de herramienta (504), en donde la parte de cuerpo (502) se puede acoplar de manera desmontable a un extremo distal de una parte endoscópica del instrumento quirúrgico: en donde el montaje de herramienta (504) incluye el primer y segundo elementos de mordaza, el primer elemento de mordaza incluye un montaje de cartucho (508) que posee múltiples grapas y el segundo elemento de mordaza incluye un montaje de yunque (506), el primer elemento de mordaza puede moverse con respecto al segundo elemento de mordaza entre posiciones separadas y aproximadas, en donde el montaje de herramienta (504) comprende además un pasador pivote (900) configurado para acoplarse de forma pivotal al primer y segundo elementos de mordaza, de forma tal que al menos un elemento de mordaza pueda girar con respecto al otro alrededor del pasador pivote, caracterizado por que el pasador pivote (900) incluye una parte base (906) y una primera y segunda partes escalonadas (902, 904) que se extienden de forma radial hacia afuera desde la parte base, en donde el par de partes escalonadas (902, 904) posee diámetros distintos, la primera y segunda partes escalonadas (902, 904) se configuran para acoplarse de manera pivotal al primer y segundo elementos de mordaza, respectivamente.

Antecedentes
Campo tecnico
La presente descripcion se refiere en terminos generales a instrumentos para unir tejido de manera quirurgica y, mas especlficamente, a un pasador pivote que une de forma giratoria a un par de elementos de mordaza de un instrumento quirurgico.
Antecedentes de la tecnica relacionada
Se conocen en la tecnica diversos tipos de instrumentos quirurgicos utilizados para unir tejido de manera quirurgica, y se utilizan comunmente, por ejemplo, para cerrar tejido u organos en transeccion, reseccion y anastomosis; y para la oclusion de organos en intervenciones toracicas y abdominales.
Un ejemplo de dicho instrumento quirurgico es un instrumento de grapado quirurgico, que incluye tlpicamente un montaje de yunque, un montaje de cartucho para brindar soporte a un conjunto de grapas quirurgicas, un mecanismo de aproximacion para aproximar los montajes de cartucho y yunque, y un mecanismo de disparo para expulsar las grapas quirurgicas del montaje de cartucho.
Para lograr resultados adecuados, p. ej., una formacion de grapas congruente, es necesaria una alineacion adecuada del yunque con el cartucho. Por consiguiente, es necesario un mecanismo de aproximacion que alinee de manera adecuada los montajes de cartucho y yunque para una formacion adecuada de las grapas. Se conocen diversos tipos de conexiones pivotales para instrumentos quirurgicos a partir de DE 20 20l1 107977, US 2004/267311 y WO 2009/003575.
US 2012/080473 representa la tecnica anterior mas cercana.
Compendio
La invencion se define en la reivindicacion independiente 1. Se definen realizaciones preferidas en las reivindicaciones dependientes.
De acuerdo con una realizacion de la presente descripcion, se proporciona un montaje de unidad de carga adaptado para acoplarse de manera desmontable a un instrumento quirurgico. El montaje de unidad de carga incluye un montaje de herramienta que incluye un primer y segundo elementos de mordaza y un pasador pivote configurados para acoplar de manera pivotal al primer y segundo elementos de mordaza. El primer elemento de mordaza es movil con respecto al segundo miembro de mordaza entre las posiciones separada y aproximada, en donde el pasador pivote incluye una parte base y una primera y segunda partes escalonadas que se extienden de manera radial hacia el exterior desde la parte base. La primera y segunda partes escalonadas se configuran para acoplar de manera pivotal al primer y segundo elementos de mordaza, respectivamente.
En una realizacion, el par de partes escalonadas puede disponerse de manera concentrica.
Ademas, el par de partes escalonadas puede tener diametros distintos. Un diametro de la primera parte escalonada puede ser menor que un diametro de la segunda parte escalonada.
En otra realizacion, al menos una parte de la parte base puede incluir roscas. En aun otra realizacion, el primer elemento de mordaza puede incluir un montaje de cartucho que posee multiples grapas, y el segundo elemento de mordaza puede incluir un montaje de yunque. El montaje de unidad de carga puede incluir ademas un miembro alargado que incluye una parte de montaje que define un hueco que esta configurado y posee una dimension para recibir el pasador pivote en este. El primer elemento de mordaza puede incluir un par de paredes, donde cada una define un agujero. Al menos una parte de la parte de montaje puede disponerse entre el par de paredes del primer elemento de mordaza, de forma tal que el hueco y los agujeros se alineen para recibir el pasador pivote. Ademas, el segundo elemento de mordaza puede incluir un par de paredes, donde cada una define un agujero. En particular, al menos una parte del par de paredes del primer elemento de mordaza puede recibirse entre el par de paredes del segundo elemento de mordaza, de forma tal que los agujeros del primer y segundo elementos de mordaza y el hueco se alineen para recibir el pasador pivote.
En aun otra realizacion, la parte de cuerpo del pasador pivote puede recibirse de forma roscada en el agujero de la parte de montaje. La segunda parte escalonada puede incluir una muesca configurada para recibir una herramienta para ajustar el pasador pivote. El primer y segundo elementos de mordaza pueden ser curvos con respecto a un eje longitudinal definido por el miembro alargado.

Descripcion detallada
Se describen en detalle con respecto a los dibujos realizaciones del instrumento quirurgico y unidad de carga para su uso con este descritos en la presente, en donde los numeros de referencia similares indican elementos correspondientes en cada una de las diversas vistas. Como es comun en la tecnica, el termino “proximal” hace referencia a la parte o al componente mas cercano al usuario u operario, p. ej., cirujano o medico, mientras que el termino “distal” hace referencia a la parte o al componente mas alejado del usuario.
Con referencia a las Figuras 1-4, se indica con el numero de referencia 10 un instrumento de grapado quirurgico de la presente descripcion. El instrumento de grapado quirurgico 10 incluye un montaje de mango 12 que incluye un gatillo accionador 22, una parte endoscopica 18 que se extiende de manera distal desde el montaje de mango 12, y una unidad de carga (o “DLU”, por sus siglas en ingles) 500 que se acopla de manera desmontable a la parte endoscopica 18. Se describen detalles adicionales de un instrumento endoscopico de grapado quirurgico en una patente estadounidense de propiedad comun con n.° 6,953,139 otorgada a Milliman et al.
Con referencia particular a las Figuras 3 y 4, la DLU 500 incluye una parte de cuerpo proximal 502 y un montaje de herramienta 504. Una parte de cuerpo proximal 502 define un eje longitudinal “A-A” y se acopla de manera desmontable a un extremo distal de la parte endoscopica 18 del instrumento quirurgico. El montaje de herramienta 504 incluye un par de elementos de mordaza que incluyen un montaje de yunque 506 y un montaje de cartucho 508. Al menos un elemento de mordaza puede girar con respecto al otro alrededor de un pivote 555. En particular, el montaje de cartucho 508 puede girar con respecto al montaje de yunque 506 y puede moverse entre una posicion abierta o desbloqueada y una posicion cerrada o aproximada. El montaje de cartucho 508 se fuerza hacia la posicion abierta a traves de un miembro de desviacion 533, p. ej., un par de resortes de compresion, dispuesto entre una cubierta de yunque 510 y el cartucho 518 (Figura 7).
Con respecto ahora a la Figura 4, el montaje de yunque 506 y un montaje de cartucho 508 son curvos con respecto al eje longitudinal “A-A” definido por la parte de cuerpo proximal 502. Los elementos de mordaza curvos, en comparacion con los elementos de mordaza rectos, pueden facilitar el acceso a regiones pelvicas inferiores, p. ej., durante una reseccion anterior baja (“LAR”, por sus siglas en ingles). Ademas, la inclusion de elementos de mordaza curvos puede permitir una mayor visualizacion hacia un sitio quirurgico y tambien puede otorgar mas espacio para que un cirujano pueda manipular el tejido objetivo o los elementos de mordaza en si con su mano.
El termino “distal” se refiere tlpicamente a la parte o componente del instrumento que se encuentre mas alejada del usuario. Tal como se usa en la presente, los terminos “distal” y “proximal” toman en cuenta la curvatura de las partes curvas del instrumento quirurgico 10 de la presente descripcion. Por ejemplo, “distal” hace referencia a la parte de la parte curva que se encuentra mas alejada del usuario, a lo largo de una trayectoria definida por la parte curva, tal como la trayectoria C-C que se muestra en la Figura 4. Es decir, si bien una parte intermedia de una parte curva puede encontrarse mas lejos del usuario durante el uso, se considera “distal” la parte de la parte curva que se

encuentra mas alejada a lo largo de su eje longitudinal. El montaje de yunque 506 tiene un extremo proximal 506a y un extremo distal 506b, y el montaje de cartucho 508 tiene un extremo proximal 508a y un extremo distal 508b.
Con referencia a las Figuras 5-7, el montaje de yunque 506 incluye una cubierta de yunque 510 y una placa de yunque 512 que incluye multiples depresiones que forman grapas 514 (Figura 6). La placa de yunque 512 se fija a una parte inferior de la cubierta de yunque 510 para definir un canal 511 (Figura 5) entre la placa 512 y la cubierta 510. Cuando el montaje de herramienta 504 se encuentra en la posicion aproximada, las depresiones que forman grapas 514 se colocan en una alineacion yuxtapuesta con el montaje de cartucho 508.
El montaje de cartucho 508 incluye un portador curvo 516 que recibe un cartucho curvo 518 a traves de una conexion a presion. El cartucho 518 incluye un par de puntales de soporte 524 que se apoyan en las paredes laterales 517 del portador 516 para estabilizar el cartucho 518 sobre el portador 516. Los puntales de soporte 524 tambien fijan la altura o ubicacion del cartucho 518 con respecto a la placa de yunque 512. Una superficie externa del portador 516 incluye una superficie de leva inclinada 516a.
Con referencia continua a las Figuras 5-7, el cartucho de grapas 518 incluye una ranura central 526 que es curva con respecto al eje longitudinal A-A y tres filas curvas de las ranuras de retencion de grapas 528 colocadas a cada lado de la ranura longitudinal curva 526. Cada ranura 528 se configura para recibir una grapa 530 en esta. El cartucho 518 tambien define multiples ranuras de cunas de leva 529 (Figura 6) que reciben los impulsores de grapas 532 y que se encuentran abiertas en la parte inferior (es decir, alejadas de la superficie que entra en contacto con el tejido 540) para permitir que pase a traves de estas una gula de accionamiento curvada de forma longitudinal 536. La gula de accionamiento 536 pasa a traves de las ranuras de cuna de leva 529 y fuerza a los impulsores de las grapas 532 hacia las grapas respectivas 530. Luego se impulsa a las grapas 530 de sus ranuras de retencion 528 respectivas.
Con referencia a la Figura 7, la parte de cuerpo proximal 502 de la DLU 500 incluye un cuerpo interno 503 formado a partir de las mitades longitudinales moldeadas 503a y 503b y un montaje de accionamiento 560. La parte de cuerpo proximal 502 se acopla al montaje de herramienta 504 a traves de un ensamblaje de montado 570. El ensamblaje de montado 570 tiene un par de extensiones 576. Cada extension 576 tiene un hueco transversal 578. El par de extensiones 576 se extiende al menos parcialmente hacia el interior de un extremo proximal del portador 516. En particular, una parte del par de extensiones 576 se dispone entre un par de paredes laterales proximales 591 del portador 516. El par de paredes laterales proximales 591 se dispone dentro de un par de paredes laterales proximales 515 de la cubierta de yunque 510, a traves de la cual los agujeros 580 definidos en las paredes laterales proximales 591 del portador 516 y los agujeros 577 definidos en las paredes laterales proximales 515 de la cubierta de yunque 510 se alinean con los huecos 578 del ensamblaje de montado 570 (Figura 10). Los agujeros 577, 580 y los huecos 578 se configuran y se les da dimensiones adecuadas para recibir el tornillo 900 para fijar de manera pivotal el montaje de cartucho 508 con el ensamblaje de montado 570, el cual, a su vez, habilita el pivote del montaje de cartucho 508 con respecto al montaje de yunque 506.
Con respecto ahora a las Figuras 7-10, cada tornillo 900 incluye la primera y segunda partes escalonadas 902, 904 y una parte base 906 que posee las roscas 906a. En particular, la primera parte escalonada 902 se configura y se le da dimensiones adecuadas para recibirla en el agujero 580 definido en el portador 516. La primera parte escalonada 902 proporciona una alineacion y una superficie de apoyo para que el portador 516 se acople a esta de manera pivotal. La primera parte escalonada 902 incluye el primer diametro D1 y una primera longitud L1. La primera longitud L1 de la primera parte escalonada 902 se adapta para definir un hueco controlado L4 (Figura 10) entre la pared lateral proximal 591 de la cubierta de yunque 510 y el ensamblaje de montado 570. Ademas, la primera parte escalonada 902 incluye una parte de transicion 902a que posee un perfil concavo para reducir la concentracion del esfuerzo a lo largo de esta.
La segunda parte escalonada 904 se configura y se le da dimensiones adecuadas para recibirla en el agujero 577 definido en la cubierta de yunque 510. La segunda parte escalonada 904 incluye un segundo diametro D2 y una segunda longitud L2. La primera parte escalonada 902 se dispone de manera concentrica con la segunda parte escalonada 904 y la parte base 906. El segundo diametro D2 se configura y se le da dimensiones adecuadas para alinear las depresiones 514 de la placa de yunque 512 con las ranuras de retencion 528 del cartucho 518. Ademas, a la segunda longitud L2 de la segunda parte escalonada 904 se le da una tolerancia especlfica para permitir que el montaje de yunque estatico 506 flote mientras se encuentra acoplado de manera fija al montaje de cartucho 508. Al menos una parte de la parte base 906 del tornillo 900 incluye las roscas 906a configuradas para acoplarse de manera roscada a los huecos 578 definidos en el ensamblaje de montado 570. El tornillo 900 incluye una superficie lateral 915 que define una muesca 905 configurada para recibir una herramienta (no se muestra) para ajustar el tornillo 900 en el hueco 578 del ensamblaje de montado 570.
En contraposition con los montajes de pivote convencionales, el tornillo 900 permite una configuration con mayor torsion y una mayor seguridad del tornillo con un ancho de canal controlado. Como resultado, se logra una formation de grapas mas constante, as! como una mejor alineacion del yunque con respecto al cartucho.
Con respecto nuevamente a la Figura 7, el ensamblaje de montado 570 se acopla de manera fija a la mitad longitudinal 503a mediante un par de protuberancias verticales 584. Las protuberancias verticales 584 se extienden

hacia arriba desde el ensamblaje de montado 570 y se ajustan de manera friccional en las muescas correspondientes (no se muestran) de la mitad longitudinal 503a.
Con referencia a la Figura 7, cuando el montaje de accionamiento 560 se introduce de manera distal dentro del montaje de herramienta 504, una parte de barra superior 606b del elemento de abrazadera 606 se mueve dentro del canal 511 (Figura 5) definido entre la placa de yunque 512 y la cubierta de yunque 510, y la parte de barra inferior 606c se mueve sobre una superficie exterior del portador 516. Cuando la barra inferior 606c se acopla y se mueve sobre la superficie de leva 516a, el montaje de cartucho 508 gira desde la posicion abierta hacia la posicion cerrada.
En uso, el tejido se coloca entre los elementos de mordaza (Figuras 11 y 12) y se aproximan las mordazas (Figura 13) entre si. El accionamiento del accionador 22 mediante golpes sucesivos provoca un avance distal de la barra impulsora 30 (Figura 2), de forma tal que la barra impulsora 30 empuje a un montaje de accionamiento 560 (Figura 7) a traves del cartucho 518. Se proporcionan detalles adicionales del avance distal de la barra impulsora 30 a traves del accionamiento del accionador 22 en la patente estadounidense n.° 6,953,139 otorgada a Milliman et al.
El movimiento del ensamblaje de montado 560 y, en particular, un elemento de abrazadera 606 fijado a este, mueve una gula de accionador curvada de forma longitudinal 536 a traves del cartucho 518. Una gula 536 se mueve a traves del cartucho 518, las cunas de leva curvadas de forma longitudinal 534 (Figura 7) de la gula de accionamiento 536 se acoplan de forma secuencial a los impulsores 532 para mover los impulsores 532 de forma vertical dentro de las ranuras de retencion de grapas 528 y expulsar las grapas 530 hacia las depresiones que forman grapas 514 de la placa de yunque 512.
Luego de la expulsion de las grapas 530 de las ranuras de retencion 528 (y hacia el tejido), un filo 606d del elemento de abrazadera 606 corta el tejido grapado a medida que el filo 606d se desplaza a traves de la ranura curva 526 del cartucho 518.
Si bien la descripcion que antecede contiene muchos detalles, estos detalles no deben interpretarse como limitaciones del alcance de la presente descripcion, sino meramente como ilustraciones de diversas realizaciones de esta. Por ejemplo, si bien el tope de tejido 700 (Figuras 7 y 11) se muestra dispuesto en una parte distal de la DLU 500, tambien se concibe que pueda utilizarse el tornillo 900 con una DLU que posea un tope de tejido en una parte proximal de esta. En dicha configuracion, las partes escalonadas se extienden mas alla del tope de tejido, de forma tal que las partes escalonadas sirvan como una superficie de apoyo para el tope de tejido. Por lo tanto, la descripcion que antecede no deberla interpretarse como taxativa, sino como mero ejemplo de las diversas realizaciones. Los expertos en la tecnica preveran otras modificaciones dentro del alcance de las reivindicaciones adjuntas a la presente.

Un montaje de unidad de carga adaptado para acoplarse de manera desmontable a un instrumento quirurgico, en donde el montaje de unidad de carga comprende una parte de cuerpo (502) y un montaje de herramienta (504), en donde la parte de cuerpo (502) se puede acoplar de manera desmontable a un extremo distal de una parte endoscopica del instrumento quirurgico:en donde el montaje de herramienta (504) incluye el primer y segundo elementos de mordaza, el primer elemento de mordaza incluye un montaje de cartucho (508) que posee multiples grapas y el segundo elemento de mordaza incluye un montaje de yunque (506), el primer elemento de mordaza puede moverse con respecto al segundo elemento de mordaza entre posiciones separadas y aproximadas,en donde el montaje de herramienta (504) comprende ademas un pasador pivote (900)configurado para acoplarse de forma pivotal al primer y segundo elementos de mordaza, de forma tal que al menos un elemento de mordaza pueda girar con respecto al otro alrededor del pasador pivote, caracterizado por que el pasador pivote (900)incluye una parte base (906) y una primera y segunda partes escalonadas (902, 904) que se extienden de forma radial hacia afuera desde la parte base, en donde el par de partes escalonadas (902, 904) posee diametros distintos, la primera y segunda partes escalonadas (902, 904) se configuran para acoplarse de manera pivotal al primer y segundo elementos de mordaza, respectivamente.
- 2. El montaje de unidad de carga segun la reivindicacion 1, en donde el par de partes escalonadas (902, 904) se dispone de manera concentrica.
- 3. El montaje de unidad de carga segun cualquier reivindicacion precedente, en donde un diametro de la primera parte escalonada (902) es menor que un diametro de la segunda parte escalonada (904).
- 4. El montaje de unidad de carga segun cualquier reivindicacion precedente, en donde al menos una parte de la parte base (906) incluye roscas (906a).
- 5. El montaje de unidad de carga segun cualquier reivindicacion precedente, que comprende ademas un miembro alargado que incluye una parte de montaje (570) que define un hueco (578) que esta configurado y se le da dimensiones adecuadas para recibir el pasador pivote (900) en este.
- 6. El montaje de unidad de carga segun cualquier reivindicacion precedente, en donde el primer elemento de mordaza incluye un par de paredes (591), y cada una define un agujero (580).
- 7. El montaje de unidad de carga segun la reivindicacion 6, en donde al menos una parte de la parte de montaje (570) se dispone entre el par de paredes (591) del primer elemento de mordaza, de forma tal que el hueco (578) y los agujeros (580) se alineen para recibir el pasador pivote (900).
- 8. El montaje de unidad de carga segun cualquier reivindicacion precedente, en donde el segundo elemento de mordaza incluye un par de paredes (515), y cada una define un agujero (577).
- 9. El montaje de unidad de carga segun la reivindicacion 8, que depende de la reivindicacion 7, en donde el par de paredes (591) del primer elemento de mordaza se recibe al menos parcialmente entre el par de paredes (515) del segundo elemento de mordaza, de forma tal que los agujeros (577, 580) del primer y segundo elementos de mordaza (515, 591) y el hueco (578) se alineen para recibir el pasador pivote (900).
- 10. El montaje de unidad de carga segun la reivindicacion 9, en donde la parte base (906) del pasador pivote (900) se recibe de manera roscada en el hueco (578) de la parte de montaje (570).
- 11. El montaje de unidad de carga segun cualquier reivindicacion precedente, en donde la segunda parte escalonada incluye una muesca configurada para recibir una herramienta para ajustar el pasador pivote (900).
- 12. El montaje de unidad de carga segun la reivindicacion 5 o cualquiera de las reivindicaciones 6 a 11, que depende de la reivindicacion 5, en donde el primer y segundo elementos de mordaza son curvos con respecto a un eje longitudinal definido por el miembro alargado.
