Abstract
1. Una composición de heparán sulfato aislada o purificada en la que el componente de heparán sulfato es al menos 97 % de heparán sulfato HS/BMP2 que puede unirse específicamente a la SEC ID Nº: 1 o 6, o a la BMP2, en la que después de la digestión con heparín liasas (heparinasa, heparitinasa I y II) hasta el final y después de someter los fragmentos de disacárido resultantes a HPLC de intercambio aniónico fuerte (SAX-HPLC), el heparán sulfato HS/BMP2 tiene una composición de disacáridos que comprende: UA-GlcN 8,2 % (± 3 %); UA-GlcNAc 17,5 % (± 3 %); UA-GlcN,6S 3,7 % (± 3 %); UA,2S-GlcN 4,5 % (± 3 %); UA-GlcNS 16,2 % (± 3 %); UA,2S-GlcNS,6S 0,5 % (± 3 %); en la que la digestión se realiza disolviendo una muestra de HS/BMP2 (100 μg) en acetato sódico 100 mM / acetato cálcico 0,2 M (pH 7,0); digiriendo secuencialmente la muestra con heparinasa (10 mU/ml) a 37 ºC durante 2 horas, seguido de heparitinasa I (10 mU/ml) a 37 ºC durante 1 hora, seguido de heparitinasa II (10 mU/ml) a 37 ºC durante 18 horas, seguido de una alícuota de cada dicha liasa a 37 ºC durante 6 horas, en la que después de la digestión las muestras se procesan en una columna de BioGel P-2 (1 x 120 cm) equilibrada con NH4HCO3 0,25 M, en la que el pico que comprende los disacáridos se liofiliza y después se disuelve en el agua acidificada (pH 3,5 con HCl) y después se hace pasar sobre una columna ProPac PA-1 de SAX-HPLC, en la que los disacáridos se eluyen con un gradiente lineal de NaCl de 0 a 1,0 M (pH, 3,5) durante 60 minutos a un caudal de 1 ml/min.

Por ejemplo, la curva de desplazamiento de ángulo ilustrada en la Figura 44 puede usarse para diferenciar el HS/BMP2 de otros heparán sulfatos.
5 Adicionalmente se ilustran las diferencias estructurales del HS/BMP2 comparadas con las de los heparán sulfatos que no se unen a la proteína BMP2 realizando cromatografía líquida a alta presión de intercambio aniónico fuerte (SAX-HPLC). La Figura 40 ilustra el espectro SAX-HPLC de HS/BMP2, que puede compararse con los espectros SAX-HPLC de heparán sulfatos que no se unen a la proteína BMP2 (Figuras 41 y 42).
10 Para identificar el HS/BMP2 los inventores idearon un método que implicaba el enriquecimiento de moléculas de glucosaminoglucano que presentan unión a polipéptidos particulares que tienen un dominio de unión a heparina. 30 Los inventores también proporcionan glucosaminoglucanos aislados, identificados por su capacidad para modular el crecimiento o la diferenciación de células o tejidos. Para hacer esto, proporcionan un método de identificación de glucosaminoglucanos que podían estimular o inhibir el crecimiento y/o la diferenciación de células y/o tejidos, comprendiendo el método:

- proporcionar un soporte sólido que tenga moléculas polipeptídicas adheridas al soporte, en el que el polipéptido comprenda un dominio de unión a heparina;

- (iv)
- disociar los glucosaminoglucanos de los complejos de polipéptido-glucosaminoglucano;
- (v)
- recoger los glucosaminoglucanos disociados.
- (vi)
- añadir los glucosaminoglucanos recogidos a células o tejidos en los que esté presente una proteína que contenga la secuencia de aminoácidos del dominio de unión a heparina;
45 (vii) medir uno o más de: proliferación de las células, diferenciación de las células, expresión de uno o más marcadores de proteína.
Los inventores usaron estos métodos para identificar un GAG capaz de unirse a BMP2 (que denominaron HS/BMP2
o HS3 o HS-3), en el que el polipéptido usado en la metodología de los inventores comprende el dominio de unión a 50 heparina de la SEC ID Nº: 1 o 6, obtenido de la secuencia de aminoácidos de BMP2.
En la metodología de los inventores, la mezcla que comprende los GAG puede contener glucosaminoglucanos sintéticos. Sin embargo, se prefieren GAG obtenidos de células o tejidos. Por ejemplo, la mezcla puede contener matriz extracelular en la que el material de la matriz extracelular se obtiene raspando tejido vivo in situ (es decir 55 directamente del tejido del cuerpo del ser humano o animal del cual se obtiene) o raspando tejido (vivo o muerto) que se ha extraído del cuerpo de un ser humano o animal. Como alternativa, el material de la matriz extracelular puede obtenerse de células que se crecen en cultivo. El material de la matriz extracelular puede obtenerse de tejido conectivo o de células del tejido conectivo, por ejemplo, hueso, cartílago, músculo, grasa, ligamento o tendón. Para el aislamiento del HS/BMP2, una fuente adecuada de material de la matriz extracelular son los osteoblastos, tales

El HS/BMP2 puede usarse en la reparación o regeneración de tejido corporal, especialmente regeneración ósea, regeneración neuronal, construcción de tejido esquelético, reparación de lesiones cardiovasculares y expansión y autorrenovación de células madre embrionarias y adultas. Por consiguiente, el HS/BMP2 puede usarse para prevenir
o tratar una amplia serie de enfermedades y lesiones, incluyendo osteoartritis, reemplazo de cartílago, rotura de huesos de cualquier tipo (por ejemplo, tratamientos de fusión de discos intervertebrales, roturas de huesos largos, defectos craneales), regeneración de defectos óseos críticos o sin unión.
El uso del HS/BMP2 en la reparación, regeneración o reemplazo de tejido puede implicar su uso en la curación de heridas, por ejemplo aceleración de curación de heridas, curación de escaras o de tejido óseo e injerto tisular.
En otro aspecto, la presente divulgación proporciona un armazón biológico que comprende HS/BMP2. En algunas realizaciones, los armazones biológicos de la presente invención pueden usarse en aplicaciones ortopédicas, vasculares, protésicas, de piel y córnea. En otro aspecto, la presente invención proporciona uno o más GAG (incluyendo HS/BMP2) aislados por el método descrito para su uso como un adyuvante. El adyuvante puede ser un inmunoadyuvante.
En otro aspecto, la presente invención proporciona formulaciones farmacéuticamente aceptables que comprenden una mezcla de compuestos que comprende uno o más GAG, estando dicha mezcla enriquecida con respecto a HS/BMP2. En otro aspecto, la invención proporciona formulaciones farmacéuticamente aceptables que comprenden:
- (i)
- una mezcla de compuestos que comprenden uno o más GAG, estando dicha mezcla enriquecida con respecto a HS/BMP2; y
- (ii)
- BMP-2,
para la administración por separado, simultánea o secuencial. En una realización preferida la formulación comprende la mezcla de compuestos que comprende en uno o más GAG, estando dicha mezcla enriquecida con respecto a HS/BMP2 y BMP-2 en mezcla íntima, y se administra simultáneamente al paciente que necesita el tratamiento.
En otro aspecto de la presente invención se proporciona un kit para su uso en la reparación o regeneración de tejido, comprendiendo dicho kit (i) una cantidad predeterminada de HS/BMP2, y (ii) una cantidad determinada de BMP2.
Los compuestos de las mezclas enriquecidas de la presente invención pueden administrarse a un sujeto como una sal farmacéuticamente aceptable de los mismos. Por ejemplo, las sales básicas de los compuestos de las mezclas enriquecidas de la presente invención incluyen, pero sin limitación, las formadas con cationes farmacéuticamente aceptables, tales como sodio, potasio, litio, calcio, magnesio, amoníaco y alquilamonio. La presente invención incluye en su ámbito sales catiónicas, por ejemplo las sales de sodio o potasio.
Se apreciará que los compuestos de las mezclas enriquecidas de la presente invención que llevan un grupo de ácido carboxílico pueden suministrarse en forma de un profármaco administrable, en el que la porción ácida está esterificada (para que tenga la forma-CO2R’). El término “pro-fármaco” se refiere específicamente a la conversión del grupo –OR’ a un grupo -OH, o al anión carboxilato del mismo, in vivo. Por consiguiente, los profármacos de la presente invención pueden actuar para potenciar la adsorción del fármaco y/o el suministro del fármaco en las células. La conversión in vivo del profármaco puede facilitarse bien por enzimas celulares, tales como lipasas y esterasas o bien por escisión química tal como por hidrólisis del éster in vivo.
Para su administración, los medicamentos y las composiciones farmacéuticas de acuerdo con aspectos de la presente invención pueden formularse mediante diversas vías, incluyendo, pero sin limitación, inyección en el sitio de enfermedad o lesión. Los medicamentos y composiciones pueden formularse en forma fluida o sólida. Las formulaciones fluidas pueden formularse para su administración por inyección a una región seleccionada del cuerpo humano o animal.
