Abstract

Aparato de tratamiento de heridas que utiliza
presión reducida.
La presente invención se refiere a un aparato
para la cicatrización de heridas y más particularmente para
prevenir el avance de quemaduras de espesor parcial, conocidas, por
ejemplo, del documento US-A 4 969 880.
Cuando una persona padece una quemadura, se
dañan las capas dérmica y epidérmica en la región de la herida. El
cierre de la herida resultante es importante para prevenir la
perdida de fluidos corporales y la invasión por parte de
microorganismos. En el caso de una quemadura de espesor parcial, el
tejido epitelial y subcutáneo adyacente a la herida migrará hacia
fuera y finalmente crecerá nuevo tejido sobre la herida. Se ha
desarrollado una amplia serie de recubrimientos de heridas para
acelerar el cierre de la herida y permitir que continúen los
procesos naturales de reparación del tejido dañado.
El pronóstico de una herida producida por una
quemadura depende de la gravedad de la lesión y particularmente de
la profundidad de la quemadura. En general, una quemadura de espesor
parcial cicatrizará más rápidamente y con menos complicaciones que
una quemadura que penetra profundamente. Se ha observado que las
quemaduras de espesor parcial con frecuencia se deterioran y se
vuelven quemaduras más graves, más profundas, si no se tratan
rápidamente tras producirse la lesión por quemadura.
Las manos padecen lesiones por quemadura con más
frecuencia que otras partes del cuerpo. Probablemente, esto se debe
a la reacción natural de intentar proteger la cara con las manos y,
en muchos casos, la lesión por quemadura está en el dorso de las
manos. Las otras partes del cuerpo que padecen quemaduras con más
frecuencia pueden ser los brazos, los pies y las piernas.
La presente invención busca proporcionar un
aparato para tratar lesiones en una parte del cuerpo, especialmente
lesiones producidas por quemaduras.
Según un aspecto de la presente invención se
proporciona un aparato para estimular la cicatrización de heridas
que comprende una envoltura para alojar una parte afectada del
cuerpo, incluyendo dicha envoltura una cubierta sustancialmente
estanca al aire y un relleno poroso dentro de la cubierta, estando
adaptado dicho relleno para ponerse en contacto con la superficie
de la herida, y medios de conexión para conectar el interior de la
envoltura a una fuente de presión negativa, teniendo la cubierta una
abertura que puede volver a sellarse que permite inspeccionar la
herida a intervalos.
Por cubierta sustancialmente "estanca al
aire" quiere decirse una que es lo suficientemente estanca al
aire como para que, mediante la aplicación de succión al relleno
poroso, puede mantenerse una presión inferior a la ambiente dentro
de la envoltura. Sin embargo, no es necesario que el material de la
envoltura sea totalmente oclusivo al aire.
Se ha descubierto que cuando se aplica
tratamiento de presión negativa usando el aparato de la invención a
una quemadura en un plazo de tiempo relativamente corto tras
producirse la lesión (por ejemplo, en un plazo de aproximadamente
12 horas), no sólo mejora la velocidad de cicatrización sino que se
detiene el avance de una quemadura de espesor parcial hacia una
lesión más profunda.
En una realización, la envoltura del aparato de
la invención comprende un guante, manga o calcetín. Por ejemplo, el
aparato puede incluir un guante formado de una espuma de caucho o
plástico flexible que está contenida dentro de una cubierta de baja
porosidad de aire. Normalmente, la espuma de plástico flexible es
espuma de poliuretano o poli(alcohol vinílico) (pva) que
tiene células que se comunican entre sí o una combinación de tales
espumas, por ejemplo, tal como un material laminado. En un material
laminado de este tipo, la capa de pva puede estar adyacente a la
herida.
Las características adicionales de la presente
solicitud se harán evidentes a partir de la siguiente descripción y
los dibujos adjuntos, en los que:
la figura 1 es una vista en perspectiva en
despiece ordenado del relleno poroso;
la figura 2 es una vista en perspectiva cuando
se monta el relleno poroso;
la figura 3 es una vista en perspectiva del
relleno poroso dentro de su cubierta;
las figuras 4a a 4d muestran varias vistas de un
conector para conectar de forma neumática el relleno poroso a una
fuente de presión negativa; y
la figura 5 es una vista en planta de una
cubierta modificada.
La realización mostrada en los dibujos adjuntos
está diseñada para su uso en el tratamiento de quemaduras en la
mano.
Se apreciará que son posibles diversas
modificaciones apropiadas para el tratamiento de quemaduras en otras
partes del cuerpo, tales como pies, dentro del alcance de la
invención.
Las figuras 1, 2 y 3 muestran un aparato (10)
para tratar heridas en las manos que comprende un relleno poroso
que tiene una base (22) inferior, una sección (24) media y una
sección (26) superior incorporadas dentro de una cubierta (12) de
baja porosidad de aire. El relleno poroso está en la forma de un
guante o mitón y puede construirse fijando la sección (26) superior
a la base (22) mientras se retiene la sección (24) media dentro de
la cavidad así formada. Normalmente, el relleno poroso es una espuma
de plástico reticulada, y puede formarse pegando o soldando juntas
las diferentes secciones. Cuando se coloca dentro del relleno, se
mantiene la mano en su sitio con los dedos extendidos mediante
separadores (25) de dedos y ranuras (14) de tipo de corte en V. Tal
como se muestra en las figuras 2 y 3, la espuma puede conformarse
para proporcionar un compartimento (27) de soporte separado para el
dedo pulgar para ayudar a conseguir las posiciones óptimas de los
dedos y el pulgar para su cicatrización. Sin embargo, esto no es
esencial. El relleno está hecho preferiblemente de una espuma
reticulada tal como poliuretano tal como se describe en la solicitud
PCT WO 96/05873, espuma de poli(alcohol vinílico) o una
combinación de las mismas.
La figura 3 muestra el relleno montado tras su
inserción dentro de una cubierta (12). La cubierta (12) es una
envoltura formada de un material laminar impermeable al aire, por
ejemplo, una película de poliuretano o poliolefina, y se dimensiona
para encerrar el relleno poroso con forma de guante. Un extremo de
la cubierta (12) tiene una abertura grande que puede sellarse
mediante un medio (4) que puede volver a sellarse fácilmente tal
como un cierre de tipo cremallera usado en bolsas para alimentos. El
otro extremo (8) incluye un adhesivo (9) de resina acrílica
sensible a la presión y sustancialmente impermeable, cuya parte
inferior se fija como un cierre ajustado a la piel del paciente. El
extremo (8) abierto puede recubrirse sobre su superficie interior
con un adhesivo (9) de resina acrílica sensible a la presión con el
fin de sellar la cubierta a la piel del paciente, por ejemplo, en
la muñeca o la parte inferior del brazo. Como alternativa, el
extremo (8) puede sellarse en la muñeca del paciente con un trozo
de cinta adhesiva aparte, tal como una película de poliuretano
recubierta con un adhesivo sensible a la presión. El extremo (8)
abierto está abierto y es cónico tal como se muestra.
Proporcionando una conicidad, el extremo abierto puede cortarse a un
tamaño de manera que la abertura se ajustará perfectamente
alrededor de la muñeca del paciente. Unido a la cubierta (12) en la
región de una parte central del relleno poroso hay un conector
(100). El conector (100) puede estar unido a la cubierta mediante
adhesivo.
La figura 5 es una vista en planta de una
cubierta similar a la cubierta (12) mostrada en la figura 3. Se
usan los mismos números de referencia para indicar las partes
correspondientes. La cubierta mostrada en la figura 5 se diferencia
de la mostrada en la figura 3 en que el extremo (8) para la unión a
la muñeca del paciente tiene una conicidad algo más grande y está
diseñado de modo que el extremo puede cortarse para adaptarse al
paciente. El conector (100) tiene generalmente una brida (101)
circular cuya cara inferior (es decir, la cara que durante su uso
está en contacto con la hoja 20 de espuma) está formada con pequeños
botones salientes. La construcción de este aspecto del conector es
tal como se describe en la solicitud de patente británica número
2.333.965. En vez de usar un sellado de bloqueo de tipo cremallera,
puede usarse un sellado de tipo "Velcro". En esta realización,
un sellado (4) de bloqueo de tipo cremallera está formado de
polietileno y éste está unido al resto de la cubierta, que está
formada de poliuretano, mediante cinta adhesiva.
Las figuras 4a a 4d muestra diversas vistas del
conector (100) y se observará que comprende una parte (101) de
brida de plástico moldeado y un orificio de succión que tiene un
conducto (102) y una abertura (106) colocados centralmente. El
conector (100) está firmemente unido a la cubierta mediante un
adhesivo. El conducto se extiende a través de un agujero cortado en
la cubierta y la superficie superior de la brida 101 está unida con
adhesivo a la cubierta (12). El conducto (102) está dimensionado
para aceptar como un ajuste estrechamente deslizante, el extremo de
un tubo (30) de única o múltiples luces que sale de debajo de la
cubierta (12) de herida. El tubo (30) puede construirse tal como se
describe en la solicitud de patente en tramitación junto con la
presente WO 97/18007. Cuando se usa un tubo de múltiples luces,
puede utilizarse una luz para medir la presión en el sitio de la
quemadura. Está también dentro del alcance de esta invención irrigar
la quemadura u otra herida a través de una de las luces o mediante
un conector separado del relleno de espuma. El conector o conectores
pueden usarse para introducir fármacos, por ejemplo, antibióticos,
al sitio de la herida. El paño (12) de cubierta está hecho
preferiblemente de una película flexible de baja permeabilidad de
aire tal como poliuretano y puede incluir una capa protectora de
polietileno. En la solicitud de patente británica nº 2.333.965 se
describen materiales adecuados.
Durante su uso, se introduce la mano de un
paciente que tiene una lesión por quemadura dentro de la cubierta
(12) exterior mediante el extremo (8) abierto. La abertura (4) que
puede volver a sellarse puede abrirse entonces y volverse a doblar
para exponer la mano lesionada. Se introduce entonces la mano dentro
del relleno poroso que puede montarse previamente o montarse in
situ alrededor de la mano lesionada. En este último caso, puede
ser conveniente fijar la sección (26) superior a la sección (24)
inferior suturando o grapando, en vez de pegar o soldar la espuma.
Con el relleno de espuma en su sitio rodeando la mano lesionada, se
retira la cubierta (12) sobre el relleno poroso y se vuelve a
sellar la abertura (4). Se conecta después el conducto (102)
mediante un tubo a una bomba de succión, por ejemplo, usando la
técnica descrita en el documento WO 97/18007. Puede aplicarse
presión negativa pulsada, intermitente o continua a la mano del
paciente según un programa que puede controlarse automáticamente
mediante un dispositivo de control asociado con la bomba, tal como
se describe en nuestra solicitud de patente anterior. Se ha
encontrado que el tratamiento de presión negativa usando el aparato
de la invención estimula la cicatrización de quemaduras y reduce la
progresión de la muerte celular por debajo de una lesión por
quemadura. Además, mediante la mejora el flujo sanguíneo en la zona
de la herida, se controla la infección y se estimula la granulación
de la herida.
Un efecto beneficioso adicional del tratamiento
mediante el uso del aparato de esta invención es que durante el
tratamiento, se mantiene la mano firmemente en una posición medio
cerrada, que es la posición óptima para potenciar la cicatrización.
Esto puede fomentarse adicionalmente mediante la introducción de un
entablillado rígido o semirrígido, por ejemplo, de plástico, que se
forma o moldea en la forma deseada, sujetándose con correas el
apósito plegado al entablillado durante o tras la aplicación de la
succión, de modo que puede mantenerse la posición de cicatrización
deseada tras la liberación de la succión.
Se controla la bomba de succión preferiblemente
mediante medios de control que incluyen un transductor de presión
para monitorizar la presión en el sitio de la herida tal como se
describe en nuestra solicitud PCT anterior. Puede asociarse también
un dispositivo temporizador con la bomba para proporcionar un
funcionamiento de encendido / apagado si fuese necesario a
intervalos seleccionados. El aparato puede incluir también un
recipiente ubicado entre el relleno poroso y la bomba para recoger
el exudado de la herida. Normalmente, la bomba es una bomba de
diafragma pero pueden sustituirse por otros tipos de bombas y
componentes equivalentes, tales como botellas de vacío. También
puede usarse el aparato con una pequeña fuente de succión de pared
tal como se describe en la solicitud de patente británica número
2.342.584.
Los términos y expresiones que se han utilizado
se usan como términos de descripción y no de limitación. Aunque la
presente invención se refiere principalmente a quemaduras de espesor
parcial, se entiende que la presente invención puede usarse con
heridas abiertas así como un posible tratamiento de úlceras de
decúbito.
Claims (6)
1. Un aparato para estimular la
cicatrización de heridas que comprende una envoltura para alojar
una parte afectada del cuerpo, incluyendo dicha envoltura una
cubierta (12) sustancialmente estanca al aire y un relleno (22, 24,
26) poroso dentro de la cubierta, adaptándose dicho relleno para
ponerse en contacto con la superficie de la herida, y medios (100)
de conexión para conectar el interior de la envoltura a una fuente
de presión negativa, teniendo la cubierta una abertura (4) que puede
volver a sellarse que permite inspeccionar la herida a
intervalos.
2. Aparato según la reivindicación
1, en el que la envoltura comprende un guante (20), calcetín o
manga.
3. Aparato según la reivindicación
2, que está destinado para tratar heridas en la mano y comprende un
guante (22, 24, 26) formado de espuma de caucho o plástico flexible
y una cubierta (12) externa de un material laminar impermeable al
aire.
4. Aparato según la reivindicación
3, en el que los medios de conexión comprenden un tubo (102) que se
comunica con la espuma dentro de la cubier-
ta.

las reivindicaciones anteriores, en el que la fuente de presión
negativa es una bomba de succión.
6. Aparato según una cualquiera de
las reivindicaciones anteriores, que incluye un recipiente para
recoger el exudado de la herida.
