Abstract

Description
Electrodo electroquirúrgico recubierto.
Este invento corresponde a tipos específicos de
electrodos electroquirúrgicos que se utilizan en operaciones
quirúrgicas, que tienen un recubrimiento de silicona que resiste la
formación de escaras y aumenta la facilidad de limpieza de la
formación de escaras.
Los electrodos electroquirúrgicos se utilizan en
operaciones quirúrgicas para cortar y coagular los tejidos de un
paciente. Los electrodos se utilizan para conducir energía
electroquirúrgica de alta frecuencia desde un generador al paciente.
Los cirujanos disponen de electrodos electroquirúrgicos de muchos
tamaños y formas. La mayoría de los electrodos electroquirúrgicos
son de metal, normalmente acero inoxidable, y estos electrodos
incluyen normalmente un empalme de forma que puedan insertarse en
una pieza manual o "lápiz".
El extremo de trabajo del electrodo
electroquirúrgico está sometido a altas temperaturas durante su
utilización. Esta alta temperatura hace que las proteínas, hidratos
de carbono y lípidos del cuerpo se coagulen en el tejido así como
también que se adhieran al extremo de trabajo. Este coágulo se
denomina comúnmente "escara". La escara que se adhiere al
extremo de trabajo no es deseable ya que reduce las propiedades de
corte y de coagulación del electrodo electroquirúrgico.
Usualmente se hacen esfuerzos durante la
operación quirúrgica para mantener limpio el extremo frotando o
cepillando el extremo contra una almohadilla limpiadora. Es
frecuentemente difícil limpiar el electrodo. Esto deja al cirujano
con las opciones de sustituir el electrodo durante la operación
quirúrgica, aceptando un funcionamiento reducido, o gastando el
valioso tiempo y la atención del médico para limpiar el extremo
completamente.
Se han utilizado varios métodos para tratar de
resolver el problema de la formación de escaras. Un primer método es
desarrollar mejores mecanismos para limpiar el electrodo durante la
operación quirúrgica. Un segundo método consiste en desarrollar
recubrimientos para los electrodos electroquirúrgicos para reducir
la formación de escaras y para hacer que el electrodo sea más fácil
de limpiar.
Varias patentes describen un método y un aparato
para limpiar un electrodo de extremo de lápiz electroquirúrgico. La
patente US 5.016.401 tiene una almohadilla fibrosa y abrasiva y está
montada en una base dura que podría ser unida a un paño quirúrgico.
La patente US 4.547.923 tiene un limpiador de bisturí con fibras
enrolladas apretadamente soportadas por una base unida por adhesivo
al paño quirúrgico. La acción limpiadora se produce cuando el
electrodo se inserta a través de los rollos contiguos del
dispositivo; es posiblemente un muelle. Un dispositivo similar se
encuentra en la patente US 4.852.200.
La patente US 4.925.516 reivindica la idea de una
lámina de tirar hacia afuera para exponer un adhesivo viscoso en la
base del dispositivo limpiador del bisturí. Existen varias patentes
que describen un alojamiento que tiene una ranura receptora de un
electrodo y cepillos para limpiar el electrodo quirúrgico. Las
patente son US 4.752.983, 4.704.760 y 4.087.878. La patente US
5.191.670 muestra un aparato limpiador de horquilla tubular que
tiene un cepillo en zig-zag con cerdas.
Otros documentos han descrito electrodos
electroquirúrgicos que están diseñados para resistir la formación de
escaras. La patente US 4.314.559, cedida a Corning Glass Works,
describe bisturíes quirúrgicos con electrodos bipolares adjuntos
para coagular la sangre resultante de cortar con el filo del
bisturí. La patente US 4.314.559 tiene una primera capa conductora
para cubrir el electrodo electroquirúrgico y una segunda capa de
Teflon para proporcionar una superficie no adherente. La capa
conductora es tal que partes de la misma están expuestas y forman
una conexión entre el electrodo quirúrgico y la superficie de tal
forma que el Teflon solamente llena los intersticios, inclusiones y
similares en la superficie, proporcionando así la superficie no
adherente del instrumento de corte o de coagulación.
La patente US 4.785.807 tiene una imprimación y
recubrimiento superior de polímero de Teflon sobre un extremo de
acero inoxidable decapado o abrasionado. El recubrimiento es de poco
espesor y durante la aplicación de energía electromagnética se dice
que existe un acoplamiento capacitativo para permitir el paso de
energía al tejido que se está tratando.
La patente US 4.492.231 trata de la temperatura,
la conductividad del extremo y la adherencia de la sangre seca en un
fórceps bipolar.
La patente australiana 637.755 tiene un eje
conductor con un aislamiento que proporciona aislamiento térmico y
eléctrico y resistencia a la abrasión a lo largo del eje entre sus
extremos. El aislamiento está proporcionado por un tubo de plástico
ajustado en caliente.
El documento
US-A-5.380.320 describe un
instrumento electroquirúrgico que tiene un miembro alargado
conductor, un conector para conectar un suministro externo de
energía eléctrica al extremo próximo del miembro alargado, un
elemento electroquirúrgico en el extremo distal del miembro alargado
y un recubrimiento de parileno aislante que cubre parte del miembro
alargado. El recubrimiento de parileno es de un espesor de
aproximadamente 25-150 \mum y sirve para impartir
una función de no pegajosidad al instrumento quirúrgico. Esto
permite una limpieza más fácil del instrumento.
El documento
GB-A-2 060 397 describe un
instrumento quirúrgico no adherente (por ejemplo un escalpelo
quirúrgico caldeado) que puede utilizarse para cirugía en tejidos a
temperaturas a las que se produce una hemostasis con una lesión
mínima del tejido. El instrumento quirúrgico comprende un
recubrimiento no adherente que tiene parámetros específicos,
incluida una impedancia térmica, caída de espesor y térmica a través
del mismo. Este recubrimiento puede estar en forma de una capa
maciza, una capa maciza sacrificadora o en forma de una capa líquida
sacrificadora. En el caso de que se seleccione la capa no adherente,
ésta se puede formar en el dispositivo a partir de, por ejemplo, un
polímero fluorocarbonado o una silicona. En el caso de que se
utilice un recubrimiento no adherente macizo sacrificador, se puede
utilizar una grasa de silicona, un hidrocarburo, una cera de un
éster natural o sintética o un compuesto de sulfuro.
Alternativamente, en calidad de recubrimientos no adherentes macizos
sacrificadores se pueden utilizar copolímeros de
etileno-propileno fluorados. En calidad de
recubrimiento no adherentes líquido sacrificador se pueden utilizar
aceite de silicona o fluidos perfluoropolisintéticos.
Se describe un electrodo electroquirúrgico que
resiste la formación de escaras y mejora la facilidad de limpieza de
cualquier formación, tal como escaras, teniendo un recubrimiento de
elastómero de silicona. El elastómero de silicona puede ser
polidiorganosiloxano. Se describe aquí el electrodo recubierto y el
método de fabricación.
Un primer aspecto de la presente invención
proporciona un electrodo (10) para una herramienta electroquirúrgica
a utilizar en cirugía en los tejidos de un paciente, comprendiendo
el electrodo (10):
un eje metálico (13) alargado que tiene un
extremo próximo y un extremo distal, en el que el extremo próximo
tiene una forma para ser insertado en la herramienta
electroquirúrgica para conducir energía electroquirúrgica, en donde
el extremo distal tiene una forma a modo de cuchilla con al menos
dos superficies principales (14) que son generalmente paralelas y
están unidas por un borde (15);
una parte extrema de paciente en el extremo
distal para manipulación del tejido del paciente, en donde la parte
del extremo del paciente, antes de ser recubierta, tiene una
rugosidad superficial en el intervalo de aproximadamente 3,175 a
16,129 \mucm Ra (127 a 635 \mucm Ra);
caracterizado porque el electrodo comprende,
además, un recubrimiento de base (12) de material de imprimación en
la parte del extremo del paciente, comprendiendo el recubrimiento de
base una resina de polisiloxano con un catalizador reticulante;
y
un recubrimiento superior (11) sobre el

Preferiblemente, el espesor no uniforme
proporciona una conductividad eléctrica menor a lo largo de las
superficies principales (14) que a lo largo del borde (15).
Preferiblemente, el valor dieléctrico mayor
resulta de un recubrimiento superior generalmente más grueso a
través de principalmente las superficies principales (14).
Un aspecto del diseño es que se puede utilizar un
recubrimiento de silicona relativamente grueso. El material de
silicona tiene un espesor suficiente en la zona plana de la cuchilla
para resistir el voltaje ES (electroquirúrgico) sin interrupción. El
camino electroquirúrgico es principalmente a través de los bordes y
extremo de la cuchilla, en los que el recubrimiento puede tener
relativamente poco espesor o no existir. La parte plana de la
cuchilla permanece aislada en la mayoría de las condiciones
electroquirúrgicas. El recubrimiento grueso también proporciona alta
resistencia al desgarramiento y una mayor resistencia debido al
enlace cohesivo a sí mismo. Esto lo hace más resistente al desgaste
mecánico.
La composición de los recubrimientos del
electrodo incluye elastómeros de silicona en forma de adhesivos,
dispersiones o cauchos líquidos. El polímero fundamental es
generalmente aunque no limitado a la forma de R_{1}R_{2}SiO en
el que los grupos R son fenilo, metilo, vinilo, u otros grupos
colgantes. Además de los anteriores componentes, la composición del
recubrimiento puede contener algunos o todos los siguientes
componentes, que incluyen facilitadores de adherencia,
estabilizadores térmicos, productos de relleno, plastificantes,
mejoradores de desmoldeo, agentes de degradación y colorantes.
Existen varios procesos para depositar el
elastómero de silicona. En una realización, el elastómero de
silicona está en forma de una dispersión, y la deposición es
mediante inmersión. Las propiedades viscoelásticas de las
dispersiones son tales que cuando se ha realizado la inmersión, los
recubrimientos resultantes son muy delgados en el extremo y bordes
de la cuchilla y mucho más gruesos en la zona plana media. En una
realización preferida el material delgado en el borde es demasiado
delgado para aislar esta zona de la corriente electroquirúrgica y,
cuando la energía electroquirúrgica se detiene, inmediatamente
conduce corriente. Con los bordes conduciendo y la zona plana
aislada, se mejora la capacidad para cortar por la creación de una
alta densidad de corriente en el borde que rápidamente genera un
barrera de vapor para cortar. El proceso de inmersión también crea
un recubrimiento grueso en la zona plana con un perfil curvo que
puede mejorar la facilidad de limpieza debido a que cuando la
cuchilla se frota contra una superficie, la curva proporciona un
alto punto de contacto y por tanto una mayor presión en la parte
plana de la cuchilla, mayor en el centro y disminuyendo hacia
el
borde.

La cuchilla recubierta de silicona es fácil de
limpiar y no adhesiva debido a que la silicona no se empapa, lo que
proporciona una superficie de unión muy pequeña a la escara. Además,
la superficie del elastómero cede ante las acciones mecánicas (frote
o corte). Esto rompe la unión con la escara. Además, el tejido en
contacto con la superficie de silicona no alcanza las temperaturas
requeridas para el pegado significativo de la escara debido a que la
superficie de silicona está aislada térmicamente y existe
relativamente poca conducción de corriente electroquirúrgica en esa
zona. Esto representa una zona significativa de la cuchilla.
La cuchilla con recubrimiento de silicona
explicada tiene ventajas sobre las cuchillas electroquirúrgicas
existentes actualmente. Proporciona un mejor corte (más consistente
desde el comienzo y requiere menor energía) debido a que los bordes
de la cuchilla son conductores electroquirúrgicos en tanto que la
parte plana no es significativamente conductora. Esto puede permitir
que se genere más rápidamente una barrera de vapor, que es deseable
para cortar. Hay también una ejecución de limpieza más consistente
debido a que la parte plana de la cuchilla, que representa la mayor
zona de superficie, permanece sustancialmente aislada (sin poros ni
deterioro) durante un periodo largo de uso. El perfil en curva que
se extiende desde la zona plana de la cuchilla mejorará la capacidad
de limpieza. Un cirujano también puede aplicar energía
electroquirúrgica a hemostatos con el borde de la cuchilla sin daño
al recubrimiento. La cuchilla con revestimiento también se puede
curvar sin daño importante al recubrimiento.
Los adhesivos de silicona pueden ser un
elastómero de silicona autonivelante de un componente, que no
contenga disolventes. Un ejemplo es el NuSil
Med-1511, suministrado por NuSil Technology, Inc.,
situada en Carpinteria, California. Se puede diluir en un disolvente
orgánico hasta el 30%-70% de sólidos, preferiblemente hasta el 50%
de sólidos. El mecanismo de curado con acetoxina de este sistema se
produce a temperatura interior y del 20% al 60% de humedad relativa.
Durante el proceso de curado, el adhesivo de silicona emite vapor de
ácido acético como un subproducto.
La dispersión de silicona puede ser un sistema de
dos componentes de líquidos de viscosidad media que están mezclados
en una relación de 1:1 en peso o volumen. Un ejemplo es el NuSil
Med-6640. Puede ser un 25% de sólidos dispersos en
un disolvente orgánico y puede ser posteriormente diluido al 7% de
sólidos. La dispersión puede realizarse desde una goma de silicona
de alta consistencia con 5.000 grados de polimerización de media. El
mecanismo de curado por adición de platino de este sistema se
produce a temperaturas elevadas.
Los cauchos de silicona líquida puede ser un
sistema de dos componentes que contiene el 100% de pasta de
elastómero de silicona que están mezclados en una relación de 1:1.
Un ejemplo es el Applied SiLSR-50, suministrado por
Applied Silicone Corporation, situada en Ventura, California. La
desproporción molecular de estos componentes puede mejorar la
adherencia al sustrato. El elastómero puede estar diluido en un
disolvente orgánico al 20%-50% de sólidos, siendo preferiblemente el
30% de sólidos. El mecanismo de curado por adición de platino de
este sistema se produce a temperaturas elevadas. La dureza
durométrica del LSR puede estar comprendida en el intervalo
5-70, aunque es preferible 50.
Una cuchilla recubierta de silicona puede ser
fabricada de la siguiente forma. Un electrodo electroquirúrgico
puede tener sus superficies exteriores rugosas con el fin de mejorar
la adherencia de una imprimación. En la realización preferida el
electrodo sin recubrimiento se trata con chorro de arena de forma
que tenga una rugosidad superficial del orden de 3,175 a 16,129
\mucm Ra (127 a 635 \mucm Ra) pero preferiblemente de 11,43 a
14,478 \mucm Ra (444,5 a 571,5) o mayor. El proceso de tratamiento
con chorro de arena puede ser realizado con gránulos de óxido de
aluminio de 16 a 100 (es decir, óxido de aluminio con un tamaño
medio de partícula de 122-1854 micras), aunque
también se pueden utilizar otras técnicas de corrugación.
Se aplica una imprimación al electrodo rugoso. La
imprimación es una resina de polisiloxano. Puede ser una mezcla de
dos silanos tetrafuncionales, un titanato tetrafuncional, y un
catalizador adecuado de reticulación. El espesor de la imprimación
es normalmente menor de 100 \ring{A}.
El electrodo imprimado está recubierto con
polímero de elastómero de silicona. Varios tipos de polímeros de
silicona son efectivos. La realización preferida utiliza
polidimetilsiloxano de metilvinilo con función de bloque final de
vinilo. El espesor total del recubrimiento de silicona es
nominalmente de 193,04 a 1.290,32 \mucm (3 a 20 milésimas de
pulgada). La uniformidad del espesor se ajusta más preferiblemente
con mayor espesor en ciertas zonas para proporcionar más resistencia
dieléctrica donde se prefiere que no haya paso de energía
electromagnética de alta frecuencia para mejorar las propiedades de
no adherencia y de facilidad de limpieza.
Se dispone de varios métodos de curado de
elastómeros de silicona. El método de curado preferido es una
reacción de adición que utiliza una catálisis de un compuesto de
platino orgánico y calor. Otros métodos de curado incluyen
reacciones de condensación utilizando la humedad ambiental, el
curado con humedad iniciado por rayos ultravioleta y la
vulcanización con peróxido.
La figura 1 es una vista en perspectiva de un
electrodo recubierto no uniformemente que tiene una forma de
cuchilla.
La figura 2 es una vista de la sección
transversal a lo largo de la línea 2-2 de la figura
1 y que muestra el electrodo metálico conductor con un recubrimiento
de imprimación y un recubrimiento superior de silicona. La
imprimación y la silicona se muestran ampliadas con fines de
ilustración.
La figura 3 es una vista longitudinal de la
sección transversal del electrodo de la figura 1 tal como se ve
desde el plano de líneas 3-3 de ella y que muestra
el electrodo metálico conductor con la imprimación y la silicona
ampliadas y no a escala con fines ilustrativos.
Un electrodo electroquirúrgico 10 recubierto, tal
como se muestra en la figura 1, resiste la formación de escaras y
mejora la facilidad de limpieza de cualquier formación tal como
escaras. El electrodo 10 tiene un recubrimiento 11 de elastómero de
silicona, tal como se muestra en las figuras 2 y 3, que
principalmente comprende polidiorganosiloxano. El electrodo
recubierto 10 y sus métodos de fabricación se describen aquí. Varias
técnicas de fabricación alternativas se describen por medio de
ejemplos.
En una realización un electrodo electroquirúrgico
13 de acero inoxidable de calidad médica puede tener sus superficies
exteriores rugosas con el fin de mejorar la adherencia de una
imprimación 12. El electrodo sin recubrir es tratado con chorro de
arena de forma que tenga una rugosidad superficial del orden de
3,226 a 16,129 \mucm Ra (127 a 635 \mucm Ra) pero
preferiblemente de 11,43 a 14,478 \mucm Ra (175 a 225). El proceso
de tratamiento con chorro de arena puede ser realizado con gránulos
de 16 a 100 de óxido de aluminio, aunque también se pueden utilizar
otras técnicas de corrugación. El pretratamiento de corrugación para
mejorar la adherencia puede incluir granallado, limpieza en tambor,
textura estampada, perforaciones a través del sustrato metálico
conductor mediante EDM por ejemplo, atacado o erosión química con
ácido o sosa cáustica.
Se aplica una imprimación 12 al sustrato 13
metálico conductor del electrodo rugoso. La imprimación 12 es
preferiblemente una resina de polisiloxano. El espesor de la
imprimación 12 es normalmente menor de 100 \ring{A} y la
imprimación 12 se aplica uniformemente.
El electrodo imprimado se recubre por encima con
un polímero 11 de elastómero de silicona. Varios tipos de polímeros
de silicona son efectivos. Una realización tiene un
polidimetilsiloxano de metilvinilo con funcionalidad de bloque final
de vinilo y material de relleno, preferiblemente un material de
relleno de sílice amorfo. El espesor total del recubrimiento de
silicona es nominalmente de 193,04 a 1.290,32 \mucm (3 a 20
milésimas de pulgada).
El espesor del recubrimiento 11 de silicona no es
uniforme. El recubrimiento 11 de silicona está a lo largo de los

muestra en las figuras 2 y 3 más delgado, en el que el paso de
corriente eléctrica de alta frecuencia es deseable para cortar. El
espesor del recubrimiento 11 de silicona es mayor en las superficies
principales de la cuchilla, tal como se muestra en la figura 2 con
el fin de mejorar la facilidad de limpieza. El término
"espesor" está pensado para describir la dimensión entre el
sustrato 13 y una superficie exterior 16 del recubrimiento.
La significación de un recubrimiento 12 no
uniforme es que la corriente eléctrica de alta frecuencia será
conducida preferentemente a través de regiones del electrodo 10 en
las que la resistencia eléctrica es baja. Si el recubrimiento en los
bordes 15 del electrodo 10 es delgado, o no existe, entonces la
corriente será conducida preferentemente a través de los bordes 15.
Las superficies planas o principales 14 tienen un recubrimiento más
grueso que proporcionará mayor aislamiento eléctrico que el que hay
en los bordes 15. Esto puede producir una mayor eficacia de corte y
una menor tendencia a la formación de escaras en las superficies
principales 14.
Se ha visto mediante experimentos que la
radiación ionizante de hasta 10 megarads tiene un efecto beneficioso
sobre el recubrimiento de silicona. En particular, el recubrimiento
de silicona irradiado mostró una adherencia mejorada y una
pegajosidad reducida.
Siguen varios ejemplos adicionales de métodos de
fabricación de una cuchilla electroquirúrgica recubierta. Los
ejemplos 4, 5, y 6 son variaciones preferidas del mismo proceso de
fabricación básico. El ejemplo 9 es también una realización
preferida.
Cuchillas de electrodo de acero inoxidable de la
serie 304 tratadas con chorro de arena con gránulos 80 de óxido de
aluminio (es decir, partículas de óxido de aluminio con un tamaño
medio de partícula de 165 micras) a una rugosidad superficial de
3,226 a 5,156 \mucm Ra (127-203,2 \mucm Ra)
fueron limpiadas con ultrasonidos en un baño de acetona durante 15
minutos y después se les dio una imprimación con
SP-120. Se permitió el secado de la imprimación al
aire durante una hora en condiciones ambientales. Las cuchillas
fueron recubiertas por inmersión en una solución de NuvaSil 5083, un
adhesivo curado por humedad/UV suministrado por Loctite Corporation
situada en Hartford, Connecticut. La solución fue disuelta hasta el
14% en peso de sólidos en xileno. Se permitió que las cuchillas se
evaporasen súbitamente en condiciones ambientales durante 30 minutos
y después fueron situadas bajo una luz ultravioleta que proporciona
400 mW/cm^{2} de energía UV a una longitud de onda de 365 nm
durante 30 segundos en cada lado de la cuchilla. Las cuchillas
recubiertas se dejaron curar al aire en condiciones ambientales
durante 48 horas.
Cuchillas del electrodo fueron tratadas con
chorro de arena con medios de óxido de aluminio de gránulos de 80
(es decir, partículas de óxido de aluminio con un tamaño medio de
partícula de 165 micras) hasta una rugosidad superficial de 3,226 a
5,156 \mucm Ra (127-203,2 \mucm Ra). Después
fueron limpiadas ultrasónicamente en un baño de acetona durante 15
minutos. Las cuchillas fueron después imprimadas con NuSil
CF6-135, suministrado por NuSil Technology, Inc. Se
permitió al agente imprimador secarse al aire durante 30 minutos en
condiciones ambientales. Las cuchillas fueron después recubiertas
por inmersión en una solución formada por un 44% de dispersión de
elastómero de silicona de NuSil MED-6640 y 56% de
xileno en peso. La concentración de solución de mezcla resultante de
sólidos elastoméricos fue el 11%. De esta solución se obtuvieron dos
tipos de cuchillas recubiertas, un tipo fue sumergido solamente una
vez mientras que al otro tipo se le permitió evaporarse súbitamente
durante 20 minutos en condiciones ambientales y después fue
sumergida nuevamente en la solución. Se permitió a ambos tipos
evaporarse súbitamente durante un mínimo de 20 minutos después de su
última inmersión, y después fueron curados en un horno de convección
de circulación forzada de aire a 75º\pm2ºC durante 45 minutos,
seguido por 135 minutos en un horno de convección de circulación
forzada de aire a 150º\pm5ºC. Este proceso proporcionó a las
cuchillas un espesor de película de 51,562 a 96,774 \mucm (0,8 a
1,5 milésimas de pulgada).
Cuchillas de electrocirugía preparadas como en el
Ejemplo 2 anterior fueron recubiertas con la misma solución de
sólidos del 11% en peso de NuSil MED-6640 en xileno.
El método de aplicación del recubrimiento fue un aerosol
electrostático, con la cuchilla como la parte de toma de tierra del
circuito. Después de 1-3 pasos de la boquilla del
aerosol, se permitió a las cuchillas recubiertas evaporarse
súbitamente en condiciones ambientales durante 30 minutos antes del
curado de la misma manera que las cuchillas en el Ejemplo 2. El
espesor de la película estaba comprendido entre 64,516 y 161,29
\mucm (1,0 a 2,5 milésimas de pulgada).
Las cuchillas y el NuSil MED-6640
del Ejemplo 2 se utilizaron en un proceso de inmersión múltiple para
producir películas más gruesas en el producto terminado. Los sólidos
en peso de la solución fueron reducidos al 9% con la adición de más
xileno. Este cambio fue realizado para mejorar la uniformidad de la
capa de recubrimiento a lo largo de la longitud de la cuchilla. Las
cuchillas fueron sumergidas perpendicularmente a la superficie de la
solución, sumergidas en la solución hasta una profundidad que cubría
completamente la parte plana de la cuchilla, retiradas con el mismo
ángulo de entrada, basculadas 180º y mantenidas durante 10 segundos,
y después rotadas en una plantilla helicoidal compleja para
garantizar el mismo espesor de la película a lo largo de la longitud
de la cuchilla. Después de 20 minutos de rotación, se repitió el
proceso de inmersión hasta un total de 8 veces. Después de la última
rotación de 20 minutos, las cuchillas fueron colocadas en un horno
de convección de aire forzado y curadas durante 135 minutos a 160ºC.
En cada una de las cuchillas individuales el espesor de la película
estaba comprendido entre 193,548 y 387,096 \mucm (3 a 6 milésimas
de pulgada) con el recubrimiento mayor en el extremo de la
cuchilla.
Dos conjuntos individuales de cuchillas de
electrodos tratadas con chorro de arena con medios de óxido de
aluminio de gránulos de 80 (es decir, partículas de óxido de
aluminio con un tamaño medio de partícula de 165 micras) hasta una
rugosidad superficial de 3,226 a 7,874 \mucm Ra
(127-304,8 \mucm Ra). Las cuchillas fueron
limpiadas después ultrasónicamente en un baño de acetona durante 15
minutos. Las cuchillas fueron después imprimadas con
CF6-135. Se permitió al agente imprimador secarse al
aire durante 30 minutos en condiciones ambientales. Las cuchillas
fueron después recubiertas por inmersión en una solución de
MED-6640 diluida al 9% de sólidos por peso en
xileno. Las cuchillas fueron sumergidas perpendicularmente a la
superficie de la solución, sumergidas en la solución hasta una
profundidad que cubría completamente la parte plana de la cuchilla,
retiradas con el mismo ángulo de entrada, basculadas 180º y
mantenidas durante 6 segundos, y después rotadas en una plantilla
helicoidal compleja para garantizar el mismo espesor de la película
a lo largo de la longitud de la cuchilla. Después de 20 minutos de
rotación, se repitió el proceso de inmersión hasta un total de 6 a
10 veces. Después de la última rotación de 20 minutos, las cuchillas
fueron colocadas en un horno de convección de aire forzado y curadas
durante 135 minutos a 160ºC. En cada una de las cuchillas
individuales el espesor de la película estaba comprendido entre
225,806 \mucm (3,5 milésimas de pulgada) para las cuchillas
sumergidas 6 veces y 387,096 \mucm (6 milésimas de pulgada) para
las cuchillas sumergidas 10 veces.
Las cuchillas del electrodo fueron tratadas con
chorro de arena con medios de óxido de aluminio de gránulos de 80
(es decir, partículas de óxido de aluminio con un tamaño medio de
partícula de 165 micras) hasta una rugosidad superficial de 7,874
\mucm Ra (304,8 \mucm Ra). Las cuchillas fueron después
limpiadas ultrasónicamente en un baño de acetona durante 15 minutos.
Las cuchillas fueron después imprimadas con CF6-135.
Se permitió al agente imprimador secarse al aire durante 30 minutos
en condiciones ambientales. Las cuchillas fueron después recubiertas
por inmersión en una solución de MED-6640 diluida al
9% de sólidos por peso en xileno. Las cuchillas fueron sumergidas
perpendicularmente a la superficie de la solución, sumergidas en la
solución hasta una profundidad que cubría completamente la parte
plana de la cuchilla, retiradas con el mismo ángulo de entrada,
basculadas 180º y mantenidas durante 6 segundos, y después rotadas
en una plantilla helicoidal compleja para garantizar el mismo
espesor de la película a lo largo de la longitud de la cuchilla.
Después de 20 minutos de rotación, se repitió el proceso de
inmersión hasta un total de 13 veces. Después de la última rotación
de 20 minutos, las cuchillas fueron colocadas en un horno de
convección de aire forzado y curadas durante 135 minutos a 160ºC. En
cada una de las cuchillas individuales el espesor de la película
estaba comprendido entre 322,58 a 516,128 \mucm (5 a 8 milésimas
de pulgada).
Cuchillas del electrodo fueron tratadas con
chorro de arena con medios de óxido de aluminio para conseguir una
rugosidad superficial de 7,874 \mucm Ra (120 Ra). Las cuchillas
después fueron limpiadas ultrasónicamente en un baño de acetona
durante 15 minutos. Agujeros con diámetros comprendidos entre 0,127
mm a 1,905 mm (0,005 pulgadas a 0,075 pulgadas) fueron perforados a
través de la parte plana de la cuchilla para proporcionar un sitio
para que el elastómero de silicona encapsulara la cuchilla con
adherencia mejorada. Las cuchillas fueron después colocadas en un
molde de compresión/transferencia y el NuSil
MED-4750 fue obligado a entrar en la cavidad,
recubriendo las dos caras de la cuchilla y penetrando la superficie
de la cuchilla por los agujeros perforados. Después de 10 minutos en
el molde a 116ºC (240ºF), el molde fue abierto y se retiró la
cuchilla recubierta. En cada una de las cuchillas individuales el
espesor de la película estaba comprendido entre 129,032 y 258,064
\mucm (2 a 4 milésimas de pulgada).
Las cuchillas fueron preparadas de la misma forma
que en el Ejemplo 7, siendo NuSil MED-4720 el
elastómero utilizado en lugar del NuSil
MED-4750.
Las cuchillas fueron preparadas de la misma forma
y recubiertas por inmersión de la misma forma que en el Ejemplo 6.
Antes de curar el elastómero de silicona se aplicó un aerosol
antiadherencia de Teflon a la superficie de las cuchillas
recubiertas justo antes de colocar las cuchillas en el horno de
curado. El aerosol antiadherencia fue Tiolon X-20,
suministrado por Tiodize Co., de Huntington Beach, CA, que es una
mezcla de partículas de politetrafluoroetileno dispersas en
disolvente orgánico y un propelente de aerosol. Las cuchillas fueron
después curadas como en el Ejemplo 6.
Las cuchillas del electrodo fueron tratadas con
chorro de arena con medios de óxido de aluminio hasta una rugosidad
superficial de 3,226 a 16,129 \mucm Ra (50-250
Ra), preferiblemente 12,903 \mucm Ra (200 Ra). A continuación, las
cuchillas fueron limpiadas ultrasónicamente en un baño de acetona
durante 15 minutos. Las cuchillas fueron después imprimadas con
NuSil SP-120 por inmersión de las cuchillas en el
agente imprimador y permitiéndolas secarse al aire en condiciones
ambientales durante 30 minutos. Las cuchillas fueron después
recubiertas por inmersión en una solución de adhesivo de silicona
MED-1511 de NuSil Technology, con un 30%-70% de
sólidos en tolueno. La solución de adhesivo de silicona fue
mantenida en nitrógeno para evitar el curado de la silicona debido
al contacto con humedad. Las cuchillas imprimadas fueron sumergidas
en la solución de silicona y se les permitió evaporar súbitamente el
disolvente y endurecer durante 1 hora. Las cuchillas fueron después
sumergidas una vez más y se les permitió reticularse completamente
por medio de un mecanismo de curado acetoxi durante 24 horas en
condiciones ambientales. El espesor del recubrimiento resultante en
el extremo del electrodo fue de 515,62 \mucm (8 milésimas de
pulgada).
Las cuchillas del electrodo, tratadas con chorro
de arena con medios de óxido de aluminio hasta una rugosidad
superficial de 3,226 a 16,129 \mucm Ra (50-250 Ra)
(preferiblemente 12,903 \mucm Ra (50-250 Ra),
fueron limpiadas ultrasónicamente en un baño de acetona durante 15
minutos. Las cuchillas fueron después imprimadas con un agente
imprimador de calidad médica de Applied Silicone para metales y
plásticos mediante inmersión de las cuchillas en el agente
imprimador y se les permitió secarse al aire en condiciones
ambientales durante 30 minutos. Las cuchillas fueron después
recubiertas por inmersión en una solución de dos partes de caucho de
silicona líquido (relación 1:1, durómetro 50) de Applied Silicone,
con 20%-50% de sólidos (preferiblemente 33%) en gasolina, y se les
permitió evaporarse súbitamente en un horno de convección de aire a
temperatura normal durante 10 minutos. Este procedimiento de
inmersión se repitió cuatro veces más para conseguir un espesor de
recubrimiento de 6,452 \mucm (10 milésimas de pulgada) en el
extremo del electrodo. El recubrimiento fue después curado durante 1
hora a 165ºC.
Electrodos recubiertos similares a los del
Ejemplo 7 fueron sometidos a tratamiento iónico con el fin de
favorecer la reticulación y reducir la pegajosidad de la superficie
de silicona. Las cuchillas fueron sometidas a plasma de argón
excitado por RF en vacío durante 7 minutos.
Electrodos recubiertos similares a los del
Ejemplo 7 fueron sometidos a tratamiento iónico con el fin de
favorecer la reticulación y reducir la pegajosidad de la superficie
de silicona. Las cuchillas fueron sometidas a dosis de 2,5; 5,0; y
10,0 megarads de radiación con haz de electrones.
Claims (3)
1. Un electrodo (10) para una herramienta
electroquirúrgica que se va a utilizar para cirugía en el tejido de
un paciente, comprendiendo el electrodo (10):
Un primer aspecto de la presente invención
proporciona un electrodo (10) para una herramienta electroquirúrgica
a utilizar en cirugía en los tejidos de un paciente, comprendiendo
el electrodo (10):
un eje metálico (13) alargado que tiene un
extremo próximo y un extremo distal, en el que el extremo próximo
tiene una forma para ser insertado en la herramienta
electroquirúrgica para conducir energía electroquirúrgica, en donde
el extremo distal tiene una forma a modo de cuchilla con al menos
dos superficies principales (14) que son generalmente paralelas y
están unidas por un borde (15);
una parte extrema de paciente en el extremo
distal para manipulación del tejido del paciente, en donde la parte
del extremo del paciente, antes de ser recubierta, tiene una
rugosidad superficial en el intervalo de aproximadamente 3,175 a
16,129 \mucm Ra (127 a
635 \mucm Ra);
635 \mucm Ra);
caracterizado porque el
electrodo comprende, además, un recubrimiento de base (12) de
material de imprimación en la parte del extremo del paciente,
comprendiendo el recubrimiento de base una resina de polisiloxano
con un catalizador reticulante;
y
y
un recubrimiento superior (11) sobre el
recubrimiento de base, comprendiendo el recubrimiento superior un
polímero basado en elastómero de silicona, en donde el recubrimiento
superior tiene un espesor no uniforme a lo largo de la parte extrema
del paciente, en donde las superficies principales (14) están
recubiertas para lograr un valor dieléctrico mayor que el borde de
unión.
2. El electrodo de la reivindicación 1, en el que
el espesor no uniforme proporciona menor conductividad eléctrica
sobre las superficies principales (14) que a lo largo del borde
(15).
3. El electrodo de la reivindicación 1, en el que
el mayor valor dieléctrico proviene de un recubrimiento superior
generalmente más grueso a través fundamentalmente de las superficies
principales (14).
