Die Erfindung betrifft eine kristalline Modifikation von Cefuroximaxetil (Polymorph), pharmazeutische Zusammensetzungen, welche diese Modifikation enthalten, und deren Verwendung.
Cefuroximaxetil (ATC-Code J01 DA45) ist ein Breitbandantibiotikum der Klasse der Cephalosporine, welches üblicherweise oral verabreicht wird. Eine Verabreichung von Cefuroximaxetil ist indiziert bei Infektionen der Atemwege, des Urogenitaltraktes und bei Infektionen im Hals-Nasen-Ohren-Bereich. Bei Infektionen der Weichteilgewebe und bei Infektionen der Knochen und Gelenke kann ebenfalls eine Anwendung erfolgen (vgl. LJ. Scott et al., Drugs 2001 , 61 , 1455-1500).
Cefuroximaxetil ist der 1 -Acetoxyethylester von Cefuroxim, d.h. von (6R,7R)-3-carba- moyloxymethyl-7-[(Z)-2-(fur-2-yl)-2-methoxyiminoacetamido]ceph-3-em-4-carbon- säure (vgl. US 3,974,153 und US 4,267,320; ATC-Code J01 DA06). Soweit für die pharmazeutische Anwendung zugelassen, handelt es sich bei Cefuroximaxetil üblicherweise um ein Gemisch zweier Diastereomere. Im Stand der Technik sind Verfahren zur Isolation der einzelnen Diastereomere beschrieben (vgl. DE-OS-27 06 413 und US 5,063,224).
Im Sinne der Beschreibung handelt es sich bei Cefuroximaxetil bevorzugt um das pharmazeutisch verwendbare Gemisch der Diastereomere, wie es beispielsweise in USP 23, Seiten 315/316 definiert ist. Dabei liegen die beiden Diastereomere in einem Verhältnis von etwa 1 :1 vor. Gemäß USP 23 wird eine Probe des Gemisches der Diastereomere chromatographiert. Dem A-Isomer entspricht dann ein Peak rA, dem B-Isomer ein Peak rB. Das Verhältnis der Diastereomere wird dann gemäß USP 23, Seite 315, rechte Spalte, bestimmt nach der Formel ΓA/^A+ΓB). Dieses Verhältnis muss zwischen 0,48 und 0,55 liegen.
Es sind verschiedene feste Modifikationen von Cefuroximaxetil im Stand der Technik bekannt.
Für die Herstellung oraler Darreichungsformen wird üblicherweise die amorphe Form von Cefuroximaxetil eingesetzt (vgl. DE-OS 34 27 828, EP-A 107 276 und US 4,562,181). Neben der amorphen Form von Cefuroximaxetil sind verschiedene kristalline Modifikationen von Cefuroximaxetil bekannt. So offenbart GB-A-15 71 683 eine Modifikation, welche im folgenden als "α-Modifikation" bezeichnet wird. Ein IR- Spektrum wird in GB-A-21 45409 offenbart. EP-A-757 991 offenbart eine Modifikation, welche im folgenden als "ß-Modifikation" bezeichnet wird. EP-A-937 727 offenbart eine Modifikation, welche im folgenden als "γ-Modifikation" bezeichnet wird. Ferner offenbart EP-A-937 727 ein Hemihydrat, welches etwa zu 95% aus dem R- Isomer besteht. DE 10 2005 019458 beschreibt eine Modifikation, welche im folgenden als "δ-Modifikation" bezeichnet wird.
Verschiedene Polymorphe desselben pharmazeutischen Wirkstoffs unterscheiden sich grundsätzlich in ihren Eigenschaften.
Einerseits können die speziellen physikalischen Eigenschaften einer bestimmten Modifikation bei ihrer Verarbeitung und Lagerung von Interesse sein, wie z.B. thermodynamische Stabilität, Kristallmorphologie [insbesondere Gestalt, Form, Partikelgröße, Farbe], Dichte, Schüttdichte, Härte, Deformierbarkeit, kalorimetrisches Verhalten [insbesondere Schmelzpunkt], Löslichkeitseigenschaften [insbesondere intrinsische Löslichkeitsgeschwindigkeit und Gleichgewichtslöslichkeit], Hygroskopie, relatives Feuchtigkeitsprofil, Klebrigkeit, etc.
Andererseits kann die kristalline Modifikation auch verbesserte chemische Eigenschaften aufweisen. Beispielsweise ist es bekannt, dass eine geringere Hygroskopie zu einer verbesserten chemischen Stabilität und längeren Haltbarkeit von chemischen Verbindungen führen kann.
Die bisher bekannten Modifikationen (Polymorphe) von Cefuroximaxetil haben gewisse Nachteile. So weisen sie mitunter keine ausreichende thermodynamische Stabilität auf. Die Stabilität der Modifikation ist jedoch ein sehr wesentlicher Aspekt beim Polymorphismus. Nur durch Verwendung der stabilsten Modifikation im Arzneimittel kann nämlich sichergestellt werden, dass während einer Lagerung in der
Formulierung keine polymorphe Umwandlung stattfindet. Dies ist insofern von besonderer Bedeutung, als dass sich ansonsten infolge einer Umwandlung von einer weniger stabilen Modifikation in eine stabilere Modifikation die Eigenschaften des Arzneimittels ändern können. Im Hinblick auf die pharmakologischen Eigenschaften einer Darreichungsform bedeutet dies, dass sich infolge der polymorphen Umwandlung beispielsweise die Löslichkeit des Wirkstoffs ändern kann, womit eine Änderung des Freisetzungsverhaltens und damit eine Änderung der Bioverfügbarkeit einhergeht. Letztlich resultiert daraus eine unzureichende Lagerstabilität der Darreichungsform.
Der Erfindung liegt die Aufgabe zugrunde, eine kristalline Modifikation von Cefur- oximaxetil bereitzustellen, welche Vorteile gegenüber den Modifikationen des Standes der Technik aufweist. Die kristalline Modifikation sollte auf einfache Art und Weise auch in größeren Mengen hergestellt werden können und sich insbesondere durch eine hohe thermodynamische Stabilität auszeichnen.
Es wurde überraschend gefunden, dass sich eine neue kristalline Modifikation von Cefuroximaxetil herstellen lässt, welche Vorteile gegenüber den bisher bekannten kristallinen Modifikationen von Cefuroximaxetil aufweist. Zur terminologischen Abgrenzung der neuen kristallinen Modifikation von Cefuroximaxetil von den bisher im Stand der Technik bekannten Modifikationen wird die neue kristalline Modifikation nachfolgend auch als "ε-Modifikation" bezeichnet.
Es wurde überraschend gefunden, dass sich die erfindungsgemäße ε-Modifikation von Cefuroximaxetil u.a. durch Präzipitation aus einer Lösung von amorphem Cefuroximaxetil in Aceton durch Zugabe von Wasser und anschließendes Rühren der Suspension herstellen lässt. Dazu kann eine filtrierte Lösung von amorphem Cefuroximaxetil in Aceton vorgelegt und die Präzipitation durch langsame Zugabe von Wasser als Fällungsmittel induziert werden. Die eingesetzte Menge an Wasser beträgt dabei vorzugsweise mindestens das Einfache, bevorzugter mindestens das Anderthalbfache und noch bevorzugter mindestens das Zweifache der eingesetzten Menge an Aceton. Anschließend sollte die Suspension für mehrere Stunden gerührt, der Niederschlag ggf. abfiltriert und danach ggf. im Vakuum getrocknet werden. Durch dieses Verfahren ist es ohne weiteres möglich, die erfindungsgemäße ε-
Modifikation von Cefuroximaxetil auch im Gramm- und Kilogramm-Maßstab in praktisch reiner Form herzustellen.
Figur 1 zeigt ein FT-Raman-Spektrum, Figur 2 ein Röntgenpulverdiffraktogramm der erfindungsgemäßen Modifikation von Cefuroximaxetil (ε-Modifikation).
Die erfindungsgemäße kristalline ε-Modifikation von Cefuroximaxetil ist lagerstabil, sowohl in physikalischer als auch in chemischer Hinsicht.
Mit Hilfe binärer Mischungen aus der ε-Modifikation mit der α-, ß- bzw. γ-Modifikation in Methanol und Aceton konnte gezeigt werden, dass die erfindungsgemäße ε- Modifikation unter diesen Bedingungen jeweils stabiler als die α-, ß- und die γ- Modifikation ist. Daher kann eine spontane Umwandlung der ε-Modifikation in diese weniger stabilen Modifikationen aus thermodynamischen Gründen ausgeschlossen werden.
Durch die Tatsache, dass für Cefuroximaxetil neben der amorphen Modifikation und der erfindungsgemäßen kristallinen ε-Modifikation bereits weitere kristalline Modifikationen bekannt sind, gewinnt dieser Aspekt zusätzlich an Bedeutung. Cefuroximaxetil zeichnet sich nämlich offenbar durch eine ausgeprägte Tendenz zum Polymorphismus aus, was nicht bei jedem Arzneistoff der Fall ist. Infolge der vergleichsweise großen Zahl metastabiler Modifikationen wird aber eine polymorphe Umwandlung während der Lagerung grundsätzlich wahrscheinlicher. In solchen Fällen besteht daher ein besonderer Bedarf daran, die nach Möglichkeit stabilste polymorphe Modifikation bereitzustellen.
Ein FT-Ramanspektrum der neuen kristallinen Modifikation von Cefuroximaxetil (ε-Modifikation) ist in Figur 1 dargestellt. Das FT-Raman-Spektrum der erfindungsgemäßen kristallinen Modifikation von Cefuroximaxetil umfasst Signale bei 1661 ± 3 (bevorzugt ± 2, bevorzugter ± 1 ) cm"1, 1644 ± 3 (bevorzugt ± 2, bevorzugter ± 1 ) cm"1 und 1597 ± 3 (bevorzugt ± 2, bevorzugter ± 1) cm"1. Bevorzugt umfasst das FT- Raman-Spektrum zusätzlich ein Signal bei 1404 ± 3 (bevorzugt ± 2, bevorzugter ± 1) cm"1 und/oder 1349 ± 3 (bevorzugt ± 2, bevorzugter ± 1) cm"1.

Ein Röntgendiffraktogramm der neuen kristallinen Modifikation von Cefuroximaxetil (ε-Modifikation) ist in Figur 2 dargestellt. Das Röntgenpulverdiffraktogramm der erfindungsgemäßen ε-Modifikation umfasst bevorzugt Reflexe bei 23,35 ± 0,20 (bevorzugt ± 0,15, bevorzugter ± 0,10) °2Θ und/oder 33,56 ± 0,20 (bevorzugt ± 0,15, bevorzugter ± 0,10) °2Θ. Bevorzugt umfasst das Röntgenpulverdiffraktogramm zusätzlich Reflexe bei 15,75 ± 0,20 (bevorzugt ± 0,15, bevorzugter ± 0,10) °2Θ, 18,76 ± 0,20 (bevorzugt ± 0,15, bevorzugter ± 0,10) °2Θ, 24,18 ± 0,20 (bevorzugt ± 0,15, bevorzugter ± 0,10) °2Θ, 24,92 ± 0,20 (bevorzugt ± 0,15, bevorzugter ± 0,10) °2Θ und/oder 26,18 ± 0,20 (bevorzugt ± 0,15, bevorzugter ± 0,10) °2Θ.
Ein weiterer Aspekt der Erfindung betrifft ein Verfahren zur Herstellung einer kristallinen Modifikation von Cefuroximaxetil umfassend die Schritte
a) Lösen von amorphem Cefuroximaxetil in einem ausreichenden Volumen an Aceton; b) Zugeben von Wasser zu der in Schritt a) erhaltenen Lösung in einer Menge, die ausreicht, um das Cefuroximaxetil zu präzipitieren; c) Rühren der erhaltenen Suspension; d) ggf. Abfiltrieren des erhaltenen Niederschlags; und e) ggf. Trocknen des erhaltenen Niederschlags.
Bevorzugt handelt es sich bei der kristallinen Modifikation von Cefuroximaxetil um die vorstehend beschriebene ε-Modifikation. Bei dem erfindungsgemäßen Verfahren wird die amorphe Form von Cefuroximaxetil als Edukt verwendet.
Bevorzugt wird die in Schritt a) erhaltene Lösung filtriert, bevor in Schritt b) Wasser als Fällungsmittel zugesetzt wird. Die in Schritt a) erhaltene Lösung weist vorzugsweise eine spezifische Konzentration der amorphen Form von Cefuroximaxetil in Aceton im Bereich von 5,0 bis 15 Gew.-%, bevorzugter 7,5 bis 12,5 Gew.-% und besonders bevorzugt 9,0 bis 11 Gew.-% auf.
In Schritt b) des erfindungsgemäßen Verfahrens wird der in Schritt a) erhaltenen Lösung Wasser in einer Menge zugesetzt, die ausreicht, um das Cefuroximaxetil zu präzipitieren. Die Zugabe des Wassers als Fällungsmittel erfolgt vorzugsweise
langsam, bevorzugt tropfenweise über einen längeren Zeitraum, um die Fällung nicht schlagartig, sondern behutsam zu induzieren. Bevorzugt erfolgt die Zugabe bei Raumtemperatur. In Abhängigkeit der eingesetzten Mengen ist dem Fachmann bekannt, über welchen Zeitraum die Zugabe des Wassers sinnvoll ist, insbesondere um auch Okklusionen und Inklusionen zu vermeiden.
Die zugesetzte Wassermenge reicht aus, um die Präzipitation des Cefuroximaxetils zu induzieren. Bevorzugt liegt das Verhältnis des Volumens des Wassers zum Volumen des Acetons im Bereich von 1 :1 bis 5:1 , bevorzugter 1 ,5:1 bis 4:1 , noch bevorzugter 1 ,75:1 bis 3:1.
In Schritt c) des erfindungsgemäßen Verfahrens wird die in Schritt b) erhaltene Suspension gerührt. Vorzugsweise erfolgt das Rühren für mindestens 6 h, bevorzugter mindestens 12 h, noch bevorzugter mindestens 18 h und insbesondere mindestens 24 h, vorzugsweise bei Raumtemperatur.
Im optionalen Schritt e) des erfindungsgemäßen Verfahrens erfolgt eine Trocknung, vorzugsweise im Vakuum und/oder bei erhöhter Temperatur, vorzugsweise im Bereich von 300C bis 500C, bevorzugter 35°C bis 45°C und insbesondere bei 40°C.
In einer alternativen Ausführungsform des erfindungsgemäßen Verfahrens wird nicht von der amorphen Form von Cef u roxi maxetil, sondern von der α-Modifikation von Cefuroximaxetil als Edukt ausgegangen. In diesem Fall wird jedoch keine Lösung in Aceton, sondern eine Lösung in THF hergestellt und durch Zugabe von Wasser als Fällungsmittel die Präzipitation eingeleitet. Die zugesetzte Wassermenge reicht aus, um die Präzipitation des Cefuroximaxetils zu induzieren. Bevorzugt liegt das Verhältnis des Volumens des Wassers zum Volumen des THF im Bereich von 0,5:1 bis 2:1 , bevorzugter 0,75:1 bis 1 ,5:1 , noch bevorzugter 0,8:1 bis 1 :1.
Unter diesen Bedingungen kann sich zunächst ein öliger Niederschlag bilden, welcher sich allerdings bei Lagerung für mehrere Tage, bevorzugt eine Woche, bei Raumtemperatur in die kristalline ε-Modifikation umwandelt. Diese kann dann abfiltriert und ggf. im Vakuum getrocknet werden.
Ein weiterer Aspekt der Erfindung betrifft die kristalline Modifikation von Cefuroxim- axetil, welche durch eines der vorstehend beschriebenen Verfahren erhältlich ist.
Ein weiterer Aspekt der Erfindung betrifft pharmazeutische Zusammensetzungen, welche die erfindungsgemäße Modifikation von Cefuroximaxetil (ε-Modifikation) und einen pharmazeutisch verträglichen Träger umfassen.
In einer bevorzugten Ausführungsform enthält die erfindungsgemäße Zusammensetzung zumindest ein Carrageenan, welches vorzugsweise ausgewählt ist aus der Gruppe bestehend aus κ-Carrageenaη, λ-Carrageenan und ι-Carrageenan, vorzugsweise κ-Carrageenan. Carrageenane sind sulfatisierte Polysaccharide, die als Strukturkomponenten in den Zellwänden bestimmter Rotalgen-Gattungen vorkommen. κ-Carrageenan kann beispielsweise aus Kappaphycus alverezii, Chondrus crispus und Sarcothalia crispata gewonnen werden.
κ-Carrageenaη, λ-Carrageenan und ι-Carrageenan sind Polysaccharide, die sich insbesondere in Anzahl und Position von Sulfatestergruppen unterscheiden. Wenn nicht anderweitig spezifiziert, bedeutet der Begriff "Carrageenan" im Sinne der Beschreibung vorzugsweise zumindest ein Carrageenan ausgewählt aus der Gruppe bestehend aus κ-Carrageenan, λ-Carrageenan und ι-Carrageenan. Im Stand der Technik sind noch weitere Carrageenane bekannt, beispielsweise α-Carrageenan, ß-Carrageenan, γ-Carrageenan und ε-Carrageenan.
Chemisch lässt sich eine Leitstruktur von *>Carrageenan bevorzugt auch bezeichnen mit "-(1 →3)-ß-D-galactopyranose-4-sulfat-(1 →4)-3,6-anhydro-α-D-galactopyranose- (1→3)-", die Leitstruktur von ι-Carrageenan mit "-(1→3)-ß-D-galactopyranose-4- sulfat-(1→4)-3,6-anhydro-α-D-galactopyranose-2-sulfat-(1 →3)-" und die Leitstruktur von λ-Carrageenan mit "-(1— ►3)-ß-D-galactopyranose-2-sulfat-(1→4)-α-D- galactopyranose-2,6-disulfat-(1→3)-".
Im Sinne der Beschreibung umfasst κ-Carrageenan bevorzugt sulfatisierte Polygalak- tosid-Substrukturen, welche sich von D-Galaktose und 3,6-anhydro-D-Galaktose ableiten.
Beispiele solcher Substrukturen sind:
Bevorzugt enthält die erfindungsgemäße Zusammensetzung ein Carrageenan, das durchschnittlich pro Pyranoseeinheit weniger als 1 ,5 Sulfatäquivalente, bevorzugter weniger als 1 ,0 Sulfatäquivalente und insbesondere 0,6 ± 0,1 Sulfatäquivalente aufweist.
Vorzugsweise enthält die erfindungsgemäße Zusammensetzung κ-Carrageenan, bevorzugt als Natrium-, Kalium oder Calciumsalz. κ-Carrageenan bzw. seine Salze sind kommerziell erhältlich, beispielsweise unter der Bezeichnung Gelcarin®.
Hinsichtlich weiterer Einzelheiten zu κ-Carrageenan und zur Spezifikaton der Carrageenane kann beispielsweise verwiesen werden auf T.H.M. In einer bevorzugten Ausführungsform der erfindungsgemäßen Zusammensetzung enthält sie wenigstens 5,5 Gew.-%, bevorzugter wenigstens 6,0 Gew.-%, noch bevorzugter wenigstens 6,5 Gew.-%, am bevorzugtesten wenigstens 7,0 Gew.-% und insbesondere wenigstens 7,5 Gew.-% Carrageenan, vorzugsweise κ-Carragee- nan. Besonders bevorzugt enthält die erfindungsgemäße Zusammensetzung 7,5 bis 50 Gew.-%, bevorzugter 10 bis 40 Gew.-% und insbesondere 20 bis 30 Gew.-%
Carrageenan, vorzugsweise κ-Carrageenan, bezogen auf das Gesamtgewicht der Zusammensetzung.
Bevorzugt liegt das Gewichtsverhältnis der ε-Modifikation von Cefuroximaxetil zu Carrageenan im Bereich von 0,5:1 bis 5,0:1 , bevorzugter 1 ,0:1 bis 4,0:1 , noch bevorzugter 1 ,5:1 bis 3,5:1 , am bevorzugtesten 2,0:1 bis 3,0:1 und insbesondere 2,2:1 bis 2,6:1.
In einer bevorzugten Ausführungsform enthält die erfindungsgemäße Zusammensetzung Tricalciumphosphat und/oder wenigstens einen Sucroseester. In einer besonders bevorzugten Ausführungsform enthält die erfindungsgemäße Zusammensetzung sowohl Tricalciumphosphat als auch zumindest einen Sucroseester.
Bevorzugte Sucroseester weisen einen Hydrophilie-Lipohilie-Balance (HLB) Wert von 10 bis 17, bevorzugter 11 bis 16, insbesondere 13 bis 15 auf.
Bevorzugt enthält die erfindungsgemäße Zusammensetzung den Sucroseester in einer Menge von 1 ,0 bis 10 Gew.-%, bevorzugter 2,0 bis 8,0 Gew.-% und insbesondere 4,0 bis 6,0 Gew.-%, bezogen auf das Gesamtgewicht der Zusammensetzung.
Erfindungsgemäß bevorzugt handelt es sich bei dem Sucroseester um Sucrose Stearat, Sucrose Palmitat, Sucrose Laurat, Sucrose Behenat, Sucrose Oleat, Sucrose Erucat oder deren Mischungen. Besonders bevorzugt handelt es sich um Sucrose Palmitat, welches einen HLB von ca. 15 aufweist und beispielsweise unter der Typenbezeichnung "S-1570" bei der Firma Mitsubishi kommerziell erhältlich ist.
Enthält die erfindungsgemäße Zusammensetzung Tricalciumphosphat und Carrageenan, so beträgt das Gewichtsverhältnis von Tricalciumphosphat zu Carrageenan bevorzugt 1 :1 bis 1 :10, insbesondere 1 :2 bis 1 :6.
Als Tricalciumphosphat wird bevorzugt ein fein verteiltes Pulver mit einer mittleren Teilchengröße < 50 μm eingesetzt.
Weiterhin kann die erfindungsgemäße Zusammensetzung übliche Hilfsstoffe enthalten, wie z.B. Füllmittel, Bindemittel, Gleitmittel, Antioxidationsmittel, Farbstoffe oder Konservierungsstoffe. Derartige Hilfsstoffe sind dem Fachmann bekannt. In diesem Zusammenhang kann beispielsweise vollumfänglich verwiesen werden auf HP. Fiedler, Lexikon der Hilfsstoffe für Pharmazie, Kosmetik und angrenzende Gebiete, 2 Bde., Editio Cantor, Aulendorff, 2002; D.E. Bugay, Pharmaceutical Excipients (Drugs and the Pharmaceutical Sciences, V. 94) (Hardcover), CRC, 1998.
In einer bevorzugten Ausführungsform enthält die erfindungsgemäße Zusammensetzung keine mikrokristalline Cellulose oder andere Sphäronisierungshilfsstoffe, wie z.B. niedrig substituierte Hydroxypropylcellulose, Hydroxypropylmethylcellulose, Hydroxymethylcellulose, Pulvercellulose, Natrium Carboxymethylcellulose oder Polyvinylpyrrolidon, sofern sie noch nicht als Darreichungsform mit einem Überzug versehen wurde.
Ein weiterer Aspekt der Erfindung betrifft Pellets, vorzugsweise Extrusionspellets, welche die erfindungsgemäße Zusammensetzung enthalten.
Ein weiterer Aspekt der Erfindung betrifft eine feste pharmazeutische Darreichungsform enthaltend die erfindungsgemäße ε-Modifikation von Cefuroximaxetil, wobei das Cefuroximaxetil unter physiologischen Bedingungen bei einem pH-Wert von 6 bis 7 innerhalb von 30 Minuten in einer Menge von wenigstens 75%, vorzugsweise wenigstens 85%, freigesetzt wird, nachdem sich zunächst ein gegebenenfalls vorhandener Überzug aufgelöst hat. Vorzugsweise ist die Darreichungsform multipartikulär.
Ferner betrifft ein weiterer Aspekt der Erfindung eine vorzugsweise multipartikuläre, pharmazeutische Darreichungsform umfassend eine vorstehend beschriebene pharmazeutische Zusammensetzung, vorzugsweise vorstehend beschriebene Pellets.
Bevorzugte Ausführungsformen der erfindungsgemäßen Darreichungsform, wie z.B. bevorzugte Mengenverhältnisse der eingesetzten Komponenten, sind vorstehend im
Zusammenhang mit der erfindungsgemäßen pharmazeutischen Zusammensetzung beschrieben.
Die erfindungsgemäße Darreichungsform ist bevorzugt zur oralen Verabreichung konfektioniert. Sie ist bevorzugt multipartikulär. Bevorzugt umfasst sie Granulate, Pellets, vorzugsweise Extrusionspellets, Mikrogranulate oder Mikropellets. Besonders bevorzugt liegt die erfindungsgemäße Darreichungsform in Form von Extrusionspellets, vorzugsweise in sphärischer Form, vor. Diese können ggf. in Gelatinekapseln gefüllt werden.
Die Partikel der erfindungsgemäßen multipartikulären Darreichungsform weisen bevorzugt einen mittleren Durchmesser (Größe) < 1000 μm, bevorzugter < 900 μm, noch bevorzugter < 800 μm, am bevorzugtesten < 700 μm und insbesondere zwischen 250 und 700 μm auf.
In einer bevorzugten Ausführungsform liegt die erfindungsgemäße Darreichungsform als Tablette, insbesondere als Filmtablette vor. Dabei sind die Tabletten besonders bevorzugt durch Tablettierung der erfindungsgemäßen Pellets, vorzugsweise Extrusionspellets, unter Zusatz der üblichen Tablettierungshilfsstoffe erhältlich.
In einer bevorzugten Ausführungsform der erfindungsgemäßen Darreichungsform weist sie einen Magensaft resistenten und/oder Speichel resistenten Überzug auf. Vorzugsweise wird vor dem Aufbringen dieses Überzugs zusätzlich ein Schutzüberzug aufgebracht, welcher den Kern isoliert.
Diese Überzüge werden je nach Art und Funktion des Überzuges bevorzugt in einer Menge von 1 ,0 bis 50 Gew.-%, bevorzugter 2,0 bis 25 Gew.-%, bezogen auf das Gesamtgewicht der Darreichungsformen, aufgetragen.
Als Materialien für einen Magensaft resistenten Überzug eignen sich vorzugsweise Methacrylsäure/Alkylmethacrylat-Copolymere, vorzugsweise Copolymerisate von Methacrylsäure/Methylmethacrylat mit einem molaren Verhältnis der Monomeren von 1 :1 bis 1 :2, wie Eudragit L®, Eudragit S® oder Eudragit L30D-55®, oder Ethylmeth- acrylat, die sich bei einem pH-Wert von > pH 6 schnell auflösen. Weiterhin können
als Magensaft resistente Überzüge solche auf Basis von Cellulosen, die dem Fachmann bekannt sind, aufgetragen werden. Die Auftragung der Überzüge kann mit entsprechenden Lösungen oder Dispersionen in organischem oder wässrigem Medium erfolgen, wobei ein wässriges Medium bevorzugt ist. Als Speichel resistente Überzüge eignen sich Überzüge auf Basis von Eudragit E, Eudragit EPO.
Dem Fachmann ist es bekannt, dass den bekannten Überzugsmaterialien übliche Weichmacher, Farbstoffe, Gleitmittel, wie Talkum und/oder Magnesiumstearat, zugesetzt werden sollen bzw. können.
Erfindungsgemäß wird unter „Magensaft" sowohl die natürliche Komposition des Magensafts als auch die dem Fachmann bekannten künstlichen Magensaft ähnlichen Zubereitungen (pH 1 ,2 bis 2,0) verstanden. Ebenso werden unter „Freisetzung in Dünndarm" sowohl die Freisetzung im natürlichen Dünndarmsaft als auch die Freisetzung in Dünndarmsaft ähnlichen Zubereitungen bei pH-Werten von 6 bis 7, vorzugsweise pH 6,4 bis 6,8 verstanden.
Die erfindungsgemäßen Darreichungsformen zeichnen sich bevorzugt dadurch aus, dass sie eine hohe Auflösungsgeschwindigkeit aufweisen und das Cefuroximaxetil bevorzugt innerhalb von 30 Minuten zu mindestens 85% freigesetzt wird, nachdem sich zunächst ein gegebenenfalls vorhandener Überzug aufgelöst hat. Die pH- abhängige Auflösungsdauer eines solchen Überzugs kann durch einfache Vorversuche in entsprechenden Standardpufferlösungen bestimmt werden.
Die Freisetzungsrate von Cefuroximaxetil wird bevorzugt bestimmt gemäß der Methode beschrieben im „Guidance for lndustry, Waiverof ln Vivo Bioavailability and Bioequivalence Studies for Immediate-Release Solid Oral Dosage Forms based on a Biopharmaceutics Classification System., Seiten 1-3 / 7, Herausgeber U.S. Department of Health and Human Services, Food and Drug Administration, Center for Drug Evaluation and Research (CDER)1 August 2000, BP". Dazu wird eine Freisetzungsapparatur mit Blattrührer gemäß U.S. Pharmacopeia eingesetzt und die Freisetzung bei einer Temperatur von 37°C und einer Umdrehungsgeschwindigkeit von 150 min'1 in 1000 ml künstlichem Darmsaft (Natriumphosphatpuffer, pH 6,8) und 0,3% SDS bzw. in 1000 ml künstlichem Magensaft (pH 1 ,2) und 0,5% Tween 80 als
Freisetzungsmedium gemessen. Die jeweils zu einem Zeitpunkt freigesetzte Menge des Wirkstoffes kann durch HPLC bestimmt werden. Die Detektion kann mit Hilfe eines UV-Detektors bei 282 nm erfolgen.
Der Wirkstoffgehalt der erfindungsgemäßen Darreichungsform ist bevorzugt an eine einmal oder zweimal tägliche Verabreichung an einen Erwachsenen oder einen pädiatrischen Patienten angepasst. Bei Erwachsenen liegt die tägliche Dosis üblicherweise im Bereich von 250 bis 1000 mg pro Tag, bei Kindern abhängig von Alter und Gewicht, üblicherweise im Bereich von 80 bis 500 mg pro Tag. Bevorzugt umfasst die erfindungsgemäße Darreichungsform pro Dosiseinheit eine Äquivalentmenge bezogen auf 125, 250 oder 500 mg Cefuroxim.
In einer bevorzugten Ausführungsform setzt die erfindungsgemäße Darreichungsform Cefuroximaxetil, nachdem sich zunächst ein gegebenenfalls vorhandener Überzug aufgelöst hat, bei einem pH-Wert von 6 bis 7 innerhalb von 30 Minuten in einer Menge von wenigstens 75%, bevorzugt wenigstens 80%, bevorzugter wenigstens 85%, am bevorzugtesten wenigstens 90% und insbesondere wenigstens 95% frei, bestimmt gemäß der Methode beschrieben im „Guidance for lndustry, Waiverof ln Vivo Bioavailability and Bioequivalence Studies for Immediate-Release Solid Oral Dosage Forms based on a Biopharmaceutics Classification System., Seiten 1-3 / 7, Herausgeber U.S. Department of Health and Human Services, Food and Drug Administration, Center for Drug Evaluation and Research (CDER), August 2000, BP".
In einer besonders bevorzugten, erfindungsgemäßen Darreichungsform liegt die multipartikuläre Darreichungsform in Form von sphärischen Extrusionspellets vor. Diese können vorzugsweise als Einmaldosis gemäß einem Darreichungssystem umfassend einen Trinkhalm mit vorzugsweise beweglicher Sperrvorrichtung, wie sie in WO 03/079957, WO 2004/000202, WO2004/000264 beschrieben ist, vorliegen. Die Sperrvorrichtung ist vorzugsweise ein Pfropfen, dessen Querschnitt vorzugsweise dem Querschnitt des Trinkhalms angepasst ist. Der Pfropfen kann zwischen zwei Anschlägen bewegbar sein. Durch die Anschläge wird der Trinkhalm verengt, wobei die Verengungen so ausgelegt sind, dass der Pfropfen, nicht jedoch
die multipartikuläre Darreichungsform dadurch zurückgehalten werden. Bevorzugt erfolgt eine Verabreichung an den Patienten mit einer Transportflüssigkeit. Die Beschreibungen der Darreichungssysteme in den genannten Veröffentlichungen werden hiermit als Referenz eingeführt und gelten als Teil der vorliegenden Offenbarung.
Ein weiterer Gegenstand der vorliegenden Erfindung ist ein Darreichungssystem umfassend eine erfindungsgemäße Darreichungsform, vorzugsweise als Einmaldosis, angeordnet in einem Trinkhalm mit wenigstens einer vorzugsweise beweglichen Sperrvorrichtung zur Verabreichung mit Hilfe einer Transportflüssigkeit an einen menschlichen Patienten.
Als Transportflüssigkeiten eignen sich partikelfreie Getränke, vorzugsweise wässrige Flüssigkeiten, wie z. B. Wasser, Tee, Fruchtsäfte, Limonaden, wobei Transportflüssigkeiten mit einem sauren pH-Wert bei Einsatz von Magensaft resistent ausgerüsteten, d.h. überzogenen, multipartikulären Darreichungsformen, wie z. B. Extrusions- pellets, bevorzugt sind.

Die erfindungsgemäße kristalline Modifikation von Cefuroximaxetil eignet sich als Arzneimittel, bevorzugt zur Herstellung eines Medikamentes zur Vorbeugung und/oder Behandlung von Infektion, d.h. Infektionen, die durch Cefuroxim-empfind- liche Erreger verursacht sind, insbesondere Infektionen der oberen Atemwege einschließlich des Hals- und Ohrenbereichs (z.B. Otitis media, Sinusitis, Pharyngitis, Tonsillitis), Infektionen der unteren Atemwege (z.B. akute Exazerbation der chronischen Bronchitis, Pneumonie), Infektionen des HNO-Bereichs, der Knochen, der Gelenke und bei Sepsis, Infektionen der Haut und des Weichteilgewebes, Infektionen der Nieren und/oder der ableitenden Harnwege, der Geschlechtsorgane, akute unkomplizierte Gonorrhoe, oder zur perioperativen Prophylaxe bei erhöhter Gefährdung der Patienten durch Infektionen.

Die nachfolgenden Beispiele dienen zur näheren Erläuterung der Erfindung, sind jedoch nicht einschränkend hinsichtlich ihres Umfangs auszulegen:
Beispiel 1 :
Herstellung der erfindungsgemäßen, kristallinen Modifikation von Cefuroximaxetil:
a) 505,1 mg amorphes Cefuroximaxetil wurden in 6 ml Aceton gelöst. Stufenweise wurden 9 ml Wasser zugegeben. Die erhaltene Suspension wurde über Nacht gerührt. Der Niederschlag wurde abfiltriert und im Vakuum bei 400C getrocknet. Durch FT-Raman-Spektroskopie konnte nachgewiesen werden, dass es sich um die erfindungsgemäße ε-Modifikation handelte.
b) 501 ,2 mg amorphes Cefuroximaxetil wurden in 6 ml Aceton gelöst. Die Lösung wurde filtriert und es wurden stufenweise 9 ml Wasser zugegeben. Die erhaltene Suspension wurde über Nacht gerührt. Der Niederschlag wurde abfiltriert und im Vakuum bei 4O0C getrocknet. Durch FT-Raman-Spektroskopie konnte nachgewiesen werden, dass es sich um die erfindungsgemäße ε-Modifikation handelte.
c) 3 g amorphes Cefuroximaxetil wurden in 25 ml Aceton gelöst. Die Lösung wurde filtriert und es wurden stufenweise 30 ml Wasser zugegeben. Die erhaltene Suspension wurde über Nacht gerührt. Der Niederschlag wurde abfiltriert und im Vakuum bei 400C getrocknet. Durch FT-Raman-Spektroskopie konnte nachgewiesen werden, dass es sich um die erfindungsgemäße ε-Modifikation handelte.
d) 515,9 mg Cefuroximaxetil der α-Modifikation wurden in 3 ml THF gelöst. Stufenweise wurden 2,5 ml Wasser zugegeben. Man erhielt einen öligen Niederschlag, welcher sich nach Lagerung für eine Woche bei Raumtemperatur in einen Feststoff umwandelte. Der Feststoff wurde abfiltriert und im Vakuum getrocknet. Durch FT-Raman-Spektroskopie konnte nachgewiesen werden, dass es sich um die erfindungsgemäße ε-Modifikation handelte.
Beispiel 2:
Die in Beispiel 1 a) erhaltene, erfindungsgemäße kristalline Modifikation von Cefuroximaxetil (ε-Modifikation) wurde durch FT-Raman-Spektroskopie charakterisiert. Dazu wurde auf einem FT-Raman-Spektrometer der Fa. Bruker, Typ
RFS100 (Nd.YAG 1064 nm Anregung, 100 mW Laser, Ge-Detektor, 64 Scans, 25- 3500 cm'1, Auflösung 2 cm'1) ein FT-Raman-Spektrum aufgenommen, welches in Figur 1 abgebildet ist.
Die gemessenen Ramansignale (λ [cm"1]) der erfindungsgemäßen ε-Modifikation sind in nachfolgender Tabelle zusammengestellt und den entsprechenden Signalen der α-, ß- und γ-Modifikation gegenübergestellt:
Beispiel 3:
Die erfindungsgemäße kristalline Modifikation von Cefuroximaxetil (ε-Modifikation) wurde röntgendiffraktometrisch charakterisiert. Dazu wurde ein Röntgen-Pulver- diffraktogramm (XRPD) mit einem Diffraktometer der Fa. Philips, Typ X'Pert PW 3040 (oder Philips PW 1710) (Cu Kα-Strahlung, Umgebungstemperatur und Umgebungsfeuchtigkeit, Schrittweite 0,02 °2Θ, 2 oder 2,4 s Schritt"1, 2-50 °2Θ) aufgenommen, welches in Figur 2 abgebildet ist.
Die gemessenen Reflexe (°2θ Werte) der erfindungsgemäßen ε-Modifikation sind in folgender Tabelle zusammengefasst:
Claims
1. Kristalline Modifikation von Cefuroximaxetil, deren FT-Raman-Spektrum Signale bei 1661 ± 3 cm*1, 1644 ± 3 cm"1 und 1597 ± 3 cm"1 umfasst.
2. Modifikation nach Anspruch 1 , dadurch gekennzeichnet, dass ihr FT-Raman- Spektrum zusätzlich ein Signal bei 1404 ± 3 cm"1 und/oder 1349 ± 3 cm'1 umfasst.
3. Modifikation nach Anspruch 1 oder 2, dadurch gekennzeichnet, dass ihr Röntgen- pulverdiffraktogramm Reflexe bei 23,35 ± 0,20°2Θ und/oder 33,56 ± 0,20°2Θ umfasst.
4. Modifikation nach Anspruch 3, dadurch gekennzeichnet, dass ihr Röntgenpul verdiffraktog ramm einen oder mehrere Reflexe umfasst ausgewählt aus der Gruppe bestehend aus 15,75 ± 0,20°2Θ, 18,76 ± 0,20°2Θ, 24,18 ± 0,20°2Θ, 24,92 ± 0,20°2Θ und/oder 26,18 ± 0,20°2Θ.
5. Verfahren zur Herstellung einer kristallinen Modifikation von Cefuroximaxetil umfassend die Schritte
a) Lösen von amorphem Cefuroximaxetil in einem ausreichenden Volumen an Aceton; b) Zugeben von Wasser zu der in Schritt a) erhaltenen Lösung in einer Menge, die ausreicht, um das Cefuroximaxetil zu präzipitieren; c) Rühren der erhaltenen Suspension; d) ggf. Abfiltrieren des erhaltenen Niederschlags; und e) ggf. Trocknen des erhaltenen Niederschlags.
6. Verfahren nach Anspruch 5, dadurch gekennzeichnet, dass die kristalline Modifikation von Cefuroximaxetil eine Modifikation nach einem der Ansprüche 1 bis 5 ist.
7. Verfahren nach Anspruch 5 oder 6, dadurch gekennzeichnet, dass in Schritt b) das Verhältnis des Volumens des Wassers zum Volumen des Acetons im Bereich von 1 :1 bis 5:1 liegt.
8. Verfahren nach einem der Ansprüche 5 bis 7, dadurch gekennzeichnet, dass Schritt c) für mindestens 6 h durchgeführt wird.
9. Verfahren nach einem der Ansprüche 5 bis 8, dadurch gekennzeichnet, dass Schritt e) im Vakuum und/oder bei erhöhter Temperatur erfolgt.
10. Verfahren nach einem der Ansprüche 5 bis 9, dadurch gekennzeichnet, dass Schritt e) im Bereich von 35°C bis 45°C erfolgt.
11. Kristalline Modifikation von Cefuroximaxetil erhältlich durch ein Verfahren nach einem der Ansprüche 5 bis 10.
12. Pharmazeutische Zusammensetzung umfassend eine kristalline Modifikation von Cefuroximaxetil nach einem der Ansprüche 1 bis 4 oder 11 und einen pharmazeutisch verträglichen Träger.
13. Pharmazeutische Darreichungsform umfassend eine pharmazeutische Zusammensetzung nach Anspruch 12.
14. Darreichungsform nach Anspruch 13, dadurch gekennzeichnet, dass die zur oralen Verabreichung konfektioniert ist.
15. Kristalline Modifikation von Cefuroximaxetil nach einem der Ansprüche 1 bis 4 oder 11 als Arzneimittel.
16. Verwendung einer kristallinen Modifikation von Cefuroximaxetil nach einem der Ansprüche 1 bis 4 oder 11 zur Herstellung eines Medikamentes zur Vorbeugung und/oder Behandlung einer bakteriellen Infektion.
