Abstract
Die Erfindung betrifft eine lyophilisierte pharmazeutische Zubereitung mit einem Immuncytokin. Die Zubereitung weist eine erhöhte Lagerstabilität auch bei erhöhten Temperaturen auf und kann nach Rekonstitution parenteral als Arzneimittel appliziert werden.
Description
Lyophilisierte Zubereitung enthaltend Immuncytokine
Die vorliegende Erfindung betrifft eine stabile lyophilisierte pharmazeutische Zubereitung enthaltend Immuncytokine sowie die Herstellung der lyophilisierten pharmazeutischen Zubereitung.
Immuncytokine sind Konjugate bestehend aus Antikörpern und Cytokinen, wobei die carboxyterminalen Enden der beiden schweren Immunglobulinketten der Antikörper jeweils mit den N-terminalen Enden eines Cytokins verknüpft sind.
Antikörper sind bestimmte Glykoproteine mit Schutzwirkung, die in Blut, Lymphe und Körpersekreten als Folge einer Immunisierung durch Antigene auftreten und mit diesen eine Antigen-Antikörper-Reaktion eingehen. Antikörper gehören zu den Immunglobulinen (Ig) und können in 5 Klassen unterteilt werden: IgA, IgD, IgE, IgG, and IgM, die teilweise wiederum in weitere Unterklassen (Isotypen) unterteilt werden können, beispielsweise in lgG1 , lgG2, lgG3, lgG4, IgA, and lgA2. In den Immuncytokinen enthalten sind alle IgG-Antikörper. Sie umfassen monoklonale Antiköper, polyklonale Antikörper und multispezifische Antikörper, wie beispielsweise bispezifische
Antikörper.
Cytokine sind Polypeptide, die von Zellen endokrin oder parakrin, d.h. in das Blut bzw. das umgebende Gewebe, ausgeschieden werden und nach Bindung an spezifischen Rezeptoren die Funktionen (meist Teilung und
Wachstum, z.B. aber auch Fortbewegung ) anderer Zellen beeinflussen. In manchen Fällen sind die Cytokine produzierenden Zellen selbst Ziel dieser (dann autokrin genannten) Regulation. Cytokine regulieren unter anderem das komplizierte Wechselspiel der Zellen des Immunsystems. Beispiele von Cytokinen sind Lymphokine, Monokine und konventionelle
Polypeptidhormone. Cytokine umfassen Wachstumshormone wie humanes
Wachstumshormon, humanes N-methionyl-Wachstumshormon und bovines Wachstumshormon, Parathormon, Thyroxin, Insulin, Proinsulin, Relaxin, Prorelaxin, Glycoproteinhormone wie follikelstimulierendes Hormon (FSH), Thyrotropin (TSH) und Lutropin (LH), hepatischer Wachstumsfaktor, Fibroblasten-Wachstumsfaktor, Prolactin, Placenta-
Lactogen, Maus-gonadotropin-assoziiertes Peptid, Inhibin, Activin, vasculärer endothelialer Wachstumsfaktor (VEGF), Integrin, Thrombopoietin (TPO), Nerven-Wachstumsfaktoren wie NGFß, Plättchen- Wachstumsfaktor, transformierende Wachstumsfaktoren (TGFs) wie TGFα und TGFß, Erythropoietin (EPO), Interferone wie IFNα, IFNß und IFNγ, hämatopoetische Wachstumsfaktoren wie M-CSF, GM-CSF and G- CSF, Interleukine (IL) wie IL-1 , IL-1a, IL-2, IL-3, IL-4, IL-5, IL-6, IL-7, IL-8, IL-9, IL-10, IL-11 , IL-12 sowie Tumornekrosefaktoren (TNFs) wie TNFα oder TNFß.
Immuncytokine sind wie die vorgenannten Antikörper und Cytokine Peptidwirkstoffe und können daher nicht enteral resorbiert werden. Zur therapeutischen Anwendung müssen sie daher in der Regel parenteral als Lösung appliziert werden.
Ein Problem bei der Formulierung von Lösungen mit Peptidwirkstoffen ist deren Neigung zur Aggregation und zur Bildung von Proteinmultimeren. Dieses Problem ist aber in Abhängigkeit von den physikochemischen Eigenschaften der jeweils betroffenen Wirkstoffe unterschiedlich ausgeprägt. Während Proteine mit hydrophilem Charakter in wässriger
Lösung weniger zur Bildung von Aggregaten neigen ergibt sich für Proteine mit hydrophobem Charakter verstärkte Neigung zur Aggregation.
Antikörper bestehen aus 2 anti-parallelen Faltblättern, die sandwichartig zueinander angeordnet sind (konservierte Domänen). In den Faltblättern wechseln sich hydrophobe und hydrophile Aminosäuren ab, wobei die
hydrophoben Seitenketten beider Faltblätter jeweils zueinander und somit in das Innere der Sandwichstruktur und die hydrophilen Aminosäuren jeweils nach außen gerichtet sind (J. Klein, Immunologie, Verlag Chemie, Weinheim, 1991). Die nach außen gerichteten hydrophilen Aminosäuren führen in wässriger Lösung zur Solubilisierung der Antikörper und verhindern damit die Interaktion zwischen verschiedenen Antikörpern. Antikörper weisen somit nur eine geringe Oberflächenhydrophobizität und Aggregationsneigung auf.
Aufgrund der vorgenannten Eigenschaften sind Lösungen von Antikörpern vergleichsweise einfach stabil zu formulieren. Ein Beispiel für ein kommerziell erhältliches Produkt ist Rituxan®, eine wässrige Formulierung enthaltend den monoklonalen Antikörper Rituximab, einen anorganischen Puffer sowie Polysorbat. Durch Entzug des Wassers mittels Gefriertrocknung kann die Stabilität der an sich schon relativ stabilen wässrigen Lösungen von Antikörpern noch weiter erhöht werden. Vor Applikation werden die erhaltenen Lyophilisate dann durch Zugabe von Wasser zu der wässrigen Lösung rekonstituiert. Ein Beispiel für ein derartiges Produkt ist Remicade®, das neben dem monoklonalen Antikörper Infliximab, einem anorganischen Puffer und einem Polysorbat noch zusätzlich einen Zucker als Gefrierschutzmittel bzw. Gerüstbildner enthält.
WO 98/22136 A2 offenbart eine lyophilisierte Zubereitung enthaltend einen Antikörper, einen Zucker oder Aminozucker, eine Aminosäure und ein
Tensid. Zwar wird die Zubereitung für Antikörper im allgemeinen beansprucht, als Ausführungsbeispiel offenbart sind jedoch nur Zubereitungen mit monoklonalen Antikörpern, die gegen den Hepatitis B- Virus (AK HBV) gerichtet sind, sowie jeweils eine Zubereitung mit einem Antikörper gegen L-Selektin (Anti-L-Selektin) und einem Antikörper gegen den Anti-L-Nerve-Growth-Factor-Rezeptor (Anti-L-NGFR).
Cytokine enthalten keine konservierten Domänen, die wie bei Antikörpern eine gute Wasserlöslichkeit bedingen könnten. Sie neigen daher in wässriger Lösung verstärkt zur Aggregation. Dies gilt besonders für Cytokine die als gemeinsames Strukturmotiv ein Bündel aus vier α-Helices enthalten (so genannte 4 α-helix bündle cytokines) und aufgrund dieses
Strukturmotivs eine ausgeprägte Hydrophobizität aufweisen. Mit Hydrophobizität einhergehende hydrophobe Wechselwirkungen wiederum sind oft die Ursache / der Mechanismus für Aggregation (Hora-MS and Chen-B (1999) Biopharm., Ind. Perspect., 217-248). Cytokine, die als gemeinsames Strukturmotiv ein Bündel aus vier α-Helices enthalten, sind viele Interleukine, insbesondere IL-2, IL-3, IL-4, IL-5, IL-6, IL-7, IL-10, IL-11 und IL-12, Interferone, insbesondere IFNß und IFNγ, hämatopoetischen Wachstumsfaktoren wie M-CSF, GM-CSF, G-CSF, Erythropoietin (EPO) und Stammzellen-Faktor (SCF). In der Literatur explizit als hydrophob beschrieben sind beispielsweise IL-2 (Robb-RJ et al. (1983) PNAS
80:5990-4; US 5,580,856), IL-4 (Sharma-S et al. (1987) 235:1489-92), IL-5 (Takatsu-K et al. (1985) Jl 134:382-9), G-CSF (US 5,104,65).
Die starke Neigung von Cytokinen, insbesondere der 4 α-helix bündle cytokines erfordert besondere Maßnahmen zu deren Stabilisierung.
Beispielsweise enthält das Produkt Proleukin®, ein Lyophilisat mit dem Wirkstoff lnterleukin-2, als Hilfsstoffe einen Zucker, einen anorganischen Puffer sowie ein anionisches Detergenz (Natriumlaurylsulfat). Anionische Detergenzien, insbesondere wenn das Arzneimittel zur parenteralen Verabreichung vorgesehen ist, sind aus toxikologischer Sicht aber äußerst bedenklich.
Als weiteres Beispiel, das die besonderen Schwierigkeiten bei der Stabilisierung von Cytokinen enthaltenden Formulierungen aufzeigt, sind die auf dem Markt befindlichen Produkte mit Interferon ß (Avonex®, Betaferon®, Rebif®): alle diese Produkte enthalten zur Stabilisierung
Albumin, das aus toxikologischer Sicht, insbesondere im Hinblick auf
unerwünschte Immunreaktionen, ebenfalls als äußerst kritisch zu bewerten ist.
Auf Grund der vorgenannten Unterschiede zwischen Cytokinen und Antikörpern unterscheiden sich auch die aus jeweils einem Antikörper und zwei Cytokinen zusammengesetzten Immuncytokine in ihren physikochemischen Eigenschaften deutlich von denen der Antikörper. Insbesondere Immuncytokine, die ein Cytokin mit vier α-Helixbündeln enthalten, neigen aufgrund der damit einhergehenden ausgeprägten Hydrophobizität in wässriger stark zur Bildung von Agglomeraten und sind schwierig zu stabilisieren.
Es war Aufgabe der vorliegenden Erfindung eine stabilisierte Zubereitung für Immuncytokine zur Verfügung zu stellen. Die Zubereitung sollte keine toxikologisch bedenklichen Hilfsstoffe enthalten, unter erhöhten
Stressbedingungen, wie erhöhter Temperatur und Luftfeuchtigkeit, über längere Zeit stabil sein und mit einem wässrigen Lösungsmittel zu einer applikationsfertigen Lösung mit hohem Wirkstoffanteil rekonstituierbar sein.
Überraschenderweise konnte eine diesen Anforderungen entsprechende
Zubereitung zur Verfügung gestellt werden, indem eine wässrige gepufferte Lösung, die neben einem Immuncytokin einen Zucker oder einen Aminozucker, eine Aminosäure und ein Tensid enthält, gefriergetrocknet wird. Gegenstand der vorliegenden Erfindung ist daher eine stabile lyophilisierte Zubereitung, enthaltend ein Immuncytokin, einen Zucker oder einen Aminozucker, eine Aminosäure und ein Tensid.
Vorzugsweise enthält die Zubereitung ein Immuncytokin das als Cytokinbestandteil Cytokine, die ausgewählt sind aus der Gruppe von Cytokinen, die als gemeinsames Strukturmerkmal ein Bündel aus vier α-
Helices aufweisen, insbesondere ein Interleukin, vorzugsweise IL-2, IL-3,
IL-4, IL-5, IL-6, IL-7, IL-10, IL-11 und/oder IL-12, ein Interferon, vorzugsweise IFNß und/oder IFNγ, und/oder einen hämatopoetischen Wachstumsfaktor, vorzugsweise M-CSF, GM-CSF, G-CSF, EPO oder SCF. Besonders bevorzugt enthält die Zusammensetzung ein Immuncytokin mit lnterleukin-2 (IL-2).
Die erfindungsgemäße Zubereitung ist physiologisch gut verträglich, leicht herstellbar, exakt dosierbar und über die Dauer der Lagerung und auch nach mehrfachen Einfrier- und Auftauvorgängen hinsichtlich Gehalt, Zersetzungsprodukten und Aggregaten stabil. Sie kann bei
Kühlschranktemperatur (2-8°C) und bei Raumtemperatur (23-27°C, 60 % relative Luftfeuchtigkeit (r.F.) über einen Zeitraum von mindestens drei Monaten bis zu einem Zeitraum von zwei Jahren stabil gelagert werden. Überraschenderweise ist die erfindungsgemäße Zubereitung auch bei höheren Temperaturen und Luftfeuchtigkeiten, beispielsweise bei einer
Temperatur von 40°C und 75% r.F. über den genannten Zeitraum stabil lagerbar.
Die lyophilisierte Zubereitung kann in einfacher Weise durch Zugabe eines wässrigen Lösungsmittels, beispielsweise Wasser für Injektionszwecke oder einer isotonischen wässrigen Lösung, zu einer applikationsfertigen partikelfreien Lösung rekonstituiert werden. Die rekonstituierte Lösung ist über einen Zeitraum von etwa 5 Tagen stabil, wird aber besonders bevorzugt innerhalb von 24 Stunden appliziert.
Vorteilhaft können aus der erfindungsgemäßen Zubereitung durch Rekonstitution mit wässrigen Lösungsmitteln Immuncytokin-haltige Lösungen mit einem pH-Wert von 5 bis 8, bevorzugt mit einem pH-Wert von 5,6 bis 7,4, besonders bevorzugt mit einem pH-Wert von 6-7 und einer Osmolalität von 250 bis 350 mOsmol/kg hergestellt werden. Die rekonstituierte Zubereitung kann somit weitgehend schmerzfrei intravenös,
intraarteriell und auch subkutan direkt verabreicht werden. Daneben kann die Zubereitung auch Infusionslösungen, wie z.B. Glukoselösung, isotonische Kochsalzlösung oder Ringerlösung, die auch weitere Wirkstoffe enthalten können, zugesetzt werden, so dass auch größere Wirkstoffmengen appliziert werden können.
Gemäß einer bevorzugten Ausführungsform der Erfindung besteht die lyophilisierte pharmazeutische Zubereitung im wesentlichen aus einem Immuncytokin, einem Zucker oder Aminozucker, einer Aminosäure, einem Puffer und einem Tensid.
Die erfindungsgemäße Zubereitung ermöglicht die Herstellung von Immuncytokinlösungen, die in ihrer Konzentration den klinischen Erfordernissen angepasst sind. Bevorzugt sind Immuncytokinlösungen mit einer Immuncytokinkonzentration von ca. 0,1 bis 25 mg/ml, besonders bevorzugt sind 1 bis 10 mg/ml, ganz besonders bevorzugt sind 1 bis 5 mg/ml.
In der erfindungsgemäßen Zubereitung können als Zucker Mono-, Di- oder Trisaccharide eingesetzt werden. Diese Zucker können sowohl alleine oder in Mischungen mit Zuckeralkoholen (z.B. Mannitol) eingesetzt werden. Beispielhaft genannt seien als Monosaccharide Glucose, Mannose, Galactose, Fructose und Sorbose, als Disaccharide Saccharose, Lactose, Maltose oder Trehalose und als Trisaccharid Raffinose. Bevorzugt sind Saccharose, Lactose, Maltose oder Trehalose enthalten, besonders bevorzugt sind Saccharose und Maltose.
Ebenso können Aminozucker enthalten sein, d.h. Monosaccharide, die anstelle einer Hydroxygruppe eine primäre, sekundäre oder tertiäre Amino- oder eine acylierte Aminogruppe (-NH-CO-R) besitzen. Erfindungsgemäß besonders bevorzugt sind hierbei Glucosamin, N-Methylglucosamin,
Galactosamin und Neuraminsäure.
Der enthaltene Zucker/Aminozucker liegt in der erfindungsgemäßen Zubereitung in einer Menge vor, dass dieser nach Rekonstitution mit der vorgesehenen Volumen an Lösungsmittel in der erhaltenen Lösung in einer Konzentration von cirka 1 bis 200 mg/ml vorliegt. Bevorzugt liegt der Zucker in der rekonstituierten Lösung in einer Konzentration von 15 bis 30 mg/ml vor.
Als erfindungsgemäß verwendete Aminosäuren kommen basische, saure oder neutrale Aminosäuren in Frage, beispielsweise Arginin, Histidin, Ornithin, Lysin, Glycin u.a.. Vorzugsweise werden die Aminosäuren in Form ihrer anorganischen Salze (vorteilhaft in Form der Salzsäuresalze, d.h. als Aminosäurehydrochloride) eingesetzt. Für den Fall, dass die freien Aminosäuren eingesetzt werden, erfolgt die Einstellung des gewünschten pH-Wertes durch Zugabe einer geeigneten physiologisch verträglichen Puffersubstanz, wie z.B. einer organischen oder anorganischen Säure wie
Citronensäure und Phosphorsäure, Schwefelsäure, Essigsäure, Ameisensäure oder deren Salze. Bevorzugt sind Citrate und Phosphate, mit denen besonders stabile Lyophilisate erhalten werden.
Als Aminosäuren bevorzugt sind Arginin, Lysin oder Ornithin. Daneben können auch saure Aminosäuren, wie z.B. Glutaminsäure und Asparaginsäure, oder neutrale Aminosäuren, wie z.B. Isoleucin, Leucin und Alanin, bzw. aromatische Aminosäuren, wie z.B. Phenylalanin, Tyrosin oder Tryptophan, Verwendung finden. Der Aminosäuregehalt in der erfindungsgemäßen Zubereitung beträgt 1 bis 200 mMol/l, bevorzugt 40 bis
100 mMol/l, besonders bevorzugt sind 40-80 mMol/l (jeweils bezogen auf die rekonstituierte Lösung).
Als Tenside einsetzbar sind alle in pharmazeutischen Zubereitungen üblicherweise verwendeten Tenside, vorzugsweise nichtionische Tenside, insbesondere Polysorbate und Polyoxyethylen-Polyoxypropylen-Polymere. Besonders bevorzugt sind Polyoxyethylensorbitanfettsäureester,
insbesondere Polyoxyethylen(20)-sorbitanmonolaurat und Polyoxyethylen(20)-sorbitanmonooleat. Erfindungsgemäß ist in der Zubereitung 0,001 bis 1 Gew.-%, bevorzugt 0,005 bis 0,5 Gew.-% und besonders bevorzugt 0,01 bis 0,15 Gew.-% enthalten (jeweils bezogen auf die rekonstituierte Lösung).
Soweit die erfindungsgemäße Zubereitung Puffer enthalten sind, können hierfür grundsätzlich alle physiologisch verträglichen Substanzen eingesetzt werden, die zur Einstellung des gewünschten pH-Wertes geeignet sind. Die Menge an Puffersubstanz wird dabei so gewählt, dass nach Rekonstitution der lyophilisierten Zubereitung, beispielweise mit Wasser für Injektionszwecke, die erhaltene wässrige Lösung eine Pufferkonzentration von 5 mMol/l bis 50 mMol/l, vorzugsweise 10 bis 20 mMol/l aufweist. Als Puffer bevorzugt sind Citratpuffer oder Phosphatpuffer. Geeignete Phosphatpuffer sind Lösungen der Mono- und/oder Di-Natrium- und Kaliumsalze der Phosphorsäure, wie Dinatriumhydrogenphosphat oder Kaliumdihydrogenphosphat, sowie Mischungen der Natrium- und Kaliumsalze, wie beispielsweise Mischungen aus Dinatriumhydrogenphosphat und Kaliumdihydrogenphosphat.
Soweit durch die osmotischen Eigenschaften des Immuncytokins und durch die zur Stabilisierung eingesetzten Hilfsstoffe die rekonstituierte Lösung nicht bereits isotonisch ist, kann weiterhin ein Isotonisierungsmittel, bevorzugt ein physiologisch verträgliches Salz, wie beispielsweise Natrium- oder Kaliumchlorid, oder ein physiologisch verträgliches Polyol oder ein
Zucker, wie beispielsweise Glucose oder Glycerin oder Mannitol, in einer zur Isotonisierung erforderlichen Menge enthalten sein. Darüber hinaus können die erfindungsgemäßen Lyophilisate weitere physiologisch verträgliche Hilfsstoffe, wie z.B. Antioxidantien wie Ascorbinsäure oder Glutathion, Konservierungsmittel wie Phenol, m-
Cresol, Methyl- oder Propylparaben, Chlorbutanol, Thiomersal oder Benzalkoniumchlorid, oder weitere Stabilisatoren, Gerüstbildner und
Lösungsvermittler wie Polyethylenglykole (PEG), z.B. PEG 3000, 3350, 4000 oder 6000 oder Cyclodextrine, z.B. Hydroxypropyl-ß-cyclodextrin, Sulfobutylethyl-ß-cyclodextrin oder γ-Cyclodextrin oder Dextrane enthalten.
Die erfindungsgemäße Zubereitung kann hergestellt werden, indem man eine wässrige Zubereitung, enthaltend ein Immuncytokin als Wirkstoff und einen Zucker oder Aminozucker, eine Aminosäure und ein Tensid sowie gegebenenfalls weitere pharmazeutische Hilfsstoffe, herstellt und die Lösung anschließend lyophilisiert.
Die wässrige Zubereitung kann hergestellt werden, indem man einer ein Immuncytokin enthaltenden Lösung die genannten Hilfsstoffe zugefügt werden. Zweckmäßigerweise wird hierzu einer Lösung mit einer definierten Konzentration an Immuncytokin, wie sie bei dessen Herstellung gewonnen wird, mit definierten Volumina an Stammlösungen, die die genannten weiteren Hilfsstoffe in definierter Konzentration enthalten, versetzt und gegebenenfalls mit Wasser auf die vorberechnete Konzentration verdünnt. Alternativ können die Hilfsstoffe der das Immuncytokin enthaltenden Ausgangslösung auch als Feststoffe zugesetzt werden. Liegt das Immuncytokin als Feststoff, beispielsweise als Lyophilisat, vor, kann die erfindungsgemäße Zubereitung hergestellt werden, indem jeweilige Immuncytokin zunächst in Wasser oder einer einen oder mehrere der weiteren Hilfsstoffe enthaltenden wässrigen Lösung gelöst und anschließend mit den jeweils erforderlichen Mengen an die weiteren Hilfsstoffe enthaltenden Stammlösungen, mit den weiteren Hilfsstoffen in fester Form und/oder Wasser versetzt werden. Zweckmäßigerweise kann das Immuncytokin auch direkt in einer alle weiteren Hilfsstoffe enthaltenden Lösung gelöst werden. Vorteilhaft kann einer oder mehrere der in der erfindungsgemäßen Zubereitung enthaltenen Hilfsstoffe bereits während oder zum Schluss des
Herstellungsverfahrens des jeweiligen Immuncytokins zugegeben werden.
Bevorzugt kann dies dadurch erfolgen, indem das Immuncytokin im letzten Schritt der nach seiner Herstellung erfolgenden Aufreinigung direkt in einer einen, mehrere oder alle weiteren Hilfsstoffe enthaltenden wässrigen Lösung gelöst bzw. mittels geeigneter Verfahren wie das einer Tangentialflussfiltration umgepuffert wird. Dann müssen zur Herstellung der
Zubereitung die jeweiligen weiteren Inhaltsstoff/e nur noch in jeweils geringerer Menge und/oder gar nicht zugesetzt werden. Besonders bevorzugt ist, wenn der jeweilige Inhaltsstoff im letzten Schritt der nach seiner Herstellung erfolgenden Aufreinigung direkt in einer alle weiteren Hilfsstoffe enthaltenden wässrigen Lösung gelöst wird, so dass direkt die zu lyophilisierende Lösung erhalten wird.
Die den jeweiligen Immuncytokin sowie die Hilfsstoffe enthaltende Lösung wird auf einen pH-Wert von 5 bis 8 eingestellt, sterilfiltriert und gefriergetrocknet.
Die erhaltene lyophilisierte Zubereitung kann durch Zugabe eines wässrigen Lösungsmittels zu einer wässrigen Zubereitung rekonstituiert werden, die direkt, insbesondere parenteral, applizierbar ist. Gegenstand der vorliegenden Erfindung ist daher auch eine wässrige pharmazeutische Zubereitung von Immuncytokinen erhältlich durch Rekonstitution des erfindungsgemäßen Lyophilisats mit einem wässrigen Lösungsmittel.
Bevorzugt weist die rekonstituierte wässrige pharmazeutische Zubereitung einen pH-Wert von 5 - 8, vorzugsweise einen pH-Wert von 5,6 - 7,4 und besonders bevorzugt einen pH-Wert von 6,0-7,0 aufweist
Die Beispiele, ohne darauf beschränkt zu sein, erläutern die Erfindung.

Untersuchungen zur Stabilität der Zubereitungen
Die Stabilität der erfindungsgemäßen Zubereitungen wurde in Haltbarkeitsstudien überprüft. Hierzu wurden die hergestellten Lyophilisate bei verschiedenen Temperaturen eingelagert, zu bestimmten Zeiten ausgelagert und mit geeigneten analytischen Methoden untersucht. Die
Klimabedingung 40°C mit einer relativen Luftfeuchtigkeit (r.F.) von 75% wurde als Stressbedingung ausgewählt, um bei den verschiedenen
Formulierungen schnell zu Unterschieden hinsichtlich der Stabilität zu gelangen. Mögliche Instabilitäten äußern sich bei Immuncytokinen vornehmlich in der Bildung von Aggregaten als auch in der Bildung von Abbauprodukten. Abbauprodukte und lösliche Aggregate werden bevorzugt durch Größenausschlusschromatographie (HPLC-SEC) erfasst während visuelle Inspektion und Trübungsmessungen zum Nachweis von sichtbaren Aggregaten dienen. Der ebenfalls zur Evaluierung der Zubereitungen herangezogene ELISA-Test dient zur Überprüfung der Integrität und der Bindungsfähigkeit am Rezeptor. Zusammen mit der UV Photometrie bei einer Wellenlänge von 280 nm dient er zusätzlich der Bestimmung des
Gehaltes.
Analytische Testmethoden:
Aussehen
Die hergestellten Formulierungen wurden visuell mit Hilfe einer Kaltlichtquelle auf Partikel und auf das auftreten einer möglichen Trübung untersucht.
Proteinkonzentration: A2βoι nm
Die Absorption der hergestellten Proteinlösungen bei einer Wellenlänge von 280 nm wurde zur Konzentrationsbestimmung der hergestellten Zubereitungen herangezogen. Der Extinktionskoeffizient von 1 ,41 für den Wirkstoff huKS-IL2 (EMD 273066) wurde durch quantitative Aminosäureanalyse bestimmt. Für die eigentliche Messung wurden die
Proteinlösungen in Triplikaten soweit herunterverdünnt, dass die Absorption der Testlösungen zwischen 0,1 und 1 ,0 (entsprechend einer Proteinkonzentration von 0,5 mg/ml) Absorptionseinheiten lag. Die Absorption der wirkstoffhaltigen Testlösungen wurde gegen eine entsprechende wirkstofffreie Referenzlösung gemessen.
Reinheit: Größenausschlusschromatographie, SEC-HPLC
Die Größenausschlusschromatographie (size exclusion chromatography, SEC) ist eine Analysemethode, mit der die Reinheit und der Monomeren- /Aggregatanteil der hergestellten Zubereitungen bestimmt werden kann.
Die Komponenten der Testlösungen werden aufgrund ihrer Molekülgröße über eine spezielle poröse HPLC-Säule aufgetrennt. Größere Moleküle werden zusammen mit dem Ausschlussvolumen eluiert, während kleinere Moleküle zu unterschiedlichem Ausmaß in die Poren der Stationären Phase eindringen und durch diese mehr oder weniger stark zurückgehalten werden. Kleinere Moleküle wie Abbauprodukte von huKS-IL2 erscheinen deshalb bei späteren Retentionszeiten als huKS-IL2-Monomere und besonders als huKS-IL2-Aggregate, die als erstes von der Säule eluiert werden.
Konzentration und Integrität des Wirkstoffmoleküls: KSA-ELISA
Bei dieser Anaysemethode (ELISA, enzyme linked immunosorbent assay) werden Mikrotiterplatten mit dem für huKS-IL2 spezifischen Antigen (EPCAM- oder KSA-Antigen) beschichtet. huKS-IL2-Moleküle der zu bestimmenden Testlösung binden über ihren Antikörperanteil an das
Antigen und werden so an die Mikrotiterplatte gebunden. Nach Zugabe von biotinyliertem anti-IL2-Antiserum reagieren die anti-IL2-Antikörper mit den IL2-Molekülteilen der gebundenen huKS-IL2-Moleküle. Überschüssige anti- IL2-Moleküle werden durch Waschen der Mikrotiterplatte entfernt. Zugegebenes Streptavidin-Peroxidase-Konjugat wird über das Biotin gebunden und oxidiert die in einem weiteren Schritt zugegebene Leukoform des Farbstoffes Tetramethylbenzidin (TMB) zum blauen Farbstoff. Die Oxidationsreaktion wird durch Zugabe von Phosphorsäure nach einer definierten Zeit gestoppt. Hierbei kommt es zu einer Gelbfärbung der Lösung, die bei einer Wellenlänge von 450 nm
quantifiziert werden kann. Die Konzentration der Proteintestlösungen ist hierbei proportional zur ermittelten Absorption bei dieser Wellenlänge.
Ergebnisse:
Die Ergebnisse der Tabellen 1 und 2 (Chargen 8020 und 8021) belegen klar die Qualität und die Stabilität der hergestellten Zubereitungen. Die Klimabedingung 40°C mit einer relativen Luftfeuchtigkeit (r.F.) von 75% wurde als Stressbedingung ausgewählt, um bei den verschiedenen
Formulierungen schnell zu Unterschieden hinsichtlich der Stabilität zu gelangen. Überraschenderweise sind die o.g. Zubereitungen selbst bei erhöhten Lagerungstemperaturen und erhöhter relativer Luftfeuchtigkeit (40°C / 75% r.F.) über einen Zeitraum von >6 Monaten stabil. Eine entsprechende Vergleichsformulierung 1 (Tabelle 3, Charge 8008, Beispiel
5), bei der statt der Disaccharide (Saccharose bzw. Maltose) der Zuckeralkohol Mannitoi als Gerüstbildner eingesetzt wurde, war bei der entsprechenden Streßbedingung bereits nach 4 Wochen aggregiert. Nach 26 Wochen Lagerung bei Raumtemperatur (25°C, 60% r.h.) war das entsprechende Klimamuster ebenfalls von sichtbaren Aggregaten durchsetzt. Die Vergleichsformulierung 1 kann somit im Gegensatz zur erfindungsgemäßen Zubereitung nur gekühlt gelagert werden.
Beispielformulierung 4 (Charge 8591) ist ein weiteres Beispiel für die herausragende Stabilität der erfindungsgemäßen Zubereitung und zeigt zudem, dass sich die Formulierung auch auf erhöhte Proteinkonzentrationen übertragen lässt (siehe Tabelle 4). Diese Formulierung war über eine Dauer von 14 Wochen bei einer Temperatur von 40°C (75% r.H.) eingelagert
In einer weiteren Versuchsserie wurde überprüft, ob alle Hilfsstoffe in der erfindungsgemäßen Zubereitungen tatsächlich zur Stabilisierung benötigt
werden. Hierbei enthielt die Versuchscharge 8431 alle Bestandteile der erfindungsgemäßen Formulierung, während bei den Vergleichsformulierungen einzelne Komponenten fehlten:
- Formulierung nach Beispiel 3 (Charge 8431): Alle Komponenten sind enthalten
- Vergleichsformulierung 2 (Beispiel 6, Charge 8434): Ohne Argininzusatz
- Vergleichsformulierung 3 (Beispiel 7, Charge 8430): Ohne Tween 80- Zusatz - Vergleichsformulierung 4 (Beispiel 8, Charge 8429): Ohne Zusatz von
Tween 80 und Arginin
Das in Grafik 1 dargestellte Ergebnis belegt deutlich, dass der Zusatz der Aminosäure für die Stabilität der erfindungsgemäßen Zubereitung zwingend erforderlich ist. Nach dieser Grafik scheint der Zusatz von Tween
80 zwar nicht erforderlich zu sein, was allerdings nicht ganz den Tatsachen entspricht. Tween 80 wird bereits während der Proteinaufreinigung während der Herstellung der Wirkstofflösungen zugesetzt, um die Bildung vor allem sichtbarer Aggregate zu verhindern. Die eingesetzte Tween 80- Konzentration liegt hierbei oberhalb der kritischen
Mizellbildungskonzentration (die CMC für Tween 80 liegt bei 0.001%). Im Falle der Versuchsserie wurde versucht, sowohl Arginin als auch Tween durch das sogenannte Tangentialflußfiltrationsverfahren zu entfernen. Hierbei wurde ein Pufferaustausch mittels Diafiltration über eine 50 kD- Membran vorgenommen. Tween 80-Mizellen liegen allerdings zum Teil oberhalb der Membranausschlussgrenze und werden deshalb nicht restlos entfernt.
Der Zusatz von Tween 80 in der erfindungsgemäßen Zubereitung ist für die Wiederauflösung der Lyophilisate und für die Stabilität der nach
Rekonstitution erhaltenen Lösung zwingend erforderlich, wie eine weitere Studie belegt. Ausgehend von einer Tween 80 enthaltenden EMD 273066
(huKS-IL2)-Wirkstofflösung wurde hierbei der Hilfsstoff Tween 80 durch Affinitätschromatografie über eine Protein-A-Säule abgetrennt. Die erhaltene Tween 80 freie Lösung wurde mit steigenden Mengen Tween 80 versetzt. Die erhaltenen Zubereitungen wurden in 2 ml Gläsern bei einer Temperatur von 25°C mit Hilfe eines Laborschüttlers über eine Zeitraum von 21 Tagen gestresst. Die gestressten Formulierungen wurden täglich visuell kontrolliert und zu bestimmten Zeitpunkten hinsichtlich ihres Proteingehaltes photometrisch analysiert. Das Ergebnis dieser Studie ist in Grafik 2 dargestellt. Im Falle der Zubereitungen ohne den Zusatz von Tween 80 war bereits nach einem Tag eine leichte Trübung festzustellen, die sich im Laufe der Zeit sichtbar verstärkte. Nach einem Zeitraum von 21 Tagen wurde zudem, wie in der Grafik dargestellt, eine starke Gehaltsabnahme verzeichnet.

Claims
1. Lyophilisierte pharmazeutische Zubereitung von Immuncytokinen, enthaltend ein Immuncytokin, einen Zucker oder einen Aminozucker, eine Aminosäure und ein Tensid
2. Lyophilisierte pharmazeutische Zubereitung gemäß Anspruch 1 , dadurch gekennzeichnet, dass als Immuncytokin ein Immuncytokin enthalten ist, das als Cytokinbestandteil Cytokine enthält, die ausgewählt sind aus der Gruppe von Cytokinen, die als gemeinsames Strukturmerkmal ein Bündel aus vier α-Helices aufweisen
3. Lyophilisierte pharmazeutische Zubereitung gemäß Anspruch 2, dadurch gekennzeichnet, dass das Immunocytokin als Cytokinbestandteil ein Interleukin, ein Interferon und/oder einen hämatopoetischen
Wachstumsfaktor enthält
4. Lyophilisierte pharmazeutische Zubereitung gemäß Anspruch 3, dadurch gekennzeichnet, dass das Immunocytokin als Cytokinbestandteil lnterleukin-2 (IL-2) enthält
5. Lyophilisierte pharmazeutische Zubereitung gemäß einem oder mehreren der Ansprüche 1 bis 4, dadurch gekennzeichnet, dass diese im wesentlichen aus einem Immuncytokin, einem Zucker oder Aminozucker, einer Aminosäure, einem Puffer und einem Tensid besteht
6. Lyophilisierte pharmazeutische Zubereitung gemäß einem oder mehreren der Ansprüche 1 bis 5, dadurch gekennzeichnet, dass der Zucker ein Mono-, Di- oder Trisaccharid, vorzugsweise Saccharose, Lactose, Maltose oder Trehalose ist
7. Lyophilisierte pharmazeutische Zubereitung gemäß einem oder mehreren der Ansprüche 1 bis 5, dadurch gekennzeichnet, dass der Aminozucker Glucosamin, N-Methylglucosamin, Galactosamin oder Neuraminsäure ist
8. Lyophilisierte pharmazeutische Zubereitung gemäß einem oder mehreren der Ansprüche 1 bis 7, dadurch gekennzeichnet, dass die Aminosäure eine basische, saure oder neutrale Aminosäure, vorzugsweise Arginin, Lysin, oder Ornithin, ist
9. Lyophilisierte pharmazeutische Zubereitung gemäß einem oder mehreren der Ansprüche 1 bis 8, dadurch gekennzeichnet, dass das Tensid ein nichtionisches Tensid ist
10. Lyophilisierte pharmazeutische Zubereitung gemäß Anspruch 9, dadurch gekennzeichnet, dass das Tensid ein Polysorbat oder ein Polyoxyethylen-
Polyoxypropylen-Polymer ist
11. Lyophilisierte pharmazeutische Zubereitung gemäß Anspruch 10, dadurch gekennzeichnet, dadurch gekennzeichnet, dass das Tensid Polyoxyethylensorbitan-Fettsäureester Polyoxyethylen(20)- sorbitanmonooleat oder Polyoxyethylen(20)-sorbitanmonolaurat ist
12. Lyophilisierte pharmazeutische Zubereitung gemäß einem oder mehreren der Ansprüche 1 bis 11 , dadurch gekennzeichnet, dass weiterhin ein Isotonisierungsmittel in einer zur Isotonisierung erforderlichen Menge enthalten ist
13. Wässrige pharmazeutische Zubereitung von Immuncytokinen erhältlich durch Rekonstitution des Lyophilisats nach einem oder mehreren der Ansprüche 1 bis 12 mit einem wässrigen Lösungsmittel
14. Wässrige pharmazeutische Zubereitung nach Anspruch 13, dadurch gekennzeichnet, dass die Lösung einen pH-Wert von 5 - 8, vorzugsweise von 5,6 - 7,4 aufweist
15. Wässrige pharmazeutische Zubereitung nach Anspruch 14, dadurch gekennzeichnet, dass die Lösung einen pH-Wert von 6-7 aufweist
16. Verfahren zur Herstellung einer lyophilisierten pharmazeutischen
Zubereitung gemäß einem oder mehreren der Ansprüche 1 bis 12, dadurch gekennzeichnet, dass man eine wässrige Zubereitung, enthaltend ein
Immuncytokin, einen Zucker oder Aminozucker, eine Aminosäure, ein Tensid sowie gegebenenfalls weitere pharmazeutische Hilfsstoffe, herstellt und die Lösung anschließend lyophilisiert
