Description
- Die vorliegende Erfindung betrifft eine neue therapeutische Verwendung von L-Carnitin oder Acyl-L-carnitin-Derivaten und deren pharmakologisch verträglichen Salzen für die Prophylaxe von Krankheiten oder die therapeutische Behandlung von zellulären Störungen, die von hohen Ceramidspiegeln begleitet werden.
- Ceramid ist das Basismolekül der Struktur von Sphingolipiden und deren Metabolismus. Alle Sphingolipide enthalten Ceramid als hydrophile Hauptkomponente und stammen von Ceramid über Biosynthesewege ab, welche hauptsächlich dessen 1-Hydroxyposition modifizieren. Sphingolipide wiederum spielen eine wichtige Rolle bei der Transduktion des Signals über die zelluläre Membran.
- Ceramid spielt eine wichtige Rolle bei der Transduktion des Signals über die zelluläre Membran. Moleküle, die imstande sind, auf die intrazellulären Rezeptoren (d. h. Calcitriol) oder die Transmembranrezeptoren [d. h. Gamma-Interferon (IFN-γ), Interleukin-1 (IL-1) und den Nervenwachstumsfaktor (NGF)] einzuwirken, hydrolysieren Sphingomyelin zu Ceramid. Ceramid aktiviert Phosphatasen und Proteinkinasen und, von einem biologischen Standpunkt aus, induziert zelluläre Apoptosis, Wachstum und Zelldifferenzierung, moduliert die Expression von Cyclooxygenasen und Phospholipasen und die Aktivierung von Nuklear-Faktoren kB (NFkB) [K. Kuno et al., J. Leukoc. Biol., 56(5): 542-7; M. G. Cifone et al., J. Exp. Med., 180(4): 1547-52; R. Kolesnick, Mol. Chem. Neuropathol., 21(2-3): 287-97; W. D. Jarvis et al., Proc. Natl. Acad. Sci. U.S.A, 91(1): 73-7; L. M. Obeid et al., Science, 259(5102): 1769-71].
- Es ist nun gefunden worden, dass Variationen der Konzentration oder im Metabolismus von Ceramid zur Pathogenese zahlreicher Krankheiten beitragen oder dazu beitragen, dass metabolische zelluläre Störungen induziert werden. Leider existieren bis heute keine Verfahren, um Ceramidspiegel in vivo zu reduzieren.
- Dementsprechend ist es ein Gegenstand dieser Erfindung eine neue Verwendung von L-Carnitin und Acyl-L-carnitinen und ihren pharmakologisch verträglichen Salzen bereitzustellen, um Ceramidspiegel in vivo zu reduzieren.
- Ein zweiter Gegenstand der vorliegenden Erfindung ist es, eine neue Verwendung von L-Carnitin und Acyl-L-carnitinen und ihren pharmakologisch verträglichen Salzen bereitzustellen, für die Prophylaxe von Krankheiten oder die therapeutische Behandlung von zellulären Störungen, die von hohen Ceramidspiegeln begleitet werden.
- Es ist tatsächlich gefunden worden, dass die Verabreichung hoher Dosen von L-Carnitin und Acyl-L-carnitinen und ihren pharmakologisch verträglichen Salzen Ceramidspiegel reduziert, und solche Verbindungen können somit verwendet werden zur Behandlung von Krankheiten, die durch hohe Ceramidspiegel charakterisiert sind.
- Insbesondere ist gefunden worden, dass L-Carnitin und Acyl-L-carnitine und ihren pharmakologisch verträglichen Salze verwendet werden können für die Behandlung von Krankheiten, die durch hohe Ceramidspiegel charakterisiert sind.
- In Übereinstimmung mit der Erfindung wird eine neue Verwendung von L-Carnitin und Acyl-L-carnitinen und ihren pharmakologisch verträglichen Salzen bereitgestellt, um die Ceramidspiegel in vivo abzusenken.
- Diese Verbindungen sind kommerziell erhältlich. Diese Verbindungen können als inneres Salz oder als pharmazeutisch verträgliche Salze verwendet werden.
- Acylderivate von L-Carnitin können auch in der vorliegenden Erfindung verwendet werden. C2-6-Acyl-L-carnitine, welche linear oder verzweigt sind, können verwendet werden. Diese Säuren sind den Pharmakologen und Fachleuten auf dem Gebiet der pharmazeutischen Technik wohlbekannt. Insbesondere bevorzugte Acylgruppen sind Acetyl, Propionyl, Butyryl, Valeryl und Isovaleryl.
- Geeignete pharmazeutische Salze können gebildet werden zwischen den obigen basischen Aminosäuren und jedem konventionellen Anion wie Chlorid, Bromid, Iodid oder einem sauren Aspartat wie Aspartat, einem sauren Citrat wie Citrat, einem sauren Tartrat wie Tartrat, einem sauren Phosphat wie Phosphat, einem sauren Fumarat, einem Glycophosphat wie Glucophosphat, saurem Lactat, saurem Maleat, Orotat; saurem Oxalat, insbesondere Oxalsäure; einem Sulfat, insbesondere bevorzugt Sulfat, Trichloracetat, Trifluoracetat und Methansulfonat.
- Beispiele von Krankheiten oder Störungen, die durch erhöhte Ceramidspiegel charakterisiert sind, schließen entzündliche Darmerkrankungen, disseminierte intravasale Koagulation, Fieber, Hepatosplenomegalie, verbunden mit entzündlichen oder metabolischen Lebererkrankungen, Endomyokarditis, Kapillarthrombose, Meningoenzephalitis aufgrund infektiöser Agenzien, Organtransplantation, rheumatoide Arthritis und Bindegewebeerkrankungen und Autoimmunerkrankungen, Hyperthyreoidismus, Schädigungen durch Strahlungen und/oder chemotherapeutische Agenzien und chronisches Erschöpfungssyndrom ein.
- Weil die Verwendung einiger Arzneimittel auch hohe Ceramidspiegel induzieren kann, zieht die vorliegende Erfindung auch in Betracht Ceramidspiegel bei Patienten zu erniedrigen, die mit einem solchen Arzneimittel behandelt werden. Zum Beispiel kann eine basische Aminosäure in Übereinstimmung mit der vorliegenden Erfindung gemeinsam verabreicht werden mit Corticosteroiden (wie Dexamethason), entzündungshemmenden (wie Indomethacin), antiviralen (wie Interferon), immunsuppressiven (wie Cyclosporin), Chemotherapie-Agenzien (wie Adriamicin), die Immunantwort verstärkenden (wie Immunoglobulinen und Vaccinen) und endokrinologischen Agenzien (wie Metimazol), um erhöhte Ceramidspiegel zu verhindern.
- Normale Ceramidspiegel bei gesunden Patienten sind abhängig von Alter, Größe und Gewicht des Individuums, aber sie liegen allgemein innerhalb des Bereiches von 5 bis 50 Pikomol/106 Zellen (bevorzugt Lymphozyten des peripheren Blutes). Spiegel, die höher sind als 50 Pikomol/106 Zellen, werden als hohe Spiegel angesehen. Die Verwendung von erfindungsgemäßen basischen Aminosäuren, acylierten basischen Aminosäuren und deren pharmakologisch verträglichen Salzen reduziert solche hohen Spiegel um mindestens 25 %.
- Allgemein müssen L-Carnitin oder eines seiner Acylderivate in Übereinstimmung mit der vorliegenden Erfindung in Konzentrationen verabreicht werden, welche Ceramidspiegel um mindestens 25 % reduzieren. Entsprechend wird dieses Ergebnis durch die Verabreichung von 50 mg bis ungefähr 15 g/Tag basischer Aminosäuren auf oralem oder parenteralem Weg erzielt. Bevorzugt sollten hohe Mengen L-Carnitin und Acyl-L-carnitine und ihrer pharmakologisch verträglichen Salze verabreicht werden, d. h. >1 g pro Tag, >2 g pro Tag; insbesondere bevorzugt 4-10 g pro Tag.
- Die Überwachung von Ceramidspiegeln kann entweder durch direkte Überwachung von Ceramidspiegeln in einer Zelle (wie einem Lymphozyt) oder durch indirekte Überwachung der Konzentrationen eines Ceramid-Metaboliten in einer Zelle ausgeführt werden. Bevorzugt werden die Ceramidspiegel eines Patienten sowohl vor als auch im Anschluss an die Verabreichung von L-Carnitin und Acyl-L-carnitinen und ihrer pharmakologisch verträglichen Salzen überwacht, um das Ausmaß der Reduktion zu bewerten. Die Überwachung kann zu jeder Zeit im Anschluss an die Verabreichung beginnen, aber sie wird geeigneterweise nach 3 Stunden begonnen, um genaue Ergebnisse sicherzustellen. Die Überwachung kann unbegrenzt fortgesetzt werden.
- Ceramidspiegel können direkt gemessen werden durch Isolierung von Lymphozyten des peripheren Blutes eines Patienten. Danach werden die Zellen zentrifugiert, um den Überstand zu eliminieren, und die Lipide werden aus dem Zellpellet entfernt. Die organische Phase, die das Ceramid enthält, kann unter Verwendung des "DAG-Kinase-Assays" auf die Phosphorylierung des Ceramids untersucht werden, was dann mittels Autoradiographie nachgewiesen wird [M. G. Cifone et al., J. Exp. Med., 180(4): 1547-52].
- Nachdem diese Erfindung allgemein beschrieben wurde, kann weiteres Verständnis durch Bezugnahme auf bestimmte spezifische Beispiele erlangt werden, die hierin nur zum Zweck der Veranschaulichung bereitgestellt werden und nicht zur Begrenzung vorgesehen sind, falls es nicht anders angegeben wird.
- Beispiel 1
- Lymphozyten des peripheren Blutes wurden gemäß der klassischen Methodologien isoliert. Die Zellen wurden mit L-Carnitin (200 μg/ml) oder mit Isovaleryl-L-carnitin 30 Minuten lang bei 37 °C, und anschließend weitere 30 Minuten lang mit einem monoklonalen Anti-Fas-Antikörper inkubiert. Die Zellen wurden dann zentrifugiert, wodurch der Überstand eliminiert wurde, und das Zellpellet wurde delipidiert. Die organische Phase (das Ceramid enthaltend) wurde mit dem "DAG-Kinase-Assay" auf die Phosphorylierung von Ceramid untersucht, was anschließend mittels Autoradiographie nachgewiesen wurde.
- Die Ergebnisse werden unten in Tabelle 1 angegeben. Tabelle 1

Ein Anti-Fas-Antikörper wurde eingesetzt, um die Produktion von Ceramid von einem Grundwert (20 Pikomol pro 106 Zellen) auf 81,6 Pikomol pro 106 Zellen zu erhöhen.
- Es wurde somit gezeigt, dass L-Carnitin und Isovaleryl-L-carnitin die Synthese von Ceramid in vitro hemmen.
- Beispiel 2
- Zwei Patienten, die von symptomatischer Neuromyopathie (chronisches Erschöpfungssyndrom) betroffen sind, wurden mit 3 g pro Tag L-Carnitin über den oralen Weg zwei Monate lang behandelt.
- Ceramid wurde in den Muskeln vor und nach der Verabreichung gemessen.
- Die Ergebnisse sind unten in Tabelle 2 angegeben.
- Beispiel 3
- Vier von Hyperthyreoidismus betroffene Patienten, die mit Metimazol (15 mg über oralen Weg, täglich) mehr als acht Monate lang behandelt worden sind, wurden 4 Wochen lang mit 8 g pro Tag L-Carnitin über den oralen Weg behandelt.
- Das Lymphozyt-assoziierte Ceramid wurde vor und nach der Behandlung bestimmt.
- Die Ergebnisse werden in der folgenden Tabelle 3 gezeigt.
- Beispiel 4
- Drei Patienten mit Hepatosplenomegalie aufgrund von viraler Hepatitis des C-Typs wurden mit 4 g L-Carnitin-Bolus über den intravenösen Weg behandelt.
- Das Lymphozyt-assoziierte Ceramid wurde vor und 3 und 48 Stunden nach der Infusion bestimmt.
- Die Ergebnisse sind in der folgenden Tabelle 4 gezeigt.
- Es ist offensichtlich, dass die Verabreichung eines Bolus L-Carnitin den Anstieg der Ceramidspiegel schon 3 Stunden nach der Infusion hemmte. Dieser Effekt hielt mindestens zwei Tage lang an.
- Beispiel 5 (kein Teil der Erfindung)
- Vier Patienten, die von Proteinkatabolismus und Lipiddepletion als eine Folge von tuberkularer Infektion betroffen waren, wurden zwei Wochen lang mit 8 g pro Tag L-Carnitin über den parenteralen Weg behandelt.
- Das mit Lymphozyten des peripheren Bluts assoziierte Ceramid wurde vor und nach der Behandlung bestimmt.
- Die Ergebnisse sind in der folgenden Tabelle 5 gezeigt.
Claims (3)
- Verwendung von L-Carnitin oder eines Acyl-L-carnitins, worin das Acyl ausgewählt ist aus der Gruppe, die Acetyl, Propionyl, Butyryl, Valeryl und Isovaleryl oder eines ihrer pharmazeutisch verträglichen Salze umfasst, zur Herstellung eines Medikaments für die Prophylaxe oder therapeutische Behandlung von Krankheiten oder zellulären Störungen, die durch hohe Spiegel von Ceramid gekennzeichnet sind, wobei die genannten Krankheiten oder Störungen entzündliche Darmerkrankungen, disseminierte intravasale Koagulation, Kapillarthrombose, Meningoenzephalitis aufgrund von infektiösen Agenzien, Bindegewebeerkrankungen und Schäden durch Bestrahlungen umfassen.
- Verwendung nach Anspruch 1, worin die basische Aminosäure, die acylierte basische Aminosäure, L-Carnitin, das Acyl-L-carnitin oder das pharmazeutisch verträgliche Salz davon, gemeinsam mit Corticosteroiden, entzündungshemmenden Agenzien, antiviralen Agenzien, Immunosuppressiva, zystostatischen Agenzien und immunstimulierenden Mitteln verabreicht werden.
- Verwendung von Isovaleryl-L-carnitin zur Herstellung eines Medikaments zur Prophylaxe oder therapeutischen Behandlung von Krankheiten oder zellulären Störungen, die durch hohe Pegel an Ceramid hervorgebracht werden, wobei die genannten Krankheiten oder Störungen entzündliche Darmerkrankungen, disseminierte intravasale Koagulation, Fieber, Hepatosplenomegalie, verbunden mit entzündlichen oder metabolischen Lebererkrankungen, Endomyokarditis, Kapillarthrombose, Meningoenzephalitis aufgrund von infektiösen Agenzien, Störungen, die durch Organtransplantation induziert werden, rheumatoide Arthritis und Bindegewebeerkrankungen, Autoimmunerkrankungen, Hyperthyreose, Schäden durch Bestrahlungen und/oder chemotherapeutische Agenzien und chronisches Erschöpfungssyndrom umfassen.
