Description
- Die Erfindung betrifft einen Filter-Katheter wie er im Überbegriff des Anspruchs 1 beschrieben ist. Ein derartiger Katheter ist aus dem Patent FR-A-2 606 642 bekannt. Die flexiblen Bänder zwischen den Aussparungen können eine aufgeblähte Form durch Verschiebung der inneren Leitung in proximaler Richtung gegenüber dem Kanal einnehmen. Als Folge davon werden die Bänder aufgefaltet nach Art von Regenschirm-Stäbchen bis gegen die Wand eines Blutgefäßes. Der so gebildete Filter dient insbesondere dazu, Blutgerinsel zurück zu halten.
- Die Erfindung betrifft die Verbesserung eines Filter-Katheters dieser an sich bekannten Art.
- Dieser Zweck wird bezüglich des Filter-Katheters gemäß der Erfindung durch die kennzeichenden Merkmale des Anspruchs 1 erreicht.
- Durch die Leitung kann man in geeigneter Weise ein therapeutisches Behandlungsmittel an den Ort des Filters bringen. Ein derartiges Mittel ist beispielsweise ein Antigerinnungsmittel vom Heparin-Typ oder ein Fibrinolit-Mittel. Auf diese Weise können die aufgefangenen Blutgerinsel aufgelöst werden.
- Weitere Merkmale und Vorteile ergeben sich deutlich aus der folgenden Beschreibung einer bevorzugten Ausführungsform des Filter-Katheters zur zeitweiligen Verwendung, das in den beigefügten Zeichnungen dargestellt ist, in denen
-
1 eine schematische Schnittansicht des Filters in Ruhestellung ist, -
2 eine schematische Schnittansicht des aufgeweiteten Filters ist, -
3 eine Querschnittsansicht des Filters gemäß der AchseD in2 ist, -
4 eine schematische Schnittansicht des distalen Endes eines Filters ist, -
5 eine schematische Seitenansicht der Vorrichtung zum Halten des Filter-Katheters während der Behandlung zur Heißfixierung ist. - Der Filter-Katheter
1 der Erfindung setzt sich im wesentlichen aus einem hohlen Kanal2 und einer hohlen Leitung3 zusammen, die koaxial im Inneren des Kanals2 angeordnet ist. Diese beiden Elemente, Kanal2 und Leitung3 bestehen aus Polytetrafluorethylen. - In der vorliegenden Beschreibung sind die Begriffe „proximal" und „distal" so verwendet, daß sie den Teil des Körpers des Patienten als Bezug nehmen, durch den der Filter eingeführt wird. Somit ist das distale Ende des Filters, der in den
1 und2 rechts dargestellt ist, die am weitesten von diesem Teil des Körpers entfernte Ende und ist das proximale Ende, das am nächsten Liegende. - Die distalen Enden
2a des Kanals2 bzw.3a der Leitung3 sind warmverschweißt zusammengefügt sowie um einen Metallring4 aus rostfreiem Stahl, Platin oder Gold angeordnet. Dieser Ring4 ist aus einem strahlungshalbdurchlässigen Material, das während der Positionierung des Filters erfaßbar ist. - Wie sich das deutlich aus den
1 und2 ergibt, ist der Ring4 zwischen dem Kanal2 und dem distalen Ende3a der Leitung3 angeordnet, wobei das distale Ende2a des Kanals in das Innere der Leitung3 gefaltet ist und das distale Ende3a der Leitung abdeckt. Die Wärmeverschweißung erreicht die Zusammenfügung dieser Elemente und bildet so das Ende15 des Filter-Katheters. - Gemäß einem anderen Ausführungsbeispiels, das in
4 dargestellt ist, wirkt die Wärmeverschweißung dagegen so, daß das distale Ende3a der inneren Leitung3 jenseits des Metallrings4 mündet und sich nach außen erweitert, wo es mit dem distalen Ende2a des Kanals2 in Kontakt kommt. - Stromauf seines distalen Endes
2a weist der Kanal2 sechs Einschnitte5 auf, die in Längsrichtung über eine Länge d von 30 mm eingebracht sind; sie sind über den Umfang des Kanals gleichmäßig verteilt. Diese Einschnitte5 begrenzen sechs identische Bänder6 , deren jedes einen Winkelraum von 60° des Kanals2 einnimmt. Bei einem Kanal2 mit 3 mm Durchmesser besitzt jedes Band 6 eine Größe von etwa 1,5 mm. - Die Leitung
3 ist hohl und weist einen Zentralkanal7 auf, in dem ein Metallstab8 frei bewegbar ist, der als Führungsstab bei der Einführung des Filter-Katheters dient. - Am proximalen Ende, jedoch außerhalb des Körper des Patienten ist der Kanal
2 an einer Halte- und Versorgungseinrichtung9 fixiert. Diese Einrichtung9 weist ein Zylinderrohr sowie endseitige Abdeckteile11 und12 auf. Das erste ringförmige Abdeckteil11 verbindet das proximale Ende2b des Kanals2 am vorderen Teil des Rohrs10 . Das zweite Abedeckteil12 umgibt die Leitung3 und schließt den hinteren Teil des Rohrs10 ab. Ein Winkelstück13 mündet schräg in den Körper des Rohrs10 ein. - Das proximale Ende
3b der Leitung3 ragt über das Rohr10 hinaus. Das Abdeckteil12 umschließt dieses distale Ende3b in dichter Weise; andererseits kann die Leitung3 in Längsrichtung in dem Abdeckteil12 gegenüber dem Rohr10 und damit dem Kanal2 gleiten. - Eine Blockierabdeckung
14 dient zur Sicherstellung der Blockierung der Leitung3 gegen das Abdeckteil12 . - Das Rohr
10 ist in an sich bekannter Weise nahe dem Patienten an einem Gestell befestigt, das 2 Systeme zur Versorgung mit zu injizierenden Mitteln trägt, wobei das Erstere in dem proximalen Ende3b der Leitung mündet und wobei das Letztere in dem Winkelstück13 des Rohrs10 mündet. - Die Nutzung des Filter-Katheters erfolgt folgendermaßen:
In einer ersten Phase wird der Führungsstab8 auf perkutanem Wege in die geeignete Vene eingeführt bis sein distales Ende8a sich in der unteren Hohlvene des Patienten in Lage befindet. - In einer zweiten Phase läßt man von außerhalb des Körpers des Patienten den Filter-Katheter
1 längs des Führungsstabes8 gleiten bis der Ring4 sich in der Hohlvene in Lage befindet, dort, wo der Filter einzusetzen bzw. zu implantieren ist. - Während der Durchführung dieser zweiten Phase sind die Bänder
6 in ihrer Ruhestellung, d. h. daß sie sich in der Erstreckung bzw. Verlängerung des Kanals2 befinden. - In einer dritten Phase, die durch
2 dargestellt ist und die dem Aufblähen oder Aufweiten des Filters entspricht, ist das Blockierabdeckteil14 entsperrt derart, daß die Leitung3 in dem Abdeckten12 gleiten kann. Dann verschiebt man mit Vorsicht das proximale Ende3b der Leitung3 nach hinten in Richtung des Pfeils F. In dieser Phase wird die Leitung3 verschoben ebenso wie der Ring4 , der mit dem distalen Ende3a der Leitung fest verbunden ist. Unter der Voraussetzung, daß das Rohr14 und damit der Kanal2 in Lage gehalten sind, kann das Rückpolen des Rings4 in Richtung des Pfeils F nur dank des Vorhandenseins der flexiblen Bänder6 erfolgen. Jedes Band 6 faltet sich individuell auf unter Bildung einer homogenen Querverformung des Kanals in dem Bereich der Einschnitte5 . Die Verschiebung und die Länge der Bänder6 sind derart gewählt, daß in der Aufweitungsposition die mittleren oder medianen Teile15 der Bänder6 , die der Ebene D der Krümmung entsprechen, sich an die Innenwand16 der Vene anlegen. Wenn einmal der Filter richtig aufgeweitet ist, wird das Blockierabdeckten14 wieder derart verriegelt, daß es jede Verschiebung der Leitung3 gegenüber dem Kanal2 verhindert und somit die Bänder6 in ihrem in Querrichtung verformten Zustand hält. - Wie sich das deutlich aus
3 ergibt, die einen Schnitt gemäß der EbeneD darstellt, sind die sechs Bänder6 in Querrichtung gegenüber dem Kanal2 aufgeweitet und verdecken teilweise das Innere17 der Vene18 . Auf diese Weise werden die sich im Inneren17 der Vene18 bewegenden Blutgerinsel durch die Bänder16 aufgehalten während das Blut frei zirkulieren kann. - Vor einer Nutzung des Filter-Katheters wird dieses einer Wärmebehandlung zur Heißfixierung unterworfen. Diese Behandlung besteht im Vorsehen eines Wärmeschocks an dem distalen Teil des Filters, während die flexiblen Bänder
6 in ihrem aufgeweiteten Zustand sind, wie in2 dargestellt. Die Dauer dieser Behandlung und deren Temperatur sind dabei derart bestimmt, daß die durch die Bänder eingenommene Form in irgend einer Art und Weise in der Polymerstruktur des Polytetrafluorethylens gespeichert ist. Diese Behandlung verbessert den Widerstand gegenüber einer Verformung der Bänder6 und vermeidet ein Einsinken oder Einfallen des Filters im Inneren der Vene18 und die Gefahr einer Trombogenese aufgrund von im Blutfluß erzeugten Turbulenzen durch eine Heterogenität der Anordnung der Bänder6 in der Vene. - Gemäß einem anderen Ausführungsbeispiel der Heißfixierungsbehandlung, wie sie in
5 dargestellt ist, wird die thermische oder Wärmebehandlung an dem distalen Teil des Filter-Katheters ausgeübt während die flexiblen Bänder6 eine verdrehte Form besitzen. Zu diesem Zweck wird in der inneren Leitung3 ein Dorn19 eingepaßt, um den distalen Teil des Filter-Katheters zu verfestigen; das distale Ende2a des Kanals2 wird in einem stromaufseitigen Zweig20 eines Halterungsteils21 in Lage blockiert, wobei der Kanal2 einer Rotation um sich selbst unterworfen wird, die den flexiblen Bändern6 die verdrehte in5 dargestellte Form gibt, woraufhin der Kanal2 stromauf dieser flexiblen Bänder in dem hinteren Zweig22 des Halterungsteils21 blockiert wird. Das Halterungsteil21 und der distale Teil des Filter-Katheters werden in einem Brennofen angeordnet, beispielsweise einem, der mit Mikrowellen arbeitet, um dort die Wärmebehandlung während 4 bis 5 Minuten durchzuführen. Es ist anzumerken, daß während der Nutzung des Filters die flexiblen Bänder6 die während der Wärmebehandlung aufgezwungene wendelförmige Form einnehmen möchten. Selbstverständlich können auf dem Halterungsteil21 die Bänder6 die Ruhelage einnehmen, wie es in5 dargestellt ist, oder auch die aufgeweitete Lage. - Ein therapeutisches Behandlungsmittel kann stromauf des Filters durch die Leitung
3 injiziert werden, ausgehend von seinem proximalen Ende3b . Es handelt sich um ein Antiblutgerinnungsmittel vom Heparintyp oder ein Fibrinilot. Die Leitung3 kann in der den Bändern6 zuweisenden Zone Öffnungen beispielsweise in Wendelform aufweisen. Auf diese Weise kann das therapeutische Behandlungsmittel, das in die Leitung3 eingeführt ist, gleichzeitig stromauf und in Höhe des Filters selbst verteilt werden. - Das gleiche Mittel oder auch ein anderes therapeutisches Behandlungsmittel kann in die Zone unmittelbar stromab des Filters durch den Kanal
2 ausgehend von dem Winkelstück13 des Rohrs10 injiziert werden. - Wenn einmal die Behandlung als zufriedenstellen beurteilt ist, erfolgt das Zurückziehen bzw. Entfernen des Filters in folgender Weise. Das Blockierabdeckteil
14 wird entsperrt, und das proximale Ende3b der Leitung3 wird in Gegenrichtung zum Pfeil F zurückgestoßen bis die länglichen Teile oder flexiblen Bänder6 ihre Ruhelage wieder eingenommen haben, geradelinig und parallel zueinander. Dann wird das Abdeckteil14 von neuem wieder verriegelt und die Anordnung aus Leitung3 und Kanal2 wird aus der Vene herausgezogen. - Untersuchungen bzw. Versuche mit Hilfe des Filter-Katheters gemäß der Erfindung haben zur Feststellung geführt, daß die Bildung von Blutgerinseln aufgrund des Vorhandenseins des Filters in der Vene nicht mehr vorkommt.
- Die Erfindung ist nicht auf das beschriebene beispielhafte Ausführungsbeispiel beschränkt, sondern umfaßt alle Weiterbildungen, die sich aus dem Anspruch 1 ergeben. Insbesondere kann der Führungsstab
8 hohl ausgebildet sein und die Injektion eines Behandlungsmittels stromauf des Filters selbst vorsehen deutlich jenseits des distalen Endes des Filter-Katheters1 .
Claims (2)
- Filter-Katheter zur teilweisen und zumindest zeitweiligen Unterbrechung eines Blutgefäßes, mit einem Kanal (
2 ) und mit einem distalen Ende und einem proximalen Ende, wobei an seinem distalen Ende symmetrisch über den Umfang verteilt Längseinschnitte vorgesehen sind, die flexible Bänder (6 ) begrenzen, wobei sich in einem Lumen des Kanals (2 ) eine innere Leitung (3 ) mit einem distalen Ende und einem proximalen Ende befindet, wobei der Kanal (2 ) und der inneren Leitung (3 ) an ihren distalen Enden (2a ,3a ) fest miteinander verbunden sind, dadurch gekennzeichnet, daß zwischen dem Lumen des Kanals (2 ) und der inneren Leitung (3 ) eine Leitung gebildet ist für das Einführen eines Behandlungsmittels, wobei die Leitung einerseits mit dem Filterelement und andererseits mit einem Innenraum einer Einrichtung (9 ) zum Halten und zum Versorgen verbunden ist, die im wesentlichen zylindrisch und koaxial zum Kanal (2 ) ist, an einem ihrer Enden mit diesem Kanal (2 ) fest verbunden ist und an einem ihrer anderen Ende die innere Leitung (3 ) einsperrt, jedoch eine Verschiebbarkeit in abdichtende Weise erlaubt. - Filter-Katheter nach Anspruch 1 dadurch gekennzeichnet, daß die Halte- und Versorgungseinrichtung (
9 ) Abdichtmittel zur gleitenden Aufnahme der inneren Leitung aufweist.
