Description

  • Die Erfindung betrifft ein Verfahren zur Herstellung eines Filter-Katheters wie es im Oberbegriff des Anspruchs 1 beschrieben ist.
  • Ein mit diesem Verfahren hergestellter Filter-Katheter ist aus dem Patent FR-A-2 606 642 bekannt.
  • Um den Filter in eine aktive Lage zu bringen, d. h. um die flexiblen Bänder in deren aufgeweiteten Zustand zu verformen wird der Kanal in distaler Richtung auf der inneren Leitung verschoben. Als Folge davon wird der Kanal einer Kompressionskraft ausgesetzt. Der Kanal muß daher derart dimensioniert sein, daß er diesen Kräften widerstehen kann.
  • Die Erfindung bezweckt die Angabe eines Verfahrens wie es in der Einleitung beschrieben ist, mittels dem ein verbesserter Filter-Katheter hergestellt werden kann.
  • Dieser Zweck wird mittels des Verfahrens gemäß der Erfindung aufgrund der im Kennzeichen des Anspruches 1 angegebenen Merkmale erreicht.
  • Es ist anzumerken, daß aus der US-A-4 832 055 ein Filter bekannt ist, der zur Aufweitung in einem Blutgefäß bestimmt ist. Der Filter weist mehrere Fäden auf, die in eine nicht aufgeweitete und verdrehte Form vorgeformt sind. Die Fäden sind metallisch und verdreht, damit sie eine abgeflachte Form einnehmen können wenn sie aufgeweitet sind.
  • Die Erfindung betrifft und erreicht auch ein Filterelement wie es im Anspruch 2 angegeben ist.
  • Andere Merkmale und Vorteile ergeben sich deutlich aus der folgenden Beschreibung eines bevorzugten Ausführungsbeispiels des Filter-Katheters zur zeitweisen Verwendung sowie seines Herstellungsverfahrens, wie es in den beigefügten Zeichnungen dargestellt ist, worin
  • Fig. 1 eine schematische Schnittansicht des Filters in Ruhestellung ist,
  • Fig. 2 eine schematische Schnittansicht des aufgeweiteten Filters ist,
  • Fig. 3 eine Querschnittsansicht des Filters gemäß der Achse D in Fig. 2 ist,
  • Fig. 4 eine schematische Schnittansicht des distalen Endes eines Filters ist,
  • Fig. 5 eine schematische Seitenansicht der Vorrichtung zum Halten des Filter-Katheters während der Behandlung zur Heißfixierung ist.
  • Der Filter-Katheter 1 der Erfindung setzt sich im wesentlichen aus einem hohlen Kanal 2 und einer hohlen Leitung 3 zusammen, die koaxial im Inneren des Kanals 2 angeordnet ist. Diese beiden Elemente, Kanal 2 und Leitung 3 bestehen aus Polytetrafluorethylen.
  • In der vorliegenden Beschreibung sind die Begriffe "proximal" und "distal" so verwendet, daß sie den Teil des Körpers des Patienten als Bezug nehmen, durch den der Filter eingeführt wird. Somit ist das distale Ende des Filters, der in den Fig. 1 und 2 rechts dargestellt ist, die am weitesten von diesem Teil des Körpers entfernte Ende und ist das proximale Ende, das am nächsten Liegende.
  • Die distalen Enden 2a des Kanals 2 bzw. 3a der Leitung 3 sind warmverschweißt zusammengefügt sowie um einen Metallring 4 aus rostfreiem Stahl, Platin oder Gold angeordnet. Dieser Ring 4 ist aus einem strahlungshalbdurchlässigen Material, das während der Positionierung des Filters erfaßbar ist.
  • Wie sich das deutlich aus den Fig. 1 und 2 ergibt, ist der Ring 4 zwischen dem Kanal 2 und dem distalen Ende 3a der Leitung 3 angeordnet, wobei das distale Ende 2a des Kanals in das Innere der Leitung 3 gefaltet ist und das distale Ende 3a der Leitung abdeckt. Die Wärmeverschweißung erreicht die Zusammenfügung dieser Elemente und bildet so das Ende 15 des Filter-Katheters.
  • Gemäß einem anderen Ausführungsbeispiels, das in Fig. 4 dargestellt ist, wirkt die Wärmeverschweißung dagegen so, daß das distale Ende 3a der inneren Leitung 3 jenseits des Metallrings 4 mündet und sich nach außen erweitert, wo es mit dem distalen Ende 2a des Kanals 2 in Kontakt kommt.
  • Stromauf seines distalen Endes 2a weist der Kanal 2 sechs Einschnitte 5 auf, die in Längsrichtung über eine Länge d von 30 mm eingebracht sind; sie sind über den Umfang des Kanals gleichmäßig verteilt. Diese Einschnitte 5 begrenzen sechs identische Bänder 6, deren jedes einen Winkelraum von 60º des Kanals 2 einnimmt. Bei einem Kanal 2 mit 3 mm Durchmesser besitzt jedes Band 6 eine Größe von etwa 1,5 mm.
  • Die Leitung 3 ist hohl und weist einen Zentralkanal 7 auf, in dem ein Metallstab 8 frei bewegbar ist, der als Führungsstab bei der Einführung des Filter-Katheters dient.
  • Am proximalen Ende, jedoch außerhalb des Körper des Patienten ist der Kanal 2 an einer Halte- und Versorgungseinrichtung 9 fixiert. Diese Einrichtung 9 weist ein Zylinderrohr sowie endseitige Abdeckteile 11 und 12 auf. Das erste ringförmige Abdeckteil 11 verbindet das proximale Ende 2b des Kanals 2 am vorderen Teil des Rohrs 10. Das zweite Abedeckteil 12 umgibt die Leitung 3 und schließt den hinteren Teil des Rohrs 10 ab. Ein Winkelstück 13 mündet schräg in den Körper des Rohrs 10 ein.
  • Das proximale Ende 3b der Leitung 3 ragt über das Rohr 10 hinaus. Das Abdeckteil 12 umschließt dieses distale Ende 3b in dichter Weise; andererseits kann die Leitung 3 in Längsrichtung in dem Abdeckteil 12 gegenüber dem Rohr 10 und damit dem Kanal 2 gleiten.
  • Eine Blockierabdeckung 14 dient zur Sicherstellung der Blockierung der Leitung 3 gegen das Abdeckteil 12.
  • Das Rohr 10 ist in an sich bekannter Weise nahe dem Patienten an einem Gestell befestigt, das 2 Systeme zur Versorgung mit zu injizierenden Mitteln trägt, wobei das Erstere in dem proximalen Ende 3b der Leitung mündet und wobei das Letztere in dem Winkelstück 13 des Rohrs 10 mündet.
  • Die Nutzung des Filter-Katheters erfolgt folgendermaßen:
  • In einer ersten Phase wird der Führungsstab 8 auf perkutanem Wege in die geeignete Vene eingeführt bis sein distales Ende 8a sich in der unteren Hohlvene des Patienten in Lage befindet.
  • In einer zweiten Phase läßt man von außerhalb des Körpers des Patienten den Filter- Katheter 1 längs des Führungsstabes 8 gleiten bis der Ring 4 sich in der Hohlvene in Lage befindet, dort, wo der Filter einzusetzen bzw. zu implantieren ist. Während der Durchführung dieser zweiten Phase sind die Bänder 6 in ihrer Ruhestellung, d. h. daß sie sich in der Erstreckung bzw. Verlängerung des Kanals 2 befinden.
  • In einer dritten Phase, die durch Fig. 2 dargestellt ist und die dem Aufblähen oder Aufweiten des Filters entspricht, ist das Blockierabdeckteil 14 entsperrt derart, daß die Leitung 3 in dem Abdeckteil 12 gleiten kann. Dann verschiebt man mit Vorsicht das proximale Ende 3b der Leitung 3 nach hinten in Richtung des Pfeils F. In dieser Phase wird die Leitung 3 verschoben ebenso wie der Ring 4 der mit dem distalen Ende 3a der Leitung fest verbunden ist. Unter der Voraussetzung, daß das Rohr 10 und damit der Kanal 2 in Lage gehalten sind, kann das Rückholen des Rings 4 in Richtung des Pfeils F nur dank des Vorhandensseins der flexiblen Bänder 6 erfolgen. Jedes Band 6 faltet sich individuell auf unter Bildung einer homogenen Querverformung des Kanals in dem Bereich der Einschnitte 5. Die Verschiebung und die Länge der Bänder 6 sind derart gewählt, daß in der Aufweitungsposition die mittleren oder medianen Teile 15 der Bänder 6, die der Ebene D der Krümmung entsprechen, sich an die Innenwand 16 der Vene anlegen. Wenn einmal der Filter richtig aufgeweitet ist, wird das Blockierabdeckteil 14 wieder derart verriegelt, daß es jede Verschiebung der Leitung 3 gegenüber dem Kanal 2 verhindert und somit die Bänder 6 in ihrem in Querrichtung verformten Zustand hält.
  • Wie sich das deutlich aus Fig. 3 ergibt, die einen Schnitt gemäß der Ebene D darstellt, sind die sechs Bänder 6 in Querrichtung gegenüber dem Kanal 2 aufgeweitet und verdecken teilweise das Innere 17 der Vene 18. Auf diese Weise werden die sich im Inneren 17 der Vene 18 bewegenden Blutgerinsel durch die Bänder 16 aufgehalten während das Blut frei zirkulieren kann.
  • Vor einer Nutzung des Filter-Katheters wird dieses einer Wärmebehandlung zur Heißfixierung unterworfen. Diese Behandlung besteht im Vorsehen eines Wärmeschocks an dem distalen Teil des Filters, während die flexiblen Bänder 6 in ihrem aufgeweiteten Zustand sind, wie in Fig. 2 dargestellt. Die Dauer dieser Behandlung und deren Temperatur sind dabei derart bestimmt, daß die durch die Bänder eingenommene Form in irgend einer Art und Weise in der Polymerstruktur des Polytetrafluorethylens gespeichert ist. Diese Behandlung verbessert den Widerstand gegenüber einer Verformung der Bänder 6 und vermeidet ein Einsinken oder Einfallen des Filters im Inneren der Vene 18 und die Gefahr einer Trombogenese aufgrund von im Blutfluß erzeugten Turbulenzen durch eine Heterogenität der Anordnung der Bänder 6 in der Vene. Die thermische oder Wärmebehandlung (vgl. Fig. 5) wird am dem distalen Teil des Filter-Katheters ausgeübt während die flexiblen Bänder 6 eine verdrehte Form besitzen. Zu diesem Zweck wird in der inneren Leitung 3 ein Dorn 19 eingepaßt, um den distalen Teil des Filter-Katheters zu verfestigen; das distale Ende 2a des Kanals 2 wird in einem stromaufseitigen Zweig 20 eines Halterungsteils 21 in Lage blockiert, wobei der Kanal 2 einer Rotation um sich selbst unterworfen wird, die den flexiblen Bändern 6 die verdrehte in Fig. 5 dargestellte Form gibt, woraufhin der Kanal 2 stromauf dieser flexiblen Bänder in dem hinteren Zweig 22 des Halterungsteils 21 blockiert wird. Das Halterungsteil 21 und der distale Teil des Filter-Katheters werden in einem Brennofen angeordnet, beispielsweise einem, der mit Mikrowellen arbeitet, um dort die Wärmebehandlung während 4 bis 5 Minuten durchzuführen. Es ist anzumerken, daß während der Nutzung des Filters die flexiblen Bänder 6 die während der Wärmebehandlung aufgezwungene wendelförmige Form einnehmen möchten.
  • Ein therapeutisches Behandlungsmittel kann stromauf des Filters durch die Leitung 3 injiziert werden, ausgehend von seinem proximalen Ende 3b. Es handelt sich um ein Antiblutgerinnungsmittel vom Heparintyp oder ein Fibrinilot. Die Leitung 3 kann in der den Bändern 6 zuweisenden Zone Öffnungen beispielsweise in Wendelform aufweisen. Auf diese Weise kann das therapeutische Behandlungsmittel, das in die Leitung 3 eingeführt ist, gleichzeitig stromauf und in Höhe des Filters selbst verteilt werden.
  • Das gleiche Mittel oder auch ein anderes therapeutisches Behandlungsmittel kann in die Zone unmittelbar stromab des Filters durch den Kanal 2 ausgehend von dem Winkelstück 13 des Rohrs 10 injiziert werden.
  • Wenn einmal die Behandlung als zufriedenstellen beurteilt ist, erfolgt das Zurückziehen bzw. Entfernen des Filters in folgender Weise. Das Blockierabdeckteil 14 wird entsperrt, und das proximale Ende 3b der Leitung 3 wird in Gegenrichtung zum Pfeil F zurückgestoßen bis die länglichen Teile oder flexiblen Bänder 6 ihre Ruhelage wieder eingenommen haben, geradelinig und parallel zueinander. Dann wird das Abdeckteil 14 von neuem wieder verriegelt und die Anordnung aus Leitung 3 und Kanal 2 wird aus der Vene herausgezogen. Untersuchungen bzw. Versuche mit Hilfe des Filter-Katheters gemäß der Erfindung haben zur Feststellung geführt, daß die Bildung von Blutgerinseln aufgrund des Vorhandenseins des Filters in der Vene nicht mehr vorkommt.
  • Die Erfindung ist nicht auf das beschriebene beispielhafte Ausführungsbeispiel beschränkt, sondern umfaßt alle Weiterbildungen, die sich aus dem Anspruch 1 ergeben. Insbesondere kann der Führungsstab 8 hohl ausgebildet sein und die Injektion eines Behandlungsmittels stromauf des Filters selbst vorsehen deutlich jenseits des distalen Endes des Filter-Katheters 1.

Claims (2)

1. Verfahren zur Herstellung eines Filters zum teilweisen und zumindest zeitweisen Unterbrechen eines Blutgefäßes, mit dem Vorsehen eines Kanals (2) mit einem distalen Ende und einem proximalen Ende, dem Vorsehen im distalen Abschnitt von länglichen Einschnitten, die symmetrisch über den Umfang verteilt sind und die flexible Bänder (6) begrenzen, dem Vorsehen einer inneren Leitung (3), ebenfalls aus Politetrafluorethylen, mit einem distalen Ende und einem proximalen Ende, dem Anbringen der Innenleitung (6) in dem Kanal und der einzigen festen Verbindung an den distalen Enden (2a, 3a) des Kanals (2) und der Leitung (3), dadurch gekennzeichnet, daß die flexiblen Bänder, die durch die länglichen Einschnitte begrenzt sind, in deren aufgeweiteten Zustand verformt werden, daß der distale Abschnitt der die Einschnitte das Kanal aufweist, verdreht wird und in der Verdrehung gehalten wird und daß die flexiblen Bänder einer Heißfixierungsbehandlung über eine solche Dauer und bei einer solchen Temperatur unterworfen werden, das die aufgeweitete und verdrehte Form gespeichert wird.
2. Gemäß dem Verfahren nach Anspruch 1 hergestellter Filter-Katheter zum teilweisen und zumindest zeitweisen Unterbrechen eines Blutgefäßes, mit einem Kanal (2), der an seinem distalen Abschnitt symmetrisch über den Umfang verteilt längliche Einschnitte aufweist, die flexible Bänder (6) begrenzen, und einer inneren Leitung (3) in dem Kanal (2), wobei der Kanal (2) und die innere Leitung (3) an ihren distalen Enden (2a, 3a) fest miteinander verbunden sind, dadurch gekennzeichnet, daß die flexiblen Bänder (6) eine aufgeweitete und verdrehte Form im unbelasteten Zustand aufweisen, wobei die Form durch Heißfixierung erreicht ist.
