  • Die vorliegende Erfindung bezieht sich auf ein Filter zur teilweisen und zumindest vorübergehenden Unterbrechung einer Vene, wie im Oberbegriff von Anspruch 1 beschrieben.
  • Ein solches Filter ist insbesondere aus der FR-A-2 606 642 bekannt. Man hat festgestellt, daß das Vorhandensein eines solchen Katheter-Filters in einer Vene das Auftreten von Blutklümpchen in unmittelbarer Nähe des Filters hervorrufen konnte.
  • Der Erfindung liegt die Aufgabe zugrunde, ein Katheter-Filter der vorgenannten Bauart vorzuschlagen, das dem festgestellten Nachteil abhilft.
  • Dieses Ziel wird erreicht durch ein Filter zur teilweisen und zumindest vorübergehenden Unterbrechung einer Vene gemäß der Erfindung, wie im kennzeichnenden Teil von Anspruch 1 näher beschrieben.
  • Das Polytetrafluorethylen selbst ist schon sehr wenig thrombogen. Um die Verbindung des Kanals und der inneren Leitung durch Thermoschweißen zu erzielen, weist das erfindungsgemäße Katheter-Filter zu seinem distalen Ende hin außer Polytetrafluorethylen kein weiteres mit dem Blutstrom in Berührung kommendes Material auf. Da das Röntgen-undurchlässige Element ein Ring aus einem Röntgen-undurchlässigen Material wie Gold ist, der zwischen dem Kanal und der inneren Leitung eingeschlossen und an ihnen während des Thermoschweißens befestigt wird, tritt dieses Röntgen-undurchlässige Element nicht in Erscheinung und kann weder ein Verhaken mit der Venenwand noch die eventuelle Bildung von Blutklümpchen die verursachen.
  • Somit stellt die Erfindung ein Katheher-Filter bereit, das keine Bildung von Blutklümpchen hervorruft.
  • Es sei darauf hingewiesen, daß die Verwendung von Polytetrafluorethylen für einen Katheter an sich bekannt ist, wie z.B. aus der GB-A-2 017 499. Die Verwendung eines Ringes aus einem Röntgen-undurchlässigen Material ist ebenfalls an sich bekannt, wie z.B. aus der FR-A-2 580 504.
  • Weitere Merkmale und Vorteile ergeben sich deutlicher aus der folgenden Beschreibung der in den Zeichnungen dargestellten bevorzugten Ausführungsform des Katheter-Filters zur vorübergehenden Verwendung, es zeigt:
  • Fig. 1 eine schematische Schnittansicht des Filters in der Ruhestellung;
  • Fig. 2 eine schematische Schnittansicht des ausgebreiteten Filters;
  • Fig. 3 eine Querschnittsanscht des Filters von Fig. 2 entlang der Achse D;
  • Fig. 4 eine schematische Schnittansicht des distalen Endes eines Filters; und
  • Fig. 5 eine schematische Seitenansicht der Haltevorrichtung des Katheter-Filters während der Thermofixierbehandlung.
  • Das Katheter-Filter 1 gemäß der Erfindung besteht im wesentlichen aus einem hohlen Kanal 2 und einer hohlen Leitung 3, die koaxial im Innern des Kanals 2 angeordnet ist.
  • Diese beiden Elemente, der Kanal 2 und die Leitung 3, bestehen aus Polytetrafluorethylen.
  • Im vorliegenden Text werden die Begriffe "proximal" und "distal" verwendet, indem man den Teil des Patientenkörpers als Bezugspunkt nimmt, durch den das Filter eingeführt wird. Somit ist das in Fig. 1 und 2 rechts dargestellte distale Ende des Filters das von diesem Körperteil am weitesten entfernte Ende und das proximale Ende das ihm am nächsten liegende Ende.
  • Die distalen Enden 2a des Kanals 2 bzw. 3a der Leitung 3 sind miteinander und um einen metallischen Ring 4 herum, aus rostfreiem Stahl, Platin oder Gold, thermoverschweißt. Dieser Ring 4 besteht aus einem Röntgen-undurchlässigen Material, das während der Positionierung des Filters detektierbar ist.
  • Wie sich aus Fig. 1 und 2 deutlich ergibt, ist der Ring 4 zwischen dem Kanal 2 und dem distalen Ende 3a der Leitung 3 angeordnet, wobei das distale Ende 2a des Kanals zum Innern der Leitung 3 umgeschlagen ist und das distale Ende 3a der Leitung überdeckt. Das Thermoschweißen bewirkt die Zusammenfügung dieser Elemente, um das Ende 15 des Katheter-Filters zu bilden.
  • Gemäß einer anderen, in Fig. 4 gezeigten Ausführungsform findet das Thermoschweißen statt, wenn das distale Ende 3a der inneren Leitung 3 über den metallischen Ring 4 hinaustritt und sich nach außen hin erweitert, wobei es mit dem distalen Ende 2a des Kanals 2 in Berührung kommt.
  • Vor seinem distalen Ende 2a weist der Kanal 2 sechs Einschnitte 5 auf, die in der Längsrichtung auf einer Länge d von 30 mm eingearbeitet sind; sie sind regelmäßig um den Umfang des Kanals verteilt. Diese Einschnitte 5 begrenzen sechs identische Bänder, von denen jedes einen Winkelraum von 60º des Kanals 2 einnimmt. Für einen Kanal 2 mit 3 mm Durchmesser hat jedes Band 6 eine Breite von etwa 1,5 mm.
  • Die Leitung 3 ist hohl und hat einen mittigen Kanal 7, in dem eine metallische Stange 8 frei gleiten kann, die als Führungsstange beim Einführen des Katheter-Filters dient.
  • Zum proximalen Ende hin, jedoch außerhalb des Patientenkörpers, ist der Kanal 2 an einer Halte- und Versorgungsvorrichtung 9 befestigt. Diese Vorrichtung 9 umfaßt ein zylindrisches Rohrstück 10 sowie Endringe 11 und 12. Der erste Kreisring 11 schließt das proximale Ende 2b des Kanals 2 an den vorderen Teil des Rohrstücks 10 an. Der zweite Ring 12 umgibt die Leitung 3 und verschließt den hinteren Teil des Rohrstück 10. Ein Winkelstück 13 mündet schräg in den Körper des Rohrstücks 10.
  • Das proximale Ende 3b der Leitung 3 geht über das Rohrstück 10 hinaus. Der Ring 12 umklammert dieses distale Ende 3b abdichtend; dennoch kann die Leitung 3 in Längsrichtung in dem Ring 12 in bezug auf das Rohrstück 10 und somit den Kanal 2 gleiten.
  • Ein Blockierring 14 kann die Blockierung der Leitung 3 gegen den Ring 12 bewirken.
  • Das Rohrstück 10 wird auf bekannte Weise in der Nähe des Patienten auf einem Halter befestigt, der zwei Versorgungssysteme für Injektionsprodukte trägt, die einerseits in das proximale Ende 3b der Leitung und andererseits in das Winkelstück 13 des Rohrstücks 10 münden.
  • Die Verwendung des Katheter-Filters findet unter den folgenden Bedingungen statt.
  • In einer ersten Phase wird die Führungsstange 8 auf perkutanem Wege in die passende Vene eingeführt, bis sich ihr distales Ende 8a in der unteren Hohlvene des Patienten in Position befindet.
  • In einer zweiten Phase läßt man von außerhalb des Körpers des Patienten das Katheter-Filter 1 entlang der Führungsstange 8 gleiten, bis sich der Ring 4 in der Hohlvene dort in Position befindet, wo das Filter implantiert werden muß.
  • Während des Verlaufs dieser zweiten Phase sind die Bänder 6 in Ruhestellung, d.h., sie sind in der Verlängerung des Kanals 2.
  • In einer in Fig. 2 gezeigten und der Entfaltung des Filters entsprechenden dritten Phase wird der Blockierring 14 gelöst, um es der Leitung 3 zu ermöglichen, im Ring 12 zu gleiten. Dann verschiebt man vorsichtig das proximale Ende 3b der Leitung 3 nach hinten hin in Richtung des Pfeiles F. In dieser Phase verschiebt sich die gesamte Leitung 3 sowie der mit dem distalen Ende 3a der Leitung verbundene Ring 4. Da das Rohrstück 10 und somit der Kanal 2 in ihrer Position gehalten werden, kann das Zurückweichen des Rings 4 in der Richtung des Pfeiles F nur dank des Vorhandenseins der biegsamen Bänder 6 erfolgen. Jedes Band 6 faltet sich individuell und erzeugt hierdurch eine homogene Querverformung des Kanals im Bereich der Einschnitte 5. Die Verschiebung und die Länge der Bänder 6 werden so ausgewählt, daß sich in der Entfaltungsposition die der Krümmungsebene D entsprechenden Mittelteile 15 der Bänder 6 an der Innenwand 16 der Vene abstützen. Sobald das Filter richtig entfaltet ist, wird der Blockierring 14 erneut gespannt, um jegliche Verschiebung der Leitung 3 in bezug auf den Kanal 2 zu verhindern und somit die Bänder 6 in ihrem Querverformungszustand zu halten.
  • Wie sich aus Fig. 3 deutlich ergibt, die ein Schnitt entlang der Ebene D ist, sind die sechs Bänder 6 in bezug auf den Kanal 2 in Querrichtung entfaltet und versperren teilweise das Innere 17 der Vene 18. Somit werden die sich im Innern 17 der Vene 18 bewegenden Blutklümpchen durch die Bänder 6 aufgehalten, während das Blut frei fließen kann.
  • Vor der Verwendung des Katheter-Filters hat man dieses einer thermischen Behandlung zur Thermofixierung ausgesetzt. Diese Behandlung besteht darin, daß man den distalen Teil des Filters einem thermischen Schock aussetzt, während die biegsamen Bänder 6, wie in Fig. 2 gezeigt, in ihrem entfalteten Zustand sind. Die Dauer dieser Behandlung und die Temperatur werden so bestimmt, daß die durch die Bänder eingenommene Form auf irgendeine Weise in der Polymerstruktur des Polytetrafluorethylens gespeichert wird. Diese Behandlung erhöht den Verformungswiderstand der Bänder 6 und verhindert das Absinken des Filters im Innern der Vene 18 und die Gefahr einer Thrombogenese aufgrund der Turbulenzen, die im Blutstrom durch die Heterogenität der Anordnung der Bänder 6 in der Vene erzeugt werden.
  • Gemäß einer anderen Ausführungsform der in Fig. 5 dargestellten Thermofixierbehandlung wird die thermische Behandlung auf den distalen Teil des Katheter-Filters angewandt, während die biegsamen Bänder 6 eine verdrillte Form haben. Hierzu schiebt man einen Dorn 19 in die innere Leitung 3, um den distalen Teil des Katheter-Filters zu versteifen; man blockiert in einer Position das distale Ende 2a des Kanals 2 in dem vorderen Schenkel 20 eines Halteträgers 21, man läßt den Kanal 2 eine Drehung um sich selbst ausführen, die den biegsamen Bändern 6 die in Fig. 5 dargestellte verdrillte Form verleiht, woraufhin man den Kanal 2 in einer Position stromauf von den biegsamen Bändern 6 in dem hinteren Schenkel 22 des Halteträgers 21 blockiert. Der Träger 21 und der distale Teil des Katheter-Filters werden in einen Ofen zum Garen, z.B. mittels Mikrowellenstrahlung, gebracht, um dort der thermischen Behandlung während 4 bis 5 Minuten ausgesetzt zu werden. Man stellt fest, daß bei der Verwendung des Filters die biegsamen Bänder dazu neigen, die während der thermischen Behandlung verliehene Schraubenform einzunehmen. Auf dem Halteträger 21 können die Bänder 6 natürlich in der Ruhestellung, wie in Fig. 5 dargestellt, oder im entfalteten Zustand sein.
  • Ein Produkt zur therapeutischen Behandlung kann stromauf von dem Filter durch die Leitung 3 hindurch von deren proximalen Ende 3b aus injiziert werden. Es handelt sich um ein Heparin-Antikoaguliermittel oder um ein fibrinolytisches Mittel. Die Leitung 3 kann in dem den Bändern 6 gegenüberliegenden Bereich z.B. spiralförmig angeordnete Öffnungen aufweisen. Somit kann das in die Leitung 3 eingeführte Produkt zur therapeutischen Behandlung gleichzeitig stromaufseitig und auf der Höhe des Filters selbst verteilt werden.
  • Dasselbe Produkt oder ggfs. ein anderes Produkt zur therapeutischen Behandlung kann in den Bereich unmittelbar stromab von dem Filter durch den Kanal 2 hindurch von dem Winkelstück 13 des Rohrstücks 10 aus injiziert werden.
  • Wenn dann die Behandlung als ausreichend beurteilt wird, wird das Filter auf die folgende Weise herausgezogen: Der Blockierring 14 wird gelöst, und das proximale Ende 3b der Leitung 3 wird entgegengesetzt zur Richtung des Pfeiles F geschoben, bis die Zungen 6 wieder in ihre Ruhestellung gelangen, und zwar geradlinig und parallel zueinander. Der Ring 14 wird dann erneut blockiert, und die aus der Leitung 3 und dem Kanal 2 bestehende Einheit wird aus der Vene herausgezogen.
  • Die mit Hilfe des erfindungsgemäßen Katheter-Filters durchgeführten Versuche ließen erkennen, daß aufgrund der Anwesenheit des Filters in der Vene keinerlei Blutklümpchen gebildet wurden.
  • Die vorliegende Erfindung ist nicht auf die Ausführungsform beschränkt, die soeben beispielhaft und nicht erschöpfend beschrieben wurde, sondern deckt sämtliche Abwandlungen im Rahmen der folgenden Ansprüche ab. Insbesondere kann die Führungsstange 8 hohl sein und die Injektion eines Behandlungsproduktes stromab von dem Filter sogar weit hinter dem distalen Ende des Katheter-Filters 1 ermöglichen.

Claims (7)

1. Filter zur teilweisen und zumindest vorübergehenden Unterbrechung eine Vene, welches aufweist:
a) einen Kanal (2), der zu seinem distalen Ende hin sich in der Längsrichtung erstreckende Einkerbungen hat, die auf seinem Umfang symmetrisch verteilt sind und biegsame Bänder (6) begrenzen;
b) und eine Leitung (3) im Innern des Kanals, wobei der Kanal (2) und die Leitung (3) über ihre distalen Enden (2a, 3a) verbunden sind und das distale Ende des Filters mit einem strahlungsundurchlässigen Element ausgestattet ist; dadurch gekennzeichnet, daß der Kanal (2) und die innere Leitung aus Polytetrafluorethylen bestehen, wobei die distalen Enden (2a, 3a) des Kanals (2) und der Leitung (3) thermoverschweißt sind, und daß das Röntgenundurchlässige Element einen Ring (4) in einem Röntgenundurchlässigen Material aufweist, das zwischen den distalen Enden (2a, 3a) des Kanals (2) und der Leitung (3) enthalten ist und an ihnen beim Thermoverschweißen befestigt wird.
2. Filter nach Anspruch 1, dadurch gekennzeichnet, daß das distale Ende (2a) des Kanals zum Innern der Leitung (3) hin umgeschlagen ist und einerseits den Ring (4) und andererseits das distale Ende (3a) der Leitung (3) überdeckt.
3. Filter nach Anspruch 1, dadurch gekennzeichnet, daß das distale Ende (3a) der inneren Leitung (3) sich nach außen hin über den metallischen Ring (4) erweitert.
4. Filter nach Anspruch 1, dadurch gekennzeichnet, daß die Leitung (3) Öffnungen in dem Bereich gegenüber der Bänder (6) aufweist.
5. Filter nach Anspruch 1, dadurch gekennzeichnet, daß der distale Teil des Filters einer Thermofixierbehandlung ausgesetzt wurde und die biegsamen Bänder (6) einen verbesserten Verformungswiderstand haben.
6. Filter nach Anspruch 5, dadurch gekennzeichnet, daß nach Beendigung der Thermofixierbehandlung, während der die Bänder (6) im entfalteten Zustand waren, das die biegsamen Bänder (6) bildende Polytetrafluorethylen die entfaltete Form im Gedächtnis hat.
7. Filter nach einem der Ansprüche 5 oder 6, dadurch gekennzeichnet, daß nach Beendigung der Thermofixierbehandlung, während der die Bänder (6) zu einer Schraube verdreht waren, das die biegsamen Bänder (6) bildende Polytetrafluorethylen die Schraubenform im Gedächtnis hat.
