- 1. Gebiet der Erfindung
- Die Erfindung betrifft Bandscheibenprothesen, die zum Ersatz menschlicher Bandscheiben verwendet werden, die durch Trauma oder Erkrankung geschädigt sind.
- 2. Beschreibung der verwandten Technik
- Altern Menschen, nimmt der Wassergehalt der Kerne in ihren Bandscheiben ab. Zugleich nimmt der Kollagengehalt der Kerne zu; eine solche Degeneration bzw. ein solcher Verschleiß der Scheiben führt zu einem steiferen Kern, der stärker dem umgebenden Ring ähnelt. Diese Änderungen in der Bandscheibe können den Ring gegenüber traumatischen Verletzungen, z. B. Umfangsrissen, anfälliger machen. Mehrfache Risse in einem Ring können einen "Spalt" erzeugen, durch den der Kern entweichen kann. Gewöhnlich bezeichnet man dieses Ereignis als Bandscheibenvorfall, der Schmerzen verursacht, da der verschobene Kern auf benachbarte Nerven drückt.
- Bandscheibenverschleiß zerstört auch die Belastungsmuster in der Wirbelsäule. Ferner beobachteten Experten eine direkte Korrelation zwischen Bandscheibenverschleiß und der Bildung lateral wachsender Osteophyten, die dazu führen können, daß Druck auf benachbartes Nervengewebe ausgeübt wird. Allgemein führt Bandscheibenverschleiß letztlich zu Schmerzen. Am häufigsten tritt der Verschleiß im Lumbalbereich (Lendengegend) von Patienten auf und führt zum Ischiassyndrom, einem Schmerz, der in der Lendengegend beginnt und im Ischiasnerv im Bein nach unten ausstrahlt.
- Zur Behandlung von Problemen im Zusammenhang mit intervertebralem (oder in der Wirbelsäule auftretendem) Bandscheibenverschleiß gibt es zwei chirurgische Standardverfahren: Diskektomie und Arthrodese. Bei der Diskektomie wird eine Bandscheibe teilweise oder vollständig entfernt. Das Verfah ren beinhaltet die Entfernung von Bandscheibenmaterial, das auf Umgebungsnerven treffen kann, wodurch damit zusammenhängende Schmerzen in der Lendengegend oder in den Beinen gemildert werden. Allerdings können Diskektomien mitunter zu umverteilten Belastungsspannungen der Wirbelsäule, postoperativer Wirbelsäuleninstabilität oder anderen Problemen führen. Obwohl einigen Statistiken zufolge 48% bis 86% der Diskektomien anfangs erfolgreich sind, d. h. daß der Patient nach der Operation keine Schmerzen spürt, kehren die Schmerzen im Lauf der Zeit oft zurück. Zehn Jahre nach der Operation leiden 60% bis 70% der Patienten wieder unter Rückenschmerzen, und 25% bis 30% spüren wieder ein Ischiassyndrom.
- Die Arthrodese (oder Spondylosyndese bzw. Wirbelversteifung, bzw. -fusion) ist eine weitere verbreitete operative Behandlung für Probleme der Bandscheibendegeneration. Die Wirbelfusion kommt zum Einsatz, um die Wirbelsäule daran zu hindern, sich in einem spezifischen Wirbelsäulengebiet zu bewegen, und wird oft im Zusammenhang mit einer Diskektomie durchgeführt. Das Verfahren erhöht die Stabilität der Wirbelsäule eines Patienten um beeinträchtigte Wirbelkörper einer geschädigten oder entfernten Bandscheibe und verhindert weiteren Verschleiß des behandelten Gebiets.
- Unmittelbar nach Arthrodese zeigen zwischen 50% und 94% der Patienten gute bis ausgezeichnete Ergebnisse. Jedoch können Jahre nach der Operation Probleme auftreten, darunter Spinalstenose, vertebrale Osteophytenbildung und Bandscheibendegeneration benachbart zur Behandlungsstelle. Allgemein kehren durch diese Probleme die Schmerzen zurück.
- Daher besteht Bedarf an einem Verfahren und einer Einrichtung zur Behandlung von Bandscheibenverschleiß und verwandten Problemen, die die Langzeitprobleme reduzieren oder eliminieren, die mit derzeit eingesetzten Behandlungen einhergehen, z. B. Diskektomie oder Arthrodese. Dem Fachmann ist klar, daß eine Lösung der zuvor beschriebenen Probleme eine Bandscheibenprothese erfordert. Eine praktikable Bandscheibenprothese kann zu vielfältigen zusätzlichen Nutzeffekten führen, u. a. kürzere Patientengenesungszeiten, geringere Anzahl von Betroffenen mit chronischen Rückenschmerzen und we niger Revisionswirbelfusionen. Allgemein verwendet der Fachmann eines von drei Gestaltungskonzepten für Bandscheibenprothesen, die alle Nachteile haben, die letztlich ihren Erfolg begrenzen. Zu diesen Konzepten zählen Kernersatz, mechanische Bandscheiben und Verbund-(Composit-)Bandscheiben.
- Beim Kernersatz wird ein geschädigter Kern entfernt und ersetzt. Dieser Weg setzt voraus, daß der Ring und die Endplatten nicht beeinträchtigt sind. Folglich ist Kernersatz nur in Frühstadien des Bandscheibenverschleißes von Nutzen, wo minimaler Segmentkollaps aufgetreten ist. Zudem muß beim Kernersatz ein Loch im Ring plaziert werden, um die Kernprothese einzusetzen. Da die Kernprothese danach unbefestigt ist, wird sie mitunter aus der Bandscheibe durch das Loch ausgestoßen. Die US-A-5077701 (2002) offenbart einen geschmeidigen Reifen, der zum Verstärken eines Rings verwendet werden kann, wobei der Reifen einen Anschluß zum Befüllen haben kann.
- Mechanische Bandscheiben und Verbundbandscheiben, das zweite und dritte Konzept für den Bandscheibenersatz, verfügen allgemein über ein Polymerzentrum, das von metallischen Endplatten begrenzt ist, die wachstumsfördernde Oberflächen zur Langzeitfixation an umgebendem Knochen und Zinken oder Stacheln haben, um Kurzzeitfixation daran Rechnung zu tragen. Diese Bandscheiben erfordern ein relativ invasives Operationsverfahren, bei dem ein großer Einschnitt direkt vor dem betroffenen Gebiet (anterior) vorgenommen und ein Weg zwischen dem vorderen Einschnitt und der gewünschten Operationsstelle hergestellt wird. Ferner weisen mechanische und Verbundbandscheiben allgemein parallele Endplatten auf, die nicht natürlichen Endplatten entsprechen, die normalerweise nicht parallele benachbarte Wirbelkörper haben. Diese Nichtübereinstimmung zwischen dem Fachmann bekannten prothetischen Bandscheiben und natürlichen Bandscheiben kann zu Belastungs- und Deformationsproblemen für die Wirbelsäule eines Patienten führen.
- Somit besteht Bedarf an einer künstlichen Bandscheibe und einem Verfahren zu ihrer Verwendung zur Behandlung von Bandscheibenverschleiß und verwandten Problemen, mit dem man die Wirbelsäulenfunktion in allen Stadien des Bandscheibenverschleißes wieder herstellt oder beibehält.
- Weiterhin besteht Bedarf an einer künstlichen Bandscheibe und einem Verfahren zu ihrer Verwendung zur Behandlung von Bandscheibenverschleiß und verwandten Problemen, wobei die Bandscheibe ein prothetisches Analogon für jede Komponente einer natürlichen Bandscheibe aufweist.
- Ferner besteht Bedarf an einer künstlichen Bandscheibe und einem Verfahren zu ihrer Verwendung zur Behandlung von Bandscheibenverschleiß und verwandten Problemen, das sich auf eine Art und Weise verwenden läßt, die für den Patienten minimal invasiv ist.
- Außerdem besteht Bedarf an einer künstlichen Bandscheibe und einem Verfahren zu ihrer Verwendung zur Behandlung von Bandscheibenverschleiß und verwandten Problemen, das eine anatomisch korrektere Bandscheibe nutzt.
- ZUSAMMENFASSUNG
- Bei der Erfindung handelt es sich in einer Form um eine künstliche Bandscheibenprothese mit einem Ring, einem Kern und einem Paar Endplatten. Die Bandscheibenprothese der Erfindung ist expandierbar, so daß sie in einen Patientenkörper minimal invasiv eingesetzt werden kann.
- Allgemein verfügt die Erfindung über einen expandierbaren, allgemein ringförmigen Annulus (Ring), der in expandiertem Zustand die Form, die Funktionseigenschaften und mechanischen Eigenschaften eines natürlichen Bandscheibenrings hat. Der Ring umgibt einen prothetischen Bandscheibenkern. Der Kern der Erfindung weist eine expandierbare Hülle auf, die im expandierten Zustand die Form, die Funktionseigenschaften und mechanischen Eigenschaften eines natürlichen Bandscheibenkerns hat. Schließlich weist die Erfindung ein Paar flexible Bandscheibenendplatten auf, die über und unter dem prothetischen Ring befestigt sind, wodurch der prothetische Kern im Ring eingeschlossen ist. Die Endplatten sorgen für Stabilität der Bandscheibenprothese und bilden eine Einrichtung zur Fixation an Wirbelkörpern, die benachbart zum behandelten Gebiet liegen.
- Ferner wird ein Verfahren zum Einsetzen einer Bandscheibenprothese in einen menschlichen Körper mit Hilfe weniger invasiver Techniken als bekannte chirurgische Verfahren offenbart.
- Ein Vorteil der Erfindung ist, daß die Bandscheibe der Erfindung bei der Behandlung von Bandscheibenverschleiß und verwandten Problemen von Nutzen ist und sie die Wirbelsäulenfunktion in allen Stadien von Bandscheibenverschleiß wiederherstellt oder beibehält.
- Ein weiterer Vorteil der Erfindung ist, daß die Bandscheibe der Erfindung ein prothetisches Analogon für jede Komponente einer natürlichen Bandscheibe aufweist.
- Noch ein weiterer Vorteil der Erfindung ist, daß sie bei der Behandlung von Bandscheibenverschleiß und verwandten Problemen auf eine Art und Weise verwendet werden kann, die für den Patienten minimal invasiv ist.
- Zusätzlich besteht ein Vorteil der Erfindung darin, daß sie eine anatomisch korrektere Bandscheibenprothese nutzt. Diese und weitere Vorteile und Merkmale der Erfindung werden dem Fachmann anhand der beigefügten Ansprüche und Zeichnungen deutlich sein.

- Die zuvor erwähnten und weitere Merkmale und Aufgaben der Erfindung und die Art und Weise ihrer Realisierung sowie ein besseres Verständnis der Erfindung gehen aus der nachfolgenden Beschreibung einer Ausführungsform der Erfindung im Zusammenhang mit den beigefügten Zeichnungen hervor.

Das hierin veranschaulichte Beispiel zeigt nur eine exemplarische Ausführungsform der Erfindung.
- NÄHERE BESCHREIBUNG
- Im Gebrauch hierin gelten die folgenden Richtungsbestimmungen: Anterior bzw. vorn, vorderer usw. und posterior bzw. hinten, hinterer usw. bezeichnen Positionen näher zur Vorderseite bzw. Rückseite des Körpers. Für die hier beschriebene Bandscheibe bezieht sich also anterior oder vorderer usw. auf den Abschnitt der Bandscheibe, der näher zur Vorderseite des Körpers liegt. Proximal und distal bezeichnen Positionen, die näher zur Wurzel der Struktur bzw. weiter davon entfernt liegen. Beispielsweise liegt die distale Wirbelsäule näher zu den Füßen, und die proximale Wirbelsäule liegt näher zum Kopf. Die Adjektive medial und lateral bezeichnen Positionen näher zur Medianebene bzw. weiter entfernt von der Medianebene. Die Medianebene ist eine gedachte vertikale Ebene durch die Mitte des Körpers, die den Körper in eine rechte und eine linke Hälfte aufteilt.
- Die Erfindung betrifft prothetische oder künstliche Bandscheibenimplantate. Die hier beschriebene Erfindung kann verwendet werden, Bandscheiben zu ersetzen, die durch Verletzung oder Erkrankung geschädigt sind. Obwohl die Erfindung hier anhand einer menschlichen Bandscheibe beschrieben wird, wird der Fachmann erkennen, daß die Offenbarung und die beigefügten Ansprüche der Erfindung auch auf jedes Wirbeltier anwendbar sind.

1 und2 zeigen zunächst eine Perspektiv- bzw. Explosionsansicht einer erfindungsgemäßen Bandscheibenprothese. Die Bandscheibe weist eine anatomisch geformte obere Endplatte100a , einen expandierbaren Kern120 , einen komprimierbaren Ring140 und eine untere anatomisch geformte Endplatte100b auf. - In
3 ist eine Perspektivansicht des prothetischen Kerns120 gezeigt. Der Kern120 weist einen flexiblen oder expandierbaren Beutel mit einer Außenfläche und einem hohlen Innenraum auf. Im Gebrauch hierin bezeichnet "flexibel" einen verformbaren Aufbau, der so komprimiert werden kann, daß der Aufbau weniger als den maximalen Volumenbetrag einnimmt, der für den Aufbau theoretisch möglich ist. "Expandierbar" beinhaltet "flexibel", bezeichnet aber auch Aufbauten, deren Außenflächengröße durch Erhöhen des Innendrucks des Aufbaus erheblich vergrößert werden kann, z. B. durch Füllen eines solchen Innenbereichs mit einem gewünschten Fluid. "Auffüllbar" oder "Aufblasbar" bezieht sich auf Aufbauten, die mit einem Fluid gefüllt werden können, bis der flexible oder expandierbare Aufbau ein Endvolumen oder eine Endform annimmt. Der Kern120 ist aus einem biokompatiblen Gewebematerial mit geeigneter Festigkeit und Reißbeständigkeit aufgebaut, um die dynamische Belastung aufzunehmen, die von einer Bandscheibe gefordert wird. Vorzugsweise weist der Kern120 ein gewebtes Polymermaterial auf, z. B. Polyethylen, Polyamid, Polypropylen, Polyester, Polycarbonat, Polysulfon, Polymethylmethacrylat, oder alternativ faserartiges Hydrogel oder Glas. Ferner kann der Kern120 Keramikfasern zur Verstärkung aufweisen. - Gemäß
4 wird der Kern120 auf eine gewünschte Endgröße aufgefüllt oder aufgeblasen, so wie der Begriff hierin verwendet wird. Der Kern120 ist so aufgebaut, daß er im aufgeblasenen Zustand in der Mitte des Rings140 (später beschrieben) und zwischen den Endplatten100a und100b (auch später beschrieben) eingeschlossen sein kann. In seinem aufgefüllten Zustand belegt der Kern120 vorzugsweise im wesentlichen den gesamten Raum in der Mitte des Rings140 und von den Endplatten100a und100b begrenzt. - In
5 ist eine Seitenquerschnittansicht eines aufgeblasenen bzw. aufgefüllten Kerns120 gezeigt. In seinem aufgefüllten Zustand ist der Kern120 mit einem Fluid130 gefüllt, das die mechanischen Eigenschaften eines natürlichen Bandscheibenkerns hat. Zu geeigneten Fluiden zum Füllen oder Aufblasen des Kerns120 zählen Polyethylen, Polyamid, Polypropylen, Polyester, Polycarbonat, Polysulfon, Polymethylmethacrylat, Hydrogel und Silikongummi. Vorzugsweise weist das Fluid ein "Hydrogel" mit den o. g. mechanischen Eigenschaften für natürliche Bandscheibenkerne auf. - Um den Kern
120 aufzublasen, ist ein Zugangsanschluß125 durch die Wand des Kerns120 gemäß5 angeordnet. Der Anschluß125 weist eine Bohrung mit jeder gewünschten Form auf, wobei die Bohrung mit der Außenfläche des Kerns120 und dem Inneren des Kerns120 kommuniziert. Das Fluid130 gemäß5 wird durch den Anschluß125 auf jede gewünschte Weise eingebracht, z. B. durch Einspritzen oder Einpumpen, bis das Fluid130 den gesamten Innenraum des Kerns120 im wesentlichen gefüllt und ihn auf eine gewünschte Endform oder ein gewünschtes Endvolumen geeignet aufgeblasen hat, so daß der Kern120 in einer Bandscheibenprothese verwendbar ist. Vorzugsweise weist der Anschluß125 ein Ein-Richtungs-Ventil auf, so daß das Fluid130 nicht ausströmt. Alternativ verhindert der Ring140 (später beschrieben) jeden Rückfluß von Fluid130 aus dem Kern120 . - In
6 bis9 ist ein flexibler, prothetischer Bandscheibenring140 gemäß einer exemplarischen Ausführungsform der Erfindung gezeigt. Gemäß7 –9 weist der Ring140 im expandierten Zustand eine ringartige Form mit einer Innenfläche auf, die zum Einschließen des Kerns120 geeignet ist. Ferner weist der Ring140 eine Außenfläche und einen Innenraum gemäß9 auf, wodurch ein hohler Abschnitt im Ring140 gebildet ist. - Vorzugsweise kommt die Ringform des Rings
140 der anatomischen Form eines natürlichen Bandscheibenrings gemäß8 nahe. Im abgelassenen Zustand gemäß6 ist der Ring140 flexibler, so daß er komprimiert und durch einen relativ kleinen Einschnitt oder Weg eingesetzt werden kann. - Der Ring
140 ist aus einem biokompatiblen Material mit geeigneter Festigkeit und anderen mechanischen Eigenschaften in seinem aufgefüllten Zustand aufgebaut, um schädlichen Wirbelsäulenbewegungen auf die gleiche Weise wie ein natürlicher Bandscheibenring zu widerstehen. Vorzugsweise weist der Ring140 ein Polymermaterial auf, z. B. Polyethylen, Polyamid, Polypropylen, Polyester, Polycarbonat, Polysulfon, Polymethylmethacrylat, oder alternativ faserartiges Hydrogel oder Glas. Ferner kann der Ring140 Keramikfasern zur Verstärkung aufweisen. - Zusätzlich ist gemäß
1 und2 der Ring140 vorzugsweise durch Weben oder Flechten einer faserartigen Form des Polymermaterials auf eine Weise aufgebaut, die dem Fachmann in der Technik der Herstellung flexibler oder expandierbarer Polymerbehälter bekannt ist. - Gemäß
7 und8 ist der Ring140 flexibel und in eine gewünschte Endgröße und -form auffüllbar, so wie diese Begriffe hier verwendet werden. Der Ring140 ist so aufgebaut, daß er beim Auffüllen eine abgeschrägte Form in medialer oder lateraler Ansicht von der Seite gemäß10 annimmt. - Mit erneutem Bezug auf
1 und2 ist der Ring140 an den flexiblen Endplatten100a und100b (später näher beschrieben) so befestigt dargestellt, daß der Ring140 an den Endplatten100a und100b bei seinem Auffüllen befestigt bleibt. Schließlich bilden die Endplatten100a und100b sowie der Ring140 den halbstarren Außenabschnitt einer Bandscheibenprothese mit dem Kern120 , der in der Mitte des Rings140 und zwischen den Endplatten100a und100b angeordnet ist. - In
9 ist eine Seitenquerschnittansicht des aufgeblasenen Rings140 gezeigt. In seinem aufgefüllten Zustand ist der Ring140 mit einem anfangs flüssigen Material150 gefüllt, das bei seiner Härtung halbstarr wird. Je nach Material kann die Härtung auf jedem technisch bekannten Weg erfolgen, z. B. Einsetzen eines weiteren Materials, Ausüben von Wärme, Einwirkenlassen von Luft oder indem man nur genügend Zeit verstreichen läßt. Nach Härtung sollte das Material die mechanischen Eigenschaften eines natürlichen Bandscheibenrings haben. In einer alternativen Ausführungsform härtet das Fluid150 nicht. In dieser Ausführungsform ist der Ring140 ausreichend durchwirkt, so daß beim Auffüllen mit Fluid150 der Ring140 unter ausreichenden Innendruck gesetzt wird, um die Materialeigenschaften eines natürlichen Bandscheibenrings anzunehmen. Zu geeigneten Fluiden zum Füllen oder Aufblasen des Rings140 zählen Polyethylen, Polyamid, Polypropylen, Polyester, Polycarbonat, Polysulfon, Polymethylmethacrylat, Hydrogel und Silikongummi. Vorzugsweise weist das Fluid ein "Hydrogel" mit den o. g. mechanischen Eigenschaften für natürliche Bandscheibenringe auf. - In
9 ist eine Seitenquerschnittansicht des Rings140 an der Linie A-A in8 gezeigt. Der Ring140 weist ferner einen Zugangsanschluß145 und eine Öffnung146 auf. Um den Ring140 aufzufüllen, ist der Zugangsanschluß145 durch die Wand des Rings140 gemäß9 angeordnet. Der Anschluß145 weist eine Bohrung mit jeder gewünschten Form auf, wobei die Bohrung mit der Außenfläche des Rings140 und dem Inneren des Rings140 kommuniziert. Das Fluid150 gemäß9 wird durch den Anschluß145 auf jede gewünschte Weise eingebracht, z. B. durch Einspritzen oder Einpumpen, bis das Fluid150 den gesamten Innenraum des Rings140 im wesentlichen gefüllt und ihn auf seine gewünschte Endform aufgefüllt hat, d. h. zu einer Ringprothese mit einer anatomischen Form. - Wie zuvor erwähnt, weist der Ring
140 ferner die Öffnung146 auf. Die Öffnung146 ist durch die Wand des Rings140 so angeordnet, daß sich der Zugangsanschluß125 des Kerns120 durch sie erstrecken kann, was24 zeigt. Dadurch können der Kern120 und Ring140 beide nach Bedarf aufgeblasen werden. Dieses Verfahren wird in diesem Abschnitt der Anmeldung später näher beschrieben. - In
2 ist ein Paar prothetische Endplatten100a ,100b (gemeinsam die Endplatte100 ) zum Gebrauch mit der Bandscheibenprothese10 dargestellt.11 zeigt eine Perspektivansicht der Endplatte100 . Die Endplatte100 ist flexibel und weist vorzugsweise gewebte oder geflochtene Metallfasern auf, wobei die Fasern aus der Gruppe ausgewählt sind, die aus Titan, Tantal, Cobalt-Chrom-Legierung, Edelstahl und Nitinol besteht. Alternativ könnte die Endplatte100 ein Polymer- oder Keramikmaterial in einer Form aufweisen, die eine flexible Platte mit mechanischen Eigenschaften bereitstellt, die denen einer natürlichen Bandscheibenendplatte ähneln. - In
12 ist eine übertriebene Seitenansicht der Endplatte100 und benachbarter Spongiosa gezeigt. Während einer Operation zum Bandscheibenersatz entfernt der Arzt die natürliche Bandscheibe des Patienten, entfernt dann operativ einen Teil der Kompakta von den umgebenden Wirbelkörpern und legt dadurch die darunterliegende Spongiosa frei. Die Endplatte100 weist eine Knochenkontaktfläche112 und eine Ringkontaktfläche114 auf. Beim Einsetzen der Bandscheibenprothese10 in einen Wirbelkörperraum während einer Operation zum Bandscheibenersatz wird die Knochenkontaktfläche112 an der freiliegenden Spongiosa benachbarter resezierter Wirbelkörper darüber und darunter angeordnet. - Notwendig ist, daß die Bandscheibenprothese eine gute Initialfixation des Implantats unmittelbar nach Implantation erreicht. Druckkräfte in der Wirbelsäule unterstützen die Fixation, aber es besteht immer noch ein Verschiebungsrisiko der Bandscheibe
10 lateral, medial, posterior oder anterior bei ihrer Implantation. Folglich besteht Bedarf an einer Einrichtung zur kurzfristigen Bandscheibenfixation. - Bei den meisten Gelenkarthroplastiken erreicht man Initialfixation durch die geeignete Plazierung chirurgischer Schrauben, bis eine Langzeitfixation, oft über Knochenwachstum in das Implantat, hergestellt werden kann. Aufgrund von Nerven und anderen anatomischen Hindernissen ist es aber schwierig, operativen Zugang zu einem Intervertebralraum zu erlangen, und Schrauben können Umgebungsnerven oder andere Weichteile schädigen. Folglich weisen die meisten Bandscheibenprothesen eine Gruppe von Stacheln auf, die von der Knochenkontaktfläche der Endplatte vorstehen und in benachbarte Spongiosa eingesetzt werden, um die Initialfixation herzustellen, ein hierin als "Makrofixation" bezeichnetes System. Freilich erfordert die Makrofixation eine ausreichend breite Spreizung des Wirbelkörperraums beim Einsetzen der Prothese, um den Stacheln Rechnung zu tragen. Dieser Vorgang kann zu einem traumatischeren Eingriff für den Patienten führen.
Claims (21)
- Bandscheibenprothese mit einem Ring (
140 ), der einen flexiblen Ring mit gewünschter Größe und Form aufweist, wobei der Ring eine Außenseite und einen hohlen Innenraum aufweist, der Ring ferner eine Öffnung (145 ) aufweist, die Öffnung (145 ) mit der Außenseite und dem Innenraum kommuniziert, und der Innenraum mit einem ersten Fluid (150 ) gefüllt werden kann, so daß der Ring eine Größe und Form annimmt, die für einen Bandscheibenring geeignet ist, dadurch gekennzeichnet, daß der Ring (140 ) einen Kern (120 ) umgibt, der aufweist: eine flexible, expandierbare Hülle mit einem hohlen Innenraum, wobei der Innenraum mit einem zweiten Fluid (130 ) auf eine anpassungsfähige Endgröße und -form aufgefüllt werden kann, wobei die Form und Größe für einen Bandscheibenkern geeignet ist. - Bandscheibenprothese nach Anspruch 1, dadurch gekennzeichnet, daß der Ring ein geflochtenes Fasermaterial aufweist.
- Bandscheibenprothese nach Anspruch 1 oder 2, dadurch gekennzeichnet, daß der Ring die mechanischen Eigenschaften eines natürlichen Rings aufweist.
- Bandscheibenprothese nach Anspruch 1, dadurch gekennzeichnet, daß der Ring eine asymmetrische Form annimmt.
- Bandscheibenprothese nach Anspruch 1, dadurch gekennzeichnet, daß der Ring ein Material aufweist, das aus der Gruppe ausgewählt ist, die aus Polyethylen, Polyamid, Polypropylen, Polyester, Polycarbonat, Polysulfon, Polymethylmethacrylat, Hydrogel und Glas besteht.
- Bandscheibenprothese nach Anspruch 1, dadurch gekennzeichnet, daß das erste Fluid (
150 ) ein Material aufweist, das die mechanischen Eigenschaften eines natürlichen Rings hat. - Bandscheibenprothese nach Anspruch 1, dadurch gekennzeichnet, daß das erste Fluid (
150 ) ein Material aufweist, das aus der Gruppe ausgewählt ist, die aus Polyethylen, Polyamid, Polypropylen, Polyester, Polycarbonat, Polysulfon, Polymethylmethacrylat, Hydrogel und Silikongummi besteht. - Bandscheibenprothese nach Anspruch 1, dadurch gekennzeichnet, daß der Kern ein flexibles, biokompatibles Material mit den mechanischen Eigenschaften eines natürlichen Kerns aufweist.
- Bandscheibenprothese nach Anspruch 1, dadurch gekennzeichnet, daß der Kern ein Material aufweist, das aus der Gruppe ausgewählt ist, die aus Polyethylen, Polyamid, Polypropylen, Polyester, Polycarbonat, Polysulfon, Polymethylmethacrylat und Hydrogel besteht.
- Bandscheibenprothese nach Anspruch 1, dadurch gekennzeichnet, daß der Kern auf eine asymmetrische Form expandierbar ist.
- Bandscheibenprothese nach Anspruch 1, dadurch gekennzeichnet, daß das zweite Fluid (
130 ) ein Material mit den mechanischen Eigenschaften eines natürlichen Kerns aufweist. - Bandscheibenprothese nach Anspruch 1, dadurch gekennzeichnet, daß das zweite Fluid (
130 ) ein Material aufweist, das aus der Gruppe ausgewählt ist, die aus Polyethylen, Polyamid, Polypropylen, Polyester, Polycarbo nat, Polysulfon, Polymethylmethacrylat, Hydrogel und Silikongummi besteht. - Bandscheibenprothese nach einem der vorstehenden Ansprüche, dadurch gekennzeichnet, daß sie ferner aufweist: ein Paar Bandscheibenprothesen-Endplatten (
100a ,100b ), wobei jede Endplatte (100a ,100b ) des Paars Endplatten flexibel ist und eine relativ flache Form hat, jede Endplatte (100a ,100b ) eine geeignete Größe und Form für eine Bandscheibe hat, jede Endplatte eine Knochenkontaktfläche (112 ) und eine Ringkontaktfläche (114 ) aufweist, und dadurch, daß die Knochenkontaktfläche (112 ) gegenüber Knochenwachstum darin zur Langzeitfixation an benachbartem natürlichen Knochen empfänglich ist, und dadurch, daß die Knochenkontaktfläche (112 ) ferner eine Einrichtung (110 ) zur Initialkurzzeitfixation an benachbartem Knochen in einem gewünschten Wirbelkörperraum aufweist; wobei der Ring (140 ) an der Ringkontaktfläche (114 ) jeder Endplatte des Paars Endplatten so angebracht ist, daß der Kern (120 ) vom Ring (140 ) umgeben ist und zwischen dem Paar Endplatten (100a ,100b ) liegt. - Bandscheibenprothese nach einem der vorstehenden Ansprüche, dadurch gekennzeichnet, daß der Kern (
120 ) am Ring (140 ) angebracht ist. - Bandscheibenprothese nach Anspruch 13, dadurch gekennzeichnet, daß der Kern (
120 ) an jeder Endplatte (100a ,100b ) des Paars Endplatten angebracht ist. - Bandscheibenprothese nach Anspruch 1, dadurch gekennzeichnet, daß der Kern (
120 ) ein Material aufweist, das aus der Gruppe ausgewählt ist, die aus Polyethylen, Polyamid, Polypropylen, Polyester, Polycarbonat, Polysulfon, Polymethylmethacrylat, Hydrogel und Glas besteht. - Bandscheibenprothese nach Anspruch 13, dadurch gekennzeichnet, daß das Metall aus der Gruppe ausgewählt ist, die aus Titan, Tantal, Edelstahl, Nitinol und Cobalt-Chrom-Legierung besteht.
- Bandscheibenprothese nach Anspruch 13, dadurch gekennzeichnet, daß die Endplatten (
100a ,100b ) gewebte biokompatible Metallfasern aufweisen. - Bandscheibenprothese nach Anspruch 13, dadurch gekennzeichnet, daß die Einrichtung (
110 ) zur Initialkurzzeitfixation an benachbartem Knochen mehrere Vorsprünge aufweist, die sich von der Knochenkontaktfläche (112 ) der Endplatte (100 ) erstrecken, wobei die Vorsprünge (110 ) ausreichend klein sind, um sich in die Poren benachbarter Spongiosa einzupassen, wodurch sie für Initialfixation der Bandscheibe von anterior nach posterior und von lateral nach medial sorgen. - Bandscheibenprothese nach Anspruch 13, dadurch gekennzeichnet, daß die Knochenkontaktfläche (
