Abstract
【解決手段】 人工椎間板10は、流体を充填された可撓性髄核部120と、可撓性を有した繊維織布製の輪部140と、一対の金属織布製の終板100a、100bとを備える。終板100a、100bは、隣接する骨への初期固定のための巨視的固定手段と、内部への骨の成長を許容する骨成長表面とを備える。人工椎間板10は概ね解剖学的に適した構造及び形状を有している。

Claims (43)
- 所望されるサイズ及び形状の可撓性リングからなり、該リングが、体積可変の空間を占有し得るように圧縮可能であり、外部と、中空内部と、該外部と該内部との間を連通させる開口部とを含み、前記内部は、前記リングが椎間板輪部に適したサイズ及び形状に拡張するように硬化可能な流体を用いて膨張可能になっている人工椎間板輪部と、
中空内部を有した可撓性のシェルからなる人工椎間板髄核部であって、該人工椎間板髄核部が体積可変の空間を占有し得るように圧縮可能であり、前記内部が椎間板髄核部に適した最終形状になるように流体を用いて膨張可能になっている人工椎間板髄核部と、
各々が可撓性を有し且つ比較的平坦な形状を有した一対の人工椎間板終板であって、該人工椎間板終板の各々が、椎間板に適したサイズ及び形状を有し、骨接触表面と、輪部接触表面とを備え、該骨接触表面が、隣接する自家骨への長期固定のために内部への骨成長を許容するようになっており、所望される椎骨空間における隣接する骨への初期の短期固定のための手段をさらに備えている人工椎間板終板と、
を備え、前記人工椎間板輪部は、該人工椎間板髄核部が前記一対の人工椎間板終板の間において前記人工椎間板輪部によって周囲を取り囲まれるように前記一対の人工椎間板終板の各々の輪部接触表面に結合されることを特徴とする人工椎間板。 - 前記人工椎間板髄核部は前記人工椎間板輪部に結合される、請求項1に記載の人工椎間板。
- 前記人工椎間板髄核部は前記一対の人工椎間板終板の各々に結合される、請求項1に記載の人工椎間板。
- 前記リングは編組繊維材料からなる、請求項1に記載の人工椎間板。
- 前記リングは椎間板線維輪の材料特性を有する材料からなる、請求項1に記載の人工椎間板。

- 前記人工椎間板髄核部は、ポリエチレン、ポリアミド、ポリプロピレン、ポリエステル、ポリカーボネート、ポリスルホン、ポリメチルメチルアクリレート、ヒドロゲル及びガラスからなる群から選択された材料からなる、請求項1に記載の人工椎間板。

- 前記隣接する骨への初期の短期固定のための手段は、前記人工椎間板終板の骨接触表面から延びる複数の突起からなり、該突起は隣接する海綿骨の細孔内に入るに十分に小さく、初期における前記人工椎間板の前後方向及び内外方向の固定を提供する、請求項1に記載の人工椎間板。
- 所望されるサイズ及び形状の可撓性リングからなり、該リングが、外部と、中空内部と、該外部と該内部との間を連通させる開口部とを含み、前記内部は、前記リングが椎間板輪部に適したサイズ及び形状をとるように流体を充填可能となっていることを特徴とする人工椎間板輪部。
- 前記リングが編組繊維材料からなる、請求項18に記載の人工椎間板輪部。
- 前記リングが自家椎間板線維輪の材料特性を有している、請求項18又は請求項19に記載の人工椎間板輪部。
- 前記リングは非対称形状をとる、請求項18に記載の人工椎間板輪部。

- 中空内部を有した拡張可能な可撓性のシェルからなり、該内部が流体を用いて椎間板髄核部に適した最終サイズ及び形状に膨張可能であることを特徴とする人工椎間板髄核部。
- 前記人工椎間板髄核部は自家椎間板髄核の材料特性を有した可撓性且つ生体適合性の材料からなる、請求項25に記載の人工椎間板髄核部。
- 前記人工椎間板髄核部は、ポリエチレン、ポリアミド、ポリプロピレン、ポリエステル、ポリカーボネート、ポリスルホン、ポリメチルメチルアクリレート及びヒドロゲルからなる群から選択された材料からなる、請求項25に記載の人工椎間板髄核部。
- 前記人工椎間板髄核部は非対称形状に拡張可能である、請求項25に記載の人工椎間板髄核部。
- 前記流体は自家椎間板髄核の材料特性を有した材料からなる、請求項25に記載の人工椎間板髄核部。

- 可撓性を有し且つ比較的平坦な形状を有した人工椎間板終板であって、該人工椎間板終板が椎間板に適したサイズ及び形状を有し且つ骨接触表面を備え、該骨接触表面は、隣接する自家骨への長期固定のために内部への骨の成長を許容するようになっており、所望される椎骨空間における隣接する骨への初期の短期固定のための手段をさらに備えることを特徴とする人工椎間板終板。
- 金属は、チタン、タンタル、ステンレス鋼、ニチノール及びコバルトクロム合金からなる群から選択される、請求項31に記載の人工椎間板終板。
- 前記人工椎間板終板は、生体適合性金属繊維織布からなる、請求項31に記載の人工椎間板終板。
- 前記隣接する骨への初期の短期固定のための手段は前記人工椎間板終板の骨接触表面から延びる複数の突起からなり、該突起は、隣接する海綿骨の細孔に入るに十分に小さく、前記人工椎間板の前後方向及び内外方向の固定を提供する、請求項31に記載の人工椎間板終板。
- ヒトの表皮に切開部を形成するステップと、
前記切開部から自家椎間板まで外科的に経路を形成するステップと、
前記自家椎間板を除去して、椎骨が隣接した椎骨空間を形成するステップと、
海綿骨が露出するように前記椎骨の一部を切除するステップと、
人工椎間板を挿入するステップと、
所望の最終形状に前記人工椎間板を膨張させるステップと、
を含むことを特徴とする人工椎間板を移植する方法。 - 前記切開部の寸法は約2cmから4cmである、請求項35に記載の人工椎間板を移植する方法。
- 前記人工椎間板は、拡張可能な髄核部と、拡張可能な輪部と、一対の可撓性終板とを備える、請求項35に記載の人工椎間板を移植する方法。
- 前記人工椎間板を膨張させるステップは、前記髄核部に第1の流体を充填し、前記輪部に第2の流体を充填することを含む、請求項35に記載の人工椎間板を移植する方法。
- 前記第1の流体及び前記第2の流体は同じものである、請求項38に記載の人工椎間板を移植する方法。
- 前記第1の流体及び前記第2の流体は、ポリウレタン、ヒドロゲル、ポリスルホン、ポリアリールエーテルケトン、ポリカーボネート、エラストマ及びシリコーンゴムからなる群から選択された材料を含む、請求項38に記載の人工椎間板を移植する方法。
