Description
- Die vorliegende Erfindung betrifft Verfahren zur Herstellung eines chirurgischen Implantates. Der am nächsten kommende bisherige Stand der Technik ist das
US-A-3855638 . - Es gibt viele verschiedene Arten von chirurgischen Implantaten und Vorrichtungen, einschließlich orthopädischen Implantaten (Hüfte, Schulter, Knie, Knöchel, Ellbogen), Schädel-Gesichts-Implantate und Wirbelsäulenimplantate (wie beispielsweise ein spinales Körbchen oder eine spinale Bandscheibe). Derartige Implantate werden im Allgemeinen aus biokompatiblen Metallen hergestellt, wie beispielsweise Titan, auf Titan basierenden Legierungen, nichtrostender Stahl oder Kobalt-Chrom-Legierungen, in Abhängigkeit von den Festigkeits- und Gewichtsüberlegungen Eine weitere Überlegung, insbesondere bei Implantaten, die Knochen ersetzen, ist der Elastizitätsmodul des Implantates. Je näher der Elastizitätsmodul dem des natürlichen Knochens kommt, desto besser ist die Spannungsneuverteilung auf den benachbarten Knochen. Eine bessere Spannungsverteilung führt zu einer verlängerten Nutzlebensdauer des Implantates.
- Um die Bindung der umgebenden Gewebe mit einem Implantat zu verstärken, ist die Aufbringung einer Beschichtung auf die Oberfläche des Implantates bekannt, beispielsweise durch Plasmaspritzen, das die Oberfläche des Implantates aufraut. Die Beschichtung kann von unterschiedlicher Zusammensetzung zum Implantat sein, um die Abnutzungsbeständigkeit zu verbessern, und/oder um eine verbesserte Biokompatibilität bereitzustellen (beispielsweise ist TON extrem inert und kann als eine Beschichtung auf aluminium- oder vanadiumhaltige Implantate aufgebracht werden, um ein Auslaugen jener Metalle aus dem Implantat zu verhindern.
- Es ist ein Ziel der vorliegenden Erfindung, ein verbessertes chirurgisches Implantat bereitzustellen, das vorzugsweise eine verbesserte Knochenkompatibilität und/oder Abnutzungsbeständigkeit und/oder Nutzlebensdauer aufweist und/oder fester ist als die bekannten Implantate.
- Entsprechend der vorliegenden Erfindung wird ein Verfahren zur Herstellung eines chirurgischen Implantates bereitgestellt, das einen Kernbereich und einen porösen Oberflächenbereich aufweist, der sich über mindestens einen Teil des Kernbereiches erstreckt, wobei der poröse Oberflächenbereich einen vorgegebenen Porenvolumenanteil aufweist, wobei das Verfahren die folgenden Schritte aufweist: (i) Zuführen von Metallpulver mit einer vorgegebenen Teilchengrößenverteilung um einen vorgeformten Kern in einer abdichtbaren Kapsel; (ii) Verringern des Druckes innerhalb der Kapsel auf einen vorgegebenen Druck unterhalb des atmosphärischen Druckes; (iii) Unterdrucksetzen der Kapsel mit einem Prozessgas auf einen vorgegebenen Druck höher als der vorgegebene Druck des Schrittes (ii); (iv) Abdichten der Kapsel; (v) Erwärmen der unter Druck gesetzten abgedichteten Kapsel auf eine erhöhte Temperatur und einen erhöhten Druck über ehre vorgegebene Zeit, um ein Implantatvorprodukt herzustellen; (vi) Abkühlen der abgedichteten Kapsel; und (vii) Erwärmen des Implantatvorproduktes über eine vorgegebene Zeit bei einer erhöhten Temperatur und einem vorgegebenen Druck unterhalb des atmosphärischen Druckes, um dadurch eine Porosität im Implantatvorprodukt zu erzeugen.
- Das Verfahren ermöglicht die Bildung eines chirurgischen Implantates, das einen Kernbereich und einen porösen Oberflächenbereich aufweist, der sich über mindestens einen Teil des Kernbereiches erstreckt, und wobei der poröse Oberflächenbereich einen vorgegebenen Porenvolumenanteil aufweist.
- Es wird verstanden werden, dass, wie er hierin verwendet wird, der „Porenvolumenanteil" eines porösen Bereiches ein Verhältnis des Porenvolumens in jenem Bereich zum Gesamtvolumen jenes Bereiches ist, ausgedrückt als Prozentwert.
- Das Implantat kann irgendeine Form aufweisen, wie sie durch seine beabsichtigte Anwendung erforderlich ist. Beispielsweise kann das Implantat länglich (z.B. zylindrisch) oder scheibenförmig sein. Das Implantat kann eine unregelmäßige Form aufweisen, und der poröse Oberflächenbereich kann in der Dicke über den Kernbereich variieren.
- Vorzugsweise beträgt der Porenvolumenanteil innerhalb des porösen Oberflächenbereiches 20 bis 50 %. Der vorgegebene Porenvolumenanteil wird entsprechend der Beschaffenheit des Implantes ausgewählt. Ein höherer Porenvolumenanteil führt zu einem leichteren Implantat mit einem niedrigeren Elastizitätsmodul.
- Vorzugsweise werden die Poren miteinander verbunden und innerhalb des porösen Oberflächenbereiches im Wesentlichen gleichmäßig verteilt. Vorzugsweise weisen mindestens einige (und vorzugsweise die Mehrheit) der Poren eine Größe im Bereich von 100 μm bis etwa 750 μm auf und mehr bevorzugt von etwa 200 μm bis 500 μm.
- Vorzugsweise ist der poröse Oberflächenbereich mindestens etwa 1 mm dick, und er ist mehr bevorzugt etwa 2 mm bis etwa 5 mm dick, am meisten bevorzugt 2 mm bis 3 mm dick.
- Einzelne Bereiche innerhalb des porösen Oberflächenbereiches können eine unterschiedliche Porengrößenverteilung und/oder einen unterschiedlichen Porenvolumenanteil aufweisen, so dass ein Porengrößen- und/oder Porenvolumenanteilgradient innerhalb des porösen Oberflächenbereiches vorhanden ist. Beispielsweise kann ein Porenvolumenanteilgradient längs einer Achse des Implantates und/oder senkrecht zur Achse des Implantates vorhanden sein.
- Der Kernbereich kann vollständig dicht oder porös sein. Der Grad der Porosität des Kernbereiches kann größer als, kleiner als oder der gleiche wie der Oberflächenbereich sein.
- Vorzugsweise ist der Kernbereich relativ weniger porös als der Oberflächenbereich.
- Vorzugsweise weist der Kernbereich eine Dichte von 0,7 bis 1,0 der theoretischen Dichte auf (d.h., von 0 bis 30 % Porosität).
- Der Kernbereich und der poröse Oberflächenbereich können aus den gleichen oder unterschiedlichen Materialien konstruiert werden. Besonders geeignete Materialien umfassen Titan (beispielsweise Titan von technischer Reinheit [ASTM B 338 GR 2]), nichtrostenden Stahl, Legierungen auf Titanbasis (beispielsweise Ti-Al-V-Legierungen und Ti-Al-Nb-Legierungen) und Legierungen auf Kobalt-Chrom-Basis.
- Besonders bevorzugte Materialien sind Titan von technischer Reinheit, Ti-6Al-4V, Ti-6Al-7Nb, Stellit 21 und nichtrostender Stahl 316L.
- Vorzugsweise wird der poröse Oberflächenbereich am Kernbereich diffusionsgebunden. Daher wird man verstehen, dass die Grenzfläche zwischen dem Kernbereich und dem porösen Oberflächenbereich nicht eine geringe Festigkeit in das Implantat einbringt.
- Vorzugsweise weist das Pulver, das beim Schritt (i) verwendet wird, einen Teilchengrößenbereich von etwa 50 μm bis etwa 750 μm auf (d.h., im Wesentlichen alle Teilchen liegen innerhalb des spezifizierten Größenbereiches). Das Pulver kann beispielsweise durch Gaszerstäubung oder mechanischen Abbrieb hergestellt werden.
- Zweckmäßigerweise kann der Kern mit der Kapsel zusammenhängend sein, obgleich der Kern freistehend sein kann. In beiden Fällen kann der Kern aus dem gleichen Material wie oder einem anderen Material als das Pulver bestehen und aus dem gleichen Material oder einem anderen Material wie der Rest der Kapsel.
- Vorzugsweise betragt der vorgegebene Druck des Schrittes (ii) 10–3 mBar (0,1 Pa) oder weniger. Vorzugsweise beträgt der vorgegebene Druck des Schrittes (vii) 10–3 mBar (0,1 Pa) oder weniger.
- Das Prozessgas des Schrittes (iii) kann ein nichtreaktionsfähiges Gas (beispielsweise Argon) oder ein reaktionsfähiges Gas (beispielsweise Stickstoff) oder eine Mischung des reaktionsfähigen und nichtreaktionsfähigen Gases sein, wobei das nichtreaktionsfähige Gas als ein Träger oder Verdünnungsmittel für das reaktionsfähige Gas dient Wenn Stickstoff im Prozessgas vorhanden ist, werden die Oberflächen der eventuell gebildeten Poren nitriert, wodurch die Abnutzungsbeständigkeit und die chemische Reaktionsträgheit erhöht werden. Die Verwendung von Stickstoff ist besonders vorteilhaft, wenn das Metallpulver Titan enthält. Als eine Alternative kann ein separater Nitrierschritt nach dem Schritt (vii) durchgeführt werden, vorzugsweise mit einer Temperatur von etwa 800°C bis etwa 1000°C und mit einem Druck von etwa 10 bis 100 MPa über 1 bis 8 Stunden.
- Vorzugsweise beträgt der Druck des Schrittes (iii) von etwa 1 Bar (1 × 105 Pa) bis etwa 5 Bar (5 × 105 Pa) Überdruck.
- Vorzugsweise beträgt die erhöhte Temperatur des Schrittes (v) von etwa 850 bis etwa 1100°C. Vorzugsweise wird der Schritt (v) über eine Zeitdauer von etwa 1 Stunde bis etwa 4 Stunden durchgeführt.
- Beim Schritt (vi) wird die abgedichtete Kapsel (und das Implantatvorprodukt darin) vorzugsweise auf Raumtemperatur abgekühlt. Der Schritt (vii) kann beim Implantatvorprodukt in der Kapsel durchgeführt werden. Alternativ kann das Implantatvorprodukt von der Kapsel nach dem Schritt (vi) vor dem Schritt (vii) getrennt werden. Die Kapsel wird konventionell durch beispielsweise deren Abarbeiten auf einer Drehmaschine entfernt. Vorzugsweise beträgt die erhöhte Temperatur des Schrittes (vii) von etwa 900 bis etwa 1300°C. Vorzugsweise wird der Schritt (vii) über nicht mehr als etwa 80 Stunden durchgeführt, vorzugsweise nicht mehr als etwa 60 Stunden und noch mehr bevorzugt nicht mehr als etwa 48 Stunden. Vorzugsweise wird der Schritt (vii) über mindestens 6 Stunden und mehr bevorzugt über mindestens 12 Stunden durchgeführt.
- Wenn der Schritt (vii) mit dem Implantatvorprodukt in der Kapsel durchgeführt wird, wird die Kapsel vom Implantat nach dem Schritt (vii) durch beispielsweise Bearbeitung getrennt.
- Es wird verstanden werden, dass das nach dem vorangehend angeführten Verfahren hergestellte Implantat die Form der Innenflächen der Kapsel aufweisen wird, so dass zusätzliche Bearbeitungsschritte erforderlich sein können, um die endgültige geforderte Form für das Implantat zu erhalten. Ein Implantat mit einer Form im Wesentlichen so, wie sie gefordert wird, kann jedoch hergestellt werden, indem eine Kapsel mit einer geeignet geformten Innenfläche verwendet wird. Beispiele für Verfahren, mittels denen eine geeignet geformte Kapsel hergestellt werden kann, umfassen das Galvanoformen und die direkte Laserherstellung.
- Bei einer geringfügigen Abwandlung des vorangehend angeführten Verfahrens wird das Metallpulver vor der Einkapselung teilweise verdichtet. Das kann beispielsweise durch selektives Lasersintern einer Mischung des Metallpulvers und eines polymeren Bindemittels bewirkt werden.
- Verschiedene Pulverfraktionen können verwendet werden, um verschiedene Bereiche des Implantates zu bilden, und so ist diese Abwandlung besonders nützlich für die Herstellung von Implantaten, die einen Porositätsgradienten innerhalb des porösen Bereiches aufweisen.
- Ein weiteres Verfahren zur Herstellung eines chirurgischen Implantates des ersten Aspektes kann die folgenden Schritte aufweisen:
- (i) selektives Sintern aufeinanderfolgender Schichten von Metallteilchen, um dadurch ein Implantatvorprodukt oder eine erforderliche Form zu bilden; und
- (ii) Erwärmen des Implantatvorproduktes, um dadurch das Implantat zu bilden.
- Vorzugsweise wird der Schritt (ii) unter verringertem Druck durchgeführt, beispielsweise in einem Vakuumofen.
- Der Schritt (i) kann durch Sintern der Teilchen um einen Kern bewirkt werden. Vorzugsweise zeigt der Kern eine theoretische Dichte von 0,7 bis 1,0 (d.h., eine Porosität von 0 bis 30 %). Der Kern kann jedoch die gleiche, eine höhere oder niedrigere Dichte als der resultierende poröse Oberflächenbereich aufweisen.
- Vorzugsweise weisen im Wesentlichen alle Metallteilchen eine Größe von 750 μm oder kleiner auf, mehr bevorzugt 500 μm oder kleiner und am meisten bevorzugt 150 μm oder kleiner. Vorzugsweise weisen die Teilchen eine Größe von mindestens 50 μm auf. Vorzugsweise sind die Metallteilchen in einer Beimischung mit einem Bindemittel, wie beispielsweise einem leichten und/oder wärmeempfindlichen polymeren Bindemittel. Mehr bevorzugt wird der Schritt (i) durch Abtasten eines Lasers (typischerweise mit einer Leistung von 20 bis 40 W) über die Mischung des Metallpulvers und des Bindemittels bewirkt.
- Vorzugsweise wird der Schritt (ii) von etwa 1000°C bis etwa 1300°C durchgeführt. Es wird verstanden werden, dass beim Schritt (ii) jegliches vorhandenes Bindemittel weggebrannt wird.
- Es wird verstanden werden, dass der Schritt (i) durchgeführt werden kann, um Bereiche einer Großporigkeit zu erhalten. Beispielsweise kann der lasergesinterte Bereich mit Vertiefungen, die sich nach innen von der Oberfläche des Implantates erstrecken, und/oder Kammern innerhalb des Implantates versehen werden. Außerdem können derartige Kammern und/oder Vertiefungen miteinander verbunden werden, um Kanäle oder Durchgänge im Implantat zu liefern. Es wird ebenfalls verstanden werden, dass eine derartige Großporigkeit in das Implantat eingeführt werden kann, das nach dem modifizierten Verfahren des ersten Aspektes hergestellt wird, das das selektive Lasersintern anwendet.

- Ein 100 mm langer zylindrischer Behälter aus Titan von technischer Reinheit (ASTM B 338 GR 2) mit einem Innendurchmesser von etwa 22 mm und einem Außendurchmesser von 25,4 mm wurde mit einem gaszerstäubten Pulver gefüllt, das aus Teilchen der Ti-6Al-4V-Legierung (Crucible Research, USA) besteht, die eine vorgegebene Teilchengrößenverteilung aufweisen. Wo es erforderlich ist, wurde das Pulver durch Siebe mit geeigneter Maschenweite geführt, um eine spezielle Teilchengrößenfraktion zu erhalten. Der Behälter wurde ohne Schwingung gefüllt, d.h., nur „Rütteldichte". Die Luft wurde am dem Behälter evakuiert (Druck < 0,1 Pa) und mit Argon (105 Pa Überdruck) über 2 Minuten wieder gefüllt. Nach dem Abdichten wurde der Behälter über 4 Stunden bei 950°C mit einem Außendruck von 100 MPa isostatisch heißgepresst. Anschließend durfte sich der Behälter auf Raumtemperatur und atmosphärischen Druck abkühlen, wobei Ar unter Druck im Legierungsgrundgefüge verbleibt. Das Implantatvorprodukt wurde aus dem Behälter maschinell bearbeitet und bei 1050°C über 48 Stunden unter Vakuum (< 0,1 Pa) wärmebehandelt.
- Der Grad der Porosität des resultierenden Implantates wird für verschiedene Pulverfraktionen nachfolgend in der Tabelle 1 angegeben. TABELLE 1

Das isostatische Heißpressen wurde bei 1000°C durchgeführt, und die anschließende Wärmebehandlung (nach dem Entfernen aus dem Behälter) wurde bei 1050°C über 48 Stunden vorgenommen. - Das Beispiel 1 wurde mit verschiedenen Pulvern wiederholt. Nach der Wärmebehandlung (1100°C über 48 Stunden) wurden die Implantate unter Druck in reinem Stickstoff bei 950°C und 50 MPa über 6 Stunden nitriert, um die Innenfläche der Poren mit einer dünnen Schicht Titannitrid zu beschichten. - Ein 12,7 mm voller zylindrischer Kern aus Ti-6Al-4V wurde mittig in dem Titanbehälter angeordnet, wie vorangehend für das Beispiel 1 beschrieben. Der Zwischenraum zwischen dem Kern und der inneren Zylinderwand wurde mit Ti-6Al-4V-Pulver (Teilchengröße 500 bis 710 μm) gefüllt. Der Behälter wurde luftleer gemacht und mit Ar bis zu einem Überdruck von 105 Pa wieder gefüllt, abgedichtet und bei 950°C über 4 Stunden bei 100 MPa isostatisch heißgepresst. Die anschließende Wärmebehandlung nach dem Entfernen aus dem Behälter erfolgte bei 1050°C über 48 Stunden. Das führte zu einer Probe mit einer porösen Beschichtung mit einer Porosität von 29 ± 5 % mit 43 % Poren von > 50 μm, 16 % > 100 μm und 6 % > 150 μm.
- BEISPIEL 8
- Das Beispiel 1 wurde bei Verwendung eines Pulvers mit einer Teilchengröße < 125 μm wiederholt. Nach dem isostatischen Heißpressen wird das Teil bearbeitet, um den Behälter zu entfernen, und um den Durchmesser des Teils auf ~ 17 mm zu verringern. Anders als beim Beispiel 1 wurde in diesem Stadium keine Wärmebehandlung durchgeführt. Der resultierende Zylinder wurde danach als der Kern in der gleichen Weise wie beim Beispiel 7 verwendet, wobei der Behälter einen Außendurchmesser von 38,1 mm und einen Innendurchmesser von annähernd 35 mm aufweist. Das Beispiel 7 wurde danach wiederholt, wobei der Kern in diesem Fall mit Pulver der Größe 250 bis 425 μm umgeben wurde. Das resultierende Implantat zeigte einen porösen Kern mit einer mittleren Porengröße von 26 μm (Porosität 28,9 ± 2,1 %) und eine poröse Außenschicht mit einer mittleren Porengröße von 113 μm (Porosität 24,9 ± 1,9 %).
- Das Beispiel 8 zeigt die Fähigkeit des Verfahrens, Implantate mit vorgegebenen Bereichen von unterschiedlicher Porosität in einer steuerbaren Weise herzustellen. Außerdem wird verstanden werden, dass die Verfahrensweise erweitert werden kann, um ein Implantat mit mehr als zwei unterschiedlichen Schichten von unterschiedlicher Porosität herzustellen, d.h., abgestufte poröse Strukturen können hergestellt werden.
- BEISPIEL 9
- Das Beispiel 1 wurde bei Verwendung eines Pulvers mit einer vorgegebenen Größenverteilung wiederholt. Nach der Wärmebehandlung (1100°C über 48 Stunden) wurden Proben zu Zugfestigkeitsprüflingen bearbeitet, so dass die Eigenschaften des porösen Materials bewertet werden konnten. Die Ergebnisse werden in der Tabelle 7 gezeigt. TABELLE 7

- Um zu zeigen, dass bei den Beispielen, wo ein vorgeformter Kern mit einem porösen Oberflächenbereich verbunden wird, eine starke Bindung an der Grenzfläche zwischen dem Kern und den Oberflächenbereichen vorhanden ist, wurden Zugfestigkeitsversuche durchgeführt. Bei diesen Versuchen wurde das Pulver (Ti-6Al-4V) gleichzeitig verdichtet und mit einem vollen Ti-6Al-4V-Stab so diffusionsgebunden, dass die Grenzfläche im Mittelpunkt der Messlänge der Probe zu finden war. Zwei Pulverfraktionen wurden bewertet, 125 bis 150 μm (Versuchsprobe a) und 150 bis 250 μm (Versuchsprobe b). In jedem Fall wurde der Probenbehälter mit Ar bei einem Überdruck von 105 Pa wieder gefüllt. Die anfängliche Stufe des isostatischen Heißpressens wurde bei 950°C über 4 Stunden bei 100 MPa durchgeführt. Die anschließende Wärmebehandlungsstufe wurde bei 1100°C über 48 Stunden durchgeführt. Die resultierende Porosität betrug etwa 40 % (Versuchsprobe a) und 35 % (Versuchsprobe b).
- In beiden Fallen betrug die Zugreißfestigkeit etwa 400 MPa, und ein Bruch erfolgte entfernt von der Grenzfläche innerhalb der Masse des porösen Materials. Routinemäßige metallografische Untersuchungen bestätigten, dass die Grenzfläche unbeeinflusst blieb, und dass die Grenzfläche frei von einer Verunreinigung (d.h., Oxiden) und von dem benachbarten Grundgefüge nichtunterscheidbar war.
- BEISPIEL 11
- Mit Bezugnahme auf
1 wurden die gezeigten Teile durch selektives Lasersintern hergestellt. Der Laser wird durch einen Computer gesteuert, der mit einer CAD-Datei geladen wird, die die Konfiguration des Teils enthält. Das Teil wurde auf einem Pulverbett hergestellt, das auf 160°C (typische Temperatur 100 bis 200°C) erwärmt wurde. Pulver mit einer Teilchengröße von < 150 μm und 15 Vol-% Bindemittel (typischerweise 5 bis 25 Vol-%) wurde auf 60°C (typischerweise 50 bis 100°C) erwärmt und auf das Pulverbett mit einer Aufbauhöhe von 200 μm pro Schicht (typischerweise 100 bis 500 μm gebracht. Das selektive Sintern wurde bei Verwendung eines 25 W Lasers bewirkt. Nachdem das Teil aufgebaut wurde, wurde bei 400°C über 30 Minuten (typischerweise 200 bis 500°C über 30 bis 120 Minuten) ein Entbinden bewirkt, wonach das Sintern bei 1100°C (typischerweise 1000 bis 1350°C) bewirkt wurde. - Wie in
1 gesehen werden kann, weist das Teil2 einen im Allgemeinen porösen zylindrischen Bereich (Porosität von etwa 20 bis 25 %) mit einer regelmäßigen Anordnung von runden Vertiefungen6 um seinen äußeren Umfang herum („Großporigkeit”) auf. Die Vertiefungen6 zeigen einen Umfang von 0,75 mm und erstrecken sich um 3 mm nach innen. Das Teil12 ist gleich dem Teil2 , außer dass die Vertiefungen6 einen Durchmesser von 2 mm aufweisen. - Das Teil
22 ist ebenfalls gleich dem Teil2 , außer dass die Vertiefungen6 einen Durchmesser von 1 mm aufweisen und 5 mm tief sind. Außerdem ist eine kreisförmige Anordnung von Kanälen8 vorhanden. Jeder Kanal zeigt einen Durchmesser von 1,5 mm und erstreckt sich von der oberen Fläche des Teils bis zu seiner Basis. - Mit Bezugnahme auf
2a bis2c wird schematisch eine spinale Ersatzbandscheibe gezeigt. Die Scheibe weist eine per se bekannte biomedizinische halbkristalline polymere Scheibe30 (beispielsweise Polyurethan oder Polyethylen) auf, die schichtartig zwischen einem Paar scheibenartiger Metallendplatten32 in Übereinstimmung mit der vorliegenden Erfindung angeordnet ist. Die polymere Scheibe30 wird an den Endplatten32 durch Klebstoff gesichert, obgleich verstanden wird, dass die Elemente miteinander verschmolzen oder die Endplatten32 mit der polymeren Scheibe30 durchdrungen werden können. - Die Scheiben
32 selbst sind im Allgemeinen nierenförmig (2b ) und weisen einen im Wesentlichen nichtporösen Kernbereich32a benachbart der polymeren Scheibe30 und einen porösen Außenbereich (32b ) auf, der von der polymeren Scheibe (30 ) entfernt ist (2c ). Der poröse Außenbereich erleichtert das Einwachsen des Gewebes. - Mit Bezugnahme auf
3a bis3c ist ein spinales Körbchen40 im Allgemeinen in der Form eines hohlen Tuben mit einem rechteckigen Querschnitt vorhanden. Ein Kern42 am einem im Wesentlichen vollständig dichten Metall wird durch einen porösen äußeren Metallbereich44 (erleichtert das Einwachsen des Gewebes) umgeben. Ein Durchgang46 erstreckt sich durch die Mitte des Körbchens40 . - Aus dem Vorangegangenen wird leicht ersichtlich, das durch sorgfältige Auswahl der Pulverfraktion und der Verfahrensparameter eine gewünschte Porosität und Porengrößenverteilung des porösen Oberflächenbereiches entsprechend dem speziellen Endverwendungszweck erhalten werden kann, für den das Implantat vorgesehen ist. Obgleich die vorangegangenen Beispiele zylindrische Stäbe sind, wird man verstehen, dass sie leicht zu einer geeigneten Form in Abhängigkeit von der vorgeschlagenen Verwendung bearbeitet werden können. Gleichermaßen wird man verstehen, dass der Behälter, der für die Herstellung des Implantatvorproduktes verwendet wird, nicht zylindrisch sein muss und von irgendeiner gewünschten Form sein kann.
Claims (13)
- Verfahren zur Herstellung eines chirurgischen Implantates, das einen Kernbereich und einen porösen Oberflächenbereich aufweist, der sich über mindestens einen Teil des Kernbereiches erstreckt, wobei der poröse Oberflächenbereich einen vorgegebenen Porenvolumenanteil aufweist, wobei das Verfahren die folgenden Schritte aufweist: (i) Zuführen von Metallpulver mit einer vorgegebenen Teilchengrößenverteilung um einen vorgeformten Kern in einer abdichtbaren Kapsel; (ii) Verringern des Druckes innerhalb der Kapsel auf einen vorgegebenen Druck unterhalb des atmosphärischen Druckes; (iii) Unterdrucksetzen der Kapsel mit einem Prozessgas auf einen vorgegebenen Druck höher als der vorgegebene Druck des Schrittes (ii); (iv) Abdichten der Kapsel; (v) Erwärmen der unter Druck gesetzten abgedichteten Kapsel auf eine erhöhte Temperatur und einen erhöhten Druck über eine vorgegebene Zeit, um ein Implantatvorprodukt herzustellen; (vi) Abkühlen der abgedichteten Kapsel; und (vii) Erwärmen des Implantatvorproduktes über eine vorgegebene Zeit bei einer erhöhten Temperatur und einem vorgegebenen Druck unterhalb des atmosphärischen Druckes, um dadurch eine Porosität im Implantatvorprodukt zu erzeugen.
- Verfahren nach Anspruch 1, bei dem das beim Schritt (i) verwendete Pulver einen Teilchengrößenbereich von etwa 50 μm bis etwa 750 μm aufweist.
- Verfahren nach Anspruch 1 oder 2, bei dem der vorgegebene Druck des Schrittes (ii) 10–3 mBar (0,1 Pa) oder weniger beträgt.
- Verfahren nach einem der Ansprüche 1 bis 3, bei dem der vorgegebene Druck des Schrittes (vii) 10–3 mBar (0,1 Pa) oder weniger beträgt.
- Verfahren nach einem der Ansprüche 1 bis 4, bei dem das Prozessgas des Schrittes (iii) ein nichtreaktionsfähiges Gas oder ein reaktionsfähiges Gas oder eine Mischung des reaktionsfähigen und nichtreaktionsfähigen Gases ist, wobei das nichtreaktionsfähige Gas als ein Träger oder Verdünnungsmittel für das reaktionsfähige Gas dient.
- Verfahren nach Anspruch 5, bei dem Stickstoff als das reaktionsfähige Gas verwendet wird.
- Verfahren nach einem der Ansprüche 1 bis 5, bei dem ein Nitrierschritt nach dem Schritt (vii) durchgeführt wird.
- Verfahren nach einem der Ansprüche 1 bis 7, bei dem der Druck des Schrittes (iii) von etwa 1 × 105 Pa bis etwa 5 × 105 Pa Überdruck beträgt.
- Verfahren nach einem der Ansprüche 1 bis 8, bei dem die erhöhte Temperatur des Schrittes (v) von etwa 850 bis etwa 1100°C beträgt.
- Verfahren nach einem der Ansprüche 1 bis 9, bei dem die erhöhte Temperatur des Schrittes (vii) von etwa 900 bis etwa 1300°C beträgt.
- Verfahren nach einem der Ansprüche 1 bis 10, bei dem das Implantatvorprodukt von der Kapsel nach dem Schritt (vi) vor dem Schritt (vii) getrennt wird.
- Verfahren nach einem der Ansprüche 1 bis 11, bei dem die Kapsel vom Implantatvorprodukt nach dem Schritt (vii) durch Bearbeitung getrennt wird.
- Verfahren nach einem der Ansprüche 1 bis 12, bei dem das Metallpulver vor der Einkapselung teilweise verdichtet wird, vorzugsweise durch selektives Lasersintern einer Mischung des Metallpulvers und eines polymeren Bindemittels.
