Description
この種の外科用インプラントは、通常、生体適合性のある金属、例えばチタン、チタン合金、ステンレス鋼またはコバルトクロミウム合金から、強度や重量を考慮して調製されている。
他の、特に骨の代りをする外科用インプラントにおいて重要な点は、外科用インプラントの弾性率である。
弾性率が自然の骨に近けれれば近いほど、隣接した骨への応力再分散が良くなる。
良好な応力分散は、結果として外科用インプラントの使用寿命を延ばすことになる。
被覆層としては、耐摩耗性を改良する、および/または、生体適合性を与えるために、外科用インプラントと異なる組成物を適用してもよい。
例えば、アルミニウム又はバナジウムを含む外科用インプラントに、外科用インプラントからそれらの金属が浸出するのを妨げるために、極めて不活性な窒化チタンが被覆層として適用される。
(1) コア領域と、
(2) このコア領域の少なくも一部に展延する多孔性の表層領域
からなり、前記多孔性の表層領域は、予め定められた空孔率を有する。
例えば、外科用インプラントは、円筒状のように展延したものであっても、板状であってもよい。
また、外科用インプラントは、不規則な形状を有したものでもよく、多孔性の表層領域はコア領域の上に設けられ、その厚みは変化させ得るものである。
この空孔率は、外科用インプラントの特性に応じて、あらかじめ所定の値に決定される。
高い空孔率は、結果として、低い弾性率を有する軽量の外科用インプラントを与える。
空孔の少なくも一部は、好ましくは、その多くが100μmから約750μmの範囲の大きさを有しているもので、より好ましくは、約200μmから500μmのものである。
より好ましくは、約2mmから約5mmの厚さを有するもので、特に好ましくは2mmから3mmの厚さを有するものである。
例えば、空孔率が外科用インプラントの軸にそって、および/または軸と垂直方向に勾配を有しいてもよい
コア領域の多孔性は、表層領域より大きくても、小さくてもあるいは同等であってもよい。
特に適切な材料は、市販品純度(ASTM B 338 GR2)のチタン、ステンレス鋼、Ti−Al−V合金、Ti−Al−Nb合金などのチタン系合金、およびコバルト−クロミウム系合金などである。
それによれば、コア領域および多孔性の表層領域間の界面が、外科用インプラントに弱点を齎すことはない。
(1)密封可能なカプセル内の予め形成されたコアの周囲に、所定の粒径分布を有する金属粉末を載置する工程。
(2)カプセル内を、大気圧以下の所定の圧力に減圧する工程。
(3)工程(2)における所定圧力よりも高い所定圧力まで、前記カプセルを加工用ガスで加圧する工程。
(4)前記カプセルを密封する工程。
(5)前記加圧され密封されたカプセルを高温高圧下に所定時間加熱し、インプラント前駆体を調整する工程。
(6)前記密封されたカプセルを冷却する工程。
および
(7)前記インプラント前駆体を、高温且つ大気圧以下の所定圧力下に所定時間加熱し、前記インプラント前駆体に多孔性を付与する工程。
粉末は、例えば、ガス噴霧装置または機械的摩損によって調製することができる。
いずれの場合においても、コアは、粉末と同一又は異質、残存するカプセルと同一又は異質の材料からなるものである。
工程(7)における所定の圧力は、10−3mbar(0.1Pa)以下であることが好ましい。
金属粉末がチタンを含有しているときは、窒素の使用は特に好適である。
窒化は、工程(7)の後に、好ましくは約800℃〜約1000℃、10〜100MPaの圧力で、1〜8時間の別工程として行なうことができる。
工程(5)は、約1時間から約5時間実施されるのが好ましい。
工程(7)は、カプセル内のインプラント前駆体に実施される。
あるいはインプラント前駆体は、工程(6)の後で工程(7)の前に、カプセルから分離される。
カプセルは、例えば、旋盤による機械加工による取り外しのように、簡単に分離することができる。
工程(7)の高温は、好ましくは、約900から約1300℃である。
工程(7)は、好ましくは約80時間以内、より好ましくは約60時間、特に好ましくは約48時間以内で実施される。
工程(7)は、好ましくは少なくとも6時間、より好ましくは少なくとも12時間実施される。
しかしながら、内表面が適切に形成されたカプセルを使用すれば、要求されるものと実質的に同じ形状を有する外科用インプラントを調製することができる。
適切に形成されたカプセルの調製法としては、電鋳および直接レーザー加工などが挙げられる。
これは、金属粉および高分子バインダの混合物に、選択的レーザー焼結を実施することにより達成される。
異なる粉末成分が、インプラントの異なる領域を形成するために用いられ、そのような変更は、特に、多孔性領域内で多孔性に勾配をもたせるのに有用である。
内径約22mm、外径約25.4mm、長さ100mmの市販品純度のチタン(ASTM B 338 GR2)製の円筒状の容器に、所定の粒度分布を有し、ガス噴霧法で調製されたTi−6Al−4V合金(クルシブル リサーチ製(USA))からなる粉末を充填した。
特定の粒度分別物を得るには、必要であれば、粉末を適当なメッシュのふるいを通過させる。容器は振動させずに、すなわちタップ密度状態に、満たされる。
容器内の空気は、0.1Pa以下になるようにして除去され、正圧105Paのアルゴンが2分間封入された。
密封した後、容器には、外部圧力100MPa、温度950℃で4時間の熱間静水圧処理が施された。
ついで、容器は放冷されて室温に、そして合金基質内に、アルゴンが圧力下に残存している大気圧状態とされた。
インプラント前駆体は、容器から機械加工により取り出され、0.1Pa以下の真空下に温度1050℃で48時間熱処理された。
粒度分布が180〜250μmと180〜500μmの粉末を用いて、実施例1を繰返した。
熱間静水圧処理は1000℃で行なわれ、ついで、容器から分離した後の熱処理を1050℃で48時間行ない、その結果を表2に示した。
粒度分布が、250〜500μmの粉末を用いて、実施例1を繰返した。
熱間静水圧処理は1000℃で行ない、ついで、容器から分離した後の熱処理を1050℃で12時間から48時間行ない、その結果を表3に示した。
粒度分布が150μm以下の粉末を用いて、実施例1を繰返した。
容器から分離した後の熱処理を1050℃で行ない、その結果を表4に示した。
粒度分布が250〜425μmの粉末を用いて、実施例1を繰返した。
容器から分離した後の熱処理を、各種温度で48時間行ない、その結果を表5に示す。
各種の粉末を用いて、実施例1を繰返した。
1100℃で48時間の熱処理後、インプラントは、純窒素下に、950℃、50MPaの条件下に6時間、圧力窒化を行ない、コアの内表面を窒化チタンの薄層で被覆した。
その結果を表6に示した。
12.7mmのTi−6Al−4Vの円柱状のコアを、実施例1に示したチタン製の容器の中心に配置した。
コアと円筒体の、内壁の間の隙間には、粒度500〜710μmのTi−6Al−4V粉末を充填した。
容器は減圧され、正圧105Paのアルゴンを満たし、密封し、950℃、100MPaで4時間、熱間静水圧処理を施した。
ついで、容器から分離し、温度1050℃で48時間熱処理した。
得られた試料は、孔径50μm以上が43%、100μm以上が16%、150μm以上が6%で、空孔率が29±5%である、多孔性の被覆層を有するものであった。
粒径が125μm以下の粉末を用いて、実施例1を繰返した。
熱間静水圧処理した後、部材を機械加工により容器から分離し、また、直径を17mmに減少させた。
実施例1と異なり、この段階では、熱処理は行なわなかった。
実施例7と同様に、得られた円筒体をコアとして使用した。
容器は外径38.1mm、内径約35mmである。
実施例7が繰返され、この場合においては、コアは粒径250〜425μmの粉末で覆われている。
得られたインプラントは、平均孔径26μm、空孔率28.9±2.1%の多孔性のコアと、平均孔径113μm、空孔率24.9±1.9%の多孔性の外層を有するものであった。
また、その製造方法は、異なる多孔性の異なる層を、2層以上有するインプラントの調製に応用でき、すなわち、段階的な多孔性構造を有するインプラントを製造することができることを示している。
所定の粒度分布を有する粉末を用いて、実施例1を繰返した。
1100℃で48時間の熱処理の後、試料は、多孔性材料の特性を評価するために、抗張力試験片に機械加工された。その結果は表7に示した。
あらかじめ形成されたコアと、多孔性表層領域が結合している実施例において、コアと多孔性表層領域の界面に、強力な結合が存在するか否かを評価するために、抗張力試験が実施された。
これらの試験において、Ti−6Al−4V粉末は、固められるとともに、界面が試験片の標識の中央にくるように、Ti−6Al−4V固形体に拡散接合される。
125〜150μm(試験片a)と、150〜250μm(試験片b)の2種の分別粉末が評価された。
いずれの場合においても、試験容器は、正圧105Paまでアルゴンが充填された。
最初の熱間静水圧処理工程は950℃、100MPaで4時間行なわれ、つぎの熱処理工程は1100℃で48時間行なった。
得られた多孔性は、約40%(試験片a)と約35%(試験片b)であった。
常法の金属組織試験により界面は影響を受けず、また、界面は、酸化物などの汚染を受けず、隣接する組織と見分けのつかないものであった。
丸い凹部6は、外周が0.75mmで、内部に向って3mm展延している。
部材12は、凹部6の直径が2mmであることを除けば、部材2と同等である。
各通路は、直径が1.5mmで、部材の表面の頂部から底部まで展延している。
このディスクは、この発明による、一対のディスク形状の、金属製端末板32の間に挟持された、それ自身公知の生医学的半晶質ディスク30(例えば、ポリウレタン又はポリエチレン)から構成されている。
それらの素材は、共に溶融することも、重合性ディスク30が端末板32に浸透させることもできるが、重合性ディスク30は、端末板32に接着剤により固定されている。
多孔性外部領域は、組織が内方成育するのを助成する。
実質的に完全に緻密な金属からなるコア42は、組織の成育を助成する、多孔性の外部金属領域に囲まれている。通路46は、ケージ40の中央を通って展延している。
同様に、インプラント前駆体を調製するために用いる容器は、円筒状である必要はなく、所望のいかなる形状もとり得ることができるものである。
Claims (22)
- コア領域と、前記コア領域の少なくも一部に展延する多孔性の表層領域からなり、前記多孔性の表層領域は、所定の空孔率を有する外科用インプラントの製造方法であって、次の工程からなる外科用インプラントの製造方法。
(1)密封可能なカプセル内の予め形成されたコアの周囲に、所定の粒径分布を有する金属粉末を載置する工程
(2)カプセル内を、大気圧以下の所定の圧力に減圧する工程
(3)工程(2)における所定圧力よりも高い所定圧力まで、前記カプセルを加工用ガスで加圧する工程
(4)前記カプセルを密封する工程
(5)前記加圧され密封されたカプセルを高温高圧下に所定時間加熱し、インプラント前駆体を調整する工程
(6)前記密封されたカプセルを冷却する工程
および
(7)前記インプラント前駆体を、高温且つ大気圧以下の所定圧力下に所定時間加熱し、それにより、前記インプラント前駆体に多孔性を付与する工程 - 前記工程(1)において使用する粉末が、
粒径が約50μmから約750μmの範囲にある
請求項1に記載の外科用インプラントの製造方法。 - 前記工程(2)における所定の圧力が、
10−3mbar(0.1Pa)以下である
請求項1又は2に記載の外科用インプラントの製造方法。 - 前記工程(7)における所定の圧力が、

不活性ガス、活性ガス又は活性ガスのキャリヤー、又は希釈剤として機能する不活性ガスと活性ガスとの混合物である

窒素である

カプセル化の前に部分的に強化、好ましくは金属粉末および高分子バインダの混合物を選択的レーザー焼結することにより強化されたものである
請求項1〜12のいずれかに記載の外科用インプラントの製造方法。 - 請求項1〜13のいずれかに記載の外科用インプラントの製造方法で製造された、コア領域と、前記コア領域の少なくも一部に展延する多孔性の表層領域からなり、前記多孔性の表層領域は、所定の空孔率を有する
外科用インプラント。 - 前記多孔性の表層領域の空孔率が、
20〜50%である
請求項14に記載の外科用インプラント。 - 前記空孔が、
相互に連通し、多孔性の表層領域に、実質的に均一に分散されている

理論密度の1.0から0.7の密度を有する
請求項14〜19のいずれかに記載の外科用インプラント。 - 前記コア領域および/または多孔性の表層領域が、
チタン、市販品純度(ASTM B 338 GR2)のチタン、ステンレス鋼、チタン系合金、Ti−Al−V合金、Ti−Al−Nb合金またはコバルト−クロミウム系合金で調製されている請求項14〜20のいずれかに記載のインプラント。 - 前記コア領域および/または多孔性の表層領域が、
Ti−6Al−4V、Ti−6Al−7Nb、ステライト21(商標名)またはステンレス鋼316Lで調製されている
