Description
Foreliggende oppfinnelse angår en fremgangsmåte for fremstilling av et kirurgisk benimplantat.
Det finnes mange forskjellige typer kirurgiske implantater og anordninger, inkludert ortopediske- (hofte, kne, ankel, albue), hode/ansikts- og ryggradsimplantater (så som ryggradsrammer og -skiver). Slike implantater er vanligvis laget av biokompatible metaller så som titan, titanbaserte legeringer, rustfritt stål eller kobolt-krom legeringer, avhengig av styrke- og vektoverveielser. Et annet hensyn, spesielt i forhold til implantater som erstatter ben, er elastisitetsmodulen til implantatet. Jo nærmere elastisitetsmodulen er til naturlig ben, jo bedre blir belastningsredistribusjonen til nærliggende ben. Bedret belastningsredistribusjon medfører en forlenget nyttig levetid for implantatet.
For å øke bindingen av omliggende vev med et implantat, er det kjent å påføre et belegg på overflaten til implantatet, for eksempel ved plasma sprøyting, som fører til at overflaten til implantatet blir ru. Dette belegget kan ha en forskjellig sammensetning enn implantatet for å forbedre slitasjemotstanden og/eller for å forbedre biokompatibiliteten (f.eks. er TiN ekstremt inert og kan påføres som et belegg på aluminium- eller vanadiuminneholdende implantater for å hindre utvasking (eng: leaching) av disse metallene fra implantatet.
Publikasjonen DE 2127843 angår en kirurgisk proteseanordning med porøst metallbelegg. SE 468502 viser kirurgiske implantater med et kjerneområde og et porøst område dannet av et keramisk pulver, her anvendes ikke en forhåndsdannet kjerne. US 4542539 viser et biologisk kompatibelt kirurgisk proteseimplantat laget med et flerlagsbelegg av metalliske partikler med en porestørrelsesgradient.
Publikasjonene US 5306309, US 5458643, US 5986169, US 6149688, US 2025238 viser andre implantater og proteser og fremgangsmåter for fremstilling av slike.
Det er et formål med foreliggende oppfinnelse å fremskaffe et forbedret kirurgisk implantat, som fortrinnsvis har forbedret kompatibilitet med ben og/eller slitasjemotstand og/eller levetid og/eller er sterkere enn kjente implantater.
Det er forstått at "porevolumfraksjon", slik ordet blir brukt her, av et porøst område, er forholdet mellom porevolumet i et område og det totale volumet av samme område, uttrykt som en prosentdel.
Implantatet kan være i en hvilken som helst form, styrt av hvilke behov som finnes i implantatets påtenkte bruksområde. For eksempel kan implantatet være langstrakt (f.eks. sylindrisk) eller skiveformet. Implantatet kan ha en uregelmessig form, og det porøse overflateområde kan variere i tykkelse over kjerneområdet.
Fortrinnsvis vil porevolumfraksjonen i det porøse overflateområde være mellom 20 til 50%. Den forhåndsbestemte porevolumfraksjonen blir valgt i henhold til hvilke egenskaper som ønskes av implantatet. En høyere porevolumfraksjon medfører et lettere implantat med en lavere elastisitetsmodul.
Fortrinnsvis er porene forbundet med hverandre, og i hovedsak enhetlig fordelt innenfor det porøse overflateområde. Fortrinnsvis vil i det minste noen (og fortrinnsvis flertallet) av porene har en størrelse i området 100 - 750 um, og mer fortrinnsvis i området 200 - 500 um.
Fortrinnsvis er det porøse overflateområde minst omtrent 1 mm tykk, og mer fortrinnsvis i området 2-5 mm tykk, mest fortrinnsvis 2-3 mm tykk.
Forskjellige områder innenfor det porøse overflateområde kan ha forskjellig porestørrelsesfordeling og/eller forskjellig porevolumfraksjon slik at det er en porestørrelse og/eller porevolumfraksjongradient innefor det porøse overflateområde. For eksempel kan det være en porevolumfraksjongradient langs lengdeaksen til implantatet og/eller vinkelrett på lengderetningen til implantatet.
Kjerneområdet kan være helt kompakt eller porøs. Graden av porøsitet i kjerneområdet kan være mer enn, mindre enn eller det samme som overflateområdet.
Fortrinnsvis er kjerneområdet relativt mindre porøs enn overflateområdet.
Fortrinnsvis har kjerneområdet en tetthet på fra 0,7 til 1,0 av teoretisk tetthet (dvs. fra 0 til 30% porøsitet).
Kjerneområdet og det porøse overflateområde kan være konstruert av samme eller forskjellige materialer. Spesielt egnede materialer inkluderer titan (f.eks. kommersielt ren (ASTM B 338 GR 2) titan), rustfritt stål, titanbaserte legeringer (f.eks. Ti-Al-V legeringer og Ti-Al-Nb legeringer) og kobolt-krom-baserte legeringer.
Spesielt foretrukne materialer er kommersielt ren titan, TieAUV, TieAlyNb, Stellite 21 og rustfritt stål 316L.
Fortrinnsvis er det porøse overflateområde diffusjonsbundet til kjerneområdet. Dermed er det forstått at grenseflaten mellom kjerneområdet og det porøse overflateområde ikke introduserer en svakhet i implantatet.
Den foreliggende oppfinnelsen er relatert til en fremgangsmåte for fremstilling av et kirurgisk benimplantat omfattende et kjerneområde og et porøst overflateområde som strekker seg over i det minste en del av nevnte kjerneområde, hvor det porøse overflateområde har en forhåndsbestemt porevolumandel. Oppfinnelsen er kjennetegnet ved at fremgangsmåten omfatter følgende trinn;
i. å anordne metallisk pulver som har en forhåndsbestemt partikkel størrelsesfordeling, rundt en forhåndsdannet kjerne i en forseglbar kapsel; ii. å redusere trykket inne i nevnte kapsel til et forhåndsbestemt trykk under
atmosfærisk trykk;
iii, å trykksette nevnte kapsel med en prosessgass til et forhåndsbestemt trykk
høyere enn det forhåndsbestemte trykket i trinn (ii);
i v. å forsegle nevnte kapsel; v. å varme opp den gassfylte og forseglede kapselen ved en forhøyet temperatur og et forhøyet trykk i en forhåndsbestemt periode for å danne et implantat forstadium; vi. å kjøle ned nevnte kapsel, og; vii. å varme opp nevnte implantat forstadium i en forhåndsbestemt tid ved en forhøyet temperatur og et forhåndsbestemt trykk under atmosfærisk trykk, og dermed danne porøsitet i nevnte implantat forstadium.
Fortrinnsvis har pulveret benyttet i trinn (i) en partikkelstørrelsesfordeling fra omtrent 50 um til omtrent 750 um (dvs. i hovedsak alle partikler er innenfor det spesifiserte størrelsesområdet). Pulveret kan være produsert, for eksempel, ved gassatomisering eller mekanisk slitasje.
Det kan være praktisk med en kjerne som er integrert med kapselen, men kjernen kan også være frittstående. I begge tilfeller kan kjernen være av samme eller forskjellig materiale fra pulveret, og av det samme eller forskjellig materiale fra resten av kapselen.
Fortrinnsvis er det forhåndsbestemte trykket i trinn (ii) IO"<3> mbar (0,1 Pa) eller mindre. Fortrinnsvis er det forhåndsbestemte trykket i trinn (vii) IO"<3> mbar (0,1 Pa) eller mindre.
Prosessgassen i trinn (iii) kan være en ikke-reaktiv gass (f.eks. argon), eller en reaktiv gass (f.eks. nitrogen) eller en blanding av reaktive og ikke-reaktive gasser, hvor den ikke-reaktive gassen tjener som en bærer eller fortynner av den reaktive gassen. Når nitrogen er til stede i prosessgassen vil overflaten av porene som etter hvert dannes bli nitrert, og dermed økes slitasjemotstand og kjemisk uvirksomhet. Bruk av nitrogen er spesielt fordelaktig når det metalliske pulveret inneholder titan. Som et alternativ kan et separat nitreringstrinn utføres etter trinn (vii), fortrinnsvis ved en temperatur på fra omtrent 800°C til omtrent 1000°C, og ved et trykk på fra omtrent 10 til 100 MPa, i 1 til 8 timer.
Fortrinnsvis er trykket i trinn (iii) fra omtrent 1 bar (1 x 10<5> Pa) til omtrent 5 bar
(5 x IO<5> Pa) positivt trykk.
Fortrinnsvis er den forhøyede temperaturen i trinn (v) fra omtrent 850°C til omtrent 1100°C. Fortrinnsvis blir trinn (v) utført over en tidsperiode fra omtrent 1 time til omtrent 4 timer.
I trinn (vi) blir den forseglede kapselen (og implantat forstadiet inne i den) fortrinnsvis kjølet ned til romtemperatur. Trinn (vii) kan utføres på implantat forstadie i kapselen. Alternativt kan implantat forstadiet separeres fra kapselen etter trinn (vi) og før trinn (vii). Kapselen kan greit fjernes ved, for eksempel, maskinering på en dreiebenk. Fortrinnsvis er den forhøyede temperaturen i trinn (vii) fra omtrent 900°C til omtrent 1300°C. Fortrinnsvis blir trinn (vii) utført i en tidsperiode på ikke mer enn omtrent 80 timer, fortrinnsvis ikke mer enn omtrent 60 timer og mer fortrinnsvis ikke mer enn omtrent 48 timer. Fortrinnsvis blir trinn (vii) utført i en tidsperiode på minst 6 timer, og mer fortrinnsvis i minst 12 timer.
Dersom trinn (vii) utføres med implantat forstadiet inne i kapselen, blir kapselen fjernet fra implantatet etter trinn (vii), for eksempel ved maskinell maskinering.
Det er forstått at implantatet dannet ved bruk av fremgangsmåten over vil ha tatt form etter de indre overflatene til kapselen, slik at videre maskineringstrinn kan være påkrevd for å oppnå den endelige og ønskede formen på implantatet. Det er imidlertid også mulig å produsere et implantat som i hovedsak har den ønskede formen ved å benytte en kapsel som har en egnet form på den innvendige overflaten. Eksempler på fremgangsmåter som kan produsere en hensiktsmessig formet kapsel inkluderer galvanoformgivning (eng: electro-forming) og direkte laserformgivning.
Ved en liten endring av ovennevnte fremgangsmåte, er det metalliske pulveret delvis formet i fast masse i forkant av at kapselen lukkes. Dette kan oppnåes ved, for eksempel, selektiv lasersintring av en blanding av det metalliske pulveret og et polymerbindemiddel. Forskjellige pulverandeler kan benyttes til å danne forskjellige områder i implantatet, og dermed er denne endringen spesielt egnet for å lage implantater som har en porøsitetsgradient innefor det porøse området.
Det er også beskrevet en utførelsesform som omfatter følgende trinn; (i) å selektivt sintre etterfølgende lag av metalliske partikler at det dannes et implantat forstadie med en egnet form, og; (ii) å varme opp nevnte implantat forstadie slik at nevnte implantat dannes.
Fortrinnsvis blir trinn i utført med redusert trykk, for eksempel i en vakuumovn.
Trinn (i) kan utføres ved å sintre partiklene rundt en kjerne. Fortrinnsvis vil nevnte kjerne ha 0,7 til 1,0 av teoretisk tetthet (dvs. ha en porøsitet på fra 0 til 30%). Kjernen kan imidlertid ha samme, høyere eller lavere tetthet enn det resulterende porøse overflateområdet.
Fortrinnsvis vil i hovedsak alle nevnte metalliske partikler ha en størrelse på 750 um eller mindre, mer fortrinnsvis 500 um eller mindre, og mest fortrinnsvis 150 um eller mindre. Fortrinnsvis vil nevnte partikler ha en størrelse på minst 50 um. Fortrinnsvis er nevnte partikler i en blanding med ett bindemiddel, så som et lys-og/eller varmesensitivt polymerisk bindemiddel. Mer fortrinnsvis blir trinn (i) utført ved å føre en laser (typisk med en effekt på 20 til 40 W) over nevnte blanding av metallisk pulver og bindemiddel.
Fortrinnsvis blir trinn (ii) utført med en temperatur fra omtrent 1000°C til omtrent 1300°C. Det er forstått at i løpet av trinn (ii) vil alle spor av et eventuelt bindemiddel bli brent bort.
Der er forstått at trinn (i) kan benyttes for å lage områder med makroporøsitet. For eksempel kan det lasersintrede området utstyres med forsenkninger som strekker seg innover fra overflaten av implantatet, og/eller kammere inne i implantatet. I tillegg kan nevnte kammere og/eller forsenkninger bli koblet sammen for å danne kanaler eller passasjer i implantatet. Det er også forstått at slik makroporøsitet kan introduseres inn i implantatet produsert ved fremgangsmåten i samsvar med oppfinnelsen.
Utførelse av oppfinnelsen vil nå bli beskrevet ved hjelp av eksempler, med referanse til vedlagte illustrasjoner hvor;

EKSEMPEL 1:
En 100 mm lang sylindrisk beholder av kommersielt ren titan (ASTM B 338 GR 2) som har en indre diameter på omtrent 22 mm og en ytre diameter på 25,4 mm ble
fylt med gassatomisert pulver som omfattet partikler av TieAUV-legering (Crucible Research, USA) med en forhåndsbestemt partikkelstørrelsesfordeling. Der hvor det var nødvendig ble partiklene filtrert gjennom siler med hensiktsmessig maskevidde for å oppnå en spesiell partikkelstørrelsesandel. Beholderen ble fylt uten vibrasjon, dvs. kun til "innfyllingstetthet". Luften ble sugd ut av beholderen (trykk <0,1 Pa) og igjen fylt med argon (10<5> Pa overtrykk) i 2 minutter. Etter forsegling ble beholderen HIPped i 4 timer ved 950°C og med et ytre trykk på 100 MPa. Deretter fikk beholderen kjøle seg ned til romtemperatur og atmosfærisk trykk, mens Ar
fremdeles var under trykk i legeringsmatrisen. Implantat forstadiet ble hentet ut av beholderen maskinelt, og varmebehandlet ved 1050°C i 48 timer i vakuum (<0,1 Pa).
Porøsitetsgraden i det resulterende implantatet, fremstilt med forskjellige pulverfraksj oner, er vist i tabell 1 under.
EKSEMPEL 2:
Eksempel 1 ble gjentatt med pulver som hadde en størrelsesfordeling på 180 - 250 um og 150 - 500 um. HIPping ble utført ved 1000°C og etterfølgende varmebehandling (etter at beholderen var fjernet) var ved 1050°C i 48 timer. Resultatene er vist i tabell 2.
EKSEMPEL 3:
Eksempel 1 ble gjentatt med pulver som hadde en størrelsesfordeling på 250 - 500 um. HIPping ble utført ved 1000°C og etterfølgende varmebehandling (etter at beholderen var fjernet) var ved 1050°C i fra 12 og opp til 48 timer. Resultatene er vist i tabell 3.
EKSEMPEL 4:
Eksempel 1 ble gjentatt med pulver som hadde en størrelsesfordeling på < 150 um. Etterfølgende varmebehandling, etter at beholderen var fjernet, var ved 1050°C. Resultatene er vist i tabell 4.
EKSEMPEL 5:
Eksempel 1 ble gjentatt med pulver som hadde en størrelsesfordeling på 250 - 425 um. Etterfølgende varmebehandling, etter at beholderen var fjernet, var i 48 timer ved forskjellige temperaturer. Resultatene er vist i tabell 5.
EKSEMPEL 6:
Eksempel 1 ble gjentatt med forskjellige pulvere. Etter varmebehandlingen (1100°C i 48 timer) ble implantatene trykknitrert i rent nitrogen, ved 950°C og 50 MPa i 6 timer, for å dekke den indre overflaten av porene med et tynt lag av titannitritt. Resultatene er vst i tabell 6.
EKSEMPEL 7: En 12,7 mm massiv sylindrisk kjerne av TiéAUV ble plassert i midten av titanbeholderen beskrevet over i eksempel 1 .Mellomrommet mellom kjernen og den indre side av sylinderveggen ble fylt med TiéAUV-pulver (partikkelstørrelse 500 - 710 um). Beholderen ble evakuert og fylt med Ar til 105 Pa overtrykk. Beholderen ble forseglet og HIPpet ved 950°C i 4 timer ved 100 MPa. Den etterfølgende varmebehandligen, etter fjerning fra beholderen, var ved 1050°C i 48 timer. Dette resulterte i en prøve som hadde et porøst belegg med 29±5% porøsitet, 43% av porene var >50 um, 16% var >100 um og 6% var >150 um.
EKSEMPEL 8:
Eksempel 1 ble gjentatt med pulver som hadde partikkelstørrelse <125 um. Etter HIPping ble delen maskinert for å fjerne beholderen og for å redusere diameteren til ~17 mm. Til forskjell fra eksempel 1 ble det ikke utført varmebehandling på dette stadiet. Den resulterende sylinderen ble deretter benyttet som kjerne, på samme vis som i eksempel 7, hvor beholderen hadde en ekstern diameter på 38,1 mm, og en intern diameter på ca. 35 mm. Deretter ble eksempel 7 gjentatt, med den forskjell at kjernen ble omgitt av pulver med en partikkelstørrelse på 250 - 425 um. Det resulterende implantatet hadde en porøs kjerne med en gjennomsnittlig porestørrelse på 26 um (28,9±2,1% porøsitet) og et porøst ytre lag med en gjennomsnittlig porestørrelse på 113 um (24,9±1,9% porøsitet).
Eksempel 8 viser fremgangsmåtens evne til å produsere implantater som har forhåndsbestemte områder med forskjellige porøsiteter på et kontrollert vis. I tillegg er det forstått at prosedyren kan utvides til å produsere et implantat som har mer enn 2 forskjellige lag med forskjellig porøsitet, dvs. en gradert porøs struktur kan fremstilles.
EKSEMPEL 9:
Eksempel 1 ble gjentatt med pulver som har en forhåndsbestemt størrelsesfordeling. Etter varmebehandlingen (1100°C i 48 timer), ble prøvene behandlet maskinelt til strekkprøvestykker, slik at egenskapene til det porøse materialet kunne prøves ut. Resultatene er vist i tabell 7.
EKSEMPEL 10:
For å vise at, der hvor eksemplene har vist at en forhåndsprodusert kjerne blir bundet til et porøs overflateområde, er det en sterk binding i grenseflaten mellom kjernen og overflateområdene, ble det utført strekkstyrke tester. I disse testene ble pulveret (T16AI4V) samtidig samlet i en masse og diffusjonsbundet til en massiv TiéAl/tV-barre slik at grenseflaten var ved midtpunktet av målelengden av prøven. To pulverandeler ble prøvet ut, 125 - 150 nm (prøve a) og 150 - 250 um (prøve b). I hvert tilfelle ble beholderen fyllt opp med Ar til IO<5> Pa overtrykk. Det innledende HIP trinnet ble utført ved 950°C i 4 timer ved 100 MPa. Det etterfølgende oppvarmingstrinnet ble utført ved 1100°C i 48 timer. Den resulterende porøsiteten var på ca. 40% (prøve a) og 35% (prøve b).
I begge tilfeller var strekk UTS omtrent 400 MPa, og ved feil oppsto langt vekk fra kontaktflaten inne i hoveddelen av det porøse materialet. Rutinemessige metallografiske undersøkelser bekreftet at grenseflaten fortsatt var upåvirket, og at kontaktflaten var fri for forurensinger (f.eks. oksider) og ikke til å skjelne fra den nærliggende matrise.
EKSEMPEL 11:
Med henvisning til fig. 1, var delene som vises på illustrasjonen fremstilt ved selektiv laser sintring. Laseren er kontrollert av en computer, som er lastet med en CAD-fil som inneholder oppsettet av delen. Delen ble fremstilt på en pulverseng som var oppvarmet til 160°C (typisk temperatur 100 - 200°C). Pulver med en partikkelstørrelse <150 (im og 15% (volum) bindemiddel (typisk 5 til 25% volum) ble varmet opp til 60°C (typisk 50 - 100°C) og anordnet på pulversengen med en høyde på 200 um per lag (typisk 100 - 500 (im). Selektiv sintring ble utført ved bruk av en 25W laser. Etter at delen var blitt bygget opp ble bindingen fjernet ved 400°C i 30 minutter (typisk 200 - 500°C i 30 - 120 minutter), og etterpå ble sintring utført ved 1100°C (typisk 1000 - 1350°C).
Del 2 har, som det fremgår av figur 1, et generelt porøst sylindrisk område
(porøsitet 20-25%), med et regulært mønster av runde fordypninger 6 rundt sin ytre periferi ("makroporøsitet"). Fordypningene 6 er 0,75 mm i omkrets, og strekker seg 3 mm innover . Del 12 likner del 2, med den forskjell at fordypningene 6 er 2 mm i diameter.
Del 22 likner også del 2, med den forskjell at fordypningene 6 er 1 mm i diameter og 5 mm dype. I tillegg er et sirkulært mønster av kanaler 8 fremskaffet. Hver kanal er 1,5 mm i diameter og strekker seg fra topp-overflaten av delen og ned til basen.
Med henvisning til figurene 2a til 2c, er en erstatnings-ryggradsskive vist skjematisk. Skiven omfatter i og for seg kjent biomedisinsk halvkrystalinsk polymer skive 30 (for eksempel polyutretan eller polyetylen) som et lag mellom et par skiveformede metalliske endeplater 32 i henhold til foreliggende oppfinnelse. Polymerskiven 30 festes til endeplatene 32 med et klebemiddel, selv om det er forstått at elementene kan smeltes sammen, eller at endeplatene 32 kan infiltreres med polymerskiven 30.
Skivene 32 er i seg selv generelt nyreformede (fig. 2b) og omfatter et i hovedsak ikke-porøst kjerneområde 32a ved siden av polymerskiven 30, og et porøst ytre område 32b, fjernt fra polymerskiven 30 (fig 2c). Det porøse ytre område letter inngroing av vev.
Med henvisning til figurene 3a til 3c, har en ryggradsramme 40, generelt form som en hul knoll (eng: tuber) med et rektangulært tverrsnitt. En kjerne 42 av i hovedsak helt tett metall er omgitt av et porøst ytre metallområde 44 (som letter inngroing av vev). En passasje 46 strekker seg gjennom midten av rammen 40.
Det fremstår klart av det overstående at ved nøyaktig utvelgelse av pulverandeler og prosessparametere, kan en ønsket porøsitet og porestørrelsesfordeling i det porøse overflateområde oppnås, i forhold til den spesielle slutt-anvendelsen implantatet er tiltenkt. Selv om de overstående eksemplene er sylindriske staver, er det forstått at de enkelt kan bearbeides maskinelt til en hensiktsmessig form i forhold til tiltenkt anvendelse. Samtidig er det forstått at beholderen som benyttes for å danne implantat forstadiet trenger ikke å være sylindrisk og kan ha en hvilken som helst ønsket form.

2. Fremgangsmåte i henhold til krav 1,
