Description
- Gebiet der Erfindung
- Diese Erfindung betrifft die Behandlung von Entzündungskrankheiten.
- Hintergrund der Erfindung
- Zytokine gehören zu einer großen Gruppe von Polypeptid- oder Glycopeptid-Signalstoffen, die in extrem geringen Konzentrationen als Regulatoren der Zellvermehrung und als essentielle Vermittler von Entzündungs- und Immunreaktionen wirken. Die Produktion und Funktionen von Zytokinen werden durch Zytokine selbst und durch mehrere andere Faktoren streng reguliert. Die meisten Zytokine wirken lokal und sind an einer Anzahl von entzündlichen Leiden oder Zuständen beteiligt. Diese umfassen rheumatoide Arthritis (RA), Osteoarthrose (OA), Arthritis psoriatica, Psoriasis, Colitis ulcerosa und Morbus Crohn.
- Die Antimalaria-Verbindungen Chloroquin und Hydroxychloroquin sind als breit wirksame, moderat wirkungsvolle Inhibitoren von Zytokinen bekannt. Solche Antimalaria-Mittel sind zu wichtigen „disease-modifying antirheumatic agents" (DMARD; „Erkrankungsmodifizierende antirheumatische Mittel") bei der in der zweiten Reihe erfolgenden Behandlung von rheumatoider Arthritis und anderer Entzündungskrankheiten geworden. Andere Mittel in dieser Klasse umfassen Gold, Penicillamin, Methotrexat und Cyclosporine, von denen alle hohe Aktivität aufweisen. Jedoch wird die Nützlichkeit von diesen letztgenannten Arzneimitteln für die Behandlung einer chronischen Erkrankung, wie rheumatoider Arthritis, durch ernsthafte Nebenwirkungen begrenzt. Die Antimalaria-Mittel in der DMARD-Klasse werden so angesehen, dass sie ein moderateres Nebenwirkungsprofil aufweisen, obwohl ihnen möglicherweise die Wirkkraft von einigen der anderen Mittel fehlt. Jedoch gibt es nach wie vor Bedenken hinsichtlich der Okularen Nebenwirkungen von sowohl Chloroquin als auch Hydroxychloroquin. So kann postuliert werden, dass ein Arzneimittel für die Behandlung von Arthritis, das ein verbessertes Wirksamkeit-gegenüber-Nebenwirkungs-Profil gegenüber Hydroxychloroquin, dem bedeutendsten Antimalaria-Arzneimittel in der DMARD-Klasse, aufweist, ein signifikantes klinisches Potential aufweisen würde.
- In Bezug auf die Wirkkraft gegen Malaria ist Mefloquin eines der wirkungsvollsten Arzneimittel, welches sowohl für Prophylaxe als auch Behandlung indiziert ist, und dieses weist besondere Nützlichkeit für eine Verwendung bei der Chloroquin-resistenten Malaria auf. Chloroquin ist die Hauptstütze der Antimalaria-Behandlung und -Prophylaxe gewesen, aber das Auftreten von Chloroquin-Resistenz bei Plasmodium falciparum, dem Stamm mit der höchsten Letalität, hat begonnen, dessen Nützlichkeit einzuschränken. So hat sich Mefloquin zur bevorzugten Verbindung für die Prophylaxe und Behandlung von bösartiger Malaria entwickelt.
- In Tiermodellen sind Mefloquin-Enantiomere hinsichtlich Wirksamkeit gegen Plasmodium-Spezies ausgewertet worden. Diese Untersuchungen kamen zu dem Schluss, dass es keinen Unterschied bei den Enantiomeren hinsichtlich der Wirkkraft gegen Malaria gab.
- Zusammenfassung der Erfindung
- Es ist überraschenderweise festgestellt worden, dass (+)-Mefloquin wirkungsvolle antirheumatische Eigenschaften aufweist. Die Verwendung des im Wesentlichen reinen Enantiomers kann die Wirksamkeit maximieren und unerwünschte Nebenwirkungen verringern. (+)-Erythro-Mefloquin ist ein wirkungsvollerer Inhibitor von Zytokinen, die an der Entzündungsreaktion beteiligt sind. (+)-Erythro-Mefloquin supprimiert die Proliferation von humanen Lymphozyten.
- Beschreibung der Erfindung
- Die Erfindung basiert, wenigstens zum Teil, auf der Entdeckung, dass Mefloquin ein breites Profil der Zytokin-Inhibition zeigt, was mit der RA-Therapie durch Antimalaria-Mittel konsistent ist. Zusätzlich ist gezeigt worden, dass die Isomere von Mefloquin gute Aktivität gegen Interleukin-8 (IL-8) zeigen. Sowohl Chloroquin als auch Hydroxychloroquin sind gegenüber IL-8 inaktiv, und dieses Zytokin ist an dem Fortschreiten der Entzündung und Gewebezerstörung, welche beim Fortschreiten von RA und OA auftreten, beteiligt. Dies ist ein wichtiger Aspekt des verbesserten Profils von Mefloquin-Isomeren bei der Behandlung von entzündlichen Leiden oder Zuständen. Zusätzlich haben, wie in Tabelle 1 gezeigt, die Isomere von Mefloquin verglichen mit Chloroquin und Hydroxychloroquin eine überlegene Aktivität gegenüber IL-2, einem Zytokin, welches an der Zerstörung von Bindegewebe bei RA und OA beteiligt ist.
- Dieses Inhibitionsprofil hat signifikante Aktivität in einem vorklinischen ex vivo-Assay der Gewebezerstörung in dem Rinder-Nasenknorpel-Modell gezeigt. Die Ergebnisse sind in Tabelle 2 gezeigt.
- Für eine Verwendung im Rahmen der Erfindung kann der Wirkstoff z.B. zusammen mit einem Träger, Vehikel oder Verdünnungsmittel formuliert und verabreicht werden durch Vorgehensweisen, die in diesem Fachgebiet bekannt sind, einschließlich jenen, die bereits für das Racemat vorgeschlagen worden sind. Geeignete Zusammensetzungen werden von der beabsichtigten Verabreichungsroute, die beispielsweise oral, topisch, nasal, rektal, sublingual, bukkal oder transdermal sein kann, abhängen. Es können Zusammensetzungen für eine länger andauernde, verzögerte, zeitlich abgestimmte oder unverzügliche Freisetzung verwendet werden.
- Die Menge des Mittels, die verabreicht werden sollte, kann leicht durch den Fachmann bestimmt werden unter Berücksichtigung der üblichen Faktoren, wie des Typs des Patienten, der Natur des zu behandelnden Leidens oder Zustands und des Verabreichungswegs. Die Menge des Enantiomers kann höher oder die gleiche sein wie jene für das Racemat oder kann abhängig von der Coverabreichung von anderen Arzneimitteln modifiziert werden.
- Leiden oder Zustände, die behandelt werden können, umfassen Leiden oder Zustände, bei welchen eine Knorpelzerstörung auftritt, entzündliche Leiden oder Zustände und jene, die durch IL-2 und IL-6 vermittelt werden, z.B. rheumatoide Arthritis, Asthma, Psoriasis, Arthritis psoriatica, Morbus Crohn, Reizkolon und systemischer Lupus erythematodes. Andere relevante Zustände oder Leiden sind Colitis ulcerosa, COPD (chronisch-obstruktive Lungener krankung) und Asthma. Der Patient kann eine Veranlagung für ZNS-Nebenwirkungen aufweisen und/oder kann sich einer gleichzeitigen Therapie mit einem anderen Arzneimittel unterziehen.
- Abhängig von den relativen Aktivitäten der individuellen Enantiomere kann es bevorzugt sein, eine Mischung, z.B. ein Racemat, oder im Wesentlichen ein Enantiomer zu verabreichen. Das gewünschte Enantiomer kann in einem wenigstens 50%-igen, 70%-igen, 90%-igen, 95%-igen oder 99%-igen Überschuss bezogen auf ein jegliches anderes vorliegen. Der Wirkstoff kann in einer jeglichen aktiven Form, z.B. als Salz oder Nicht-Salz, verwendet werden.
- Die Verwendung von (+)-erythro-Mefloquin ist bevorzugt. Es scheint, dass diese Verbindung besonders nützlich ist, um die gewünschte Wirkung ohne Gewebezerstörung bereitzustellen, und sicher in einer relativ hohen Dosierung verabreicht werden kann.
Claims (3)
- Verwendung von (+)-erythro-Mefloquin, welches im Wesentlichen frei von (–)-erythro-Mefloquin ist, für die Herstellung eines Arzneimittels für die Behandlung einer Entzündungskrankheit, ausgewählt aus rheumatoider Arthritis, Osteoarthrose, Arthritis psoriatica, Psoriasis, Morbus Crohn, systemischem Lupus erythematodes, Colitis ulcerosa, COPD (chronisch-obstruktiver Lungenerkrankung) und Asthma.
- Verwendung nach Anspruch 1, wobei die Entzündungskrankheit rheumatoide Arthritis ist.
- Verwendung nach Anspruch 1, wobei die Entzündungskrankheit Osteoarthrose ist.
