Description
- Psoriasis ist eine der weit verbreitesten chronischen Erkrankungen. Sie befällt etwa 2% der erwachsenen weißen Bevölkerung, wobei die Patienten in den Altersgruppen zwischen 20 und 50 Jahren die schwersten Symptome zeigen.
- Psoriasis ist durch eine deutlich beschleunigte Geschwindigkeit des Epidermalumsatzes gekennzeichnet. Anstelle der normalen Dauer von 28 Tagen vom Zeitpunkt der Zellteilung in den Basalschichten bis zum Abstoßen der Zellen aus der Stratum Corneum dauert es bei Psoriasis nur etwa 4 Tage.
- Die Ursachen und der Mechanismus der Entwicklung von Psoriasis sind unbekannt. Aus diesem Grund gibt es bis jetzt keine vollständig wirksame Behandlung für dieses Leiden. Es wurde eine große Zahl von Versuchen unternommen, angefangen von den sehr alten Versuchen auf der Basis von natürlichen Teersorten bis zu moderneren Versuchen unter Verwendung von Steroiden, Sporalen usw.. Teersorten sind bei der Verwendung unsauber und weisen lediglich eine begrenzte Wirkung auf. Ihre Kombination mit Schwefel und Salicylsäure ist nicht viel besser. Diese Therapie wird häufig durch Verwendung von UV-Bestrahlung entweder aus natürlichen Quellen (Sonnenschein) oder künstlichen Quellen (Lampen) ergänzt. Weitere verwendete Verbindungen sind Steroide, Azaribin, Methotrexat, Psoralen und Retinsäurederivate. Alle diese Verbindungen besitzen eine eher hohe Toxizität. Ihre Langzeitverwendung kann zu schädlichen Nebenwirkungen führen.
- Ein möglicher Ansatz für eine Therapie dieser Erkrankung besteht in dem Versuch, den Zellmetabolismus zu beeinflussen, der selbstverständlich in psoriatischen Zellen viel aktiver ist als in normalen Zellen.
- Vor einigen Jahren wurde eine neue Behandlung vorgeschlagen. Diese basiert auf der Verwendung von Fumarsäure in Form ihrer einfachen Mono- oder Diester oder ihrer Metallsalze. Die Behandlung basiert auf der Theorie, daß in den psoriatischen Teilen der Haut ein Ungleichgewicht im Dicarbonsäurezyklus vorliegt, das zu niedereren Gehalten an Fumarat führt. Diese Theorie scheint durch die Tatsache bestätigt zu werden, daß einige Aminosäuren, wie Glycin, in psoriatischer Haut, verglichen mit ihrem Gehalt in normaler Haut, in geringeren Mengen vorhanden sind. Da diese Aminosäuren auch aus dem Dicarbonsäurezyklus stammen, ist ihre Anwesenheit in geringeren Mengen eine weitere Bekräftigung der obigen Theorie.
- Mit der Verwendung von Fumaratestern und Salzen zur Behandlung von Psoriasis befassen sich eine Reihe von Patentanmeldungen. Die DE-A-2 530 372 (13.1.77) beschreibt die Verwendung von Fumarsäure, Fumarsäureestern, wie Monoethyl- und Monomethylfumarat, Dimethylfumarat, einigen Salzen der Monoester, wie dem Mangan-, Calcium-, Zink-, Eisensalz usw.. Alle diese Verbindungen können mit weiteren Bestandteilen, wie Weinsäure, Citronensäure, Zucker und inerten Füllstoffen vermischt werden. Einige dieser Rezepturen eignen sich zur internen Verwendung und einige zur externen (topischen) Applikation. Verwandte Anmeldungen (vgl. DE-A-2 840 498 (10.4.80) und DE-A-2 901 452 (17.7.80)) beschreiben die Zugabe von Glycin, 1-Methionin und 1-Cystein zu den obigen Gemischen von Fumaratestern und -salzen. Die neue EP-A-0 188 7419 (30.7.86) beansprucht die Verwendung von Fumaratestern, von Alkoholen mit 1 bis 8 Kohlenstoffatomen, Estern höherer Alkohole (C&sub6;-C&sub2;&sub4;), Metallsalzen der Monoester und Estern der Diole, von Glycerin und anderen hydroxylhaltigen Verbindungen. Eine weitere Patentschrift (die DE-A-3 232 883) erwähnt die Herstellung von Fumarsäuresalzen mit verschiedenen Caffein-8-ethern. Die Salze sind kristallin und lassen sich zur Herstellung von Tabletten, Kapseln usw. in Kombination mit Metallsalzen von Fumarsäureestern (vgl. oben) und ferner unter wahlweiser Zugabe von Aminosäuren, wie Cystein und Methionin, und von Vitamin C verwenden. Die DE-A-2 703 964 offenbart Arzneimittelzubereitungen zur Behandlung von Psoriasis, die Fumarsäure oder Derivate hiervon in Kombination mit Glycin und wahlweise mit sekundären Phosphaten umfassen.
- Bezüglich der Verwendung der obigen Stoffe bei der Therapie von Psoriasis bestehen ernste Probleme. Kurzkettige Fumaratester sind im allgemeinen reizende Materialien, die häufig beim Zusichnehmen einen unerwünschten Acidoseeffekt liefern. Metallsalze der Halbester werden im Magen rasch in die freien Säuren und das jeweilige Metallhydrochlorid umgewandelt. Dasselbe passiert mit dem Caffeinethersalz. Die Ester sind bei Raumtemperatur flüssig, so daß sie zur Umwandlung in Tabletten an relativ große Mengen inerter Träger adsorbiert oder mit relativ großen Mengen inerter Träger vermischt werden müssen.
- Des weiteren besitzen sie einen starken charakteristischen Geruch. Ferner ist ihre Toxikologie noch nicht eingehend untersucht. Gemäß einer mit Mäusen durchgeführten Untersuchung weist Monoethylfumarat und Dimethylfumarat bei oraler Gabe einen LD&sub5;&sub0;-Wert über 100 mg/kg auf. Monoethylfumarat war bei intraperitonealer Gabe toxischer (W. Raab, H&G Nr. 10 (1984)). Diese Fumaratester sind in hohem Maße hautreizend und können zu einer Kontakturticaria führen (Lahty et al., Contact Dermatitis 3, 139-140 (1985)).
- Zusammenfassend läßt sich sagen, daß sich Mono- und Diester von Fumarsäure bei der Behandlung von Psoriasis als wirksam erwiesen haben, wie das Experiment mit mehreren tausend Patienten zeigt (vgl. beispielsweise G. Schafer, Fumarsäuretherapie der Psoriasis, Ärztliche Praxis 30, 61, Seite 1757-58 (1978); ferner Selecta 15, Seite 1260-61 (1984)). Die Ester reizen das Verdauungssystem und die Haut und ihre Toxikologie wurde nicht klar untersucht. Ferner lassen sie sich schwierig zu Tabletten formulieren.
- Jüngste Studien haben gezeigt, daß in psoriatischer Haut der Gehalt an Glycin und Serin etwa 25% niedriger liegt als in normaler Haut (Thaler und Mitarbeiter, J. Invest. Dermatol. 75, 156-158 (1980); ferner Steinert und Mitarbeiter, Biochemistry of Normal and Abnormal Epidermal Differentiation, Herausgeber I.A. Bereinstein und M. Seiji, Tokyo University Press, Seite 391-406 (1980)). Dieser Mangel kann mit dem Fumaratungleichgewicht oder weiteren unbekannten Gründen in Verbindung stehen. Die Zugabe von Glycin als solche zu derartigen Rezepturen kann nicht viel zu der therapeutischen Wirkung beitragen, da dieses wasserlösliche Material rasch in allgemeine Metabolismusprozesse hineingelangt, so daß bestenfalls sein Wert dem von Nahrungsmittelzusatz ähnelt.
- Wir haben festgestellt, daß eine Verknüpfung von Aminosäuren, wie Glycin, Serin usw., mit Fumarsäure über chemische Bindungen, wie über Amidgruppen, zu Konjugaten führt, die eine sehr-hohe Wirksamkeit bei der Behandlung von Psoriasis aufweisen. Diese Konjugatverbindungen sind in den meisten Fällen stabile kristalline Feststoffe. Sie lassen sich leicht zu Tabletten, Salben oder ähnlichen galenischen Formen formulieren. Die Amidbindung ist bekanntermaßen hydrolysestabiler als eine Estergruppe (vgl. beispielsweise J. Marach, Advanced Organic Chemistry, 3. Ausgabe, Seite 339, J. Wiley & Sons, New York (1985)), so daß die Fumaramidoaminosäuren viel langsamer in das Fumarat und die freie Aminosäure umgewandelt werden. Sie werden ohne Schwierigkeiten durch das Verdauungssystem absorbiert, da bekannt ist, daß Amide gute Löslichkeitseigenschaften sowohl mit hydrophilen als auch lipophilen Verbindungen aufweisen.
- In ihrem breitesten Aspekt betrifft die vorliegende Erfindung folglich Zusammensetzungen und Verfahren zur Abgabe eines Fumarsäurerestes und einer oder mehrerer Aminosäuren an Menschen. Es wurde festgestellt, daß die erfindungsgemäßen Zusammensetzungen die Symptome von Psoriasis lindern. Ferner wurde festgestellt, daß die erfindungsgemäßen Zusammensetzungen bei oraler Verabreichung die Verdauung und den Appetit stimulieren und bei oraler oder topischer Verabreichung die Bräunungseffekte der Sonne verringern.
- Die erfindungsgemäßen Verbindungen umfassen Verbindungen der Formel:
- worin R¹ und R², die gleich oder verschieden sind, jeweils
- (a) ein Ester einer Aminosäure oder
- (b) eine OH-Gruppe bedeuten, wobei gilt, daß lediglich einer der Reste R¹ und R² für eine OH-Gruppe stehen kann und wobei die Aminosäuren unter Glycin, Serin, Valin, Histidin, Methionin, Threonin, Leucin, Isoleucin, Cystein, Cystin, Tyrosin, Prolin, Hydroxyprolin, Tryptophan, Asparaginsäure, Glutaminsäure, Lysin und Arginin sowie ihren Derivaten, beispielsweise Estern und Salzen, ausgewählt sind, oder pharmazeutisch akzeptable Salze derselben.
- In den Rezepturen kann das Fumaramid von Ethylglycinat oder von Natriumglycinat verwendet werden. Mit anderen Worten ist es möglich, die Carbonsäuregruppe der Aminosäure zur weiteren Veränderung der Löslichkeit und der anderen Eigenschaften der Verbindung zu verwenden. Da darüber hinaus Fumarsäure zwei Carbonsäuregruppen aufweist, ist es möglich, gemischte Amide, wie das Glycin/Serin-Fumaramid, herzustellen und zu verwenden.
- Die Aminosäureester der Verbindungen sind zweckmäßigerweise Alkylester mit 1 bis 4 Kohlenstoffatomen in der Alkylgruppe.
- Die erfindungsgemäßen Zusammensetzungen können die oben beschriebenen aktiven Verbindungen zusammen mit einem auf dem einschlägigen Fachgebiet bekannten, pharmazeutisch akzeptablen Träger enthalten. Die Träger können Vehikel zur sofortigen oder verzögerten Freigabe umfassen und in den verschiedensten, auch auf dem einschlägigen Fachgebiet bekannten Dosierformen vorliegen.
- Die erfindungsgemäßen Verfahren umfassen im breiten Rahmen ein Verfahren zur Abgabe von Resten von Fumarsäure und/oder Aminosäuren an Patienten durch Verabreichung der erfindungsgemäßen Zusammensetzungen entweder oral oder topisch, wie es die Umstände vorgeben. Die erfindungsgemäßen Zusammensetzungen können zur Linderung der Symptome von Psoriasis verwendet werden. Ferner können sie zur Stimulation des Appetits und zur Verringerung der Bräunungswirkungen der Sonne verwendet werden.
- Die erfindungsgemäßen Materialien sind nicht hautreizend. Vorläufige toxikologische Untersuchungen mit dem Diethylester von Diglycylfumaramid lieferten einen LD&sub5;&sub0;-Wert von mehr 5 g/kg bei oraler Gabe an Ratten. Die Amidkonjugate sind mild und nicht reizend. Glycin wird in einigen Rezepturen von Aspirintabletten mit dem Ziel der Verringerung der Reizung des Magens verwendet. Jede beliebige im Magen durch Hydrolyse des Amids gebildete Glycinmenge wirkt in dieser Hinsicht in der Tat in günstiger Weise.
- Die vorliegende Erfindung wird anhand des folgenden Beispiels veranschaulicht.
- Alle Mengen sind in Gewichtsteilen angegeben.
- Beispiel: Ethylester von Diglycylfumaramid (EGFA) 13,9 Teile Glycinetheresterhydrochlorid, 7,65 Teile Fumarylchlorid und 8,8 Teile Natriumhydroxid wurden in Wasser in der obigen Weise umgesetzt. Nach Reinigung ist das erhaltene Material ein schmutzigweißes Pulver. N (berechnet) 9,7; gefunden: 10,1.
Claims (18)
1. Verbindung der Formel:
worin R¹ und R², die gleich oder verschieden sind,
jeweils
(a) einen Ester einer Aminosäure oder
(b) eine OH-Gruppe bedeuten, wobei gilt, daß lediglich
einer der Reste R¹ und R² für eine OH-Gruppe steht und wobei
die Aminosäuren aus Glycin, Serin, Valin, Histidin, Threonin,
Leucin, Isoleucin, Cystein, Cystin, Methionin, Tyrosin,
Prolin, Hydroxyprolin, Tryptophan, Asparaginsäure,
Glutaminsäure, Lysin und Arginin ausgewählt sind,
oder ein pharmazeutisch akzeptables Salz derselben.
2. Verbindung nach Anspruch 1, wobei die Aminosäurereste R¹
und R² über eine Ainidbindung an den Fumarsäurerest gebunden
sind.
3. Verbindung nach Anspruch 1, wobei die Aminosäuren aus
Glycin und Serin ausgewählt sind.
4. Verbindung nach Anspruch 1, wobei es sich bei den
Aminosäureestern um C&sub1;-C&sub4;-Alkylester handelt.
5. Verbindung, ausgewählt aus den C&sub1;-C&sub4;-Alkylestern von
Diglycin-fumaramid, Glycin-serin-fumaramid, Di-serin-fumaramid
und Glycin-lauryl-fumaramid.
6. Verbindung, ausgewählt aus der Gruppe C&sub1;-C&sub4;-Alkylester
von Glycin-serin-fumaramid, Di-serin-fumaramid und
Glycinlauryl-fumaramid.
7. Der Diethylester von Di-glycyl-fumaramid.
8. Arzneimittelzubereitung, enthaltend eine aktive
Verbindung der Formel:
oder ein pharmazeutisch akzeptables Salz derselben,
wobei R¹ und R², die gleich oder verschieden sind, jeweils
(a) einen Ester einer Aminosäure oder
(b) eine OH-Gruppe bedeuten, wobei gilt, daß lediglich
einer der Reste R¹ und R² für eine OH-Gruppe steht und wobei
die Aminosäuren aus Glycin, Serin, Valin, Histidin, Threonin,
Leucin, Isoleucin, Cystein, Cystin, Methionin, Phenylalanin,
Tyrosin, Prolin, Hydroxyprolin, Tryptophan, Asparaginsäure,
Glutaminsäure, Lysin und Arginin ausgewählt sind,
sowie einen pharmazeutisch akzeptablen Träger.
9. Zubereitung nach Anspruch 8 zur Verabreichung per os.
10. Zubereitung nach Anspruch 8 zur topischen Applikation.
11. Zubereitung nach Anspruch 8, 9 oder 10, wobei der aktive
Bestandteil aus der Gruppe Ester von Mono-glycyl-fumaramid,
Di-glycyl-fumaramid, Mono-serin-fumaramid und
Di-serin-fumaramid ausgewählt ist.
12. Arzneimittelzubereitung nach Ansprüchen 8 bis 11 zur
Linderung der Symptome von Psoriasis und anderer
Hautkrankheiten.
13. Arzneimittelzubereitung nach Anspruch 8 zur Anregung der
Verdauung oder des Appetits.
14. Arzneimittelzubereitung nach Anspruch 8 zur Verminderung
des Bräunungseffekts bei Sonneneinwirkung.
15. Arzneimittelzubereitung nach Anspruch 10 zur Abgabe der
Reste von Fumarsäure und/oder einer oder mehrerer
Aminosäure(n) an Menschen.
16. Verwendung einer Zubereitung nach Anspruch 10 zur
Herstellung eines Medikaments zur Behandlung von Psoriasis.
17. Verwendung einer Zubereitung nach Anspruch 13 zur
Herstellung eines Medikaments zur Anregung der Verdauung oder
des Appetits beim Menschen.
18. Verwendung einer Zubereitung nach Anspruch 14 zur
Verminderung des Bräunungseffekts bei Sonneneinwirkung.
