Description
- Diese Erfindung bezieht sich vorrangig auf die Behandlung von bestimmten Krankheiten und Störungen auf dem Gebiet der Humanmedizin, jedoch nicht ausschließlich dort, und auf Zusammensetzungen für die dortige Verwendung.
- Die essentiellen Fettsäuren (EFAs) bestehen aus zwei Typen, der n-3 (oder Omega-3) Serie, die von Alpha- Linolensäure erhalten wird und der n-6 (oder Omega-6) Serie, die von Linolsäure erhalten wird. Linolsäure und Alpha-Linolensäure sind unter dem Gesichtspunkt wie Vitamine, daß sie nicht im Körper produziert werden können und daher durch die Nahrung zugeführt werden müssen. Der Körper kann sie entlang der Pfade unten metabolisieren und es wird angenommen, daß ein derartiger Metabolismus wesentlich ist, wenn sie ihre Funktionen erfüllen sollen. Die Pfade sind: n-6 n-3 Struktur Octadecadiensäure Octadecatriensäuren Octadecatetraensäure Eicosatriensäure Eicosatetraensäuren Docosatetraensäure Docosapentaensäure Docosahexaensäure
- Die Säuren sind in den natürlichen Gesamt-cis Konfigurationen. In der n-6 Serie sind übliche Namen für die 18 : 2, 18 : 3, 20 : 3, 20 : 4 und 22 : 4 Säuren Linolsäure, Gamma-Linolensäure (GLA), Dihomo-Gamma- Linolensäure (DGLA), Arachidonsäure (AA) und Adrensäure. In der n-3 Serie wird nur Alpha-Linolensäure (18 : 3) üblicherweise durch einen nicht systematischen Namen benannt.
- Die Verlängerungsstadien in dem natürlichen Stoffwechselpfad sind viel schneller als die Desaturationen. Die Sequenz, von der angenommen wird, daß sie durch gewöhnliche Enzyme in zwei Pfaden vermittelt wird, ist: n-6 n-3 Desaturase Elongation
- Die n-3 Säuren werden bevorzugt metabolisiert und als ein Resultat sind die Plasmastände von 18 : 3 sehr niedrig und 18 : 4 und 20:4 gibt es lediglich in Spuren. Im Gegensatz dazu sind die n-6 Säuren insgesamt normal aufspürbar, wobei 18 : 3 einen geringen Stand hat und anscheinend schneller zu 20:3 konvertiert wird als ihre relativ langsame Herstellung aus 18 : 2 beträgt. Aus dem Journal of Holistic Medicine, Vol. 3, No. 2, (1981), ist es bekannt, daß GLA und andere n-6 Serien essentielle Fettsäuren eine Wirkung auf Prostaglandin E1 Niveaus in dem Körper haben, womit therapeutische Vorteile verbunden sind.
- Atopie ist eine vererbte Tendenz dazu, eine oder mehrere Gruppen von Krankheiten zu entwickeln, von denen Ekzeme, Asthma und allergische Rhinitis die größeren sind. Einige Leute haben nur Ekzeme, andere haben nur Asthma oder Rhinitis, andere haben zwei oder mehrere dieser Zustände, andere haben keine, können aber wiederum Kinder haben, die eines dieser Probleme haben. Atopie ist verbunden mit abnormalen Immun-Reaktionen und es wird im allgemeinen angenommen, daß man die vererbte Abnormalität bei Immunreaktionen haben muß, um eine klinische Krankheit zu erhalten, gekoppelt mit dem Aussetzen auf irgendeinen Umweltfaktor, der diese Abnormalität in Erscheinung bringt.
- Leute mit atopischer Vererbbarkeit, die das Glück haben, nicht einem Umweltfaktor ausgesetzt zu werden, werden keine offensichtliche Krankheit entwickeln, aber können die Abnormalität auf ihre Kinder vererben. Die genaue Art und Weise der Vererbung ist noch nicht genau bekannt. Eine Anzahl von Allergenen und Umweltfaktoren, die eine klinische atopische Krankheit auslösen können, sind bekannt, aber es ist wahrscheinlich, daß viele andere nicht bekannt sind. Epidermiologische Obersichten haben gezeigt, daß Migräne, Crohn's Krankheit, ulcerative Colitis, Otitis Media und das nephrotische Syndrom in beträchtlicher Weise häufiger bei Leuten mit Atopie vorkommen, als bei Leuten ohne Atopie. Das Vorhandensein von Atopie erhöht beträchtlich das Risiko eines dieser Zustände zu bekommen.
- Wir haben herausgefunden, daß Menschen mit atopischen Ekzemen eine Störung bei der Umwandlung von Linolsäure zu GLA aufweisen. Während die Linolsäure Niveaus normal oder erhöht sind, sind die Niveaus von DGLA und AA niedrig, wodurch gezeigt wird, daß eine Blockade in dem ersten Schritt des n-6 Stoffwechselpfades vorhanden ist. Die Darreichung von GLA in Form von Nachtkerzenöl (evening primrose oil) korrigiert zum Teil die DGLA und AA Defizite.
- Wir haben ebenfalls herausgefunden, daß Patienten mit schmerzhaften knotigen Brüsten, die nicht durch Brustkrebs verursacht worden sind (gutartige Brusterkrankung) normale oder erhöhte Niveaus an Linolsäure und geringe Niveaus an DGLA, AA, 22 : 4 n-6 und 22 : 5 n-6 aufweisen. Die Darreichung von GLA in Form von Nachtkerzenöl korrigiert teilweise die biochemischen Defizite und schafft eine klinische Verbesserung besonders bei solchen Patienten, deren Zustand der Brust sich vor jeder Menstruationsperiode verschlechtert hat. Patienten, die Brustprobleme hatten, die keine Beziehung zur Menstruation aufwiesen, zeigten eine geringere Reaktion.
- Wir haben darüber hinaus Blutproben analysiert von Patienten, die atopisches Ekzem und gutartige Brusterkrankung hatten, und wir haben herausgefunden, daß während Alpha-Linolensäure Niveaus normal sind, die Niveaus der Langketten n-3 Säuren, 20 : 5 n-3, 22 : 5 n-3 und 22 : 6 n-3 niedrig sind, besonders bei Patienten, deren Brustkrankheit nicht in Beziehung zur Menstruation stand. - Das Bestimmungsverfahren war:
- Blutproben (1 ml) wurden mit Chloroform:Methanol (2 : 1) extrahiert. Das Extrakt wurde durch Sodium Sulphat gefiltert, bis zur Trockenheit evaporiert und in 0.5 ml Chloroform:Methanol aufgenommen. Die Lipidfraktionen wurden durch Dünnschichtchromatographie auf Silica-Gel-Platten separiert. Die Phospholipidfraktion, die genommen wurde, da sie am genauesten die Veränderungen essentieller Fettsäure widerspiegelt, wurde unter Verwendung von Boron Trifluorid-Methanol mthyliert. Die resultierenden Methylester der Fettsäuren wurden separiert und unter Verwendung eines Hewlett Packard 5880 Gas Chromatograph mit einer sechs Fuß Säule, die mit 10% Silar auf Chromosorb WAW 106/230 gepackt war, gemessen. Das Trägergas war Helium (30 ml/min). Die Ofentemperatur wurde programmiert, um von 165ºC auf 190ºC bei 2ºC/min. anzusteigen. Die Detektortemperatur betrug 220ºC und die Injektortemperatur 200ºC. Die Retentionszeiten und Peak- Bereiche wurden automatisch durch einen Hewlett Packard Level 4 Integrator berechnet. Peaks wurden aufgrund eines Vergleiches mit Standardfettsäure- Methylestern identifiziert. Da Patienten mit diesen Störungen nicht nur einen Mangel an Langketten EFAs der n-6 Serien haben, sondern ebenfalls an Langketten EFAs der n-3 Serie, erfordert eine gesamte Korrektur der Symptome eine Korrektur beider Defizite. Es ist unsere Ansicht, daß solche kombinierten Defizite ein allgemeines Merkmal von Patienten mit atopischen Störungen und Störungen verbunden mit Atopie sind, von Patienten mit gutartiger Brusterkrankung, von Patienten mit Brustkrebs (der häufiger auftritt, wenn es eine vorher bestehende nicht krebsartige Brusterkrankung gibt) und Krebserkrankungen im allgemeinen, und von Patienten mit anderen Störungen, die mit Defekten in dem Stoffwechsel der n-6 Fettsäuren verbunden sind. Derartige Störungen schließen Alkoholismus und Diabetis ein, die sich in die Umwandlung von Linolsäure zu GLA einmischen. Es gibt z. B. ein starkes Anzeichen dafür, daß die Umwandlung von Linolsäure zu GLA in Krebszellen eingeschränkt oder insgesamt abwesend ist. Darüber hinaus wandelt in normalen Geweben das gleiche Enzym Linolsäure zu GLA um, wie es Alpha-Linolensäure desatoriert, um 18 : 4 n-3 zu bilden. In beiden Fällen ist die Aktivität eine Delta-6-Desaturase Aktivität und das Vorhandensein von einer der Säuren hemmt die Desaturation der anderen. Daher mangelt es Krebszellen und jeden anderen Zellen, wo die Delta-6-Desaturase Aktivität gering ist und insbesondere bei solchen von alkohol- und diabetiskranken Patienten an den Metaboliten der Alpha-Linolsäure.
- Ohne die Begrenzung auf irgendeine Theorie folgt aus der obigen Diskussion, daß es zwei grundlegende Oberlegungen gibt.
- A. Korrektur des Mangels. Wenn die Niveaus von normalen Körperbestandteilen in einer Krankheitsphase unterhalb normal sind, dann ist es eine vernünftige Annahme, daß diese niedrigen Niveaus dazu beitragen und möglicherweise die Krankheiten verursachen und daher sollten sie korrigiert werden. Wir haben gezeigt, daß anormale Niveaus der Hochfettsäuren in der Tat in den hier besprochenen Bedingungen vorhanden sind.
- B. Korrektur eines 1-Serie/2-Serie PG Gleichgewichtes. Aus vielen früheren Patentanmeldungen und anderen Publikationen der Arbeit des Erfinders scheint es, daß die Wirkungen der 1-Serie PG's und anderer Stoffwechselprodukte, die aus DGLA erhalten werden, beinahe sämtlich wünschenswert oder neutral sind, aber daß die Wirkungen der 2-Serie PG's und anderer Stoffwechselprodukte, die von Arachidonsäure erhalten werden, sehr gemischt sind, wobei einige wünschenswert sind und einige in hohem Maße unerwünscht sind. Studien der Wechselwirkungen zwischen dem Stoffwechsel der n-6 Säuren und dem der n-3 Säuren haben gezeigt, daß Elongationsreaktionen (z. B. GLA zu DGLA) hochwirksam sind und daß es nur eine geringe Konkurrenz in dem einen oder anderen Art gibt. Im Gegensatz dazu konkurrieren die beiden Serien von Fettsäuren miteinander wirksam um die Desaturationsprozesse. Das bedeutet, daß die n-3 Fettsäuren sowohl mit der Delta-6 als auch Delta-5 Desaturation in die n-6 Serie einmischen. Diese Konkurrenz scheint selbst dann vorzukommen, wenn die n-3 Fettsäure nicht tatsächlich ein Substrat für das beteiligte Enzym darstellt. Zum Beispiel hemmt 20 : 5 n-3 im Sinne einer Konkurrenz die Delta-6 Desaturation. Eine Konsequenz davon ist, daß das Vorhandensein von n-3 Fettsäuren in einer Kombination zu einer gewissen Hemmung der Umwandlung von DGLA zu Arachidonsäure durch die Delta-5 Desaturase führt. Als ein Resultat des Vorhandenseins der n-3 EFAs wird die Wirksamkeit von GLA oder DGLA bei dem Anstieg der Beziehung von DGLA Produkten zu Arachidonsäure-Produkten deswegen erhöht. Die vorliegende Erfindung ergänzt daher frühere Patente des Erfinders, bei denen das 1-Serie/2-Serie PG Gleichgewicht betont wurde, wobei in Anbetracht dessen GLA und DGLA die bevorzugten n-6 Säuren zur Verwendung in der vorliegenden Erfindung sind.
- In Anbetracht der oben ausgeführten Diskussion kann die vorliegende Erfindung folgendermaßen zusammengefaßt werden:
- Eine pharmazeutische oder diätetische Zusammensetzung zur Verwendung bei der Behandlung von Brust- oder anderem Krebs, umfassend die effektiven Mengen einer oder mehrerer der Metaboliten von Linolsäure (GLA, DGLA, AA, 22 : 4 n-6 oder 22 : 5 n-6) und einem oder mehreren der Metabolite von Alpha-Linolensäure (18 : 4 n-3, 20 : 4 n-3, 20 : 5 n-3, 22 : 5 n-3 oder 22 : 6 n-3) als solche oder in Form eines Esters, Salzes, Ami des oder anderen Derivates, das sich in dem Körper dazu umwandelt, allein oder in einem akzeptablen pharmazeutischen Carrier oder Verdünner.
- Die Säuren können gereicht werden in der Form der Säure selbst oder als ein Ester, Amid, Salz oder irgendeinem anderen funktionalen Derivat, das geeignet ist, zu einer biologisch aktiven Form der Säure innerhalb des Körpers umgewandelt zu werden und kann von natürlichen oder synthetischen Quellen stammen.
- Die meisten Studien haben gezeigt, daß der Stoffwechsel der n-3 EFAs effizienter ist als der der n-6 EFAs. Bei der Beurteilung ihres Vorhandenseins in verschiedenen Körpergeweben sind die wichtigen n-3 EFAs daher 20 : 5 n-3 und 22 : 6 n-3. Daher wird als besonders wünschenswert die Verwendung von Kombinationen von 18 : 4 n-3 oder 20 : 4 n-3 angesehen, welche auf 20:5 n-3 und 22 : 6 n-3 angehoben werden können, oder von diesen Säuren selbst mit GLA oder DGLA.
- Die Dosen für jede Säure rangieren von 1 mg bis 50 g täglich, vorzugsweise 50 mg bis 5 g täglich, üblicherweise in gewöhnlichen Gelatinekapseln.
- Konventionelle Quellen der Säuren schließen Nachtkerzenöl (Oenothera) ein, das ca. 9% GLA enthält und Öle von See- oder Zugfischen, die beträchtliche Mengen von 20 : 5 n-3 und 22 : 6 n-3 enthalten.
- Geeignete physiologisch funktionale Derivate, die in dem Körper zu den Säuren umwandelbar sind, um in die oben beschriebenen Stoffwechselpfade zu gelangen, sind physiologisch akzeptable Salze, Ester (insbesondere Glyzeride und einfache C&sub1; bis C&sub4; Alkyl Ester) Amide und Phospholipide. Indirekte Identifikation von brauchbaren Derivaten ist gegeben, indem im Körper die Säure selbst die vorteilhafte Wirkung zeigt, jedoch kann die Umwandlung direkt durch Gas- Chromatograph-Analyse von Konzentrationen im Blut, Körperfett oder anderem Gewebe durch Standardtechniken, wie z. B. von Pelick et al. p. 23 "Analysis of Lipids and Lipoproteins" Ed. Perkins, American Oil Chemists Society, Champaign, Illinois, U.S.A. gezeigt werden.
- Die Erfindung wird im wesentlichen beschrieben anhand der Verwendung von pharmazeutischen Zusammensetzungen, aber es ist so zu verstehen, daß die Gamma-Linolen und andere Säuren, die diätetische Ergänzungen sind, in eine diätetische Margarine oder andere Lebensmittel eingesetzt werden könnten: Die Verwendung von derartigen Lebensmitteln, die möglicherweise andere aktive Materialien beinhalten können und im wesentlichen in dieser Beschreibung als diätetische oder pharmazeutische Zusammensetzungen genannt werden, sind innerhalb des Bereiches der Erfindung und daher innerhalb des Begriffes Pharmazeutische Zusammensetzungen od. dgl., der in den Ansprüchen verwendet wird.
- Es ist zu verstehen, daß da, wo eine Störung derart auftritt, die eine Behandlung von Tieren verlangt, die Erfindung, die im wesentlichen anhand der Humanmedizin und Behandlung beschrieben wurde, gleichfalls auf dem Gebiet der Veterinärmedizin anwendbar ist.
- Die Zusammensetzungen, die gemäß der Erfindung verwendet werden, gibt es üblicherweise in einer geeigneten Form für die orale, rektale, parenterale oder lokale Darreichung in einem geeigneten pharmazeutischen Vehiculum, wie im Detail z. B. in der englischen Patentschrift 1 082 624 beschrieben wurde und in jedem Fall der im allgemeinen sehr bekannt ist für jede bestimmte Art der Zubereitung. So können z. B. Tabletten, Kapseln, einnehmbare Flüssigkeit oder Puderzubereitungen, Cremes und Lotionen für die lokale Anwendung oder Zäpfchen zubereitet werden, wie es jeweils verlangt wird. Injizierbare Lösungen von hydrolysiertem Oenotheraöl können zubereitet werden unter Verwendung von Albumin, um die freie Säure zu lösen.
- Vorteilhafterweise wird ein Konservierungsmittel den Zubereitungen beigefügt. Es wurde herausgefunden, daß Alpha-Tocopherol in einer Konzentration von ca. 0.1 Gew. % für diesen Zweck geeignet ist.
- Es ist zu verstehen, daß die absolute Quantität aktiver Zutaten, die in jeder Dosierungseinheit vorhanden ist, nicht das angemessene der Rate und der anzuwendenden Darreichungsart übersteigen soll, aber daß andererseits es ebenfalls wünschenswert ist, daß die gewünschte Darreichungsrate aufgrund einer geringen Anzahl von Dosen erzielt wird. Die Darreichungsrate wird darüber hinaus abhängig sein von der präzisen pharmakologischen gewünschten Wirkung.
- Im nachfolgenden werden spezifische Beispiele der Erfindung genannt:
- Eine Kapsel, enthaltend 60% Nachtkerzenöl und 40% Makrelenöl, verabreicht in 0.5 g Kapseln sechs mal täglich für die hierin aufgeführten Umstände.
- Eine Kapsel, enthaltend 70% Nachtkerzenöl und 30% Lachsöl, verabreicht in 0.5 g Kapseln acht mal täglich für die hierin aufgeführten Umstände.
- Eine 0.25 g Kapsel, enthaltend 150 mg GLA und 100 mg 20 : 5 n-3, verabreicht drei mal täglich für die hierin aufgelisteten Umstände.
- Eine Kapsel, enthaltend 50 mg DGLA, 50 mg AA, 20 mg 22 : 4 n-6, 20 mg 22 : 5 n-6, 50 mg 20 : 5 n-3, 20 mg 22 : 5 n-3 und 20 mg 22 : 6 n-3, eingenommen vier mal täglich für die hierin aufgelisteten Umstände.
Claims (3)
1. Die Verwendung von einem oder mehreren der Metabolite
der Linolsäure (GLA, DGLA, AA, 22 : 4 n-6 oder 22 : 5 n-6)
zusammen mit einem oder mehreren der Metabolite der
Alpha-Linolensäure (18 : 4 n-3, 20 : 4 n-3, 20 : 5 n-3, 22 : 5 n-3
oder 22 : 6 n-3), als solche oder in der Form eines Esters,
Salzes, Armides oder anderen Derivates, das in dem
Körper dazu umwandelbar ist, und alleine oder in einem
akzeptablen pharmazeutischen Carrier oder Verdünner,
für die Zubereitung eines Medikamentes, das durch eine
pharmazeutische oder diätetische Zusammensetzung

die Zusammensetzung in einer Dosierungseinheitsform
ist, die zur Verabreichung von 1 mg bis 50 g/täglich,
vorteilhafterweise 50 mg bis 5 g/täglich von jeder
besagten Säure geeignet ist.
