发明背景
自身免疫疾病包含许多目前最具破坏性和难以处理的疾病,其中代表性的自身免疫疾病包括糖尿病、葡萄膜视网膜炎和多发性硬化等等。

本发明的特定实施方案的详细描述
本发明提供了产生富含预定的自身抗原特异性调节T细胞的组合物的方法、所得组合物及其使用方法。在一个实施方案中,本发明提供了一种调节对象体内自身免疫反应的方法,所述方法包括:(a)获得对象相容的细胞群;(b)从所述细胞群中产生富含自身抗原特异性调节T细胞、优选预定的自身抗原特异性调节T细胞的组合物;以及(c)将所述组合物导入所述对象中以调节所述对象体内的所述自身免疫反应。
在特定的实施方案中,所述细胞群得自所述对象、得自与所述对象不同的供体、和/或从外周血中收获。获得的细胞群包含自身抗原特异性调节T细胞(Treg),可以衍生自其中存在自身抗原特异性Treg细胞的任何来源,如外周血、胸腺、淋巴结、脾和骨髓。在某些实施方案中,Treg细胞来源于尸体组织。

所述产生步骤从所述细胞群中提供了一种富含预定的自身抗原特异性调节T细胞的组合物,优选地特异于与靶定的自身免疫反应相关的预定的自身抗原,优选地预定与靶定的自身免疫反应相关。在特定的实施方案中,所述产生步骤包括从所述获得的细胞群中扩增所述抗原特异性调节T细胞,和/或富集所述自身抗原特异性调节T细胞。

在特定的实施方案中,所述扩增通过将细胞群与自身抗原特异性调节T细胞刺激组合物接触而实现。所述自身抗原特异性调节T细胞优选被扩增至少50倍,优选至少100、200、300、500和800倍。自身抗原特异性调节T细胞刺激组合物促进表达识别希望的自身抗原的T细胞受体的自身抗原特异性调节T细胞的存活、生长和/或扩增。
优选的刺激组合物通过抗原特异性结合并激活T细胞受体复合物而刺激T细胞。可以使用各种抗原特异性TCR结合试剂,包括交联的肽结合的MHC分子、抗体和模拟物。在一个优选的实施方案中,所述组合物包含II类MHC/自身抗原性肽复合物,特别是这种MHC/肽复合物的聚集体。这些复合物至少包含MHC II类分子的胞外肽结合结构域,其中功能性结合了自身免疫原性肽。所述复合物可以在溶液或者悬浮液中,或者固定在基质上,如存在于细胞特别是APC表面上。本领域已知许多可用于产生功能性II类MHC/肽复合物的方法,如可以见于文献中。

在另一个实施方案中,所述自身抗原性肽是能与MHC II类分子复合的模拟表位肽。模拟表位肽在下面的文献及在实施例1中描述。使用自身抗原肽从其它常规T细胞中扩增Treg的方案包括使用自身抗原特异性MHC-肽四聚物、肽脉冲的DC(Yamazaki,et al,2003,JExp Med 198:235-47)或者人工APC(Maus et al.Nat.Biotechnol.20:143-8,2002)以不依赖于细胞表面表型的方式从患者中扩增Treg。另外,组合体外和体内方法可增强治疗效力。例如,近来的研究示出给予自身抗原、改变的肽配体及甚至非特异性刺激如FcR未结合性抗-CD3 mAb可以促进抗原特异性Treg活性(Apostolou et al.J.Exp.Med.199:1401-8,2003;Belghith et al.Nat.Med.9:1202-8,2003)。因此,将体内免疫诱导Treg与离体扩增组合是有益的,反之亦然。
在某些实施方案中,所述刺激组合物可进一步包括一或多种额外的物质,例如共刺激剂、第二种调节T细胞刺激剂,或者通常促进T细胞存活和/或生长的物质。
在某些实施方案中,所述共刺激剂是特异于TCR共刺激物如CD28或者GITR的抗体或配体,如下文所描述。在特定的实施方案中,所述共刺激剂是激动剂抗体,如结合CD28的激动剂抗体。
另外,所述刺激组合物包含第二种调节T细胞刺激剂。举例性的刺激剂包括粒细胞集落刺激因子、白细胞介素如IL-2、IL-6、IL-7、IL-13和IL-15、及肝细胞生长因子(HGF)。在特定的实施方案中,所述第二种刺激剂是细胞因子,如白细胞介素,例如白细胞介素-2。

将扩增的和/或增强的自身抗原特异性调节T细胞导入对象中以调节自身免疫反应。例如,所述对象可患有特征在于具有进行性或复发性自身免疫反应的疾病或病症,如表A所列出的疾病/病症。在特定的实施方案中,所述调节包括抑制。Treg可作为“特洛伊木马”将抑制性或者其它生物学因子例如IL-4(Yamamoto et al.J Immunol.166:4973-80,2001)、干细胞生长因子、血管发生调节物、遗传缺陷等等输送至炎症部位。例如,foxp3的过表达已经示出将其它病原性T细胞转化为Treg(Jaeckel et al.Diabetes.2004 Dec 10;[Epub]),多克隆扩增的Treg可以用编码抗原特异性TCR的基因加上foxp3转导,以产生大量的和有效的强力抗原特异性Treg(Mekala,et al.,Blood.2004 Nov4;[Epub])。因此,这些抗原特异性方法在使Treg特异性和功能最佳的同时降低了对高细胞数的要求。
抗原特异性Treg应在这样的感染性疾病中特别提及:其中感染的病原性不是病原体的致细胞病变作用的结果,而是对感染因子的免疫炎症应答所致的组织损害的结果。在如丙型肝炎或者HSV诱导的角膜炎症的疾病中,Treg治疗提供了控制病毒诱导的免疫炎症疾病的一种独特的机会(Suvas et al.J.Immunol.172:4123-4132,2004)。病毒,如柯萨奇病毒,已知导致胰腺炎并且与I型糖尿病的产生相关。因此,靶定表达的病毒抗原的Treg可用于抑制感染所致的局部组织损害并减轻激发自身免疫疾病发生的炎症。

在特定的实施方案中,所述自身抗原特异性调节T细胞当给予对象时有效调节自身免疫反应。使用扩增的和/或富集的自身抗原特异性调节细胞的有效和最佳剂量及治疗方案通过现有T细胞输注治疗的大量临床经验获知,并可根据经验进一步确定。
本发明的方法被发现在治疗其中希望调节宿主的异常免疫应答的各种不同病变中有作用。宿主中异常的免疫应答是指对象中以自身免疫应答为特征的任何免疫反应(例如自身免疫疾病)。通常当对象的免疫系统将自身抗原识别为外源抗原时发生自身免疫应答,导致自身反应性效应免疫细胞产生。自身反应性效应免疫细胞包括来自各种谱系的细胞,包括但不限于细胞毒性T细胞、辅助T细胞和B细胞。虽然精确的机制不同,但患有自身免疫疾病的宿主中自身反应性效应免疫细胞的存在导致宿主的组织和细胞破坏,引起病理症状。本领域技术人员已知确定宿主中存在这种细胞及因此存在自身免疫疾病(如宿主中抗原特异性自身免疫疾病)的各种测试,并易于应用于本发明的方法中。感兴趣的方法包括但不限于:Autoimmunity.2003 Sep-Nov;36(6-7):361-6;J Pediatr Hematol Oncol.2003 Dec;25 Suppl 1:S57-61;Proteomics.2003 Nov;3(11):2077-84;Autoimmun Rev.2003 Jan;2(1):43-9中所描述的方法。
治疗是指至少达到宿主中与异常免疫应答相关的症状减轻,所述减轻以广义使用,是指至少与被治疗的病变相关的参数(例如症状)的量级降低。这样,治疗也包括其中病理学病变或者至少与之相关的症状完全被抑制的情况,例如阻止其发生或者停止例如终止,由此宿主不再患有该病变,或者至少不再具有该病变特征性症状。

所述扩增方法通常包括首先从人体或患者中提取混合的T细胞群,从该细胞群中分离富含Treg细胞的亚群。为了使效力最大化,将该亚群富集至至少90%、优选至少95%、更优选至少98%的Treg细胞,优选CD4+CD25+CD62L+Treg细胞。通常通过选择特异于免疫抑制性Treg的细胞表面标记并使用自动细胞淘选如荧光激活的细胞淘选(FACS)、固相磁珠等方法分离而富集细胞。为了增强富集,可以将阳性选择与针对包含特异于非Treg细胞类型的表面标记的细胞进行的阴性选择(例如去除携带CD8、CD11b、CD16、CD19、CD36和CD56的细胞)组合,如下文所例证。

IL-2典型地以重组形式存在,其中IL-2的有效量典型为200-2500 IU IL-2/ml。我们已经发现使用范围在500-2500、优选1000-2000 IU IL-2/ml的非常规升高的IL-2浓度增加扩增。亚群的靶Treg细胞优选被扩增至少50倍、优选地至少100、200或300倍。根据经验确定最大的扩增,并且根据细胞类型、温育条件等而有所不同。在示例性的实施方案中,发现最大扩增为大约300、500和800倍。
扩增的Treg细胞的抑制功能可以在体外或在体内检测。例如,在体外,扩增的Treg细胞可以示出对在存在携带Fc受体的细胞的条件下用抗CD3刺激的CD25-T细胞的增殖的抑制,或者对经放射处理的异源脾细胞刺激的CD25-T细胞的增殖的抑制。体内动物模型和人临床免疫抑制方案的合适实例在下文进一步描述。
在特定的实施方案中,TCR/CD3激活物和扩增的Treg细胞是自身抗原特异性的。例如,在特定的这种实施方案中,向被诊断患有糖尿病(见例如Mayfield et al.,Diagnosis and classification of diabetesmellitus:new criteria,Am Fam Physician.1998 Oct15;58(6):1355-62,1369-70)并且具有葡萄糖稳态如空腹血糖(FPG)、餐后血糖(PPG)和葡萄糖耐量(GTT)损伤指征的患者中导入有效量的离体扩增的Treg细胞,使得损伤的葡萄糖稳态得以改善,特别是其中所述改善选自FPG为110mg/dL或更低、2小时PPG为140mg/dL或更低、以及摄取75-g葡萄糖2小时后GTT为140mg/dL或者更低。因此,本发明提供了过继细胞免疫治疗的方法和组合物,所述方法包括向需要治疗的患者中导入有效量的本发明的离体扩增的Treg细胞。
这些应用通常包括再次导入从相同患者中提取的扩增的Treg细胞,尽管所述方法也可以用于过继细胞免疫治疗以治疗与移植相关的移植物抗宿主疾病,所述移植特别是使用衍生自供体组织的Treg进行的骨髓移植。
如在经论证的动物模型和人临床实验中所表明的,如本文所述扩增的Treg的过继转移有效抑制各种病原性自身免疫应答,包括糖尿病、GVHD、狼疮、类风湿性关节炎、银屑病、多发性硬化、退化性心脏病(例如Ziad Mallat,et al.Induction of a Regulatory T Cell Type 1Response Reduces the Development of Atherosclerosis in ApolipoproteinEBKnockout Mice,Circulation.2003 Sep 9;108(10):1232-7)、炎症性肠道疾病(克隆氏症(Crohn′s disease))等等,如在下文所示例。
在我们的过继细胞转移方案中,混合的T细胞群最初从靶供体中提取。根据应用,所述T细胞可以在疾病好转期间或者在疾病活动期间提取。典型地通过抽取全血并通过白细胞去除术(leukopheresis)收获粒细胞而提取。例如,大体积白细胞去除术(LVL)显示出使血液白细胞产量最大化。使用连续流式去除设备(Spectra,COBE BCT),收获物达到20×106细胞/L。通过在白细胞去除期间持续输入钙而避免低血钙症状。在大约100-300分钟的时间内收获相应于大约4个总血液体积的15-45升液体。
收获的淋巴细胞可以基于Treg特异性细胞标记如CD4、CD25和CD62通过流式细胞分选或者其它细胞分离技术而分离、如本文所述扩增、然后输入患者,以进行免疫抑制,其中所述患者典型地为细胞供体(除了在供体和受体不同的GVHD情况中)。或者,细胞可以在扩增之前和/或之后冷冻,以用于贮存和/或运输。对于抗原非特异性扩增,输入大约109-1011个Treg;对于抗原特异性扩增,治疗有效的输入典型地需要大约107-109个Treg细胞。
移植物抗宿主疾病(GVHD)

在共移植扩增的Treg细胞后,Tconv细胞的移植物抗肿瘤(GVT)活性也得以保持。通过临床特征和存活力、肿瘤生长和排斥情况评价GVHD。在BMT时间注射的20-luc/yfp细胞迁移至骨髓,导致具有肝脏和淋巴器官二级浸润的白血病(Edinger,et al.Blood 101,640-648(2003)。移植了来自C57BL/6动物的TCD BM并共注射了1×104A20-luc/yfp白血病细胞的BALB/c小鼠在第36天之前死于白血病,这通过随时间的生物发光成像(BLI)信号强度的增加而证明。BLI图像示出在移植后5天肿瘤细胞浸润肱骨、股骨和胸骨的骨髓,及在第15天之前浸润其它器官,包括脾。接受TCD BM和Tconv细胞的动物死亡甚至早于GVHD,但是示出A20-luc/yfp白血病的最初移植(engraftment),其肿瘤细胞分布与仅接受骨髓的对照组相似。相反,接受Tconv细胞和CD4+CD25+Treg细胞的大多数动物在60天的观察期间存活。无一动物示出白血病进展,尽管所有动物在第5天均示出来自骨髓的初始肿瘤信号,表明T细胞移植不干扰A20-luc/yfp细胞的移植,但是当动物通过供体CD4+CD25+Treg细胞保护而免于致死性GVHD时,实现了主动根除白血病细胞。这些数据表明扩增的CD4+CD25+Treg细胞对GVHD的抑制不消除过继转移的供体Tconv细胞的GVT活性。
多发性硬化(MS)

类风湿性关节炎(RA)

银屑病

额外实施例
实施例1:体外扩增的抗原特异性Treg细胞抑制自身免疫糖尿病
在此我们描述了一种从有自身免疫倾向的非肥胖型糖尿病(NOD)小鼠中扩增抗原特异性Treg的强有力的方法。在少于2周的时间内体外扩增直至200倍的Treg表达一种经典的Treg表型,该表型保留这个亚类所有典型特征,包括表达CD25、CD62L、FoxP3和GITR,及在体外和体内均具有抑制效应T细胞功能的作用。当与多克隆Treg相对比时,使用自身抗原特异性T细胞显著增强扩增的NOD Treg在体内抑制前糖尿病和糖尿病小鼠的糖尿病的能力。抗原特异性Treg有效抑制Treg-缺陷的CD28-/-小鼠中糖尿病的发展,在慢性糖尿病动物中阻断同种同基因胰岛移植排斥,与先前的报道相反,首次示出在新发作疾病的小鼠中Treg逆转糖尿病。因此,这是首次表明少量抗原特异性Treg在疾病发作后可逆转糖尿病,提供了对于自身免疫的一种新的细胞免疫治疗的方法。
调节T细胞从自身抗原特异性TCR转基因NOD小鼠中扩增。先前的研究示出在16-24周龄的NOD小鼠中,随着时间的过去Treg的数目和功能与临床疾病相关地减少。然而,治疗性使用这些细胞的能力通过少数细胞残留在血液循环和淋巴器官中而显示治疗不当(<5%的CD4+T细胞在NOD小鼠中,<2%的CD4+T细胞在T1D人体中)。另外,由于基于抗原特异性难以选择细胞,因此需要大量的细胞。因此,基于观察到在TCR转基因(Tg)小鼠中存在的这些细胞可以用共同固定的抗CD3和抗CD28抗体加上外源IL-2驱动进细胞周期,我们开发了一种用于快速和有效地扩增自身抗原特异性Treg的技术。用抗-CD3/抗-CD28包被的珠在存在IL-2的条件下培养的FACS纯化的NOD Treg在11天中扩增150-225倍。通常地,CD4+CD25-T细胞扩增更强(在多个实验中扩增范围是300-800倍)。因此,纯度>98%的CD4+CD25+CD62+T细胞被优选用于成功扩增Treg,因为CD25-CD4+或者CD8+T细胞的少量污染影响扩增Treg的能力。
先前的研究报道了分离自年轻的NOD小鼠的CD4+CD25+Treg抑制来自糖尿病NOD小鼠的Teff细胞将疾病转移至免疫缺陷的NOD小鼠的能力。然而,所述方法效率低,在这种背景下Treg的抑制作用需要0.5∶1或者1∶1比率的Treg∶Teff,这可能是由于抗原特异性Treg的低前体频率所致。因此,我们检验了来自两种不同的抗原特异性TCR Tg小鼠的Treg是否可以在体外使用与多克隆NOD Treg相同的方法扩增。BDC2.5 TCR Tg小鼠表达特异于在β细胞颗粒中表达的胰岛抗原的TCR,而GAD286 TCR Tg识别衍生自胰岛抗原谷氨酸脱羧酶(GAD)的肽。从BDC2.5和GAD286小鼠中纯化Treg并使用抗-CD3/抗-CD28加上IL-2混合物扩增。基于先前示出与TCRαβ反应的MHC-肽四聚体的有效染色,BDC2.5细胞表达转基因的TCRαβ,扩增的GAD286 Treg表达Tg TCRβ链。来自BDC2.5 TCR Tg小鼠的CD4+CD62L+CD25-和Treg可以使用固定的MHC-肽二聚体以相似效率扩增。这些结果表明在野生型小鼠和TCR Tg小鼠中均存在可以使用这个方案扩增的CD4+CD25+CD62L+群。
我们接下来通过流式细胞计量术、Western印迹和实时PCR检验扩增的Treg的表型。扩增的Treg与扩增的CD25-T细胞相比保持高水平的CD25表达,而CD62L的表达在这两种细胞类型中均保持较高。另外,定量PCR示出所有Treg与相似扩增的CD25-T细胞相比均表达高水平的SOCS2、PD-1和CTLA-4。另外,最近鉴别的标记神经毡蛋白(neuropilin)和TRAIL在扩增的Treg中也高水平表达。在扩增的Treg上观察到高水平的细胞表面GITR表达,然而这个先前鉴别的Treg标记在扩增的CD25-T细胞上也被诱导。应注意定量PCR研究是对5个单独扩增的Treg群(既包括多克隆也包括BDC2.5 TCRTg Treg)进行的,相关表达是高度可再现的。最后,我们检验了最近鉴别的Treg谱系标记-FoxP3。通过RT-PCR和Western印迹分析示出,扩增的Treg表达的FoxP3水平与在新鲜Treg中观察到的结果相似,显著高于在新鲜的或扩增的CD25-T细胞中观察到的结果。RNA表达(10倍)和蛋白质的量(20倍)与先前对新鲜Treg的研究一致,尽管在CD25-Teff细胞中FoxP3显然有一些增加,这表明培养条件在CD25-亚类中可诱导一些调节T细胞。
我们还检验了扩增的Treg分泌细胞因子的能力。与激活的CD25-T细胞不同,Treg不产生IL-2或者IFNγ,但是表达免疫抑制性细胞因子IL-10和TGFβ。因此,Treg的广泛激活和增殖不改变Treg的表型,其保持与CD25-T细胞亚类不同。
体外扩增的Treg的功能活性
先前的研究已经示出Treg可有效地抑制抗-CD3和脾APC刺激的CD25-T细胞的增殖性应答。扩增的NOD Treg有效抑制增殖性应答和包括IL-2和IFNγ在内的细胞因子产生。事实上,在多个实验中,扩增的Treg与新鲜的NOD Treg相比抑制作用明显更好。所述抑制通常在Treg∶Teff比率<1∶10时观察到。使用来自TCR Tg小鼠的扩增的Treg观察到相似结果,扩增的BDC2.5 Treg有效抑制BDC2.5以及多克隆NOD T细胞的增殖性应答。对Treg抑制作用机制的检测证实了其它研究,表明需要细胞与细胞之间的接触。尽管扩增的Treg表达明显水平的IL-10和TGFβ,但是通过向体外培养物中加入抗-IL-10、抗-TGFβ或者这两种抗体的组合不影响抑制活性。这些结果与许多Treg抑制模型一致,其中细胞与细胞的接触是在体外背景下免疫抑制的主要方式。
为了进一步评价扩增的Treg的抗原特异性并确定扩增的Treg是否是组成型抑制的,检测来自正常BALB/c小鼠的扩增的Treg抑制来自OVA特异性DO11.10 TCR Tg小鼠的T细胞的能力。将Treg和DO11.10 Tg Teff细胞在存在OVA抗原(仅激活Teff细胞)或者抗-CD3(既激活Teff也激活Treg)的条件下共培养。扩增的BALB/c Treg不抑制OVA肽刺激的DO11.10 T细胞的增殖性应答。然而,在低Treg∶Teff比率下,抗-CD3应答完全被抑制,支持了扩增的Treg缺乏组成型抑制活性及需要抗原特异性激活Treg以进行有效免疫抑制。这个结果也排除了这种很小的可能性,即细胞通过表达高水平CD25消耗可利用的IL-2而抑制培养物。
扩增的Treg的体内存活和激活
Treg在体内有效抑制免疫应答需要细胞迁移至合适位置、对抗原产生应答及长时间存活。我们最近已经观察到CD28/B7途径的封闭导致Treg在体内迅速丧失,并随后丧失关键的免疫调节。因此,我们检验了扩增的Treg在体内存活和增殖的能力。将来自NOD、BDC2.5和GAD286小鼠的扩增的Treg用CSFE标记并转移至正常的非淋巴细胞减少(non-lymphopenic)的NOD小鼠中。在转移后7天,处死小鼠并检测CSFE+细胞的数目作为存活和增殖的读数。从用来自不同品系小鼠的扩增的Treg转移的小鼠中回收大量的CSFE+细胞。事实上,在转移至少50天后观察到CFSE+Treg以及Thy1.1标记的Treg,其数目与以同样方式转移新鲜Treg观察到的结果相等。
接下来,我们分析了过继转移的Treg应答抗原及在体内增殖的能力。为了消除淋巴细胞减少症引起的增殖的潜力,将Treg移至正常小鼠中。基于CSFE稀释,Treg少量但明显地增殖。然而,在胰腺淋巴结(pancLN)中没有NOD Treg的选择性增殖表明在NOD Treg集合(repertoire)中没有明显数量的胰岛自身抗原特异性细胞。事实上,与在其它LN细胞中观察到的结果相比,在NOD pancLN细胞中扩增的Treg的量更小,这提示了NOD中缺失胰岛特异性Treg的可能性。与NOD Treg相反,来自BDC2.5 Tg小鼠的Treg在pancLN中广泛地和选择性地增殖和扩增,在7天的时间中分裂至少3-4次。令人感兴趣地,增殖的Treg减量调节CD62L表达。这是令人惊奇的,因为所述细胞在转移之前已经在体外经历多次增殖循环,并且保持高水平CD62L表达。与BDC2.5 Treg相反,GAD286 Treg在体内不增殖。先前的研究提示这两种TCR Tg小鼠在胸腺发育方面显著不同。BDC2.5Tg小鼠在胸腺中不对胰岛特异性进行阴性选择,但是产生少量但可再现的Treg,这些Treg被表明通过这些动物中残留的潜在效应细胞而阻断疾病。对比之下,GAD286 TCR Tg T细胞在胸腺中进行阴性选择,由此逃脱的细胞利用另外的TCRα链。尽管外周GAD286 TCRTg细胞在体外应答GAD肽,但是反应性较弱,并且与BDC2.5相反,这些细胞不能基于过继转移而诱导糖尿病,这表明“不存在”自身反应性部分。因此,使用扩增的Treg的这些结果提示BCD2.5 TCR Tg小鼠而非GAD286 TCR Tg小鼠具有循环的自身反应性Treg,其在体内留驻并存活,并且接受另外的信号以进一步激活和扩增抗原特异性亚类。
体外扩增的Treg抑制体内糖尿病的过继转移
接下来,我们检测了与激活的BDC2.5 T细胞体内共转移至NOD.RAG小鼠之后,扩增的BDC2.5 Treg抑制糖尿病的能力。Treg有效阻断糖尿病的转移,在低如1∶9的Treg∶Teff比率即可发挥作用,而GAD286 Treg甚至在Treg∶Teff为1∶1比率时也不发挥保护作用。事实上,扩增的BDC2.5 Treg抑制多克隆T细胞介导的疾病。少如2×106个扩增的BDC2.5 Treg阻断25×106个致糖尿病的NOD脾细胞转移疾病的能力。来自BDC2.5小鼠的扩增的抗原特异性Treg与扩增的多克隆NOD Treg相比在防止糖尿病发生方面有效得多,这与上述独特的增殖差异一致。与使用四分之一数目的抗原特异性BDC2.5Treg对疾病转移的总体阻断相比,多如8×106个扩增的NOD Treg仅在25%的致糖尿病细胞转移的RAG受体中预防糖尿病。这个结果与先前的结论一致,提示在这种背景下需要多克隆Treg∶Teff细胞的高比率,以有效抑制疾病转移。重要的是这些数据表明Treg的体外反应性不能预测其在体内对这种疾病的功能。
在非淋巴细胞减少症背景下,扩增的Treg在体内预防糖尿病。尽管有多个模型表明Treg的免疫调节活性,但许多系统基于利用淋巴细胞减少的小鼠增强Treg增殖的过继转移模型8,21-24。因此,我们检测了扩增的Treg在非淋巴细胞减少的动物模型中预防糖尿病的能力。先前的研究已经示出CD28-/-NOD小鼠具有正常数目的T细胞和Th1应答。事实上,这些小鼠由于Th2和极其依赖CD28的Treg中的缺陷而产生恶化的自身免疫。因此,我们检测了野生型扩增的BDC2.5 Treg是否能被转移进CD28-/-NOD小鼠中并且延缓或预防疾病的发生。将5×105个Treg转移进5周龄的CD28-/-NOD小鼠中并监测糖尿病情况。扩增的BDC2.5 Treg的转移在所有检测的小鼠中在长达转移后20周的时间内预防了糖尿病的发生。相反,转移扩增的NOD Treg对疾病发生无作用。这些结果表明抗原特异性扩增的Treg在体内针对完全功能性病原性T细胞应答起作用。
扩增的Treg在体内逆转糖尿病
Treg治疗的最终应用依赖于能治疗正处于疾病状态中的个体。因此,我们检测了这些扩增的Treg在糖尿病的明显(frank)模型中的调节作用。首先,我们检测了扩增的BDC2.5 Treg阻断同种同基因NOD胰岛移植物的排斥反应的能力。使用胰岛素沉淀至少2周使糖尿病NOD小鼠保持血糖量正常。此时为小鼠只移植500个同种同基因胰岛细胞或者再联合移植扩增的Treg。共转移2×106个BDC2.5而非5×106NOD,扩增的Treg阻断对同种同基因胰岛的排斥,这与抑制细胞阻断这种背景下的进行中的自身免疫的能力一致。更重要地,扩增的BDC2.5 Treg的过继转移在明显具有糖尿病的NOD小鼠中逆转糖尿病。在这种背景下,将1×107个Treg转移进基于血液葡萄糖水平升高(>300mg/dL)而诊断新近发生疾病的NOD小鼠中。转移的Treg在60%的检测小鼠中逆转糖尿病。因此,扩增的Treg在进行中的自身免疫背景下极其有效地阻断和逆转糖尿病。
我们观察到Treg能逆转糖尿病,这表明我们的方法在临床自身免疫治疗中的可应用性,其中Treg分离自疾病好转中的(例如SLE或MS)或者发病后不久(例如T1D)的患者。然后将细胞扩增并在疾病活性最大时重新导入以缓和炎症应答。我们发现这种治疗可以与Rapamycin、抗-CD3或者导致病原性细胞消除而不影响Treg的其它药物组合应用。这些治疗方法一起既降低了短期病原性应答,同时又恢复了用于长期耐受诱导的稳态平衡。
实施例2:功能性内源抗原特异性CD4+CD25+调节T细胞从NOD小鼠中的扩增:抗原特异性CD4+CD25+调节T细胞控制自身免疫糖尿病
CD4+CD25+Foxp3+调节T细胞(Treg)对于控制自身免疫是关键的。有证据提示Treg的产生和功能依赖于抗原特异性。尽管如此,有关抗原特异性Treg在天然背景下的产生的知识还是很少。在这个实施例中,我们鉴别和鉴定了识别胰岛肽模拟物及在非肥胖糖尿病小鼠中天然产生的Treg。抗原特异性Treg表达原型表面标记和细胞因子。尽管以抗原特异性方式激活,但是扩增的Treg在体外和体内均能进行旁观者抑制(bystander suppression)。重要的是,胰岛肽模拟物特异性Treg与多克隆Treg相比在抑制自身免疫糖尿病方面更有效。我们的描述论证了Treg可用作器官特异性自身免疫的治疗剂。
自身免疫I型糖尿病(T1D)是由于保持自身抗原耐受性的机制被破坏,导致T细胞介导的胰腺产生胰岛素的胰岛细胞被破坏所致。潜在病原性自身反应性T细胞存在于正常的外周T细胞集合(repertoire)中,但是在健康个体中部分受到抑制细胞或者调节T细胞(Treg)的控制(1,2)。在Treg类别中,CD4+CD25+Treg是对于控制自身免疫重要的独特细胞亚类(3,4)。在CD4+CD25+Treg或者Foxp3(控制CD4+CD25+Treg的产生和抑制功能的转录因子)中有缺陷的小鼠和人患有多器官自身免疫疾病(5-11)。降低或者削弱的CD4+CD25+Treg的抑制功能与T1D、多发性硬化、类风湿性关节炎及其它自身免疫疾病相关(12-16)。对比之下,多克隆CD4+CD25+调节T细胞的转移在许多系统、包括非肥胖型糖尿病(NOD)小鼠(一种T1D小鼠模型)的自身免疫糖尿病中防止自身免疫(17,18)。然而,所述方法效率低并且需要转移大量的Treg。我们最近描述了一种使用IL-2与用抗-CD3和抗-CD28mAb包被的珠的组合在体外扩增胰岛抗原特异性CD4+CD25+BDC2.5 T细胞受体转基因(TCR Tg+)Treg的方法(19)。与多克隆NOD Treg相比,使用明显较少数目的Treg,扩增的胰岛特异性Treg在NOD小鼠中有效阻断和逆转糖尿病,表明Treg的抗原特异性对于治疗效力是重要的。因此,有效的临床治疗依赖于从多克隆细胞群中鉴别和扩增相关的抗原特异性Treg的能力(20)。
关于在天然条件下CD4+CD25+Treg的抗原特异性了解的相对较少。在本发明的研究中,我们假想由于BDC2.5 TCR Tg+小鼠具有高百分比的胰岛抗原特异性Treg,因此这种特异性可能在常规的NOD小鼠中也存在(19,21,22)。因此,我们调整了在BDC2.5 Treg研究中使用的扩增方案,用呈递BDC2.5 TCR模拟表位肽1040-31(p31)的重组II类MHC I-Ag7取代抗-CD3 mAb(23)。使用模拟表位肽是因为内源BDC2.5抗原还未鉴别(24)。为了确定p31-I-Ag7珠是否可以从多克隆细胞群中扩增低频率抗原特异性细胞,将BDC2.5 TCRTg+Treg接种进来自NOD小鼠的多克隆CD4+CD25+Treg细胞中。p31-I-Ag7和抗-CD28包被的珠在存在外源IL-2的条件下极为有效地扩增CD4+CD25+BDC2.5 TCRTg+Treg。最初以0.1%BDC2.5 TCR Tg+Treg接种的培养物扩增大约4倍,而以0.01%和0.001%BDC2.5 TCR Tg+Treg接种的培养物不明显扩增。然而,使用p31-I-Ag7多聚体检测抗原特异性细胞的流式细胞计量分析表明BDC2.5 TCR Tg+Treg在所有培养物中均扩增。在最低接种密度情况中,BDC2.5 TCR Tg+Treg从占细胞群的0.001%生长至34.3%。这反映了在培养期间高于12次的细胞分裂,导致在10天培养期间抗原特异性细胞扩增接近5000倍。为了保证CD4+CD25+Treg在用肽-1-Ag7包被的珠扩增后保持调节活性,使用新鲜分离的CD4+CD25-BDC2.5 Tg+应答细胞组合扩增的CD4+CD25+BDC2.5 Tg+Treg的滴定进行抑制分析。在用BDC2.5模拟表位肽1040-31刺激的培养物中,扩增的CD4+CD25+BDC2.5 Treg以剂量依赖性方式有效抑制CD4+CD25-T细胞应答。另外,抑制活性在多次体外刺激后不丧失。最初以0.001%接种的及在用肽-I-Ag7珠刺激两次后扩增为50%的CD4+CD25+BDC2.5 Tg+细胞抑制p31肽刺激的CD4+CD25-BDC2.5 Tg+细胞。因此,即使当抗原特异性BDC2.5TCR Tg+Treg细胞代表极少百分比的总多克隆Treg群时,所述方法仍可以大量扩增保留抑制功能的抗原特异性Treg。
我们接下来应用这种方法从常规NOD小鼠中扩增抗原特异性CD4+CD25+Treg。将来自NOD小鼠的CD4+CD25+CD62L+细胞用p31-I-Ag7珠如材料及方法(Material and Methods)部分所述进行培养(25)。在7-14天期间,与最初输入的细胞相比总细胞群典型扩增7-10倍。流式细胞计量分析表明在用p31-I-Ag7珠扩增后,多至10%的CD4+CD25+细胞对于p31-I-Ag7多聚体染色阳性,而用抗-CD3包被的珠扩增的CD4+CD25+细胞对于p31-I-Ag7不呈现高于背景水平的染色阳性。在相同培养条件下,CD4+CD25-CD62L+T效应细胞(Teff)典型地扩增10倍,40-50%对于p31-I-Ag7多聚体染色阳性。然而,基于体外增殖分析,多聚体染色显然低估了p31-I-Ag7-反应性Treg细胞。将扩增的Treg细胞用羧基荧光素琥珀酰亚氨酯(CFSE)标记,并在存在抗原呈递细胞(APC)和抗-CD28条件下与p31肽或者对照卵清蛋白(OVA)肽一起培养。CFSE稀释分析结果示出50%以上的p31-I-Ag7培养的Treg进入细胞周期,相比之下在卵清蛋白刺激的培养物中背景增殖为14%。用OVA肽见到的高度的背景增殖可以反映出由于培养物中珠的持续存在导致所述培养物不是100%静止的。在刺激之前,当通过流式细胞计量分析p31-I-Ag7多聚体结合时,这个细胞群仅具有6.4%p31-反应性细胞。这些结果提示低亲合力的T细胞很难通过多聚体染色检测到,并且可以反映出TCR特异于与BDC2.5特异性的p31模拟表位不完全相同的内源抗原的事实。为了探究这种解释的正确性,我们检测了p31-I-Ag7-扩增的T细胞的Vβ集合(repertoire)。BDC2.5 T细胞受体表达衍生自Vβ4家族的TCRβ(26)。然而,当p31-I-Ag7扩增的Treg和Teff细胞用p31-I-Ag7多聚体和不同的TCRVβ试剂共染色时,这两个细胞群均不是单克隆的。相反,这两个细胞群示出具有一些Vβ群的广泛集合(repertoire)。令人感兴趣地,尽管在Treg培养物中有大量Vβ4+p31-I-Ag7多聚体+T细胞,但是在这个代表性培养物中也有许多其它TCR Vβ,例如分别占p31-I-Ag7多聚体+Treg的10.2%和13.7%的Vβ2和Vβ12。TCR Vβ4+T细胞通常存在于p31-I-Ag7多聚体+T效应细胞群中,但是百分比较低。这些结果一起表明针对胰岛肽模拟物反应的Treg和Teff细胞的广泛集合(repertoire)留驻在常规NOD小鼠中。结果还表明胰岛肽模拟物反应性Treg集合(repertoire)与胰岛肽模拟物反应性Teff集合(repertoire)不同。
如先前对于抗CD3扩增的培养物所观察到的结果,肽-I-Ag7-扩增的Treg在整个培养期间保留CD4+CD25+CD62L+表型,而以相似方式培养的CD4+CD25-CD62L+则相反。CD4+CD25-CD62L+细胞在激活后呈CD25high,但是在初始激活之后与p31-I-Ag7反应性Treg细胞相比轻微地减量调节CD25。大多数p31-I-Ag7-反应性T效应细胞在培养期间减量调节CD62L。我们还使用定量实时PCR检测了扩增的Treg和Teff细胞表达Treg谱系标记Foxp3的情况。为了保证p31-I-Ag7反应性细胞被分析,在分析之前通过荧光激活的细胞淘选(FACS)将p31-I-Ag7扩增的Treg分选为p31-I-Ag7-多聚体阳性和阴性群。在代表性实验中,扩增的p31-I-Ag7-多聚体+Treg与扩增的p31-I-Ag7-多聚体阳性T效应细胞相比表达大约多3000倍的Foxp3。当通过流式细胞计量术分析时,扩增的p31-1-Ag7Treg也表达最典型的Treg表面标记CTLA-4、ICOS和GITR。我们接下来检测p31-I-Ag7扩增的Treg在用抗原攻击后细胞因子的分泌情况。与先前对于Treg报道的数据一致,p31-I-Ag7扩增的Treg表达低水平的促炎细胞因子IL-2、IL-4和IFNγ,及表达高水平的抗炎细胞因子IL-10。
先前的研究示出CD4+CD25+Treg可以抑制CD4+效应细胞体外增殖,并且所述抑制作用依赖于CD4+CD25+Treg通过其TCR的刺激。因此,检测扩增的p31-I-Ag7Treg细胞在体外的抑制活性和特异性。当培养物用多克隆激活物抗CD3刺激时,扩增的p31-I-Ag7以剂量特异性方式有效抑制新鲜分离的多克隆CD4+T细胞和抗原特异性CD4+BDC2.5 TCR Tg+小鼠的增殖。更重要的是当用1040-31肽刺激培养物时,p31-I-Ag7 Treg抑制BDC2.5 TCRTg+CD4+T细胞的增殖,表明培养物中1040-31肽反应性细胞的特异性抑制。相反,扩增的CD4+CD25-p31-I-Ag7Teff不能抑制新鲜分离的BDC2.5 CD4+T细胞,并且导致增殖的增加。令人感兴趣地,p31-I-Ag7扩增的Treg而不是Teff细胞在没有CD28共刺激的情况下对刺激无反应性(对p31肽和抗-CD3均无反应性),这与对于新鲜分离的Treg的报道一致。为了进一步鉴定扩增的Treg的抗原特异性,评估扩增的多克隆Treg和p31-I-Ag7扩增的Treg通过抗CD3的多克隆T细胞激活或者抗原特异性T细胞激活而抑制BDC2.5 TCR Tg+或者谷氨酸脱羧酶肽286-特异性(GAD286)TCR Tg+CD4+细胞的能力(27)。当用抗CD3刺激时,多克隆Treg和p31-I-Ag7扩增的Treg均抑制BDC2.5 TCR Tg+应答细胞,而只有p31-I-Ag7扩增的Treg抑制用BDC2.5 1040-31肽刺激的培养物。相似地,当用抗CD3刺激时,多克隆Treg和p31-I-Ag7扩增的Treg均抑制GAD286 TCR Tg+CD4+T细胞的应答。然而,当用GAD(286-300)肽刺激时,多克隆Treg群和p31-I-Ag7扩增的Treg群均不抑制GAD286 TCR Tg+应答细胞。最重要地,当用GAD(286-300)和1040-31肽一起刺激培养物时,p31-1-Ag7扩增的Treg能抑制GAD286 TCR Tg+应答细胞。综合起来,这些数据表明肽-I-Ag7-扩增的Treg的抑制活性依赖于通过TCR的抗原特异性刺激,尽管一旦用同源抗原刺激则p31-I-Ag7扩增的Treg能发挥旁观者抑制作用。
然后我们测试了小数目p31-I-Ag7-扩增的Treg在CD28-/-NOD小鼠中抑制多克隆T细胞介导的糖尿病的能力。CD28-/-NOD小鼠具有正常数目的效应T细胞和Th1应答,并且由于依赖于CD28在外周血中处于稳态的Treg缺陷而经历加速形式的自身免疫糖尿病(17,28)。先前的研究示出转移大量(8-20×106)的多克隆Treg可以延缓或者预防糖尿病的发生(17)。因此,我们检测了转移进CD28-/-小鼠中的p31-I-Ag7-扩增的Treg是否能预防糖尿病。将少如1.8-2×106个p31-I-Ag7扩增的Treg转移进5-7周龄小鼠中,在55%的小鼠中在长至15周的时间内预防糖尿病发生。基于流式细胞计量术,转移的细胞群典型含有~10%p31-I-Ag7多聚体+细胞,尽管基于CFSE增殖分析的绝对频率毫无疑问地更高。因此,我们估计转移的细胞群含有106或者更少的抗原特异性Treg细胞,这基本上低于用多克隆NODTreg进行的相似研究。因此,抗原特异性p31-I-Ag7扩增的细胞高度有效地预防完全功能性多克隆T细胞应答诱导的糖尿病发生。另外,p31-I-Ag7扩增的Treg对自身免疫的抑制是器官特异性的,因为接受p31-I-Ag7扩增的Treg并且得以保护在15和16周龄免于糖尿病发生的动物与具有糖尿病并在8-10周龄检测的未处理的和多克隆Treg处理的小鼠相比,表现出在唾液腺和甲状腺中较高程度的淋巴细胞浸润。
在这个实施例中,我们论证了具有与胰岛肽模拟物反应性的抗原特异性CD4+CD25+Foxp3+Treg留驻在糖尿病易感性NOD小鼠的外周血中。另外,我们论证了抗原特异性细胞可以在体外从多克隆细胞群中选择性扩增,并且这些扩增的Treg保留新鲜分离的CD4+CD25+Foxp3+Treg的表型和功能特征。我们示出扩增的抗原特异性Treg在体内高度有效地控制器官特异性自身免疫。这些结果支持了CD4+CD25+Treg的免疫调节作用依赖于Treg的抗原特异性的先前研究结果,并且与提示Treg以抗原非特异性方式通过竞争T细胞生态位(niche)而起作用的报道不一致(19,22,29)。
我们的结果提供了基于Treg进行临床治疗的方法,所述方法需要从外周血中扩增器官特异性Treg。即使扩增了具有有限集合(repertoire)的少数自身抗原特异性Treg,这些细胞也可以是临床有效的,因为其具有通过旁观者细胞因子产生和/或内源调节细胞募集而抑制多克隆T细胞应答的能力。已经鉴别了许多器官特异性抗原是造成自身免疫疾病如T1D和多发性硬化的因素,并且目前可利用的人MHC多聚体试剂可以用于扩增人器官特异性Treg以用于治疗自身免疫疾病(2)。

实施例3:在扩增的Treg细胞过继转移后狼疮性肾炎的临床好转

研究原因:调节T淋巴细胞(Treg)在控制自身免疫中的重要性目前在各种实验动物模型中已充分确定(McHughet al.,The role ofsuppressor T cells in regulation of immune responses.J Allergy ClinImmunol 10:693-702,2002)。另外,许多研究提示Treg不足是导致人自身免疫疾病的深层原因。最重要的是发现CD4+CD25+调节T细胞作为免疫稳态的必要成分提供了对于主动免疫调节和长期耐受性诱导的一个潜在的治疗机会。然而,Treg仅代表少数(<2%)的人CD4+T细胞、在自身免疫人体中数目和功能降低、并且通常被认为对增殖无反应性。我们开发了一种强有力的方法从人体中扩增Treg细胞。在不到3周的时间扩增直至200倍的细胞表达传统的Treg表型(CD4+,CD25+,CD62Lhi,GITR+和FoxP3+),并且发挥抑制T细胞效应细胞增殖和细胞因子产生的功能。
本研究设计为测试从狼疮性肾炎患者中扩增的CD4+CD25+细胞的安全性和效力。初步的结果提示系统性红斑狼疮(SLE)患者根据疾病的状态而具有不同的Treg活性,在好转期间具有Treg高活性,而在复发其间活性降低(Crispin et al.,Quantification of regulatory T cellsin patients with systemic lupus erythematosus.J Autoimmun 21:273-276,2003)。我们试图确定是否可以从好转期的患者中鉴别、选择并扩增Treg,通过低温冷藏贮存这些细胞,并在疾病复发时再导入这些细胞。设计这项研究以测试这种自体Treg疗法的安全性,并确定这种治疗对于疾病进程和免疫学参数的影响。
研究设计/治疗方案
这项研究又两个阶段组成。在第一阶段,我们从5名活动期狼疮性肾炎患者和5名狼疮性肾炎处于好转期的患者(停止免疫抑制治疗)中扩增Treg。这个阶段表明从疾病活动期和非活动期患者中扩增Treg的相对可行性,并论证了扩增的细胞具有的功能。
第二阶段是使用两种不同的设计方案在活动期狼疮性肾炎患者中输入Treg的公开标记实验(open-label trial)。在第一种方案中,从疾病活动期患者中扩增功能性Treg,然后让这些患者立即接受其自身扩增的细胞。进入标准除了活动期狼疮性肾炎之外,还包括存在抗-dsDNA抗体和补体低下。进入标准还包括对肾炎的合适的基本治疗(例如强的松、霉酚酸吗啉乙酯、硫唑嘌呤等等)。然而,环磷酰胺(CTX)治疗排除在外,因为CTX很可能干扰Treg的效力。主要终点是自体移植的安全性,通过监测一般临床参数和疾病活动性评估。次要终点(疾病活动性、血清学和机械研究)在治疗后第30、60、90、180和365天测定。
第二种方案在从活动期狼疮性肾炎患者中不能收集到足够的Treg的情况下应用。在此,在疾病好转期从患者收获细胞,离体扩增,然后冷冻制备以在疾病复发时输入。除了较大的患者群和较长的时间之外,这个实验设计与上述设计相同。
方法和材料与下述在糖尿病患者中过继细胞转移研究的第一阶段描述的方法和材料相同。我们选择的FDA-许可的抗-CD3/抗-CD28包被的珠和特定的单克隆抗体分别得自Xcyte Therapies和BectonDickinson。我们调整了淘选和扩增程序以扩增并低温保存来自各个患者的大约109Treg。
初级结果:安全性
二级结果:1)肾功能,通过肌酸酐、蛋白尿和尿沉渣评价;2)狼疮血清学,通过抗-dsDNA抗体和补体评价;3)疾病活动指数,通过SLEDAI和/或BILAG及对患者整体评估评价;4)机理学研究。
机理学研究:1)抗-dsDNA和补体的测定;2)产生自身抗体的B细胞的频数评价;3)循环淋巴细胞的表型分析以检测Treg表型;4)评价Treg活性;5)对Th1和Treg细胞因子的ELISPOT。
解释:这个研究论证了衍生自狼疮性肾炎患者的Treg细胞对患者的健康或疾病进展无不利作用,过继转移治疗方法改善了这些患者的肾功能。
实施例4:扩增的Treg细胞过继转移后糖尿病的临床好转
这个实验论证了在扩增的Treg细胞过继转移后糖尿病的临床好转。我们的研究淋巴细胞收集和输注方案是从Rapoport,et al.:Molecular remissionof CML after autotransplantation followed byadoptive transfer of costimulated autologous T cells.Bone MarrowTransplant(Epub ahead of print,Oct 27,2004)的方法加以调整而成。患者合格性和参与
我们根据Institutional Review Board指导方针要求从所有患者获得事先的书面知情同意书。需要患者患有基于临床特征和实验室特征的糖尿病。需要足够的肾、心、肺和肝功能,患者不可以具有活动期感染或者HIV血清学阳性。
稳态淋巴细胞收集
首先使用自动细胞分离仪(Cobe Spectra细胞收集仪或者相等功能设备)对患者进行稳态白细胞去除。大约20-30升的血液通过大内径导管加工,获得大约1.5×108单核细胞/kg体重。冷冻保存这些细胞以随后在抗-CD3/抗-CD28培养系统中进行扩增。
T淋巴细胞的离体共刺激和扩增
将T细胞根据FDA批准的研究性新药应用中规定的方法培养。在自体移植阶段第0天左右,解冻冷藏的单核细胞并用含有1%的人血清白蛋白的PBS洗涤三次。如果在最初冷藏时不进行,则使用磁性珠在密闭系统中去除单核细胞。然后将细胞接种进含有X-VIVO并补加了5%收集的AB血清的透气培养瓶(Baxter Oncology,Deeriield,IL,USA)中。加入具有固定的抗-CD3(OKT3)和抗-CD28(9.3)单克隆抗体顺磁性珠(比率为2∶1的珠:CD3+细胞),IL-2(1000 IU/ml)补充的培养物保持多至14天,之后进行收获并制备以进行输入(如前)。每日计数细胞,加入补加了1000IU/ml IL-2的新鲜培养基以保持细胞密度为0.75-2×106/ml。在完成细胞培养后,使用Baxter FenwalMaxsep磁性细胞分离设备除去磁珠。在除去磁珠后,洗涤细胞,浓缩并再悬浮于含有1%人血清白蛋白的100-250ml Plasmalyte A中,使用Baxter Fenwal收获系统进行。
离体扩增的T细胞的再输入
在释放之前,所有收获的产物均需要符合细胞存活(70%)、无菌(细菌和真菌培养阴性,内毒素分析阴性)和珠污染(<100珠/3×106细胞)的标准。将收获的细胞通过信使从细胞生产机构转运至患者,并在当天输入。细胞在20-60分钟内输入而无白细胞过滤。患者可以常规提前服用对乙酰氨基酚和苯海拉明。
实施例5:扩增的Treg细胞过继转移后糖尿病的临床好转
这个实验论证了在扩增的Treg细胞过继转移后糖尿病临床好转。我们的研究淋巴细胞收集和输入方案是从Rapoport,et al.:Molecularremissionof CML after autotransplantation followed by adoptive transferof costimulated autologous T cells.Bone Marrow Transplant(Epub aheadofprint,Oct 27,2004)的方法加以调整而成。
患者合格性和参与
我们根据Institutional Review Board指导方针要求从所有患者获得事先的书面知情同意书。需要患者患有基于临床特征和实验室特征的糖尿病。需要足够的肾、心、肺和肝功能,患者不可以具有活动期感染或者HIV血清学阳性。
稳态淋巴细胞收集
首先使用自动细胞分离仪(Cobe Spectra细胞收集仪或者相等功能设备)对患者进行稳态白细胞去除。大约20-30升的血液通过大内径导管加工,获得大约1.5×108单核细胞/kg体重。冷冻保存这些细胞以随后在MHC II类分子/肽复合物共刺激剂培养系统中进行扩增。
T淋巴细胞的离体的共刺激和扩增
将T细胞根据FDA批准的研究性新药物应用中规定的方法培养。在自体移植阶段第0天左右,解冻冷藏的单核细胞并用具有1%的人血清白蛋白的PBS洗涤三次。如果在最初冷藏时不进行,则使用磁性珠在密闭系统中去除单核细胞。然后将细胞接种进含有X-VIVO并补加了5%收集的AB血清的透气培养瓶(Baxter Oncology,Deeriield,IL,USA)中。加入具有固定的MHC复合物II型DQ0602/胰岛素B肽(氨基酸5-15)和抗-CD28(9.3)单克隆抗体顺磁性珠(比率为2∶1的珠:CD3+细胞),及IL-2(1000IU/ml)补充的培养物保持多至14天,之后进行收获并制备以进行输入(如前)。每日计数细胞,加入补加了1000IU/ml IL-2的新鲜培养基以保持细胞密度为0.75-2×106/ml。在完成细胞培养后,使用Baxter Fenwal Maxsep磁性细胞分离设备除去磁珠。在除去磁珠后,洗涤细胞,浓缩并再悬浮于含有1%人血清白蛋白的100-250ml Plasmalyte A中,使用Baxter Fenwal收获系统进行。
离体扩增的T细胞的再输入
在释放之前,所有收获的产物均需要符合细胞存活(70%)、无菌(细菌和真菌培养阴性,内毒素分析阴性)和珠污染(<100珠/3×106细胞)的标准。将收获的细胞通过信使从细胞生产机构运输至患者,并在当天输入。细胞在20-60分钟内输入而无白细胞过滤。患者可以常规提前服用对乙酰氨基酚和苯海拉明。

Claims (40)
1.一种调节对象自身免疫应答的方法,所述方法包括:
获得对象可相容的细胞群;
从所述细胞群中产生富含预定的自身抗原特异性调节T细胞的组合物;以及
将所述组合物导入所述对象,以调节所述对象中的所述自身免疫应答。
2.权利要求1的方法,其中所述细胞群得自所述对象。
3.权利要求1的方法,其中所述细胞群得自与所述对象不同的供体。
4.权利要求1的方法,其中所述细胞群从外周血中收获。
5.权利要求1的方法,其中所述产生步骤包括扩增所述抗原特异性调节T细胞。
6.权利要求5的方法,其中所述扩增通过将所述细胞群与自身抗原特异性调节T细胞刺激组合物接触而实现。
7.权利要求5的方法,其中调节T细胞在进行所述扩增步骤之前从所述细胞群中富集。
8.权利要求5的方法,其中调节T细胞在进行所述扩增步骤之后从所述细胞群中富集。
9.权利要求6的方法,其中所述刺激组合物包含II类MHC/自身抗原性肽复合物。
10.权利要求6的方法,其中所述刺激组合物包含一种共刺激剂。
11.权利要求10的方法,其中所述共刺激剂是一种激动剂抗体。
12.权利要求11的方法,其中所述激动剂抗体结合CD28。

24.权利要求22的组合物,其中所述自身抗原特异性调节T细胞当被给予对象时有效调节自身免疫应答。
25.一种产生权利要求22的自身抗原特异性调节T细胞的组合物的试剂盒,所述试剂盒包含:
一种自身抗原特异性T细胞受体刺激剂;及
一种共刺激剂。
26.权利要求25的试剂盒,其中所述刺激剂是II类MHC/自身抗原性肽复合物。
27.权利要求25的试剂盒,其中所述共刺激剂是一种激动剂抗体。
28.权利要求27的试剂盒,其中所述抗体结合CD28。
29.权利要求25的试剂盒,其中所述试剂盒进一步包含第二种调节T细胞刺激剂。
30.权利要求29的试剂盒,其中所述第二种刺激剂是一种细胞因子。

37.一种过继细胞免疫治疗的方法,所述方法包括如下步骤:
从经诊断患有糖尿病并且具有选自空腹血糖(FPG)、餐后血糖(PPG)和葡萄糖耐量(GTT)的葡萄糖稳态的损伤指征的患者中提取混合的T细胞群;
通过阴性和阳性免疫选择和细胞淘选从所述细胞群中分离包含>98%CD4+CD25+T细胞(Treg细胞)的亚群;
通过将所述亚群与有效量的(i)选自TCR/CD3的多价抗体和配体的TCR/CD3激活物;(ii)选自CD28的多价抗体和配体的TCR共刺激物激活物;和(iii)IL-2接触而将所述亚群的Treg细胞扩增至少100倍,以获得离体扩增的Treg细胞,其中有效量的IL-2是200-2500 IU IL-2/ml;
将107-1011个离体扩增的Treg细胞导入患者;以及
检测损伤的葡萄糖稳态的改善。
38.权利要求37的方法,其中所述改善选自FPG为110mg/dL或更低、2小时PPG为140mg/dL或更低、以及摄取75-g葡萄糖2小时后GTT为140mg/dL或者更低。
39.权利要求37的方法,其中所述TCR/CD3激活物是一种抗-CD3抗体,TCR共刺激物激活物是抗CD28抗体,其中抗CD3和抗CD28抗体固定在Treg细胞∶珠的比率为1∶1至1∶2的顺磁性珠上。
40.权利要求37的方法,其中所述TCR/CD3激活物是一种MHC-肽多聚体,其中所述肽是一种与糖尿病相关的自身抗原肽,所述糖尿病相关的自身抗原选自谷氨酸脱羧酶(GAD)、胰岛细胞自身抗原(ICA)和胰岛素,TCR共刺激物激活物是抗-CD28抗体,导入步骤是将107-109离体扩增的Treg细胞导入患者中。
